APPARAAT VOOR INTRAORALE DISTRACTIEOSTEOTOMIE
Huidige uitvinding heeft betrekking op een distractieapparaat voor de verlenging van een onderkaak na corticale osteotomie en verstelbare hechting van minstens twee beensegmenten met het oog op een dynamische osteosynthese met reconstructie van het onderkaakbeen onder uitzetting van de afstanden tussen beide tegenoverliggende beensegmenten door middel van geleidelijke rekking.
Ze vindt toepassing in maxillofaciale chirurgie, in het bijzonder in orthognathische chirurgie bij patienten met mandibulaire retrognathie. Het gaat om een gelaatvorm waarbij de onderkaak, die te kort is, achter het frontaal vlak van het voorhoofd ligt. De uitvinding heeft als doelstelling de occlusale sluiting van de tanden te herstellen, de ademhaling te verbeteren en het gelaat in proporties te brengen. Ze biedt voor de groep jongere patiënten, die tot nu toe zonder blijvend skeletaal resultaat, met functionele orthodontische apparatuur werd behandeld, een beter alternatief aan. Onderkaakverlengingen worden nu routinematig verwezenlijkt, in de regel eer na de tweede groeispurt door middel van een "sagittal split". Onderkaakverlengingen zijn echter op jongere leeftijd geindiceerd bij syndroompatiëntjes met uitgesproken onderkaakhypoplasie.
Door DE-A-195 023 (Kreidler-Wangerin) is een apparatuur voor distractieosteotomie met uittrekking van minstens twee beensegmenten beschreven. Het distractieapparaat wordt ingebracht in beide kaakhoeken na het vrijleggen van het bot en een sagittale splitsinsosteotomie. Door een transorale toegang is een bicorticale schroeffixatie op de mondbodem aangebracht.
De distractiecilinder gaat na doorboring van de mucosa,

door het lange vestibulum en eindigt voor de binnenste hoeken van de mond. Het aanbrengen van het inwendig apparaat moet gebeuren via incisies in de huid van de wang. Het apparaat ligt parallel met de onderste tandenrij, waardoor bilaterale toepassing kan leiden tot een krachtmoment in het kaakgewricht.
Onderhavige uitvinding heeft tot doel aan bovenvermelde nadelen te verhelpen en verschaft een distractieapparaat van het type zoals in het inleidend gedeelte van de bijgaande conclusie 1 omschreven, dat via de mond geaktiveerd kan worden
Dit wordt volgens de uitvinding bereikt doordat het apparaat volgende delen omvat : - een distale verankeringsschroef, waarvan het schroefgedeelte in een boring in een opstijgende tak van de onderkaak (ramus mandibularis) is ingeschroefd en waarvan het vrije uiteinde een veelhoekige kop, bij voorkeur een zeskantkop en een scharnierbus bezit ; - een expansieschroef scharnierend verbonden met bovengenoemde verankeringsschroef door middel van een bevestigingsschroef die ookals gewrichtstap dient, die in bovengenoemde scharnierbus past ;
- een schroefstop die het gewrichtorgaan in een starre positie in een onveranderlijke richting vasthoudt, en - een verbindingsstuk met binnenschroef voor de vasthechting van een bevestigingsplaat die met schroeven op een zijde van de doorgebeitelde mandibulaire arcade aangebracht wordt.
Het distractiegedeelte bestaat uit een telescopische schroef. De telescopische schroef laat toe het distractiegedeelte zeer compact te houden, waardoor het ook na volledige rekking na distractie intra-oraal blijft. De beperkte lengte van het apparaat laat lipsluiting toe en daaruit resulteert een meer comfortabel gevoel voor het eten en drinken. Het systeem

