Abstract
본 발명은 신규한 GLP-1, GIP, 및 글루카곤 수용체 모두에 활성을 갖는 삼중 활성체의 신장 질환에 대한 예방 또는 치료 용도에 관한 것이다.
Description
본 발명은 GLP-1, GIP, 및 글루카곤 수용체 모두에 활성을 갖는 삼중 활성체 및 이의 용도에 관한 것이다.
GLP-1 (Glucagon-like peptide-1) 및 GIP (Glucose-dependent insulinotropic polypeptide)는 대표적인 위장 호르몬이자 신경 호르몬으로서 음식물 섭취에 따른 혈중 당 농도 조절에 관여하는 물질이다. GLP-1은 음식물 섭취에 자극을 받아 소장에서 분비되는 호르몬으로 혈당 농도 의존적으로 췌장에서의 인슐린 분비를 촉진하고 글루카곤의 분비를 억제하여 혈당 농도를 낮추는 작용을 돕는다.
또한, GLP-1과 더불어 음식섭취에 자극을 받아 분비되는 위장 호르몬의 하나인 GIP는 소장의 K 세포로부터 분비되는 42개 아미노산으로 구성된 호르몬으로서 혈당 농도에 의존적으로 췌장에서의 인슐린 분비를 촉진하고 혈당 농도를 낮추는데 도움을 주는 기능을 수행하며 GLP-1의 활성 증가 효과, 항염증 효과 등이 보고되었다.
글루카곤 (Glucagon)은 췌장에서 분비되는 펩타이드 호르몬으로 전술한 두 물질과 함께 혈중 당 농도 조절 작용에 관여한다. 글루카곤은 약물 치료 또는 질병, 호르몬이나 효소 결핍 등의 원인으로 혈당이 떨어지기 시작하면 췌장에서 생산된다. 글루카곤은 간에 신호하여 글리코겐을 분해하여 글루코스를 방출하도록 유도하고, 혈당 수준을 정상 수준까지 높이는 역할을 한다. 뿐만 아니라, 글루카곤은 혈당 상승효과 이외에 동물과 사람에서의 식욕억제 및 지방세포의 호르몬 민감성 리파제 (hormone sensitive lipase)를 활성화시켜 지방 분해를 촉진 및 에너지대사 (energy expenditure)를 촉진하여 항-비만 효과를 나타냄이 보고되었다.
기존에 알려진 GLP-1 수용체, GIP 수용체, 또는 글루카곤 수용체에 대한 단일 작용제에 비해, 효능 증대나 부작용 개선 등을 위해 상기 수용체에 동시에 작용할 수 있는 물질의 필요성이 최근 대두되었고, GLP-1, GIP, 및 글루카곤 수용체에 작용할 수 있는 펩타이드 및 이의 결합체가 개발되었다 (WO2017-116204; WO2017-116205).
한편, 신장 (콩팥, kidney)은 횡격막의 아래쪽, 배의 뒤에 위치하는 장기로, 사구체와 보먼주머니로 구성된 신소체를 포함하는 네프론, 세뇨관 (근위 세뇨관, 헨레고리, 원위세뇨관, 집합관)으로 이루어져 있다. 신장의 주요 기능은 소변을 만드는 것으로, 노폐물 (요소, 요산, 크레아티닌 등)을 제거하고, 수분과 염분의 양, 전해질 및 산-염기 균형을 조절하며, 인슐린, 글루카곤 등의 여러 호르몬을 분해하거나 대사 시키는 기능도 담당한다. 또한, 포도당 생성 (renal gluconeogenesis), 이용, 재흡수의 기전을 통해 포도당 항상성에도 관여한다.
이와 같은 다양한 기능을 담당하는 신장이 기능을 정상적으로 수행하지 못하거나 신장 구조에 이상이 초래된 것을 신장 질환이라고 한다. 이와 같은 신장 질환의 예로는 급성 신부전, 급성 신장염, 당뇨병성 신부전, 사구체 신염, 만성 신장 질환, 만성 신부전, 세관 장애, 신증후군, 신우신염, 신성 고혈압, 요독증, 고혈압성 신장병, 신장 섬유증, 신장 아밀로이드증, 요독증성 발작, 신장 결석, 수신증, 신장낭종, 신장종양, 다낭성 신장 질환, 급성 신염 증후군, 무증상 요이상, 진통제성 신병증, 신동맥 협착, 및 신경화증 등이 있다.
이러한 신장 질환을 적절히 치료하지 못하여 신장 기능이 저하되거나 신장이 손상될 경우 투석이나 신장 이식이 필요하기 때문에, 치료 시기를 놓치지 않고 신장 질환을 치료하는 것이 필요하지만, 아직까지는 효과적인 신장 질환 치료제가 개발되지 않았다.
효과적이면서도 안전한 신장 질환의 치료제의 개발이 요구된다.
