Appareil médical pour le traitement de troubles circulatoires.
L'invention a pour objet un appareil médical pour le traitement de troubles eirculatoires, destiné à apporter, dans des conditions d'activité que ne parviennent pas à atteindre les méthodes utilisées à l'heure actuelle, un remède à la fois efficace et durable à certains troubles circulatoires imputables tant à une déficience d'ordre fonctionnel qu'à des causes accidentelles diverses et trouvant une adaptation immédiate dans le traitement d'affeetions latentes ou passagères telles que cellulite, varices, suites d'entorses ou de fractures, fatigue ou déchirures musculaires résultant d'exercices violents, ou autres états anormaux de nature analogue.
L'appareil faisant l'objet de l'invention est destiné à faire subir à la partie du corps affectée une série de compressions contrôlables amorcées au niveau extrême de la zone traitée et progressivement étendues en direction du coeur et des poumons, chacune de ces compressions étant suivie d'une décompres- sion avec laquelle elle constitue un cycle complet élémentaire du traitement, qui sera répété autant de fois que le nécessite l'état du patient.
Il est caractérisé en ce qu'il comprend une enceinte pneumatique destinée à embrasser, par l'une de ses faces, la région affectée et dont la face opposée est contenue par une enveloppe sensiblement inextensible à même d'être convenablement assujettie à la partie intéressée du corps du patient, ladite enceinte étant à même d'être alternativement gonflée en progressant suivant un itinéraire et une direction déterminés, puis dégonflée, par manoeuvres successives d'un distributeur relié, d'autre part, à un générateur de fluide sous pression.
L'appareil a ainsi pour but d'as silrer, dans le sens opportun, la propagation continue d'une compression d'étendue croissante propre à susciter une expression progressive des résidus toxiques contenus dans les tissus, compression de laquelle il est, au surplus, apte à contrôler le degré de pénétration en profondeur.
Il sera d'abord rappelé quelques notions anatomiquessuccinctesconcernant,àtitre d'exemple, d'un cas maladif typique, la genèse de la manifestation de la cellulite.
Le corps humain présente un triple réseau circulatoire : artériel, veineux et lymphatique, dont le troisième aecuse une importance considérable en raison de ce qu'il contribue, dans une très large part, à garantir l'élimination des déchets de l'organisme, tels, notamment, que le C02, dont l'effort musculaire charge-con- eurremment le sang et la lymphe.
Cette dernière se localise et circule autour des muscles et, notoirement, dans le tissu con jonetif embrassant les différents organes, en sorte que peut être envisagée, d'une façon on élé- mentaire, 1'existence d'un véritable réservoir conjonctive-lymphatique.
Tout trouble circulatoire se manifestant dans cette zone particulière donne aussitôt naissance à son engorgement qui provoque, a son tour, une irritation, puis une inflamma- tion dont découle l'affection couramment désignée sous le terme de cellulite .
Or, la masse des produits toxiques dérivant de leur accumulation dans le tissu conjonetif se trouve, en cette occurence, bloquée au sein de celui-ci sous l'influence rétentive qu'exerce spontanément, au moindre choc sur 1'épiderme ou par suite d'une excitation ré flexe quelconque, le système vagosympathique.
De ces constatations, il appert que doivent etre, dès lors, rigoureusement proscrits les massages manuels forts, auxquels il est, d'ordi- naire recouru dans le but d'opérer l'élimination artificielle des déchets résiduels considé- rés ; par ailleurs, les actions superficielles à même intention consistant en des affleure ments, attouchements, pincements et autres contacts comparables ne peuvent conduire qu'à des résultats pratiquement inappré- ciables, en vertu du fait qu'aussi bien exécu- tés soient-ils, ils déclenchent, au niveau de la peau, des phénomènes de crispation plus nocifs qu'utiles.
L'appareil proposé a pour but de permettre, au contraire, de faire subir soit à la totalité, soit à une portion fractionnelle d'un membre ou éventuellement encore à toute autre région déficiente du corps, une compression enveloppante de puissance absolument contrôlable, à laquelle est progressivement imprimée une expansion dirigée vers le coeur et les poumons, réussissant à solliciter, eonjoin tement à une activation intensive de la eirculation veineuse de retour, un vidage effectif du réservoir conjonctivo-lymphatique entralnant un assouplissement notable des muscles, sans que soit, néanmoins, en rien contrarié le régime du flux artériel.
