在图34和34A中,就在将股骨杯20与髋臼球21分离之前,将工具91置于假体上。髋臼接合环96的斜边缘96b与髋臼杯23的圆周斜边缘22a接合,并且股骨接合组件95与股骨杯20的颈25或股骨杯20的外半球表面或者与股骨杯20的颈25和外半球表面这两者相接合。也显示出股骨植入物26,并且股骨接合组件95也可以接合植入物26的顶部(即近端)。衬垫27显示于股骨杯20中。髋臼球21附着于髋臼杯的柄28。
当手柄92和93相互挤压时,股骨杯与髋臼球分离。
图2的托盘底部2中的组件包括股骨杯取出器31和髋臼球取出器51的元件。(还参见图7-22A。)发明人于2016年8月2日提交的共同未决的美国临时申请序号62/369,901中所述的股骨杯取出器类型可以用于替代取出器31。将临时申请通过引用整体并入本文。
图7的股骨杯取出器31具有附着于起子33的远端的叉形楔32。起子33包括在其近端的手柄34和在其远端具有接头36的杆35。杆35的近端附着于手柄34的远端。
通过莫氏锥体附着于股骨植入物41的股骨杯40显示于图8的截面中。股骨杯40具有颈42,并且股骨植入物的近端由参考数字43指定。
当将其内附着有股骨杯的股骨植入物植入股骨的近端时,移除股骨杯而不干扰股骨植入物是可取的。这样,可将股骨植入物维持在股骨内而不干扰骨长入。因此,“打破”股骨杯与股骨植入物之间安全的莫氏锥体连接而不拉扯植入物是可取的。这通过利用本发明的手术工具来实现。
根据本发明的方法,相对于图9中所示的股骨杯40和股骨植入物41放置股骨杯取出器31的楔子32。然后用锤子敲手柄34的近端以用足以向上举起杯的力向股骨杯40驱动楔子32,同时保持股骨植入物安全嵌入股骨中。因此,杯不再通过莫氏锥体附着于股骨植入物41。该位置显示于图10中。然后可以用手拿起并移除杯。
股骨杯取出器31还可以与常规的髋关节植入物一起使用,其中股骨球通过莫氏锥体附着于股骨植入物。
本发明的髋臼球取出器51的第一实施方案在图1中以立体图显示。工具51是本发明的髋臼球取出器的第二实施方案,并且其在图12中以立面图显示,并且在图22中以部分分解图显示。所述工具包括钳口52及其远端的钟形手柄53。提供的工具51或51a的钳口元件具有两种或多于两种的不同尺寸,并且提供的工具51或51a的钟形手柄元件具有两种或多于两种的不同尺寸,每一种均适用于协调工作以移除具有特定尺寸或尺寸范围的髋臼球。外杆54沿其长度具有任选的滚花部分55。在工具51的近端,轴螺帽56附着于杆54,并且在工具51a的近端,轴螺帽56a附着于杆54。轴螺帽56的表面任选地是滚花的,并且其上放射状安置有孔57,以当操作工具时出于杠杆作用接受止挡件58。将钳口轴螺帽59或59a螺旋地拧到钳口杆60。钳口杆60显示于图13、15和16的截面中,以及在图18中以正视图显示。图22和22A显示了钳口杆60的螺纹近端61。
工具51与51a之间的差异与轴螺帽56和56a之间的差异以及钳口轴螺帽59和59a之间的差异有关。轴螺帽56和59是滚花的并且在滚花部分提供孔57以接受止挡件58,所述止挡件58用于在工具操作中提供杠杆作用。轴螺帽56和59a也是滚花的,但是它们分别具有六边形部分64和65。在六边形部分64和65中向止挡件58提供孔57a。六边形部分允许外科医生使用扳手来替代一些或全部止挡件,从而在工具操作期间获得增加的杠杆作用。