본 발명의 하나의 목적은 GLP-1 (Glucagon-like peptide-1) 수용체, GIP (Glucose-dependent insulinotropic polypeptide) 수용체, 및 글루카곤 수용체에 대해 활성을 갖는 펩타이드를 포함하는 신장 질환 예방 또는 치료용 약학적 조성물을 제공하는 것이다.
본 발명의 다른 하나의 목적은 상기 펩타이드 또는 약학적 조성물을 필요로 하는 개체에 투여하는 단계를 포함하는 신장 질환 예방 또는 치료 방법을 제공하는 것이다.
본 발명의 또 다른 하나의 목적은 상기 펩타이드 또는 약학적 조성물의 신장 질환 예방 또는 치료 용도를 제공하는 것이다.
본 발명의 또 다른 하나의 목적은 신장 질환 예방 또는 치료를 위한 약제를 제공하기 위한 상기 펩타이드 또는 약학적 조성물의 용도를 제공하는 것이다.
본 발명의 GLP-1 (Glucagon-like peptide-1) 수용체, GIP (Glucose-dependent insulinotropic polypeptide) 수용체, 및 글루카곤 수용체에 대해 활성을 갖는 펩타이드는 3개 수용체를 동시에 활성화시킴으로써, 신장 질환에 대한 우수한 예방 또는 치료 효과를 가질 수 있다.

한편, 본 발명에 따른 삼중 활성체는 하기 i) 내지 iii) 중 하나 이상의 활성, 구체적으로 유의한 활성을 보유하는 것을 특징으로 한다:
i) GLP-1 수용체의 활성화; ii) GIP 수용체의 활성화; 및 iii) 글루카곤 수용체의 활성화.
여기서, 수용체를 활성화시킨다는 것은, 천연형 대비 수용체에 대한 in vitro 활성이 약 0.1% 이상, 약 1% 이상, 약 2% 이상, 약 3 % 이상, 약 4% 이상, 약 5% 이상, 약 6% 이상, 약 7% 이상, 약 8% 이상, 약 9% 이상, 약 10 % 이상, 약 20% 이상, 약 30% 이상, 약 40% 이상, 약 50 % 이상, 약 60% 이상, 약 70% 이상, 약 80% 이상, 약 90 % 이상, 약 100% 이상을 나타내는 경우를 예로 들 수 있다. 그러나, 이에 제한되는 것은 아니다.
본 발명에서 용어, "약"은 ±0.5, ±0.4, ±0.3, ±0.2, ±0.1 등을 모두 포함하는 범위로, 약 이란 용어 뒤에 나오는 수치와 동등하거나 유사한 범위의 수치를 모두 포함하나, 이에 제한되지 않는다.
이와 같은 삼중 활성체의 활성은 당업계에 공지된 방법으로 측정될 수 있으며, 특정한 방법에 제한되지 않는다. 한 예로, 실험예 1에 개시된 것과 같은 방법으로 측정될 수 있으나, 이에 제한되지 않는다.
본 발명의 삼중 활성체는 GLP-1, GIP, 및 글루카곤 수용체에 대해 모두 활성을 가지면서도 각 수용체에 대한 상대 활성 간의 비율은 다양하게 나타날 수 있는데, 구체적으로, 본 발명에 따른 삼중 활성체는 천연형 글루카곤 대비 글루카곤 수용체에 대한 상대적 활성에 비해 천연형 GLP-1 및 GIP 대비 GLP-1 수용체 및 GIP 수용체에 대한 상대적 활성이 모두 높은 것일 수 있다. 각 수용체에 대해 나타내는 활성의 정도는 삼중 활성체의 체내 활성과 효능을 결정하는데 중요하며, 본 발명의 삼중 활성체는 GLP-1, GIP, 및 글루카곤 수용체에 대한 삼중 활성 작용이 최적화되어 신장 질환 치료에 있어서 상승적 효과를 기대할 수 있다.
구체적으로, 상기 삼중 활성체는 천연형 글루카곤 대비 글루카곤 수용체에 대한 상대적 활성에 비해 천연형 GLP-1 대비 GLP-1 수용체에 대한 상대적 활성이 각각 1.5배 이상, 2배 이상, 3배 이상, 4배 이상, 5배 이상, 10배 이상, 15배 이상, 20배 이상, 또는 25배 이상, 또는 30배 이하인 것일 수 있으나, 이에 제한되지 않는다. 또한, 상기 삼중 활성체는 천연형 글루카곤 대비 글루카곤 수용체에 대한 상대적 활성에 비해 천연형 GIP 대비 GIP 수용체에 대한 상대적 활성이 각각 1.5배 이상, 2배 이상, 3배 이상, 4배 이상, 10배 이상, 또는 15배 이상, 또는 20배 이하인 것일 수 있으나, 이에 제한되지 않는다.