Le dessin annexé représente, à titre d'exemple, deux formes d'exécution de l'appareil faisant l'objet de l'invention.
La fig. 1 est une vue de l'ensemble d'une première forme d'exécution de l'appareil spé- eialement conditionnée en vue de la réception d'une jambe, offrant certains arrachements conventionnels propices à la conception de sa structure.
La fig. 2 montre, par une vue en coupe longitudinale, à plus grande échelle, le détail du dispositif pneumatique de l'appareil représenté à la fig. 1.
La fig. 3 représente schématiquement la vue en plan d'une deuxième forme d'exécu- tion de l'appareil destinée à la zone dorsale.
Enfin, la fig. 4 est une vue en coupe transversale effectuée par i--A de la précé- dente.
L'appareil représenté aux fig. 1 et 2 comprend une enceinte pneumatique constituée par une chambre tubulaire 1, de longueur et section adéquates, établie en une matière plastique souple telle que du caoutchouc, affectant, à l'état vide, l'allure aplatie d'un ruban et des deux extrémités obturées de laquelle émergent transversalement deux ajutages 2 et 3, de même nature, qui communiquent avec la capacité de cette dernière.
Cette chambre 1, initialement enroulée en hélice autour de la jambe, simulée en 4, à la manière d'une bande accusant, d'une spire à la suivante, un certain recouvrement 5, est ensuite enveloppée d'une gaine sensiblement inextensible 6'confectionnée, par exemple, en une toile de qualité appropriée, galbée à la demande de la conformation moyenne du membre considéré et dont les lisières de jonction 7 sont pourvues de crochets 8 préposés à la réception d'un lacet 9'permettant, grâce à la faculté de réglage de sa tension, d'imposer à ladite gaine 6 un serrage de puissance variable qu'il est ainsi loisible d'accroître ou de relâcher à volonté en certains points judi- cieusement localisés.
Aux deux ajutages 2 et 3 viennent s'accou- pler, par des raccords 10, deux canalisations flexibles 11 et 12 accédant individuellement aux embouts 13 et 14, perpendiculairement orientés, d'un distributeur 15 consistant en un robinet à quatre voies, dont le boisseau tournant 16, percé d'une lumière diamétrale 17, est manoeuvrable à l'aide d'une poignée d'actionnement 18.
Des deux embouts complémentaires de ce distributeur, respectivement opposés aux précités 14 et 13, le premier, 19, débouche direc- tement dans l'atmosphère, tandis que le second, 20, est connecté, au moyen de deux conduites souples 21 et 22, entre lesquelles s'in terpose un élément chauffant 23 utilisable au gré du praticient, au mano-détendeur 24 d'un générateur de fluide sous pression sous la forme cl'une bouteille portative d'air comprimé 25.
L'élément chauffant 23 est constitué d'un tronçon de tube rigide autour duquel est aménagée une résistance électrique alimentée par un conducteur 26, reliable à une prise de cou- rant.
Enfin, sur un piquage 27 de l'embout 13 du distributeur est branché un manomètre 28, convenablement gradué, susceptible de déceler à tout instant la pression régnant dans la chambre pneumatique 1.
L'appareil peut être, en outre, complété par une soupape de décharge inviolable, nonfigurée, à office de limiteur de pression, opportunément tarée et installée sur le circuit, jouant le rôle d'un dispositif de sécurité propre à pallier tous risques d'accidents éven- tuels.
Ceci posé, l'utilisation et le fonctionnement de l'appareil décrit sont alors les suivants :
La poignée d'actionnement 18 du distributeur 15 étant préalablement placée à la situation moyenne 18a de son débattement, en vertu de laquelle la lumière du boisseau 16 occupe l'orientation oblique 17a répondant à l'obturation simultanée de tous les circuits, le praticien, après avoir mesuré, à l'extrémité du membre envisagé, la tension artérielle du patient, règle le mano-détendeur 24 en sorte que l'air débité, à son issue, par la bouteille 25 manifeste la pression convenant aux sujé- tions caractéristiques du cas à traiter.
