본원은 다공성 구조층 및 하이드로겔을 포함하는 복합 접착 구조체 및 이를 포함하는 의료기기에 관한 것으로서, 특히, 다공성 구조층 및 하이드로겔을 포함하는 복합 접착 구조체를 포함하는 체내 삽입형 소자에 관한 것이다.

Description

다공성 구조층 및 하이드로겔을 포함하는 복합 접착 구조체
본원은 다공성 구조층 및 하이드로겔을 포함하는 복합 접착 구조체 및 이를 포함하는 의료기기에 관한 것이다.
체내 삽입형 소자 (Implantable device)는 질병을 진단하거나 치료목적으로 몸속에 삽입하는 의료기기이다. 그러나, 생리적 활동 중 신체 움직임의 역학으로 인해 삽입된 소자가 초기 위치에서 이동할 수 있다. 본 발명은 혈관 및 식도, 기관지, 담도 및 요관을 포함하는 기타 신체 통로의 개방성(openness)을 유지하는 역할을 하는 스텐트의 이동에 중점을 둔다. 체내 삽입 후 근육의 연동운동(peristalsis)과 같은 비혈관의 수축과 이완은 관 내의 압력 차이를 발생한다. 이는 부드럽고 습한 인체 조직과 스텐트에 사용되는 금속, 산화물과 폴리머와 같은 외부에서 삽입된 재료 사이의 기계적, 생물학적 불일치를 강조하여, 이동 문제를 야기한다. 이러한 이동은 치료 효능을 저하시키고, 잠재적으로 주변 조직을 손상시키며, 부작용의 위험을 증가시킬 수 있다. 예를 들어, 스텐트 이동을 방지하기 위해 클립(clip), 스텐트 고정, 바브(barb), 플립(flip), 및 형태 변형 등 다양한 방법을 사용하여 스텐트를 안전하게 고정한다. 이러한 방법은 효과적이기는 하지만 각 환자의 개별 요구에 맞게 맞춤화해야 하며, 주변 조직의 손상이나 흉터를 초래하고, 삽입 후 환자의 고통과 이물감을 동반한다.
마찬가지로, 출혈 조절 및 상처 봉합 영역에서는 봉합사(suture) 및 스테이플(staple)과 같은 전통적인 침습적 방법이 계속해서 널리 사용되고 있다. 그러나, 최근 연구는 견고한 하이드로겔과 건조 폴리머 필름의 특성을 활용하는 무봉합 기술의 출현으로 발전해오고 있다. 생체적합성 하이드로겔 접착제는 특정 조직에 공유 결합하여 상당한 접착 수준을 달성하는 능력이 뛰어나다. 높은 수분 함량, 부드러움 및 신축성(stretchability) 덕분에, 하이드로겔은 부드러운 생물학적 조직의 기계적 특성을 모방하는 데 적합하므로, 특히 전통적인 봉합사와 스테이플이 덜 효과적일 수 있는 상황에서 유리하다.
최근 연구에서는 하이드로겔을 기판에 효과적으로 접착하기 위한 전략에 집중하였다. 이는 표면 브리징(bridging)/개시(initiation) 및 하이드로겔 페인팅 기술을 사용하는 것을 포함한다. 특히, 표면 브리지(bridge) 방법은 실란 또는 실란 커플링제(예를 들어, TMSPMA 및 APTES)와 같은 브리지 분자를 활용하여 하이드로겔과 기판 사이에 내구성 있는 결합을 설정한다. 그러나, 이 접근 방식은 기판과 하이드로겔 모두의 최적화가 필요하므로 번거로울 수 있다. 인상적인 물리적 특성을 지닌 하이드로겔의 개발에도 불구하고, 하이드로겔과 고체 재료 사이의 취약한 결합은 종종 장치 및 시스템에서의 통합과 기능성을 방해하는 중요한 문제를 야기한다.

본원은 다공성 구조층 및 하이드로겔을 포함하는 복합 접착 구조체 및 이를 포함하는 의료기기를 제공하고자 한다.
그러나, 본원이 해결하고자 하는 과제는 이상에서 언급한 과제로 제한되지 않으며, 언급되지 않은 또 다른 과제들은 아래의 기재로부터 통상의 기술자에게 명확하게 이해될 수 있을 것이다.
본원의 제 1 측면은, 기재; 상기 기재 상에 형성되는 다공성 구조층; 및 상기 다공성 구조층 상에 형성되며, 하이드로겔을 포함하는 하이드로겔 층을 포함하는, 복합 접착 구조체를 제공한다.
본원의 제 2 측면은, 제 1 측면에 따른 복합 접착 구조체를 포함하는, 의료 기기를 제공한다.
본원의 제 3 측면은, (a) 기재 상에 다공성 구조층을 형성하는 단계; 및 (b) 상기 다공성 구조층 상에 하이드로겔 층을 형성하는 단계를 포함하는, 복합 접착 구조체의 제조 방법을 제공한다.
본원의 구현예들에 따른 복합 접착 구조체는 상기 다공성 구조층 및 상기 하이드로겔 사이의 강한 접착력으로 인하여, 생체 조직에 대한 접착력이 강화되고 생체에 적용되었을 때 이동이 방지되는 효과가 있다.

본원의 일 구현예에 있어서, 상기 의료용 소재의 상기 하이드로겔의 면적 및 두께에 대한 팽윤비는 평평한 기판에 형성되는 하이드로겔과 비교하여 약 3 배 이상 감소하는 것일 수 있다. 본원의 일 구현예에 있어서, 상기 의료용 소재의 상기 하이드로겔의 현저히 감소하는 팽윤비로 인하여, 팽윤으로 인한 압축 응력에 의해 발생할 수 있는 하이드로겔의 주름, 버클(buckle), 및 박리 현상이 방지될 수 있다.

본원의 일 구현예에 있어서, 상기 하이드로겔은 폴리비닐알코올(poly (vinyl alcohol); PVA); 알지네이트(Alg), 폴리에틸렌글리콜(PEG), 키토산(Chitosan), 젤라틴(Gelatin), 폴리아크릴산(PAAc), 폴리아크릴아마이드(PAM), 폴리나이팜(PNIPAM), 아가(Agar), 폴리(2-아크릴아미도-2-메틸프로판술폰산)(PAMPS), 폴리비닐피롤리돈, 폴리에틸렌옥사이드, 메틸비닐에테르무수말레인산공중합체, 이소부틸렌무수말레인산공중합체, 메타아크릴산아크릴산부틸공중합체, 메톡시에틸렌무수말레인산공중합체, 폴리우레탄, 히알루론산, 알긴산, 도파민, 셀룰로오스, 콜라겐, 및 이들의 유도체에서 선택되는 하나 이상의 친수성 고분자를 포함하는 것일 수 있으나, 이에 제한되지 않을 수 있다.