또한, 상기 삼중 활성체는 천연형 GLP-1, 천연형 GIP, 및 천연형 글루카곤 중 어느 하나 대비 체내 반감기가 증가된 것일 수 있으나, 특별히 이에 제한되는 것은 아니다. 본 발명의 삼중 활성체는 GLP-1 수용체, GIP 수용체, 및 글루카곤 수용체에 모두 활성을 나타내면서 특히 글루카곤 수용체에 대한 활성이 상대적으로 낮은 특징을 갖고, 동시에, 체내 반감기 증가로 약효를 오래 유지할 수 있어 신장 질환의 효과적인 치료제로 이용될 수 있다.
특별히 이에 제한되는 것은 아니나, 이러한 상기 삼중 활성체는 비자연적으로 발생하는 (non-naturally occurring) 것일 수 있다.
상기 삼중 활성체를 포함하는 본 발명의 약학적 조성물은 신장 질환의 예방 또는 치료 효과를 나타낼 수 있다.
본 발명에서 용어, “신장 질환”은 신장이 기능을 정상적으로 수행하지 못하거나 신장 구조에 이상이 초래된 상태를 의미한다. 신장의 손상으로 인한 기능의 저하는 신장 및 관련 구조의 증대, 신장의 위축, 체액량의 변화, 전해질 불균형, 대사성 산증, 가스교환장애, 항감염 기능 손상, 요독성 독소의 축적 등을 초래한다. 특히, 치료되지 않은 신장 질환은 만성 및 말기 신부전으로 발전하는 경우가 많아 이식이 요구되기도 한다.
이와 같은 신장 질환의 대표적인 예로는 급성으로 발전하는 급성 신 손상 (acute kidney injury)과 신장 이상이 만성으로 진행된 만성 신장 질환 (chronic kidney disease, CKD)을 들 수 있다. 본 발명의 신장 질환에는 급성 신 손상, 급성 신부전, 급성 신장염, 당뇨병성 신부전, 사구체 신염, 만성 신장 질환, 만성 신부전, 세관 장애, 신증후군, 신우신염, 신성 고혈압, 요독증, 고혈압성 신장병, 신장 섬유증, 신장 아밀로이드증, 요독증성 발작, 신장 결석, 수신증, 신장낭종, 신장종양, 다낭성 신장 질환, 급성 신염 증후군, 무증상 요이상, 진통제성 신병증, 신동맥 협착, 및 신경화증 등이 포함될 수 있다.
본 발명에서 용어, “급성 신 손상 (acute kidney injury)”은 신기능의 급격한 저하를 특징으로 하는 질환으로, 신세포의 손상으로 신기능이 수시간에서 수일에 걸쳐 급속하게 악화되는 질환이다. 이러한 급속한 신 손상으로 인해 신체 내에 질소 노폐물 축적, 혈액 내 고질소혈증 등이 일어나게 되고 결국 투석이나 사망에 이르게 된다. 짧은 시간 내에 크레아티닌의 농도가 상승하거나, 소변량이 감소하는 것으로 진단하며, 급성 신 손상이 일어나는 이유로는 신전성 원인 (pre-renal type), 신성 원인 (renal type), 및 신후성 원인 (post-renal type)이 알려져 있다.
상기 급성 신 손상은 급성 신부전 또는 급성 신장염을 포함할 수 있으나, 이에 제한되지 않는다.
본 발명의 실시예에서는 일측요로폐쇄 (unilateral ureteral obstruction, UUO) 마우스 모델을 사용하여 급성 신 손상에 대한 본 발명의 삼중 활성체의 치료 효과를 확인한 결과, 본 발명의 삼중 활성체에 의해 신장 섬유화 개선 효과가 나타나는 것을 확인하였다. UUO는 요로가 폐쇄된 한 쪽 신장의 기능을 손상시킬 뿐 아니라 신장 염증 및 신장 섬유화가 발생하는 모델로, 단기간 내에 신조직이 손상된 개체에서 치료제의 효능 평가에 효과적인 것으로 알려져 있다.
본 발명에서 용어, “만성 신장 질환 (chronic kidney disease, CKD)”은 만성 콩팥병과 혼용되어 사용될 수 있으며, 3개월 이상 신장의 구조적 또는 기능적 이상이 존재하거나 신장의 손상여부와 관계없이 사구체여과율이 3개월 이상 60 mL/분/1.73 m2 이하로 감소하였을 때로 정의되는 독립된 질환명이다. 만성 신장 질환은 진행에 따라 5단계로 나누어 임상에 적용된다. 높은 혈당으로 인한 신장에 손상이 있거나, 혈압이 높은 상태가 오래 유지될 경우 만성 신장 질환의 발병 위험이 높으며, 이 외에도, 사구체 신장염, 다낭성 신장 질환, 자가 면역 질환 (예를 들어, 루푸스병) 등도 만성 신장 질환의 원인으로 알려져 있다. 만성 신장 질환 개체는 사구체 고혈압 및 단백뇨를 동반하는 경우가 많아서, 혈압을 낮추고 단백뇨를 감소시키는 것이 중요하다.
