发明背景

血小板是血液凝固所必需的血液组分。血小板是亚细胞血液组分,它没有细胞核,但有细胞膜、受体、酶、颗粒和其它细胞成分,因此血液中血小板能够对若干因子起反应而引起血凝块的形成。当患者在战场上发生大量外伤性失血、接触化学品或受到高剂量辐射和发生各种其它医学状况,如血小板减少时,尤其是为治疗白血病患者而清除骨髓后,需要进行血小板输入。血小板的储存寿命短(FDA和AABB规定一般仅5天)导致战场上和民用卫生系统血小板短缺。
在目前出于医学目的储存的所有血液细胞组分中,血小板最脆弱。目前没有临床上可应用的方法长期储存血小板。对于现代卫生机构而言,血小板储存期为5天,留出测试和运输时间后,转换成临床储存期为3-4天。许多血库在保持血小板新鲜和储存方面有后勤困难。向军用战地医院可靠地提供血小板的困难甚至更大。

仍然明显缺乏对血小板形成过程和血小板从巨核细胞出芽过程的理解。接受的论点是,诸因子的联合作用,包括血浆和内皮结合的膜因子、巨核细胞细胞骨架或细胞器重排和血流造成的剪切力,综合导致成熟血小板从巨核细胞上形成的前体血小板结构上分离下来。然而,此观点未得到充分证明,血小板形成和从巨核细胞分离下来仍然是亟待揭示的研究领域。
发明概述
本发明小结了产生人血小板的方法,其包括以下步骤:在有利于人胚胎干细胞分化为造血谱系细胞的条件下培养人胚胎干细胞;培养造血谱系细胞成为巨核细胞;培养巨核细胞产生血小板;和回收血小板。
本发明也小结了以治疗有效量体外产生所需量的人血小板。
本发明特征是体外产生的血小板不会结合人血流中所遭遇的因子。

发明详述
本文考虑的是通过体外培养和从人胚胎干细胞开始分化的方法产生血小板。体外培养诱导人胚胎干细胞(hES细胞)产生巨核细胞,再培养这些巨核细胞产生有生物学功能的人血小板。认为该方法可通过三个主要步骤完成。第一个主要步骤是使人ES细胞定向分化为造血细胞,进而,可以几种方式完成分化过程。本文详细描述了使hES细胞分化为造血谱系细胞的两种方法。在造血分化方法的一种技术中,采用目前已知的技术培养人胚胎干细胞(ES细胞)以形成胚状体(embryoid body,EB)。培养该胚状体,使其开始分化为各种分化细胞类型,然后使胚状体解聚;用巨核细胞前体选择性培养基配成细胞悬液。借助时程cDNA微阵列分析的帮助,我们确定了最优时机来收获具有最高造血潜能的明确的造血细胞。已经证明足以产生造血细胞的其它技术要求人ES细胞接触基质细胞,这种接触能引起ES细胞优先分化为造血谱系的细胞。这些方法任一的结果是将细胞,主要是ES细胞衍生的造血细胞培养至某种纯度。我们特别优选胚状体方法,不仅因为它没有各种类型的饲细胞的污染,而且可用成分明确的无血清培养基培养。在该方法的第二主要部分中,然后使这些造血细胞接触含有生长因子的巨核细胞形成选择培养基,所述生长因子能特异性促进巨核细胞形成和成熟。最后,使这些成熟的巨核细胞接触血小板形成培养基,以促进体外血小板生产。在所有这些过程中,可避免采用动物或人的血清和血浆。
血小板是从hES细胞产生人用生物产品的示范性靶点,因为血小板不携带染色体遗传物质。可认为血小板是母体巨核细胞的胞质片段。重要的是,血小板具有可导致粘附、凝集和颗粒分泌的细胞表面因子。由于血小板成熟过程和血小板从巨核细胞上脱落的过程都是了解甚少的过程,还不知道是否可从人ES细胞衍生的体外细胞培养物回收到有生物功能的血小板。本文揭示,可从这些细胞培养物回收有用量的血小板。
重要的是,本文也证明了经体外细胞培养的人巨核细胞能形成血小板并使其脱落。由于对此过程有关的详细生物学机制的知识不确定,目前还不了解此过程会否或可否在培养物中发生。本文结果证明该过程可发生并确实发生。
