BESCHREIBUNG
Die Erfindung betrifft die Verwendung von l-Isopropyl amino-3-[o-(1-pyrrolyl)-phenoxy]-2-propanol oder eines pharmazeutisch verwendbaren, nichttoxischen Salzes davon und einen solchen Wirkstoff enthaltende pharmazeutische Präparate zur Behandlung von Angstzuständen.
Die Therapie von Angstzuständen gewinnt zunehmend an Bedeutung. Für die Behandlung derartiger Symptome wird eine Vielzahl an Präparaten, insbesondere Anxiolytika, angeboten. Allerdings wird der therapeutische Effekt einer Reihe von Medikamenten von unerwünschten Begleiterscheinungen überlagert, die z.B. im Bereich des Zentralnervensystems auftreten können. So kann die Verwendung bestimmter Medikamente zu ausgeprägten sedativen Effekten und bei länger anhaltender Therapie zu unerwünschten Gewöhnungseffekten führen.
In der Psychopharmakotherapie von Angstzuständen treten neuerdings Beta-Rezeptoren-Blocker in den Vordergrund des Interesses [Cuche, H.; Deniker, P.: Pharmacopsychiat. 13, 273 ff (1980)]. Von diesen wird angenommen, dass sie ohne wesentliche Nebenwirkungen und ohne Gewöh nungsgefahr gewisse Angstsymptome beheben können [K.
Rickels et al., Current Therapeutic Research, Vol. 40, S. 149 (1986)].
Ziel vielfacher Bemühungen ist die Bereitstellung potenter Anxiolytika aus der Gruppe der Beta-Rezeptoren-Blokker für die Behandlung von Angstzuständen, die zudem, bei entsprechender Dosierung, praktisch aber keine Beta-Rezeptoren-blockierende Eigenschaften zeigen sollen.
Als besonders vorteilhaft hat sich die Verwendung von 1 Isopropylamino-3-[o-(1-pyrrolyl)-phenoxy]-2-propanol oder eines pharmazeutisch verwendbaren, nichttoxischen Salzes davon herausgestellt.
Pharmazeutisch verwendbare Salze dieses Wirkstoffs sind dessen pharmazeutisch verwendbare Säureadditionssalze, beispielsweise mit starken anorganischen Säuren, wie Mineralsäuren, mit Sulfaminsäuren, wie Cycloniederalkylsulfaminsäuren, mit starken organischen Carbonsäuren, wie Niederalkancarbonsäuren, gegebenenfalls ungesättigten Dicarbonsäuren oder durch Hydroxy und/oder Oxo substituierten Niederalkancarbonsäuren, oder mit Sulfonsäuren, wie Niederalkan- oder gegebenenfalls substituierten Benzolsulfonsäuren, beispielsweise Sulfate, Phosphate, Hydrohalogenide, wie Hydrochloride oder Hydrobromide, Cyclohexylsulfamate, Acetate, Malonate, Oxalate, Maleinate, Fumarate, Citrate, Tartrate, Pyruvate sowie Sulfonate, insbesondere Methansulfonate, ferner Benzol- oder p-Toluolsulfonate.
Dabei sind unter mit Nieder bezeichneten Teilstrukturen, sofern nicht abweichend definiert, vorzugsweise solche zu verstehen, die bis und mit 7, vor allem bis und mit 4 Kohlenstoffatome (C-Atome) enthalten.
Infolge der engen Beziehung zwischen dem Wirkstoff in freier Form und in Form seines pharmazeutisch verwendbaren Salzes sind vorstehend und nachfolgend unter der freien Verbindung bzw. seinen pharmazeutisch verwendbaren Salzen sinn- und zweckgemäss gegebenenfalls auch die entsprechenden pharmazeutisch verwendbaren Salze bzw. die freie Verbindung zu verstehen.
Der erfindungsgemäss verwendbare Wirkstoff kann auch in Form seiner Hydrate vorliegen oder andere pharmazeutisch unbedenkliche Lösungsmittel, beispielsweise solche, die zur Kristallisation des entsprechenden Wirkstoffs, der in fester Form vorliegt, verwendet werden, einschliessen.