이하, 본원에 대하여 실시예를 이용하여 좀더 구체적으로 설명하지만, 하기 실시예는 본원의 이해를 돕기 위하여 예시하는 것일 뿐, 본원의 내용이 하기 실시예에 한정되는 것은 아니다.
[실시예]
1. 재료 및 방법
1.1 재료
(하이드로겔 등의 접착제와) 견고하게 맞물리기 위한 나노 연결부(robust interlocking nanoconnector; RINC) 합성을 위한 시약은 트리에탄올아민(triethanolamine; TEA), 나트륨 살리실산염(sodium salicylate; NaSal), 세틸트리메틸암모늄 클로라이드 용액(cetyltrimethylammonium chloride; CTAC), 및 테트라에틸 오르토실리케이트(tetraethyl orthosilicate; TEOS)이다. 하이드로겔 전구체 제조를 위해, 아크릴산(acrylic acid; AAc), 폴리(비닐 알코올)(poly(vinyl alcohol) (PVA); Mw = 146,000 내지 186,000), 폴리(에틸렌 글리콜) 디메타크릴레이트(poly(ethylene glycol) dimethacrylate; PEGDMA), 아크릴산 N-하이드록시석신이미드 에스테르(acrylic acid N-hydroxysuccinimide ester; AAc-NHS ester), 및 α-케토글루타르산(α-ketoglutaric acid)을 사용하였다.
1.2 다양한 기재 상에서 RINC 합성
실리콘 웨이퍼, 니티놀 와이어, 니티놀 스텐트 및 유리에 RINC를 합성하기 전, 아세톤과 에탄올을 순서대로 초음파 처리하여 세척하고, 탈이온수로 헹군 후 오븐에서 건조하였다. 마찬가지로, 실리콘 멤브레인 및 PTFE 필름은 이소프로필 알코올(IPA)로 초음파 처리하고, 탈이온수로 헹군 후 오븐 건조하였다. 세척된 모든 기재는 RINC 합성 직전에 40초 동안 O2 플라즈마 처리를 거쳤다.
1.3 실리콘 웨이퍼, 니티놀 와이어 및 니티놀 스텐트 상에서 일반적인 RINC 합성
RINC의 일반적인 합성에서는 272 mg의 TEA를 80℃, 95 mL의 탈이온수에서 교반하면서 용해시킨 후, 다양한 기재(실리콘 웨이퍼, 니티놀 와이어, 및 니티놀 스텐트)를 투입하였다. 이 용액에 NaSal(1136 mg), CTAC(5.56 mL), TEOS(10 mL)를 첨가하여 교반하였고, 생성된 RINC 코팅된 기재를 에탄올로 여러 번 헹구어 잔류 반응물을 제거하였다.
1.4 실리콘 웨이퍼에서 RINC의 기공 직경 및 두께 제어
실리콘 웨이퍼에서 RINC의 기공 직경과 두께는 특정 단계의 지속 시간이나 일반적인 합성 공정 전반에 걸쳐 사용되는 시약의 양을 조정하여 제어되었다. 구체적으로, TEOS 합성 시간을 3 분에서 3 시간으로 변경함으로써 기공 두께를 5 nm에서 500 nm로 조작하였다.
1.5 하이드로겔 전구체의 제조
700 mg의 PVA를 90℃의 6.6 mL의 탈이온수에 혼합하고, 3.3 mL의 AAc, 20 mg의 α-케토글루타레이트(α-ketoglutarate), 4.55 uL의 PEGDMA 및 30 mg의 AAc-NHS 에스테르를 첨가하였다. 마지막으로, 혼합물을 완전히 교반하고 자외선(UV) 빛을 피하여 보관하였다.
1.6 기재 상에 하이드로겔 전구체 적용
하이드로겔 딥코팅(dip coating)
실리콘 웨이퍼, 니티놀 와이어, 및 니티놀 스텐트를 13 mm min-1의 속도에서 딥 코팅법(dip-coating)을 통해 하이드로겔로 코팅하고, UV 챔버(365 nm, 15 W)에서 30분간 경화하여, 하이드로겔이 코팅된 매끈하고 평평한 면(hydrogel-coated surface; HFLAT) 및 하이드로겔이 함침되어 견고하게 맞물린 나노 연결부(hydrogel-impregnated robust interlocking nanoconnector; HiRINC) 코팅을 제조하였다.
하이드로겔의 캐스트 코팅 (Cast coating)
건조 상태에서 랩 전단 테스트를 위해 PDMS 몰드를 15 × 15 mm2 실리콘 웨이퍼 중앙에 놓고, PDMS 몰드의 구멍을 350 μL의 하이드로겔 전구체로 채웠다. 하이드로겔 전구체의 캐스팅 후, 또 다른 15 × 15 mm2 실리콘 웨이퍼를 PDMS 몰드 상에 부드럽게 배치하여 10 × 10 mm2의 정사각형 모양의 중첩 영역을 형성한 후, UV 램프(365 nm, 15 W 전력)에서 1 시간 동안 경화시켰다.
박리 테스트 샘플은 15 × 35 mm2 PDMS 몰드에 하이드로겔 전구체를 캐스팅한 후, 15 × 30 mm2 실리콘 웨이퍼를 몰드 가장자리에 배치하여 10 × 30 mm2의 직사각형 중첩 영역을 형성하였다. 실리콘 웨이퍼를 배치한 후, UV(365 nm, 15 W 전력)를 사전 경화된 90개 박리 테스트 샘플에 1 시간 동안 노출시켰다.
1.7 특성평가를 위한 계면 강도 및 접착력 측정
계면 강도 및 접착력은 디지털 힘 게이지(ESM303, Mark-10) 및 힘 테스트 스탠드(F305-EM, Mark-10)를 갖춘 기계적 테스트 시스템을 이용하여 측정하였다. 계면 강도 및 이동 변위(travel displacement)는 13 mm min-1의 변형율(strain rate)에서 기록되었다. 접착 강도 및 이동 변위는 50 mm min-1의 변형 속도에서 기록되었다.
