Abstract
本发明公开了一种辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂。本发明提供的试剂由序列表的序列1至序列表的序列96所示的96条DNA组成。本发明提供的辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂包括用于检测5个染色体区段的96种DNA探针。应用96种DNA探针的杂交信号,可以得到5个染色体片段的信号处理数据,根据5个染色体区段的信号处理数据,按照一定判断标准,可以辅助诊断非浸润性膀胱癌和浸润性膀胱癌,具有较高的准确率,且相对临床病理诊断法,操作更为简单,具有较强的实用性和临床应用价值。

技术领域
本发明涉及一种辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂。
背景技术
膀胱癌是严重威胁人类健康的一种恶性肿瘤,在美国和欧洲地区,膀胱癌在男性恶性肿瘤中所占比例分别是10%和5%,男性的发病率为女性的3~4倍,平均发病年龄为65~70岁。在中国,膀胱癌的发病率同样位居男性恶性肿瘤的前列。根据组织病理学及临床表现,膀胱尿路上皮癌大致分为两类:非浸润性膀胱尿路上皮癌和浸润性膀胱尿路上皮癌。超过70%的膀胱尿路上皮癌病变局限于上皮及粘膜固有层,包括原位癌和非浸润性肿瘤;这类病人术后经常出现复发,其中5~30%会进展为肌肉浸润性病变。剩余不足30%的病人属于浸润性膀胱尿路上皮癌,此类病人预后不良且病情进展迅速,5年生存率小于50%。
目前,对于这两种类型膀胱癌所采用的治疗方案不同,对非浸润性膀胱癌患者多采取经膀胱镜下电切肿瘤,而浸润性膀胱癌患者则采取膀胱切除术治疗。因此,术前准确区分膀胱癌为非浸润性或浸润性对于制定合理的手术术式至关重要。在实际工作中,临床医生主要根据膀胱镜下所见肿瘤大小、有蒂或无蒂来粗略判断肿瘤类型。但是,当肿瘤较大或肿瘤蒂不明显时,区分非浸润性和浸润性癌就十分困难,只能依靠术后的组织病理学方法进行诊断。因此,是否能找出有效的分子标志物来辅助术前鉴别诊断浸润性和非浸润性膀胱癌,是一个有待回答的重要临床问题。
基因芯片技术广泛应用于药物研发、临床筛查等多个领域,包括肿瘤的分子分型,具有快速、灵敏、高通量等优点。微阵列比较基因组杂交技术(array Comparative GenomicHybri dization,array CGH)是基因芯片的一种,能够快速、全面地筛查肿瘤基因组结构功能的变化.如果能够通过基因芯片技术从非浸润性和浸润性膀胱癌的差异DNA片段/区域中筛选出一组具有鉴别意义的分子标志,对于辅助膀胱癌的正确诊断意义重大。
发明内容
本发明的目的是提供一种辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂。
本发明提供的辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂,由序列表的序列1至序列表的序列96所示的96条DNA组成。
本发明还保护含序列表的序列1至序列表的序列96所示的96条DNA的DNA芯片。
所述试剂可用于制备辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂盒。

具体实施方式
以下的实施例便于更好地理解本发明,但并不限定本发明。下述实施例中的实验方法,如无特殊说明,均为常规方法。下述实施例中所用的试验材料,如无特殊说明,均为自常规生化试剂商店购买得到的。以下实施例中的定量试验,均设置三次重复实验,结果取平均值。
实施例1、辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂的制备
辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂由96条DNA探针组成;96条DNA探针的核苷酸序列如表1所示,针对不同的染色体片段。表1中,序列1至序列5为针对1p36.13(chr1:16218_16270)的探针,序列6至序列10为针对1q32.1(chr1:201426_201543)的探针,序列11和序列12为针对5q11.2(chr5:54469_54489)的探针,序列13至序列91为针对9q21.31-q21.33(chr9:82707_89606)的探针;序列92至序列96为针对9q33.2(chr9:122723_122836)的探针。表1中的序列1至序列96与序列表中的序列1至序列96一一对应。

实施例2、实施例1的试剂在辅助诊断非浸润性膀胱癌和浸润性膀胱癌中的应用
在知情同意的前提下,用实施例1的试剂分别对65例膀胱癌患者进行检测。
一、采用金标准进行诊断
65例膀胱癌患者先采用临床病理(金标准)方法进行诊断。诊断结果为:非浸润性膀胱癌患者29例,浸润性膀胱癌患者36例。各个病例的诊断结果见表8。
二、采用本发明的试剂进行诊断
1、提取膀胱癌组织的基因组DNA。
2、将提取的基因组DNA进行array CGH芯片杂交
采用的是Agilent公司的芯片技术平台,所用44k array CGH属于高密度芯片,每个芯片含用43000个探针,能够覆盖全基因组的超过30000个基因,分辨率为35kb;该芯片中含有表1中所列的96条探针。
3、芯片经过Agi lent芯片扫描仪,以5um分辨率扫描,用Feature Extraction 9.1从芯片扫描图片中提取各探针的信号值。
4、利用R project(Team,R.D.C.R:A language and environment for statisticalcomputing.R Foundation for Statistical Computing,Vienna,Austria.ISBN3-900051-07-0,URL http://www.R-project.org(2009).;Fridlyand,J.& Dimitrov,P.aCGH:Classes and functions for Array Comparative Genomic Hybridization data.Rpackage version 1.18.1.(2009).)对探针信号值进行分析,进行均一化(normalization)和片段化(segmentation)的程序处理。
每个患者96个探针信号值的均一化后数据见表2至表6。

首先根据chr9:82707_89606片段数据进行评判,chr9:82707_89606片段数据小于等于-0.514的患者为候选的非浸润性膀胱癌患者,chr9:82707_89606片段数据大于-0.514的患者对chr1:16218_16270片段数据进行评判;
chr1:16218_16270片段数据小于等于0.055的患者对chr1:201426_201543片段数据进行评判,chr1:16218_16270片段数据大于0.055的患者对chr9:122723_122836片段数据进行评判;
chr1:201426_201543数据小于等于0.056的患者为候选的浸润性膀胱癌患者,chr1:201426_201543数据大于0.056的患者为候选的非浸润性膀胱癌患者;
chr9:122723_122836数据小于等于-0.111的患者为候选的浸润性膀胱癌患者,chr9:122723_122836数据大于-0.111的患者对chr5:54469_54489片段数据进行评判;
chr5:54469_54489数据小于等于-0.287的患者为候选的浸润性膀胱癌患者,chr5:54469_54489数据大于-0.287的患者为候选的非浸润性膀胱癌患者。
以第1号病例为例,按上述方法进行评判。首先,chr9:82707_89606片段的值为-0.079,-0.079>-0.514,因此再观察第二个指标;chr1:16218_16270片段的值为0.019,小于0.055,因此再观察第三个指标;chr1:201426_201543片段的值为0.056,因此判断该病例类型为“2”型,即浸润性膀胱癌。根据上述判断标准,65例患者的判断结果见表8。

Claims (4)
1.辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂,由序列表的序列1至序列表的序列96所示的96条DNA组成。
2.含序列表的序列1至序列表的序列96所示的96条DNA的用于辅助诊断非浸润性膀胱癌和浸润性膀胱癌的DNA芯片。
3.权利要求1所述试剂在制备辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂盒中的应用。
4.一种辅助诊断非浸润性膀胱癌和浸润性膀胱癌的试剂盒,包括权利要求1所述的试剂。
