背景技术

预防或治疗的方法
本发明的技术涉及通过施用某些芳香族阳离子肽来治疗或预防疾病。
所述芳香族阳离子肽是水溶性且高极性的。尽管有这些特性,但所述肽能够容易地穿过细胞膜。芳香族阳离子肽通常包括通过肽键共价连接的最少两个或三个氨基酸、或最少四个氨基酸。芳香族阳离子肽中存在的氨基酸的最大数目为通过肽键共价连接的约20个氨基酸。在一些实施例中,氨基酸的最大数目为约12个、约9个或约6个。
芳香族阳离子肽的氨基酸可以是任何氨基酸。如本文所用,术语“氨基酸”用于指包含至少一个氨基和至少一个羧基的任何有机分子。通常,至少一个氨基位于相对于羧基的α位。所述氨基酸可以是天然存在的。天然存在的氨基酸包括(例如)通常在哺乳动物蛋白中发现的二十种最常见的左旋(L)氨基酸,即,丙氨酸(Ala)、精氨酸(Arg)、天冬酰胺(Asn)、天冬氨酸(Asp)、半胱氨酸(Cys)、谷氨酰胺(Gln)、谷氨酸(Glu)、甘氨酸(Gly)、组氨酸(His)、异亮氨酸(He)、亮氨酸(Leu)、赖氨酸(Lys)、甲硫氨酸(Met)、苯丙氨酸(Phe)、脯氨酸(Pro)、丝氨酸(Ser)、苏氨酸(Thr)、色氨酸(Trp)、酪氨酸(Tyr)、和缬氨酸(Val)。其他天然存在的氨基酸包括(例如)与蛋白质合成无关的代谢过程中合成的氨基酸。例如,氨基酸鸟氨酸和瓜氨酸是在哺乳动物新陈代谢中在尿素生成期间合成的。天然存在的氨基酸的另一个例子包括羟基脯氨酸(Hyp)。
所述肽任选地包含一种或多种非天然存在的氨基酸。最佳地,所述肽不具有天然存在的氨基酸。所述非天然存在的氨基酸可以是左旋(L-)、右旋(D-)、或其混合物。非天然存在的氨基酸是那些通常不在活生物体的正常代谢过程中合成且不在蛋白质中天然存在的氨基酸。此外,非天然存在的氨基酸合适地也不会被常见的蛋白酶识别。非天然存在的氨基酸可以存在于肽中的任何位置处。例如,非天然存在的氨基酸可位于N-端、C-端、或者位于N-端和C-端之间的任何位置处。
非天然的氨基酸可(例如)包括天然氨基酸中不存在的烷基、芳基或烷基芳基。非天然烷基氨基酸的一些例子包括α-氨基丁酸、β-氨基丁酸、Y-氨基丁酸、δ-氨基戊酸和ε-氨基己酸。非天然的芳基氨基酸的一些例子包括邻氨基苯甲酸、间氨基苯甲酸和对氨基苯甲酸。非天然的烷基芳基氨基酸的一些例子包括邻氨基苯乙酸、间氨基苯乙酸和对氨基苯乙酸、以及γ-苯基-β-氨基丁酸。非天然存在的氨基酸包括天然存在的氨基酸的衍生物。天然存在的氨基酸的衍生物可以(例如)包括对天然存在的氨基酸添加一种或多种化学基团。
例如,可以将一种或多种化学基团添加至苯基丙氨酸或酪氨酸残基的芳香环的2’、3’、4’、5’或6’位、或者色氨酸残基的苯并环的4’、5’、6’或7’位中的一者或多者。所述基团可以是可添加至芳香环的任何化学基团。此类基团的一些例子包括支链的或非支链的C1-C4烷基,诸如甲基、乙基、正丙基、异丙基、丁基、异丁基或叔丁基;C1-C4烷基氧基(即,烷氧基);氨基;C1-C4烷基氨基以及C1-C4二烷基氨基(例如,甲基氨基、二甲基氨基);硝基;羟基;卤素(即,氟、氯、溴或碘)。天然存在的氨基酸的非天然存在的衍生物的一些具体例子包括正缬氨酸(Nva)和正亮氨酸(Nle)。
