La présente invention concerne des dispositifs grâce auxquels on peut administrer un médicament à ou par des patients inspirant à travers les dispositifs en question.
Les médicaments peuvent se présenter sous une forme solide finement divisée ou sous une forme liquide. On connaît parfaitement à présent des dispositifs de ce genre destinés à l'administration des médicaments contenus dans des gélules à des patients souffrant de troubles bronchiques comme, par exemple l'asthme bronchique. Il est bien connu qu'un médicament en poudre ou sous toute autre forme finement divisée peut être présenté en gélules ou capsules qui sont logées par un patient dans un dispositif du genre ce celui que l'on appelle quelquefois un "insufflateur". Le médicament est ensuite libéré de la gélule ou capsule et inhalé par le patient, habituellement par la bouche, mais quelquefois par le nez.
Les mémoires descriptifs de la demande de brevet PCT possédant le numéro de publication W082/01470 et le mémoire descriptif du brevet britannique NO 1387954 décrivent tous deux des dispositifs pour administrer un médicament sous une forme finement divisée à partir de gélules ou capsules.
Dans chacun des dispositifs antérieurement décrits, les gélules sont montées sur un élément de support rotatif sur lequel chaque gélule peut, à son tour, être amenée en une position dans laquelle elle est ouverte pour tolérer la sortie du médicament de la gélule, afin de lui permettre d'être inhalé par un patient inspirant à travers l'embouchure du dispositif. Dans le dispositif décrit dans le

mémoire descriptif du brevet britannique NO 1387954, les gélules peuvent être montées dans un emballage dit à bulles ou ampoules.
L'emploi des gélules destinées à contenir des médica-, ments, réalisées en gélatine, n'est pas dépourvu de certains désavantages. La gélatine est un produit relativement instable et manque de résistance physique, si bien que les gélules doivent être protégées par emballage, par exemple, dans des bouteilles en verre. La dégradation par l'environnement, tant des gélules que de leur contenu peut se produire après une durée de temps relativement courte.
La présente invention a donc plus particulièrement pour objet d'administrer des médicaments aux patients du genre précité, d'une manière plus commode qu'il n'était possible de le faire jusqu'à présent et qui évite la nécessité de recourir à l'emballage des médicaments dans des gélules. Le dispositif suivant la présente invention permet le recours à la technique de l'emballage de médicaments par leur introduction dans des emballages à bulles, c'est- à-dire des emballages qui se composent d'une feuille, qui peut être stratifiée, en une pellicule ou matière plastique qui sert de support et qui est dotée d'un certain nombre de récipients susceptibles de se rompre ou de s'ouvrir, appelés "bulles" ou "ampoules" comprenant une feuille assujettie sur une première feuille pour former un couvercle ou une cloche.
De tels emballages à bulles sont d'un usage courant dans le cas de comprimés de n'importe quelle sorte ou type, mais la demanderesse a découvert à présent que l'on pouvait également les utiliser avec des médicaments se présentant sous une forme finement divisée ou sous une forme liquide.
La présente invention a par conséquent pour objet un dispositif destiné à l'administration de médicaments à des patients, caractérisé en ce qu'il se compose d'un logement pourvu d'une chambre cylindrique, d'une entrée d'air dans la

chambre, d'un appui à l'intérieur de la chambre, agencé pour supporter un étui comportant un récipient à médicament ou une multiplicité de récipients de ce genre disposés en un cercle, un piston pouvant fonctionner de manière à coopérer avec un récipient qui coïncide avec lui en vue d'ouvrir le récipient en manière telle que l'air inspiré par un patient amène le médicament à en être libéré, un moyen pour faire tourner un étui sur l'appui afin de faire coïncideer le récipient ou chacun des récipients individuels, à son tour, avec le piston et,
en communication avec l'intérieur de la chambre, une sortie à travers laquelle un patient peut inspirer, si bien que le médicament est libéré d'un récipient et entraîné dans le courant d'air produit par le patient, de façon à passer par la sortie. La sortie est constituée, commodément, mais non essentiellement, d'une embouchure par laquelle un patient peut inspirer.
Le dispositif suivant la présente invention convient à l'administration de toute une série de médicaments tels que, par exemple, le salbutamol, le dipropionate de beclo- méthasone et le cromoglycate disodique.
Les dessins schématiques qui accompagnent le présent mémoire descriptif illustrent certaines formes de réalisation de la présente invention. Sur ces dessins : - la figure 1 représente une vue en perspective écorchée d'un dispositif suivant une forme de réalisation de l'invention, - la figure 2 représente une vue détaillée du mécanisme à piston du dispositif précité, - la figure 3 représente une vue en élévation d'une autre forme de réalisation de l'invention, - la figure 4 représente une vue écorchée de la forme de réalisation de l'invention illustrée sur la figure 3, et - la figure 5 représente une vue en perspective d'une autre forme de réalisation encore de l'invention.