1.8 팽윤 (swelling) 시간 접착 테스트
팽윤에 의한 랩 전단 강도 감소에 대한 나노구조의 영향을 비교하기 위해, 랩 전단 테스트 샘플은 미리 정해진 팽윤 시간 동안 탈이온수에서 팽윤을 겪었다. 각 팽윤 기간 후에 랩 전단 강도를 측정하여 다양한 시간 간격을 제외하고 모든 조건을 일관되게 유지하였다.
1.9 생체 외 랩 전단 테스트
하이드로겔 및 돼지 조직 사이의 강한 접착력을 입증하기 위해, 랩 전단 테스트 전, HFLAT 및 HiRINC의 하이드로겔 표면을 조직에 30 초 동안 1 kPa의 부드러운 압력으로 적용하였다.
1.10 접착력 계면 약화(fatigue) 시험
계면 약화 임계값을 평가하기 위해 동일한 실험 설정을 사용하여 반복 하중(힘 제어 모드)에서 랩 전단 테스트를 수행하였다. 여러 사이클에 걸쳐 수행된 랩 전단 테스트에서, 순환 랩 전단 강도 F는 N 사이클(N ≤ 100) 동안 하이드로겔 코팅에 적용되었으며, t로 표시되는 계면 균열 확장이 기록되었다.
1.11 크로스 컷(Cross cut) 테스트
코팅의 접착 강도는 하이드로겔 코팅된 기재에 블레이드를 사용하여 평가되었다. 블레이드를 사용하여 코팅된 기재 상에 2세트의 십자형 스크래치를 생성했으며, 한 방향으로 3개, 및 수직 방향으로 3개 스크래치를 만들어 십자형 패턴을 만들었다. 팽윤 조건을 위해 하이드로겔로 코팅된 기재를 탈이온수에 1 시간 동안 담가 두었다. 마지막으로, 테이프 조각(스카치 테이프, 3M)을 긁힌 샘플에 붙이고 5초 이내에 꾸준히 제거한 후, 1 kPa의 힘을 가하고 180° 방향 각도로 당겼다.
1.12 안티 팽윤(anti swelling) 테스트
실리콘 웨이퍼 상에 경화된 하이드로겔(Φ = 10mm, 0.5T)을 제조한 후, 샘플을 실온의 탈이온수에 담가 하이드로겔의 팽윤 성능을 테스트하였다. 팽윤비(swelling ratio; SR)는 하기 식으로 계산하였다: 팽윤비(%)=(Wt-W0)/W0 × 100(%)
1.13 생체 내 스텐트 접착력 테스트
쥐 모델의 생체 내 랩 전단 테스트를 위해, 스텐트를 식도에 도입하기 전 세 그룹의 스텐트를 끈으로 묶었다. 끈을 기계적 시험 장치에 묶은 후, 전술한 방법으로 랩 전단 테스트를 수행하였다. 활용된 스텐트를 회수하고, SEM을 이용하여 각 스텐트 그룹의 잔여 하이드로겔 코팅 이미지를 캡쳐하였다.
2. 결과
2.1 접착 특성
본 발명은 '견고하게 맞물리기 위한 나노 연결부(robust interlocking nanoconnector; RINC)'라고 불리는 결함이 없는 다공성 구조층을 가지는 나노필름의 접착 특성에 대한 조사 결과를 제시한다. 'RINC'의 독특한 나노구조를 활용하여, 하이드로겔과의 접착을 향상시켜, 하이드로겔-기재 접착을 향상시키는 혁신적인 접근 방식을 선보이는 복합체인 '하이드로겔이 함침되어 견고하게 맞물린 나노 연결부(hydrogel-impregnated robust interlocking nanoconnector; HiRINC)'를 탄생시켰다. 고배율 주사전자현미경(scanning electron microscope; SEM) 이미지(도 2의 b)는 'RINC'의 다공성 구조를 보여주고, 약 150 nm의 나노필름의 일관된 두께를 자세히 보여주어, 하이드로겔 통합(integration)에 중요한 기공(pore)의 존재를 확인한다. 하이드로겔을 적용하면 'RINC'가 'HiRINC'로 변환된다. 이 과정은 스핀 코팅(spin-coating), 몰드 캐스팅(mold-casting) 또는 딥 코팅(dip-coating)과 같은 방법을 통해 수행되며, 도 2의 c에 표시된 바와 같이 투명하고 부드러운 하이드로겔 층을 생성한다. 단면도는 하이드로겔이 RINC의 기공으로 광범위하게 침투하는 것을 강조하며, 하이드로겔과 다공성 실리카 나노필름 사이의 직접적인 접촉과 기계적 맞물림을 보여준다.
3000 cm-1 내지 3700 cm-1 범위의 하이드록실(OH) 결합 영역에서 FTIR 분광학 분석(도 2의 d)을 통해 하이드로겔 내 결합을 정량적으로 평가할 수 있다. 본 발명자들은 3200 cm-1에서 하이드로겔 내의 분자 내 결합 및 3400 cm-1에서 분자간 하이드로겔-실라놀(hydrogel-silanol) 결합을 주목하였다. 평평한 실리콘 웨이퍼 상의 하이드로겔로 구성된 '하이드로겔이 코팅된 매끈하고 평평한 면(hydrogel-coated surface; HFLAT)'로 불리는 샘플은 3200 cm-1에서 더 높은 적분 면적(15.58)을 나타냈는데, 이는 하이드로겔 내의 강한 분자 내 결합을 나타낸다. 대조적으로, 3400 cm-1에서 적분 면적은 더 낮았으며 (4.56), 이는 '매끈하고 평평한 면(FLAT)'로 표시되는, 하이드로겔 및 처리되지 않은 실리콘 웨이퍼 사이의 분자간 결합이 더 약하다는 것을 의미한다. 이러한 관찰은 하이드로겔-실리콘 계면에서 제한된 상호작용을 암시한다. 대조적으로, HiRINC 샘플은 3200 cm-1에서 약간 더 낮은 적분 면적(13.22)을 나타냈는데, 이는 분자 내 결합이 약간 감소했음을 의미한다. 그러나, 3400 cm-1에서 적분 면적(11.48)의 현저한 증가가 관찰되었는데, 이는 분자간 하이드로겔-실라놀 결합이 더 강해지고, 따라서 실리콘 웨이퍼 상의 다공성 실리카 나노구조와 상호작용이 강화되었음을 나타낸다.
