Abstract

技术领域
本发明涉及用于治疗早产婴儿的组合物。特别地,本发明涉及外源的肺表面活性物质联合抗氧化酶用于降低早产婴儿的肺氧化性损伤和发展为支气管肺发育异常风险的用途。
发明背景
人的肺由大量小的称为肺泡的空气囊组成,在其中进行血液与肺的空气空间之间的气体交换。在健康个体中,这种交换是由包含蛋白质的表面活性剂复合物介导的,所述复合物防止肺在呼气结束时的塌陷。
肺表面活性剂复合物主要由脂质构成,并包含少量的各种蛋白质。这种复合物的水平不足导致肺的机能紊乱。这种综合征被称为呼吸窘迫综合征(RDS),而且它通常影响早产婴儿。
RDS的治疗主要是利用外源的肺表面活性物质制剂进行的替代治疗。
外源的肺表面活性物质目前作为在盐水溶液中的悬浮液通过气管内滴注给予保持在用氧进行机械换气的情况下的插管的早产婴儿。
虽然所述治疗已经大大增加了出生后的生存率,但是经历RDS生存下来的幼儿具有发展为支气管肺发育异常(BPD)的高风险,支气管肺发育异常(BPD)是一种损害肺发育并最终导致削弱的呼吸的并发症。
证据表明肺组织的氧化损伤在BPD的病理发生中起重要作用,而且高氧症中降低的抗氧化酶活性导致对支气管肺发育异常增加的敏感性。
为了中和反应性氧类的作用,已经建议为表面活性剂制剂增补抗氧化酶,如过氧化物歧化酶和催化酶,早产婴儿的肺中经常缺乏这些酶。
然而,至今没有提供关于所述酶在人体中的治疗有效,而不改变表面活性物质的表面活性或导致其他副作用迹象的剂量。例如,已经报道通过过氧化物歧化酶清除反应性氧类可能损害吞噬细胞的杀菌活性。
因此仍然需要开发一种安全药物,其能有效地减少氧化胁迫的标记并因此降低遭受RDS的婴儿中BPD的风险。
发明概述
第一个方面,本发明涉及一种联合2000到10000U/kg单位剂量的CuZn过氧化物歧化酶和2000到4000U/kg单位剂量的催化酶的外源肺表面活性物质,其用于预防支气管肺发育异常(BPD)。
优选地,外源的肺表面活性物质是经修饰的天然肺表面活性物质。优选的经修饰的天然肺表面活性物质是猪肺磷脂(poractant alfa),更优选以100到200mg/kg的单位剂量施用。
第二个方面,本发明涉及一种组合:
-外源的肺表面活性物质;
-2000到10000U/kg单位剂量的CuZn过氧化物歧化酶;2000到4000U/kg单位剂量的催化酶,用于同时,顺序或单独的施用以预防支气管肺发育异常(BPD),优选用于同时施用。
第三个方面,本发明涉及一种药物,其含有外源的肺表面活性物质,与2000到10000U/kg单位剂量的CuZn过氧化物歧化酶和2000到4000U/kg单位剂量的催化酶的固定组合。
所述药物有利地为适于吸入或气管内施用的形式。
第四个方面,本发明涉及一种试剂盒,其含有:a)第一单位剂型的外源的肺表面活性物质和药学上可接受的载体或稀释剂;b)第二单位剂型的2000到10000U/kg单位剂量的CuZn过氧化物歧化酶,和任选地药学上可接受的载体或稀释剂;c)第三单位剂型的2000到4000U/kg单位剂量的催化酶,和任选的药学上可接受的载体或稀释剂;
d)用于容纳所述第一,第二和第三剂型的容器。
根据一个特定的方面,过氧化物歧化酶和催化酶可以存在于单个剂型中,同时任选地和一种药学上可接受的载体或稀释剂。
第五个方面,本发明涉及外源的肺表面活性物质联合2000到10000U/kg单位剂量的CuZn过氧化物歧化酶和2000到4000U/kg单位剂量的催化酶用于制备预防支气管肺发育异常(BPD)的药物的应用。
