Abstract

Description
本发明涉及改进或与其有关的震颤稳定的装置和方法,尤其涉及陀螺装置,用于控制身体部位的生理性和病理性的震颤,尤其是手震颤。
非自主性肌肉震颤发生在一系列神经病学疾病中,特别是退行性疾病,如帕金森病。
已经提出了使用陀螺仪调解手震颤的许多建议。US5058571描述了一种早期的方案,其中电池驱动陀螺仪通过带子抵靠手的背面固定。陀螺仪旨在保持其旋转轴线的方向,并抵抗试图导致该方向发生变化的任何动作。因此,使用陀螺仪的理论是,肌肉震颤的发作引起手中的运动,但是陀螺仪抵抗该运动,基本上抵消了该震颤。

陀螺仪是具有可绕轴线自由旋转的旋转盘的装置。当盘旋转时,陀螺仪将抵抗所施加的耦合的动作并趋于保持固定的取向。如果陀螺仪被旋转移位,则通过装置围绕相互垂直于盘旋转轴线和装置移动的轴线的轴的章动,可以节省角动量。
陀螺仪将施加陀螺力矩,其幅度与盘的惯性矩、盘的角速度和章动角速度成正比。陀螺力矩的方向向量与盘的角速度和装置的章动的角速度的矢量叉积成正比。
本发明的装置包括多个陀螺装置,其围绕装置所施加的主体部分被间隔开。当身体的平衡状态受到扰动时,诸如在震颤或旋转位移期间,多个陀螺装置在身体上共同施加累积的净陀螺力矩,但允许使用较小的陀螺仪,由此将陀螺仪的质量在身体部位上展开,使得装置更容易佩戴,并且还减小了装置的体积,从而与具有较大陀螺仪的已知装置相比,对灵敏度和运动阻碍程度更小。
图1示出了根据本发明的装置的实施例,其具有在手部12的背面上安装有陀螺装置11的手套10的形式。在所示的实施例中,手套10是开放式或无指型的,以允许手指13和拇指14自由运动。优选地,手套被形成为用于陀螺装置的织物支撑件,其通过带子适当地附接到佩戴者的手腕、手指和拇指,使用钩环式可调节固定装置。所述织物优选为柔软舒适的材料,其可以在较长时间内舒适地佩戴。在优选的实施方案中,所述织物是在WO 2014/127291中描述的类型,其中范德华力在柔性硅树脂织物表面和佩戴者的皮肤之间展开,以将织物保持在适当位置。
图2是陀螺装置的优选实施例的分解透视图。所述陀螺仪包括可旋转盘20,适当地是由无刷直流电动机21驱动的金属盘,例如黄铜盘。在所示的实施例中,陀螺仪由控制电路23控制的电池22形式的小直流电源驱动。在替代布置中,电源和控制电路远离陀螺装置。
陀螺仪的电动机21安装在陀螺仪台24上,陀螺仪台24又安装在安装在铰链板30上的进动铰链25上。铰链板30安装在牢固地固定在设备的手套10上的转台31上。因此,在使用中,陀螺仪的轴线能够通过由进动铰链25和转台31的组合提供的双轴锚定而进动。
进动和旋转轴的控制进动确保了所得到的装置的阻力矢量总是基本上与震颤矢量相反。弹性阻尼器设置在铰链板30和陀螺仪台24之间,以限制进动角度,即进动陀螺仪的轴线与垂直于铰链板30的轴线之间的角度。
在某些实施例中,磁体被并入到所述或每个陀螺装置中,或者代替弹性阻尼器,或者另外控制陀螺仪的进动。例如,一个或多个磁盘或环或环形磁铁设置在陀螺仪盘20周围,以控制其进动。替代布置同样适用,例如弹簧和可变电磁体。此外,可以例如通过步进马达和齿轮装置来控制转台的定向,以通过单个陀螺仪进一步控制多个震颤轴。可附接到手套的外壳32包围陀螺仪,并且在所示的实施例中,提供形式为开关33的致动器,以使马达开启和关闭。
图3以静止方向(图3a)和进动方向(图3b)的横截面中更详细地示出陀螺装置。图3c示出了包括步进电机齿轮组件以向转台提供额外的控制,从而提供进动的附加控制。替代控制系统同样适用,例如其它直接驱动组件或皮带驱动组件。
