Abstract

A tool for inserting screws into portions of a fractured jawbone, with a pair of jaws (4) carrying sleeves (9) which position and insert the two screws (10), has a further sleeve (12) attached to each jaw, alongside the first sleeve (9). This carries a sliding sleeve (17) with a spring (18) between which exerts pressure against an adjusting screw (19) inside so that a spindle (13) with a roller (16) on its end can be set a suitable distance from the point of insertion of the screw so that the tool can be manipulated to align the portion of jawbone at the correct angle for the insertion.

   Es ist bereits eine Kieferrepositionszange bekannt, deren Backen je mit einer Vorrichtung zum Festhalten einer in die Kieferbruchteile eingeschraubten Schraube versehen ist. Mit einer solchen Zange lassen sich die Kieferbruchteile gegeneinanderdrücken und in zusammengedrückter Stellung, aber auch in einer auseinanderliegenden Stellung festhalten. Es ist aber mit dieser Zange nicht möglich, die durch die Achsen der in die Kieferbruchteile eingeschraubten Schraube gegebene Lage dieser Kieferbruchteile, das heisst den Winkel zwischen den Zangenbacken und den Kieferbruchteilen, zu ändern und/oder in ganz bestimmter Weise festzuhalten. Der Lösung dieses Problemes dient nun die Kieferrepositionszange nach der vorliegenden Erfindung.

  Auch sie ist an jeder Backe mit einer Vorrichtung zum Festhalten einer in die Kieferbruchstücke eingeschraubten Schraube versehen und ist nun dadurch gekennzeichnet, dass neben jeder dieser beiden Festhaltevorrichtungen eine um sie verschwenkbare und fixierbare, in der Richtung der Backenachse verstellbare Abstützvorrichtung vorhanden ist.



   Nachfolgend wird anhand einer Zeichnung ein Ausführungsbeispiel der Erfindung beschrieben. In der Zeichnung zeigen:
Fig. 1 eine Ansicht der ganzen Zange,
Fig. 2 einen Ausschnitt davon in grösserem Massstab.



   Die in der Fig. 1 dargestellte Kieferrepositionszange weist zwei Handgriffe 1 und 2 und zwei Backen 3 und 4 auf, wobei der Handgriff 1 und die Backe 3 einerseits über den Gelenkbolzen 5 mit dem Handgriff 2 und der Backe 4 anderseits gelenkig verbunden sind. Mit einer Klemmvorrichtung 6 kann der am Handgriff 1 angelenkte Lenker 7 festgeklemmt und damit der   Öffnungswinkel    der Zange fixiert werden. Auf jedem der beiden zylindrischen Backenenden 8 sitzt eine Hülse 9, die zum Festhalten einer in ein Kieferfragment eingeschraubten Schraube 10 dient. Die Hülse 5 ihrerseits wird von einer Überwurfmutter 11 festgehalten, die sich an einem Bund 8a der Backe 8 abstützt. Zangen mit diesen Bestandteilen sind bekannt und bei den Kieferchirurgen in Gebrauch.



  Bei der Zange nach der vorliegenden Erfindung ist nun an jeder der frei drehbaren Hülsen 9, die sich durch Anziehen der Überwurfmutter 11 fixieren lassen, eine weitere Hülse 12 befestigt. in welcher sich ein in Richtung der Backenachse 14 verschiebbarer und durch eine Schraube 15 gegen Verdrehung gesicherter Stift 13 befindet. Diese Schraube 15 gewährleistet durch den Schlitz 12a in der Hülse 12 hindurch eine feste Verbindung zur Hülse 17, an deren unterem Ende sich die Feder 18 abstützt, deren anderes Ende auf einer Schulter 12b der Hülse 12 aufsteht. Durch diese Feder 18 wird der Stift 13 also gegen die Handgriffe 1 und 2 hingezogen.

  In dem mit 12c bezeichneten Innengewinde am Ende der Hülse 12 ist eine mit einem Rändelrad 19a versehene Schraube 19 eingeschraubt, die dazu dient, den Stift 13, der an seinem freien Ende mit einer Abstützrolle 16 versehen ist, entgegen der Kraft der Feder 18 in Richtung von den Handgriffen 1 und 2 weg zu verschieben.



   Bei der vorstehend beschriebenen Zange ist es nun möglich, mittels der Abstützvorrichtung 13/16 die Ebene eines Kieferfragmentes, das durch die Schraube 10 an einer Zangenbacke befestigt ist, mittels der Abstützvorrichtung 13/16, relativ zur Backenachse 14 zu neigen, und zwar genau in der gewünschten Richtung und im gewünschten Mass, indem man zuerst die Abstützvorrichtung 13/16 um die Achse 14 in die richtige Lage verschwenkt und sie dann in dieser Lage durch Anziehen der Überwurfmutter 11 fixiert, worauf man durch Drehen der Schraube 19 die Kippbewegung ausübt. Wenn dieses Vorgehen bei beiden Zangenbacken durchgeführt wird, können die Kieferfragmente in jeder gewünschten Lage zusammengefügt werden.

The solution to this problem is now provided by the jaw reduction forceps according to the present invention.

  It is also provided on each jaw with a device for holding a screw screwed into the jaw fragments and is now characterized in that next to each of these two holding devices there is a support device which can be pivoted and fixed around it and is adjustable in the direction of the jaw axis.



   An exemplary embodiment of the invention is described below with reference to a drawing. In the drawing show:

   The jaw reduction forceps shown in Fig. 1 has two handles 1 and 2 and two jaws 3 and 4, the handle 1 and the jaw 3 being hinged on the one hand via the hinge pin 5 to the handle 2 and the jaw 4 on the other hand. With a clamping device 6, the link 7 articulated on the handle 1 can be clamped and the opening angle of the pliers can be fixed. On each of the two cylindrical jaw ends 8 there is a sleeve 9 which is used to hold a screw 10 screwed into a jaw fragment. The sleeve 5 in turn is held in place by a union nut 11 which is supported on a collar 8a of the jaw 8. Forceps with these components are known and are used by oral surgeons.



  In the pliers according to the present invention, a further sleeve 12 is now attached to each of the freely rotatable sleeves 9, which can be fixed by tightening the union nut 11. in which there is a pin 13 which is displaceable in the direction of the jaw axis 14 and secured against rotation by a screw 15. This screw 15 ensures, through the slot 12a in the sleeve 12, a firm connection to the sleeve 17, at the lower end of which the spring 18 is supported, the other end of which rests on a shoulder 12b of the sleeve 12. The pin 13 is thus drawn towards the handles 1 and 2 by this spring 18.

  A screw 19 provided with a knurled wheel 19a is screwed into the internal thread designated by 12c at the end of the sleeve 12, which screw serves to push the pin 13, which is provided at its free end with a support roller 16, against the force of the spring 18 in the direction to move away from handles 1 and 2.



   In the case of the pliers described above, it is now possible, by means of the support device 13/16, to incline the plane of a jaw fragment, which is attached to a pliers jaw by the screw 10, relative to the jaw axis 14 by means of the support device 13/16, precisely in the desired direction and to the desired extent by first pivoting the support device 13/16 about the axis 14 into the correct position and then fixing it in this position by tightening the union nut 11, whereupon the tilting movement is exercised by turning the screw 19. If this procedure is carried out with both forceps jaws, the jaw fragments can be joined together in any desired position.
