BESCHREIBUNG
Die vorliegende Erfindung betrifft eine Vorrichtung zum vereinfachten Ansetzen eines Urinals bei männlichen, inkontinenten Personen und ein entsprechendes Urinal mit einer passenden Manschette.
Ein beträchtlicher Teil der Spitalpatienten, insbesondere ältere Leute, leiden heute unter Harninkontinenz, d. h. sie sind nicht in der Lage den Harnabgang zu kontrollieren. Die Ursache kann entweder im psychischen Bereich liegen oder aber von einem körperlichen Gebrechen herrühren. In den Spitälern und den Pflegeheimen müssen insbesondere die chronisch Kranken die unter Inkontinenz leiden, vom Pflegepersonal intensiver betreut werden.
Es sind bisher verschiedene mechanische Mittel bekannt, welche bei inkontinenten Personen angewendet werden. Beispielsweise die Blasensonde. Diese wird durch die männliche Harnröhre in die Harnblase eingeführt. Die Sonde enthält Kanäle, der eine dient dem Harnabgang und der andere dient zum Einspritzen einer Flüssigkeit, welche einen kleinen Ballon in der Nähe des Sondenendes auffüllt, welcher die Harnröhre von der Blase abdichten soll. Der Harnabgang erfolgt durch den zweiten Kanal und wird am Ausgang in einen Urinbeutel geleitet. Diese Sonden werden im Spital in Abständen von 1 bis 3 Wochen gewechselt. Bei der Erstanwendung sitzen solche Sonden in der Regel sehr gut und es gibt keine grossen Probleme. Solche Sonden sind preislich verhältnismässig günstig und sind für den Patienten sicher und von aussen nicht sichtbar und diskret zu tragen.
Die Sonde hat jedoch den Nachteil, dass sie von qualifiziertem Pflegepersonal oder von einem Arzt eingeführt werden muss.
Trotzdem die Blase mindestens alle Wochen einmal gespült werden muss, besteht eine Infektionsgefahr. Tatsächlich leiden die meisten Sonden-Patienten mit der Zeit an einer Harnleiter- oder Blaseninfektion. Nicht nur die Unannehmlichkeiten einer weiteren Krankheit, sondern ebenfalls die verhältnismässig hohen Folgekosten sind der Nachteil dieses Systems.
Weiter ist noch zu vermerken, dass sich der Blasenmuskel recht schnell dem aufgeblasenen Dichtungsballon der Sonde anpasst und dieser immer stärker aufgeblasen werden muss. Dadurch kann die Blase nicht mehr wirkungsvoll gegen die Harnröhre abgedichtet werden. Ebenso muss noch beigefügt werden, dass sich die Blase an die neue Situation anpasst und der Blasenmuskel erlahmt. Es ist auch möglich, dass unberechenbare Patienten die Sonde herausziehen können.
Ein weiteres Mittel zur Trockenhaltung von inkontinenten Patienten besteht in hochsaugfähigen Krankenunterlagen, Windeln und Höschenwindeln, die nach dem Gebrauch weggeworfen werden. Diese Mittel bestehen grundsätzlich aus dem gleichen Material wie die weitverbreiteten Wegwerf Windeln für Säuglinge. Mit den heutigen Materialien hergestellt, sind diese Mittel verhältuismäsig sicher, sie führen je doch zu nicht unerheblichen Kosten, da in der Regel mehrere solche Krankenunterlagen, bzw. Windeln verwendet werden müssen. Ein Nachteil besteht darin, dass solche Erzeugnisse sehr voluminös sind und zur Lagerhaltung viel Platz erforderlich ist.
Ein weiteres Mittel für die Anwendung bei männlichen Patienten besteht in einem Urinal, welches durch eine Penis Manschette in Form eines Rolltrichters angeschlossen wird.
Die Manschette wird über das Glied gerollt und darum herum mit einem Klebeband fixiert. Diese Art der Befestigung eines Urinals hat den Vorteil, dass die Manschette von aussen unbemerkt getragen wird, das Urinal in der Nacht nicht gewechselt werden muss und der Patient nicht geweckt werden muss, womit das das Pflegepersonal entlastet wird. Dieses System ist preislich günstiger als die beiden vorgenannten Mittel und kann vom Patienten gegebenenfalls auch selbst angewendet werden.
Diese Manschette hat jedoch den Nachteil, dass eine Befestigung bei gewissen Patienten schwierig ist und für das Pflegepersonal eine eher unangenehme Vorrichtung darstellt.