肽中氨基酸的修饰的另一个例子是将肽的天冬氨酸或谷氨酸残基的羧基衍生化。衍生化的一个例子是与氨或伯胺或仲胺例如甲胺、乙胺、二甲胺或二乙胺的酰胺化。衍生化的另一个例子包括与例如甲醇或乙醇的酯化。另一种此类修饰包括赖氨酸、精氨酸或组氨酸残基的氨基的衍生化。例如,这些氨基可以被酰化。一些合适的酰基包括(例如)苯甲酰基或者包括上文提及的C1-C4烷基中任一者的烷酰基,诸如乙酰基或丙酰基。
非天然存在的氨基酸优选地对常见蛋白酶具有抵抗性,并且更优选地对常见蛋白酶不敏感。对蛋白酶具有抵抗性或者不敏感的非天然存在的氨基酸的例子包括上文提及的任一种天然存在的L-氨基酸的右旋(D-)形式、以及L-和/或D-非天然存在的氨基酸。尽管D-氨基酸存在于通过与细胞的正常核糖体蛋白合成机制不同的方法而合成的某些肽抗生素中,但它们通常不存在于蛋白质中。如本文所用,D-氨基酸被认为是非天然存在的氨基酸。
为最大程度降低蛋白酶敏感性,肽应当具有少于5个、优选地少于4个、更优选地少于3个、并最优选地少于2个由常见蛋白酶识别的连续L-氨基酸,而与氨基酸是天然存在的或非天然存在的无关。在一些实施例中,肽仅具有D-氨基酸,而不具有L-氨基酸。如果肽包含蛋白酶敏感的氨基酸序列,则所述氨基酸中的至少一个优选地为非天然存在的D-氨基酸,由此提供蛋白酶抗性。蛋白酶敏感序列的例子包括可由常见蛋白酶诸如肽链内切酶和胰蛋白酶容易地切割的两个或更多个连续的碱性氨基酸。碱性氨基酸的例子包括精氨酸、赖氨酸和组氨酸。
与肽中氨基酸残基的总数目相比,在生理pH下,芳香族阳离子肽应当具有最少数目的净正电荷。生理pH下的净正电荷的最少数目将在下文称为(pm)。肽中氨基酸残基的总数目将在下文称为(r)。下文讨论的净正电荷的最少数目都是在生理pH下。如本文所用的术语“生理pH”是指哺乳动物体的组织和器官的细胞中的正常pH。例如,人的生理pH通常为大约7.4,而哺乳动物中的正常生理pH可以为从约7.0至约7.8的任一pH。
如本文所用的“净电荷”是指存在于肽中的氨基酸所携带的正电荷的数目和负电荷的数目的平衡。在本说明书中,应当理解,净电荷是在生理pH下测定的。在生理pH下带正电的天然存在的氨基酸包括L-赖氨酸、L-精氨酸和L-组氨酸。在生理pH下带负电的天然存在的氨基酸包括L-天冬氨酸和L-谷氨酸。
通常,肽具有带正电的N-端氨基和带负电的C-端羧基。所述电荷在生理pH下彼此抵消。作为计算净电荷的例子,肽Tyr-Arg-Phe-Lys-Glu-His-Trp-D-Arg具有一个带负电的氨基酸(即,Glu)和四个带正电的氨基酸(即,两个Arg残基、一个Lys和一个His)。因此,上述肽具有3个净正电荷。

