发明内容
本发明涉及一种用于更换在脊柱中受损伤的椎间盘的装置。该装置包括具有外表面和内表面的第一保持器具,所述第一保持器具的所述外表面与脊柱的第一椎骨可接合。第二保持器具具有外表面和内表面,所述第二保持器具的所述外表面与脊柱的第二椎骨可接合。弹性芯体具有第一表面和第二表面。所述第一表面与第一保持器具的内表面接合。所述第二表面与第二保持器具的内表面接合。所述弹性芯体具有从所述第一表面附近延伸至所述第二表面附近的渐缩的径向外表面。
在本发明的另一方面中,一种用于更换在脊柱中受损伤的椎间盘的装置包括具有外表面和内表面的第一保持器具,所述第一保持器具的所述外表面与脊柱的第一椎骨可接合。第二保持器具具有外表面和内表面,所述第二保持器具的所述外表面与脊柱的第二椎骨可接合。所述第一保持器具具有朝着第二保持器具延伸的突缘。弹性芯体具有第一表面、第二表面和在第一表面与第二表面之间延伸的径向外表面。所述第一表面与第一保持器具的内表面接合。所述第二表面与第二保持器具的内表面接合。内凹表面在第一表面与径向外表面之间延伸。在所述装置没有负载时,该芯体的内凹表面与所述突缘接合。

具体实施方式
本发明涉及一种人造椎间盘或者假体用以更换人的脊柱中的受损伤或者退变的椎间盘。图1至8示出了用以更换脊柱中的受损伤或者退变的椎间盘的人造椎间盘或者假体10的一个示例性实施方案。人造椎间盘10(图1)用于更换位于人脊柱的上下椎骨(未示出)附近之间的受损伤的椎间盘。所述装置或者人造椎间盘10预期可以用在人脊柱的颈椎骨之间。
装置10(图1)包括上或者第一保持器具20,下或者第二保持器具80,以及插入并附接至所述保持器具的弹性芯体120。用在本文中的术语“上”和“下”参考装置10在人体内时的定向。
上保持器具20包括上或者第一保持环或者部件22和端盖24。上保持部件22是刚性的,并由生物相容性材料制成,例如生物相容性金属或者聚合物。上保持部件22预期能够由钛合金制成。上保持部件22具有与椎骨可接合的外或者上表面26。上保持部件22的内表面28附着或者结合至弹性芯体120。内表面28预期可以内凹和/或具有烧结到内表面上的珠体(未示出)或者蚀刻到内表面上的纹理(未示出),从而帮助将上保持部件22连接至芯体120。
多个肋材30(图1至2)从外表面26延伸。尽管上保持部件22显示为具有两个肋材30,但该上保持部件预期可以具有任何数量的肋材30。肋材30从椎间盘10的后侧32彼此大体平行地延伸至该椎间盘的前侧34。肋材30预期可以在任何所需方向延伸。肋材30的每一个具有多个突起36。尽管每个肋材30显示为具有四个突起36,然而肋材可以具有任何所需数量的突起。肋材30延伸的方向可以由椎间盘10的插入方向确定。肋材30和突起36与椎骨接合从而将装置10保持在位于椎骨之间的位置中。
工具接合突起40也从邻近椎间盘10的前侧34的外表面26延伸。工具(未示出)与突起40接合以使装置在椎骨之间的插入过程中保持该装置10。外表面26还具有烧结在该外表面上的珠体(未示出)或者蚀刻到该外表面上的纹理(未示出)用以在椎骨之间进一步保持装置10。
轴向延伸开口42(图2至图3以及图5)延伸通过上保持部件22的外表面26和内表面28。开口42居中放置在肋材30之间。尽管开口42显示为椭圆形状,然而该开口预期可以具有任何所需尺寸和形状。开口42还预期可以不为轴向放置。
上保持部件22具有至少部分地限定了开口42的渐缩表面44。该渐缩表面44(图3)朝着人造椎间盘10的轴线46从外表面26至内表面28渐缩。渐缩表面44与邻近外表面26处的轴线46相隔第一距离。渐缩表面44与邻近内表面28处的轴线46相隔第二距离,该第二距离小于所述第一距离。凹进或者凹槽48形成在渐缩表面44内,用于将端盖24连接至保持部件22。