blijft onopvallend, wat een sociaal voordeel is. Een aangepaste schroeflengte voor de distale verankeringsvijs en de schroefjes om de laterale bevestigingsplaat te bevestigen laat toe de n. alveolaris inferior te vermijden, alsook de tandwortels, dit in tegenstelling tot de uitwendige distractors met bicorticale pennen. Het gevoel in de lip en de kauwfunctie blijven gegarandeerd zodat er geen gevaar van bijtletsels in de lip, de tong noch de wang bestaat.
Een week na de ingreep kunnen de ouders met activatie van de distractor aanvangen. De activatie bedraagt elke keer 1 mm per dag. Ongeveer twee maanden hierna wordt de distractor verwijderd. Complete benige consolidatie van de distractieplaats gebeurt aan beide zijden.
Volgens een bijzonderheid van de uitvinding is de verankeringsschroef aan zijn vrije uiteinde van een veelhoekige kop, bij voorkeur van een zeskantkop en een scharnierbus voorzien en in een boring van een loodrechte botstuk ingebracht wordt.
De expansieschroef is een mechanische vijzel is, die uit drie delen bestaat die in elkaar passen, waarbij het middenste element de grootste diameter heeft, om als verbindingsmof te fungeren en aan zijn uiteinden van binnenste windingen van tegenoverliggende richting en verschillende grootten voorzien is, waarin twee tegenoverliggende telescopische armen passen die in elkaar kunnen verschuiven en nagenoeg de zelfde lengte hebben als het middenste verbindingsmof, zodat zij volledig in en uit de middenste verbindingsmof kunnen schuiven.
Een onderlegring, bekend als "washer" laat toe de verankering dicht tegen een botkant te plaatsen, zonder dat deze kan afbreken zoals bij schroeven met een conische kop.
Er zijn geen uitwendige incisies in het gelaat.

Het risiko van kosmetische schade wordt herleid tot nul.
De ingreep is minder traumatisch en kan als "one-day case" doorgaan, zonder dagenlange ziekenhuisopname, omdat geen krachtaanwendig nodig is bij de osteotomie.
Bij het verwijderen van de wijsheidstandkiem wordt het bot gemakkelijk verder verzwakt buccaal en linguaal door middel van een corticale osteotomie. De incisie voor het verwijderen van de kies wordt achterwaarts verlengd om de verankeringselementen aan te brengen. Het verticale scharnierpunt tussen het distale verankeringsgeddeelte en de laterale bevestigingsplaat laat vanuit biomechanische standpunt een fysiologische repositionering van de kaakkop toe.
Tevens kan voor ee zuiver sagittale verlenging worden gekozen, zonder afhankelijk te zijn van de horizontale hoek tussen de opstijgende tak en horizontale tak.
Het apparaat is uit titanium vervaardigd. Dit materiaal is bekend voor zijn optimale biocompatibiliteit, zowel bij de opname in het bot (osteointegratie) als de opname in de weke delen. Er zijn geen allergieën op titanium beschreven. Dimetalosen worden uitgesloten.
Het beperkte volume van het apparaat volgens de uitvinding vergeleken met externe activatoren laat de materiaalkosten drastisch drukken. Beschadiging van het apparaat of van de botfixatie bij sport, spel, slaaphouding is minder te vrezen dan bij externe fixatoren.
Deze en andere kenmerken van de uitvinding zullen uit de volgende beschrijving voortvloeien waarin verwezen is naar de bijgevoegde tekeningen die een uitvoeringsvorm van een apparaat volgens de uitvinding tonen.