Aufgabe der vorliegenden Erfindung war es demzufolge, eine Vorrichtung zur Verfügung zu stellen, durch welche ein Urinal in einer für das Personal problemlosen Weise wirksam angesetzt werden kann.
Gegenstand der vorliegenden Erfindung ist demzufolge, die in den Patentansprüchen definierte Vorrichtung und das dazu passende Urinal. Bei der Anwendung der erfindungsgemässen becherförmigen Vorrichtung wird die Manschette des Urinals, die im wesentlichen dem bekannten Rolltrichter entspricht und mit einer Klammer an der Ausgangsöffnung verschlossen ist, über die Öffnung der becherförmigen Vorrichtung gezogen. In der so verschlossenen Vorrichtung wird nun ein Unterdruck erzeugt, dadurch wird die Manschette in Richtung des Becherinnern aufgeblasen und das Innere der Manschette wird problemlos zugänglich. Damit ist ein einfaches und schnelles Überziehen über das männliche Glied, bzw. der gesamten äusseren, männlichen Geschlechtsteile, einschliesslich der Hoden, möglich.
Dieser sowohl für den Patienten, wie auch für das Pflegepersonal etwas unangenehme Arbeitsgang wird mittels der erfindungsgemässen Vorrichtung auf die möglichst angenehmste Weise durchgeführt, indem der Druck im Innern des becherförmigen Gerätes wieder der Umgebung angepasst wird und sich die Manschette langsam an das Glied und gegebenenfalls auch an die Hodengegend anpasst. Der Rand der Manschette wird dabei vom Rand der erfindungsgemässen Vorrichtung weggeschoben. Das Überziehen der Manschette mit dem erfindungsgemässen Gerät führt zu einer präzisen Anpassung der Manschette, wodurch ein optimaler Schutz und folgedessen das Auslaufen des Urins verunmöglicht wird, sogar wenn sie bei einem unruhigem Patienten angesetzt wird.
Es ist jedoch auch möglich, dass die Manschette des erfindungsgemässen Urinals an der Eingangsöffnung ein zusätzliches Befestigungsmittel aufweist. Dies kann beispielsweise ein Band oder ein netzartiges Gebilde sein, die zur Sicherung der Manschette um die Hoden herum befestigt werden können.
Solche Vorrichtungen, welche über die gesamten äusseren, männlichen Geschlechtsteile angewandt werden, können in der Nähe der Eingangsöffnung eine grössere, innere Weite aufweisen oder können den anatomischen Formen angepasst werden. Die dabei verwendete gummielastische Manschette des Urinals muss natürlich in einem solchen Fall ebenfalls dieser Form angepasst werden. Dabei muss aber berücksichtigt werden, dass die Manschette in der Hodengegend keinen starken Druck ausüben kann, aber an den übrigen Stellung so eng anliegen muss, dass eine Abdichtung gewährleistet ist.
Bei der Anwendung der erfindungsgemässen Vorrichtung, muss die Ausflussöffnung der Manschette abgedichtet sein. Dies geschieht zweckmässigerweise durch eine Klammer oder einen Quetschhahn. Es ist natürlich auch möglich, dass die Manschette beispielsweise durch eine aufgeschweisste Folie abgedichtet ist, welche nach dem Ansetzen leicht entfernt werden kann, um den Schlauch des Urinbeutels anzuschliessen.
Zur Erzeugung eines Unterdrucks im Innern der Vorrichtung kann sie einen Stutzen aufweisen, der vorzugsweise in unteren Bereich, d. h. in der Nähe der der Becheröffnung entgegengesetzten Seite liegt. Die Erzeugung des Unterdrucks kann beispielsweise durch eine einfache mechanische Saugvorrichtung erfolgen. Beispielsweise durch einen Peleus Ball, wie er beim Pipettieren verwendet wird. Alternativ kann die Vorrichtung auch so ausgebildet sein, dass sie zusammengedrückt werden kann, wobei sein Inhalt ein kleineres Volumen umfasst und es danach, unter Bildung eines grösseren Volumens, wiederum in seine ursprüngliche Form zurückgehen kann. In diesem Fall kann die Vorrichtung ein Ventil aufweisen, welches beispielsweise durch Druck geöffnet werden kann und beim Loslassen geschlossen ist.
