Description
技术领域
本发明涉及用于生成组合图像的一种方法和一种混合成像模态。
背景技术
正电子发射断层造影术(简称PET)是这样的一种成像方法,借助其可以示出检查对象之内的放射性物质的分布。在PET中使用发射质子的放射性核素,其中为了记录测量数据而围绕检查对象布置探测器环。在一个发射的质子与一个电子湮灭的情况下释放出两个光子,这两个光子以彼此相反的方向运动。如果借助探测器环在预规定的时间段内采集到两个光子,这被评价为巧合且由此被评价为湮灭现象。
在此单次的湮灭现象还不足以做出关于空间分布的推论。只有通过多次湮灭现象的记载才能从单次“响应线,line of response”(简称LOR)中算出正电子发射断层造影图像数据组。以下将正电子发射断层造影图像数据组的记录理解为湮灭现象的位置分辨的记载以及随后的正电子发射断层造影图像数据组的计算。
根据放射性核素的放射性和所期望信号强度来改变记录持续时间,其持续大约至少一分钟。
放射性核素穿过身体仅仅支持关于放射性核素自身传播路径的陈述。为了能够涉及关于新陈代谢过程的陈述,公知地,将新陈代谢产物配备以放射性核素,其中生物体无法区别原有的和配备了放射性核素的新陈代谢产物。此种注射物质也被称为为放射性药品。
一种公知且经常使用的放射性药品是氟代脱氧葡萄糖(18F-Fluordesoxyglucose)(FDG),其代替葡萄糖代谢。肿瘤细胞相比正常细胞具有更快的新陈代谢。在此也吸收FDG和代谢FDG-6-磷酸盐。因为之后不再发生其它的转换,FDG在肿瘤细胞内积聚。
除了肿瘤细胞以外在其它的处理葡萄糖的组织内也发生积聚。这在一般情况下是不加批判的,因为该身体区域或组织是公知的且可以在正电子发射断层造影图像数据组中被识别。
从Kamel E.M.等人的论文Occult lung infarction may induce falseinterpretation of 18F-FDG PET in primary staging of pulmonary malignancies,Eur J Nucl Med Mol Imaging,32:641-646,2005得知,在肺部的微小梗塞处也会发生FDG的积聚而不是仅仅在肺部肿瘤处,从而发生对正电子发射断层造影图像数据组的错误解释。
发明内容
本发明要解决的技术问题是,给出一种方法和混合成像模态,借助其大体上获得一种在正电子发射断层造影图像数据组中的放大信号强度区域的更好的区分。
该技术问题根据本发明通过用于生成组合图像的方法来解决。本发明的具有优势的扩展是从属权利要求的内容。
根据本发明记录正电子发射断层造影图像数据组和灌注图像数据组。这些既不必对相同的检查区域进行成像,也不必具有相同的分辨率,层厚等等。这些图像数据组仅仅必须在检查对象的成像部分重叠,其中各自信息的组合被看作是必要的。图像数据组的配准也必须是可行的,从而可以进行图像数据组的有意义的组合。
类似于正电子发射断层造影图像数据组的记录,可行的是,将灌注图像数据组的记录理解为得到位置分辨的灌注信息或对此进行表现的图像的所有步骤。在此一个或多个位置分辨的“原始”图像数据组的信号可以任意处理和组合。本质上仅仅是获得位置分辨的灌注信息。
这些不必被绝对量化,也即是说灌注图像数据组不必呈现:灌注在特定的图像元素中表现为4ml/(g×min)。,如果确定相对的量化就是足够的,即,当图像元素的通过相应数值代表的信号强度与其它图像元素对比有双倍强度时,其对应双倍的灌注。
在此不言而喻地假定,图像数据组由图像元素或像素或者体素构成,其中每个图像元素对应于有一个数值,该数值对应所记录的信号强度。图像数据组可以作为矩阵、阵列或任意其它形式存储。为了显示,每个数值对应于灰度值或颜色值,其取决于所使用的颜色调色板。这可以在图像边缘示出和由其可以获悉,哪些颜色对应于哪些数值或值域。在此惯常地设置灰度调色板和颜色调色板。
通常如此构建灰度调色板,使得黑色对应于图像中成像数值的最低值而白色对应于最高值。