第六个方面,本发明涉及一种通过给需要的患者施用外源的肺表面活性物质联合2000到10000U/kg单位剂量的CuZn过氧化物歧化酶和2000到4000U/kg单位剂量的催化酶来预防支气管肺发育异常的方法。
定义
术语“抗氧化酶”指催化过氧化物歧化成为氧和过氧化氢的酶,该过氧化物是细胞中主要的反应性氧类之一。
“CuZn过氧化物歧化酶”指具有与无论是人类,牛或其他哺乳动物来源的,天然存在的CuZn过氧化物歧化酶的氨基酸序列相同的,或基本上相同的氨基酸序列,并具有这种天然存在的CuZn过氧化物歧化酶的生物学活性的蛋白。

“单位剂量”指单个剂量中施用给患者的表面活性物质或酶的量。
对于酶,剂量表示为U/kg体重。
1U定义为每分钟催化1微摩尔底物转变的酶的量。
由于早产婴儿的体重为大约500克到1500克,可据此计算单位剂量。
例如,2000到10000U/kg单位剂量的CuZn过氧化物歧化酶相当于大约1000到15000U,而2000到4000U/kg剂量单位的催化酶相当于大约1000到6000U。
术语“支气管肺发育异常(BPD)”指慢性肺紊乱,亦称慢性肺疾病(CLD),其是未解决的或异常修复的肺损伤的结果。
BPD一般发生在极低出生体重(VLBW)的婴儿中,其持续肺损伤,所述肺损伤是由于早期机械换气而致的肺氧中毒和气压损伤的结果。
术语“外源的肺表面活性物质”指模拟可用于预防和/或治疗与表面活性物质缺乏或机能障碍相关的疾病的内源性肺表面活性物质的组合物。
根据Wilson D(Expert Opin Pharmacother 2001,2(9),1479-1493),外源的肺表面活性物质可以分成4种不同的类型:
i)“天然”表面活性物质,其是从肺或羊水回收的完整物,而不是提取物,并且具有天然,内源表面活性物质的脂质和蛋白质组成。由于它们不能被灭菌,因为加热使亲水性蛋白质SP-A和SP-D变性,它们携带可能的感染风险。这些表面活性物质是不能商业获得的;
ii)“经修饰的天然”表面活性物质,其是切碎的哺乳动物肺或肺冲洗液的脂质提取物。由于生产过程中使用的脂质提取工艺,亲水性蛋白质SP-A和SP-D丢失。取决于提取方法,这些制剂具有可变含量的SP-B和SP-C,可包含非表面活性物质脂质,蛋白质或其他的组分。一些市售的经修饰的天然表面活性物质,如Survanta(vide ultra)包含合成组分如三棕榈酸甘油酯,二棕榈酰磷脂酰胆碱和棕榈酸。
iii)“人工”表面活性物质,其只是合成的化合物的混合物,主要是磷脂及其他脂质,其被制备成模拟脂质组成和天然表面活性物质的行为。它们缺乏表面活性物质载脂蛋白;
iv)“重组的”表面活性物质,其是其中添加有从动物分离的表面活性剂蛋白质/肽,或通过如WO 95/32992中描述的那些重组技术生产的蛋白质/肽,或如WO 89/06657,WO 92/22315和WO 00/47623中描述的合成的表面活性剂蛋白类似物的人工表面活性物质。
术语“猪肺磷脂”指从猪肺提取的经修饰的天然表面活性物质,其基本上由极性脂质组成,主要是磷脂和蛋白质,SP-B和SP-C。猪肺磷脂可获自商标
术语“固定组合”指其中活性物质处于固定的定量比例的组合。
“药学上可接受的”指当施用给婴儿时,不产生过敏反应或类似的不良反应的介质。
“表面活性剂活性”,用于肺表面活性物质制剂时,定义为降低表面张力的能力。
通常利用适合的装置如Wilhelmy Balance,脉冲气泡表面张力计,捕获气泡表面张力计和毛细管表面张力计,通过测定它降低表面张力的能力来测定外源表面活性剂制品的体外功效。