还重要的是,与净正电荷的总数目(pt)相比,芳香族阳离子肽具有最少数目的芳香基团。芳香基团的最少数目在下文将称为(a)。具有芳香基团的天然存在的氨基酸包括氨基酸组氨酸、色氨酸、酪氨酸和苯丙氨酸。例如,六肽Lys-Gln-Tyr-D-Arg-Phe-Trp具有2个净正电荷(由赖氨酸和精氨酸残基提供)以及3个芳香基团(由酪氨酸残基、苯丙氨酸残基和色氨酸残基贡献)。

本文所述的芳香族阳离子肽可用于预防或治疗疾病。具体地讲,本公开提供了通过施用本文所述的芳香族阳离子肽来治疗处于疾病的危险中(或易受疾病影响)的受试者的预防和治疗方法。因此,本发明方法提供了通过将有效量的芳香族阳离子肽施用给需要该芳香族阳离子肽的受试者来预防和/或治疗受试者的疾病。
在一个实施例中,上文所述的肽可用于治疗与线粒体通透性转变(MPT)有关的任何疾病或病症。减少经受MPT的线粒体的数量并预防MPT很重要,因为MPT与哺乳动物的若干常见疾病和病症有关。此类疾病和病症包括但不限于组织或器官的局部缺血和/或再灌注、缺氧、神经退行性疾病,等等。需要治疗或预防MPT的哺乳动物是那些患有这些疾病或病症的哺乳动物。
哺乳动物的组织或器官中的局部缺血是多层面的病理状况,其由氧气剥夺(缺氧)和/或葡萄糖(例如,底物)剥夺引起。组织或器官的细胞中的氧气和/或葡萄糖剥夺导致能量产生量减少或完全丧失,并随之造成跨越细胞膜的活性离子转运的功能丧失。氧气和/或葡萄糖剥夺还导致其他细胞膜中的病理改变,包括线粒体膜的通透性转变。此外,其他分子(诸如通常在线粒体内区室化的凋亡蛋白)可渗出到细胞质中并造成凋亡细胞死亡。严重的局部缺血可导致坏死细胞死亡。特定组织或器官中的局部缺血或缺氧可能由该组织或器官的供血丧失或严重减少引起。供血丧失或严重减少可(例如)由血栓栓塞性中风、冠状动脉粥样硬化、或周围性血管疾病引起。受局部缺血或缺氧影响的组织通常为肌肉,例如心肌、骨骼肌或平滑肌。受局部缺血或缺氧影响的器官可以为发生局部缺血或缺氧的任何器官。受局部缺血或缺氧影响的器官的例子包括脑、心脏、肾脏和前列腺。例如,心肌局部缺血或缺氧通常由动脉粥样硬化的阻塞或血栓性阻塞引起,动脉粥样硬化的阻塞或血栓性阻塞导致通过心脏动脉供血和毛细血管供血而向心脏组织的氧气输送减少或丧失。此类心脏局部缺血或缺氧可导致受影响的心肌疼痛和坏死,并且最终可导致心力衰竭。骨骼肌或平滑肌的局部缺血或缺氧可由类似的原因引起。例如,肠道平滑肌或四肢骨骼肌的局部缺血或缺氧也可由动脉粥样硬化的阻塞或血栓性阻塞引起。

上述芳香族阳离子肽还可用于预防或治疗胰岛素抵抗、代谢综合征、烧伤和继发性并发症、心力衰竭、糖尿病并发症(例如糖尿病视网膜病变)、眼科病症(例如脉络膜新生血管性疾病、视网膜变性和氧诱导的视网膜病变)。
上述芳香族阳离子肽还可用于减轻有需要的哺乳动物中的氧化损伤。需要减轻氧化损伤的哺乳动物为那些经受与氧化损伤有关的疾病、病症或治疗的哺乳动物。通常,氧化损伤由例如活性氧簇(ROS)和/或活性氮簇(RNS)的自由基引起。ROS和RNS的例子包括羟基自由基(HO·)、超氧阴离子自由基(O2 -)、一氧化氮(NO·)、过氧化氢(H2O2)、次氯酸(HOCl)和过氧亚硝基阴离子(ONOO-)。在一个实施例中,有需要的哺乳动物可为正经受与氧化损伤有关的治疗的哺乳动物。例如,哺乳动物可能正经受再灌注、局部缺血或缺氧。

