Die vorliegende Erfindung betrifft ein medizintechnisches Instrument für die Knochenchirurgie gemäss Oberbegriff des Anspruchs 1.
Derartige Instrumente werden sowohl in der Human- als auch in der Veterinärmedizin verwendet und sind beispielsweise aus der DE-4 029 676 bekannt. Diese Druckschrift offenbart eine Stichsäge für chirurgische Zwecke, bei welcher in einem zylindrisch geformten Handstück ein Getriebe angeordnet ist, welches eine Rotationsbewegung des Antriebs in eine Kombinationsbewegung umsetzt. Diese Kombinationsbewegung umfasst einerseits eine oszillierende Bewegung, über welche eine Pendelbewegung überlagert ist.
In der Praxis erweisen sich derartige Instrumente als schlecht führbar. Insbesondere erschweren die Vibrationen des handgeführten Instrumentes und die in der Regel blutige und/oder schleimige Arbeitsumgebung das präzise Führen des Werkzeugs. Dies ist jedoch für die Erzeugung von sauberen Schnittflächen von besonderer Bedeutung. Saubere Schnittflächen erweisen sich als unabdingbar beim Einsetzen von Prothesen. Bei den Instrumenten bekannter Art kommt es immer wieder vor, dass das am Instrument befestigte Werkzeug (z.B. Säge, Bohrer, Schaber, Meissel) sich in der Schnittkerbe verkeilt oder verklemmt. Dabei rutscht das Instrument in der Hand des operierenden Arztes und es entstehen unsaubere Schnittflächen, die das Einbringen von Prothesen erschweren und den Heilungsprozess verlängern.
Dieser Sachverhalt führt auch dazu, dass der operierende Arzt für die Führung seines Instrumentes sehr viel Kraft aufwenden muss und deshalb auch rasch ermüdet.
Darüber hinaus lassen sich diese Instrumente nur schlecht reinigen, insbesondere müssen dieselben für die Sterili sation in der Regel auseinander gebaut werden, um die mit Schmierfetten versehenen Antriebs- oder Getriebeteile von den zu sterilisierenden Bauteilen zu trennen. Nach erfolgter Sterilisation sind die Instrumente wieder zusammenzusetzen. Die Wartung dieser Instrumente erweist sich also als äusserst aufwändig.
Es ist deshalb Aufgabe der vorliegenden Erfindung, ein Instrument zu schaffen, welches vom operierenden Arzt sicher und präzise geführt werden kann. Insbesondere soll ein Instrument geschaffen werden, welches dem Operationsumfeld angepasst ist und beim Gebrauch nicht zu einer raschen Ermüdung des Chirugen führt, d.h. rutschfest in der Hand des Chirurgen liegt. Darüber hinaus soll das Instrument in einfacher Weise gereinigt und gewartet werden können.
Diese Aufgabe wird erfindungsgemäss durch ein medizintechnisches Instrument mit den Merkmalen des vorliegenden Anspruchs 1 gelöst und insbesondere durch ein Instrument, welches ein Handstück mit einer Innenkammer aufweist, in welcher ein elekt-romotorischer Antrieb für oszillierende oder rotierende Werkzeuge angebracht ist, wobei das Handstück eine Aussenfläche aufweist, die von drei parallel zu einer zentralen Längsachse liegenden Griffflächen aufgespannt ist, derart, dass die Aussenfläche einen im Wesentlichen dreieckigen Querschnitt aufweist.
Weiterbildungen dieses medizintechnischen Instrumentes weisen die Merkmale der unabhängigen Ansprüche auf. Insbesondere können die einzelnen Griffflächen leicht gewölbt sein respektive der Form einer Handinnenfläche angepasst sein.
In einer bevorzugten Ausführungsform sind auf mindestens einer der Griffflächen manuell betätigbare Steuerelemente angebracht, mit denen der Chirurg beispielsweise den elektromotorischen Antrieb oder die Lichtintensität einer Lichtquelle oder die Fördermenge von Kühlflüssigkeiten oder die Absaugleistung von Absaugvorrichtungen etc. regulieren kann.