根据已知的方法,通过测定早产动物模型中的肺力学测定外源表面活性剂制品的体内功效。
术语“严重的RDS”指一种与孕龄和/或出生体重逆相关的婴儿呼吸窘迫综合征,其根据已知的方法进行临床和/或放射学诊断。
术语“协同的”指两个化合物的活性高于预期的通过将给定分析中它们各自单独活性的加和作用。

发明详述
本发明基于出乎意料的发现,CuZn过氧化物歧化酶和催化酶,以某种剂量,协同作用于对外源肺表面活性物质的反应性氧类的清除活性。
因此,本发明涉及一种联合2000到10000U/kg单位剂量的CuZn过氧化物歧化酶和2000到4000U/kg单位剂量的催化酶的外源肺表面活性物质,用于预防支气管肺发育异常(BPD)。
与现有技术相反,以要求的单位剂量,利用所述抗氧化酶没有灭活经修饰的天然肺表面活性物质。
而且,利用酶的组合可以增强所有表面活性物质的清除活性,同时,如实施例1中所示,添加单个酶的作用似乎依赖于它们的组成。
任何外源的肺表面活性物质都适用于本发明,有利地,外源肺表面活性物质选自经修饰的天然肺表面活性物质,人工表面活性物质和重组的表面活性物质。更有利地,可使用目前用于治疗呼吸窘迫系统和其他肺疾病的经修饰的天然肺表面活性物质。这些表面活性物质包括,但不限于,牛脂质表面活性物质(BLES Biochemicals,Inc.London,Ont),calfac ant(卡尔法坦)(ONY,Inc.Amherst,NY,USA),bovactant(勃法克坦)(BoehringerIngelheim Pharma,Ingelheim,Germany),Surfactant TA(Tokyo Tanabe,Japan),猪肺磷脂(Chiesi Fafmaceutici SpA,Parma,Italy),beractant(贝拉康坦)(Abbott Laboratories,Inc.,Abbott Park,IL).
优选地,经修饰的天然肺表面活性物质是猪肺磷脂。
有利地,表面活性物质单位剂量范围为大约20mg/kg到大约300mg/kg,优选80到200mg/kg。
当然,应当理解,表面活性物质的精确剂量将取决于下列因素如给药途径,患者的年龄和状况,待治疗的疾病的严重程度和熟练医生已知的其他因素。
根据本发明的优选实施方案,猪肺磷脂以100到200mg/kg的单位剂量施用。
抗氧化酶CuZn过氧化物歧化酶和催化酶可以是不同来源的,且可以是市售的,例如获自Sigma Chemical Co.,St Luis,MO,USA。
另外,可以根据已知方法通过重组技术制备这些抗氧化酶。
有利地,使用2000到10000U/kg,优选2500到8000U/kg,更优选2500到6000U/kg剂量的人CuZn过氧化物歧化酶。
优选使用来自红细胞或肝脏的人CuZn过氧化物歧化酶,或者人重组CuZn过氧化物歧化酶。
有利地,使用人催化酶,以2000到4000U/kg,优选2500到3500U/kg,更优选3000U/kg的剂量施用。
优选使用来自红细胞或肝脏的人催化酶,或者人重组催化酶。
考虑到酶的分析,熟练技术人员可以确定以U表示的比活性与mg蛋白(酶)之间的对应。
外源的肺表面活性物质与CuZn过氧化物歧化酶和催化酶以要求的剂量的组合可以顺序,分开或同时施用。
有利地,当同时施用3种组分时,它们以固定的组合施用。