上外保持部件22的内表面28(图3)附着或者结合至弹性芯体120。上保持部件22包括外周突缘部50,该外周突缘部50朝着下保持器具80延伸。该突缘50围绕芯体120。突缘50(图3至4)具有外凸表面52,其具有朝着下保持器具80面向的最低点或者顶点54。外凸表面52的径向内侧部56从内表面28延伸至在突缘50上的顶点54。外凸表面52的径向外侧部58从顶点54向上延伸,远离下保持器具80。在没有负载施加至椎间盘10的时候,芯体120至少与外凸表面52的径向内侧部56接合。芯体120可以粘附或者结合至外凸表面的径向内侧部56。在没有负载施加至椎间盘10时,芯体120预期可以与外凸表面52的顶点54和/或径向外侧部58接合。内凹表面59从内表面28延伸至突缘50的外凸表面52。内凹表面59具有第一曲率半径R1。
内表面28预期可以内凹。邻近开口42的内表面28的一部分还预期可以与芯体120相隔,直到将预定负载施加至装置10。当将预定负载施加至装置10的时候,芯体120偏转为与所述邻近开口42的内表面28的一部分接合。当芯体120与邻近开口42的内表面28的一部分接合时,由于该芯体的进一步偏转受到保持部件22的限制,因此芯体变硬。
端盖24(图1至3)延伸进入到在保持部件22中的开口42内。端盖24是刚性的,并由生物相容性材料制成,例如生物相容性金属或者聚合物。端盖24预期能够由钛合金制成。端盖24具有面向椎骨的外表面60。该外表面60可具有烧结在外表面上的珠体(未示出)或者蚀刻到外表面上的纹理(未示出)从而进一步将椎间盘10保持在椎骨之间。端盖24的内表面62面向弹性芯体120。
端盖24(图1至图3,以及图6)包括一般为椭圆的主体64。该主体64包括在外表面60中的凹进65。工具(未示出)延伸进入到凹进65中用于将端盖24连接至保持部件22。尽管端盖24的主体64显示为椭圆形,然而主体64预期可以为允许端盖24滑动进入到在保持部件22中的开口42内的所需任何构造。
端盖24的主体64具有渐缩的径向外表面66。在端盖位于开口42中的时候,在端盖24上的该径向外表面66可与在上保持部件22的渐缩表面44接合。多个耳片68从渐缩的外表面66径向向外延伸。耳片68卡入到在上保持部件22的渐缩表面44中的凹进或者凹槽48内,以将端盖24连接至该保持部件。耳片68可以允许端盖24与保持部件22之间的相对移动,同时将端盖保持在开口42中。
下保持器具80(图2至图3,以及图7至图8)为刚性,并由与上保持部件22相同的材料制成,例如钛合金。下保持器具80具有与椎骨可接合的外表面82。下保持器具80的内表面84附着或者结合至弹性芯体120。内表面84预期可以具有烧结在内表面上的珠体(未示出)或者蚀刻到内表面上的纹理(未示出),从而帮助将下保持器具80连接至芯体120。
多个肋材86(图7)从外表面82延伸。尽管下保持器具80显示为具有两个肋材86,然而下保持器具预期可以具有任何数量的肋材86。肋材86从椎间盘10的后侧32彼此大体平行地延伸至该椎间盘的前侧34。肋材86预期可以在任何方向延伸。肋材86的每一个具有多个突起88。尽管每个肋材86显示为具有四个突起88,然而肋材可以具有任何所需数量的突起。肋材86延伸的方向由椎间盘10的插入方向确定。肋材86和突起88与椎骨接合从而将装置10保持在位于椎骨之间的位置中。
