Abstract

本发明涉及聚桂醇注射液药物应用领域,公开了一种聚桂醇注射液作为治疗剖宫产瘢痕妊娠药物的应用,该药物按如下步骤应用:步骤(1),超声造影确定病灶范围、肌层厚度、病灶部位供血情况;步骤(2),患者取截石位,常规消毒阴道、铺巾;步骤(3),在超声引导下,在孕囊周边肌层和孕囊周围注射聚桂醇注射液;直至超声下见到孕囊环状或片状强化,周边血流稀疏;其中,聚桂醇注射液规格为每10ml注射液中含有100mg的聚桂醇。本发明的聚桂醇注射液作为治疗剖宫产瘢痕妊娠药物的应用,黏附于注射部位血管内,促使纤维化条索代替病理性血管,导致孕囊周围血管永久性闭塞,阻断了病理部位开放的血管,极大地降低了人工流产手术中的出血风险。

Description

聚桂醇注射液作为治疗剖宫产瘢痕妊娠药物的应用
技术领域
本发明涉及聚桂醇注射液药物应用领域,尤其涉及了一种聚桂醇注射液作为治疗剖宫产瘢痕妊娠药物的应用。
背景技术
剖宫产瘢痕妊娠,由于子宫疤痕部位肌层薄弱甚至缺失,在人工流产术时由于子宫切口部位肌层缺乏有效地收缩力,导致局部血管持续开放,造成大出血甚至危及患者生命。
目前子宫切口妊娠治疗方法主要有以下几种:⑴药物(米非司酮片口服或者甲氨蝶呤针肌注)使胚胎活性降低,血供减少后再行胚胎清除术,此法治疗周期长,由于组织物坏死后粘连疤痕部位,在胚胎清除术时出血仍然很多,部分患者需要改行其它方法。⑵宫腔镜下妊娠物清除术。⑶高频聚焦治疗。⑷经腹或者腹腔镜下病灶楔形切除或者子宫切除术。⑸子宫动脉栓塞术,使子宫疤痕部位血供减少,然后再行胚胎清除术,此法在减少胚胎清除术中的出血方面效果肯定,但是技术难度大,专业人员少,培训周期长,设备要求高,需要昂贵的数字化显影技术,费用在10000元左右,难以在基层医院推广;且由于为血管内操作,存在血管破裂,卵巢动脉、肺动脉栓塞风险,一旦出现后果严重。子宫动脉栓塞术后患者疼痛不适明显,患者接受度差。因此,目前尚无一种有效简易并适合各级医院临床推广的治疗方法。
聚桂醇,化学名:聚氧乙烯月桂醇醚,聚桂醇注射液的主要成分是聚乙二醇单十二醚。聚桂醇注射液是治疗食道静脉曲张广泛应用的药物,目前在治疗剖宫产瘢痕妊娠中未见报道。
发明内容
本发明针对现有技术中缺少简单易行适合各级医院临床推广的剖宫产瘢痕妊娠的治疗方法的缺点,提供了一种效果明显,痛感小,技术简单,设备要求低适合于各级医院推广的聚桂醇注射液作为治疗剖宫产瘢痕妊娠药物的应用。
为了解决上述技术问题,本发明通过下述技术方案得以解决:
本发明的治疗剖宫产瘢痕妊娠药物为聚桂醇注射液。在孕囊周边肌层及孕囊周围多点注射聚桂醇注射液后,直接损伤血管内皮,促进血栓形成,黏附于注射部位血管内,继而产生炎性病变和组织纤维化,纤维化条索代替病理性血管,导致孕囊周围血管永久性闭塞,阻断了病理部位开放的血管。由于病理部位血管永久性闭塞,极大地降低了人工流产手术中的出血风险。
作为优选,该药物按如下步骤应用:
步骤(1),超声造影确定病灶范围、肌层厚度、病灶部位供血情况;
步骤(2),患者取截石位,常规消毒阴道、铺巾;
步骤(3),在阴道超声引导下,在孕囊周边肌层和孕囊周围注射聚桂醇注射液;直至超声下见到孕囊环状或片状强化,病灶部位周边血流稀疏;