因此,本发明还涉及一种包含所述3种活性组分的药物,其作为固定的组合,以药物组合物的形式,例如以溶液、分散体、悬浮液或干粉的形式。优选地,所述组合物包含悬浮于合适的生理学容许的溶剂中的要求的组合。
更优选地,制剂包含水溶液,优选无菌的水溶液,其也可包含pH缓冲剂,及其他药学上可接受的赋形剂如吐温20,吐温85或斯潘20作为湿润剂,和氯化钠作为等渗剂。
制剂可以存在于单位剂量容器或多剂量容器中,例如密封的安瓿瓶和小瓶,或储存于冷冻或冷冻干燥的(冻干)条件中,只需要在使用之前立即添加无菌液体即可。
优选地,制剂以置于单次使用的小瓶中的溶解于缓冲生理盐水(0.9%w/v氯化钠)水溶液的无菌悬浮液提供。
根据本发明的一个优选的实施方案,制剂包括浓度为40到80mg/ml,优选80mg/ml的猪肺磷脂。
所要求的组合物可以已知的方式施用,例如通过气管内滴注,喷雾给药,或通过喷射超声波雾化,或一般市售的网孔式振动雾化器给药。
优选地,所要求的组合通过气管内施用。
实际上已经发现,当以这些剂量通过该途径施用时,CuZn过氧化物歧化酶和催化酶没有下调内源抗氧化酶的mRNA转录。
当该组合物通过气管内滴注施用时,取决于呼吸窘迫综合征的严重程度,不同的方法可能都是合适的。例如,可以通过气管内滴注把所述的制剂施用给处于连续或间歇性正压换气下的早产婴儿。
做为选择,该组合的施用可以联合非侵入的肺呼吸性治疗,其包括给予正压通气。术语“无创肺呼吸治疗”指没有使用机械通气,而且根据这些方法,和通过使用熟练技术人员已知的设备,可包括持续正压通气(CPAP),双相气道正压通气(BiPAP),同步间歇指令通气(SIMV)等等的呼吸治疗。
在一个特定的实施方案中,可以如WO 2008/148469中所述的进行给药,例如,通过使用置于气管和由特别设计的鼻设备如口罩、叉或管支持呼吸的患者中的薄导管,根据称为鼻持续正压通气的方法给药。
根据熟练技术人员已知的方法,其他传递方法包括灌洗,肺冲洗等等。
其中悬浮有组合的活性组分的水溶液的体积取决于所需的浓度。
有利地,制剂的体积应当不超过5.0ml,优选4.5到2.0ml,更优选3.5到2.5ml。
做为选择,当外源的肺表面活性物质与两种抗氧化酶分开施用时,可以独立配制每种独立的活性组分。在这种情况下,单独的活性组分不必无条件地同时服用。
在分开施用的情况下,每种单独的活性组分的制剂可以同时包装于合适的容器中以形成试剂盒。
因此,本发明还涉及一种试剂盒,其包括:a)处于第一单位剂型中的外源肺表面活性物质与药学上可接受的载体或稀释剂;b)处于第二单位剂型中的2000到10000U/kg单位剂量的CuZn过氧化物歧化酶,和任选地药学上可接受的载体或稀释剂;c)处于第三单位剂型中的2000到4000U/kg单位剂量的催化酶,和任选地药学上可接受的载体或稀释剂;d)用于容纳所述第一,第二和第三剂型的容器。
根据一个特定的方面,过氧化物歧化酶和催化酶可以独特的剂型提供,以及任选药学上可接受的载体或稀释剂。
优选地,第一单位剂型是装有2.5ml的80mg/ml猪肺磷脂无菌制剂的单次使用小瓶,该无菌制剂悬浮于缓冲生理盐水(0.9%w/v氯化钠)水溶液中。
根据熟练医生确定的状况,本发明的组合物可以施用给出生后的婴儿,本发明的组合物适于预防,延缓,减轻,阻止或抑制支气管肺发育异常的发展。
本发明组合物的适合剂量学((剂量,给药频率)将取决于患者的年龄和疾病的严重程度,而且参加的临床医生可以很容易地确定。