샘플의 접착 특성을 평가하기 위하여, 계면 강도, 접착 강도, 및 접착 일(work of adhesion)(ΔE)에 집중하였다. 이러한 특성은 90° 박리 테스트, 및 도 9의 a 및 b에 설명된 바와 같이 랩 전단(lap-shear) 테스트를 사용하여 평가하였다. 도 3의 a는 두 가지 서로 다른 표면 유형(HFLAT 및 HiRINC)에 대한 박리 테스트로부터의 힘 대 변위(force versus displacement) 데이터를 나타낸다. HFLAT로 불리는 하이드로겔 코팅된 베어(bare) 실리콘 웨이퍼는 지속적으로 낮은 힘 수준을 보여, 평평한 표면과의 약한 상호 작용을 나타냈다. 대조적으로, 다공성 실리카 나노필름 상에 하이드로겔 코팅을 적용한 HiRINC는 더 높은 힘 수준을 나타내어, 다공성 구조에 기인하는 강한 접착력을 나타냈다. 랩 전단 테스트는 하이드로겔과 각 표면 유형 사이의 접착 강도를 더욱 자세히 설명한다. HFLAT의 경우, 최대 힘 지점에서 접착 실패가 발생했는데, 이는 하이드로겔과 기재 사이의 분리(detachment)를 의미한다. 대조적으로, HiRINC는 더 높은 피크 힘을 달성하고, 경계에서라기 보다는 도 9의 c 및 d에 설명된 바와 같이 하이드로겔 자체 내에서 결합(cohesive) 실패를 나타내어, 더 강한 접착력을 나타냈다. 또한, 도 9의 a 및 b는 HFLAT 및 HiRINC의 응력(stress) 동작을 대조적으로 나타낸다. HiRINC는 HFLAT에 비해 기재 사이에서 하이드로겔이 훨씬 더 많이 늘어나는 강력한 접착력을 보여주었는데, 이는 나노구조 표면이 향상된 전단 저항(shear resistance)을 제공한다는 것을 의미한다. 로 계산되는 접착 일(work of adhesion)은 HFLAT보다 HiRINC에서 힘-변위 곡선 하에서 더 큰 면적을 나타냈으며, 이는 나노구조화된 표면으로부터 하이드로겔을 박리하는데 더 큰 에너지가 필요하고, 따라서 접착력이 더 강하다는 것을 나타낸다. 도 3의 b의 막대 그래프는 HFLAT 및 HiRINC의 계면 강도(interfacial strength), 접착 강도(adhesion strength), 및 접착 일(work of adhesion)을 비교한다. 특히, HiRINC 및 HFLAT의 계면 강도는 각각 7.6 N cm-1 및 0.13 N cm-1로서, HiRINC의 계면 강도는 HFLAT의 58 배 이상이다. 이러한 현저한 차이는 접착 강도 및 접착 일에서도 반영되었으며, HiRINC의 접착 강도(145 kPa)는 HFLAT(8.67 kPa)에 비해 16 배 증가하고, HiRINC의 접착 일(1.63 kJ m-2)은 HFLAT(0.025 kJ m-2)에 비해 약 650 배 증가한 것으로 나타나, 나노구조화된 표면의 우수한 접착력을 강조한다.
도 10에 자세히 설명된 바와 같이, 평평한 표면과 달리 독특한 다공성 디자인은 기계적 결합을 촉진하여 RINC의 강력한 접착력에 기여하는 것으로 보인다. 이 가설은 다양한 상업용 테이프(캡톤(Kapton), 마스킹, 스카치 및 양면 테이프)를 사용한 90° 박리 테스트를 통해 추가로 뒷받침되었으며, 모든 테스트에서 RINC가 더 큰 계면 강도를 나타냈다. 본 발명은 접착 기술 응용 분야에서 다른 실리카 기반 재료와 구별되는 RINC의 우수한 접착 품질을 강조한다.
특히, HFLAT 및 HiRINC의 랩 전단 약화 강도(lap-shear fatigue strength)를 조사하여 반복 응력(stress) 하에서 이 접착력의 내구성을 구체적으로 평가하였다. 본 발명자들은 여러 사이클의 랩 전단 테스트에 걸쳐 일정한 힘 하에서 재료가 기존 위치로부터 이동하는 정도를 측정했다 (도 3의 c 및 d). HFLAT은 다양한 수준의 적용된 전단 응력 하에서 쉽게 하이드로겔이 탈착되는 반면 (도 3의 c), HiRINC는 하이드로겔과 NANO의 접착력이 반복적인 하중(loading)과 높은 응력 조건에서도 견고하고 안정적으로 유지되어, 내구성과 견고성이 우수하다 (도 3의 d).
2.2 내구성 및 안정성 평가
도 4의 a 및 b는 각각 초기 상태 및 팽윤 상태의 HFLAT 및 HiRINC에 대한 결과를 보여준다. 패널 Ⅰ는 코팅의 초기 상태를 보여준다. 패널 Ⅱ는 크로스 해칭(cross-hatching) 및 테이프 적용 후의 샘플을 보여주고, 패널 Ⅲ 및 Ⅳ는 각각 박리 후 기재 및 테이프를 보여준다. HFLAT의 경우, 하이드로겔 층은 도 4의 a에서 육안으로 확인 가능한 제거로 입증되는 상당한 박리를 보이며, 도 4의 b에 나타난 바와 같이 팽윤 상태에서 일관되게 유지되는 약한 접착력을 강조하여 박리를 일으키고 평가를 불가능하게 만든다 (N/A). 대조적으로, HiRINC는 초기 상태 및 팽윤 상태 모두에서 테이프 제거 동안 결합(cohesion)을 유지함으로써 우수한 접착 특성을 나타내며 다공성 실리카 나노필름 통합에 의해 강화된 견고한 결합을 강조한다.
