Abstract

本发明涉及用于眼外科手术中的晶状体乳化针,并且更具体地涉及在外科手术期间包围晶状体乳化针的套筒。
背景技术
将白内障从患者眼睛中移走并用人工的眼内晶状体替换移走的受白内障影响的晶状体是公知的。常见和可接受的实践是借助于晶状体乳化法来移走受白内障影响的晶状体。
晶状体乳化法使得晶状体能通过非常小的切口(通常在3mm的量级)从眼睛中移走。晶状体乳化法涉及使用通过机头传输入晶状体乳化针以使受影响的晶状体成为碎片的高频超声能量。一旦晶状体成为碎片或被乳化,晶状体材料就连同冲洗流体一起通过晶状体乳化针的内腔吸出、并且穿过机头和进入外科系统的收集容器。
在晶状体材料的吸出过程中,通常同时将冲洗流体流插入眼睛。这个流体流提供来防止眼睛在吸出过程中破裂。通常晶状体乳化针还提供了通常称作的同轴冲洗。这个同轴冲洗通过包围针的弹性套筒将冲洗流体流提供入眼睛。冲洗套筒通常包括位于套筒远端处的冲洗端口以使得流体从机头流入套筒和针外侧之间。虽然晶状体乳化法和同轴冲洗已经证明非常成功和安全,但是仍然强烈希望从目前平均3mm的切口进一步降低切口尺寸。一种用来实现这个目的的过程是已知为双手式白内障抽取的技术。在双手式抽取中,通常有至少两个长度在1.4mm量级的非常小的切口。为了实现这么小的尺寸,具有分离冲洗套管的冲洗部分通过一个切口插入并且仅具有吸出作用并且通常没有套筒的晶状体针通过另一个切口插入。
在双手式外科手术中,没有外套筒在吸出针周围的绝缘和密封作用,还涉及到角膜组织的过度加热,这是因为针的振动以及切口位置处针周围的流体泄漏。
因此,有利地和期望地是具有一种晶状体乳化针,其给现有技术中具有套筒的同轴晶状体针提供绝缘和密封性质,而不需要较大的切口尺寸来容纳该套筒。

具体实施方式
图1示出了根据本发明的晶状体乳化针套筒10。套筒10附接至具有远端14和近端16的晶状体乳化针12。近端16通常附接至常规的晶状体乳化机头,该机头可包括或不包括用于在晶状体乳化针套筒10和针12之间传送冲洗流体的端口。
套筒10包括具有远端20和近端22的弹性的基本上管状的细长本体部18,用于包围晶状体乳化针12的轴的一部分。近端22包括形成于本体部18的近端上的扩大段,如所示,用于包围针12的彀(在下面图2中示出)。远端20形成为紧靠针12的扩口端14的近端24。这样,本体部18的外径基本上等于扩口端14的外径,如同从图1能看出的。这允许了包围针12的套筒10在外科手术期间容易地穿过患者眼睛中长度小于2mm的切口插入。

图2示出了沿着图1中2-2截取的剖视图。如同能看到的,近端22的扩大段包围针12的晶状体针彀26。在外科手术期间,乳化的白内障组织和冲洗流体通过内腔28吸入晶状体机头(未示出),并且最终进入眼外科手术系统的泵送系统的收集容器,比如Bausch&Lomb公司市售的。冲洗流体可被引入来包围针12并且由套筒18容纳并且将最初在箭头30的方向上流动。与没有这种流体被引入针12和套筒10之间相比,这种冲洗流体的引入为套筒10提供了更多的绝缘和密封作用。螺纹32通常啮合公知的晶状体乳化机头。
图3示出了包围针34的套筒10,以形成根据本发明的一个可选实施例。针34和套筒10基本上与以上针对图1和2所述的相同,除了如图4所示,针34包括至少一个孔36并且优选地两个,如所示。孔36允许冲洗流体流入针34的内吸出内腔38。冲洗流体从晶状体乳化机头以公知的标准方式在箭头40方向上流动,并且持续地流入吸出内腔38,其中冲洗流体在箭头42的方向上从外科手术位置吸走并进入未示出的收集容器。具有这种孔36的针34允许冲洗流体在针34周围持续流动并且因而与图1和2的针相比提供了更大的冷却和绝缘作用。
图5是图1的套筒针组合的局部透视图,并且示出了包围针12的套筒10。重要的是管状部分18的外径基本上等于针12的扩口端14的外径,以使得套筒和针的组合在外科手术期间易于插入眼睛。如果管状部分18的套筒外径比扩口端14的外径大的太多,就存在着套筒10不能按照希望进入眼睛并仍然包围针12的危险和可能性;而是将在角膜切口的外侧周围聚成一团。
还重要的是,套筒18没有任何冲洗端口,如所示。冲洗端口通常出现在用于具有同轴冲洗的晶状体乳化针的套筒,并且这种端口通常出现在套筒的远端,以便将冲洗流体引入眼睛。然而,在双手式操作期间,如图6所示,冲洗流体通常通过针(比如所示的针44)引入眼睛以防止眼睛塌陷。套筒10和针12通常连接至晶状体乳化机头46,该晶状体乳化机头46可具有或不具有冲洗源48并通过角膜50插入囊包52,如所示。角膜50中的切口54通常将套筒10压在针12周围,如线56处所示。线56夸大地弯曲以示出套筒10上切口54的效果。这样,套筒10提供了绝缘和密封性质以防止流体在外科手术期间在切口位置周围泄漏。

Claims (5)
1.一种晶状体乳化针套筒,包括:
具有远端和近端的弹性的基本上管状的细长本体部,用于包围晶状体乳化针的轴的一部分;
形成于本体部近端上的扩大段,用于包围针的彀;并且
其中远端形成为紧靠针的扩口端的近端,以使得本体部的外径基本上等于扩口端的外径,以使得套筒易于穿过患者眼睛中的切口插入。
2.根据权利要求1的套筒,其中扩大段形成为从冲洗流体源接收流体流,并且其中套筒没有任何冲洗端口。
3.根据权利要求4的套筒,其中套筒由硅酮构成。
4.根据权利要求2的套筒,其中针包括至少一个孔,用于允许冲洗流体流入针的内吸出内腔。
5.根据权利要求1的套筒,其包围晶状体乳化针并且其尺寸被形成为插入长度小于2mm的切口。