상기 만성 신장 질환은 신염 증후군 (nephrotic syndrome), 세관 장애 (renal tubular dysfunction), 신성 고혈압 (renal hypertension), 요독증 (uremia), 신부전 (renal failure), 및 만성 신부전 (chronic renal failure)으로 이루어진 군에서 선택되는 어느 하나 이상의 질환을 포함할 수 있으나, 이에 제한되지 않는다.
본 발명의 실시예에서는 고혈압 및 만성 신장 질환 (CKD) 모델로 알려진 spontaneous hypertensive rat (SHR) 모델을 사용하여 만성 신장 질환에 대한 본 발명의 삼중 활성체의 치료 효과를 확인한 결과, 본 발명의 삼중 활성체에 의해 신장 기능 회복 효과가 나타나는 것을 확인하였다. SHR 모델은 고혈압에 따른 신장기능 이상이 발생하는 모델로, 만성 신장 질환의 병인학 (etiology) 측면에서 사람과 굉장히 유사하여 만성 신장 질환 치료제의 효능 평가에 효과적인 것으로 알려져 있다.
또한, 본 발명의 신장 질환은 다른 질환으로 인한 합병증, 예를 들어, 당뇨병성 신장합병증, 고혈압성 신장질환 등을 포함할 수 있으나, 이에 제한되지 않는다. 특히, 고혈압은 만성 신장 질환의 원인이기도 하지만 만성 신장 질환에 의한 합병증으로도 이해되고 있다.
본 발명에 따른 삼중 활성체, 또는 이를 포함하는 조성물은 신장 기능의 회복 효과, 및/또는 신장 섬유화 개선 효과를 가질 수 있다.
상기 신장 기능 회복은 혈액 내 노폐물을 걸러내는 신장 고유의 기능을 회복하는 것을 의미하며, 기능 회복의 정도는 당업계에 알려진 다양한 방법으로 측정될 수 있다. 측정 방법의 제한되지 않는 예로, 알부민 배출량, 사구체여과율, 단백뇨, 크레아틴 농도 등을 측정할 수 있으나, 이에 제한되지 않는다.
본 발명에서, 단백뇨 (proteinuria)는 사구체여과율이 낮아진 신장 질환 개체의 소변에 정상치보다 높은 단백질 (예를 들어, 알부민, 글로불린 등)이 포함되는 증상을 의미한다. 알부민은 요단백에서 약 40%를 차지하는 단백질로, 소변 내 알부민 수준을 측정함으로써 단백뇨 수준을 확인할 수 있다. 본 발명에 따른 삼중 활성체, 또는 이를 포함하는 조성물은 신장 기능을 회복 시켜 체외로 배출되는 단백질 (예를 들어, 알부민)의 양을 감소시키는 것일 수 있다.
또한, 본 발명에 따른 삼중 활성체, 또는 이를 포함하는 조성물은 개체에 투여되었을 때 개체의 신장 섬유화 개선 효과를 나타내는 것일 수 있다. 상기 신장 섬유화는 신장 염증과 손상으로 인해 신 조직이 섬유화되는 상태를 의미한다. 신장 섬유화는 신장 조직의 이상을 야기하고, 결국 기능 이상을 초래하기 때문에 신장 질환 치료를 위해서는 신장 섬유화를 개선하는 것이 요구된다. 상기 신장 섬유화 개선은 섬유화의 진행에 수반되는 단백질 (예를 들어, pro-collagen1α1)의 농도를 측정함으로써 측정될 수 있으나, 이에 제한되지 않는다.
본 발명에 따른 삼중 활성체, 또는 이를 포함하는 조성물에 의한 신장 기능 회복 및 신장 섬유화 개선은 서로 독립적으로 본 발명에 따른 삼중 활성체, 또는 이를 포함하는 조성물에 의해 일어날 수 있으며, 또는, 함께 일어날 수도 있다.
본 발명에서, “개체”는 신장 손상, 신장 기능 이상, 및/또는 신장 질환을 갖는 인간을 포함한 쥐, 가축 등을 포함하는 포유 동물을 의미할 수 있고, 신장 기능 회복 및 신장 섬유화 개선으로 인해 이로운 효과를 얻을 수 있다면 특별히 제한되지 않는다.
본 발명에 따른 삼중 활성체, 또는 이를 포함하는 조성물은 투여된 개체에서 하기 특성 중 하나 이상을 나타내는 것일 수 있으나, 이에 제한되지 않는다:
(a) 부형제 투여 대조군 대비 소변 알부민 크레아티닌 비율 (uACR) 감소;
(b) 부형제 투여 대조군 대비 신장 기능 회복;
(c) 부형제 투여 대조군 대비 신장 섬유화 개선; 및
(d) 부형제 투여 대조군 대비 pro-collagen1α1 발현 수준 감소.
추가적으로, 본 발명의 본 발명의 삼중 활성체는 GLP-1 수용체, GIP 수용체, 및 글루카곤 수용체에 대한 작용을 통해, 혈당 강하 효과를 나타내는 것일 수 있으나, 이에 제한되지 않는다.
