Abstract

一种模块化支架血管系统,包括有一胴体、一左锁骨下管体、一左颈总管体和一臂头管体。该胴体呈现管状、有扩张性而具有一下行端、一上行端、一左锁骨下结合部、一左颈总结合部和一臂头结合部以分别结合所述管体。所述管体具气球扩张性或自行扩张性以分别连通左锁骨下动脉、左颈总动脉和臂头动脉于该胴体。通过所述结构,本发明提供的效果得以快速为病患选择适合管径的胴体和管体。又本发明提供的效果可让医疗单位无需储备具有不同管径胴体和管体组合的大量的支架血管本体,而仅需储备相容的管体和胴体以供组合式模块化支架血管系统使用,从而降低备料成本。

Description

模块化支架血管系统
技术领域
本发明关于一种模块化支架血管系统,尤关于一种组合式模块化支架血管系统。
背景技术
如图11所示,第一种既有支架血管本体是用来提供主动脉弓(Aortic arch)手术使用以支撑主动脉弓内面,作为主动脉剥离或主动脉瘤的治疗手段,其包括有一个管状的胴体91、一个左锁骨下分枝管911、一个左颈总分枝管912和一个臂头分枝管913。
该胴体91是以设有扩张支架(expandable stent)的自膨式(self-expandable)弹性材料所制成,其具有二端914、915以连接主动脉的部位。当该胴体91的二端914、915连接于主动脉弓时将自体膨胀,以抵靠于主动脉这些部位的内面。在一端915近旁设有扩张支架,以通过该扩张支架将该胴体91连接于升主动脉(Ascending aorta)。然而,通过该扩张支架所提供的紧迫靠设效果,并不足以承受来自升主动脉的强大血流,因此难以固设该胴体91。所述左锁骨下分枝管911、左颈总分枝管912和臂头分枝管913是同样地以设有扩张支架的自膨式弹性材料所制成,而且被不可脱离地延伸自并连通于所述管状的胴体91,分别被用以通过其自膨特性连通于左锁骨下动脉(left subclavianartery)、左颈总动脉(left common carotid artery)和臂头动脉(brachiocephalic artery又称innominate artery)。
如图12所示,第二种既有支架血管本体类似于上述的第一种既有支架血管本体,其相异处在于第二种既有支架血管本体采用的弹性自膨式管状胴体92是以沿纵长方向交互设置的网架和人工血管材料所制成,而第二种既有支架血管本体采用的左锁骨下分枝管921、左颈总分枝管922和臂头分枝管923则是不可脱离地延伸自并连通于所述管状的胴体92的一个由人工血管材料构成的部分。所述第二种既有支架血管本体通过自膨特性紧迫靠设在主动脉和主动弓的分枝动脉内面。
如图11和12所示,所述第一种和第二种既有支架血管本体包括有一个管状的胴体91、92和不可脱离的分枝管911~913、921~923。在使用时可能发生虽然管状的胴体91、92能够配合病患的主动脉弓,但是全部或至少一个分枝管911~913、921~923和病患的左锁骨下动脉、左颈总动脉和臂头动脉或至少其中之一不能够配合的状况。
然而,在医疗单位接受治疗的不同病患的主动脉管径差异很大,而其左锁骨下动脉、左颈总动脉和臂头动脉的管径也是这样。因为既有支架血管本体在诸如主动脉剥离或主动脉瘤手术使用时,必需使胴体91、92和所有分枝管911~913、921~923皆能够配合病患的主动脉和分枝动脉,使得医疗单位必需花费高额的备料成本,随时准备具有各种不同管径胴体91、92和不可脱离的分枝管911~913、921~923的支架血管本体,才能够满足手术的需求,进而治疗主动脉和分枝动脉管径不同的病患。
一种解决所述不同管径既有支架血管本体库存备料过于庞大问题的方法,是使用客制(custom-made)支架血管本体。但是客制支架血管本体难以接受的昂贵的造价却成为客制支架血管本体另一个明显的缺点。而且在临床上来说,客制支架血管本体更明显的缺点在于其难以即时提供手术使用,特别是难以即时提供紧急手术使用,例如主动脉剥离手术即为一种经常发生于急诊情况的紧急手术。