Een distractieosteotomie is een chirurgische operatie om misvorming of verkromming van een gekromd of te kort been op te heffen waarbij het been wordt gesplitst in ten minste twee beensegmenten en de beensegmenten weer samen gelast door dynamische osteosynthese, namelijk geleidelijke repositie en fixatie van de beensegmenten met behulp van geleide stangen, metalen plaatjes, schroeven, stiften, botpennen of repen. zonder dat deze kan afbreken zoals bij schroeven met een conische kop.
Het distractieapparaat afgebeeld in figuur 1, dient voor de verlenging van de onderkaak, na een corticale bilaterale osteotomie. Het omvat volgende delen : - een distale verankeringsschroef 1, waarvan het schroefgedeelte 2 in een boring in een opstijgende tak van de onderkaak (ramus mandibularis) 3 is ingeschroefd en waarvan het vrije uiteinde een veelhoekige kop 4, bij voorkeur een zeskantkop en een scharnierbus 5 bezit ; - een expansieschroef 6 scharnierend verbonden met bovengenoemde verankeringsschroef 1 door middel van een bevestigingsschroef 7 die ook als gewrichttap dient, die in bovengenoemde scharnierbus 5 past ;
- een schroefstop 8 die het gewrichtorgaan in een starre positie in een onveranderlijke richting vashoudt, en - een verbindingsstuk 9 met binnenschroef 10 voor de vasthechting van een bevestigingsplaat 11 die met schroeven 12 op een zijde van de doorgebeitelde mandibulaire arcade aangebracht wordt.
De hals van het schroefgedeelte draagt een onderlegring en dient als schouderstuk voor het overschuiven van de onderlegring 13 die een uitstekende aansluiting op de opstijgende tak van de onderkaak moet bewerkstelligen. De opening van de scharnierbus 5 wordt distaal geplaatst, zodanig dat de inpassende mannelijke gewrichttap een rotatievrijheid van nagenoeg 200 om de verticale as van een bevestigingsschroef 7 heeft, en dit langs beide zijden van de verticale aslijn (Figuur 4).
De gewrichttap is vooral bestemd om de uitzettingsrichting van de expansieschroeven evenwijdig met de lengteas van de mond te houden, in tegenstelling met reeds bekende distractie apparaten die geen parallellisme van de uitzettingsgeleidingen kunnen

waarborgen (Figuur 5).
De expansieschroef 6 is een mechanische vijzel die uit drie delen bestaat die in elkaar passen. Het middenste element heeft de grootste diameter. Het fungeert als verbindingsmof 14 en is aan zijn uiteinden van binnenste windingen van tegenoverliggende richting en verschillende grootten voorzien, waarin twee tegenoverliggende telescopische armen 15,16 passen die in elkaar kunnen verschuiven en nagenoeg dezelfde lengte hebben als de verbindingsmof 14, zodat zij volledig in en uit de middenste verbindingsmof 14 kunnen schuiven.
De linker telescopische arm 16 heeft een naar buiten lopende zeshoek waarvan de zijden dus op het uiteinde kleiner zijn dan in het begin. De helling van 10 is voorzien om het verbindingsstuk 9 zodanig op te spannen met de klemvijs 17 dat de nodige stijfheid in het verbindingbeslag tussen de expansieschroef 6 en de verbindingsplaat blijft. De uitlopende zeshoek heeft binnenin een schroefdraad om de klemvijs 17 te ontvangen. De klemvijs 17 is voorzien van een veelhoekige imbus.
Het vrije uiteinde van de linker telescopische arm van de expansieschroef 16 draagt een spil met een inwendige axiale winding waarop een verbindingsbeslag met een coaxiale klemschroef 18 geklemd wordt. Het verbindingsstuk 9 heeft willekeurig vormen teneinde een optimale verbinding tot stand te brengen tussen de expansieschroef 6 en de bevestigingsplaat 11 die op de onderkaak 3 bevestigd is.
Het verbindingsstuk 9 heeft bovenaan een interne zeshoek, eveneens dezelfde coniciteit als de spil 19 van de linker telescopische arm 16. Onderaan het verbindingsstuk 9 bevindt zich een interne langwerpige kamer die passend aansluit op het uitstekend gedeelte van de laterale bevestigingsplaat 11. Hierdoor komen de

distractiekrachten en de kauwkrachten op de bevestigingsplaat 11 en niet op de bevestigingsschroef 12 (Figuur 8).
Het verbindingsstuk 9 bezit een onderste langwerpige kamering. Het wordt aan de bovenkant van de bevestigingsplaat 11 aangebracht op de langwerpige verhoging door middel van twee schroeven. De vijf boringen in het bevestingsplaat 11 onderaan dienen voor de verankering van dit element op de onderkaak 3 (Figuren 7 en 9). De bevestigingsplaat 11 dat op de rechter kaak wordt bevestigd is identiek met de linker bevestigingsplaat maar dan in spiegelbeeld.
Figuur 10 toont het distractieapparaat in uitgezette stand, op de linker kant van een schedel, na aktivatie. De aktivatie gebeurt via de mond en kan uitgevoerd worden door de ouders zelf.