Mit dieser Vorrichtung kann ohne eine zusätzliche Saugvorrichtung die Manschette zum Aufbringen vorbereitet werden: die am Ausgangsende verschlossene Manschette wird über die Becheröffnung gezogen, das Ventil wird geöffnet und die Vorrichtung wird zusammengedrückt, das Ventil wird verschlossen und die Vorrichtung geht unter Bildung eines Unterdruckes in ihrem Innern wieder in ihre ursprüngliche Form zurück. Damit der Volumenunterschied zwischen der zusammengedrückten und der entspannten Vorrichtung möglichst gross ist, kann mindestens ein Teil der Vorrichtung balgartig ausgebildet sein. Dabei kann im Balg eine Spiralfeder vorhanden sein, die dazu dient, die Vorrichtung wiederum in ihre ursprüngliche, volumenreichere Form zurückbilden.
Anstelle eines Ventils kann die mit einem balgartigen Teil versehene Vorrichtung ebenfalls so ausgestaltet sein, dass der zusammengedrückte, balgartige Teil arretiert werden kann. In diesem Fall muss die Manschette in diesem Zustand aufgebracht werden. Nach dem Aufbringen der Manschette kann der balgartige Teil expandiert werden und zieht gleichzeitig die Manschette ins Innere der Vorrichtung.
Damit durch das feste Anliegen der Manschette die Perspiration am Glied trotzdem möglicht ist, weist die Manschette vorzugsweise mindestens zum Teil Rippen auf, die bevorzugt im wesentlichen in Längsrichtung angeordnet sind. Zum gleichen Zweck und zur Ermöglichung der Luftzirkulation kann der elastische Rand der Manschette, der vorzugsweise etwa 5 mm dick ist und gegebenenfalls auch ein daran vorhandenes zusätzliches Befestigungsmittel, aus luftdurchlässigem Material, wie einem porösen Elastomer, hergestellt sein.
Wie andere dünnwandige, weiche Artikel mit hoher Festigkeit (Ballone, chirurgische Handschuhe etc.), bestehen die Manschetten von erfindungsgemässen Urinalen vorzugsweise aus elastomeren Materialien auf Basis von Naturkautschuk.
Im folgenden wird die Erfindung von lediglich Ausführungsbeispielen darstellenden Zeichnungen näher erläutert.

In den Figuren ist eine Vorrichtung zum vereinfachten Ansetzen eines Urinals bei männlichen, inkontinenten Personen und die dem Urinal zugehörige Penis-Manschette dargestellt.
Die Vorrichtung besteht aus dem becherförmigen Gefäss (1), welches die Öffnung (2) aufweist, welche durch den Rand (5) abgegrenzt ist. In der Nähe derjenigen Seite, die der Öffnung abgewandt ist, befindet sich ein Stutzen (6), um die Luft des becherförmigen Innern abzusaugen. Dies kann durch eine mechanische Saugvorrichtung, beispielsweise durch einen Peleus-Ball (7) erfolgen.
Die Manschette (3) wird über die becherförmige Öffnung (2) gestülpt, so dass das mit einer Klammer (13) verschlossene Ausflussende (14) der Manschette in das Innere des Gefässes ragt.
Durch das Absaugen von Luft aus dem becherförmigen Innern wird die verschlossene Manschette in Richtung des Becherinnern aufgeblasen. Die Vorrichtung mit der aufgeblasenen Manschette ist in Figur 3 dargestellt.
In einer speziellen Ausführungsform, wie sie in Figur 5 dargestellt ist, weist die Vorrichtung eine Ausbuchtung (11) auf. Diese Vorrichtung dient zum Aufbringen der speziellen, in Figur 6 dargestellten, Manschette, welche in der Nähe der Eingangsöffnung mit dem Rand (4) eine vergrösserte innere Weite besitzt. Diese Manschette und die zugehörige Vorrichtung dienen zum Anbringen der Manschette über die gesamten äusseren, männlichen Geschlechtsteile, einschliesslich der Hoden, gewährleisten jedoch, dass kein übermässiger Druck auf die Hodengegend ausgeübt wird.
In den Figuren 7 und 8 sind weitere Ausführungsformen der Vorrichtung dargestellt, welche einen balgartigen Teil (8) aufweist. In Figur 7 weist die Vorrichtung ebenfalls einen Stutzen (6) mit einem Ventil (16) auf. Hier kann die Manschette über den Rand (5) gezogen werden, das Ventil (16) geöffnet werden und der Balg (8) zusammengedrückt werden, wobei nach dem Schliessen des Ventils (16) und dem Ausdehnen des Balges (8) im Innern der Vorrichtung ein Unterdruck entsteht und die Manschette gegen innen aufgebläht wird. Alternativ kann das Ventil (16) ebenfalls ein Rückschlagventil sein.