所述既有支架血管本体结构上的特性,也使得执行诸如急性主动脉剥离或主动脉瘤破裂等手术的医师,在手术前或甚至手术中,需要花费可观的时间挑选支架血管本体,或预先制作好符合病人解剖构造的支架血管,以使胴体91、92和所有分枝管911~913、921~923都能够配合病患的主动脉和分枝动脉,然而如此将额外增加大量手术时间,并使手术过程的不确定性增加,进而影响手术结果。
发明内容
有鉴于前述既有支架血管本体的缺点,本发明的目的在于提供一种模块化支架血管系统。
本发明所提供的模块化支架血管系统包括有:
胴体,其呈现管状、可扩张而具有补强用支架、下降端、上升端、固态左锁骨下设置座、固态左颈总设置座和固态臂头设置座,所述下降端用于连接主动脉弓并延伸自降主动脉的部位,所述上升端用于连接到升主动脉的部位;
左锁骨下管体,其用以连接左锁骨下动脉和所述胴体、有扩张性而设置在所述胴体的左锁骨下设置座,并具有近端和远端,该近端用以连接所述胴体的左锁骨下设置座,该远端则用以连接左锁骨下动脉;
左颈总管体,其用以连接左颈总动脉和所述胴体、有扩张性而设置在所述胴体的左颈总设置座,并具有近端和远端,该近端用以连接所述胴体的左颈总设置座,该远端则用以连接左颈总动脉;
臂头管体,其用以连接臂头动脉和所述胴体、有扩张性而设置在所述胴体的臂头设置座,并具有近端和远端,该近端用以连接所述胴体的臂头设置座,该远端则用以连接臂头动脉。
换言之,本发明所提供的模块化支架血管系统,包括有:
胴体,其呈现管状而具有扩张性,且具有补强用支架、下降端、上升端、固态左锁骨下设置座、固态左颈总设置座和固态臂头设置座,所述下降端用于连接主动脉弓延伸至降主动脉的部位,所述上升端用于连接主动脉弓延伸自升主动脉的部位;
左锁骨下管体,其用以连接左锁骨下动脉和所述胴体、有扩张性而设置在所述胴体的左锁骨下设置座,并具有近端和远端,该近端用以连接所述胴体的左锁骨下设置座,该远端则用以连接左锁骨下动脉;
左颈总管体,其用以连接左颈总动脉和所述胴体、有扩张性而设置在所述胴体的左颈总设置座,并具有近端和远端,该近端用以连接所述胴体的左颈总设置座,该远端则用以连接左颈总动脉;
臂头管体,其用以连接臂头动脉和所述胴体、有扩张性而设置在所述胴体的臂头设置座,并具有近端和远端,该近端用以连接所述胴体的臂头设置座,该远端则用以连接臂头动脉。
通过所述构造,本发明可让进行手术的医师于主动脉手术分别选择适用的胴体、左锁骨下管体、左颈总管体和臂头管体,相比于选择与病患的主动脉和分枝动脉完全相符的既有支架血管本体,利用本发明可以大幅节省时间。
此外,医疗单位仅需要分别库存本发明的模块化支架血管系统的可相互组合的管体和胴体,而不需要大量库存管体和胴体管径组合相异的既有一体成形的支架血管本体,因此利用本发明可以大幅降低库存备料的成本。

具体实施方式
如图1所示,本发明的模块化支架血管系统的胴体的第一实施例,即所述胴体10呈现管状而可扩张,亦即具有扩张性。较佳的是,所述胴体10是自膨式或气球扩张式。所述胴体10包括有补强用支架,较佳的是所述支架是由网架和弹簧材料或其组合所制成。为制造所述支架可选用许多不同的材料,例如镍钛形状记忆合金(Nitinol)、不锈钢或钴铬合金等。所述胴体10另包括有下降端15、上升端16、左锁骨下设置座11、左颈总设置座12和臂头设置座13,所述胴体10通过其自膨性质或气球扩张功能来支撑主动脉内面,以作为主动脉剥离或主动脉瘤手术时的非缝线吻合方式。所述下降端15用于连接主动脉弓并延伸至降主动脉的部位;所述上升端16则连接到升主动脉的部位,以容纳来自上升端16的血流,并让血液通过下降端15流通到降主动脉、通过左锁骨下设置座11到左锁骨下动脉S、通过左颈总设置座12到左颈总动脉L和通过臂头设置座13到臂头动脉B,而所述左锁骨下设置座11、左颈总设置座12和臂头设置座13皆呈固态以设置所述左锁骨下管体42、左颈总管体43和臂头管体41。
前述胴体10可配合图5、6所示管体41、42使用,采用管体41作为臂头管体,并采用管体42作为左锁骨下管体,采用管体43作为左颈总管体。一种采用这样的结构的模块化支架血管系统具有胴体10、左锁骨下管体42、左颈总管体43和臂头管体41,所述胴体10包括有控制管14,所述控制管14延伸自所述胴体10而且连通于所述胴体10,以便对于所述胴体10、所述左锁骨下管体42、所述左颈总管体43和所述臂头管体41进行处理。
