本发明公开了一种气管插管,包括本体,外接口,气囊,缓释囊和充气口;所述缓释囊设置于所述气囊外壁,所述缓释囊内装入高吸水性树脂粉末和麻醉药粉末;所述缓释囊包括乳胶膜和滤网;所述滤网为微米滤网;所述高吸水性树脂粉末为羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂粉末,所述麻醉药粉末为盐酸利多卡因粉末。本发明的气管插管,通过高吸水性树脂形成载药胶束,质地柔软,缓释稳定,麻醉药可直达气道黏膜处,避免引发气道不适或插管应激反应;同时具备显著的抑菌性能,有效避免了插管造成的手术细菌感染。

一种气管插管
技术领域
本发明涉及医用耗材技术领域,尤其是涉及一种气管插管。
背景技术
气管插管是指将一特制的气管内导管经声门置入气管的技术称为气管插管,这一技术能为气道通畅、通气供氧、呼吸道吸引和防止误吸等提供最佳条件。在全身麻醉时,呼吸道难以保证通畅者如颅内手术、开胸手术、需俯卧位或坐位等特殊体位的全麻手术;如颈部肿瘤压迫气管,颌,面,颈,五官等全麻大手术,极度肥胖病人;全麻药对呼吸有明显抑制或应用肌松药者,都应行气管内插管。气管内插管在危重病人的抢救中发挥了重要作用,呼吸衰竭需要进行机械通气者,心肺复苏,药物中毒以及新生儿严重窒息时,都必须行气管内插管。
气管插管会带来严重不适,粗大的气管插管插入气道后可导致病人口、鼻、咽、喉的不适感、胀痛感和堵塞感;经口气管插管的病人不能闭口而感到口干,会感到口腔胀痛,甚至引起牙齿和口腔粘膜损伤。气管插管还可使机体产生一系列不良反应,即气管插管应激反应综合征,简称气管插管应激反应。气管插管刺激喉与气管内壁感受效应器,可激活机体交感与肾上腺素能系统以及肾素、血管紧张素、醛固酮系统,继之迅速促发体内儿茶酚胺类高浓度释放,并作用于靶器官和其受体,从而导致机体反射性心率增快、血压升高,甚至心律失常等。
为了避免气管插管带来的不适或引起应激反应,现有技术中通过气管插管向气道内输入麻醉药。公开号CN201453795U的专利公开了一种用于实施表面麻醉的气管插管,包括有气管插管、套囊注气管出气口、套囊、套囊注气口、套囊注气管和气管插管外口;套囊套装在气管插管的左端,套囊注气管安装在气管插管外管壁上的沿轴线设置的气管槽里,套囊注气管出气口直至套囊的内腔,套囊注气口安装在套囊注气管的另一端,气管插管外口安装在气管插管的右端,用于实施表面麻醉的气管插管还加装了局麻药注药管和局麻药注药口,局麻药注药管安装在气管插管外管壁上的药管槽里;局麻药注药管的末端封闭,并在封闭端的局麻药注药管的管壁上沿轴线方向加工有等距离分布的和局麻药注药管的内腔相连通的局麻药出药孔,局麻药注药管的另一端安装有局麻药注药口。
公开号CN2710669的专利公开了一种气管插管,包括管体、气囊和气体阀,管体为弯曲状,管体一端与呼吸机相连,另一端为插入端,管体表面在偏 向呼吸机相连一端设计有刻度;气囊位于靠近插入端一侧并包覆在管 外,气囊两端与管体气密连接,在管体壁内开有气道,气道一端开口于气囊内,另一端通过毛细管与气体阀相连,在管体壁内另开设有麻药通道,麻药通道一端封闭,另一端由毛细管引出与输药阀相连,在麻药通道上开有多个喷药孔。
上述技术方案通过气管插管向气道内直接注入或者喷洒麻醉药,气道内如果进入外来液体,容易引起呛嗑等不适。向气道直接注入或者喷洒麻醉药,进行浅度局麻,药效维持时间短;对于时长较长的大手术,需多次重复加药,操作复杂。现有技术中还存在气管插管气囊外涂抹麻醉药的技术,药物涂层薄,药效持续时间短,药物浓度不稳定,且涂层在气道内有脱落的风险。
发明内容