In besonderen Fällen erweist es sich als vorteilhaft, wenn das erfindungsgemässe Instrument mit einem zusätzlichen Handgriff in Form eines Pistolengriffs versehen ist. Es versteht sich, dass der elekt-romotorische Antrieb und/oder das Getriebe in diesem Pistolengriff untergebracht sein können. Vorteilhafterweise sollte die Innenkammer mit dem Antrieb und/oder dem Getriebe druckdicht ausgebildet sein. Damit soll verhindert werden, dass Schmieröle oder metallischer Abrieb in die Operationswunde gelangen respektive der elektromotorische Antrieb und/oder das Getriebe mit Körperflüssigkeiten verunreinigt werden.
Im Folgenden soll die Erfindung beispielhaft mithilfe der Figuren näher erläutert werden. Dabei zeigt: Fig. 1 eine schematische Darstellung eines erfindungsgemässen Instrumentes im Längsschnitt; Fig. 2 eine schematische Darstellung eines erfindungsgemässen Instrumentes im Querschnitt; Fig. 3a bis 3e verschiedene Anwendungsformen des erfindungsgemässen Instrumentes.
Fig. 1 zeigt das erfindungsgemässe Instrument in schematischer Weise und in einem Längsschnitt. Das Instrument weist eine Aussenfläche 9, eine Bodenfläche 4a und eine Stirnfläche 4b auf. Die Bodenfläche 4a und/oder Stirnfläche 4b können von der Aussenfläche 9 abnehmbar oder mit dieser verklebt/verschweisst sein. Das Innere des Instrumentes weist eine Innenkammer 2 auf, in welcher ein elektromotorischer Antrieb 5 und ein Getriebe 6 untergebracht werden können. Die zentrale Längsachse des Antriebs 5 liegt auf der zentralen Längsachse des erfindungsgemässen Instrumentes. Die Aussenfläche 9 wird, wie aus Fig. 2 ersichtlich, von drei Griffflächen 9', 9'', 9''' aufgespannt. Bei dem in Fig. 1 dargestellten Längsschnitt liegt die Grifffläche 9'' im Wesentlichen horizontal.
Im Bereich der von den beiden anderen Griffflächen 9', 9''' gebildeten Kante ist ein Versorgungskanal 3 vorgesehen, mit welchem Kühlflüssigkeit zur Operationsstelle geführt werden kann oder in welchem elektrische Kabel respektive Lichtleiter eingelegt werden können. Es versteht sich, dass die einzelnen Griffflächen leicht gewölbt sein können oder in anderer Weise der Form einer geschlossenen Handfläche angepasst sein können. Um den Antrieb 5 und/oder das Getriebe 6 vor Feuchtigkeit und anderen Verunreinigungen zu schützen, ist die Innenkammer 2 druckdicht abgeschlossen, insbesondere mithilfe einer Dichtung 8, durch welche eine Antriebsstange 7 geführt ist. Diese Dichtung 8 schliesst die Innenkammer 2 druckdicht ab, d.h. ist wasser- und gasdicht, insbesondere im Bereich von 0 bis 5 bar, vorzugsweise bis zu 3 bar (absolute pressure).
Dies erlaubt, das vorliegende Instrument in konventioneller Weise zu sterilisieren, d.h. bei Unterdruck während mindestens 5 Minuten auf ca. 180 DEG C aufzuheizen, ohne dass dabei ölige Dämpfe vom Antrieb 5 oder vom Getriebe 6 austreten. An der Antriebsstange 7 können die jeweiligen Werkzeuge 14 befestigt werden. Es versteht sich, dass die Antriebsstange 7 mit einem Getriebe bekannter Art in eine rotierende Bewegung oder in eine in Längsrichtung 11 oszillierende Bewegung oder in eine quer zur Längsrichtung schwingende Pendelbewegung oder in eine parallel zur Längsrichtung schwingende Pendelbewegung oder in eine Kombinationsbewegung versetzbar ist. Es sind auch Ausführungen denkbar, bei denen der Antrieb 5 ausserhalb des Handstücks angeordnet ist und sich in der druckdicht abgeschlossenen Innenkammer 2 lediglich das Getriebe 6 befindet.