该过程从hES细胞开始,hES细胞定义为培养中的未分化细胞。目前证明,可诱导hES细胞分化为主要是造血谱系细胞的细胞培养物。迄今为止的文献中,已知有两种不同技术能实现这种定向分化,也考虑了可能有效的其它技术。一种已知技术需要产生胚状体,该胚状体是具有三维结构的hES细胞的聚集体,该结构似乎有利于干细胞分化为定型的后代谱系。可用选择性方案分离这些能产生各种谱系分化细胞的胚状体中的某谱系细胞,如造血谱系细胞。我们进行的造血过程的详细时程分析为我们提供了这些造血前体细胞的遗传分布状况,同时,我们优化了我们的方案而得到最高产率。其它已知技术包括hES细胞与人或非人基质细胞共同培养。与基质细胞的共同培养似乎也能诱导hES细胞主要产生造血细胞,但可能要对现有技术依据的某些培养条件寻求避免方法。
发现能提高各种造血谱系细胞产率的EB法中的中间步骤是使EB片段化。EB的特征之一是EB可生长得很大,以致于超过培养基通过扩散向中心细胞提供氧气和营养物的能力。结果可能是EB中心产生坏死区域,这也引起EB细胞生长阻滞。现在发现,通过物理方法将EB切块使EB片段化,可以重启EB的生长,产生更多的分化细胞。我们了解,采用这种技术导致整个方法回收的血细胞数量显著增加。可用各种机械装置和系统进行EB的片段化或切块加工。
一旦造血谱系细胞产生后,培养细胞,以优先产生巨核细胞。优先产生巨核细胞的培养条件将提高培养物中巨核细胞相对于其它血液产品前体细胞的比例,但此方法不是绝对需要的。有利于产生未成熟和成熟的巨核细胞的条件包括用血小板生成素(TPO)、白介素3(IL3)、白介素6(IL6)和干细胞因子培养前体细胞培养物。如果存在bFGF(碱性成纤维细胞生长因子),则可进一步扩增未成熟的巨核细胞。用此方法获得的巨核细胞是CD41、CD42a、CD42b、CD61、CD62P、CD38、弱CD45阳性,但是HLA-DR、CD34、CD117阴性的细胞。正常的成熟巨核细胞这种免疫表型分布恒定。没有显著的CD45+细胞群说明,白细胞污染非常少(如果存在)。
然后通过培养使巨核细胞形成和释放血小板。虽然不完全了解引起血小板体内释放的确切机制,但可通过细胞培养使母体巨核细胞释放血小板。有四种因素可能非常重要,即剪切力、巨核细胞-内皮细胞相互作用、血浆因子、最后是巨核细胞中的分子机制。可通过物理操作培养容器例如振荡、旋转或类似方法激发对血液的剪切力。血浆蛋白和血小板受体激活的释放作用可能是通过巨核细胞本身的激活,或者按需单独添加其它因子。在某些先天性血小板异常如伯-索病和冯维乐布兰德因子(VWF)病曾报导观察到大的血小板。我们在一些胚状体衍生的血小板中观察到一些相似的大血小板。如果观察到这种现象,则是因为缺少血浆因子如VWF引起前体血小板无效地收聚血小板,可通过仅加入VWF或加入含有VWF的血浆解决这一问题。假定cGMP可促进从新生巨核细胞形成血小板。cGMP可用一氧化氮激活。我们发现,加入一氧化氮释放化合物GNSO可在2小时内使巨核细胞快速片段化为小血小板样颗粒。最后,此过程的副产物是相对纯的内皮细胞。我们通过试验测定,内皮细胞是否能帮助血小板脱落。通过所有这些技术,我们显著提高血小板形成的效率和规律性。为了理解血小板的生物学机制,我们建立了3D实时荧光显微术来记录存在或不存在血浆时血小板的释放。我们能够记录前体血小板的3D图像,目前我们正在进行时间推移研究,以更好地监测此过程。
