Description

Die vorliegende Erfindung betrifft ein chirurgisches Implantat, welches zur Verankerung im Knochen vorgesehen ist und auf seiner Aussenwand umlaufende Erhöhungen als Verankerungshilfen aufweist.
Der Erfindungsgegenstand wird nachstehend insbesondere anhand künstlicher Hüftgelenke beschrieben.
Der künstliche Hüftgelenkersatz besteht normalerweise aus einer Hüftgelenkpfanne und einer Schaftprothese. Die Hüftgelenkpfanne wird im Acetabulum und die Schaftprothese im Oberschenkelknochen (Femur) fixiert.
Bei den bekannten Hüftgelenkpfannen, kurz auch Hüft-pfannen genannt, unterscheiden wir zwei Hauptgruppen:
a) Hüftpfannen, welche mit Knochenzement im knöchernen Acetabulum einzementiert werden.
b) Hüftpfannen, welche in ihrer Aussenform so ausgebildet sind, dass sie ohne Knochenzement fixiert werden können.
Die vorliegende Erfindung betrifft chirurgische Implantate, welche zementlos fixiert werden, d.h. beispielsweise eine Hüftpfanne der Gruppe b.
Es sind zementlose Hüftpfannen bekannt, welche an der Aussenfläche z. B. Gewindezüge, Verankerungsstifte usw. aufweisen. Bei allen bisher bekannten Hüftpfannen kommt es immer wieder zu sogenannten Lockerungen, welche eine weitere Operation notwendig machen. Es wird angenommen, dass zwischen dem natürlichen, elastischen Acetabulum und der künstlichen Hüftpfanne Spannungen entstehen, welche zu diesen Lockerungen führen.
Aufgabe der Erfindung ist die Schaffung eines im Knochen zu fixierenden Implantats, z.B. einer Hüftpfanne, bei welchem diese Lockerungen im Knochen verhindert werden können.
Das chirurgische Implantat der oben beschriebenen Art zeichnet sich erfindungsgemäss dadurch aus, dass die Erhöhungen in vorbestimmten Abständen durch Einkerbungen unterbrochen sind.
Die Erhöhungen sind dabei vorzugsweise in Form von Gewindezügen vorgesehen und die Einkerbungen sägezahn-artig ausgebildet. Damit haben die Erhöhungen selbst eine sägezahnartige Form.
Ausführungsbeispiele des Erfindungsgegenstandes werden nachstehend anhand der Zeichnung noch etwas näher erläutert. Es zeigt:
Fig. 1 eine erfindungsgemäss ausgerüstete Hüftgelenk-5 pfanne, teilweise im Schnitt,
Fig. 2 die Hüftpfanne nach Fig. 1 in Draufsicht,
Fig. 3 einen Längsschnitt entlang eines erfindungsgemäss ausgebildeten Gewindezuges,
Fig. 4 eine Draufsicht auf einen Sägezahn eines Gewin-10 dezuges, und
Fig. 5 einen Teilschnitt durch eine Variante einer Hüftgelenkpfanne.
Die nachfolgende Beschreibung bezieht sich auf eine 15 Hüftgelenkpfanne, eine besonders geeignete Anwendungsform der Erfindung, und beschreibt die wesentlichsten Details in der Reihenfolge der Figuren.
Die Pfanneninnenseite 1 ist in üblicher Weise ausgebildet. Auf dieser Fläche 1 bewegt sich die Kugel der Schaft-20 prothese (nicht dargestellt). Dank der Anschrägung 2 der Öffnung der Pfanne erhält die Schaftprothese zusätzliche Bewegungsfreiheit und verhindert das Luxieren der Kugel.
Auf der rechten Seite von Fig. 1 ist eine Ansicht der Aussenfläche gezeigt. Die gesamte Oberfläche besteht aus klei-25 nen, unsymmetrischen Pyramiden 3. Diese Pyramiden 3 liegen geordnet auf einer schraubenlinienförmigen Abwicklung am Pfannenumfang. In Eindrehrichtung gesehen, haben die Pyramiden 3 (Sägezähne) eine Fläche 8 mit einer Neigung zwischen 20 und 45 Grad (s. Fig. 3 bzw. 4). Die Rücken-30 Seiten 9 der Pyramiden - wiederum in Eindrehrichtung gesehen - fallen senkrecht, Richtung Pfannenzentrum ab.
Während dem Eindrehen der Hüftpfanne verdrängen die Flächen 8 und 10 den Knochen und verdichten denselben zusätzlich. Die senkrecht angeordnete Fläche 9 auf der Rük-35 kenseite der Pyramiden verhindert das Zurückdrehen der Hüftpfanne.
Die Pyramidenformen vergrössern gleichzeitig die Berührungsfläche zwischen Hüftpfanne und tragendem Knochen. Bei Belastung der Hüftpfanne wird dadurch der entstehende 40 Druck pro Flächeneinheit verringert, und es darf mit weniger Knochenabbau gerechnet werden.
Die Pyramidenformen stützen die Hüftpfanne im Knochen in allen Richtungen ab.
Der äussere Pfannenboden 4 ist konvex (s. Fig. 1). Diese 45 Form nähert sich der ursprünglichen Acetabulumform.
Beim Einsetzen der Hüftpfanne hilft diese konvexe Partie 4 mit, die Hüftpfanne richtig zu zentrieren und in die genaue Eindrehrichtung zu bringen.
Der Übergang von der konvexen Form zu den Pyrami-50 denflächen wird grösser als bei flacher Bodengestaltung. Dieser grössere Winkel verringert in dieser Partie die Spannungen auf den umliegenden Knochen bei eingesetzter Hüftpfanne.
Die tragende Aussenseite der Hüftpfanne mit den Pyra-55 miden kann sowohl zylindrisch als auch als Kegelstumpf 5 ausgebildet sein.
Bei der Form eines Kegelstumpfes ist der seitliche Konuswinkel üblicherweise kleiner als 20 Grad (anatomisch bedingt).
60 Der Winkel des Kegelstumpfes muss nicht identisch mit dem Winkel der Pyramidenspitzen sein, d.h. die Pyramiden II am oberen grossen Umfang können höher sein als die Pyramiden 1Y unten am Übergang zum konvexen Pfannenboden, wie Fig. 5 zeigt.
es Mit dieser Winkeldifferenz kann die Hüftpfanne am oberen Umfang stärker im Acetabulum verankert werden, ohne dass unten im Grund des Acetabulums eine höhere Keilwirkung entsteht, welche das Becken sprengen könnte.