下保持器具80的外表面82还包括两个三角形突起90。三角形突起90位于在肋材86之间的椎间盘10的前侧34。突起90还与椎骨接合从而将椎间盘10保持在椎骨之间的位置中。
工具接合突起92从邻近椎间盘10的前侧34的外表面82延伸。工具接合突起92与三角形突起90相比更靠近前侧34放置。工具(未示出)与突起92接合以使装置在椎骨之间的插入过程中保持该装置10。外表面82还具有烧结在该外表面上的珠体(未示出)或者蚀刻到该外表面上的纹理(未示出)用以在椎骨之间进一步保持装置10。
下保持器具80的内表面84(图2至4以及图8)附着或者结合至弹性芯体120。下保持器具80包括外周突缘部94,该外周突缘部94朝着上保持器具20延伸。该突缘94围绕芯体120。突缘94(图4)具有外凸表面96,其具有朝着上保持器具20面向的最高点或者顶点98。外凸表面96的径向内侧部100从内表面84延伸至在突缘94上的顶点98。外凸表面96的径向外侧部102从最高点98向下延伸,远离上保持器具20。在没有负载施加至椎间盘10时,芯体120至少与外凸表面96的径向内侧部100接合。芯体120可以粘附或者结合至外凸表面96的径向内侧部100。在没有负载施加至椎间盘10时,芯体120预期可以与外凸表面96的顶点98和/或径向外侧部102接合。
内凹表面108从内表面84延伸至突缘94的外凸表面96。内凹表面108具有第二曲率半径R2。所述内凹表面108的第二曲率半径R2大于上保持部件22的内凹表面59的曲率半径R1。
保持器具80的的内表面84预期可以内凹。内表面84的一部分还预期可以与芯体120相隔,直到将预定负载施加至装置10。当预定负载施加至装置10时,芯体120偏转为与所述和芯体120相隔的内表面84的一部分接合。当芯体120与该和芯体相隔的内表面84的一部分接合时,由于该芯体的进一步偏转受到保持部件80的限制,因此芯体变硬。
弹性芯体120(图1至3)为一体件,并且可以由聚氨酯硅酮共聚物支撑。弹性芯体120可以任何本领域公知的方式附着或者结合至上和下保持器具20和80。弹性芯体120预期能够在上保持器具20与下保持器具80之间嵌入模制,压铸模制,或者注射模制。芯体120可以通过将用于芯体的材料穿过上保持部件22中的开口42注入而在上保持器具20与下保持器具80之间模制。
弹性芯体120可以为楔形。上保持器具20在邻近椎间盘10的后侧32处与下保持器具80相隔为第一距离。上保持器具20在邻近椎间盘10的前侧34处与下保持器具80相隔为第二距离,第二距离大于第一距离。上保持器具20预期可以与下保持器具80相隔任意所需距离。
芯体120具有上或者第一表面122。上表面122附着至上保持器具20的内表面28。下或者第二表面124附着至下保持器具80的内表面84。芯体120的上表面122从轴线46径向向外延伸的距离大于下表面124从该轴线径向延伸的距离。因此,上表面122延伸越过的面积大于下表面124延伸越过的面积。上表面122和下表面124预期可以外凸。
芯体120包括径向外表面126。上内凹过渡平面128(图4)在径向外表面126与上表面122之间延伸。下内凹过渡表面130在径向外表面126与下表面124之间延伸。上内凹过渡平面128与外凸表面52的径向内侧部56在上保持器具20的突缘50上接合。下内凹过渡平面130与突缘94的径向内侧部100在下保持器具80上接合。