DEVICE FOR INTRAORAL DISTRACTION OSTEOTOMY
The present invention relates to a distraction device for the extension of a mandible after cortical osteotomy and adjustable attachment of at least two leg segments for dynamic osteosynthesis with reconstruction of the mandible bone while expanding the distances between the two opposite leg segments by gradual stretching.
It finds application in maxillofacial surgery, in particular in orthognathic surgery in patients with mandibular retrognathia. It is a facial shape in which the lower jaw, which is too short, lies behind the frontal plane of the forehead. The object of the invention is to restore the occlusal closure of the teeth, to improve breathing and to proportion the face. It offers a better alternative for the group of younger patients who have hitherto been treated with functional orthodontic equipment without a permanent skeletal result. Lower jaw extensions are now routinely accomplished, usually after the second growth spurt through a "sagittal split". Lower jaw extensions are however indicated at a younger age in syndrome patients with pronounced lower jaw hypoplasia.
DE-A-195 023 (Kreidler-Wangerin) describes an equipment for distraction osteotomy with extraction of at least two leg segments. The distraction device is inserted into both jaw angles after the bone has been exposed and a sagittal splice insosteotomy. Through a transoral access, a bicortical screw fixation is applied to the floor of the mouth.
The distraction cylinder goes after piercing the mucosa,

through the long vestibulum and ends at the inner corners of the mouth. The application of the internal device should be done through incisions in the skin of the cheek. The device is parallel to the bottom tine row, allowing bilateral application to lead to a force moment in the temporomandibular joint.
The present invention aims to overcome the above-mentioned drawbacks and provides a distraction device of the type as defined in the introductory part of the appended claim 1, which can be activated by mouth
This is achieved according to the invention in that the device comprises the following parts: - a distal anchoring screw, the screw part of which is screwed into a bore in an ascending branch of the lower jaw (ramus mandibularis) and whose free end has a polygonal head, preferably a hexagonal head and has a hinge sleeve; - an expansion screw hingedly connected to the above-mentioned anchoring screw by means of a fastening screw which also serves as a joint step, which fits into the above-mentioned hinge sleeve;
- a screw plug holding the joint member in a rigid position in an unchanging direction, and - a connector with internal screw for the attachment of a mounting plate which is mounted with screws on one side of the chiselled mandibular arcade.
The distraction part consists of a telescopic screw. The telescopic screw allows to keep the distraction part very compact, so that it remains intra-oral even after full stretching after distraction. The limited length of the device allows lip closure and results in a more comfortable feeling for eating and drinking. The system

remains inconspicuous, which is a social advantage. An adapted screw length for the distal anchoring screw and the screws to fix the lateral mounting plate allows the n. avoid inferior alveolaris, as well as dental roots, as opposed to external distractors with bicortical pins. The feeling in the lip and the chewing function are guaranteed so that there is no danger of biting injuries in the lip, tongue or cheek.
Parents can start activation of the distractor one week after the procedure. The activation amounts to 1 mm per day. The distractor is removed approximately two months after. Complete bony consolidation of the distraction site occurs on both sides.
According to a feature of the invention, the anchoring screw is provided at its free end with a polygonal head, preferably with a hexagon head and a hinge sleeve, and is inserted into a bore of a perpendicular bone piece.
The expansion screw is a mechanical jack, which consists of three parts that fit together, with the center element having the largest diameter, to act as a connection sleeve and having inner turns of opposite directions and different sizes at its ends, in which two opposite telescopic arms that can slide into each other and are approximately the same length as the center joint sleeve, so that they can slide completely in and out of the center joint sleeve.
A washer, known as a "washer", allows the anchor to be placed close to a bone edge without breaking off, as is the case with conical head screws.
There are no external incisions in the face.