然后,收集和包装血小板。在第12天巨核细胞分化末期,将非粘附性巨核细胞转移到装有3μM孔径滤器的多孔板的上部孔中。在培养箱中孵育,温和振荡并加入GNSO。在下室中收集血小板(如果需要存在人血浆或生理浓度的VWF和纤维蛋白原)。依次离心纯化从这种体外系统分离得到的血小板,重悬于柠檬酸盐缓冲液,用作供体血小板。进一步低速离心(3000g 30分钟)收集的血小板,以分离其它碎片,然后通过合适孔径的滤器过滤,以去除任何有核细胞。如此产生的含有血小板的产品特征是可将血小板浓缩至任何所需浓度。可进一步纯化体外产生的血小板,成为无血清或无血浆的产品,以适合具体的临床需要。可以并应该对所有容器进行灭菌以降低细菌污染,这是常规来源供体血小板的共同问题。
如此从体外细胞培养产生的血小板不同于目前科学研究或医疗所用的血小板,因为这些血小板不曾接触过血流。在生物体内产生的血小板不能完全与血浆分离。结果是,目前医用的包装血小板常常携带少量白细胞和血浆污染物,这些污染物可在一些患者中引起输血反应。此体外系统从人ES细胞分化产生的血小板将不含白细胞,并且从未接触过血清或血浆。如果培养或分离过程中加入过纤维蛋白原或VWF,此体外系统产生的血小板仅携带纤维蛋白原或VWF。
相关的问题是,一些免疫球蛋白自发性地粘附于血小板。因此,从献血员分离的血小板不可避免地携带该献血员的免疫球蛋白分子,这是产生副反应的另一个可能的原因。在体外从ES细胞产生的血小板不曾接触过IgG,因此不含它们。″ABO″血型抗原也出现在血小板上,虽然很弱。仍不了解血小板输注时偶然发生的ABO血型反应是源自血小板或是源自血清污染物。血小板中不存在Rh因子。因此,此法产生的血小板更适合医学和科学应用,也不难与常规分离技术产生的血小板相区分。
实施例
胚状体的造血前体细胞
胚状体(EB)形成是用于研究小鼠和人ES细胞的造血分化的一种方法。然而,不像小鼠ES细胞那样,人ES细胞的单细胞悬液不能有效形成胚状体。但是,为了由人ES细胞形成胚状体,用0.5mg/ml分散酶消化小鼠胚胎成纤维细胞(MEF)上培养的人ES细胞的完整集落5分钟,形成小细胞集落。然后在含血清的干细胞培养基(20%FCS)中使这些细胞集落进一步凝集。容易区分的细胞团块与不参与形成细胞团块而凋亡的单个细胞一起培养6天后,开始形成胚状体。培养12天后,胚状体组装成早期卵黄囊胚胎结构。制作该胚状体的切片随后用CD34抗体免疫标记该切片显示有卵黄囊的组织学特征。发现非粘附性造血前体细胞存在于小血管和内皮衬里的内腔中,显示这些细胞为CD34阳性细胞。然后,用37℃胰蛋白酶消化(0.05%胰蛋白酶/0.53mM EDTA)处理胚状体。将约105含有原代造血前体细胞的胚状体衍生细胞接种于甲基纤维素培养基(Stem Cell Inc.Canada)培养10-12天。然后通过天然红色或用单克隆抗-CD41或CD61抗体作免疫标记检测红细胞和巨核细胞集落形成单位(CFU)。第12天该明确的造血前体细胞形成了巨噬细胞和粒细胞集落。此活性代表第一波原代和明确的造血过程。我们现在建立了不含动物和人血清的无血清胚状体培养体系。
胚状体-衍生的巨核细胞前体的体外扩增
