Description
Hüftstützschiene
Die Erfindung betrifft eine Hüftstützschiene insbesondere zur Behandlung angeborener Hüftschäden und Dysplasia bei Kindern.
Eine Hüftstützschiene dient dazu, den Oberschenkel so zu stützen, dass der Druck des Gelenkkopfes gegen den äusseren oberen Rand der flachen Gelenkpfanne ausgeübt wird. Die vorliegende Erfindung bezweckt eine Hüftschiene so auszubilden, dass sie die erwähnte Aufgabe wirksam erfüllt, wobei der Winkel, die Grösse der Kraft und die Lage der Schiene überwacht und reguliert werden können.
Ferner bezweckt die Erfindung eine Hüftstützschiene so auszubilden, dass sie eine bessere seitliche Stabilität besitzt und schnell am Kinde angebracht oder entfernt werden kann. Die erfindungsgemässe Schiene kann leicht gewaschen werden, ist hygienisch und verhältnismässig bequem.
Ferner bezweckt die Erfindung die Schiene für Kinder so auszubilden, dass sie über oder unter den Windeln befestigt werden kann, wobei die Konstruktion einfach und billig ist.
Weitere Vorteile ergeben sich aus den Einzelheiten eines Ausführungsbeispieles, das anhand der beigefügten Zeichnung beschrieben ist. Es zeigt:
Fig. 1 eine Vorderansicht der Hüftstützschiene gemäss der Erfindung, bei der die Befestigungsmittel für die rechte Oberschenkel-Manschette in geöffnetem Zustand dargestellt sind, während die linke Oberschenkel-Manschette geschlossen ist.
Fig. 2 eine Ansicht der Hüftstützschiene von hinten, bei der die rechte innere Oberschenkel-Manschetten-Zunge nach aussen gebogen ist und die Befestigungsmittel dieser Manschette offen sind.
Die Hüftstützschiene gemäss der Erfindung weist ein Paar die Oberschenkel umschliessende Manschetten 10 und 12 auf. Die Manschetten 10 und 12 sind vorzugsweise aus leichten, biegsamen, nicht giftigen Polyäthylene Schichten hergestellt und die Kanten sind mit Polstern 14 versehen, die an die Manschettenkanten angenäht sind. Die Manschetten weisen eine rohrförmige Gestalt auf, welche durch vordere und hintere Träger gehalten werden, wobei jede Manschette eine obere Zutrittsöffnung aufweist, welche durch die Enden 16 und 18 der Manschette begrenzt ist. Die Polster 14 sind ebenfalls an die Enden der Manschetten angenäht. Diese Manschetten-Polster 14 weisen vorzugsweise Schaumgummi oder ähnliches Material auf, das in eine weiche Hülle z. B. aus synthetischem Leder eingewickelt ist, das nicht feuchtigkeitsdurchlässig ist.
Eine innere Zunge 20 aus weichem biegsamem Material ist mit einem Ende an der Innenseite der Manschetten neben dem Ende 16 derselben angenäht und ist so lang, dass sie die Innenseite der Manschetten-Enden überbrückt und an den Schenkeln des Kindes anliegt. Vorzugsweise wird zum Geschlossenhalten der Manschetten um die Schenkel des Kindes ein biegsames Band 22 verwendet, dessen untere Seite 24 eine locker gewobene sehr reissfeste Faser aufweist. Das Band 22 ist durch einen Saum 26 am Ende 16 der Manschette angenäht. Das Manschettenende 18 ist an seiner Aussenseite mit einem Streifen 28 versehen, der starre Schleifen aufweist, welche in die Fasern des Bandes 22 eingreifen, wenn das Band gegen den Streifen gepresst wird, damit der Streifen das Band geschlossen hält.
Diese Befestigungsvorrichtung ist unter dem Markennamen Velcro > im Handel erhältlich und gehört nicht zur Erfindung, Nachdem die Beine des Kindes in die Manschetten gelegt wurden, wird die innere Zunge 20 um die Beine zwischen die Innenseite der Manschette gewikkelt und überbrückt die Enden derselben. Danach werden die Manschetten gegen die Beine des Kindes gedrückt und abgemessen und das Band 22 wird gegen den Streifen 28 gedrückt, der die Manschetten um die Beine des Kindes festhält. Die Zunge 20 verhindert eine Berührung der Haut des Kindes mit der Unterseite 24 des Bandes 22 und verhindert eine Reizung.