在一个控制器装置上任何情况下至少存在一个颜色调色板,其对于对应控制器装置的使用者而言是熟悉的。根据本发明在一个步骤中设置,使得灌注图像数据组的颜色显示被逆转。更准确地说是,颜色调色板的颜色与数值(尤其是存在于灌注图像数据组中的数值)之间的对应被颠倒或逆转。这些可以通过三种方式发生。
首先一种可以对于灌注图像数据组中的每个图像元素的数值确定其倒数。这些以如下公式进行:
倒数=1/数值
然后该倒数如同上文中所描述的那样分别被分配颜色调色板的一个颜色值并且从而产生了灌注图像数据组。当然在此导致一定的失真,因为数一,二和三的距离是相等的,而它们的倒数1,1/2和1/3的距离却不是。
在另外一种构造中因此对于每个图像元素根据公式:
数值_new=数值_max-数值_old
来确定数值。数值_max是灌注图像数据组的所有图像元素的数值的最高值。由此原有的最大数值变成零也就是最小数值。所有其它计算出的新的数值都大于零并且位于零和从原先的最小数值获得的数值之间。原则上可以使用每个其它的、甚至没有在灌注图像数据组中出现过的数值作为数值_max。然后最低值不再等于零,而是大于或小于零。因此会产生“偏移(Offsets)”。
替代地也可以逆转颜色调色板自身,也就是颜色“红”对应于最低数值,等等。也就是最低数值对应于这样的颜色,其一般情况下被分配给最高数值,反之亦然。
颜色调色板的逆转显而易见不受限于所描述的颜色调色板,而是对于任意其它颜色调色板类似地进行。
替代地或附加地为了颜色调色板或灰度调色板的逆转,可以设置阙值和仅仅使用灌注图像数据组的这样的位于阙值以下的区域或图像元素以生成组合图像。在此显而易见对于每个图像元素将数值与阙值相比较并且保留或丢弃。由此立刻可见的是,正电子发射断层造影图像数据组的哪些信号区域位于低灌注区域中而哪些不是。低灌注在此意味着位于阙值以下。
阙值可以在灌注的绝对量化的情况下作为数值预先给定。在仅相对量化的情况下其可以作为百分数取决于灌注图像数据组的图像元素的所有数值的最大值或平均值而被给定。
在逆转灌注图像数据组或提取灌注图像数据组的位于阙值之下的区域之后,灌注图像数据组和正电子发射断层造影图像数据组被叠加成为组合图像,其中在背景中示出灌注图像数据组而在前景中示出正电子发射断层造影图像数据组。
以该方式和方法,将在正电子发射断层造影图像数据组中具有增强的信号强度的、位于减少的灌注的区域之内的区域用警告色加以强调。
在此优选给正电子发射断层造影图像数据组分配这样的颜色调色板,其与灌注图像数据组的调色板相偏差。由此组合图像的由正电子发射断层造影图像数据组所得到的区域,可以更容易地被识别和更好地与灌注图像数据组的区域相区分。
在一种构造中使用磁共振设备作为第二成像模态。磁共振断层造影术是一种公知的、用于显示解剖学结构以及官能参数的成像方法。尤其公知的是,借助磁共振断层造影术来生成灌注图像数据组。
在本发明的一种改进方案中为借助磁共振断层造影术记录灌注图像数据组可以采用一种动脉自旋标记(ASL)的方法。在此准备位于测量层或测量体积上游的层。该准备也被称为自旋标志或自旋标记。所准备的自旋灌注于组织的测量层中而改变所灌注区域的信号强度。由此根据信号改变推断出灌注。为了确定信号改变而记录参考图像,其中在其余相同的记录条件下却不发生所述准备。
存在两种不同的ASL种类,意即脉冲动脉自旋标记(PASL)和持续动脉自旋标记(CASL)。它们的不同之处仅在于如何准备信号的方式和方法,二者都可供使用。如此准备的自旋有时候被标识为内源的示踪剂,而应用的造影剂被称为外源的示踪剂。在本申请中以造影剂来表示待应用的物质,当所述物质存在于检查对象的成像区域时在图像中的对比度表现被改变。
在替代的构造中可以借助造影剂来确定灌注图像数据组。这些通常应用于检查区域的周边,然后被输送进入其中。基于应用之前的检查区域的持续或点状成像,在这期间或一段时间之后,尤其是造影剂注入的动脉阶段期间,可以对血管中的造影剂团和组织中的造影剂的记录进行可视化以及计算位置分辨的灌注信息。
