Abstract

Description
技术领域
本发明涉及医疗手术中用的锚和缝线,特别涉及适用于将腱、肌肉或组织粘附于病人骨头上的组合的缝线锁和锚。
发明背景
如医疗技术的专业人员熟知的,医疗手术中完成的缝线打结绑扎如将组织绑扎在骨头上确实是外科医生进行的手术中最薄弱的环节之一。手术采用关节内窥镜检查,或者部分或全部打开待修复的区域,就属于这种情况。众所周知,该技术中外科医生在缝线上进行的关节内窥镜检查的打结没有两个是相同的,同样知道的是,一个熟知的问题是,外科医生不可能打结到以最佳方式将组织固定在骨头上。换句话说,绑扎的组织不可能完全紧靠或粘附在骨头上。
虽然本发明考虑的是处理肌腱套撕裂,目的是获得一种解决上述打结问题的缝线,但显然,本发明的器械也可用于其它外科手术。肌腱套撕裂十分普遍,由于将组织绑扎到骨头上很困难,这对外科医生成为越来越严重的问题。肌腱套是一组通过其腱而附着在上臂骨(肱骨)顶部上的肌肉。这些组织中的撕裂常需要医生修复。取决于撕裂的严重性,留给外科医生的选择,或者是结合关节内窥镜检查和小切口的小型切开修复的关节内窥镜检查修复,或者是切开外科手术。不管是那种手术,组织最终都绑扎在骨头上,常常利用一个固定在骨头中的锚和一根绑扎在组织和锚上而后打结的缝线。本发明指向一种独特的装置,该装置将组织固定在骨头上并合并地调整,使得该组织绑扎在骨头上而尽可能密切地绑紧在骨头上。因此,本发明涉及一种用于将组织绑扎到骨头上的器械,一种自动固定缝线的器械,一种当使用本发明的装置时外科医生能操纵该缝线而使组织尽可能密切地绑紧在骨头上的方法。
这种修复肩膀的器械和方法完全不同于Opus医生制成的自动套系统,该系统利用灵巧缝合法缝线装置和大n头状骨无结固定植入物。该缝线装置利用一种机械褥垫缝合法来消除打结绑扎。该装置将缝合直接安置在组织中。一旦缝合置入套中,外科医生就加载和展开大n头状骨无结植入物。该手术可与本发明区分清楚。
发明概要
本发明的一个目的是,提供一种医疗手术用的绑扎和锁定缝线端部的改进的装置。
本发明的一个特点是提供一种缝线锁定装置,该装置包括一个插入一空心圆筒形主体内而沿轴向向该主体中的一个壁移动的销,并提供在空心圆筒形主体中的孔,缝线的两端穿过孔内,而该销向上移动而靠着该壁被推动,从而在该销和该壁之间挤压缝线的线长。轴向的槽形成于该主体的侧壁中而用于当销在该主体中沿轴向移动时引导该销。一个凹部形成于该主体中并限定一壁,该壁的轮廓与该销互补。该凹部的顶面做成凸起的表面或锯齿形,而该销为多边形,其中包括平行于凹部侧壁而取向的平表面。该圆筒形主体的外径可以有对置的平面,其中设置这些孔,而这些孔允许缝线穿过该主体,从而使缝线安置在销和壁之间。

发明详述
如上所述,虽然本发明被认为用于在处理病人的肌腱套的医疗手术中的绑扎缝线,但如该技术的专业人员将理解的,本发明也可以用于其它医疗手术,包括(但不限于)不仅用于将组织绑扎在骨头上,而且将组织绑扎在组织上等等,只要该用于将锚插入人体部分的工具具有驱动该缝线锁定装置的能力或使用一个没有该锚的类似工具。
如图1和图2中最清楚地看到的,本发明的缝线锁定装置10固定在一个熟知的锚12上,锚12包括一个脆性的轴14(修改成适应本发明),其远端是一个尖端16(图9)和可弯曲部分12a,后者包括环状底环部分18、可弯曲的臂或翼26和一个顶环部分20,顶环20与缝线锁定装置10接合或与其制成一体。当展开时,锚12的翼26如图所示地弯曲而操作,极像一个帆鳍栓并被强制卡入骨头结构中而将锚固定就位。在此前的锚中,缝线24将穿过组织30并通过锚中形成的孔(未示出),而缝线24的两端由外科医生以熟知方式打结。显然,如上所述,因为由医生打结,所以医生很难又绑缝线又同时迫使组织附着在骨头表面附近。如下面将更详细地描述的,本发明的缝线锁定装置10基本上将缝线结扎和锁定就位,从而解决了这个问题。