Il amène ensuite cette poignée 18 à la position 18"tracée en traits pleins, où la lumière ci-avant évoquée, s'alignant, en 17, avec les orifices des embouts 20 et 13, autorise l'admission du fluide acheminé par les conduites 21. et 22 et le tube axial d'intercommunication de l'élément chauffant 23, dans la canalisa. tion 11 aboutissant à l'ajutage d'entrée 2 de la chambre pneumatique 1.
Ainsi que 1'explicitent conjointement les figures d'ensemble 1 et de détail 2, ce fluide s'épanche, dès lors, dans ladite chambre 1, le long du parcours hélicoïdal de laquelle il progresse et dont l'expansion volumétrique, per ceptible en 29, que l'inextensibilité de la gaine 6, la contenant, astreint à s'effectuer en direction du centre, crée une compression évoluant vers la racine du membre et d'intensité constamment contrôlable par simple lecture du manomètre 28.
Lorsque le remplissage de cette chambre 1 est accompli, l'opérateur fait osciller la poignée 18 de 18e en 18b, imprimant au boisseau 16 une rotation de 90 degrés à la suite de laquelle la lumière 17 vient, eii 1-7b, se situer en regard des embouts 14 et 19 du distribu- teur, réalisant la mise à l'air libre de la canal-sation d'éehappement 12 qui réunit à ce dernier l'ajutage d'évacuation 3.
Faisant, le cas échéant, entrer en jeu l'élé ment chauffant 23 dans le but d'obtenir concurremment une action thermique subsidiaire. il répète ce cycle double de gonflage et dé- gonflage consécutifs autant de fois que l'exige l'état du membre affecté, déclenchant, de la sorte, un train d'ondes successives de compression aptes à refouler les résidus toxiques amoncelés dans le réservoir conjonctivo-lymphatique vers les poumons où leur élimination par combustion peut être encore avantageu- sement favorisée grâce à une oxygénation lente auxiliaire par inhalation, propice à accroître l'efficacité du résultat et dont dé- rive, en outre, une disparition accélérée de la fatigue ressentie.
Du point de vue médical, il peut être intéressant que soit réservé le loisir de discipliner et régir le mouvement de l'onde compressive en faisant varier, à la demande, sa vitesse de propagation soit tout le long de ce membre, soit en certaines seulement de ses fractions.
Une solution très simple satisfaisant à ce pro blèmeconsisteàutiliserunechambretubu- laire souple comportant une paroi externe renforcée, laquelle, accusant, de ce fait, une déformabilité restreinte, est susceptible, en fonction du degré de tension qui lui est imposé au cours de l'enroulement initial, de freiner plus ou moins la progression du fluide de remplissage.
En variante, il pourrait, d'ailleurs, dans le même objectif, être conservé l'usage d'une chambre d'épaisseur uniformément mince sur laquelle serait apposée une bande élastique supplémentaire s'immisçant entre elle et la gaine inextensible et apte à conférer la même possibilité de réglage.
Il est aisé de concevoir qu'il ne puisse être envisagé de procéder impunément à un tel enrobage de toutes les parties du corps et, notamment, de l'abdomen ou du con où les plans résistants font défaut, cependant encore qu'il procurerait, sur la poitrine elle-même, une impression de striction fort désagréable.
Pour ce motif, il a été prévu d'établir différents modèles d'appareils qui s'accordent respectivement à ces diverses applications. La deuxième forme d'exécution représentée aux fig. 3 et 4 est spécialement adaptée au traitement de la région dorsale. Elle comprend une armature rigide 30, confectionnée en toute matière appropriée, limitée inférieurement par une face d'assise plane 31 et supérieurement par une paroi concave 32, de courbure adéquate, sur laquelle vient reposer une enceinte souple de contour rectangulaire 33, dont la structure rappelle celle d'un matelas pneumatique.