본 발명의 실시예에서는 위하여 일반 마우스에서 복강내 당부하검사 (ipGTT)를 수행한 결과, 본 발명의 삼중 활성체에 의한 혈당 강하 효과가 나타나는 것을 확인하였다.
본 발명에 따른 삼중 활성체, 또는 이를 포함하는 조성물은 GLP-1 수용체, GIP 수용체, 및 글루카곤 수용체에 동시에 작용할 수 있는 활성을 바탕으로 상기와 같은 특성을 가져 신장 질환에 대한 예방 또는 치료 효과를 가질 수 있다.
특별히 이에 제한되지는 않으나, 본 발명의 삼중 활성체, 또는 이를 포함하는 조성물로 예방 또는 치료될 수 있는 신장 질환으로는 급성 신 손상, 급성 신부전, 급성 신장염, 당뇨병성 신부전, 사구체 신염, 만성 신장 질환, 만성 신부전, 세관 장애, 신증후군, 신우신염, 신성 고혈압, 요독증, 고혈압성 신장병, 신장 섬유증, 신장 아밀로이드증, 요독증성 발작, 신장 결석, 수신증, 신장낭종, 신장종양, 다낭성 신장 질환, 급성 신염 증후군, 무증상 요이상, 진통제성 신병증, 신동맥 협착, 및 신경화증 등을 들 수 있다.
본 발명의 약학적 조성물은 삼중 활성체를 포함하여 GLP-1 수용체, GIP 수용체, 및 글루카곤 수용체에 대한 작용을 통해 신장 기능 회복 및/또는 신장 섬유화 개선 효과를 나타낼 수 있다. 특히, 높은 혈당은 사구체의 미세 혈관을 손상시켜 신장질환을 악화시킬 수 있으므로, 글루카곤 수용체에 대한 활성이 지나치게 높지 않도록 적정 수준으로 유지되는 것이 필요한데, 본 발명의 삼중 활성체는 천연형 GLP-1 및 GIP 대비 GLP-1 수용체 및 GIP 수용체에 대한 각각의 상대 활성이 천연형 글루카곤 대비 글루카곤 수용체에 대한 상대 활성 보다 상대적으로 높아 신장 질환에 대한 우수한 예방 또는 치료 효과를 나타낼 수 있으나, 이에 제한되지 않는다. 구체적으로, 본 발명의 삼중 활성체는 GLP-1 수용체, GIP 수용체, 및 글루카곤 수용체에 대해 동시 활성을 나타내면서도 최적화된 활성비를 나타내기 때문에 혈당 강하 효능이 우수한 이점이 있고, 이러한 혈당 강하 효능이 사구체 미세 혈관 손상을 방지하여 신장 질환의 예방, 치료에 우수한 효과를 나타낼 수 있다.

아실화 (acylation)는 펩타이드 약물의 약동학적 및 약력학적 특성을 개선하기 위한 방법으로 알려져 있다. 펩타이드 약물의 경우, 체내 효소 분해로 인해 약효를 발휘하기 어려운 문제점이 있다. 이에, 지방산을 펩타이드에 부착시키는 펩타이드의 아실화를 통해 효소 작용 부위를 차단함으로써 펩타이드 약물의 안정성을 높이고 반감기를 증가시킬 수 있다. 본 발명의 목적상, 본 발명의 삼중 활성체는 반감기 증가를 위해 아실화된 형태일 수 있다. 본 발명의 아실화된 삼중 활성체는 GLP-1 수용체, GIP 수용체, 및 글루카곤 수용체에 모두 활성을 나타내는 동시에 체내 반감기가 증가되어 효과적인 치료제로 이용될 수 있다.
본 발명에서, 아실화 여부에 대한 기재 없이 “삼중 활성체” 또는 “펩타이드”로 기재되어 있다 하더라도, 아실화된 형태를 배제하지 않으며, “아실화된 삼중 활성체”, “아실화된 펩타이드”와 혼용되어 혼용될 수 있다.
본 발명의 아실화된 삼중 활성체는 삼중 활성체 펩타이드의 아미노산에 직접 아실기를 연결시키거나, 또는 링커를 통해 아실기를 부착한 것일 수 있다. 이에 제한되지 않으나, 상기 링커는 AEEA ((2-(2-aminoethoxy)ethoxy)acetic acid), GABA (4-Aminobutyric acid), Ava (5-Aminovaleric acid), Ahx (Aminohexanoic acid), 트리아졸 (Triazole), 및 폴리에틸렌 글리콜 (PEG)로 이루어진 그룹에서 선택되는 링커일 수 있으나, 이에 제한되지 않는다.
구체적인 예로, 상기 링커는 AEEA ((2-(2-aminoethoxy)ethoxy)acetic acid)를 포함하는 것일 수 있고, 더 구체적으로 gamma-Glutamate가 추가로 연결되는 것일 수 있으나, 이에 제한되지 않는다. 구체적인 링커의 예로, (gammaGlu)m-(AEEA)n을 들 수 있고, 상기 m 및 n은 각각 독립적으로 0, 1, 2, 3, 또는 그 이상의 정수 일 수 있으나, 이에 제한되지 않는다.
