Description
Technical Field
The invention relates to a device for clearing ectopic pregnancy focuses through a natural cavity and a using method thereof.
Background
Ectopic pregnancy is also called ectopic pregnancy, which refers to the abnormal pregnancy process of nidation and development of fertilized eggs outside the uterine cavity, so that tubal pregnancy is the most common. The poor lumen patency is often caused by the inflammation of the lumen or the periphery of the fallopian tube, which hinders the normal operation of the pregnant ovum, so that the pregnant ovum stays, lands and develops in the fallopian tube, resulting in abortion or rupture of the fallopian tube in pregnancy. Before abortion or rupture, there are no obvious symptoms, and there may be symptoms of menopause, abdominal pain, and small amount of vaginal bleeding. Once ruptured, it manifests as acute and severe abdominal pain, recurrent episodes, vaginal bleeding, and even shock; and the women who are too obese can not be clearly diagnosed by multiple examinations, so that the treatment opportunity is easily delayed, and the serious threat is brought.
If a woman is pregnant extrauterine, the current treatment methods are as follows: 1. traditional medicine conservation treatment. 2. Performing fenestration suturing on the fallopian tube. 3. A resection is performed on the fallopian tube.
The method is a painful choice for pregnant women, the traditional medicine conservation treatment is a treatment method in the early stage of ectopic pregnancy, and is also suitable for people who have no rupture of the oviduct, have an oviduct gestation mass diameter smaller than 3cm, have no obvious internal bleeding and have blood HCG smaller than 2000U/L and are suitable for medicine conservation treatment. Although the treatment method does not need surgical treatment for pregnant women, the treatment method needs chemotherapy drugs, killed embryos are polarized in situ, fallopian tubes are easy to block, fertility is affected, and therefore female patients need to use the method cautiously, and especially people with fertility requirements do not try the method easily. The oviduct resection is generally to solve the problems that the oviduct is very seriously broken and is difficult to repair, and the extrauterine pregnancy accompanied with the symptoms of hemorrhagic shock patients can only be realized by cutting off the oviduct at the affected side, and the conception and the fertility of a female can be influenced after the cutting off. The window-opening suture operation on the fallopian tube is the best of the three methods, but the window-opening suture operation needs to be carried out by a laparoscope or an abdominal operation and the like, is an invasive operation and has larger harm to a patient.
Currently, there is no better device or method for treating and removing ectopic pregnancy with minimal damage to the fallopian tubes through the natural orifices.

The specific implementation mode is as follows:
the invention is explained in further detail below with reference to the drawings.
Referring to fig. 1 to 6, an apparatus for removing ectopic pregnancy focus through natural orifice comprises a handle part 1, a controllable bending part 2 and a removing tube 3;
the clearing pipe 3 can extend out of the end part of the controllable bending part 2, the outer part 21 of the controllable bending part 2 is made of silica gel, links 22 are embedded in the silica gel at intervals, and at least one pull wire 23 penetrates through all the links and is fixed in the handle part 1;
the winding mechanism 11 is arranged in the handle part 1 and used for winding the stay wire, the negative pressure generating mechanism 12 is further arranged in the handle part 1 and used for generating negative pressure for the cleaning pipe 3, and the blockage cleaning mechanism 4 is arranged at the end part 31 of the cleaning pipe 3.
The storage battery is arranged in the handle part 1 to provide power for the winding mechanism 11, the negative pressure generating mechanism 12 and the blockage removing mechanism 4, and the handle part 1 can also be connected with an external power supply. The negative pressure generating mechanism 12 may also be generated by providing negative pressure from an external device.
The realization principle of the invention is as follows:
the using method of the equipment comprises the following steps:
s1, performing physical examination on the patient, and performing the operation according to the operation requirement; after the diagnosis of ectopic pregnancy is confirmed, whether surgical conditions, such as serious inflammation, exist is determined through a detection result, and a cleaning operation can be performed after conservative treatment.
S2, lying the patient on the back to take the lithotomy position, performing local anesthesia on the lithotomy position, and expanding the vagina by using a vaginal dilator;
s3, holding the hand grip part of the equipment by hand, extending the controllable bending part into the expanded vagina and extending the head part into the uterus; and smearing the medical lubricating liquid on the outer side of the controllable bending part 2 of the clearing device, extending the medical lubricating liquid into the vagina and finally extending the medical lubricating liquid into the uterus.