Cette enceinte 33, qui se substitue à la chambre 1 du dispositif antéeédemment dé- taillé et est pourvu, comme elle, de deux ajutages d'entrée 34 et de sortie 35, comporte, a ce titre, une série d'alvéoles oblongs 36, adja-. cents et parallèles, orientés, au montage, normalement aux génératrices de la paroi 32 et que font communiquer entre eux des orifices 37, judicieusement calibrés, par la voie. desquels lesdits alvéoles se gonflent, à tour de rôle, dans le sens concrétisé par la flèche 38.
A ces ajutages 34 et 35 sont destinées à se rattacher les deux canalisations d'alimen- t-ation et d'évacuation, antérieurement dé- signéeS sous repères 11 et 1), parvenant au distributeur d'un groupe générateur de pression identique à celui qui vient d'être exposé.
Enfin, sur les flancs latéraux 39 et 40 de l'armature 30 sont fixées un certain nombre de sangles mutuellement, assemblables 41 et 42, se prêtant à l'assujettissement du corps du sujet sur l'appareil et qui, en raison de la faculté de réglage de leur tension individuelle, remplissent un office en tous points assimilable à celui de l'équipement que constituaient, dans l'exemple primitif, la gaine 1 et son lacet de fermeture 9.
Il appert que, sous réserve de modifier à la demande la position du patient, ce même instrument peut indifféremment servir à une intervention sur la zone abdominale ou encore sur les hanches.
Lorsqu'il s'agira, par ailleurs, de traiter la région du eou, il sera fait usage d'un appareil analogue au précédent et n'en différant que par le fait que l'armature indéformable 30 sera, aux fins d'éviter la sensation de strangulation que ne laisserait de procurer la rigidité d'un tel élément, remplacée par une plaque de caoutchouc souple propre à s'adapter spontanément à la configuration personnelle de l'individu.

The subject of the invention is a medical device for the treatment of circulatory disorders, intended to provide, under conditions of activity which cannot be achieved by the methods used at present, a remedy which is both effective and lasting. certain circulatory disorders attributable both to a functional impairment and to various accidental causes and finding an immediate adaptation in the treatment of latent or transient conditions such as cellulite, varicose veins, sprains or fractures, fatigue or tears muscle resulting from violent exercise or other abnormal conditions of a similar nature.
The apparatus which is the subject of the invention is intended to subject the affected part of the body to a series of controllable compressions initiated at the extreme level of the treated area and gradually extended towards the heart and lungs, each of these compressions being followed by a decompression with which it constitutes an elementary complete cycle of treatment, which will be repeated as many times as the patient's condition requires.
It is characterized in that it comprises a pneumatic enclosure intended to embrace, by one of its faces, the affected region and whose opposite face is contained by a substantially inextensible envelope capable of being suitably secured to the part concerned. of the patient's body, said enclosure being able to be alternately inflated by progressing along a determined route and direction, then deflated, by successive maneuvers of a distributor connected, on the other hand, to a pressurized fluid generator.
The object of the apparatus is thus to silrer, in the appropriate direction, the continuous propagation of a compression of increasing extent suitable to cause a progressive expression of the toxic residues contained in the tissues, compression of which it is, moreover. , able to control the degree of deep penetration.
A few brief anatomical notions will first be recalled concerning, by way of example, a typical sickly case, the genesis of the manifestation of cellulite.
The human body presents a triple circulatory network: arterial, venous and lymphatic, the third of which is of considerable importance because of what it contributes, in a very large part, to guarantee the elimination of waste from the organism, such as , in particular, than C02, the muscular effort of which loads the blood and lymph.
The latter is localized and circulates around the muscles and, notoriously, in the connective tissue embracing the different organs, so that the existence of a real conjunctive-lymphatic reservoir can be considered, in an elementary way. .
Any circulatory disorder manifesting itself in this particular area immediately gives rise to its engorgement which in turn causes irritation and then inflammation from which the condition commonly referred to as cellulitis results.
However, the mass of toxic products deriving from their accumulation in the connective tissue is, in this occurrence, blocked within it under the retentive influence exerted spontaneously, at the slightest impact on the epidermis or as a result of 'any reflex excitation, the vagosympathetic system.