图4示出了用图1至图3的装置获得的结果。在实验中,将该装置安装在木制艺术家的接合手上,该手连接到具有质心安装在电动机主轴上的电动机的支架上,以引起手的摇晃。在装置附着在手上而不是操作的情况下,观察到每秒约6度的平均角速度幅度,幅度为约‐5.5至+3.5。相比之下,随着本发明的装置的运行,平均角速度幅度减小到大约0.5度每秒,幅度从约‐0.3到约+0.7。在诊断为帕金森氏症并表现出强烈的手震颤的一名70岁女性患者中,定性观察到相同的表现。
在一些实施例中,如下面将进一步详细描述的那样,陀螺仪盘被集成到电动机中并且用作安装有用作外壳的永磁体的电动机。
图5中的几个视图中示出了一替代实施例。在该实施例中,陀螺仪座40形成在手套元件41内或附着于该元件上。安装件40包括用于多个陀螺仪中的每一个的陀螺仪接收杯42和形成用于陀螺装置44的陀螺仪外壳的相关联的可拆卸盖43。在所示的实施例中,安装件40包括将外壳与电池外壳50连接的杆45。杯42和杆45可以与手套一体地形成或形成为可拆卸单元,允许在必要时进行更换,并允许使用具有一定范围的手套尺寸或实际上其他形式的附接到人体的单个安装布置,如皮带、袜子和鞋子等。
陀螺装置44包括陀螺仪盘51和马达52。
陀螺仪接收杯42包括用于将各个陀螺仪耦合到电池电源50所需的电连接,可选地通过陀螺仪的硬布线,但优选地通过电触点或杯内的电焊盘与在陀螺仪单元上其对应的触点或垫相挤压,并通过可拆卸盖43牢固地固定在适当的位置。
通过使用所述类型的安装件40,为每个陀螺仪单独提供外壳,如果需要,可以选择各个陀螺仪以提供彼此的不同转矩,例如,如果震颤在一个特定区域中更为普遍,或者如果震颤在身体的某些区域产生更大的影响。因此,本发明的装置可以调整以满足特定患者的需要。
应当理解,陀螺装置需要牢固地安装到手套或其他可穿戴单元上,使得陀螺仪扭矩传递到佩戴者的身体,而不是被例如手套的拉伸或其它运动吸收。可以通过使用弹性构件,张紧线或类似布置;或者通过将这些元件包含在可穿戴组件的主体内,来实现适当的张紧。
应当理解,与上述现有技术装置中使用的类型的大型陀螺仪相比,微型陀螺仪具有相当小的功率需求。即使使用多个小型陀螺仪,本发明的装置的总功率要求通常明显小于现有技术。因此,可以使用小的、轻便的和不显眼的纽扣电池。
或者,可以使用小的可充电电池,适当地是锂离子或锂离子聚合物电池。在一种布置中,可再充电电池通过诸如在电子设备领域中众所周知的插入式连接器可再充电,例如可通过引线连接到USB电源连接的连接,通常为5伏特。在替代实施例中,可再充电电池通过感应充电装置可再充电。这种布置在本领域中是众所周知的,将不再详细描述。
在另一替代实施例中,在装置中使用的陀螺仪直接由无线电力传输装置供电而不需要电池。然而,这种装置优选地包括用于提供备用电源的电池。
在某些实施例中,例如通过利用光能、电磁辐射、热梯度和运动,提供额外的功率补充。合适的布置包括:
光伏电池收集装置表面上的光能,可选地具有保护下面的电池的透明的耐刮擦保护层;
利用建立在体温和环境之间的热梯度发电的热电发电机。可选地,由电动机产生的热可用于建立相对于合适的冷却装置的热梯度
将机械应变转化为易于变形的手套的电流或电压衬里区域的压电换能器微流量能量收集器通过将振动,剪切和流动形式的动能转换成电能来发电
通过运动捕获的发电被认为是特别有意义的,特别是嵌入式微流体能量收集基于REWOD的振动收集。REWOD过程通过发电而工作,因为导电液滴改变位置或形状,从而改变其与两个带相反电荷的电介质电极的接触面积和位置。包含多个收集器的一个设计包括以下一个或多个:
在振动板之间从电动机的振动中回收能量的导电液滴(例如Galinstan);