다른 예로, 상기 링커는 말레이미드 반응기를 갖는 폴리에틸렌 글리콜 (maleimide-PEG)일 수 있으나, 이에 제한되지 않는다.
또 다른 예로, 상기 삼중 활성체 펩타이드와 아실기는 클릭 화학 반응을 통해 연결된 것일 수 있으나, 이에 제한되지 않는다.
상기 링커의 아민기, 히드록시기, 티올기 등을 통해 삼중 활성체의 아미노산의 아민기, 히드록시기, 티올기, 카르복실기 등에 아실기를 결합시킬 수 있으나, 아실기를 삼중 활성체에 결합시켜 삼중 활성체의 구조적 안정성 및 반감기 증가에 기여할 수 있다면, 특별한 종류나 길이에 제한되지 않는다. 또한, 상기 링커는 아실기에 공유적으로 결합될 수 있고, 1, 2, 3, 또는 그 이상 반복되어 아실기와 연결될 수 있으나, 이에 제한되지 않는다.
상기 아실기는 임의의 길이를 갖는 탄소쇄일 수 있고, 선형 또는 분지쇄형일 수 있다. 구체적으로, 상기 쇄는 선형 지방족 쇄, 분지형 지방족 쇄, 사이클릭 알킬 모이어티를 함유하는 쇄, 소수성 천연 생성물, 예컨대, 스테로이드, 아르알킬 쇄, 또는 아실 모이어티를 함유하는 알킬 쇄를 포함할 수 있다.
이에 제한되지 않으나, 상기 아실화된 삼중 활성체는 1개 이상, 2개 이상, 구체적으로는 1개 또는 2개의 카르복실산을 포함하는 C1-C30 직쇄 또는 분지쇄 아실기로 아실화된 것일 수 있고, 한 예로, 상기 아실기는 지방산 또는 다이카복실산, 구체적으로는 C4 내지 C30 지방산 또는 다이카복실산일 수 있으나, 이에 제한되지 않는다. 보다 구체적인 예로, C16, C18, C20, C22, C24, C26, C28, 또는 C30의 지방산 또는 다이카복실산일 수 있다. 다른 예의 아실기로는 콜산, 케노데옥시콜산, 데옥시콜산, 리토콜산, 타우로콜산, 글리코콜산, 콜레스테롤산 등의 담즙산, 숙신산 또는 숙신산 유도체, 말레산, 또는 말레산 유도체 등을 들 수 있으나, 이에 제한되지 않는다.
한 구체적인 실시예로, 상기 아실기는 C18 이상의 지방산 또는 지방 이산 또는 그 유도체일 수 있고, 더 구체적으로는 옥타데칸산, 옥타데칸이산, 리놀레산, 리놀렌산, 올레산 등일 수 있으나, 이에 제한되지 않는다.
또 다른 구체적인 실시예로, 상기 아실기는 C20 이상의 지방산 또는 지방 이산 또는 그 유도체일 수 있고, 더 구체적으로는 에이코산산, 도코산산, 이코사펜타엔산, 아드렌산 등일 수 있으나, 이에 제한되지 않는다.
본 발명의 아실화된 삼중 활성체는 당업계의 공지된 방법으로 아실기가 삼중 활성체에 부착된 것이거나, 아실화된 아미노산을 사용하여 펩타이드를 합성하여 제조된 것일 수 있으나, 이에 제한되지 않는다.
본 발명에서, 아실화된 삼중 활성체는 삼중 활성체가 포함하는 아미노산 잔기에 직접 아실기가 부착된 형태일 수 있다. 예를 들어, 에스테르, 티오에스테르, 또는 아미드 결합을 통해 아실기가 부착된 것일 수 있으나, 이에 제한되지 않는다. 구체적으로, 아실화된 삼중 활성체는 아민, 히드록실, 또는 티올기를 갖는 아미노산 잔기에 아실화가 이루어진 것일 수 있다. 예를 들어, 상기 아실화된 삼중 활성체는 N-말단 또는 C-말단에 위치한 아미노산 또는 라이신 잔기에 아실화된 것일 수 있으나, 이에 제한되지 않는다.
본 발명의 용어, “아실화된 아미노산”은 상기 아실기가 부착된 아미노산을 의미한다. 상기 아실화된 아미노산의 아미노산은 천연형 또는 비천연형 아미노산일 수 있고, 이들의 이성질체도 포함할 수 있으며, 아실기가 부착될 수 있는 한, 특정 아미노산으로 제한되지 않는다. 예를 들어, 상기 아미노산은 아실기가 부착될 수 있는 아민, 히드록실, 또는 티올기, 카르복실기를 갖는 아미노산, 보다 구체적인 예로는 라이신, 글루타민, 아스파라긴, 또는 아르기닌일 수 있으나, 이에 제한되지 않는다. 또는, 상기 아실기가 삼중 활성체의 N-말단에 연결된 것일 수 있으나, 이에 제한되지 않는다.