Dieser Wirkstoff bzw. dessen Salze sind aus der Deutschen Offenlegungsschrift Nr. 2,409,313 bekannt. Die in dieser Referenz offenbarten Substanzen können teilweise als Beta-Rezeptoren-Blocker mit erregungshemmenden Eigenschaften verwendet werden.
Es wurde nun überraschend gefunden, dass l-Isopropyl amino-3-[o-(1-pyrrolyl)-phenoxy]-2-propanoSund entsprechende pharmazeutisch verwendbare, nichttoxische Salze davon ausgeprägte anxiolytische Aktivitäten aufweist, die bei geringer Dosierung von den Beta-Rezeptoren-hemmenden Merkmalen differenziert vorliegen. Gegenüber erregungshemmenden Substanzen, wie Neuroleptika, die normalerweise eine starke sedative Wirkungskomponente zeigen, besitzt l-Isopropylamino-3-[o-(1-pyrrolyl)-phenoxy]-2- propanol keinerlei sedative Effekte.
Das charakterische anxiolytische Profil von l-Isopropyl- amino-3-[o-(1-pyrrolyl)-phenoxy]-2-propanol ergibt sich beispielsweise aus der Hemmung der Übererregtheit von Ratten, die in Isolation gehalten werden, gemessen an ihrem lokomotorischen und exploratorischen Verhalten, wobei maximale Effekte bei einer Dosierung von 0,01-0,1 mg/kg p.o. erzielt werden [analog der Methodik von R. Weinstock et al., Psychopharmacologia 30, 241ff. (1973).
Auch in dem Geller-Test [J. Geller, Psychopharmacologia, 3, S. 374ff. (1962)], in dem Ratten einer durch Fussschock induzierten Konfliktsituation ausgesetzt werden, erweist sich l-Isopropylamino-3-[o-(1-pyrrolyl)-phenoxy]-2- propanol als starkes Anxiolytikum. Bei diesem Test werden die Füsse der Versuchstiere beim Bedienen eines Hebels zur Nahrungsfreigabe periodisch Elektroschocks ausgesetzt.
Normalerweise wird nur bei Verabreichung von hohen Dosen an Anxiolytika die Reaktion auf die Bestrafung unterdrückt. Bei einer Gruppe von 29 Ratten, die eine Dosis von 20 mg/kg p.o. von 1-Isopropylamino-3-[o-1-pyrrolyl) phenoxy]-2-propanol erhalten haben, reagieren bereits 50% der Versuchstiere mit einem markanten Anstieg der Hebelbedienungshäufigkeit während der Schockphase, wobei der Anstieg bei einzelnen Tieren 300% betrug.
Insbesondere konnten signifikante anxiolytische Effekte bei Verabreichung des Wirkstoffs in sozialen Konfliktsituationen bei Rhesusaffen festgestellt werden. Die experimentelle Grundlage für diesen Sozialkonflikt-Test ist die hierarchische Rangordnung bei Rhesusaffen. Der relative Rangstatus eines Tieres ist der bestimmende Faktor für interindividuelle Kommunikation, z. B. für Aggression oder wechselseitiges soziales Putzen. Normalerweise werden Tiere mit geringem Rangstatus von höher gestellten Gruppenmitgliedern unterdrückt. Diese unterdrückten Tiere ziehen sich in eine soziale Isolierung zurück und vermeiden ängstlich jeden Sozialkontakt.
Wiederholte Wirkstoffgaben in Dosen von 0,1-0,3 mg/kg p.o. bei Gruppen von 7 Affen führte zu einer signifikanten Reduktion des ängstlichen Verhaltensmusters der Affen, die in der Rangordnung am niedrigsten eingeordnet sind. Ebenso nahmen sie aktiv Kontakt auf und wurden zu Partnern beim Pflegen, obwohl sie vorher niemals Pflegepartner waren. Diese Effekte wurden sowohl bei Gruppen mit hohem Niveau an Sozialkontakten als auch bei Gruppen mit wenig Pflegepartnerschaften beobachtet.
