技术领域
本发明涉及一种类似人体骨关节的假体,特别涉及一种人工膝关节假体。
背景技术
人工关节是指用生物相容性和机械性能良好的金属材料制成的一种类似人体骨关节的假体,利用手术方法将人工关节置换被疾病或损伤所破坏的关节面,其目的是切除病灶,清除疼痛,恢复关节的活动与原有的功能。人工关节置换具有关节活动较好,可早期下地活动,减少老年病人长期卧床的并发症等优点,在欧美发达国家,人工关节置换手术已是最常见的手术之一,我国近年来关节置换术的发展也非常迅速。其中,膝关节置换和髋关节置换是人工关节置换术中最常见的两类手术。
现有的人工关节,如人工膝关节,通常由金属材料制成,膝关节与人体骨质接触的金属面,一般都是采用微孔涂层结构,这种结构的缺陷是:涂层与金属面结合强度较差,同时涂层方式制成的微孔结构其参数不好控制,孔隙均匀度较差,影响使用效果。此外,现有的这种多孔涂层使用时容易剥落,产生金属碎屑,使得清洗、消毒和维护工作较为困难。
发明内容
为解决上述问题,本发明提供了一种人工膝关节假体。
根据本发明的一个方面,提供了一种人工膝关节假体,包括股骨髁假体、胫骨垫片和胫骨托假体,所述胫骨垫片位于所述股骨髁假体和所述胫骨托假体之间,所述胫骨托假体包括质密部和位于所述质密部底部的多孔部,所述质密部和所述多孔部3D打印制成一体,所述多孔部的底部还包括有两根侧柱和一个中央凸起,所述质密部伸入到所述侧柱和所述中央凸起的内部;所述多孔部包括多层疏松面,每层所述疏松面上设有多个微孔结构,而各层所述疏松面的孔隙率从内到外依次增大。
在一些实施方式中,所述股骨髁假体的顶部中间具有凸起的凸面,所述凸面的中间设有方形的通孔,外侧伸出有凸起,所述胫骨垫片的顶部上设有一根立柱。由此,可以具有将所述骨髁假体与所述胫骨垫片稳固结合的效果。
在一些实施方式中,所述股骨髁假体的顶部两侧均具有一根安装柱,底部两侧设有接触凸面,所述胫骨垫片的顶部两侧设有与所述接触凸面相配合的接触凹面。由此,可以具有提供另一种所述骨髁假体与所述胫骨垫片结合的实施方式。
在一些实施方式中,所述胫骨垫片的底部的前部设有凹部,后部设有卡条部,所述质密部的顶部的前部设有能够与所述凹部相配合的凸部,后部设有能够与所述卡条部相配合的卡槽部。由此,可以具有将所述胫骨垫片与所述胫骨托假体稳固结合的效果。
在一些实施方式中,所述侧柱的截面为正六边形,所述中央凸起为底部呈球形的圆柱。由此,可以具有与骨面或人体组织配合安装的效果。
在一些实施方式中,所述多孔部包括三层所述疏松面,三层所述疏松面的孔隙率从内到外依次为40%、60%和80%。由此,可以具有与质密部和人体均配合安装的效果,并保持较高的强度和紧密性。
在一些实施方式中,所述多孔部的厚度为1.2-1.8mm,所述微孔结构的孔径为0.25-0.5mm。优选地,每层所述疏松面的厚度均为0.4-0.6mm。由此,微孔部的粘结强度>25MPa,既保证了多孔部和质密部之间的结合部具有较高的力学性能,且保证了假体的强度,同时较小的厚度,又减少了金属碎屑的产生,方便清洗维护。
在一些实施方式中,所述股骨髁假体由钴铬合金制成,所述胫骨垫片由聚乙烯材料制成,所述胫骨托假体由钴铬合金或钛合金制成。由此,可以具有方便适用的效果。

具体实施方式
下面结合附图对本发明作进一步详细的说明。