Sur la forme de réalisation illustrée sur les figures 1 et 2 des dessins qui accompagnent le présent mémoire descriptif, on voit que le dispositif d'administration du médicament se compose d'un logement ou boîtier cylindrique peu profond, réalisé en une matière plastique, qui comporte une chambre cylindrique 2. La chambre est fermée à l'une de ses extrémités 3 que l'on considérera dans le présent mémoire comme formant la partie inférieure ou fond de la chambre et un couvercle amovible 4 s'adapte étroitement par-dessus la chambre à son autre extrémité.
Une sortie à embouchure 5 fait saillie vers l'extérieur à partir de la paroi cylindrique du boîtier ou logement 1 et communique avec l'intérieur de la chambre 2. Une pièce de garde perforée, non représentée, est montée dans l'embouchure afin d'empêcher toutes particules solides d'un calibre indésirablement important d'être inhalées par un patient inspirant à travers l'embouchure.
Un épaulement ou report 6 court autour de la paroi interne de la chambre 2 pour constituer un appui annulaire sur lequel vient se loger un emballage à bulles 7.
L'emballage à bulles 7 peut commodément être formé d'une pellicule stratifiée à une multiplicité de récipients ou "bulles" ou "ampoules" 8, susceptibles de se rompre, disposés en un cercle. Les bulles 8 sont remplies d'un médicament en forme de particules qui possèdent un calibre dont la granulométrie varie de 0, 5 a 10 microns. Le médicament peut se présenter avec un excipient pharmaceutiquement acceptable, tel que le lactose ou l'amidon sous forme de particules. En alternative, le médicament peut aussi se présenter sous une forme liquide.
L'emballage à bulles circulaire se présente sous la forme d'un disque et est monté de manière amovible dans la chambre de façon à pouvoir être remplacé lorsque les doses individuelles de médicaments contenues dans les bulles ont été distribuées.

La chambre 2 contient une colonne d'appui cylindrique, ouverte, centrale, 9, se dressant depuis la paroi inférieure ou de fond 3 de la chambre. Un élément à disque de serrage 10 est monté de manière amovible dans la chambre 2 et comporte, à sa partie inférieure, une multiplicité de goujons non représentés qui s'engagent dans la colonne d'appui.
L'élément de serrage 10 peut tourner à l'intérieur de la chambre. En cours d'utilisation, on place l'élément de serrage par-dessus un emballage à bulles 7, qui a déjà été introduit dans la chambre et qui repose sur l'épaulement d'appui 6. L'emballage à bulles 7 est de préférence constitué d'un disque circulaire d'une matière en pellicule stratifiée avec des bulles ou récipients 8. L'élément de serrage 10 comporte une multiplicité d'ouvertures 11 qui sont disposées en un cercle et qui sont mutuellement espacées les unes des autres de façon que chacune d'entre elles reçoit l'une des bulles 8 de l'emballage à bulles 7. Un bouton 12 fait saillie à travers une ouverture 13 prévue dans la partie supérieure du couvercle 4.
Le patient peut faire tourner ce bouton pour provoquer la rotation de l'élément de serrage 10 et, étant donné que les bulles 8 de l'emballage à bulles 7 sont situées dans des ouvertures 11 de la plaque de serrage 10, la rotation de l'élément de serrage entraîne également celle de l'emballage à bulles.