径向外表面126(图3至4)从上内凹过渡平面128至内凹过渡平面130渐缩。因此,该径向外表面126从上表面122附近至下表面124附近渐缩。渐缩表面126从上表面122附近和上内凹平面128至下内凹平面130和下表面124附近朝着椎间盘10的轴线46连续地渐缩。径向外表面126与邻近上表面122和上内凹过渡平面128处的椎间盘10的轴线46相隔第一距离。径向外表面126与邻近下表面124和下内凹过渡平面130的椎间盘10的轴线46相隔第二距离,该第二距离小于第一距离。因此,芯体120具有梯形形状的横截面。所述芯体预期可以不包括内凹过渡平面128和130。如果芯体120不包括内凹过渡平面128和130,则径向外表面126将从上表面122至下表面124渐缩。尽管径向外表面126显示为具有环绕圆周的相同的渐缩角度,但该径向外表面预期可以在芯体120的不同侧具有不同的渐缩角度。
芯体120(图2至3)具有椭圆形凹进140,其从上表面122朝着下表面124延伸。该凹进140至少部分地限定了空闲空间142,该空间142从上表面122朝着下表面124延伸。渐缩表面144从上表面122延伸至表面146(横向于椎间盘10的轴线46延伸)。表面144和146限定了在上表面中的凹进140,该凹进140与椎间盘10同轴并限定了空闲空间142。渐缩表面144从上表面122至表面146朝着椎间盘10的轴线46渐缩。渐缩表面144预期可以在凹进140的不同侧具有不同的角度。当上保持器具20与下保持器具80之间的相对移动时,弹性芯体120可以偏转进入由凹进146限定的空闲空间142中。当芯体偏转进入空闲空间142中的时候,芯体120消耗能量以限制芯体内的应力的量。
尽管将芯体120描述为具有椭圆形凹进,但凹进140预期可以具有任何所需形状或者深度,并且可以位于所述上表面122中的任意所需位置中。芯体120还可以包括从下表面124朝着上表面122延伸的凹进。芯体120还预期可以具有任意数量的凹进。
应该理解上述对本方面的描述易于进行各种修改、变化和适应,并且旨在与所附权利要求等同的含义和范围内进行相同的理解。本公开的实施方案在各个方面都被认为是示例性的,而非限制性。本发明的范围由所附权利要求来指定,而非之前的描述,并且旨在将来自于所附权利要求等同的含义和范围内的所有变化包括在内。
Claims (29)
1.一种用于更换脊柱中受损伤的椎间盘的装置,所述装置包括:
第一保持器具,所述第一保持器具具有外表面和内表面,所述第一保持器具的所述外表面能够与脊柱的第一椎骨接合;
第二保持器具,所述第二保持器具具有外表面和内表面,所述第二保持器具的所述外表面能够与脊柱的第二椎骨接合;以及
弹性芯体,所述弹性芯体具有第一表面和第二表面,所述第一表面与所述第一保持器具的所述内表面接合,所述第二表面与所述第二保持器具的所述内表面接合,所述弹性芯体具有从所述第一表面附近延伸至所述第二表面附近的渐缩的径向外表面。
2.根据权利要求1所述的装置,其中所述弹性芯体的所述第一表面固定附接至所述第一保持器具的所述内表面,所述弹性芯体的所述第二表面固定附接至所述第二保持器具的所述内表面。
3.根据权利要求1所述的装置,其中所述芯体包括在所述第一表面与所述径向外表面之间延伸的内凹表面,所述第一保持器具包括朝着所述第二保持器具延伸的突缘,在所述装置没有负载时,所述突缘与所述芯体的所述内凹表面接合。
4.根据权利要求3所述的装置,其中所述突缘包括外凸表面,所述外凸表面具有在所述第一保持器具的所述内表面与所述突缘的顶点之间延伸的径向内侧部,在所述装置没有负载时,所述外凸表面的所述径向内侧部与所述芯体的所述内凹表面接合。