如图5B和5C中所示,当锚展开时,轴升高到越过主体28的顶边缘66而暴露环形槽58。在操作的该接合处,在轴的较大直径处形成的外缘59靠着主体28的内表面上形成的台肩61而支承。通过弯曲该轴14,轴14将在该位置(环形槽)处断裂而从锚和主体28上折断,然后被丢弃。由于本发明,通过本发明的创造性的缝线锁和在锁定缝线的过程中外科医生相对于骨头拉紧缝线而使该组织的位置的尽可能靠近骨头这一事实,而将缝线锁定就位。因为由于本发明而消除了至今为止的手工打结,所以医生能够拉成缝线而使该组织紧贴骨头(绑紧),此时医生会满意这是不会发生后退的最佳位置。换句话说,本发明消除了用手绑扎缝线时通常发生的缝线下垂。

所述销(42)穿过一个在所述轴向脆性轴(14)中形成的孔,而所述销(42)的两端伸出所述轴向脆性轴(14),而接合所述沿直径方向对置的槽(40);
当所述锚(12)展开时,所述尖端部分(16)和所述底部(18)向着所述顶端部分(33)前进,而所述轴(14)在所述顶端部分(33)上方前进,从而暴露一个在所述轴向脆性轴(14)中形成的环形槽(58),由此,在所述顶端部分以上的所述轴部分从所述顶端部分以下的所述轴部分上被折断。

10.一种用于固定和锁定将组织绑扎到病人骨头上的缝线(24)的端部的组合的锚和缝线锁定装置(10),
所述缝线锁定装置(10)被固定在一个锚(12)上,该锚(12)为了医疗的目的而被固定在所述骨头上,并包括一个有顶壁部分(33)、底壁部分(32)和侧部(34、36)的圆筒形空心主体(28),侧壁中有沿直径方向对置的槽(40)和一对沿直径方向相反配置的孔(44,46),孔(44,46)与所述沿直径方向对置的槽沿周边隔开;
一个多边形销(42),被配置在所述主体(28)中,并具有伸入所述沿直径对置的槽(40)中的端部部分;
形成于所述主体的壁中的所述一对沿直径方向相反设置的孔的位置相对于所述沿直径方向对置的槽(40)成90度;
一个形成于所述主体中的凹部(60),有一个与所述销(42)沿轴向对准的锯齿形部分(61),一根预定要被锁定的缝线具有用于将组织绑扎在病人骨头上的缝线部分;
其中所述缝线(24)的一部分从所述组织的一侧伸出,而所述缝线的另一部分从所述组织的另一侧伸出,这两部分穿过所述沿直径方向对置的孔(44,46)而处在所述销(42)上;
在展开所述锚(12)时,所述销(42)被移入所述凹部(60)中,而迫使所述缝线绑扎在所述凹部(60)的所述锯齿形部分(61)和所述销(42)之间;
所述锚(12)有一环形底部(16)、一环形顶部(18)和安装于其间的向外弯曲的翼(26);

当所述锚(12)展开时,所述尖端部分(16)和所述底部向所述顶部部分(33)前进,而所述轴(14)在所述顶端部分(33)上方前进,从而暴露一个在所述轴向脆性轴(14)中形成的环形槽(58),由此,在所述顶端部分以上的所述轴部分可从所述顶端部分以下的所述轴部分被折断。