形成和培养胚状体12天后,37℃用胶原酶(1mg/ml)处理30分钟和用胰蛋白酶(0.05%胰蛋白酶/0.53mM EDTA)消化5分钟得到的细胞培养物制备单细胞悬液。将CD34+细胞与其它EB细胞分离,因为它们可能干扰造血过程。在血小板生成素(TPO)、白介素3(IL3)、白介素6(IL6)和干细胞因子(SCF)的存在下在聚-HEME表面上培养CD34+细胞,选择的所有因子能特异性促进巨核细胞分化和增殖。每106初始ES细胞获得106个CD41+巨核细胞的产量(n=6)。有趣的是,接种于胶原半固体基质时,这些巨核细胞形成极长的突起,其中珠样结构代表前体血小板。当尝试用成人造血干细胞或小鼠ES细胞来产生巨核细胞时,没有报道过这种长结构。检测到鉴定为释放的血小板的小CD41阳性细胞片段接近巨核细胞(所产生的)。通过流式细胞术,我们发现这些巨核细胞是CD41、CD42a、CD42b、CD61、CD38、CD45(弱)和CD62P阳性,但是CD34、CD117和HLA-DR阴性的细胞。此表型特征与正常的人成熟巨核细胞一致。
在基质层上人ES细胞分化为巨核细胞
OP9基质细胞系是由新生颅盖op/op缺陷型小鼠建立的细胞系,用于支持小鼠造血。op/op小鼠的巨噬细胞集落刺激因子(M-CSF)基因的编码区中有一突变。用OP9系统使人ES细胞分化为造血谱系细胞的结果类似于通过胚状体形成分化人ES细胞的方法,但基质细胞法通常可产生较高产量的更成熟的前体细胞。简要说,将人ES细胞接种于汇合的OP9基质细胞,然后用补充有20%胎牛血清(FBS)的α-MEM培养基培养。在六孔板中以每孔105个ES细胞或在10cm2培养皿中以8×105个细胞开始分化。培养6天后,ES细胞分化为造血祖细胞,细胞显示为CD34+细胞表面标记阳性。为了分化为巨核细胞,在第6天用胰蛋白酶处理细胞(0.05%胰蛋白酶/0.53mMEDTA,37℃/5%CO2)5分钟,然后转移到含10ng/ml TPO的相同培养基中的新鲜汇合OP9细胞上传代。再培养8天后,目测观察到开始出现巨核细胞。通过CD41免疫染色确认,培养物上清中约30%细胞是巨核细胞。这些巨核细胞是多核细胞,但没有胚状体-衍生巨核细胞中观察到的显著的长突起。认为这些巨核细胞是确定的巨核细胞很像成人巨核细胞。有趣的是,在培养期间,没有血小板形成的迹象,这与鼠系统很不相同。可能OP9细胞可促进和支持巨核细胞的分化和增殖,但不能支持血小板形成。这说明,小鼠和人的血小板形成机制不同,即使巨核细胞分化和增殖的一些机制相似。
巨核细胞增殖、成熟和纯化
上述方法产生的前体巨核细胞显示能够增殖,甚至移植入受者成年小鼠。该方法的下一个步骤是,我们采用bFGF进一步增殖不成熟的巨核细胞,同时阻止巨核细胞成熟。预计各巨核细胞可产生2000个血小板,106个人ES细胞(一个6孔板)会产生106个巨核细胞,随后产生约2×109个血小板,这代表约1/20个血小板单位(每单位>5.5×1010个血小板)。因此,以此预计效率,制备1单位人血小板将需要20个T75培养瓶的人ES细胞。此产量在经济上可能有或没有吸引力,但这显然在一个技术人员可以提供的范围内。
定向分化的其它技术
胚状体系统产生造血干细胞的效率低于所需效率,这是由于大部分细胞是卵黄囊细胞。然而,此系统优于OP9共培养系统,因为胚状体系统没有鼠蛋白污染。通过我们的数据,我们相信与用基质细胞的共同培养系统相反,EB系统的造血分化仍最好。为了得到更确定的造血细胞并使该方法更有效率,我们计划延长EB培养。我们尝试用机械方法使EB分解或片段化成较小片段,并且继续培养,希望这种微环境将继续支持血岛分化。我们的初步观察提示,分解后的EB可以存活并继续生长。这是第一次尝试在EB系统中进行分化。即使没有形成明确的血岛,也可实现血岛数量的显著增加。也测试了在EB培养物中加入生长因子如VEGF和SCF。
促进血小板释放和成熟