도 11의 a 내지 d은 시간 경과에 따른 직경(D0), 두께(T0) 및 무게(W0)의 변화를 추적하여 HFLAT 및 HiRINC 두 표면의 팽윤 동역학을 자세히 설명한다. 도 11의 d의 요약은 HFLAT 및 HiRINC 사이의 팽윤 비율을 대조적으로 보여준다. HFLAT는 면적 및 두께에서 최대 600%, 중량에서 최대 2000%에 달하는 뚜렷한 팽윤 비율 증가를 나타내어, 접착 계면에서 상당한 수분 흡수 및 박리를 유발한다. 대조적으로, HiRINC는 면적 및 두께에서 200% 증가, 중량에서 700% 증가하여 더 낮은 팽윤 비율을 유지하므로, 보다 제어된 팽윤 반응을 나타낸다.
HiRINC의 조직 접착 가능성을 평가하기 위하여 생체 외 돼지 위 조직을 사용하여 정량적 접착 강도 테스트를 수행하였다 (도 5의 a 내지 c). 도 5a에 나타난 결과는 하이드로겔과 조직 사이에 65 kPa의 인상적인 접착 강도를 갖는 HiRINC의 우수한 접착 성능을 보여준다. 대조적으로, 조직에 직접 부착된 하이드로겔을 포함하는 HFLAT는 5 kPa의 현저히 낮은 랩 전단 강도를 나타냈으며, 이는 표면에서의 접착 실패를 의미한다. 도 5의 b 및 c은 돼지 조직에 대한 실제 접착 테스트의 사진 증거를 제공하여 이러한 결과를 더욱 검증한다. 점선 원으로 표시된, 기재에서 하이드로겔 층의 분리는 리프팅(lifting) 과정에서 HFLAT의 즉각적인 접착 실패를 나타낸다. 대조적으로, HiRINC는 힘이 가해졌을 때 견고한 접착력을 유지하고, 하이드로겔은 조직으로부터 리프팅될 때에도 기재에 부착된 상태를 유지한다.
2.3 생물의학적 적용
에너지 분산형 X선 분광법(Energy-dispersive X-ray spectroscopy; EDS) 분석을 통해 각각 다공성 실리카 나노필름 및 하이드로겔 층에 해당하는 실리콘(Si) 및 탄소(C) 원소의 존재를 확인하였다. 도 6의 a는 RINC-스텐트 전체의 Si 원소 분포를 강조하며, SEM 이미지는 다공성 구조를 보여주며, 도 6의 b에 나타난 스텐트의 SEM 이미지와 함께 표시되는 C 매핑(mapping)은 HiRINC-스텐트에 하이드로겔 코팅이 존재하는지 확인한다. 도 6의 c 및 d는 HFLAT-NiTi 와이어 및 HiRINC-NiTi 와이어를 모두 보여주는 니티놀(NiTi) 와이어 상의 하이드로겔 층의 대조적인 동작을 설명한다. HFLAT-NiTi 와이어의 경우, 왼쪽 이미지는 건조 상태의 하이드로겔 층을 묘사하는 반면, 오른쪽 이미지는 물에 담긴 후 현저한 팽윤과 후속 박리를 보여 팽윤 상태에서 접착력이 부적절함을 나타낸다. 대조적으로, HiRINC-NiTi 와이어는 눈에 띄는 박리 없이 건조 상태 및 팽윤 상태 모두에서 하이드로겔 층의 온전함을 유지한다. 이는 HiRINC가 팽윤 상태에서도 강력한 접착력을 보이기 때문에 HFLAT와 대조되며, 이는 하이드로겔과 NiTi 와이어의 결합을 강화하는 나노구조의 역할을 시사한다. 또한, HFLAT-스텐트는 점선 윤곽선으로 표시된 바와 같이, 주름, 버클 및 박리를 특징으로 하는 현저한 하이드로겔 팽윤을 보여 물에 담갔을 때 불안정함을 시사한다 (도 6의 e). 대조적으로, HiRINC-스텐트는 팽윤을 최소화하면서 하이드로겔이 스텐트 구조에 효과적으로 부착되는 것을 보여준다 (도 6의 f). 이는 팽윤 상태에서도 하이드로겔이 스텐트에 강력하고 안정적으로 통합된다는 것을 나타낸다. 이러한 발견은 물 속에서 HFLAT-스텐트와 HiRINC-스텐트의 독특한 거동을 강조하며, 나노구조화된 디자인에 의해 제공되는 향상된 안정성과 통합성을 강조한다.
생체 내 접착 강도는 투시 유도 및 실시간 정량 평가를 위한 디지털 힘 게이지가 장착된 기계적 테스트 시스템을 사용하여 측정되었다 (도 7 의 a). 투시 유도에 따라 대조군(n=3), HFLAT-스텐트(n=3) 및 HiRINC-스텐트(n=3)를 식도에 삽입하였다. 삽입 전 스텐트의 끝 부분에 수술용 실을 부착하고, 입 밖으로 튀어나온 실의 끝 부분을 접착력을 측정하기 위한 힘 게이지에 연결하였다. 측면 및 상단 뷰가 제공되어 명확한 시각화를 제공한다. 수술용 실을 당기면 힘-변위 그래프가 생성된다. 수득된 평활화된 최대 힘 값(도 7 의 b)을 비교하면 HiRINC는 대조군(코팅되지 않은) 스텐트 및 HFLAT 코팅된 스텐트에 비해 가장 높은 값인, 약 1000 kPa를 나타냈다. 이러한 결과는 HiRINC 코팅 스텐트가 코팅되지 않은 스텐트 및 HFLAT 코팅 스텐트에 비해 식도 조직에 대한 접착력이 우수하다는 것을 시사한다. 도 12의 a 내지 c는 생체 내 랩 전단 테스트를 거친 후 베어 스텐트(FLAT-스텐트), HFLAT-스텐트 및 HiRINC-스텐트의 표면 형태를 비교하는 일련의 SEM 이미지를 보여준다. HFLAT-스텐트의 SEM 이미지는 테스트 후 넓은 영역에 걸쳐 하이드로겔의 박리를 나타낸다. 대조적으로, HiRINC-스텐트는 낮은 배율에서 랩 전단 테스트의 기계적 응력 후에도 대부분의 영역에서 하이드로겔의 일관된 코팅을 나타낸다. 고배율 이미지에서는 하이드로겔이 분리된 위치에 여전히 나노구조가 관찰된다. 이러한 통합은 하이드로겔이 스텐트에 더 강하게 접착됨을 시사하며, 잠재적으로 생물의학적 맥락에서 향상된 성능과 상관관계가 있다.