Der Abstand zwischen den Manschetten 10 und 12 und der Stützwinkel wird durch den vorderen Träger 30 und den hinteren Träger 32 (Fig. 1 und 2) gebildet. Die Träger 30 und 32 bestehen beide aus starren Metallstreifen 34 und 36 bzw. 34' und 36'.
Die Streifen 34 und 34' sind mittels Befestigungselementen z. B. Schrauben 38 und 38' an der Manschette 10 befestigt und die Streifen 36 und 36' sind mittels gleicher Befestigungselemente 40 und 40' an der Manschette 12 befestigt. Die Streifen 34 und 36 sowie 34' und 36', sind mit einer Anzahl in einer Reihe im Abstand voneinander angeordneter Löcher 42 versehen, so dass beim Aufeinanderlegen der Streifen die Löcher zueinander ausgerichtet werden können und die Länge der Träger eingestellt werden kann und somit der Abstand zwischen den Manschetten 10 und 12. Nach dem Einstellen des gewünschten Abstandes zwischen den Manschetten 10 und 12, werden Befestigungselemente z. B.
Schrauben 44 durch ein Paar der ausgerichteten Löcher 42 hindurchgesteckt, so dass die Länge der Träger festgehalten wird. Vorzugsweise sind die Streifen der Träger 30 und 32 aus Aluminium hergestellt.
Um eine seitliche Stabilität der Stützschiene am Kinde zu gewährleisten, ist eine l-förmige hintere Stütze 46 aus halbstarren Kunststoffschichten vorgesehen, die an dem hinteren Träger 32 mittels Schrauben 44 befestigt ist und die senkrecht über den Träger 32 hinausragt. Die untere Partie 48 der l-förmi- gen Stütze 46 ist ebenfalls mit einer Anzahl Löcher versehen (nicht sichtbar), welche von gleicher Grösse und in gleichen Abständen angeordnet sind, wie die Löcher 42 der hinteren Streifen 34' und 36', so dass die hintere Stütze 46 an den hinteren Träger 32 in jeder Stellung desselben angeschraubt werden kann.
Bei einer handelsüblichen Ausführung ist die hintere Stütze durch ein sich deckendes Paar durchsichtiger Polyäthylen Schichten gebildet, indem die beiden Schichten zusammengenäht sind. Ein verhältnismässig steifer Stab 50 ist zwischen die beiden Schichten eingenäht und erhöht die Steifigkeit der senkrechten Partie 52 der hinteren Stütze.
Ein den Brustkasten umfassender Gürtel ist an der oberen Partie 54 der hinteren Stütze 46 befestigt.
Der Brustgürtel besteht aus einem Paar Riemen 56 und 58. Das eine Ende des Riemens 56 ist an der hinteren Stütze angenäht und das andere Ende ist mit einer einstellbaren Schnalle 60 versehen, wie in Fig. 1 dargestellt. Die einstellbare Schnalle 60 kann in ihrer Lage gegenüber dem Riemen 56 verändert werden und eine Klammer 62, die an einem Ende des Riemens 58 befestigt ist, kann lösbar an die Schnalle
60 angehängt werden. Das andere Ende des Riemens 58 ist an der oberen Partie 54 der hinteren Stütze 46 befestigt.
Die Einstellbarkeit des Stützwinkels der Schiene wird durch Biegen der hinteren und vorderen Träger erreicht. Dieses Biegen des vorderen Trägers verteilt sich gewöhnlich über den ganzen Träger zwischen den Manschetten 10 und 12. Das Biegen des hinteren Trägers erfolgt vorzugsweise in der Mittelpartie des Trägers zwischen den beiden Manschetten und der unteren Partie 48 der hinteren Stütze.