Überraschenderweise wurde dieses ausgeprägte anxiolytische Profil bei den Standard-Beta-Rezeptoren-Blockern Oxprenolol und Propranolol nicht bzw. in weitaus geringerem Masse festgestellt.
So konnte die Übererregtheit von isoliert gehaltenen Ratten im Falle des Propranolols erst bei einer Dosis von 0,3 mg/kg p.o. maximal gehemmt werden.
Im Fussschocktest nach Geller führte eine Dosierung von 20 mg/kg Propranolol p. o. bei Ratten zu keiner signifikanten Verhaltensänderung.
Auch im Verhaltensmuster der Rhesusaffen konnte nach Verabreichung von 0,3-3,0 mg/kg p.o. Oxprenolol keine Steigerung der Sozialkontakte von Affen mit geringer Rang- ordnung beobachtet werden. Bei einer Dosierung 0,5-10,0 mg/kg p.o. Propranolol wurden die Sozialkontakte der Rhesusaffen mit niedrigem Rangstatus relativ wenig beeinflusst.
Der erfindungsgemäss zu verwendende Wirkstoff ist weiterhin ein potenter selektiver Antagonist des präsynaptischen Serotonin-lB-Rezeptors (5-HTIB), während Propranolol und Oxprenolol weniger selektiv wirken, da diese ebenso mit den postsynaptischen 5-HT1A-Rezeptor reagieren.
Somit weist l-Isopropylamino-3-[o-(l-pyrrolyl)-phen- oxy]-2-propanol ein ausgeprägtes und spezifisches anxiolytisches Profil auf, da es beispielsweise aggressives Verhalten reduziert und vor allem zu einer Verbesserung des positiven Sozialverhaltens führt. Dementsprechend kann l-Isopropyl amino-3-[o-(1-pyrrolyl)-phenoxy]-2-propanol oder ein pharmazeutisch verwendbares, nichttoxisches Salz davon bei der Behandlung von Angstzuständen, insbesondere von generalisierten (chronischen) Angstzuständen, panic disorders, obsessive compulsive behavior und Phobien, wie sozialen Phobien, verwendet werden.
Gegenstand der Erfindung ist somit die Verwendung von l-Isopropylamino-3-[o-(l -pyrrolyl)-phenoxy]-2-propanol oder eines pharmazeutisch verwendbaren, nichttoxischen Salzes zur Herstellung von pharmazeutischen Präparaten zur Behandlung von Angstzuständen.
Ein bevorzugtes pharmazeutisch verwendbares Salz ist das entsprechende Hydrochlorid.
Die vorliegende Erfindung betrifft ebenso pharmazeutische Präparate zur Behandlung von Angstzuständen, die als Wirkstoff 1-Isopropylamino-3-[o-(l -pyrrolyl)-phenoxy]-2- propanol oder ein pharmazeutisch verwendbares, nichttoxisches Salz davon enthalten.
Bei den erfindungsgemässen pharmazeutischen Präparaten, welche die erfindungsgemässe zu verwendbare Verbindung oder pharmazeutisch verwendbare Salze davon enthalten, handelt es sich um solche zur enteralen, wie oralen, ferner rektalen, und parenteralen Verabreichung an Warmblüter(n), wobei der pharmakologische Wirkstoff allein oder zusammen mit einem pharmazeutisch anwendbaren Trägermaterial enthalten ist.