Der in Fig. 2 dargestellte Querschnitt des erfindungsgemässen Instrumentes macht die Geometrie der Aussenfläche 9 und der Innenkammer 2 deutlich. Diese Innenkammer 2 ist hier als zylindrischer Hohlraum ausgebildet. Die Aussenfläche 9 wird hier von drei gleichartigen Griffflächen 9', 9'', 9''' aufgespannt. Es versteht sich, dass diese Griffflächen auch unterschiedlich dimensioniert sein können. In den von zwei Griffflächen gebildeten Eckbereichen liegen Kanäle 3, in welchen beispielsweise Kühlflüssigkeit, Strom oder Licht geführt werden kann. Es versteht sich, dass dieses Handstück aus einem besonders stabilen Material, beispielsweise Stahl, gefertigt werden kann. In bevorzugten Ausführungsformen ist dieses Handstück 1 aus Aluminium, Titan oder einem geeigneten Kunststoff gefertigt.
Fig. 3a bis 3e zeigen besondere Anwendungsformen des erfindungsgemässen medizintechnischen Instrumentes. Dabei zeigt Fig. 3a ein erfindungsgemässes Handstück 1, welches mit einer Stichsäge 14 ausgerüstet ist. Auf der oberen Grifffläche 9'' sind zwei Steuerelemente 13', 13'' angeordnet. Diese erlauben dem operierenden Arzt, die Bewegung des Sägeblatts in geeigneter Weise zu kontrollieren. Fig. 3b zeigt eine Ausführungsform für einen Saggitalschneider 15, Fig. 3c zeigt das erfindungsgemässe Instrument für einen quer zur Längsrichtung schwingenden Pendelschneider, Fig. 3d zeigt das erfindungsgemässe Instrument für einen Bohrer, während Fig. 3e dasselbe Instrument für einen Schaber zeigt. Diese Ausführungsformen machen deutlich, dass das erfindungsgemässe Instrument stirnseitig unterschiedlich ausgebildet sein kann.
Es versteht sich, dass das erfindungsgemäss geformte Instrument zusätzlich mit einem Griff in Form eines Pistolengriffs ausgerüstet sein kann und in diesem Griff Antrieb und/oder Getriebe angeordnet sein können.
Vorteilhafterweise wird das Instrument mit einer eigenen Stromversorgung versehen. Diese kann in Form eines Akkus, der allenfalls induktiv wiederaufladbar ist, einer Batterie oder eines Netzteils ausgebildet sein. Diese Stromversorgung kann im Handstück, in der Innenkammer oder im Pistolengriff untergebracht sein.
Damit erweist sich das erfindungsgemässe Instrument als sicher und präzise führbar, führt im Einsatz nicht zur raschen Ermüdung des Chirurgen und kann, wegen der druckdichten Innenkammer, in einfacher Weise gereinigt und/oder gewartet werden, insbesondere braucht das vorliegende Instrument für die Reinigung/Sterilisation nicht mehr auseinander gebaut zu werden. Die erfindungsgemässe Formgebung des Handstücks erweist sich darüber hinaus als besonders vorteilhaft, wenn während der Operation das Handstück kurzfristig beiseite gelegt werden muss, da dieses an Ort liegen bleibt, d.h. nicht wegrollen kann.

In the case of the known types of instruments, it often happens that the tool attached to the instrument (e.g. saw, drill, scraper, chisel) is wedged or jammed in the notch. The instrument slips in the hand of the operating doctor and there are unclean cut surfaces that make it difficult to insert prostheses and prolong the healing process.
This fact also means that the operating doctor has to use a lot of force to operate his instrument and therefore quickly tired.
In addition, these instruments are difficult to clean, in particular they must be disassembled for sterilization as a rule in order to separate the drive or transmission parts provided with greases from the components to be sterilized. After sterilization, the instruments must be reassembled. The maintenance of these instruments proves to be extremely complex.
It is therefore an object of the present invention to provide an instrument which can be safely and precisely guided by the operating doctor. In particular, an instrument is to be created which is adapted to the operating environment and does not lead to rapid fatigue of the surgeon during use, i.e. lies non-slip in the surgeon's hand. In addition, the instrument should be easy to clean and maintain.