Wie bereits beschrieben, sind die Pyramiden in der Eindrehrichtung schräg ansteigend ausgebildet, während die Rückseite auf das Pfannenzentrum hin steil abfällt.
Wenn die Hüftpfannen aus stark elastischem Werkstoff, z. B. Kunststoff, hergestellt werden, legen sich während dem Eindrehen die Pyramiden zurück. Bei Stillstand der Hüftpfanne werden die Pyramiden nach aussen aufgestellt und verhindern das Rückwärtsdrehen der Pfanne. Es entsteht durch diesen Effekt eine automatische Fixierung der Pfanne. Durch die Form der Pyramiden wirkt sich diese Fixation in allen Richtungen aus.
Wie Fig. 2 zeigt, befinden sich am oberen äusseren Pfannenrand zwei oder mehrere Ausschnitte 7 zum Einsetzen eines Eindrehwerkzeuges. Die Ausschnitte dürfen keinesfalls in die Anschrägung 2 (Fig. 1) hineinführen, damit während s dem Eindrehen keine Spannungen die runde innere Pfannenform deformieren können.
Die Ausschnitte können je nach Werkstoff der Hüftpfanne verschiedene Formen aufweisen.

Claims (6)

642 840 PATENTANSPRÜCHE
1. Chirurgisches Implantat, welches zur Verankerung im Knochen vorgesehen ist und auf seiner Aussenwand umlaufende Erhöhungen als Verankerungshilfen aufweist, dadurch gekennzeichnet, dass die Erhöhungen durch Einkerbungen unterbrochen sind.
2. Chirurgisches Implantat nach Anspruch 1, dadurch gekennzeichnet, dass die Erhöhungen als Gewindezüge ausgebildet sind.
3. Chirurgisches Implantat nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Einkerbungen als sägezahn-artige Unterbrechungen ausgebildet sind.
4. Chirurgisches Implantat, nach einem der Ansprüche 1—3, dadurch gekennzeichnet, dass seine Aussenwand einen rotationssymmetrischen Kegelstumpf bildet.
5. Chirurgisches Implantat nach Ansprüchen 2 und 3, dadurch gekennzeichnet, dass die ebenfalls sägezahnför-migen Erhöhungen die Schrägen in Eindrehrichtung vorn aufweisen und dass die Spitzen abgeflacht sind.
6. Chirurgisches Implantat nach einem der Ansprüche 1-5, dadurch gekennzeichnet, dass es als Hüftgelenkpfanne ausgebildet ist.