The risk of cosmetic damage is reduced to zero.
The procedure is less traumatic and can proceed as a "one-day case", without days of hospitalization, because no force is needed for the osteotomy.
When the wisdom germ is removed, the bone is easily weakened further buccally and lingually by means of a cortical osteotomy. The incision for removing the molar is extended backwards to fit the anchoring elements. The vertical pivot point between the distal anchoring section and the lateral mounting plate allows physiological repositioning of the jaw head from a biomechanical point of view.
A pure sagittal extension can also be chosen, without depending on the horizontal angle between the ascending branch and horizontal branch.
The device is made of titanium. This material is known for its optimal biocompatibility, both in bone absorption (osteointegration) and soft tissue absorption. No allergies to titanium have been described. Dimetaloses are excluded.
The limited volume of the device according to the invention compared to external activators drastically reduces material costs. Damage to the device or the bone fixation in sports, games, sleeping position is less to fear than with external fixators.

A distraction osteotomy is a surgical operation to correct malformation or curvature of a curved or short leg in which the leg is split into at least two leg segments and the leg segments are welded together again by dynamic osteosynthesis, namely gradual reposition and fixation of the leg segments using of guided rods, metal plates, screws, pins, bone pins or bars. The distraction device shown in Figure 1 serves to extend the mandible after a cortical bilateral osteotomy. It comprises the following parts: - a distal anchoring screw 1, the screw part 2 of which is screwed into a bore in an ascending branch of the lower jaw (ramus mandibularis) 3 and the free end of which has a polygonal head 4, preferably a hexagon head and a hinge sleeve 5 possession; - an expansion screw 6 hingedly connected to the above-mentioned anchoring screw 1 by means of a fastening screw 7 which also serves as a joint pin, which fits in the above-mentioned hinge sleeve 5;
- a screw plug 8 which holds the joint member in a rigid position in an invariable direction, and - a connecting piece 9 with inner screw 10 for the attachment of a mounting plate 11 which is mounted with screws 12 on one side of the chiselled mandibular arcade.
The neck of the screw portion carries a washer and serves as a shoulder piece for sliding the washer 13 to provide an excellent connection to the ascending branch of the lower jaw. The opening of the hinge sleeve 5 is placed distally such that the mating male articulation tap has a rotational clearance of approximately 200 about the vertical axis of a fastening screw 7, along both sides of the vertical axis (Figure 4).
The joint tap is mainly intended to keep the expansion direction of the expansion screws parallel to the longitudinal axis of the mouth, in contrast to already known distraction devices which cannot have parallelism of the expansion guides

safeguards (Figure 5).
The expansion screw 6 is a three-part mechanical jack that fits together. The middle element has the largest diameter. It acts as a connection sleeve 14 and is provided at its ends with opposite-direction inner turns and of different sizes, in which two opposing telescopic arms 15,16 fit which can slide together and are approximately the same length as the connection sleeve 14 so that they are fully in and can slide out of the middle connecting sleeve 14.
The left telescopic arm 16 has an outwardly extending hexagon, the sides of which are thus smaller at the end than in the beginning. The slope of 10 is provided to clamp the connecting piece 9 with the clamping screw 17 in such a way that the necessary stiffness remains in the connecting fitting between the expansion screw 6 and the connecting plate. The flared hexagon has a screw thread inside to receive the clamping screw 17. The clamping screw 17 is provided with a polygonal hexagon socket.
The free end of the left telescopic arm of the expansion screw 16 carries a spindle with an internal axial winding to which a connecting fitting with a coaxial clamping screw 18 is clamped. The connecting piece 9 has arbitrary shapes in order to establish an optimal connection between the expansion screw 6 and the mounting plate 11 which is fixed on the lower jaw 3.
The connecting piece 9 has an internal hexagon at the top, also the same conicity as the spindle 19 of the left telescopic arm 16. At the bottom of the connecting piece 9 there is an internal elongated chamber which fits flush with the protruding part of the lateral mounting plate 11. As a result, the

distraction forces and the chewing forces on the mounting plate 11 and not on the mounting screw 12 (Figure 8).
The connecting piece 9 has a lower elongated chamfer. It is attached to the elongated elevation at the top of the mounting plate 11 by two screws. The five holes in the mounting plate 11 at the bottom serve to anchor this element on the lower jaw 3 (Figures 7 and 9). The mounting plate 11 which is mounted on the right jaw is identical to the left mounting plate but in mirror image.
Figure 10 shows the distraction device in the deployed position, on the left side of a skull, after activation. The activation is done by mouth and can be performed by the parents themselves.