剂量给药的频率可以改变,但是一般为每2-3天一次。在其他的实施方案中,患者的剂量给药更频繁,例如每6-8小时,每天两次,或每天一次,或者更少的频率,例如,每周两次或甚至每周一次。在其他的实施方案中,在治疗方案的初期患者的剂量给药更频繁,而在治疗方案的后期降低频率,例如,每隔一天一次,持续一周,接着每周两次直到治疗周期结束。取决于剂型,例如,相较于液体悬浮液,气雾剂或干粉,患者可以连续剂量给药持续治疗周期的一段时间或整个治疗周期。
优选地,本发明的组合适于预防患有重型呼吸窘迫综合征(RDS)的早产婴儿的支气管肺发育异常的发展。
但是,它还可用于预防和/或治疗与内源表面活性物质缺乏或机能障碍相关的其他疾病,例如成人的呼吸窘迫综合征(ARDS),急性肺损伤(ALI)和胎粪吸入综合征(MAS)。
下面的实施例更详细的说明了本发明。
实施例1-体外评估添加CuZn过氧化物歧化酶,催化酶或两者以后经修饰的天然表面活性物质的清除活性
这种“体外”研究的目标是确定添加不同含量的人CuZn过氧化物歧化酶,人催化酶或两者以后,经修饰的天然肺表面活性物质的对H2O2的清除活性改变。使用4种经修饰的天然的市售肺表面活性物质:
人红细胞Cu/Zn过氧化物歧化酶(下文为SOD)和人红细胞过氧化氢酶(下文为CAT)购自Sigma Chemical Co.,St.Louis,MO,USA。SOD的剂量以mg表示。应用的SOD具有每mg蛋白(酶)2500-6000U的比活性。
测定选择的天然表面活性物质中的SOD和CAT活性。根据制造商的指导,使用过氧化物歧化酶分析试剂盒(Cayman Chemical,AnnArbor,MI,USA),测定SOD活性。该分析使用四唑盐用于检测由黄嘌呤氧化酶和次黄嘌呤产生的超氧化物自由基。利用具有450nm滤光器的平板读数器,通过测定96孔板中产生的超氧化物自由基的歧化来评估SOD活性。1个单位的SOD定义为抑制50%的超氧化物自由基歧化所需的酶量。SOD活性表示为U/毫克磷脂表示。
利用催化酶分析试剂盒(Cayman Chemical,Ann Arbor,MI,USA)确定催化酶活性,其利用CAT的过氧化物功能来确定酶活性。它基于最佳浓度的H2O2与甲醇的酶反应。利用4-氨基-3-肼基-5-巯基-1,2,4-三唑(Purpald;Aldrich chemical company,Milwaukee,WI,USA)作为发色团,用分光光度法测定产生的甲醛。从标准曲线获得样品的甲醛浓度。1个单位的CAT活性定义为25℃时,导致每分钟每毫克蛋白形成1.0nmol甲醛的酶量。CAT的活性记录为nmol/min/mg磷脂。
为了测定对H2O2的清除活性,相同含量的来自每种天然表面活性物质的磷脂(4.72mg=100μmol磷脂),用25,50,100,250μM H2O2于37℃孵育2小时。然后,样品以16.000x g离心45分钟。如Casella等所述,通过监控ABTS[2,2’-联氮-二-(3-乙基苯并噻唑啉-6-磺酸)自由基的形成,来测定H2O2的浓度。(Inorg Biochem 2000,79,31-40)。
而且,添加SOD或CAT以后,测定4种经修饰的天然肺表面活性物质的清除活性。特别地,分别添加1mg或2mg SOD,和3000U或30000U CAT以后,评估0.60mL0.50mL0.80mL和0.44mL的清除活性。
用100μM H2O2于37℃孵育这些样品2小时,然后如上面报道的测定H2O2的浓度。
图1报道了添加SOD以后的结果,图2报道了添加CAT以后的结果。