This object is achieved according to the invention by a medical instrument with the features of the present claim 1 and in particular by an instrument which has a handpiece with an inner chamber in which an electro-motor drive for oscillating or rotating tools is attached, the handpiece having an outer surface has, which is spanned by three parallel to a central longitudinal axis gripping surfaces, such that the outer surface has a substantially triangular cross section.
Further developments of this medical technology instrument have the features of the independent claims. In particular, the individual gripping surfaces can be slightly curved or adapted to the shape of the palm of a hand.
In a preferred embodiment, manually operable control elements are attached to at least one of the gripping surfaces, with which the surgeon can regulate, for example, the electric motor drive or the light intensity of a light source or the delivery rate of cooling liquids or the suction power of suction devices etc.
In special cases it proves to be advantageous if the instrument according to the invention is provided with an additional handle in the form of a pistol grip. It goes without saying that the electromotive drive and / or the transmission can be accommodated in this pistol grip. The inner chamber with the drive and / or the transmission should advantageously be designed to be pressure-tight. This is to prevent lubricating oils or metallic abrasion from getting into the surgical wound, or from contaminating the electromotive drive and / or the transmission with body fluids.

Fig. 1 shows the instrument according to the invention in a schematic manner and in a longitudinal section. The instrument has an outer surface 9, a bottom surface 4a and an end surface 4b. The bottom surface 4a and / or end surface 4b can be removed from the outer surface 9 or glued / welded to it. The interior of the instrument has an inner chamber 2 in which an electromotive drive 5 and a gear 6 can be accommodated. The central longitudinal axis of the drive 5 lies on the central longitudinal axis of the instrument according to the invention. As can be seen in FIG. 2, the outer surface 9 is spanned by three gripping surfaces 9 ', 9' ', 9' ''. In the longitudinal section shown in FIG. 1, the gripping surface 9 ″ is essentially horizontal.
In the area of the edge formed by the two other gripping surfaces 9 ′, 9 ″ ″, a supply channel 3 is provided, with which cooling liquid can be led to the operation site or in which electrical cables or light guides can be inserted. It goes without saying that the individual gripping surfaces can be slightly curved or can be adapted in another way to the shape of a closed palm. In order to protect the drive 5 and / or the transmission 6 from moisture and other contaminants, the inner chamber 2 is closed in a pressure-tight manner, in particular with the aid of a seal 8, through which a drive rod 7 is guided. This seal 8 closes the inner chamber 2 pressure-tight, i.e. is water and gas tight, especially in the range from 0 to 5 bar, preferably up to 3 bar (absolute pressure).
This allows the present instrument to be sterilized in a conventional manner, i.e. heat up to approx. 180 ° C for at least 5 minutes under vacuum without oily vapors escaping from the drive 5 or the gear 6. The cross section of the instrument according to the invention shown in FIG. 2 clearly shows the geometry of the outer surface 9 and the inner chamber 2. This inner chamber 2 is designed here as a cylindrical cavity. The outer surface 9 is here spanned by three similar gripping surfaces 9 ', 9' ', 9' ''. It goes without saying that these gripping surfaces can also have different dimensions. Channels 3 are located in the corner regions formed by two gripping surfaces, in which, for example, coolant, electricity or light can be guided. It goes without saying that this handpiece can be manufactured from a particularly stable material, for example steel. In preferred embodiments, this handpiece 1 is made of aluminum, titanium or a suitable plastic.
3a to 3e show special forms of application of the medical instrument according to the invention. 3a shows a handpiece 1 according to the invention, which is equipped with a jigsaw 14. It goes without saying that the instrument shaped according to the invention can additionally be equipped with a handle in the form of a pistol grip and drive and / or gear can be arranged in this handle.
The instrument is advantageously provided with its own power supply. This can be designed in the form of a rechargeable battery, which is at most inductively rechargeable, a battery or a power pack. This power supply can be accommodated in the handpiece, in the inner chamber or in the pistol grip.
The instrument according to the invention thus proves to be safe and precise to operate, does not lead to rapid fatigue of the surgeon during use and, because of the pressure-tight inner chamber, can be cleaned and / or serviced in a simple manner, in particular the present instrument does not need for cleaning / sterilization to be more disassembled. The shape of the handpiece according to the invention also proves to be particularly advantageous if the handpiece has to be put aside for a short time during the operation, since it remains in place, i.e. can't roll away.