From these observations, it appears that strong manual massages, which are usually resorted to in order to operate the artificial elimination of the residual waste considered, must therefore be strictly prohibited; on the other hand, superficial actions with the same intention consisting of outcrops, touching, pinching and other comparable contacts can only lead to practically unappreciable results, by virtue of the fact that, however well executed, they are, they trigger tightening phenomena in the skin that are more harmful than useful.
The object of the proposed device is to make it possible, on the contrary, to subject either the whole or a fractional portion of a limb or possibly even to any other deficient region of the body, an enveloping compression of absolutely controllable power, to which is gradually printed an expansion directed towards the heart and the lungs, succeeding in soliciting, together with an intensive activation of the venous return eirculation, an effective emptying of the conjunctivo-lymphatic reservoir resulting in a noticeable relaxation of the muscles, without either, nevertheless, in no way thwarted the regime of arterial flow.
The appended drawing represents, by way of example, two embodiments of the apparatus forming the subject of the invention.
Fig. 1 is a view of the assembly of a first embodiment of the apparatus specially conditioned with a view to receiving a leg, offering certain conventional cutouts favorable to the design of its structure.
Fig. 2 shows, in a view in longitudinal section, on a larger scale, the detail of the pneumatic device of the apparatus shown in FIG. 1.
Fig. 3 schematically shows a plan view of a second embodiment of the apparatus for the dorsal area.
Finally, fig. 4 is a cross-sectional view taken by i - A of the preceding one.
The apparatus shown in Figs. 1 and 2 comprises a pneumatic enclosure constituted by a tubular chamber 1, of adequate length and section, made of a flexible plastic material such as rubber, affecting, in the empty state, the flattened appearance of a strip and of the two closed ends from which emerge transversely two nozzles 2 and 3, of the same nature, which communicate with the capacity of the latter.
This chamber 1, initially wound in a helix around the leg, simulated in 4, in the manner of a band showing, from one turn to the next, a certain covering 5, is then enveloped in a substantially inextensible sheath 6 ' made, for example, from a canvas of suitable quality, curved at the request of the average conformation of the limb considered and whose junction edges 7 are provided with hooks 8 in charge of receiving a lace 9 ′ allowing, thanks to the the ability to adjust its tension, to impose on said sheath 6 a clamping of variable power which it is thus possible to increase or release at will at certain carefully located points.
To the two nozzles 2 and 3 are coupled, via connectors 10, two flexible pipes 11 and 12 individually accessing the end pieces 13 and 14, perpendicularly oriented, of a distributor 15 consisting of a four-way valve, of which the rotary plug 16, pierced with a diametral slot 17, can be operated using an actuating handle 18.
Of the two complementary nozzles of this distributor, respectively opposite to the aforementioned 14 and 13, the first, 19, opens directly into the atmosphere, while the second, 20, is connected by means of two flexible pipes 21 and 22, between which is placed a heating element 23 usable at the discretion of the practitioner, the pressure regulator 24 of a pressurized fluid generator in the form of a portable compressed air bottle 25.
The heating element 23 consists of a section of rigid tube around which is arranged an electric resistance supplied by a conductor 26, which can be connected to a current outlet.
Finally, on a tap 27 of the nozzle 13 of the distributor is connected a manometer 28, suitably graduated, capable of detecting at any time the pressure prevailing in the pneumatic chamber 1.
The device can also be completed by a tamper-proof relief valve, not configured, acting as a pressure limiter, suitably calibrated and installed on the circuit, playing the role of a safety device designed to overcome all risks of possible accidents.
This being said, the use and operation of the apparatus described are then as follows:
The actuating handle 18 of the distributor 15 being previously placed at the average position 18a of its travel, by virtue of which the lumen of the plug 16 occupies the oblique orientation 17a responding to the simultaneous closure of all the circuits, the practitioner, after having measured, at the extremity of the limb considered, the patient's arterial pressure, adjust the pressure regulator 24 so that the air delivered, at its outlet, by the bottle 25 shows the pressure suitable for the characteristic conditions of the patient. case to be treated.
It then brings this handle 18 to position 18 "drawn in solid lines, where the light mentioned above, aligning, at 17, with the orifices of the end pieces 20 and 13, allows the admission of the fluid conveyed by the pipes. 21. and 22 and the axial intercommunication tube of the heating element 23, in the pipe 11 leading to the inlet nozzle 2 of the pneumatic chamber 1.