为了评估SOD和CAT可能的协同作用,评估了添加1mg SOD和3000单位的CAT以后,0.60mL Infasurf,0.50mL Curosurf,0.80mL Survanta和0.44mL Alveofact的清除活性。图3报道了结果。
收集的数据以平均和标准偏差表示。通过这些组内的ANOVA测验,和通过这些组间的学生氏测验,分析数据的统计学显著性差异。p<0.05认为是统计上显著的。
意外地,的清除活性降低。不受理论限制,我们推测SOD的过量表达可通过不同的机制导致增加的氧化应激的事实能解释的这种特别的表现,如H2O2浓度增加,羟基形成和过氧化物介导的膜过氧化作用的抑制,其通过充当基团连锁反应中断者发挥作用。
但是,在这种情况下,CAT从3000到30000U,清除活性也没有显著地改变。
最后,从图3可以理解,当添加SOD和CAT到表面活性物质中后,在所有研究的表面活性物质中都观察到对H2O2的清除活性很显著的增加。而且,这种增加似乎高于单一添加SOD和CAT后所观察到的,这表明它们的作用是协同的。
实施例2-联合CuZn过氧化物歧化酶和催化酶的体内评估
进行体内研究以评估用添加SOD和CAT经修饰的天然肺表面活性物质Curosurf处理后患有RDS的早产羔羊中的肺组织氧化应激。
为了研究肺组织的氧化应激,测定支气管的吸出液(BAL)中的总的氢过氧化物(TH),提高的氧化蛋白产物(AOPP),异前列烷(F2-IP)和非转铁蛋白结合铁(NTBI)。
对患有RDS的12只早产羔羊(胎龄126+3天)进行了研究。分娩后立即对它们插管。最初的换气设定如下:FiO2100%,RR 60/min,PIP 16-18cm H2O,PEEP 4cm H2O。然后,调整机械换气用于保持pH>7.20,pCO235-65托耳,pO255-80托耳,SaO285-95%)。动物连接仪器,并随机接受天然的外源表面活性物质(Chiesi,Parma,Italy:200mg/kg),或相同含量的联合SOD(1mg/kg)和CAT(3000U/kg)的表面活性物质。
研究时间为6小时。改变pH,pCO2,pO2,SaO2和BE,换气设定,肺力学(潮气量,动态柔顺性,气道阻力),以预定的间隔记录平均全身血压和肺压力。
为了研究肺组织的氧化应激,测定支气管的吸出液(BAL)中的总的氢过氧化物(TH),提高的氧化蛋白产物(AOPP),异前列烷(F2-IP)和非转铁蛋白结合铁(NTBI)的量。使用Buonocore G等,Pediatr Res2000,47,221-224中报道的分光光度法,利用d-ROMs试剂盒(Diacron srl,Italy)测定TH。
结果以常规单位表示,(Ca rr单位:1Carr单位的值等于0.08mg/dL过氧化氢的浓度)。通过Witko-Sarsat V等,Kidney Int 1996,49,1304-1313中报道的方法,在微量培养板读数器上使用分光光度法测定AOPP。AOPP浓度以μmol/L氯胺T等价物表示。利用Kime等Clin Sci 1996,91,633-638描述的方法,部分改变,通过HPLC测定NTBI水平。如前面Grosso S等Brain Dev 2008,30,391-395中所述的,添加2,6-二叔丁基对甲酚(BHT)防止该过程中的氧化,然后测定BAL中的F2-IP。