确定基于芳香族阳离子肽的治疗剂的生物效果。在各种实施例中,执行合适的体外或体内测定来确定具体的基于芳香族阳离子肽的治疗剂的效果以及其施用是否指示适用于治疗。在各种实施例中,可以对代表性的动物模型执行体外测定,以确定给定的基于芳香族阳离子肽的治疗剂是否在预防或治疗疾病或医学病症方面发挥期望的效果。在对人类受试者测试之前,可以在合适的动物模型系统中测试疗法中所用的化合物,所述动物模型系统包括但不限于大鼠、小鼠、鸡、猪、牛、猴、兔等。类似地,对于体内测试,在施用给人类受试者之前,可以使用本领域已知的任一动物模型系统。
预防性方法。在一个方面,本发明提供了一种通过将预防病症的发病或进展的芳香族阳离子肽施用给受试者来预防该受试者中的疾病的方法。在预防性应用中,将芳香族阳离子肽的药物组合物或药剂以一定量施用给易患疾病或病症或者处于疾病或病症的风险中的受试者,所述量足以消除或降低疾病的风险、减轻疾病的严重性或延迟疾病的发作,包括该疾病的生物化学、组织学和/或行为的症状、该疾病的并发症和该疾病发展期间呈现的中间病理表型。预防性的芳香族阳离子肽的施用可在反常的症状特点显现之前进行,使得疾病或障碍得到预防、或者使其进展得到延迟。可以基于上文描述的筛选测定法确定适当的化合物。
治疗性方法。技术的另一方面包括出于治疗目的来治疗受试者的疾病的方法。在治疗性应用中,将一定量的组合物或药物施用给疑似患有或已患有这样的疾病的受试者,所述量足以治愈或者至少部分地抑制该疾病的症状,包括该疾病的并发症和该疾病发展中的中间病理表型。因此,本发明提供了治疗患有疾病或医学病症的个体的方法。
施用模式和有效剂量

本文所述的芳香族阳离子肽可以掺入到药物组合物以单独施用或联合施用给受试者来治疗或预防本文所述的疾病或医学病症。此类组合物通常包含活性剂和可药用的载体。如本文所用,术语“可药用的载体”包括可与药物施用相容的生理盐水、溶剂、分散介质、包衣、抗菌剂和抗真菌剂、等渗剂和吸收延缓剂等。也可将辅助性的活性化合物掺入到所述组合物中。
通常将药物组合物配制成与其计划施用途径相容。施用途径的例子包括肠胃外(例如,静脉内、皮内、腹腔内或皮下)、口服、吸入、经皮肤(局部)、眼内、离子电渗和经粘膜施用。用于肠胃外、皮内或皮下应用的溶液剂或混悬剂可包含以下组分:无菌稀释剂,诸如注射用水、生理盐水溶液、不挥发油、聚乙二醇、丙三醇、丙二醇或其他合成溶剂;抗菌剂,诸如苯甲醇或对羟基苯甲酸甲酯;抗氧化剂,诸如抗坏血酸或亚硫酸氢钠;螯合剂,诸如乙二胺四乙酸;缓冲液,诸如醋酸盐、柠檬酸盐和磷酸盐;以及用于调节渗透压的试剂,诸如氯化钠或右旋糖。可使用酸或碱(诸如盐酸或氢氧化钠)来调节pH。肠胃外制剂可以封装在由玻璃或塑料制成的安瓿瓶、一次性注射器或多剂量瓶中。为了患者或治疗医师的方便,可以以装有一个疗程(例如,治疗7天)所需的所有用具(例如,药瓶、稀释液瓶、注射器和针头)的试剂盒的形式提供给药制剂。
适于注射用途的药物组合物可以包括无菌水溶液(在水溶性情况下)或者用于临时制备无菌注射液或分散液的分散剂和无菌粉末。对于静脉内施用,合适的载体包括生理盐水、抑菌水、Cremophor ELTM(新泽西州帕西波尼的巴斯夫公司(BASF,Parsippany,N.J.))或磷酸盐缓冲盐水(PBS)。在所有情况下,用于肠胃外施用的组合物必须无菌且应当是达到易于注射的程度的流体。其在制造和存储条件下应当稳定且必须妥善保存以避免受到微生物(诸如细菌和真菌)的污染作用。

对于通过吸入法施用,所述化合物可以以气溶胶喷雾的形式从包含合适推进剂(例如,诸如二氧化碳的气体)的加压容器或分配器或者喷雾器进行递送。此类方法包括美国专利No.6,468,798中描述的那些方法。