有鉴于此,本发明的目的是针对现有技术的不足,提供一种气管插管,消除了气管插管带来的不适感和可能引起的应激反应,释放药物稳定可靠,抑菌性能优异,制备成本低廉。
为达到上述目的,本发明采用以下技术方案:一种气管插管,包括本体,外接口,气囊,缓释囊和充气口;所述缓释囊设置于所述气囊外壁,所述缓释囊内装入高吸水性树脂粉末和麻醉药粉末;所述缓释囊包括乳胶膜和滤网;所述滤网为微米滤网;
所述高吸水性树脂粉末为羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂粉末,所述麻醉药粉末为盐酸利多卡因粉末;所述羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂粉末添加量为0.05-0.08g,所述盐酸利多卡因粉末添加量为0.4-0.45g;所述滤网的孔径是7-12μm。
进一步地,所述气管插管的模拟使用过程:将气管插管包裹于含水基质内,通过充气口将气囊内充满气体;缓释囊通过吸收周边水分,高吸水性树脂粉末逐渐吸湿膨胀,逐渐将乳胶膜撑起;高吸水性树脂粉末吸水完成后,形成高分子交联胶束,充满缓释囊,高分子交联胶束将麻醉药包载起来,通过滤网向周边含水基质进行麻醉药缓释。
进一步地,所述羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂的合成是以羧甲基壳聚糖和丙烯酸为原料,采用水溶液聚合法,以过硫酸钾和亚硫酸氢钠为引发剂,N,N’-亚甲基双丙烯酰胺为交联剂。
本发明的有益效果是:
本发明的气管插管以高吸水性树脂胶束作为麻醉药载体,向气道内缓慢释放麻醉药,对气道粘膜进行浅度麻醉,药物浓度恒定,可实现长时间稳定药物释放。高吸水性树脂粉末具备超强的吸湿性,盐酸利多卡因也具备较强的吸湿性,可通过滤网吸收气道内的水分;高吸水性树脂吸水后体积大幅度增加,将缓释囊撑大,与常规气囊相比,显著提高了气管插管与气道的贴合度;缓释囊直接贴在气道粘膜上,确保胶束缓释的麻醉药可以直接到达气道黏膜,麻醉效果更加直接高效;吸水后的高吸水性树脂质地柔软,羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂生物相容性优良,极大地降低了饱胀的气囊带来的气道不适以及气管插管对气道黏膜的损伤。羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂不仅具备良好的吸水和缓释性能,抑菌性能也十分优异,有效避免了插管造成的手术细菌感染;尤其是对大肠杆菌和金黄色葡萄球菌具备显著的抑制作用,对大肠杆菌抑菌率达91.7%,对金黄色葡萄球菌抑菌率达70.6%。
作为药物缓释的重要指标,载药浓度直接影响药物的缓释压力差,滤网的孔径影响缓释速率。本发明的气管插管选择羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂,高吸水性树脂粉末添加量为0.05-0.08g,麻醉药粉末添加量为0.4-0.45g,滤网的孔径是7-12μm,该气管插管经过多次缓释实验,麻醉药缓释过程可长达48小时,麻醉药浓度保持在19-23g/L范围内。
本发明的气管插管将麻醉药浓度维持在浅度局麻的最佳浓度范围,完全不需要重复加药,既不会出现因麻醉药物浓度下降造成患者不适或应激反应,也不会发生过度麻醉。当气管插管使用完毕后,只需要排出气囊内的空气,缓释囊贴近气管插管本体,即可轻松从气道内取出气管插管,不会损伤气道内壁。
本发明的气管插管可在现有的气管插管上直接作出改进,在气囊外壁增设缓释囊,成本低廉,操作简便。