본 발명에서, 아실화된 삼중 활성체 또는 일반식 1의 아실화된 아미노산은 하기 K(1) 또는 K(2)로 표시되는 아미노산 중 어느 하나를 포함하는 것일 수 있으나, 이에 제한되지 않는다:

상기 아실화된 아미노산은 펩타이드 내 다른 아미노산과 연결된 것일 수 있다. 구체적으로, 상기 K(1) 및 K(2)의 라이신은 카르복실기 및/또는 아미노기를 통해 삼중 활성체의 인접한 다른 아미노산에 연결되고, 측쇄를 통해 링커 및 아실기와 연결될 수 있고, 다른 아미노산에 연결된 형태까지 본 발명의 K(1) 및 K(2)의 범주에 포함됨은 자명하다.
본 발명의 삼중 활성체는 N-말단으로부터 1 내지 39번째 위치의 아미노산 중 하나 또는 그 이상의 아미노산, 또는 N-말단, 또는 C-말단에 아실화된 형태일 수 있으나, 삼중 활성체로서의 활성을 보유하는 한 아실화의 위치나 아실화된 아미노산의 수는 제한되지 않는다.
구체적인 예로, 본 발명의 아실화된 삼중 활성체는 12번, 17번, 및 20번 위치 중 어느 하나 이상의 위치에 아실화된 아미노산을 포함하는 것일 수 있으나, 이에 제한되지 않는다.
본 발명의 아실화된 삼중 활성체의 일 구체적인 양태로, 12번 위치에 아실화된 아미노산을 포함할 수 있다. 구체적으로, 12번 위치에서 K(1)을 포함할 수 있으며, 보다 구체적으로는 서열번호 1 내지 7 중 어느 하나의 아미노산 서열을 포함하는 것일 수 있고, 또는 12번 위치에서 K(2)을 포함할 수 있으며, 보다 구체적으로는 서열번호 8 내지 14 중 어느 하나의 아미노산 서열을 포함하는 것일 수 있으나, 이에 제한되지 않는다.
본 발명의 아실화된 삼중 활성체의 다른 일 구체적인 실시 양태로, 17번 위치에 아실화된 아미노산을 포함할 수 있다. 구체적으로, 17번 위치에서 K(1)을 포함할 수 있으며, 보다 구체적으로는 서열번호 15 내지 21 중 어느 하나의 아미노산 서열을 포함하는 것일 수 있고, 또는 17번 위치에서 K(2)을 포함할 수 있으며, 보다 구체적으로는 서열번호 22 내지 28 중 어느 하나의 아미노산 서열을 포함하는 것일 수 있으나, 이에 제한되지 않는다.
다른 하나의 구체적인 양태로서, 상기 삼중 활성체는 서열번호 1 내지 28 중 어느 하나의 아미노산 서열을 포함하거나, 서열번호 1 내지 28 중 어느 하나의 아미노산 서열로 (필수적으로) 구성된 것일 수 있으나, 이에 제한되지 않는다.
본원에서 '특정 서열번호로 구성되는 펩타이드'라고 기재되어 있다 하더라도, 해당 서열번호의 아미노산 서열로 이루어진 펩타이드와 동일 혹은 상응하는 활성을 가지는 경우라면 해당 서열번호의 아미노산 서열 앞뒤의 무의미한 서열 추가 또는 자연적으로 발생할 수 있는 돌연변이, 혹은 이의 잠재성 돌연변이 (silent mutation)를 제외하는 것이 아니며, 이러한 서열 추가 혹은 돌연변이를 가지는 경우에도 본원의 범위 내에 속하는 것이 자명하다.
또한, 본 발명의 삼중 활성체는 서열번호 1 내지 28의 아미노산 서열과 40%, 45%, 50%, 55%, 60%, 65%, 70%, 75%, 80%, 81%, 82%, 83%, 84%, 85%, 86%, 87%, 88%, 89%, 90%, 91%, 92%, 93%, 94%, 95%, 96%, 97%, 98%, 또는 99% 이상의 상동성 또는 동일성을 가지는 아미노산 서열을 포함할 수 있으나, GLP-1 수용체, GIP 수용체, 및 글루카곤 수용체에 작용할 수 있고, 신장 질환에 대한 예방 또는 치료 효과를 나타낼 수 있는 한, 이에 제한되지 않는다.
본 발명에서 용어, '상동성 (homology)' 또는 '동일성 (identity)'은 두 개의 주어진 아미노산 서열 또는 염기 서열과 서로 관련된 정도를 의미하며 백분율로 표시될 수 있다.