再请参照图1所示,上述各管体于装置前呈压缩状态,可让一端置入左锁骨下动脉S、左颈总动脉L或臂头动脉B,而另一端则置入对应的左锁骨下设置座11、左颈总设置座12或臂头设置座13时再将其解开,使各管体张开后可以紧贴在上述结构,以达无缝线吻合的目的;而上述各管体亦可由控制管14处穿入以衔接左锁骨下动脉及左锁骨下设置座,或衔接左颈总动脉及左颈总设置座,或衔接臂头动脉与臂头设置座,而解开管体时,同样可达到无缝线吻合的目的。
所述控制管14亦可在实施手术时,用来引流血液以开始体外循环。
如图4至7所示,多种不同结构的管体41、42、43也可用以配合如图4所示的胴体30构成本发明的模块化支架血管系统以发挥其功能,此外,亦可采用如图2、3所示不同形态的胴体60、20。前述实施例虽然使用特定的管体41、42、43与胴体10,但这是为了描述本发明的模块化支架血管系统的多种具体可行实施例其中之一,而不是为了限定本发明模块化支架血管系统请求保护的范围。
本发明的模块化支架血管系统的胴体的第二实施例,即如图2所示胴体60,包括有下降端65、上升端66、左锁骨下设置座61、左颈总设置座62、臂头设置座63、控制管64和磨擦套67。所述胴体60的下降端65、上升端66、左锁骨下设置座61、左颈总设置座62、臂头设置座63和控制管64的结构相同于第一实施例的胴体10,但胴体60是利用磨擦套67来加强固定下降端65与主动脉弓相连接的部位。
本发明的模块化支架血管系统的胴体的第三实施例,即如图3所示胴体20,包括有下降端25、上升端26、左锁骨下设置座21、左颈总设置座22、臂头设置座23、控制管24和固定环28。所述胴体20的下降端25、上升端26、左锁骨下设置座21、左颈总设置座22、臂头设置座23和控制管24无论在结构上或功能上都基本相同于第一实施例的胴体10。
本发明的模块化支架血管系统的胴体的第四实施例,即如图4和图7所示胴体30包括有下降端35、上升端36、左锁骨下设置座31、左颈总设置座32、臂头设置座33和控制管34。所述胴体30无论在结构上或功能上都和第一实施例的胴体10类似,但胴体30的臂头设置座33延伸至所述胴体30内并具有内部端331,所述内部端331指向所述胴体30的上升端36,借以加强所述胴体30和臂头管体41之间连接的结构,而能够更好地承受通过所述上升端36的强劲血流带来的压力。
如图5所示,第一实施例的管体41可以被当作本发明的模块化支架血管系统的左锁骨下管体、左颈总管体或臂头管体来使用,所述管体41有扩张性。较佳的是,所述管体41是自膨式或气球扩张式且以不透血临床材料所制成,例如是聚酯材料或聚四氟乙烯材料,且其具有内面和表面且在所述管体41内面或表面设有可扩张的支架。所述支架可由网架和弹簧材料或其组合所制成。为制造所述支架可选用许多临床可接受的不同材料,例如镍钛形状记忆合金、不锈钢或钴铬合金。所述管体41具有近端和远端。如图1至4所示,所述管体41的近端用以连接所述左锁骨下设置座11、61、21、31、左颈总设置座12、62、22、32或臂头设置座13、63、23、33。所述管体41的远端则用以连接左锁骨下动脉、左颈总动脉或臂头动脉。
若以所述管体41作为左锁骨下管体使用为例,将其插入胴体10的左锁骨下设置座11和左锁骨下动脉,使所述管体41扩张,例如是通过控制管14对于所述管体41进行处理,使所述管体41扩张而能发挥作为左锁骨下管体使用的效果。扩张的管体41稳固设置在左锁骨下动脉和左锁骨下设置座11的内面,以将来自于所述胴体10的血液流通到左锁骨下动脉。
如图8所示,可使用固定元件来稳固胴体10在其下降端15与降主动脉DA的连接结构。例如采用可调整长度的束带51作为所述固定元件,所述束带51两端511相互结合以发挥稳固效果。又如图9所示,亦可采用另一种可调整长度的束带52作为所述固定元件,所述束带52两端是通过扣夹521结合以发挥稳固效果。再如图10为另种具有倒勾的可调整长度的束带设计。如图2所示胴体60,亦可以采用前述图8的束带51或图9的束带52,或者图10的束带来配合前述磨擦套67使用。