In Figur 8 ist eine analoge Vorrichtung dargestellt, die jedoch kein Ventil aufweist. Hier muss die Manschette aufgezogen werden, wenn der Balg zusammengedrückt ist, d. h.
wenn die Vorrichtung das kleinstmögliche Volumen aufweist. Damit das Aufziehen der Manschette problemlos geschuhen kann, besitzt diese Vorrichtung eines Arretierungsmittel, welches die problemlose Handhabung in gespanntem Zustand erlaubt. Ist die Manschette am Rand (5) angebracht, kann der Balg durch Öffnung des Bajonett Verschlusses (15) wiederum in seine ursprüngliche Form zurückgebracht werden, wobei sich die Manschette ebenfalls in Richtung des Becherinnern aufbläst.
DESCRIPTION
The present invention relates to a device for simplified attachment of a urinal in male, incontinent people and a corresponding urinal with a suitable cuff.
A significant proportion of hospital patients, especially the elderly, today suffer from urinary incontinence, i.e. H. they are unable to control urination. The cause can either be in the psychological area or it can come from a physical ailment. In the hospitals and nursing homes in particular the chronically ill who suffer from incontinence have to be looked after more intensively by the nursing staff.
So far, various mechanical means are known which are used in incontinent people. For example, the bladder probe. This is inserted into the bladder through the male urethra. The probe contains channels, one for urine drainage and the other for injecting a fluid that fills a small balloon near the end of the probe to seal the urethra from the bladder. The urine is discharged through the second channel and is passed into a urine bag at the exit. These probes are changed in the hospital at intervals of 1 to 3 weeks. When used for the first time, such probes usually sit very well and there are no major problems. Such probes are relatively inexpensive and are safe for the patient and are not visible and discreet from the outside.
However, the probe has the disadvantage that it must be inserted by qualified nursing staff or by a doctor.
Even though the bladder has to be rinsed at least once every week, there is a risk of infection. In fact, most probes suffer from ureteral or bladder infections over time. Not only the inconvenience of another illness, but also the relatively high follow-up costs are the disadvantage of this system.
It should also be noted that the bladder muscle adapts very quickly to the inflated sealing balloon of the probe and that it has to be inflated more and more. As a result, the bladder can no longer be effectively sealed against the urethra. It must also be added that the bladder adapts to the new situation and the bladder muscle weakens. It is also possible that unpredictable patients can pull out the probe.
Another means of keeping incontinent patients dry is by absorbent medical pads, diapers and pant diapers, which are thrown away after use. These funds consist basically of the same material as the widely used disposable diapers for infants. Manufactured with today's materials, these means are relatively safe, but they lead to not inconsiderable costs, since usually several such medical documents or diapers have to be used. A disadvantage is that such products are very voluminous and a lot of space is required for storage.
Another means for use in male patients is a urinal, which is connected by a penis cuff in the form of a roller funnel.
The cuff is rolled over the limb and fixed around it with an adhesive tape. This type of attachment of a urinal has the advantage that the cuff is carried unnoticed from the outside, the urinal does not have to be changed at night and the patient does not have to be woken up, which relieves the nursing staff. This system is cheaper than the two aforementioned means and can also be used by the patient if necessary.
However, this cuff has the disadvantage that it is difficult to attach to certain patients and is a rather unpleasant device for the nursing staff.
The object of the present invention was therefore to provide a device by means of which a urinal can be effectively placed in a manner which is problem-free for the personnel.
The present invention accordingly relates to the device defined in the claims and the matching urinal. When using the cup-shaped device according to the invention, the cuff of the urinal, which essentially corresponds to the known roller funnel and is closed with a clip at the exit opening, is pulled over the opening of the cup-shaped device. A vacuum is now generated in the device thus closed, as a result of which the cuff is inflated in the direction of the cup interior and the inside of the cuff is easily accessible. This enables simple and quick covering over the male limb or the entire outer male genitals, including the testicles.
This operation, which is somewhat unpleasant both for the patient and for the nursing staff, is carried out in the most pleasant way possible by means of the device according to the invention, in that the pressure inside the cup-shaped device is adapted to the environment again and the cuff slowly adapts to the limb and possibly also adapts to the testicle area. The edge of the cuff is pushed away from the edge of the device according to the invention. The covering of the cuff with the device according to the invention leads to a precise adjustment of the cuff, whereby an optimal protection and consequently the leakage of the urine is impossible, even if it is applied to a restless patient.