Die neuen pharmazeutischen Präparate enthalten z. B. ab etwa 0,5%, vorzugsweise ab 1,0%, des Wirkstoffs. Erfindungsgemässe pharmazeutische Präparate zur enteralen bzw. parenteralen Verabreichung sind z. B. solche in Dosiseinheitsformen, wie Dragees, Tabletten, Kapseln oder Suppositorien, ferner Ampullen. Diese werden in an sich bekannter Weise, z. B. mittels konventioneller Misch-, Granulier-, Dragier-, Lösungs- oder Lyophilisierungsverfahren hergestellt. So kann man pharmazeutische Präparate zur oralen Anwendung erhalten, indem man den Wirkstoff mit festen Trägerstoffen kombiniert, ein erhaltenes Gemisch gegebenenfalls granuliert, und das Gemisch bzw. Granulat, wenn erwünscht oder notwendig, nach Zugabe von geeigneten Hilfsstoffen zu Tabletten oder Dragée-Kernen verarbeitet.
Geeignete Trägerstoffe sind insbesondere Füllstoffe, wie Zucker, z.B. Lactose, Saccharose, Mannit oder Sorbit, Cellulosepräparate und/oder Calciumphosphate, z.B. Tricalciumphosphat oder Calciumhydrogenphosphat, ferner Bindemittel, wie Stärkekleister, unter Verwendung von Mais-, Weizen-, Reis- oder Kartoffelstärke, Gelatine, Tragakanth, Methylcellulose und/oder Polyvinylpyrrolidon, wenn erwünscht, Sprengmittel, wie die obengenannten Stärken, ferner Carboxmethylstärke, quervernetzes Polyvinylpyrrolidon, Agar, Alginsäure oder ein Salz davon, wie Natriumalginat.
Hilfsmittel sind in erster Linie Fliess-, Regulier- und Schmiermittel, z.B. Kieselsäure, Talk, Stearinsäure oder Salze davon, wie Magnesium- oder Calciumstearat, und/oder Polyethylenglykol. Dragée-Kerne werden mit geeigneten, gegebenenfalls Magensaft-resistenten Überzügen versehen, wobei man u.a. konzentrierte Zuckerlösungen, welche gegebenenfalls arabischen Gummi, Talk, Polyvinylpyrrolidon, Polyethylenglykol und/oder Titandioxid enthalten, Lacklösungen in geeigneten organischen Lösungsmitteln oder Lö sungsmittelgemische oder, zur Herstellung von Magensaftresistenten Uberzügen, Lösungen von geeigneten Cellulosepräparaten, wie Acetylcellulosephthalat oder Hydroxypropylmethylcellulosephthalat, verwendet. Den Tabletten oder Dragee-Überzügen können Farbstoffe oder Pigmente, z. B.
zur Identifizierung oder zur Kennzeichnung verschiedener Wirkstoffdosen, beigefügt werden.
Weitere oral anwendbare pharmazeutische Präparate sind Steckkapseln aus Gelatine, sowie weiche, geschlossene Kapseln aus Gelatine und einem Weichmacher, wie Glycerin oder Sorbitol. Die Steckkapseln können den Wirkstoff in Form eines Granulats, z. B. im Gemisch mit Füllstoffen, wie Lactose, Bindemitteln, wie Stärken, und/oder Gleitmitteln, wie Talk oder Magnesiumstearat, und gegebenenfalls Stabilisatoren, enthalten. In weichen Kapseln ist der Wirkstoff vorzugsweise in geeigneten Flüssigkeiten, wie fetten Ölen, Paraffinöl oder flüssigen Polyethylenglykolen, gelöst oder suspendiert, wobei ebenfalls Stabilisatoren zugefügt sein können.
Als rektal anwendbare pharmazeutische Präparate kommen z. B. Suppositorien in Betracht, welche aus einer Kombination des Wirkstoffs mit einer Suppositoriengrundmasse bestehen. Als Suppositorienmasse eignen sich z. B. natürliche oder synthetische Triglyceride, Paraffinkohlenwasserstoffe, Polyethylenglykole oder höhere Alkanole. Ferner können auch Gelatine-Rektalkapseln verwendet werden, die eine Kombination des Wirkstoffs mit einem Grundmassenstoff enthalten. Als Grundmassenstoffe kommen z.B. flüssige Triglyceride, Polyethylenglykole oder Paraffinkohlenwasserstoffe in Frage.