结合在手套的手掌侧面(例如),流体通道内衬有沿着装置背面延伸的相关电极的柔性流体腔室;
在手掌表面利用把持期间消耗的能量的压电换能器;
通过使导电液滴沿着电极切变来利用沿着腕关节的材料切变的滑动电极。通过利用环境能量,设备可以实现自主发电。
该装置还可以包括电流或电压调制电路,以允许陀螺仪盘20,51的旋转速度的变化,以允许对陀螺仪转矩进行额外的调整和微调。在包括电信模块的优选实施例中,可以通过电流或电压调整对器件进行微调,例如通过诸如由临床医生远程操作的计算机之类的计算机或借助于便携式设备,优选地是触摸屏设备(例如智能电话或平板计算机),以便患者易于使用。此功能允许在设备上运行的应用程序或程序中加入附加功能和监视,例如显示系统信息和健康指标;并允许用户定制和编程。
在某些实施例中,用于陀螺仪的外壳被密封,并且空腔填充有密度小于大气或空腔处于真空状态,以减小旋转陀螺仪上的空气阻力,从而减少功率消耗并提高效率和可靠性。
尽管迄今为止用一个或两个陀螺仪示出了本发明,但是为了考虑例如多个轴上的震颤,本发明提供了具有任何数量的陀螺仪的装置。
如图5和图6所示,在优选实施例中,陀螺仪盘51有利地由断裂线53和/或指状物54形成。断裂线53形成盘结构中的弱线,使得在损坏陀螺仪的情况下导致损坏盘的情况下,例如可能跟随硬表面落下,盘被以受控的方式断裂。这降低了盘片破裂盖帽43或整个外壳、安装和手套的容纳物的可能性。
此外,陀螺仪可以由制动系统构成,以便在出于使用和安全的原因提供盘的快速减速。再生制动优选用于补充装置的电源。
如图6所示,形成具有多个指状物54的陀螺仪盘51允许调节支腿以调节盘51的径向尺寸,从而调整陀螺仪的扭矩分布。可以调节每个支腿54的径向范围以增加盘的直径,或者可以调整单个支腿以对陀螺装置的特性进行微调。调整陀螺仪的特性的这种方法形成了本发明的一个明显且普遍的方面。
在诸如图6和图8所示的优选实施例中,指状物具有额外的弹性部件,其在失效的情况下引起对旋转轴线的收缩,从而进一步降低了制造部件破坏外壳的风险。适当地,弹性部件由弹性聚合材料或一个或多个弹簧形成。
在优选实施例中,通过抵抗控制装置(例如图7所示的抵抗带或阻尼器)向指状物提供额外的稳定性,其中使用磁流变流体阻尼器55,围绕手套的上表面设置。根据患者治疗的要求,这些元件也可围绕下表面和侧表面安装。这些抵抗方法可以将指状物耦合到主陀螺仪外壳42,以便额外控制震颤。抵抗控制装置的元件可以容纳在与陀螺仪单元分离的层内或作为陀螺仪外壳的组件。
图8示出了具有间隔开的上表面61和下表面62的圆盘形式的改良的陀螺仪盘51的两个变型,以及围绕盘60的外表面周向设置的多个孔63。每个孔63适于安装栓钉64,其将质量添加到盘,主要是到一径向向外的部分,从而提供用于调节陀螺仪的扭矩特性的替代模式。调整陀螺仪的特性的这种方法形成了本发明的一个明显且普遍的方面。
图9至图13示意性地示出了各种陀螺仪布置。各种组件以示意的方式表示,而不是字面上的表示。图9示出了常规结构,其中陀螺仪盘70通过轴71联接到一刷式或无刷直流电动机72。在图10中,通过将电动机82安装在形成在盘80内的中心圆形孔内,使组件变得更紧凑。马达82布置成通过传动轮或齿轮81驱动盘80。
在图11的实施例中,盘90直接与电动机91集成。在图12的陀螺仪中,陀螺仪本身实际上是电动机。陀螺仪盘100中心地安装在陀螺仪内,并且具有围绕其外周安装的具有交替极性的多个磁体101。组件安装在具有安装在框架104内的场磁极103的交变磁场绕组102的环形区域内。陀螺仪盘100用作电动机的转子,并以常规方式通过绕组102的相应交替的极性而旋转。图13的实施例以与图12的实施例相同的方式起作为电动机的作用,其中具有环形交变极性磁体111的盘110具有中心孔112,线圈绕组113经过该中心孔112,该绕组由引线114耦合到供电。