其中,聚桂醇注射液规格为每10ml注射液中含有100mg的聚桂醇。
作为优选,采用超声诊断仪,探头频率为3~9MHz,探头配置穿刺架进行聚桂醇的介入治疗。
作为优选,剖宫产瘢痕妊娠为子宫剖宫产切口妊娠,符合0~II级超声分级诊断标准的剖宫产瘢痕妊娠。0级:未累及肌层,与肌层分界清楚;I级:稍累及肌层,与肌层分解清楚;II级:位于肌层内,与肌层分界不清,未累及浆膜层;III级:团块或孕囊向浆膜层,并向膀胱方向凸起。
作为优选,步骤(3)之后,还包括宫内物清除步骤:聚桂醇注射治疗后12~36h后,行胚囊清除术和/或清宫术。
本发明由于采用了以上技术方案,具有显著的技术效果:
将聚桂醇注射液注入至孕囊周围,产生无菌性炎症,然后逐渐纤维化,纤维化条索代替病理性血管,导致孕囊周围血管永久性闭塞,阻断了病理部位开放的血管,极大地降低了人工流产手术中的出血风险。创伤小,恢复快,可重复性高,效果确切。临床应用两年,治疗病例100余例,平均出血量5mL,平均住院时间3.2天,平均费用3000元左右,患者无疼痛感,接受度高;技术简单,设备要求低,推广应用简单。
具体实施方式
下面结合实施例对本发明作进一步详细描述。
实施例1
聚桂醇属于聚氧乙烯月桂醇醚类化合物,在国际上用于治疗下肢静脉曲张及食道静脉曲张。聚桂醇注射后,可损伤血管内皮、促进血栓形成,阻塞血管,阻断血供来源;特别在孕囊周边肌层或孕囊周围注射后,在孕囊周围逐渐形成一层致密的纤维组织,造成血管损伤及永久性闭塞。现在国际上还没有报道在超声引导下聚桂醇注射液作为治疗剖宫产瘢痕妊娠药物的应用,由于技术简单易掌握,所需设备仅为超声仪,临床已试行2年,效果肯定,适合在各基层单位推广使用。
1治疗对象
确诊为子宫剖宫产切口妊娠患者,符合0~II级超声分级诊断标准。
2治疗前检查
术前常规检查血尿常规、胸片、心电图、肝肾功能。
3设备
采用百胜MyLab90彩色多普勒超声诊断仪,经阴道探头频率3~9MHz,配备造影匹配成像技术,探头配备穿刺架。
4方法
该药物按如下步骤应用:
步骤(1),超声造影确定病灶范围、肌层厚度、病灶部位供血情况;
步骤(2),患者排空膀胱后取简易膀胱截石位,常规消毒阴道、铺巾;
步骤(3),在阴道超声引导下,采用21g一次性穿刺针经阴道穿刺,在孕囊周边肌层和孕囊周围缓慢多点注射聚桂醇注射液;直至超声下见到孕囊环状或片状强化,周边血流稀疏;
其中,聚桂醇注射液规格为每10ml注射液中含有100mg的聚桂醇。
5宫内物清除
聚桂醇注射治疗后12~36h后,超声仪监护下行胚囊清除术和清宫术。术前开通静脉通路,常规扩张宫颈口至8号,卵圆钳钳夹前峡部孕囊或绒毛组织物,宫腔内常规予以吸刮,术毕宫颈注射缩宫素针10IU,前峡部钳出物常规送病理检查,观察24h无明显阴道流血,予以出院。
6出院后随访
术后第1天以及术后每间隔7天门诊复查血β-HCG直至正常,术后当天给予屈螺酮炔雌醇片(每片含屈螺酮3mg和炔雌醇0.03mg)每日一片共21天预防宫腔粘连,术后7天复查血肝肾功能,术后一周、一月复查B超。

3.根据权利要求1所述的聚桂醇注射液作为治疗剖宫产瘢痕妊娠药物的应用,其特征在于,剖宫产瘢痕妊娠为子宫剖宫产切口妊娠,符合0~II级超声分级诊断标准的剖宫产瘢痕妊娠。