Zur parenteralen Verabreichung eignen sich in erster Linie wässrige Lösungen eines Wirkstoffs in wasserlöslicher Form, z. B. eines wasserlöslichen Salzes, ferner Suspensionen des Wirkstoffs, wie entsprechende ölige Injektionssuspensionen, wobei man geeignete lipophile Lösungsmittel oder Vehikel, wie fette Öle, z. B. Sesamöl, oder synthetische Fettsäureester, z. B. Ethyloleat oder Triglyceride, verwendet oder wässrige Injektionssuspensionen, welche viskositätserhöhende Stoffe, z.B. Natrium-carboxymethylcellulose, Sorbit und/ oder Dextran, und gegebenenfalls auch Stabilisatoren enthalten.
Die Dosierung des Wirkstoffs hängt von der Warmblüter-Spezies, dem Alter und dem individuellen Zustand sowie der Applikationsweise ab. Entsprechende pharmazeutische Präparate werden bevorzugt einmal pro Tag verabreicht.
So können beispielsweise Tagesdosen von kleiner gleich 40 mg p.o. eingesetzt werden. Insbesondere werden Tagesdosen von etwa 10 bis 0,5 mg, beispielsweise etwa 10 bis 4 mg, in erster Linie jedoch 3,5 bis 0,5 mg, besonders bevorzugt 2 mg, p.o. verabreicht.
Aufgrund einer Dreizentren-Doppelblindstudie mit mehr als 100 stationären Patienten kann als überraschend angesehen werden, dass bei Verabreichung einer Tagesdosis von 2 mg p.o. signifikante, mindestens ebenso potente anxiolytische Effekte erzielt wurden als bei Verabreichung einer Tagesdosis von 4 mg p.o. des erfindungsgemäss zu verwendenden Wirkstoffs.
Ferner wurde festgestellt, dass beispielsweise bei niedrigen Tagesdosen, wie 2 mg und 4 mg p.o., keine cardiovaskulären Änderungen aufgetreten sind.
Die nachfolgenden Beispiele illustrieren die oben beschriebene Erfindung und sollen die Bereitstellung einer typischen Applikationsform erläutern, jedoch keinesfalls die einzigen Ausführungs- und Applikationsformen darstellen.
Ausserdem soll die Erfindung durch die Beispiele in ihrem Umfang in keiner Weise eingeschränkt werden.

Die Bestandteile des Kerns werden vermischt, und das homogene Gemisch wird in Tabletten gepresst.
Die Tabletten werden mit dem Lack beschichtet, der aus den beschriebenen Bestandteilen besteht, die wiederum z. B.
in Wasser gelöst oder suspendiert wurden.
In analoger Weise wie in den Formulierungsbeispielen können Zusammensetzungen mit unterschiedlichem Wirkstoffgehalt hergestellt werden.

The goal of many efforts is to provide potent anxiolytics from the group of beta-receptor blockers for the treatment of anxiety, which, with the appropriate dosage, should practically not show any beta-receptor-blocking properties.
The use of 1 isopropylamino-3- [o- (1-pyrrolyl) phenoxy] -2-propanol or a pharmaceutically usable, non-toxic salt thereof has proven to be particularly advantageous.
Pharmaceutically acceptable salts of this active ingredient are its pharmaceutically acceptable acid addition salts, for example with strong inorganic acids, such as mineral acids, with sulfamic acids, such as cyclo-lower alkyl sulfamic acids, with strong organic carboxylic acids, such as lower alkane carboxylic acids, optionally unsaturated dicarboxylic acids or with hydroxy and / or oxo-substituted lower alkane carboxylic acids, or with sulfonic acids , such as lower alkane or optionally substituted benzenesulfonic acids, for example sulfates, phosphates, hydrohalides, such as hydrochlorides or hydrobromides, cyclohexyl sulfamates, acetates, malonates, oxalates, maleinates, fumarates, citrates, tartrates, pyruvates and sulfonates, in particular methanesulfonates, also benzene or p- Toluenesulfonates.