S4, adjusting the head bending direction of the controllable bending part through the hand grip part to align the head bending direction to the oviduct direction;
in the above steps, the oviduct of the side where the ectopic pregnancy is located is displayed according to the detection result, then the winding mechanism 11 in the handle part 1 is adjusted, the pull wire 23 penetrating through each link 22 in the corresponding direction is wound, and along with the winding of the winding mechanism 11, the pull wire 23 firstly pulls down the link 22 at the top to the side close to the link 22.
The controllable bending part 2 is elastic because the outer part 21 is made of silica gel, and the stay wire 23 is positioned in each link 22, so the link also plays a role of limiting the stay wire 23, so that the stay wire 23 is not separated from the link when being wound, and the downward tensile force of the stay wire 23 on one side is received under the arrangement and the structure of the link 22, so that the whole controllable bending part 2 is pulled towards the direction. Of course, the pulling force received by the uppermost link is the largest, so that the uppermost bending effect of the controllable bending part 2 is the most obvious.
S5, controlling the clearing tube to extend out of the controllable bending part and extend into the oviduct and into the oviduct through the holding part;
in general, the angle of the oviduct in the uterus is constant, so the rolling mechanism 11 in the handle part 1 sets the rolling pulling wire 23 to ensure that the angle of the end part of the controllable bending part 2 corresponding to the oviduct direction is also constant, thus avoiding the trouble of readjustment.
Of course, there are other situations, that is, the sizes and angles of the uterus and the oviduct are different due to different individuals, and at this time, verification needs to be performed by combining the result of detecting the position of the uterus and the oviduct, and if the individuals have the differences, other assistance can be adopted to perform the operation.
S6, monitoring the position of the fertilized egg in the oviduct and the extending depth of the cleaning tube through a B ultrasonic instrument so as to assist the cleaning tube to clean the nidus blocked by nidation of the fertilized egg.
In the above steps, the cleaning tube 3 is controlled by the handle part 1 to extend out of the end part of the controllable bending part 2, the cleaning tube 3 extends into the oviduct, the end part 31 of the cleaning tube 3 is provided with the blockage cleaning mechanism 4, the blockage cleaning can be synchronously performed while the cleaning tube extends into the oviduct, so that the implanted fertilized eggs enter the cleaning tube 3 through the blockage cleaning mechanism 4 and finally enter the negative pressure generating mechanism 12, and the extrauterine pregnancy cleaning operation of the oviduct is completed.
On the day of the removal, the patient may be observed in hospital to prevent other accidents. If the uterus is not suffered from major hemorrhage, the patient can be discharged on the day.
In the step S6, an infrared emission device is arranged on the cleaning tube, and an infrared instrument monitor is arranged beside the operating table to monitor the position of the cleaning tube.
Further, as a modification to the embodiment, the end 24 of the controllable bending part 2 is further provided with a camera and an LED lamp. In the invention, the camera can be wireless, and mainly assists the grip part 1 to adjust the bending direction and the bending angle of the controllable bending part 2. So that blind operation is not needed in the early operation of the operator.
Further, as an improvement to the embodiment, the winding mechanism includes a servo motor, a winding gear for fixing the pull wire, and the whole winding process can be controlled by the rotating teeth (not shown) arranged on the watch body of the hand-holding part.
In the present invention, the rotating teeth (not shown) correspond to the potential difference, and the rotation of the servo motor is synchronously controlled during the rotation.
As shown in fig. a in fig. 6, as a further improvement of the embodiment, the blockage removing mechanism 4 includes a blockage removing head, the blockage removing head has a small opening 41 at the end 31 of the removing tube 3 and at the side near the end 31, a strip-shaped twisted blade 42 is erected in the small opening 41, and when negative pressure is generated, the strip-shaped twisted blade 42 rotates along with the suction force.
The negative pressure generating mechanism 21 forms negative pressure suction in the clearing pipe 3, the strip-shaped twisted blade 42 rotates along with the suction, the small opening 41 arranged on the blockage clearing head contacts with blocked tissues when the clearing pipe 3 extends into the oviduct, the tissues enter along with the small opening 41 and contact with the strip-shaped twisted blade 42, the strip-shaped twisted blade 42 rotates to generate twisting force to rub the tissues, the tissues are sucked into the negative pressure generating mechanism 21 under the lower suction, and along with the further extension of the clearing pipe 3, the tissues attached to the oviduct pipe wall and the whole fertilized eggs are rubbed and sucked into the clearing pipe 3, and finally the fertilized eggs enter the negative pressure generating mechanism 21.
With continued reference to fig. B in fig. 6, as a further improvement to the embodiment, the blockage removing mechanism 4 includes a net-shaped head 43, a rotary cutter head 44 is arranged in the net-shaped head 43, and the rotary cutter head 44 rotates under the action of negative pressure.