工具操作显示于图13-图16中。当向远端方向推挤钳口杆60时,允许钳口52开放,同时保持杆54,以使钳口脱离钟形手柄53。通过销62铰接钳口52。在图13中,当将钳口52推出钟形手柄53并推至髋臼球70之后,钳口52显示于截面中。通过莫氏锥体使髋臼球70安全附着于柄71上。柄71附着于髋臼杯72并从其凹面突出。
在图14和15中,将钟形手柄53的远端推至与髋臼杯72的圆周边缘73接合。当工具以此种方式与球和杯接合时,下一步是握紧杆54同时旋转轴螺帽59或59a,引起钳口杆向近端方向移动,以使钳口52拉扯髋臼球70,同时钟形手柄53在髋臼杯72的圆周边缘73施加相反的力。相反的力防止拉扯髋臼杯72,以便杯不被拉出髋臼。当需要另外的杠杆作用以在髋臼球70施加拉力并在髋臼杯72施加相反的推力时,将止挡杆58置于轴螺帽56或56a的孔57或57a内,并且将另一止挡杆58置于钳口轴螺帽59或59a的孔57或57a内。如果需要更多的杠杆作用,则将止挡杆58从轴螺帽56a或59a中的一个或两个中移除,并通过将扳手的开口端(未显示)置于六边形部分64或65中的一个或两个上而在一个或两个轴螺帽上使用扳手。当施加足够的力时,如图16中的间隔80所示,球与柄71分离。
一般而言,在利用来自手术托盘的工具的手术方法中,第一步是将股骨杯与髋臼球分离,然后以任意顺序,从股骨植入物中移除股骨杯以及从髋臼杯中移除髋臼球。更具体地,在将股骨杯与髋臼球分离之前,选择合适尺寸的关节分离器工具。基于植入的髋臼球和股骨杯的尺寸选择所述尺寸。类似地,在从髋臼杯移除髋臼球之前,基于植入的髋臼球的尺寸选择合适尺寸的钳口和髋臼球取出器钟形元件。
在图23和24中,髋臼杯取出器81位于附着于髋臼杯72的柄71的髋臼球5上。将髋臼杯72植入骨84中,所述骨是髋臼。取出器杯82与髋臼球5以关节连接,并且叶片86的远端位于杯72和骨84之间。在本发明的手术方法中,将取出器杯82在髋臼球5上小心旋转,以引起叶片86沿髋臼杯72的凸面移动或者靠近髋臼杯72的凸面移动。叶片86具有多种尺寸,以使图28中指定为“L”的叶片部分的长度不同。可以使用多个具有不同长度的叶片,总是从具有最短叶片长度的叶片开始,然后使用下一个最长长度的叶片等。例如,如果使用三个叶片,则首先使用最短长度的叶片,并且使叶片在髋臼杯72和骨84之间完成至少一次完整旋转。换言之,叶片86沿杯旋转至少360度。然后使用下一个最长长度的叶片,并且使叶片在髋臼杯72和骨84之间完成至少一次完整旋转。用下一个最长的叶片(即三个叶片中最长的叶片)重复该过程,从而使髋臼杯72与骨84之间的连接松动,以便髋臼杯72足够松动可以从骨84中提出。由于叶片86沿髋臼杯72的凸面移动或者靠近髋臼杯72的凸面移动,骨损伤最小化,即使在移除过程之前存在显著的骨长入。仔细进行该过程以使骨损伤的风险最小化。
除了取出器杯82和叶片86之外,髋臼杯取出器81包括手柄87,手柄87由杆88和可移动的杠杆臂89构成。在图27中以正视图显示的起子90用于使取出器杯82紧固至手柄87的杆88或者从手柄87的杆88上松开。图31提供了显示这些元件在紧固和松开操作中的位置的剖视图。
在移除髋臼杯的手术方法中,移除从髋臼杯72延伸至骨84的任何螺丝或其它接头。然后将叶片86推进至靠近髋臼杯凸面的骨中,直至取出器杯82与髋臼球5接触。然后,在对髋臼球5按压取出器杯82时,将髋臼杯取出器81在髋臼球5上旋转,从而使叶片86松开髋臼杯72与骨84之间的连接。当髋臼杯72与骨84足够松动时,在如以上所述利用具有增加的长度的叶片86之后,从骨中提出髋臼杯。