The characteristic anxiolytic profile of l-isopropylamino-3- [o- (1-pyrrolyl) phenoxy] -2-propanol results, for example, from the inhibition of overexcitement of rats, which are kept in isolation, measured by their locomotor and exploratory behavior, with maximum effects at a dose of 0.01-0.1 mg / kg po can be achieved [analogous to the methodology of R. Weinstock et al., Psychopharmacologia 30, 241ff. (1973).
Also in the Geller test [J. Geller, Psychopharmacologia, 3, pp. 374ff. (1962)], in which rats are exposed to a conflict situation induced by foot shock, l-isopropylamino-3- [o- (1-pyrrolyl) phenoxy] -2-propanol proves to be a strong anxiolytic. In this test, the feet of the experimental animals are periodically subjected to electric shocks when using a food release lever.
Usually, the response to the punishment is suppressed only when high doses of anxiolytics are administered. In a group of 29 rats receiving a dose of 20 mg / kg po.o. of 1-isopropylamino-3- [o-1-pyrrolyl) phenoxy] -2-propanol, 50% of the test animals already react with a marked increase in the lever operation frequency during the shock phase, the increase in individual animals being 300%.
In particular, significant anxiolytic effects were found when the drug was administered in social conflict situations in rhesus monkeys. The experimental basis for this social conflict test is the hierarchical ranking of rhesus monkeys. The relative rank status of an animal is the determining factor for inter-individual communication, e.g. B. for aggression or mutual social cleaning. Repeated administration of active substances in doses of 0.1-0.3 mg / kg po.o. in groups of 7 monkeys, there was a significant reduction in the fearful behavior pattern of the monkeys, which are ranked the lowest. They also actively contacted and became partners in care, even though they had never been care partners before. These effects were observed both in groups with a high level of social contact and in groups with little nursing partnerships.
Surprisingly, this pronounced anxiolytic profile was not found in the standard beta-receptor blockers oxprenolol and propranolol, or to a much lesser extent.
In the case of propranolol, the overexcitement of rats kept in isolation could only be achieved at a dose of 0.3 mg / kg p.o. maximally inhibited.
In the Geller foot shock test, a dosage of 20 mg / kg propranolol p. no significant change in behavior in rats.
Also in the behavioral pattern of the rhesus monkeys after administration of 0.3-3.0 mg / kg p.o. Oxprenolol no increase in social contacts of monkeys with low ranking are observed. At a dosage of 0.5-10.0 mg / kg po.o. Propranolol affected the social contacts of the rhesus monkeys with low rank status relatively little.
The active ingredient to be used according to the invention is also a potent selective antagonist of the presynaptic serotonin IB receptor (5-HTIB), while propranolol and oxprenolol act less selectively, since they also react with the postsynaptic 5-HT1A receptor.
Thus l-isopropylamino-3- [o- (l-pyrrolyl) phenoxy] -2-propanol has a pronounced and specific anxiolytic profile, for example because it reduces aggressive behavior and, above all, leads to an improvement in positive social behavior. The invention thus relates to the use of l-isopropylamino-3- [o- (l-pyrrolyl) phenoxy] -2-propanol or a pharmaceutically usable, non-toxic salt for the production of pharmaceutical preparations for the treatment of anxiety.
A preferred pharmaceutically usable salt is the corresponding hydrochloride.
The present invention also relates to pharmaceutical preparations for the treatment of anxiety states which contain 1-isopropylamino-3- [o- (l-pyrrolyl) phenoxy] -2-propanol or a pharmaceutically usable, non-toxic salt thereof as the active ingredient.
The pharmaceutical preparations according to the invention which contain the compound to be used according to the invention or pharmaceutically usable salts thereof are those for enteral, such as oral, further rectal, and parenteral administration to warm-blooded animals, the pharmacological active ingredient alone or together with a pharmaceutically acceptable carrier material is contained.