2.4 생체 내(In Vivo) 연구
4주간의 추적 기간 동안 대조군에서는 하루 일찍 5마리(83.3%)의 쥐에서 스텐트 이동이 발생하였다. 한 마리의 쥐에서 초기에 완전한 스텐트 이동이 발견된 것을 제외하고, 방사선 검사 결과 마지막 추적 관찰에서 이 그룹의 2마리(33.3%)의 쥐에서는 스텐트가 부분적으로 이동했고, 2마리(33.3%)의 쥐에서는 완전히 이동한 것으로 나타났다. 보다 구체적으로, 4마리의 쥐는 3일 또는 14일에 부분적인 스텐트 이동이 발생했고, 그 중 2마리는 4주차에 완전한 스텐트 이동이 진행되었다. HFLAT-스텐트 그룹에서는 4주차에 2마리(2/6, 33.3%)의 쥐에서 부분 이동이 발생하였다. 대조적으로, HiRINC-스텐트 그룹에서는 부분적 또는 완전한 스텐트 이동이 관찰되지 않았다. 대조군의 스텐트 이동 발생률은 HiRINC-스텐트 그룹(P = 0.014)에 비해 현저히 높았다. HFLAT-스텐트 군의 발생률은 HiRINC-스텐트 군에 비해 높고, 대조군에 비해 낮았으나 통계적으로 유의한 차이는 나타나지 않았다 (모두 P=0.439).
도 13의 a 내지 f은 반응 조건을 조절하여 다공성 구조층의 기공 직경 및 두께를 다양하게 조절할 수 있으며, 이에 따른 접착력의 조절이 가능함을 보여준다. 기공의 직경 및 두께를 조절한 RINC의 SEM 이미지를 표면(도 13의 a 및 d); 단면(도 13의 b 및 e)에서 관찰한 것이며, 이에 따른 랩 전단 값을 측정함으로써(도 13의 c 및 f) 반응 조건을 통한 접착력 조절이 가능함을 보여준다. 도 14의 a 내지 d는 PTFE 코팅 니티놀 스텐트 표면 상에 나노구조를 다양하게 적용하는 방법을 보여준다. 도 14의 a 및 c는 눈에 띄게 균일하고 부드러운 질감을 지닌 베어 PTFE 표면을 보여준다. 도 14의 b 및 d는 합성 후 나노 구조를 보여준다.
도 8의 a는 쥐의 식도에 스텐트, 하이드로겔이 코팅된 HFLAT-스텐트, HiRINC-스텐트를 설치하여 4주간 추적한 스텐트 이동 결과에 대한 개요도이며, 도 8의 b은 4주간의 엑스선 촬영을 통한 비교 결과이다. 대조군인 스텐트에서는 5마리(83.3%)의 쥐에서 스텐트 이동이 발생했으며, HFLAT 그룹에서는 4주차에 2마리(33.3%)의 쥐에서 부분 이동이 발생했다. 대조적으로, HiRINC-스텐트에서는 부분적 또는 완전한 스텐트 이동이 관찰되지 않았다.
3. 논의
접착 강화제 RINC는 하이드로겔 적용을 통해 조직과 스텐트 사이의 갭(gap)을 가교(bridge)하고, 이동 방지를 가능하게 하며, 쥐 식도 내에서 비침습적 장기 고정을 가능하게 함으로써 이러한 문제를 해결한다. RINC는 하이드로겔 함침 후 기계적 맞물림과 함께 수소 결합 및 반데르발스 힘을 통한 물리적 가교(cross-linking)를 나타내어, 하이드로겔에 대한 견고한 접착을 보장한다. 이식 후, 하이드로겔은 각각 수소 및 아마이드(amide) 결합을 통해 물리적 및 공유 가교를 거쳐, 생물학적 조직에 대한 강력한 접착력을 강화한다. 이 전략은 종래의 기계적 기반 스텐트 고정 접근법과는 다르다. 본 발명의 공정은 상업용 의료용 기재 상에 구멍이 난 나노구조를 덮은 후, 간단한 하이드로겔 코팅을 적용하는 것을 포함한다. 이 전략은 조직과 하이드로겔 사이의 즉각적인 결합을 보장하여, 스텐트 삽입 후에도 비침습적이고 수술이 필요 없는 고정을 가능하게 한다. 또한, 수술적 봉합이 없기 때문에 조직의 염증으로 이어질 수 있는 추가 부작용을 예방할 수 있다. 하이드로겔은 접착력을 강화할 뿐만 아니라 기계적 탄력성, 높은 생체 적합성, 섬유증 세포 과증식 최소화, 및 항염증 반응도 제공한다.
HiRINC의 설계에는 임상 적용을 가능하게 하기 위하여 몇 가지 고려 사항이 이루어졌다. 클립, 스텐트 고정, 및 바브을 사용한 초기 노력은 스텐트 고정에서 유망한 결과를 나타냈다. 그러나, 조직 손상과 관련된 수술 합병증 및 기술적으로 집약적인 제조 공정, 특히 환자별 요구 사항으로 인해 임상에서 폭넓게 적용되지 못하였다. HiRINC는 매끄러운 표면을 강화하는 천공된 나노구조를 통해 스텐트 이동 위험을 완화하고, 계면에서 기계적 연결(interlocking)을 제공한다. 상기 나노구조의 깊은 기공 채널에 유연한 하이드로겔을 통합하면 안정성이 더욱 향상된다. 이 접근 방식은 HiRINC에 강화된 접착 안정성과 내구성을 부여하여 박리, 전단 응력, 팽윤(swelling), 탈팽윤(deswelling) 및 굽힘을 포함한 문제에 효과적으로 저항한다.
4. 다양한 하이드로겔의 제조
4.1 폴리도파민(polydopamine; PDA) 기반 하이드로겔 제조
도파민(Dopamine hydrochloride) 30mg를 1M NaOH 수용액 22 mL과 함께 첨가하여 300 rpm에서 20 분간 교반하였다. 이후, 아크릴아미드(Acrylamide; AAm) 10 g를 첨가하고 10 분간 추가 교반하였다. 개시제로서 과황산암모늄(ammonium persulfate; APS) 1000 mg, 가교제로서 N,N-메틸렌비스아크릴아마이드 24 mg, 가교 촉매를 위한 트리에탄올아민(triethanolamine; TEA) 20 μL를 순차적으로 첨가하고 추가 교반하여 조성을 균일화하였다. 완성된 전구체 조성물은 냉장 보관하였으며, 사용 직전 자외선 혹은 온도 조건에 따라 가교하여 하이드로겔로 형성하였다.