Claims (10)

1.手术托盘器械及其组件,其包括一套托盘,所述一套托盘包括用于髋关节翻修手术中实施将股骨杯与髋臼球分离以及将髋臼球与植入的髋臼杯分离的工具,其中用于将髋臼球与植入的髋臼杯分离的工具包括:
尺寸设定为用于将髋臼球与髋臼杯分离的两种或更多种尺寸不同的钟形元件,和
尺寸设定为用于将髋臼球与髋臼杯分离的两种或更多种尺寸不同的钳口元件,其中所述钳口元件通过销的铰接安装在钳口杆的远端,当向远端方向推挤所述钳口杆时,允许所述钳口元件开放,
其中所述钟形元件和钳口元件被配置为当从髋臼杯中取出髋臼球时,所述钳口元件拉扯髋臼球,同时所述钟形元件与髋臼杯的圆周边缘接合,从而在髋臼杯的圆周边缘施加相反的力,防止髋臼杯被拉出髋臼。
2.如权利要求1所述的手术托盘器械及其组件,其中将股骨杯与髋臼球分离的工具包括两种或更多种工具,各工具具有不同尺寸,并且适用于具有不同尺寸的植入物,所述工具具有:
第一手柄,所述第一手柄具有位于其远端的股骨接合组件,
第二手柄,所述第二手柄具有位于其远端的髋臼接合组件,
使所述第一手柄和第二手柄互相连接的枢销。
3.如权利要求1或2所述的手术托盘器械及其组件,其还包括配置为用于从髋臼移除植入的髋臼杯的髋臼杯取出器,其中所述髋臼杯取出器包括取出器杯和叶片,所述取出器杯被配置为与植入的髋臼杯的髋臼球以关节连接,同时所述叶片的远端位于所述植入的髋臼杯和所述髋臼杯被植入的髋臼之间。
4.如权利要求2所述的手术托盘器械及其组件,其中所述将股骨杯与髋臼球分离的工具包括:
第一手柄,所述第一手柄具有位于其远端的股骨接合组件,其中所述股骨接合组件通过轴销可旋转地连接于所述第一手柄。
5.如权利要求2所述的手术托盘器械及其组件,其中所述将股骨杯与髋臼球分离的工具包括:
第二手柄,所述第二手柄具有位于其远端的髋臼接合组件,其中所述第二手柄包括铰链,所述铰链提供位于枢销和髋臼接合组件之间的第二手柄的一部分的绕轴旋转。
6.如权利要求2所述的手术托盘器械及其组件,其中所述将股骨杯与髋臼球分离的工具包括:
第二手柄,所述第二手柄具有位于其远端的髋臼接合组件,第二手柄的远端具有通过环销绕轴旋转连接于其上的髋臼接合环。
7.如权利要求6所述的手术托盘器械及其组件,其中所述髋臼接合环包括从其向外延伸的突片,所述突片被配置为保留在邻近所述髋臼接合环的第二手柄的部分中存在的凹痕中,以防止髋臼接合环在所处位置向下旋转。
8.如权利要求1所述的手术托盘器械及其组件,还包括带螺纹的钳口轴螺帽,并且其中所述钳口杆包括能与所述钳口轴螺帽的螺纹接合的近侧带螺纹部分。
9.如权利要求8所述的手术托盘器械及其组件,还包括在近端具有轴螺帽的中空外杆,所述钳口杆能够在所述外杆内接合,钟形元件能与所述外杆的远端接合,并且当所述钳口元件接合于植入的髋臼杯中的髋臼球内并且所述钟形元件的远端被推至与髋臼杯的圆周边缘接合时,所述钳口轴螺帽被旋转,引起所述钳口杆向近端方向移动,以使所述钳口元件拉扯髋臼球,同时所述钟形元件在髋臼杯的圆周边缘施加相反的力,防止髋臼杯被拉出髋臼。
10.如权利要求1或2所述的手术托盘器械及其组件,还包括:
股骨杯取出器,其具有附着于起子的远端的叉形楔,所述起子包括在其近端的手柄和在其远端具有接头的杆,所述起子的杆的近端附着于所述起子的手柄的远端。