5.根据权利要求1所述的装置,其中所述芯体包括在所述第一表面与所述径向外表面之间延伸的第一内凹表面,所述第一保持器具包括朝着所述第二保持器具延伸的突缘,在所述装置没有负载时,所述突缘与所述芯体的所述第一内凹表面接合,所述芯体包括在所述第二表面与所述径向外表面之间延伸的第二内凹表面,所述第二保持器具包括朝着所述第一保持器具延伸的突缘,在所述装置没有负载时,所述第二保持器具的所述突缘与所述芯体的所述第二内凹表面接合。
6.根据权利要求5所述的装置,其中所述第一保持器具的所述突缘包括外凸表面,所述第一保持器具的所述外凸表面具有在所述第一保持器具的所述内表面与所述突缘的顶点之间延伸的径向内侧部,在所述装置没有负载时,所述外凸表面的所述径向内侧部与所述芯体的所述第一内凹表面接合,所述第二保持器具的所述突缘包括外凸表面,所述第二保持器具的所述外凸表面具有在所述第二保持器具的所述内表面与所述突缘的顶点之间延伸的径向内侧部,在所述装置没有负载时,所述第二保持器具的所述外凸表面的所述径向内侧部与所述芯体的所述第二内凹表面接合。
7.根据权利要求1所述的装置,其中所述第一保持器具包括保持部件和端盖,所述端盖延伸进入开口中,该开口延伸穿过所述保持部件的所述内表面和所述外表面。
8.根据权利要求7所述的装置,其中所述端盖卡入到在所述保持部件的所述开口中。
9.根据权利要求1所述的装置,其中所述弹性芯体具有表面,该表面从所述第一表面和第二表面中的一个朝着所述第一表面和第二表面中的另一个延伸,并且至少部分限定了从所述第一表面和第二表面中的所述一个延伸的空闲空间。
10.根据权利要求9所述的装置,其中所述第一表面和第二表面中的所述一个包括限定了所述空闲空间的凹进。
11.根据权利要求9所述的装置,其中至少部分地限定了所述空闲空间的所述表面从所述芯体的所述第一表面延伸,所述径向外表面与邻近所述芯体的所述第一表面处的所述装置的轴线相隔第一距离,所述径向外表面与邻近所述芯体的所述第二表面处的所述装置的所述轴线相隔第二距离,该第二距离小于所述第一距离。
12.根据权利要求1所述的装置,其中所述芯体具有梯形横截面。
13.根据权利要求1所述的装置,其中所述芯体的所述第一表面延伸越过的面积大于所述芯体的所述第二表面越过的面积。
14.根据权利要求1所述的装置,其中所述第一保持器具包括朝着所述第二保持器具延伸的突缘,所述第一保持器具具有在所述第一保持器具的所述内表面与所述突缘之间延伸的第一内凹表面,所述第一内凹表面具有第一曲率半径,所述第二保持器具包括朝着所述第一保持器具延伸的突缘,所述第二保持器具具有在所述第二保持器具的所述内表面与所述突缘之间延伸的第二内凹表面,所述第二内凹表面具有与所述第一曲率半径不同的第二曲率半径。
15.根据权利要求14所述的装置,其中所述第一保持器具的所述突缘包括外凸表面,所述第一保持器具的所述突缘的所述外凸表面具有在所述第一保持器具的所述第一内凹表面与所述突缘的顶点之间延伸的径向内侧部,在所述装置没有负载时,所述外凸表面的所述径向内侧部与所述芯体的第一内凹表面接合,所述第二保持器具的所述突缘包括外凸表面,所述第二保持器具的所述突缘的该外凸表面具有在所述第二内凹表面与所述突缘的顶点之间延伸的径向内侧部,在所述装置没有负载时,所述第二保持器具的所述外凸表面的所述径向内侧部与所述芯体的所述第二内凹表面接合。
16.根据权利要求14所述的装置,其中所述芯体的所述第一表面包括限定了空闲空间的凹进,所述第一内凹表面的第一曲率半径小于所述第二保持器具的所述第二内凹表面的所述第二曲率半径。
17.根据权利要求16所述的装置,其中所述径向外表面在所述弹性芯体的所述第一内凹表面与第二内凹表面之间连续渐缩。
18.一种用于更换脊柱中受损伤的椎间盘的装置,所述装置包括:
第一保持器具,所述第一保持器具具有外表面和内表面,所述第一保持器具的所述外表面能够与脊柱的第一椎骨接合;
第二保持器具,所述第二保持器具具有外表面和内表面,所述第二保持器具的所述外表面能够与脊柱的第二椎骨接合;
所述第一保持器具具有朝着所述第二保持器具延伸的突缘,以及
弹性芯体,所述弹性芯体具有第一表面、第二表面以及在所述第一表面和第二表面之间延伸的径向外表面,所述第一表面与所述第一保持器具的所述内表面接合,所述第二表面与所述第二保持器具的所述内表面接合,内凹表面在所述第一表面与所述径向外表面之间延伸,在所述装置没有负载时,所述芯体的所述内凹表面与所述突缘接合。
19.根据权利要求18所述的装置,其中所述弹性芯体的所述第一表面固定附接至所述第一保持器具的所述内表面,所述弹性芯体的所述第二表面固定附接至所述第二保持器具的所述内表面。
20.根据权利要求18所述的装置,其中所述突缘包括外凸表面,所述突缘的所述外凸表面具有在所述第一保持器具的所述内表面与所述突缘的顶点之间延伸的径向内侧部,在所述装置没有负载时,所述外凸表面的所述径向内侧部与所述芯体的所述内凹表面接合。
21.根据权利要求18所述的装置,其中所述第二保持器具包括朝着所述第一保持器具延伸的突缘,所述芯体包括在所述第二表面与所述径向外表面之间延伸的内凹表面,在所述装置没有负载时,在所述第二表面与所述径向外表面之间延伸的所述芯体的所述内凹表面与所述第二保持器具的所述突缘接合。
22.根据权利要求21所述的装置,其中所述第一保持器具的所述突缘包括外凸表面,所述第一保持器具的所述突缘的所述外凸表面具有在所述第一保持器具的所述内表面与所述突缘的顶点之间延伸的径向内侧部,在所述装置没有负载时,所述外凸表面的所述径向内侧部与在所述第一表面与所述径向外表面之间延伸的所述内凹表面接合,所述第二保持器具的所述突缘包括外凸表面,所述第二保持器具的所述突缘的所述外凸表面具有在所述第二保持器具的所述内表面与所述突缘的顶点之间延伸的径向内侧部,在所述装置没有负载时,所述第二保持器具的所述外凸表面的所述径向内侧部与在所述第二表面与所述径向外表面之间延伸的所述内凹表面接合。
23.根据权利要求18所述的装置,其中所述第一保持器具包括保持部件和端盖,所述端盖卡入到开口内,该开口延伸穿过所述保持部件的所述内表面和外表面。
24.根据权利要求18所述的装置,其中所述第一表面和第二表面中的至少一个包括限定了空闲空间的凹进。
25.根据权利要求24所述的装置,其中所述第一表面包括所述凹进,所述径向外表面与邻近所述芯体的所述第一表面处的所述装置的轴线相隔第一距离,所述径向外表面与邻近所述芯体的所述第二表面处的所述装置的所述轴线相隔第二距离,该第二距离小于所述第一距离。
26.根据权利要求18所述的装置,其中所述芯体具有梯形横截面。
27.根据权利要求18所述的装置,其中所述芯体的所述第一表面延伸越过的面积大于所述芯体的所述第二表面越过的面积。
28.根据权利要求18所述的装置,其中所述第一保持器具具有在所述第一保持器具的所述内表面与所述突缘之间延伸的第一内凹表面,所述第一内凹表面具有第一曲率半径,所述第二保持器具包括朝着所述第一保持器具延伸的突缘,所述第二保持器具具有在所述第二保持器具的所述内表面与所述突缘之间延伸的第二内凹表面,所述第二内凹表面具有与所述第一曲率半径不同的第二曲率半径。
29.根据权利要求28所述的装置,其中所述芯体的所述第一表面包括限定了空闲空间的凹进,所述第一内凹表面的所述第一曲率半径小于所述第二保持器具的所述第二内凹表面的所述第二曲率半径。