Das Anbringen der beschriebenen Hüftstützschiene geschieht wie folgt. Durch Öffnen der Bänder 22 und durch Wegziehen der Zungen 20 von den Enden 18 der Manschetten 10 und 12, können des Kindes rechter und linker Oberschenkel in die Manschetten 10 bzw. 12 eingelegt werden. Vor dem Anbringen ermöglicht ein Messen des Kindes die Manschetten auf den gewünschten Abstand einzustellen, so dass die Stützspannung reguliert werden kann. Der Abstand der Manschetten soll so gewählt werden, dass die äussere gepolsterte Kante der Manschetten sich im Bereich des Knies befindet, aber nicht so dicht beim Kniegelenk, dass ein Biegen des Unterschenkels behindert wird.
Nachdem die Oberschenkel des Kindes sich in den Manschetten befinden, werden die Zungen 20 unter die Manschettenenden geschoben und mit den Bändern 22 werden die Manschetten geschlossen, und dank dem Eingriff des Streifens 28 in die Fasern des Bandes 28 geschlossen gehalten.
Die Partie 52 der hinteren Stütze liegt am Rückgrat des Kindes an und die Brustriemen 56 und 58 sind so eingestellt, dass sie den Brustkasten des Kindes eng umfassen und die hintere Stütze in der gewünschten Lage halten.
Dank der Verwendung von Kunststoff, Aluminium, waschbaren Gewebebändern und Zungen kann die Stützschiene leicht hygienisch sauber gehalten werden. Da die Manschetten 10 und 12 nur einen Teil der Oberschenkel des Kindes umfassen, wird ein oberhitze des Kindes in der Gegend der Windeln vermieden und die Verwendung der Bänder 22 und der Schnalle 60 ermöglicht es, die Stützschiene schnell zum Wechseln der Windeln vom Kinde zu entfernen und wieder zu befestigen. Die hintere Stütze und der Brustriemen erlauben volle Biegung des Rückens und gewährleisten, dass die Stützschiene in der gewünschten Lage bleibt.
Hip support splint
The invention relates to a hip support splint, in particular for the treatment of congenital hip damage and dysplasia in children.
A hip support splint is used to support the thigh in such a way that the pressure of the joint head is exerted against the outer upper edge of the flat joint socket. The present invention aims to design a hip splint in such a way that it effectively fulfills the mentioned object, whereby the angle, the magnitude of the force and the position of the splint can be monitored and regulated.
Furthermore, the invention aims to design a hip support splint in such a way that it has better lateral stability and can be quickly attached to or removed from the child. The splint according to the invention can be washed easily, is hygienic and relatively comfortable.
Another object of the invention is to make the splint for children so that it can be fastened over or under the diaper, the construction being simple and inexpensive.
Further advantages emerge from the details of an exemplary embodiment which is described with reference to the accompanying drawing. It shows:
1 shows a front view of the hip support splint according to the invention, in which the fastening means for the right thigh cuff are shown in the open state, while the left thigh cuff is closed.
2 shows a view of the hip support splint from the rear, in which the right inner thigh cuff tongue is bent outwards and the fastening means of this cuff are open.
The hip support splint according to the invention has a pair of cuffs 10 and 12 surrounding the thighs. The cuffs 10 and 12 are preferably made of lightweight, flexible, non-toxic polyethylene layers and the edges are provided with pads 14 which are sewn to the cuff edges. The cuffs are tubular in shape which are supported by front and rear supports, each cuff having an upper access opening defined by ends 16 and 18 of the cuff. The pads 14 are also sewn to the ends of the sleeves. These cuff pads 14 preferably have foam rubber or similar material that is z. B. is wrapped in synthetic leather that is not permeable to moisture.
An inner tongue 20 of soft, flexible material is sewn at one end to the inside of the cuffs next to the end 16 of the same and is long enough to bridge the inside of the cuff ends and lie against the thighs of the child. Preferably, a flexible band 22 is used to keep the cuffs closed around the child's thighs, the lower side 24 of which has a loosely woven, very tear-resistant fiber. The band 22 is sewn through a seam 26 at the end 16 of the cuff. The cuff end 18 is provided on its outside with a strip 28 which has rigid loops which engage the fibers of the tape 22 when the tape is pressed against the strip so that the strip holds the tape closed.