在使用造影剂以生成灌注图像数据组的情况下可能的是,应用其它成像模态。尤其可以使用X射线设备或计算机断层造影设备或超声波设备。优选使用计算机断层造影设备,因为已经存在包括正电子发射断层造影设备和计算机断层造影设备的混合成像模态并且计算机断层造影设备是公知的,借助所述计算机断层造影设备也可以生成简单的X射线检查图像。因为为了确定灌注图像数据组而重复记录单次图像数据组,所以在尽可能小的辐射负荷的情况下获得高的时间分辨率是令人向往的。这些借助X射线检查图像的记录是可行的。
优选使用一种造影剂,其在记录正电子发射断层造影图像数据组的同时作为放射性核素或放射性药物应用。需要根据成像模态来构造所述造影剂。在磁共振设备中例如可以使用钆螯合物或锰螯合物,其中额外地加入放射性核素。另外在磁共振断层造影术中公知有所谓智能的造影剂,其包含这样的部分,所述部分因为新陈代谢过程或其它过程引起造影剂的聚集。该种造影剂也被称作事件标志物或分子标志物。也可以在此种智能造影剂中加入放射性核素以形成放射性药物。
由此可能的是,根据造影剂或放射性核素的时间上或位置上的信号历程来使正电子发射断层造影图像数据组和灌注图像数据组相互配准。在此可以多次应用造影剂,但是一次性提供也可以是足够的。然后首先生成灌注图像数据组,其方式是例如追踪造影剂的记录和灌注。如果放射性核素被加入到放射性药物中,无论如何代谢需要一定的时间。这些被用于生成灌注图像数据组。尤其由于以下可行性,即一次性提供造影剂足以既用于灌注图像数据组的记录又用于正电子发射断层造影图像数据组,这使得此种造影剂的应用是令人感兴趣的,因为在患者的血管系统中仅有有限的可以引入其中的(造影剂)体积。
因为造影剂和放射性核素的同时积聚,在正电子发射断层造影图像数据组区域内构成具有增强的信号强度的区域,所述区域与在灌注图像数据组中的具有增强或减弱的信号强度的区域关于大小、距离和数量相互关联。由此,灌注图像数据组和正电子发射断层造影图像数据组无需其它辅助即可相互配准。
在使用混合成像模态的情况下自身地给出配准。然后可以使用第二成像模态的图像数据组,也就是用于生成灌注图像数据组的那些,以便对借助正电子发射断层造影设备所获得的信号进行运动修正。也可以在正电子发射断层造影设备的运行期间借助第二模态显而易见地获得检查区域的解剖图像数据组。通过多个解剖学图像数据组的比较得到运动信息,其可以在正电子发射断层造影图像数据组的计算期间被考虑。
本发明的基础的技术问题也可以通过混合成像模态来解决。这包括正电子发射断层造影设备和至少一个第二成像模态,尤其是磁共振设备和/或计算机断层造影设备,以及用于执行根据权利要求中任一项的方法所构造的控制装置。
上述方法在控制装置中的执行在此可以作为软件或(固定连线的)硬件来实施。
根据本发明的方法的具有优势的构造对应于根据本发明的混合成像模态的相应构造。为避免不必要的重复因此参考相应的方法特征及其优点。

具体实施方式
图1示出混合成像模态1,其包含磁共振设备2和控制单元4,所述磁共振设备具有布置于其中的探测器环3。为了清晰性起见,未示出磁共振设备2的大量单个组件,例如梯度线圈、激励线圈和探测线圈、患者台以及正电子发射断层造影设备。
正电子发射断层造影设备的探测器环3位于磁共振设备2的主磁场的均匀区域,从而使得可以借助两个模态同时测量。激励线圈,也被称为“身体线圈”,可以与探测器环一起被构建为一个单元,以便不多余地限制用于患者的空间。
同时,通过该固定的结构实现了对借助不同成像模态所记录的图像进行配准。作为软件在控制装置4中实现所描述的方法。尤其它可以在定位患者和提供放射性药物或造影剂之后以“按下按键”来执行。
图2示出用于记录组合图像的流程图。在步骤S1,将患者推入混合成像模态1中,对患者进行定位,并且将磁场进行均匀化。在步骤S2给出优选为18F-FDG的放射性药物。紧接着借助磁共振设备来采集参考图像(步骤S3)。之后才作为步骤S4进行给药提供MR造影剂,优选是具有钆的造影剂。直接在提供MR造影剂之后持续地以多秒(例如10秒)的间隔记录概览图像。除了记录时间点之外,这些概览图像与参考图像相同且概览图像相互之间相同。为了计算灌注图像数据组,原则上仅仅单个概览图像的记录就足够了。在步骤S5中从参考图像和概览图像中确定灌注图像数据组,其方式是从参考图像中逐个图像元素地减去概览图像。其它概览图像用于对借助探测器环3所测量的信号进行运动修正。