As the general 1 and detail 2 figures jointly explain, this fluid then flows into said chamber 1, along the helical path of which it progresses and whose volumetric expansion, per ceptible in 29, that the inextensibility of the sheath 6, containing it, forced to be carried out in the direction of the center, creates a compression evolving towards the root of the limb and of constantly controllable intensity by simply reading the manometer 28.
When the filling of this chamber 1 is accomplished, the operator oscillates the handle 18 from 18e to 18b, imparting to the bushel 16 a 90 degree rotation as a result of which the lumen 17 comes, eii 1-7b, to be located in sight of the nozzles 14 and 19 of the distributor, carrying out the venting of the exhaust duct 12 which connects to the latter the exhaust nozzle 3.
Where appropriate, bringing the heating element 23 into play with the aim of concurrently obtaining a subsidiary thermal action. it repeats this double cycle of consecutive inflation and deflation as many times as the state of the affected limb requires, thus triggering a train of successive compression waves capable of pushing back the toxic residues accumulated in the reservoir conjunctiva-lymphatic towards the lungs where their elimination by combustion can be further favorably favored thanks to a slow auxiliary oxygenation by inhalation, conducive to increasing the efficiency of the result and from which, moreover, an accelerated disappearance of fatigue derives felt.
From a medical point of view, it may be interesting that the leisure be reserved to discipline and regulate the movement of the compressive wave by varying, on demand, its speed of propagation either all along this limb, or in some only of its fractions.
A very simple solution that satisfies this problem consists in using a flexible tubular chamber comprising a reinforced external wall, which, thereby showing a restricted deformability, is liable, depending on the degree of tension imposed on it during the initial winding. , to more or less slow down the progression of the filling fluid.
As a variant, it could, moreover, with the same objective, be retained the use of a chamber of uniformly thin thickness on which would be affixed an additional elastic band interfering between it and the inextensible sheath and capable of imparting the same possibility of adjustment.
It is easy to imagine that it cannot be envisaged to proceed with impunity to such a coating of all the parts of the body and, in particular, of the abdomen or the cunt where the resistant planes are lacking, however still that it would provide, on the breast itself, a very disagreeable impression of stricture.
For this reason, it has been planned to establish different models of devices which respectively match these various applications. The second embodiment shown in FIGS. 3 and 4 is specially adapted for the treatment of the dorsal region. It comprises a rigid frame 30, made of any suitable material, limited below by a flat seat face 31 and above by a concave wall 32, of adequate curvature, on which rests a flexible enclosure of rectangular outline 33, whose structure reminiscent of an air mattress.
This enclosure 33, which replaces the chamber 1 of the previously detailed device and is provided, like it, with two inlet 34 and outlet 35 nozzles, comprises, as such, a series of oblong cells 36, adja-. hundred and parallel, oriented, on assembly, normally to the generatrices of the wall 32 and which are communicated with each other by orifices 37, judiciously calibrated, by the channel. of which said cells inflate, in turn, in the direction shown by arrow 38.
These nozzles 34 and 35 are intended to be attached to the two supply and discharge pipes, previously designated under references 11 and 1), reaching the distributor of a pressure generator group identical to that which has just been exposed.
Finally, on the lateral flanks 39 and 40 of the frame 30 are fixed a number of mutually assemblable straps 41 and 42, lending themselves to the subjection of the subject's body to the device and which, due to the ability of adjustment of their individual tension, fulfill an office in all points comparable to that of the equipment which constituted, in the primitive example, the sheath 1 and its closing lace 9.
It appears that, subject to modifying the patient's position on demand, this same instrument can equally be used for an intervention on the abdominal area or on the hips.
When it comes, moreover, to treat the region of the eye, use will be made of an apparatus similar to the preceding one and differing only in the fact that the undeformable reinforcement 30 will be, in order to avoid the feeling of strangulation that the rigidity of such an element would not fail to provide, replaced by a flexible rubber plate capable of spontaneously adapting to the personal configuration of the individual.