虽然我们已经在多种系统,包括胶原基质、OP9和聚HEME中观察到血小板形成,但我们想要更好地理解血小板释放的机制,以便优化该方法。为了实现血小板形成,我们在TPO、人血浆、人冷沉淀蛋白质和一氧化氮的存在下培养103~104个成熟巨核细胞。用抗-CD41抗体标记血小板,并用流式细胞术计数。通过显微术包括电子显微镜术检测血小板形状。真正的血小板应该是扁圆形,无突起或粘附于其它血小板。(出现)较大或连接的血小板说明所用的不是仅支持前体血小板形成的最优条件。据证明,胞外基质如纤维蛋白原和纤连蛋白能促进巨核细胞增殖和成熟。我们采用纤维蛋白原和纤连蛋白包被的平板培养巨核细胞,以测定它们对巨核细胞增殖和分化的影响。
测试血小板
血小板在对凝血酶、ADP和胶原反应时发生凝集。用凝集计(Chrono-logCorporation,www.chronolog.com)测定体外产生的血小板对不同刺激反应时凝集的能力。从上清中收获血小板并计数。用PBS洗涤106/ml个血小板,并重悬于人血浆。加入不同浓度的凝血酶、ADP和胶原,将凝集动力学与天然人血小板作比较。我们测试用0.5U/ml凝血酶可激活所产生的″血小板″和成熟的巨核细胞,它们表面表达α-颗粒释放的间接标记物CD62P。我们正在通过以下方法测试血小板的功能:
致密核心颗粒释放。先用缓冲液A(120mmol/L谷氨酸钠,5mmol/L谷氨酸钾,20mmol/L HEPES/NaOH,pH7.4,2.5mmoL EDTA,2.5mmol/L EGTA,3.15mmol/LMgCl2和1mmol/L DTT)配制的[3H]5-HT(5-羟色胺)标记106个培养的人血小板等份。用缓冲液A洗涤血小板,然后用1单位凝血酶活化。将样品放在冰上4分钟终止该反应,然后13,000g离心1分钟。收集上清,进行如下测定。通过闪烁计数测定[3H]5-HT的释放。也可用能同时测定凝集和致密核心颗粒的ATP分泌的光照凝集计(ChronologCorporation)评价致密核心颗粒释放的动力学。
α-颗粒分泌:通过流式细胞术用藻红蛋白-偶联的抗-CD62抗体AC 1.2(BectonDickinson)测定P-选择素表达监测此试验。一般将2.5μl固定血小板(109/ml)加入97.5μl抗体溶液中。15分钟后,用1ml含有0.35%BSA的Tyrode缓冲液稀释样品并分析。可计算P-选择素表达增加的百分数,并与人天然血小板作比较。
溶酶体释放:按照Holmsen和Dangelmaier所述方法测定氨基己糖苷酶。混合5ml柠檬酸盐-磷酸盐缓冲液,pH4.5和2.5ml10mmol/L底物(P-硝基苯基-N-乙酰-D-氨基葡糖苷),分成等份(100μL)加入96孔板,加入5μL反应上清。37℃孵育18小时后,加入60μL 0.08N NaOH以终止该反应。在装有405-nm滤光片的ELISA平板阅读器上读出吸光值。
用这些测试确立了由人胚胎干细胞产生的血小板的生物学活性。该体外血小板产生系统产生的血小板在功能上类似于人体的正常血小板。然而,体外产生和成熟时,所产生的血小板不难与血液产生的人血小板相区分,因为此方法产生的血小板从未接触,至少在产生过程中从未接触过人血。因此,此血小板不会粘附有正常血清因子,如纤维蛋白原、血凝因子V和VWF,而通常情况下,血小板在体内释放入血流后,会捕获这些因子。这是假设没有将显著量的这些因子加入培养物中,但有的情况下可能加入VWF以促进血小板分离,当然在输送给患者后,这种血小板会立即从受者血流中捕获这些因子。

(c)培养所述巨核细胞以产生血小板;和
(d)回收从所述巨核细胞分离的血小板。
2.如权利要求1所述的方法,其特征在于,通过促进胚状体的形成、然后从所述胚状体选择性回收造血细胞实施步骤(a)。