执行湮灭现象的采集,直至作为步骤S6从测量数据中至少计算出一个正电子发射断层造影图像数据组。
在步骤S7中通过对颜色对应进行镜像来逆转灌注图像数据组的颜色调色板。额外地在灌注图像数据组上采用阙值,其中所有高于该阙值的图像元素被弃用。
其后在步骤S8中将正电子发射断层造影图像数据组和灌注图像数据组叠加为组合图像,其中灌注图像数据组位于背景中。因为位于具有较小灌注的区域内的放射性药物的积聚,所以所有在正电子发射断层造影图像数据组中的信号区域可以立刻被可视化采集。
图3示出组合图像5,其对肺6进行成像。在此灌注图像数据组7位于背景中而正电子发射断层造影图像数据组8在前景中。在组合图像的一侧布置有颜色刻度9,其示出所使用的颜色调色板。所述调色板和其它显示相比被逆转。
灌注图像数据组7由于可展示性的原因被分成两半示出。在区域10中灌注是较高的,在区域11中是较低的。正电子发射断层造影图像数据组8的信号区域12的一部分在区域10之内而一部分在区域11之内。
由此假设,在区域11中的肺6的降低的灌注归因于肺栓塞,可以将该信息注入到信号区域12的评论中。
图4示出组合图像5的替代的图示。从灌注图像数据组7中仅示出区域11。正电子发射断层造影图像数据组8的信号区域12的一部分由此位于区域11之内而一部分位于其外。所述灌注图像数据组7不对整个检查区域进行成像,因为在生成组合图像5的情况下使用阙值,通过所述阈值将所有带有高于该阙值的数值的图像元素都过滤掉。由此压制了区域10。
图5示出组合图像5的其它替代性的图示。由灌注图像数据组7示出区域10和11。为了分隔这些区域在其中示出一条清晰可见的分隔线13。
从自身即可理解,根据本发明的方法以及根据本发明的磁共振设备相互之间紧密联系,作为方法方面来描述的本发明的特征本质上也可以用于磁共振设备。这些也可以以相反的方式适用于参考磁共振设备所描述的特征,其也可以与所述方法相关。
此外从自身可以理解,参考单个构造所描述的特征也可以在其它的构造和实施例的情况下被实现,除非这些被明确强调地不一样地描述或出于技术原因被自身禁止。
Claims (10)
1.一种用于生成组合图像(5)的方法,其具有步骤:
-记录至少一个对检查区域、尤其至少肺的一部分进行成像的正电子发射断层造影图像数据组(8),
-借助第二个成像模态来记录至少一个对所述检查区域进行成像的灌注图像数据组(7),
-对于所述灌注图像数据组(7)预先给定阙值,并且选择该灌注图像数据组(7)的位于所述阙值之下的区域,和/或逆转该灌注图像数据组(7)的颜色调色板或灰度调色板,
-将所述正电子发射断层造影图像数据组(8)和所述灌注图像数据组(7)组合成组合图像(5)。
2.根据权利要求1所述的方法,其特征在于,使用磁共振设备(2)作为第二成像模态。
3.根据权利要求2所述的方法,其特征在于,为了记录所述灌注图像数据组(7)而引入动脉自旋标记的方法。
4.根据上述权利要求中任一项所述的方法,其特征在于,使用计算机断层造影设备作为第二成像模态。
5.根据上述权利要求中任一项所述的方法,其特征在于,借助造影剂来确定所述灌注图像数据组(7)。
6.根据权利要求5所述的方法,其特征在于,使用如下的造影剂,其在记录所述正电子发射断层造影图像数据组(8)的同时被作为放射性示踪剂使用。
7.根据权利要求6所述的方法,其特征在于,根据所述造影剂在时间上的和在位置上的信号历程,对所述正电子发射断层造影图像数据组(8)和所述灌注图像数据组(7)进行相互配准。
8.根据上述权利要求中任一项所述的方法,其特征在于,根据所述灌注图像数据组的最大值或作为固定值来预先给定所述阙值。
9.根据上述权利要求中任一项所述的方法,其特征在于,借助所述第二成像模态来记录解剖学的图像数据组,这些图像数据组允许对正电子发射信号的运动修正。
10.一种混合成像模态(1),包括正电子发射断层造影设备和至少一个第二成像模态、尤其是磁共振设备(2)和/或计算机断层造影设备,以及包括控制装置(4),所述控制装置被构建为用于执行上述权利要求中任一项所述的方法。