图1示意性地显示了根据本发明的一种实施方式的人工膝关节假体的整体结构。如图1所示,人工膝关节假体包括股骨髁假体1、胫骨垫片2和胫骨托假体3,胫骨垫片2安装在胫骨托假体3上,而股骨髁假体1放置在胫骨垫片2上。其中,股骨髁假体1一般由钴铬合金制成,胫骨垫片2一般由聚乙烯材料制成,而胫骨托假体3一般由钴铬合金或钛合金制成,也可以根据需求使用其他合适的材料。
图2显示了图1所示人工膝关节假体的股骨髁假体的结构,图3-4显示了图1所示人工膝关节假体的胫骨垫片的结构,图5-6显示了图1所示人工膝关节假体的胫骨托假体的结构。如图2-6所示,股骨髁假体1的顶部中间具有凸起的凸面11,凸面11的中间设有通孔12,朝向外侧的部位伸出有一个凸起15;胫骨垫片2的顶部上设有一根立柱21,底部前部设有凹部23,后部设有卡条部24;胫骨托假体3包括上部的质密部4和底部的多孔部5,质密部4的顶部的前部设有凸部41,后部设有卡槽部42,多孔部5的底部还包括有两根侧柱51和一个中央凸起52,质密部4伸入到侧柱51和中央凸起52的内部。
其中,通孔12能够让医生在进行假体安装手术时视野较为开阔,凸起15和立柱21的作用是在膝关节进行屈曲运动时,能够辅助股骨髁进行后滚运动。而凹部23能够与凸部41相配合连接,凸部41有时会设置辅助连接的连接孔等结构,卡条部24能够与卡槽部42相配合连接,侧柱51的截面为正六边形,中央凸起52为底部呈球形的圆柱,能够与骨面或人体组织配合安装。
在安装时,先安装胫骨托假体3,即先将侧柱51和中央凸起52,然后通过凹部23与凸部41的配合将胫骨垫片2安装到胫骨托假体3上,再将股骨髁假体1安装到胫骨垫片2上。也可以先将胫骨垫片2与胫骨托假体3进行组合后安装,然后再安装股骨髁假体1。
图7显示了图6所示胫骨托假体的分层结构图。胫骨托假体3上部的质密部4的结构较为紧密,在安装时与胫骨垫片2紧密贴合,不仅方便安装,还具有加工方便、强度高和能够防碎屑等多种有益效果。而底部的多孔部5,包括侧柱51与中央凸起52的外层在内,均由多层疏松面50构成,而侧柱51和中央凸起52的内部为与质密部4相连接并为同一材质,可认为是质密部4的一部分。每层疏松面50上均设有多个微孔结构,各层疏松面50的孔隙率从内到外依次增大,其内侧与质密部4紧密连接,外侧与骨面或人体组织直接接触。此外,质密部4和多孔部5都是通过3D打印一体制成的。
较优地,疏松面50的数目设为3个。如图7所示,3个疏松面50从内到外依次为内疏松面501、中疏松面502和外疏松面503,其中最外侧的外疏松面503的空隙率最大,为80%,其有益效果是由于该层是与骨面或人体组织直接接触,较大的空隙率能够有效促进骨长入;而最内侧的内疏松面501与质密部4连接,其孔隙率为40%,具有较高的强度,使得与质密部4之间的连接能够更加紧密,从而提高整个胫骨托假体3的强度;而位于中间的中疏松面502的孔隙率为60%,能够实现外疏松面503和内疏松面501之间的过渡,以保持多孔部5的结构强度。
多孔部5的厚度仅为1.2-1.8mm,其中每层疏松面50的厚度一般相等,均为0.4-0.6mm。而微孔结构的孔径为0.25-0.5mm。由此使得微孔部的粘结强度>25MPa,既保证了多孔部和质密部之间的结合部具有较高的力学性能,且保证了假体的强度,同时较小的厚度,又减少了金属碎屑的产生,方便清洗维护。
图8示意性地显示了根据本发明的另一种实施方式的人工膝关节假体的整体结构。其中图1中所示的人工膝关节称为后交叉韧带替代型膝关节,而图8中所示的人工膝关节称为后交叉韧带保留型膝关节。如图8所述,其中各部件的位置关系与图1中相比并没有变化,只是股骨髁假体10与胫骨垫片20的结构有所不同。