Lorsque le piston 17 est enfoncé à l'encontre de l'action du ressort 20, la partie marginale inférieure 21 de ce piston passe à travers une ouverture 22 prévue dans le cylindre de piston pour passer à travers une bulle ou ampoule 8 située en alignement avec le piston. Un tel contact va ouvrir la bulle et permettre la libération du médicament qu'elle contient.
Cette fonction va ouvrir la bulle de façon que lorsqu'un patient inspire l'air qui passe à travers la bulle, le médicament soit entraîné dans le courant d'air et sorte par l'embouchure 5 par une cavité de transfert 23 qui se trouve à l'intérieur de la chambre en communication avec l'embouchure 5. Par la rotation du bouton 12, l'élément de serrage 10 et l'emballage à bulles 7 peuvent être amenés à tourner pour déplacer chaque bulle et la loger, à son tour, en-dessous du piston. Les diverses protubérances ou saillies 14 se logent, à leur tour, dans l'évidement 15 pour assurer que l'emballage à bulles soit en alignement correct avec le piston.
Il n'est pas essentiel que le piston comporte un tranchant 21 pour ouvrir la bulle. Si on le souhaite, on peut utiliser une aiguille pour perforer la bulle, ou bien le piston peut comporter une extrémité pointue ou même une extrémité émoussée, ou bien on peut avoir recours à n'importe quel autre moyen d'ouverture commode.
Le couvercle d'embouchure peut comporter un moyen de verrouillage 24 qui peut coopérer avec le piston lorsque le dispositif n'est pas en utilisation, afin d'empêcher l'actionnement accidentel du piston.

En cours d'utilisation, le patient qui a besoin d'une dose de médicament peut maintenir le dispositif avec l'embouchure engagée dans sa bouche. Le patient abaisse ensuite le piston afin d'ouvrir la bulle et donner accès au médicament qu'elle contient et inspire à travers l'embouchure, de sorte que le médicament soit entraîné dans le courant d'air et pénètre dans les poumons du patient. Si on le souhaite, l'embouchure peut être pourvue d'ouvertures d'entrée d'air 25 pour améliorer l'écoulement de l'air lorsque le patient inspire.
Selon une variante non représentée, la partie inférieure de l'emballage à bulles peut reposer sur une autre plaque de serrage au lieu de l'épaulement ou rebord d'appui 6.
L'emballage à bulles est commodément agencé de manière à comporter un nombre suffisant de doses individuelles pour qu'un patient l'utilise pendant une période commode, comme une journée et plus. Le logement ou boîtier peut être modifié en prévoyant une chambre supplémentaire, non visible, au fond, cette chambre supplémentaire étant fermée par un couvercle amovible 26. Cette chambre peut être utilisée pour conserver des emballages à bulles de rechange.
Si on le souhaite, l'embouchure peut être conçue de façon qu'un patient puisse l'utiliser pour inhaler à travers le nez.
Les figures 3 et 4 illustrent un dispositif modifié qui n'utilise pas l'élément de serrage 10. Le dispositif conçu selon cette variante comporte un boîtier ou logement 30 qui comprend une chambre 31. Une embouchure 32 s'étend vers l'extérieur à partir de la paroi cylindrique du logement ou boîtier 30 dans une direction radiale dans son ensemble et communique avec l'intérieur de la chambre 31. Une garde perforée 33 est prévue à l'entrée de l'embouchure 32. Un épaulement ou rebord 34 court tout autour de la paroi interne de la chambre 31 pour former un appui annulaire pour

The present invention relates to devices by which a drug can be administered to or by inhaling patients through the devices in question.
The drugs can be in a finely divided solid form or in a liquid form. There are now well known devices of this kind intended for the administration of medicaments contained in capsules to patients suffering from bronchial disorders such as, for example, bronchial asthma. The bubble wrap is conveniently arranged to have a sufficient number of individual doses for a patient to use it for a convenient period, such as a day or more. The hole 52 is positioned so that each hole 38 in the support element 35 is in coincidence or alignment with this hole when the support element 35 rotates.
When one of the holes 38 is in coincidence or in alignment with the hole 52, the trigger 50 can be pressed, so that its piston or plunger 51, which can be in the form of a needle, pierces the bubble 37 located in this hole (that is to say pierces the top and bottom of the bubble), so as to allow the powder to exit the bubble. Some of the powder falls into a tray-like compartment 54 inside the chamber 31. When the device is in use and the patient is breathing through the mouthpiece 32, it is, of course, essential that air can enter the interior of the chamber 31. Provision may be made any suitable air inlets. However, air can most conveniently enter through the hole 52, the piston or needle 51 being of a diameter smaller than the diameter of the hole 52, so that the latter simultaneously serves as an inlet for 'air.
Figure 5 illustrates a modified device which can conveniently be used to administer two different drugs to a patient at separate time intervals.
