背景技术
血管渗透性过高

发明概述
本发明涉及使用多激酶抑制剂诸如索拉非尼、贝伐单抗、舒尼替尼、瓦他拉尼和其它在治疗涉及血管高渗透性的各种病理状况的用途,特别涉及在治疗淋巴水肿、脑水肿、烧伤、视网膜水肿、脓毒病、心血管疾病(例如,心脏衰竭)、继发性门脉高压腹水中的用途。
这些用途在主要权利要求中描述。从属权利要求概述使用多激酶抑制的进一步有利的方式。
治疗涉及血管高渗透性的病理的多激酶抑制剂的用途代表这种没有已知的治疗、常见和虚弱渐进性状况的第一有效和良好耐受性的药理学治疗。

实施例
最近,申请人观察到在霍杰金淋巴瘤的患者中严重的双腿淋巴水肿的完全消退,所述患者在临床II期试验中接受800mg/die的多激酶抑制剂索拉非尼。在该患者中,淋巴水肿是由于与放射治疗后后遗症相关的压迫性淋巴阻塞。
作为这种观察的机理解释,本申请假设索拉非尼可能通过抑制VEGFRs已经抑制血管渗透性,最终出人意料地诱导了淋巴水肿的减小。事实上,VEGF、VEGFRs的配体的一个主要活性是增加血管渗透性,并且这种蛋白质也已知为血管渗透性因子(VPF)。
为了验证这种假设,申请人最初地评估了在淋巴水肿的小鼠尾巴模型中的多激酶抑制剂索拉非尼的作用,然后接下来使用Miles血管渗透性测定研究索拉非尼是否可能干涉血管渗透性。
试验1:淋巴水肿的小鼠尾巴模型
体重20至25g、6至8周龄的雌性C57BL/6小鼠从Charles River(米兰,意大利,欧洲)购买。小鼠在标准的实验室条件下根据申请人的机构引导进行收容。动物试验根据意大利法律(法令116/92和以后的附加)进行,其执行EU 86/109指引,并且被动物试验的研究所道德委员会批准。

试验2:小鼠血管渗透性测定
由于发现了索拉非尼处理的小鼠中显著减小的水肿形成,申请人接下来研究多激酶抑制剂索拉非尼释放可以降低血管渗透性过高。在具有外科手术诱导的尾巴淋巴水肿的未处理和索拉非尼处理小鼠中进行小鼠血管渗透性测定,所述测定使用蓝色染料伊文思蓝的脾内注射。

临床研究
为了在临床情况中进一步评估多激酶抑制剂索拉非尼的抗淋巴水肿活性,申请人在有同情心需要的基础上进行初步研究,以在具有出现在腋窝淋巴结上外科手术解剖和/或放射治疗之后获得的手臂淋巴水肿的乳腺癌患者中,评估多激酶抑制剂索拉非尼的毒性和抗淋巴水肿作用。对于这些患者没有被证明有效的其它处理选择是可用的。
患者以200mg每天的剂量口服索拉非尼。
治疗给予最多8周,或者临床显著毒性的进展或出现可能与多激酶抑制剂索拉非尼相关。
在两个月的时间段内,具有获得的手臂淋巴水肿的10个乳腺癌患者用多激酶抑制剂索拉非尼进行处理。
淋巴水肿发作的中位时间是24月(范围为6至48)。总的,索拉非尼是良好耐受性并且所有患者接受没有剂量减少的计划的处理或者处理中止,由于任何类型毒性的任何级别的出现。