4.2 키토산(Chitosan) 기반 하이드로겔 제조
키토산 400 mg과 증류수 16 mL를 첨가한 후, 아크릴산(AAc) 16 mL를 첨가하여 교반하였다. 이후, 테레프탈알데하이드(terephthalaldehyde; TPD) 240 mg를 첨가하고 상온에서 교반하였다. 이후, 폴리에틸렌글리콜(PEG) 800 mg를 첨가하여 추가 교반하였다. 이후, 과황산암모늄(APS) 400 mg 및 가교제인 N,N-메틸렌비스아크릴아마이드 200 mg를 순차적으로 첨가한 후 교반하였다. 제조된 하이드로겔 전구체 용액은 자외선 혹은 온도 조건에 따라 가교하여 하이드로겔로 형성하였다.
5. 다양한 하이드로겔의 적용 및 이에 따른 평가
5.1 나노구조 유무에 따른 폴리도파민(PDA) 기반 하이드로겔의 크로스컷(Cross-cut) 접착 성능 비교
15Х15 mm2 크기의 베어(bare) 실리콘(Si) 웨이퍼 기판에 졸-겔(sol-gel)법으로 실리카 나노구조를 형성하고, 폴리도파민(PDA) 기반 하이드로겔을 도포한 후, 70℃에서 30 분간 열경화하였다. 또한, 베어 Si 웨이퍼 기판에 실리카 나노구를 형성하지 않고, PDA 하이드로겔을 도포한 후 동일한 조건에서 열경화하였다.
크로스컷 시험(Cross-cut test)을 수행하였다. 블레이드를 이용하여 샘플 표면에 5 mm 간격의 격자 형태로 절개(crosshatch)를 형성하고, 스카치 테이프(scotch tape)를 동일한 압력으로 10 초간 접착한 후, 180° 방향으로 벗겨내어 접착층의 유지 상태를 평가하였다. 그 결과, 나노구조 기판에서는 절단 격자 부위에서도 하이드로겔이 견고하게 유지되며 박리 흔적이 거의 관찰되지 않았다. 반면, bare 기판에서는 절단 부위를 따라 하이드로겔이 광범위하게 박리되었다.
5.2 나노구조 유무에 따른 키토산 하이드로겔의 크로스컷 접착 성능 비교
키토산 기반 하이드로겔을 실리카 나노구조 유무에 따라 Si 웨이퍼에 도포하고 75℃에서 30 분간 열경화한 후 크로스컷 시험을 수행하였다. 실리카 나노구조 기판에서는 절단면에서 하이드로겔의 유지력이 우수했으며, 베어 기판에서는 절단선을 따라 명확한 박리 현상이 관찰되었다.
5.3 PDA 하이드로겔의 랩 전단 시험(lap shear test)에 따른 기계적 접착력 분석
PDA 하이드로겔을 베어(bare) 및 실리카 나노구조 Si 웨이퍼에 각각 형성한 후, 랩 전단 시험(lap shear test)을 수행하였다. 랩 전단 시험은 경화된 하이드로겔이 코팅된 두 기판을 1cm × 1cm 면적으로 중첩시킨 후, 만능 인장 시험기(universal testing machine)를 이용하여 전단 방향으로 분당 50 mm의 속도로 하중을 가해 파괴 하중과 변위를 측정하였다. PDA 함량이 상대적으로 낮은 조건(0.12 w/w% 및 0.25 w/w%)에서도, 실리카 나노구조 표면에서 최대 전단 하중이 각각 약 2.8 배(0.12 w/w%) 및 약 6 배(0.25 w/w%) 이상 증가하였고, 파괴 변위 또한 연장된 것을 확인하였다. PDA 함량이 높을수록 접착력과 에너지 흡수량이 더욱 향상되며, 실리카 나노구조의 기계적 보강 효과가 뚜렷하게 나타났다 (도 15의 a 및 b).
5.4 PDA 하이드로겔의 박리 시험(peel test)에 따른 박리 저항력 분석
PDA 기반 하이드로겔을 베어 Si 웨이퍼 및 실리카 나노구조 Si 웨이퍼 표면에 각각 형성한 후, 박리 시험(peel test)을 수행하였다. 1.5cm × 1cm 면적으로 접착된 하이드로겔 시편에 대해 전단 방향과 수직으로 13 mm/min의 속도로 인장하여 박리 저항력을 평가하였다. 그 결과, 도 16에 도시된 바와 같이, 나노구조 기판에서는 베어 기판에 비해 피크 박리 하중이 약 13 배 이상 증가하였으며, 박리 전 구간에 걸쳐 높은 하중이 지속되고 점진적으로 저하되는 하중-변위 곡선이 나타났다. 이는 나노구조 표면에 의한 기계적 맞물림 및 계면 결합력 향상에 기인한 것이다.
5.5 키토산 하이드로겔의 랩 전단 시험(lap shear test) 및 박리 시험(peel test) 분석
키토산 기반 하이드로겔을 베어 Si 웨이퍼 및 실리카 나노구조 Si 웨이퍼 표면에 각각 형성하였다. 랩 전단 시험은 1cm × 1cm 면적의 시편에 대해 50mm/min의 속도로 수행하였으며, 박리 시험은 1.5cm × 1cm 면적에 대해 13mm/min 속도로 박리 하중을 측정하였다. 그 결과, 도 17의 a 및 b에 도시된 바와 같이, 나노구조 기판에서는 베어 기판 대비 최대 전단 하중이 약 6 배 증가하였고, 파괴 변위도 개선되었다. 박리 시험에서도 피크 박리 하중이 약 16 배 이상 증가하였으며, 평균 하중이 높은 수준으로 지속되는 곡선이 관찰되었다. 이는 기계적 맞물림 및 계면 결합 향상에 기인한 것이다.