图9显示了图8所示人工膝关节假体的股骨髁假体的结构,图10显示了图8所示人工膝关节假体的胫骨垫片的结构。如图9-10所示,股骨髁假体10的顶部两侧均具有一根安装柱13,用以将股骨髁假体10安装到人体骨骼等组织上,而股骨髁假体10底部两侧设有接触凸面14;胫骨垫片20的顶部两侧设有与接触凹面22,其中接触凹面22能够与接触凸面14配合安装。
此外,该实施例中的胫骨垫片20的底部结构与上一实施例中的胫骨垫片2的底部结构相同,均具有凹部23和卡条部24,可以与胫骨托假体3的凸部41和卡槽部42相配合安装。
以上所述的仅是本发明的一些实施方式。对于本领域的普通技术人员来说,在不脱离本发明创造构思的前提下,还可以做出若干变形和改进,这些都属于本发明的保护范围。

Claims (10)

1.一种人工膝关节假体,其特征在于:包括股骨髁假体(1,10)、胫骨垫片(2,20)和胫骨托假体(3),所述胫骨垫片(2,20)位于所述股骨髁假体(1,10)和所述胫骨托假体(3)之间,所述胫骨托假体包括质密部(4)和位于所述质密部(4)底部的多孔部(5),所述质密部(4)和所述多孔部(5)3D打印制成一体,所述多孔部(5)的底部还包括有两根侧柱(51)和一个中央凸起(52),所述质密部(4)伸入到所述侧柱(51)和所述中央凸起(52)的内部;所述多孔部(5)包括多层疏松面(50),每层所述疏松面(50)上设有多个微孔结构,各层所述疏松面(50)的孔隙率从内到外依次增大。
2.根据权利要求1所述的一种人工膝关节假体,其特征在于:所述股骨髁假体(1)的顶部中间具有凸起的凸面(11),所述凸面(11)的中间设有通孔(12),外侧伸出有凸起(15),所述胫骨垫片(2)的顶部上设有一根立柱(21)。
3.根据权利要求1所述的一种人工膝关节假体,其特征在于:所述股骨髁假体(10)的顶部两侧均具有一根安装柱(13),底部两侧设有接触凸面(14),所述胫骨垫片(20)的顶部两侧设有与所述接触凸面(14)相配合的接触凹面(22)。
4.根据权利要求2或3所述的一种人工膝关节假体,其特征在于:所述胫骨垫片(2,20)的底部的前部设有凹部(23),后部设有卡条部(24),所述质密部(4)的顶部的前部设有能够与所述凹部(23)相配合的凸部(41),后部设有能够与所述卡条部(24)相配合的卡槽部(42)。
5.根据权利要求1所述的一种人工膝关节假体,其特征在于:所述侧柱(51)的截面为正六边形。
6.根据权利要求1所述的一种人工膝关节假体,其特征在于:所述中央凸起(52)为底部呈球形的圆柱。
7.根据权利要求1所述的一种人工膝关节假体,其特征在于:所述多孔部(5)包括三层所述疏松面(50),三层所述疏松面(50)的孔隙率从内到外依次为40%、60%和80%。
8.根据权利要求1所述的一种人工膝关节假体,其特征在于:所述多孔部(5)的厚度为1.2-1.8mm,所述微孔结构的孔径为0.25-0.5mm。
9.根据权利要求1所述的一种人工膝关节假体,其特征在于:每层所述疏松面(50)的厚度均为0.4-0.6mm。
10.根据权利要求1-9其中之一所述的一种人工膝关节假体,其特征在于:所述股骨髁假体(1,10)由钴铬合金制成,所述胫骨垫片(2,20)由聚乙烯材料制成,所述胫骨托假体(3)由钴铬合金或钛合金制成。