13.一种用于固定和锁定缝线端部的缝线锁定装置,
所述缝线锁定装置有一个圆筒形的主体(28),该主体(28)有一限定一空腔(31)的壁、一顶端部分和一侧面部分;
一个六边形销(42)沿侧向被设置在所述空腔(31)中,所述壁限定一个形成于所述主体(28)中的锯齿形凹部壁(61),后者靠近顶端部分(33)并与所述六边形销(42)沿轴向对准;
在所述侧壁部分中的沿直径方向相反设置的槽(40),用于引导所述六边形销(42);
在所述缝线锁定装置(10)的所述侧壁部分(34)中形成的孔(44,4 6)沿周面与所述沿直径方向对置的槽(40)隔开而用于接受预定待绑扎的所述缝线(24)的线长,从而使预定待绑扎的所述缝线在所述空腔(31)中沿侧面延伸而处在所述销(42)和所述锯齿形凹部壁(61)之间;
在展开所述缝线锁定装置(10)时,所述空腔(31)中的所述缝线线长被促使靠着所述锯齿形凹部壁(61)和所述六边形销(42),由此,由于所述销(42)靠着所述锯齿形凹部壁(61)而施加在所述缝线线长上的挤压力将所述缝线锁定就位。
14.一种如权利要求13中所述的缝线锁定装置,其特征在于,该六边形销(42)的平面部分被设置在所述空腔(31)的与所述锯齿形凹部壁(61)垂直地延伸的壁部分的附近。
15.一种用于固定和锁定将组织绑扎于病人骨头上的缝线的端部的组合的锚和缝线锁定装置,
所述缝线锁定装置被固定在一个为医疗目的固定于所述骨头中的锚(12)上,该缝线锁定装置(10)有一圆筒形空心主体(28),该主体(28)具有顶壁部分(33)、底壁部分(32)和侧壁部分(34、36),侧壁部分(34、36)带有沿直径方向对置的槽(40)和与槽(40)沿周面隔开的沿直径方向相反设置的孔(44,46);
一个多边形销(42)被配置在所述主体(28)中,并具有在所述沿直径方向相反设置的槽(40)中的端部部分;
一个在所述主体中形成的凹部,具有与所述锁沿轴向对准的锯齿形壁部分;
一根具有将组织绑扎到病人骨头上用的缝线部分的预定待锁定的缝线(24);
其中,所述缝线的一部分从所述组织的一侧伸出,而所述缝线的另一部分从所述组织的另一侧伸出,这两部分穿过所述沿直径方向对置的孔(44,46)而处在所述销(42)的上面;
在展开锚(12)时,所述多边形销(42)被移入所述凹部中,从而迫使所述缝线(24)绑扎在所述凹部(60)的所述锯齿形壁部分(61)和所述销(42)之间;
所述锚(12)有一环形底部(16)、一环形顶部(18)和安装在其间的向外弯曲的翼(26);

17.一种如权利要求15中所述的用于固定和锁定将组织绑扎在病人骨头上的缝线端部的组合的锚和缝线锁定装置,其特征在于,所述圆筒形壁的靠近所述沿直径方向相反设置的孔的侧面部分(34)是平的。
18.在医疗手术中使用的用于修复病人肌腱套中组织的绑扎缝线的方法,其特征在于,一种锚和缝线锁的组合,将缝线绑扎和锁定就位,包括以下步骤:
i)在进行医疗手术的部位附近切开病人的皮肤;
ii)在病人的肌腱套中对骨头刻痕;
iii)提供一种组合的锚和缝线锁;
iv)将缝线穿过该缝线锁和组织;
v)同时拉紧该缝线并将该锚固定在骨头中,由此,将该锚置于骨头中的动作使该缝线锁定在该缝线锁中。
19.如权利要求18中所述的在医疗手术中使用的用于修复病人肌腱套中组织的绑扎缝线的方法,其特征在于,一种锚和缝线锁的组合,将缝线绑扎和锁定就位,包括以下步骤:
v)在步骤iv)中的同时拉紧和安置的步骤中提供一种用于将该锚插入骨头中的工具,并驱动该工具。
20.在医疗手术中使用的用于修复病人的肌腱套中组织的绑扎缝线的方法,其特征在于,一个锚被固定在骨头中,一种缝线锁组合将该缝线绑扎和锁定就位,从而将该组织紧密地捆紧在骨头上,包括以下步骤:
i)在进行医疗手术的部位附近切开病人的皮肤;
ii)在病人肌腱套中对骨头刻痕;
iii)提供一种组合的锚和缝线锁;
iv)将缝线穿过病人的组织,将缝线一段长度穿过该缝线锁,使其搁在该缝线锁的销钉和锯齿形壁之间,然后使缝线另一段长度穿过该缝线锁,使其也搁在该缝线锁的销钉和锯齿形壁之间;
v)同时拉紧该缝线并将该锚安置在骨头中,由此,将该锚置于骨头中的动作使该缝线锁定在该缝线锁中。
21.如权利要求20中所述的在医疗手术中使用的用于修复病人的肌腱套中组织的绑扎缝线的方法,其特征在于,一种锚和缝线锁的组合,将该缝线绑扎和锁定就位,包括以下步骤:
v)在步骤iv)中的同时拉紧和安置的步骤中提供一种用于将该锚插入骨头中的工具,并驱动该工具。