以下是本发明的具体实施例,对本发明的技术方案作进一步的描述,但本发明并不限于这些实施例。
实施例1
一种气管插管,包括本体1,外接口2,气囊3,缓释囊4和充气口5。所述缓释囊4设置于所述气囊3外壁。所述缓释囊4内装入高吸水性树脂粉末和麻醉药粉末,所述缓释囊4包括乳胶膜42和滤网41;所述滤网41为微米滤网。
所述高吸水性树脂粉末为羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂粉末,所述麻醉药粉末为盐酸利多卡因粉末。
所述气管插管的模拟使用过程:将气管插管包裹于含水基质内,如图2所示,通过充气口5将气囊3内充满气体;缓释囊4通过吸收周边水分,高吸水性树脂粉末逐渐吸湿膨胀,逐渐将乳胶膜撑起;如图3所示,高吸水性树脂粉末吸水完成后,形成高分子交联胶束,充满缓释囊4,高分子交联胶束将麻醉药包载起来,通过滤网向周边含水基质进行麻醉药缓释。
实施例2
本实施例与实施例1的不同之处在于:所述滤网的孔径是7μm。所述乳胶膜42内边缘粘附于所述气囊3外壁,所述滤网41粘附或压合于乳胶膜42外边缘。所述滤网既可以阻止药物混合粉末以及交联吸水树脂不会外泄,又可以确保高吸水性树脂粉末快速吸湿,同时可以有效控制缓释速率,支撑麻醉药的稳定释放。
实施例3
本实施例与实施例1的不同之处在于:所述滤网的孔径是9μm。
实施例4
本实施例与实施例1的不同之处在于:所述滤网的孔径是12μm。
分别选取孔径7μm、9μm和12μm滤网的气管插管,将充气后的气管插管置入PBS缓冲液模拟体液中,恒温37℃震荡,5min内缓释囊4完成膨胀,1h后取出缓释溶液在两片卤化物晶片之间,进行红外光谱检测分析,在红外谱图中均未观察到NH3 +和COO-的伸缩振动吸收峰,证明高分子交联吸水树脂没有泄漏到缓释囊4外侧。
实施例5
本实施例与实施例2的不同之处在于:所述羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂粉末添加量为0.05g,所述盐酸利多卡因粉末添加量为0.4g。
将充气后的气管插管置入一定量的PBS缓冲液模拟体液中,恒温37℃震荡,1h开始测量缓冲液中盐酸利多卡因的浓度,每次取出2ml缓释溶液待测,采用可见—紫外分光光度法每小时测量一次;同时向模拟体液内补充2ml空白PBS缓冲液,模拟气道内麻醉药的消耗和流失。附图4为本实施例麻醉药缓释浓度测试图。
实施例6
本实施例与实施例2的不同之处在于:所述羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂粉末添加量为0.06g,所述盐酸利多卡因粉末添加量为0.42g。
将充气后的气管插管置入一定量的PBS缓冲液模拟体液中,恒温37℃震荡,1h开始测量缓冲液中盐酸利多卡因的浓度,每次取出2ml缓释溶液待测,采用可见—紫外分光光度法每小时测量一次;同时向模拟体液内补充2ml空白PBS缓冲液,模拟气道内麻醉药的消耗和流失。附图5为本实施例麻醉药缓释浓度测试图。
实施例7
本实施例与实施例2的不同之处在于:所述羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂粉末添加量为0.08g,盐酸利多卡因粉末添加量为0.45g。

实施例8
本实施例与实施例2的不同之处在于:所述羧甲基壳聚糖接枝聚丙烯酸高吸水性树脂的合成是以羧甲基壳聚糖和丙烯酸为原料,采用水溶液聚合法,以过硫酸钾和亚硫酸氢钠为引发剂,N,N’-亚甲基双丙烯酰胺为交联剂。高吸水性树脂交联聚合的制备工艺是本领域的现有技术,可参照专利CN1616506,此处不再描述。