如图6所示,第二实施例的管体42无论在结构上或功能上都基本相同于第一实施例的管体41,并具有表面和多个倒勾421。所述多个倒勾421在管体42表面环设于管体42远端和近端。当被靠设在动脉的内面时,所述多个倒勾421可以加强管体42和动脉连接的结构。所述动脉可以是左锁骨下动脉、左颈总动脉或臂头动脉。如图6、7所示,例如以所述管体42作为左锁骨下管体使用时,所述多个倒勾421勾住动脉的内面以防止脱落。所述在管体42表面环设于管体42近端的多个倒勾421则使管体42被靠设在设置座的内面时,近端所设多个倒勾421可以加强管体42和设置座连接的结构。
如图7所示,第三实施例的管体43无论在结构上或功能上都基本相同于第二实施例的管体41,并具有表面和多个倒勾431。所述多个倒勾431在管体43表面设于管体43近端。当所述管体43被插设在动脉的内面时,所述在管体43表面环设于管体43近端的多个倒勾431则使管体43被靠设在设置座的内面时,近端所设多个倒勾431可以加强管体43和设置座连接的结构。如图7所示当管体43作为左颈总管体使用时,较佳的是使通过管体43近端所设多个倒勾431加强管体43和左颈总设置座32连接的结构。
如图1至7所示,所述每一个实施例的管体41、42、43都可以设置在本发明的模块化支架血管系统的胴体10、60、20、30的任何一个设置座11~13、61~63、21~23、31~33,可以在合理的短时间内为病患依其动脉管径选出合适的管体。因此本发明的模块化支架血管系统能够为手术医师节省时间以迅速选出合适的管体和胴体,并且对于医疗单位的库存备料管理来说也是既经济又有效的。

Claims (10)

1.一种模块化支架血管系统,包括有:
胴体,其呈现管状而具有扩张性,且具有补强用支架、下降端、上升端、固态左锁骨下设置座、固态左颈总设置座和固态臂头设置座,所述下降端用于连接主动脉弓延伸至降主动脉的部位,所述上升端用于连接主动脉弓延伸自升主动脉的部位;
左锁骨下管体,其用以连接左锁骨下动脉和所述胴体、有扩张性而设置在所述胴体的左锁骨下设置座,并具有近端和远端,该近端用以连接所述胴体的左锁骨下设置座,该远端则用以连接左锁骨下动脉;
左颈总管体,其用以连接左颈总动脉和所述胴体、有扩张性而设置在所述胴体的左颈总设置座,并具有近端和远端,该近端用以连接所述胴体的左颈总设置座,该远端则用以连接左颈总动脉;
臂头管体,其用以连接臂头动脉和所述胴体、有扩张性而设置在所述胴体的臂头设置座,并具有近端和远端,该近端用以连接所述胴体的臂头设置座,该远端则用以连接臂头动脉。
2.根据权利要求1所述的模块化支架血管系统,所述胴体包括有表面和摩擦套,所述磨擦套套设在所述胴体的下降端近旁。
3.根据权利要求1所述的模块化支架血管系统,所述臂头设置座延伸至所述胴体内并具有内部端,所述内部端指向所述胴体的上升端。
4.根据权利要求1所述的模块化支架血管系统,所述左锁骨下管体具有表面且在该表面于该左锁骨下管体远端或近端环设多个倒勾。
5.根据权利要求1所述的模块化支架血管系统,所述左颈总管体具有表面且在该表面于该左颈总管体远端或近端环设多个倒勾。
6.根据权利要求1所述的模块化支架血管系统,所述臂头管体具有表面且在该表面于该臂头管体远端或近端环设多个倒勾。
7.根据权利要求1所述的模块化支架血管系统,所述胴体是自膨式或气球扩张式。
8.根据权利要求1所述的模块化支架血管系统,所述支架是埋设或设置于胴体。
9.根据权利要求1所述的模块化支架血管系统,所述左锁骨下管体是以不透血材料所制成,具有内面和表面且在该左锁骨下管体内面或表面设有可扩张的支架;
所述左颈总管体是以不透血材料所制成,具有内面和表面且在该左颈总管体内面或表面设有可扩张的支架;
所述臂头管体是以不透血材料所制成,具有内面和表面且在该臂头管体内面或表面设有可扩张的支架。
10.根据权利要求1所述的模块化支架血管系统,所述胴体的下降端与主动脉连接后,于主动脉外套设可调整长度的束带。