在某些优选实施例中,该装置包括用于去除由旋转陀螺仪产生的热量的热管理系统。适当地,热管理系统包括将热分散外壳用于陀螺仪。优选地,所述壳体包括用于增强排热的散热装置。便利地,诸如手套的可穿戴装置包括热引导以将热量分散到手套上,从而在使用中允许手套温暖使用者的手。有利地,将热反射衬里施加在陀螺装置和可穿戴元件之间以将热量远离穿戴者的身体,从而避免局部加热,并允许热管理系统提供身体的受管理变暖。
在优选实施例中,本发明的装置包括额外的功能,特别是遥测技术,以向临床医生提供关于装置的操作和监测身体的其他参数,例如血压、脉搏、温度和皮电传导水平(如随着出汗等而变化)的反馈。另外,可选地提供全球定位功能(GPS),优选地与跟踪功能一起,使得可以使用该装置跟踪患有额外健康问题(例如痴呆症)的患者。在某些优选实施例中,该装置包括一个或多个加速度计装置。这些将检测佩戴者的突然显著运动,例如在跌倒的情况下。
优选地,该装置还包括电信模块,诸如Wi‐Fi(RTM)、蓝牙(RTM)、GSM、GPRS、3G、4G及其开发成果)。因此,来自装置的数据可以被传送回临床医生或照顾者,或者在潜在的威胁生命的情况下(诸如脉搏的丢失),传送到紧急服务或其他紧急联系人。这种附加功能的构造和编程细节对于本领域技术人员将是显而易见的,将不在此进一步详细描述。
因此,该装置也可用于临床监测退行性疾病如帕金森病的进展,在该疾病中震颤可以每天都起变化;并通过药物或手术或其他技术监测缓解疾病症状的有效性。它也可以用于跟踪佩戴者的一般身体状况和健康。
对装置的进一步改良包括提供近场通信功能。需要精细运动技能的常见社会活动(例如在出纳处取回硬币和支票)对于患有帕金森病、基本震颤或其他肌肉骨骼状况的个人来说很困难,并且对这些情况可能造成严重的污名或沮丧或尴尬。近场通信可以内置在设备中,并通过适当的计算机、平板电脑或智能手机连接进行编程,例如允许非接触式支付和其他支付系统,例如Apple Pay或谷歌电子钱包(Google Wallet)。可以包括与其他设备(例如与脑起搏器)的进一步整合。
远程监控功能有助于降低医疗保健成本,增加用户特别是不可移动患者的便利性。使用GPS功能,该设备还可以充当位置跟踪器,以确定患者的位置。
虽然主要关于手震颤的稳定性进行了描述,但是本发明的装置通过单独使用或与多个装置组合,同样适用于身体的其他部分,包括头部、腿部和手臂。
该装置可用于跟踪震颤,例如物理治疗和训练情况—手术、运动和其他应用。
以下陈述总结了设备的可选附加组件及其优点:
1.社交互动功能—声音、语音和视频通讯、振动、挤压/压力、温暖。如果一个孩子想到祖父母,或者用户互相想起,他们可以挤压设备、相关设备、智能手表或发送智能手机警报,启动压力、温暖,可能添加警报、语音和视频通信。潜在地有助于防止孤立、孤独和认知衰退等。

Claims (28)

16.根据权利要求15的一种装置,其中,所述流体或混合流体陀螺仪的流体是磁流变流体。
17.根据前述任一权利要求的一种装置,其中,每个陀螺装置包括具有形成在其中的多个径向指向的弱线的陀螺仪盘。
18.根据前述任一权利要求的一种装置,其中,每个陀螺装置具有形成有多个径向指状物的陀螺仪盘。
19.根据权利要求18的一种装置,其中,所述指状件的径向长度是可调的。
20.根据前述任一权利要求的一种装置,其中,每个陀螺装置具有由电动机驱动的陀螺仪盘,并且其中该装置还包括用于调节或调制通过该电动机的电流或施加到电动机的电压的调节手段。