This fastening device is commercially available under the brand name Velcro> and does not form part of the invention. After the child's legs have been placed in the cuffs, the inner tongue 20 is wrapped around the legs between the inside of the cuff and bridges the ends of the same. The cuffs are then pressed against the child's legs and measured, and the tape 22 is pressed against the strip 28 which holds the cuffs around the child's legs. The tongue 20 prevents the child's skin from touching the underside 24 of the band 22 and prevents irritation.
The distance between the cuffs 10 and 12 and the support bracket is formed by the front support 30 and the rear support 32 (FIGS. 1 and 2). The supports 30 and 32 are both made of rigid metal strips 34 and 36 and 34 'and 36', respectively.
The strips 34 and 34 'are secured by means of fasteners, for. B. screws 38 and 38 'attached to the sleeve 10 and the strips 36 and 36' are attached to the sleeve 12 by means of the same fasteners 40 and 40 '. The strips 34 and 36 as well as 34 'and 36' are provided with a number of holes 42 arranged in a row at a distance from one another, so that when the strips are placed on top of one another, the holes can be aligned with one another and the length of the carrier can be adjusted and thus the Distance between the cuffs 10 and 12. After setting the desired distance between the cuffs 10 and 12, fasteners z. B.
Screws 44 passed through a pair of aligned holes 42 so that the length of the beams is held in place. Preferably, the strips of supports 30 and 32 are made from aluminum.
In order to ensure lateral stability of the support rail on the child, an L-shaped rear support 46 made of semi-rigid plastic layers is provided, which is fastened to the rear support 32 by means of screws 44 and which protrudes perpendicularly beyond the support 32. The lower part 48 of the L-shaped support 46 is also provided with a number of holes (not visible) which are of the same size and at the same intervals as the holes 42 of the rear strips 34 'and 36', so that the rear support 46 can be screwed to the rear bracket 32 in any position thereof.
In a commercial version, the rear support is formed by an overlapping pair of clear polyethylene layers by sewing the two layers together. A relatively stiff rod 50 is sewn between the two layers and increases the stiffness of the vertical portion 52 of the rear support.
A belt encompassing the chest is attached to the upper portion 54 of the rear support 46.
The chest belt consists of a pair of straps 56 and 58. One end of the strap 56 is sewn to the rear support and the other end is provided with an adjustable buckle 60 as shown in FIG. The adjustable buckle 60 can be repositioned relative to the strap 56 and a clip 62 attached to one end of the strap 58 is releasable on the buckle
60 can be appended. The other end of the strap 58 is attached to the upper portion 54 of the rear support 46.
The adjustability of the support angle of the rail is achieved by bending the rear and front beams. This bending of the front beam is usually distributed throughout the beam between the cuffs 10 and 12. The bending of the rear beam is preferably carried out in the central portion of the beam between the two cuffs and the lower portion 48 of the rear support.
The hip support splint described is attached as follows. By opening the straps 22 and pulling the tongues 20 away from the ends 18 of the cuffs 10 and 12, the child's right and left thighs can be placed in the cuffs 10 and 12, respectively. Before attachment, the child can measure the cuffs to the desired distance so that the support tension can be regulated. The distance between the cuffs should be chosen so that the outer padded edge of the cuffs is in the area of the knee, but not so close to the knee joint that bending of the lower leg is hindered.
After the child's thighs are in the cuffs, the tongues 20 are pushed under the cuff ends and the cuffs are closed with the straps 22 and kept closed thanks to the engagement of the strip 28 in the fibers of the strap 28.
The portion 52 of the rear support rests against the child's spine and the chest straps 56 and 58 are adjusted to tightly encircle the child's chest and hold the rear support in the desired position.
Thanks to the use of plastic, aluminum, washable fabric tapes and tongues, the support rail can easily be kept hygienically clean. Since the cuffs 10 and 12 encompass only part of the child's thighs, overheating of the child in the area of the diaper is avoided and the use of the straps 22 and the buckle 60 enables the support rail to be removed quickly from the child for changing diapers and reattach it. The rear brace and chest strap allow the back to flex fully and ensure that the brace remains in the desired position.
