一种胸腰椎骨折后路微创钉棒固定皮外复位系统之角度撑开复位栓
技术领域
本发明是应用于临床骨科、一种胸腰椎骨折后路微创钉棒固定皮外复位系统,属医疗器材。它是运用于人体胸、腰椎后纵韧带完整而不稳定骨折闭合复位内固定器材。该器材的应用,即有利于减轻手术创伤,又有利于减少手术及住院时间。
背景技术
胸腰椎骨折的治疗方法在发展,变化着。Roy-Camille等在1961年开始应用椎弓根螺钉和钢板进行胸腰椎骨折内固定。经过半个世纪的临床应用与多中心研究分析,认为椎弓根螺钉治疗胸腰椎骨折是有效的。后路内固定器械也由钉-板到钉-棒系统演进着。手术创伤也随着内固定器械改进及医生手术熟练程度提高而减小。
目前,钉-棒系统已成为胸腰椎不稳定骨折后路固定治疗的主流。钉-棒系统能为胸腰椎骨折提供稳定内固定,很好地控制骨折的旋转力、有效撑开压缩的椎体,恢复椎体间结构稳定。
随着BO理念的提出,不稳定的胸腰椎骨折内固定由开放性复位到闭合性复位转变,以便减轻软组织损伤。但是,获得和维持胸腰椎闭合复位是相当困难的。现在市面上已有相关的内固定器材出现,但仍有待改进。我们设计了一种胸腰椎骨折后路微创钉棒固定皮外复位系统,即有利于胸腰椎骨折后路微创钉棒系统后路微创钉棒系统置入,又有利于胸腰椎骨折闭合撑开复位及固定。从而,减轻后路微创钉棒系统手术创伤,减少手术时间。
发明内容
本发明的目的是为了克服现有胸腰椎骨折后路内固定器材的不足,提供了一种有效的微创内固定辅助器材。本系统有利于胸腰椎骨折后路微创钉棒系统置入,又有利于胸腰椎骨折闭合撑开复位及固定。提高胸腰椎骨折后路微创内固定钉棒系统连接棒置入的准确性,减小连接棒置入时创伤,并有效撑开压缩的椎体,恢复椎体的高度。是胸腰椎不稳定骨折,后纵韧带完整患者的最佳选择。
为了实现上述目的,本发明采用如下的技术方案:
一种胸腰椎骨折后路微创钉棒固定皮外复位系统之角度撑开复位栓,其特征是正反扣套筒(1)和方形框(2)组成角度撑开复位栓,方形框侧边设有防滑螺丝(3),另一至两端设有螺纹杆(4),一组螺纹杆的螺纹方向相反。上述各构件尺寸为:角度撑开复位栓一端方形框边长1-26mm,螺纹杆长度1-20mm,螺纹方向相反,螺纹杆与方形框角度连接角度为0-20度;角度撑开复位栓另有中间设计为方形框边,两边与螺纹杆成角度相连;正反扣套筒长度30-50mm,内侧两端螺纹方向相反,与角度撑开复位栓螺纹杆相匹配,正反扣套筒外面设为六角边形。
本发明的工作原理为:麻醉成功后,患者取俯卧位置于骨科手术床上,常规消毒铺巾。“C型臂”透视定位伤椎,经上下相邻椎体椎弓根上关节突与横突交汇处置入导针,切开入口皮肤2cm,置入导管扩张,并沿导针放置空心丝攻,进行攻丝,应用空心椎弓根钉起子固定复位套筒与长尾椎弓根钉。经导针植入长尾椎弓根钉。去除椎弓根钉起子,置入固定杆,选适当角度撑开复位栓撑开U型空心长尾椎弓根钉,复位骨折。经旋转臂置入T型棒导针于椎弓根钉尾内,经该导针置入T型棒。T型棒主棒根据需要预弯。定位片控制主棒深度。分别拧入钉帽并锁紧。去除皮外复位系统。折断椎弓根钉长尾及T型柄。清点敷料器械,关闭切口并包扎。全程在“C型臂”监视下操作。
有效益果:
一、安全性:经皮置入椎弓根钉不需要切口显露椎旁肌。全程在“C型臂”监视下操作,经伤椎上下相邻椎体椎弓根上关节突与横突交汇处置入导针,不会损伤神经、血管。胸腰椎骨折后路微创钉棒固定皮外复位系统经皮外操作,角度撑开复位器在“C型臂”监视下操作,不会过度撑开椎体。双角单峰刀片在旋转臂的导引下经皮及椎旁肌至钉尾U型凹槽内,经椎板表面通过,不进入椎管及椎间孔。不会损伤任何组织结构。
二、有效性:目前市面上有多种胸腰椎骨折后路微创系统,临床应用广泛。本系统在现有内固定器械基础上改进,适用于胸腰椎不稳定骨折,且后纵韧带完整的患者。胸腰椎骨折后路微创钉棒固定皮外复位系统提高胸腰椎骨折后路微创内固定钉棒系统连接棒置入的准确性,减小连接棒置入时创伤,并有效撑开压缩的椎体,恢复椎体的高度。从而,减轻后路微创钉棒系统手术创伤,减少手术时间。有利于患者早日康复。
本发明的优点在于:本发明提供角度撑开复位栓有利于压缩椎体的皮外撑开。角度撑开复位栓另有中间设计为方形框边,两边与螺纹杆成角度相连的设计有利于多发椎体损伤多钉固定的选择。