在单一基质中测定F2-IP。添加乙醇以除去沉淀的蛋白,且添加醋酸盐缓冲液使pH达4.0,用于15-F2tIsoP测定。使用比色酶免疫测定(Cayman Chemical,Ann Arbor,MI)之前,从利用C18和硅Sep-Paks柱(Waters Co,Milford,MA)从代谢产物纯化样品。抗体是15-F2t-IsoP(8-Iso PGF2)高特异性的。标准曲线的范围为3.9到500pg/ml,而且检测极限为2pg/ml。
用下列技术获得来自动物的支气管吸出样品:通过置于气管内导管中的8F规格的输送导管,以便尖端延伸到管的末端外1cm,利用10mL注射器滴注1mL/kg无菌的0.9%盐水。滴注盐水,并立即吸回到注射器中。
所有的样品通过离心澄清(1000rpm x 5min),上清液于70℃立即冷冻,并储存用于随后的分析。表面活性物质施用之前和之后的1,2,4和6小时,从每个动物获得支气管的吸出样品。
用过量戊巴比妥杀死动物。死后,小心打开胸腔,取出气管和肺,并置于缓冲的10%福尔马林浴中24小时。从两个肺的上和下叶获得两个随机肺样本。利用常规技术制备组织用于石蜡包埋。5微米厚的切片用苏木精-曙红染色,并由两名病理学者进行盲试显微镜检验。
分析收集的数据,并以平均和标准偏差表示。通过这些组内的ANOVA测验,和通过这些组间的学生氏测验,分析数据的统计学显著性差异。p<0.05认为是统计上显著的。
结果证实了以测试的剂量,表面活性物质与催化酶的组合获得了协同相互作用。
Claims (12)
1.一种联合2000到10000U/kg单位剂量的CuZn过氧化物歧化酶和2000到4000U/kg单位剂量的催化酶的经修饰的天然肺表面活性物质,用于通过气管内给药或吸入给药来预防早产儿支气管肺发育异常,其中所述肺表面活性物质选自猪肺磷脂,卡尔法坦,勃法克坦或贝拉康坦。
2.根据权利要求1的经修饰的天然肺表面活性物质,其中所述的表面活性物质是猪肺磷脂。
3.根据权利要求2的经修饰的天然肺表面活性物质,其中猪肺磷脂为100到200mg/kg的单位剂量。
4.根据权利要求1到3中任一项的经修饰的天然肺表面活性物质,其中CuZn过氧化物歧化酶的单位剂量为2500到6000U/kg。
5.根据权利要求1-3中任一项的经修饰的天然肺表面活性物质,其中催化酶的单位剂量为2500到3500U/kg。
6.一种药物,用于通过气管内给药或吸入给药来预防早产儿支气管肺发育异常,其包括如下固定组合:
经修饰的天然肺表面活性物质,选自猪肺磷脂,卡尔法坦,勃法克坦或贝拉康坦;
2000到10000U/kg单位剂量的CuZn过氧化物歧化酶;和2000到4000U/kg单位剂量的催化酶。
7.根据权利要求6的药物,其中所述的表面活性物质是猪肺磷脂。
8.根据权利要求7的药物,其中猪肺磷脂的单位剂量为100到200mg/kg。
9.根据权利要求6-8任一项的药物,其为溶于缓冲的生理盐水-0.9%w/v氯化钠水溶液的无菌悬浮液的形式。
10.一种试剂盒,用于通过气管内给药或吸入给药来预防早产儿支气管肺发育异常,其包括:a)处于第一单位剂型中的经修饰的天然肺表面活性物质与药学上可接受的载体或稀释剂,其中所述肺表面活性物质选自猪肺磷脂,卡尔法坦,勃法克坦或贝拉康坦;
b)处于第二单位剂型中的2000到10000U/kg单位剂量的CuZn过氧化物歧化酶,和任选地药学上可接受的载体或稀释剂;
c)处于第三单位剂型中的2000到4000U/kg单位剂量的催化酶,和任选地药学上可接受的载体或稀释剂;
d)用于容纳所述第一,第二和第三剂型的容器。
11.根据权利要求10的试剂盒,其中所述的表面活性物质是猪肺磷脂。
12.根据权利要求11的试剂盒,其中猪肺磷脂的单位剂量为100到200mg/kg。