5.6 팽윤 시간에 따른 접착력 비교
PVA-AAc 기반 하이드로겔 전구체를 각각 베어Si 웨이퍼 및 다공성 실리카 나노구조 Si 웨이퍼에 도포하고, UV경화하여 고정하였다. 이후 각 시편을 증류수에 서로 다른 시간(예: 1시간, 6시간, 12시간) 동안 팽윤시킨 후, 랩 전단 시험을 수행하였다. 시험은 1cm×1cm 중첩 면적에서 50 mm/min 속도로 진행하였다. 도 18에 도시된 바와 같이, 실리카 나노구조 기판은 12 시간 이상의 장기 팽윤 후에도 높은 접착력을 유지한 반면, 베어 기판은 접착력이 빠르게 감소하였다. 이는 기계적 맞물림과 계면 결합 효과가 수중 환경에서도 계면 안정성 향상에 기여함을 시사한다.
5.7 다공성 구조층 내 약물 담지 여부 시각화
형광 물질인 FITC를 베어 Si 웨이퍼 및 실리카 나노구조 Si 웨이퍼에 각각 9000 rpm, 1 분간 스핀코팅하여 도포하였다. 이후 UV 조사를 통해 형광 관찰을 수행한 결과, 베어 기판에서는 형광이 미미하거나 관찰되지 않았으나, 나노구조 기판에서는 형광 신호가 강하게 나타났다. 이는 다공성 구조층(다공성 실리카 나노구조)의 기공 내부로의 형광 분자의 침투 및 고정에 기인한 것으로 판단된다 (도 19).
또한, 약물 담지 및 방출 성능을 평가하기 위하여, PDA 기반 하이드로겔을 0.5 cm의 디스크 형태로 경화시킨 후, 10 mM 로다민 B(Rhodamine B) 용액 500 μL에 1 시간 동안 침지(swelling)하여 약물을 로딩(loading)하였다. 해당 시료를 증류수에 24 시간 침지한 후 촬영한 결과, 초기에는 염료가 겔 내에 균일하게 분포되어 있었으며, 시간이 경과함에 따라 외부 매질로의 방출이 관찰되어 염료가 로딩되어 있지 않은 하이드로겔과 비슷한 형상을 나타냈다. 이는 본 발명의 하이드로겔이 약물 전달 플랫폼으로서 활용 가능함을 시각적으로 입증하는 결과이다 (도 19).
5.8 박막 하이드로겔 코팅의 스프레이 코팅 공정 적용성 평가
1.6에서 사용한 하이드로겔 전구체 용액을 스프레이 방식으로 실리카 나노구조가 형성된 기판(즉, RINC가 합성된 실리콘 웨이퍼) 위에 분사하였다. 분사 거리 약 10 cm, 분사 시간 5초 조건에서 코팅을 수행한 결과, 기판 전면에 균일하고 얇은 하이드로겔 막이 형성되었으며, 또한, 실리카 나노구조 내부로 침투한 것을 확인하였다 (도 20). 스프레이 코팅 조건(압력, 거리, 분사 시간)을 조절함으로써 코팅 두께 및 침투 깊이의 정밀 제어가 가능하였다. 이는 본 발명의 하이드로겔 조성물이 박막 코팅 공정에 적용하기에 적절한 것을 입증한다.
9) 탈착 용액에 따른 하이드로겔 접착력 변화 평가
PVA-AAc 기반 하이드로겔을 실리카 나노구조가 형성된 Si 웨이퍼에 형성하고, 랩 전단 시험을 통해 접착 강도를 측정하였다. 이후 동일한 시편을 0.5M 중탄산나트륨(NaHCO3) 수용액에 침지하여, 하이드로겔의 탈착 반응을 유도하였다. 약 5 분 내지 10 분 이내에 하이드로겔의 접착력이 저하되는 것을 확인하였다. 이는 본 발명의 하이드로겔 조성물이 특정 조건 하에서 탈착 가능함을 의미한다 (도 21의 a 및 b).

  10. 제 1 항에 따른 복합 접착 구조체를 포함하는, 의료 기기.
  11. 제 10 항에 있어서,
    상기 의료 기기는 체내 삽입형 소자인 것인, 의료 기기.
  12. 제 10 항에 있어서,
    상기 의료 기기는 비혈관용 스텐트, 혈관용 스텐트, 인공 보철물, 카테터, 의학용 드레싱, 필름, 섬유, 메쉬, 분말, 마이크로스피어, 시트, 스펀지, 폼, 봉합 앵커링 기기, 도관, 플레이트 및 스크류, 약물 전달 기기, 또는 유착 방지막인 것인, 의료 기기.
  13. (a) 기재 상에 다공성 구조층을 형성하는 단계; 및
    (b) 상기 다공성 구조층 상에 하이드로겔 층을 형성하는 단계
    를 포함하는, 복합 접착 구조체의 제조 방법.
  14. 제 13 항에 있어서,
    상기 단계 (a)는, 상기 기재를 표면 처리하는 것; 또는 상기 기재의 표면에 코팅형 다공성 구조층을 형성하는 것을 포함하는 것인, 복합 접착 구조체의 제조 방법.
  15. 제 13 항에 있어서,
    상기 하이드로겔은 폴리비닐알코올(poly (vinyl alcohol); PVA); 알지네이트(Alg), 폴리에틸렌글리콜(PEG), 키토산(Chitosan), 젤라틴(Gelatin), 폴리아크릴산(PAAc), 폴리아크릴아마이드(PAM), 폴리나이팜(PNIPAM), 아가(Agar), 폴리(2-아크릴아미도-2-메틸프로판술폰산)(PAMPS), 폴리비닐피롤리돈, 폴리에틸렌옥사이드, 메틸비닐에테르무수말레인산공중합체, 이소부틸렌무수말레인산공중합체, 메타아크릴산아크릴산부틸공중합체, 메톡시에틸렌무수말레인산공중합체, 폴리우레탄, 히알루론산, 알긴산, 도파민, 셀룰로오스, 콜라겐, 및 이들의 유도체에서 선택되는 하나 이상의 친수성 고분자를 포함하는 것인, 복합 접착 구조체의 제조 방법.
  16. 제 13 항에 있어서,
    상기 단계 (b)는 딥코팅(dip-coating), 캐스트코팅(cast-coating), 드롭코팅(drop-coating), 스프레이코팅(spray-coating) 또는 스핀코팅(spin-coating)의 방법으로 수행되는 것인, 의료용 소재의 제조 방법.