However, it is also possible for the sleeve of the urinal according to the invention to have an additional fastening means at the entrance opening. This can be, for example, a band or a net-like structure that can be fastened around the testicles to secure the cuff.
Such devices, which are used over the entire outer male genitals, can have a larger inner width near the entrance opening or can be adapted to the anatomical shapes. In this case, the rubber-elastic cuff of the urinal used must of course also be adapted to this shape. However, it must be taken into account that the cuff cannot exert strong pressure in the area of the testicles, but must be so close to the other positions that a seal is guaranteed.
When using the device according to the invention, the outflow opening of the cuff must be sealed. This is expediently done by means of a clamp or a pinch valve. It is of course also possible for the cuff to be sealed, for example, by a welded-on film, which can be easily removed after attachment in order to connect the tube of the urine bag.
To generate a negative pressure in the interior of the device, it can have a nozzle, which is preferably in the lower region, ie. H. is close to the side opposite the cup opening. The vacuum can be generated, for example, by a simple mechanical suction device. For example, with a Peleus ball, as used in pipetting. Alternatively, the device can also be designed in such a way that it can be pressed together, its content comprising a smaller volume and it then being able to return to its original shape, forming a larger volume. In this case, the device can have a valve which can be opened, for example, by pressure and is closed when released.
With this device, the cuff can be prepared for application without an additional suction device: the cuff which is closed at the outlet end is pulled over the cup opening, the valve is opened and the device is compressed, the valve is closed and the device goes into it with the formation of a negative pressure Inside back to its original shape. In order for the volume difference between the compressed and the relaxed device to be as large as possible, at least a part of the device can be designed as a bellows. In this case, a spiral spring can be present in the bellows, which serves in turn to restore the device to its original, more voluminous shape.
Instead of a valve, the device provided with a bellows-like part can also be designed such that the compressed, bellows-like part can be locked. In this case, the cuff must be applied in this condition. After the cuff has been applied, the bellows-like part can be expanded and at the same time pulls the cuff into the interior of the device.
So that perspiration on the limb is nevertheless possible due to the fixed abutment of the cuff, the cuff preferably has at least in part ribs, which are preferably arranged essentially in the longitudinal direction. For the same purpose and to enable air circulation, the elastic edge of the cuff, which is preferably about 5 mm thick and optionally also an additional fastening means present thereon, can be made of air-permeable material, such as a porous elastomer.
Like other thin-walled, soft, high-strength articles (balloons, surgical gloves, etc.), the cuffs of urinals according to the invention preferably consist of elastomeric materials based on natural rubber.

The figures show a device for simplified attachment of a urinal to male, incontinent people and the penis cuff associated with the urinal.
The device consists of the cup-shaped vessel (1) which has the opening (2) which is delimited by the edge (5). In the vicinity of the side facing away from the opening there is a connection piece (6) for sucking out the air from the cup-shaped interior. This can be done by a mechanical suction device, for example by a Peleus ball (7).
The cuff (3) is placed over the cup-shaped opening (2), so that the outflow end (14) of the cuff, which is closed with a clip (13), projects into the interior of the vessel.
By sucking air out of the cup-shaped interior, the closed cuff is inflated in the direction of the cup interior. The device with the inflated cuff is shown in Figure 3.
In a special embodiment, as shown in Figure 5, the device has a bulge (11). This device is used to apply the special sleeve shown in FIG. 6, which has an enlarged inner width near the entrance opening with the edge (4). This cuff and the associated device are used to attach the cuff over the entire outer male genitals, including the testicles, but ensure that no excessive pressure is exerted on the testicle area.
FIGS. 7 and 8 show further embodiments of the device which has a bellows-like part (8). In Figure 7, the device also has a nozzle (6) with a valve (16). Here the cuff can be pulled over the edge (5), the valve (16) opened and the bellows (8) compressed, whereby after closing the valve (16) and expanding the bellows (8) inside the device a vacuum is created and the cuff is inflated towards the inside. Alternatively, the valve (16) can also be a check valve.
An analog device is shown in FIG. 8, but has no valve. Here the cuff has to be pulled up when the bellows is compressed, i. H.
if the device has the smallest possible volume. So that the cuff can be pulled open without any problems, this device has a locking means which allows easy handling in a tensioned state.