보존된 (conserved) 폴리펩티드의 서열 상동성 또는 동일성은 표준 배열 알고리즘에 의해 결정되며, 사용되는 프로그램에 의해 확립된 디폴트 갭 페널티가 함께 이용될 수 있다. 실질적으로, 상동성을 갖거나 (homologous) 또는 동일한 (identical) 서열은 일반적으로 서열 전체 또는 일부분과 중간 또는 높은 엄격한 조건 (stringent conditions)에서 하이브리드할 수 있다. 하이브리드화는 폴리뉴클레오티드에서 일반 코돈 또는 코돈 축퇴성을 고려한 코돈을 함유하는 폴리뉴클레오티드와의 하이브리드화 역시 포함됨이 자명하다.
용어 상동성 및 동일성은 종종 상호교환적으로 이용될 수 있다.
임의의 두 펩타이드 서열이 상동성, 유사성 또는 동일성을 갖는지 여부는 예를 들어, Pearson et al (1988)[Proc. Natl. Acad. Sci. 또한, 본 발명에 따른 삼중 활성체는 펩타이드의 N-말단 및/또는 C-말단이 변형되지 않은 것일 수 있으나, 생체 내의 단백질 절단 효소들로부터 보호하고 안정성을 증가시키기 위하여 이의 N-말단 및/또는 C-말단 등이 화학적으로 수식되거나 유기단으로 보호되거나, 또는 펩타이드 말단 등에 아미노산이 추가되어 변형된 형태 역시 본 발명의 범주에 포함된다.
특히, 화학적으로 합성한 펩타이드의 경우, N- 및 C-말단이 전하를 띠고 있기 때문에, 이러한 전하를 제거하기 위하여 N-말단을 아세틸화 (acetylation) 및/또는 C-말단을 아미드화 (amidation)할 수 있으나, 특별히 이에 제한되지는 않는다.
본 발명에 따른 삼중 활성체는 C-말단이 변형되지 않은 것이거나, 또는 C-말단이 아미드화된 것일 수 있으나, 이에 제한되지 않는다.
한편, 본 발명의 삼중 활성체는 분자 내 가교 (intramolecular bridge)를 포함할 수 있으며 (예컨대, 공유결합적 가교 또는 비공유결합적 가교), 구체적으로 고리를 포함하는 형태일 수 있으며, 예컨대 삼중 활성체의 16번 및 20번 아미노산 사이, 및/또는 17번 및 21번 아미노산 사이에 고리가 형성된 형태일 수 있으나, 특별히 이에 제한되는 것은 아니다.
상기 고리의 비제한적인 예로 락탐 고리를 포함할 수 있다.
본 발명의 삼중 활성체는 고리를 포함하도록, 목적하는 위치에 고리를 형성할 수 있는 아미노산을 포함하도록 변형된 것을 모두 포함한다.
예컨대, 본 발명의 삼중 활성체는 16번 및 20번 아미노산 쌍 및/또는 17번 21번 아미노산 쌍이 고리를 형성할 수 있는 글루탐산 또는 리신을 가질 수 있으나, 이에 제한되지 않는다.
이러한 고리는 상기 삼중 활성체 내의 아미노산 곁사슬 간에 형성될 수 있으며, 그 예로 리신의 곁 사슬과 글루탐산의 곁 사슬 간에 락탐 고리가 형성되는 형태일 수 있으나, 특별히 이에 제한되는 것은 아니다.
또한, 특별히 이에 제한되지 않으나, 본 발명의 삼중 활성체는 체내 반감기의 증가를 위해 활성체 분해 효소의 인식작용을 회피하기 위하여 일부 아미노산을 타 아미노산 혹은 비 천연형 화합물로 치환할 수 있다.
구체적으로, 상기 삼중 활성체의 아미노산 서열 중 두 번째 아미노산 서열의 치환을 통해 분해효소의 인식 작용을 회피하여 체내 반감기를 증가시킨 펩타이드일 수 있으나, 체내 분해 효소의 인식 작용을 회피하기 위한 아미노산 치환 또는 변경은 제한 없이 포함된다.
또한, 본 발명의 삼중 활성체는 L-형 혹은 D-형 아미노산, 및/또는 비-천연형 아미노산을 이용한 변형; 및/또는 천연형 서열을 개질, 예를 들어 측쇄 작용기의 변형, 분자 내 공유결합, 예컨대, 측쇄 간 고리 형성, 메틸화, 아실화, 유비퀴틴화, 인산화, 아미노헥산화, 바이오틴화 등과 같이 개질함으로써 변형된 것을 모두 포함할 수 있다.
또한, 본 발명의 삼중 활성체의 아미노 및/또는 카르복시 말단에 하나 또는 그 이상의 아미노산이 추가된 것을 모두 포함할 수 있다.
상기 치환되거나 추가되는 아미노산은 인간 단백질에서 통상적으로 관찰되는 20개의 아미노산뿐만 아니라 비정형 또는 비-자연적 발생 아미노산을 사용할 수 있는 것으로, 비정형 아미노산 및 아미노산이 포함된 펩타이드와 정형적인 펩타이드 서열은 상업화적으로 합성 및 구매가 가능하다.
아미노산 유도체도 마찬가지 방식으로 입수할 수 있는데, 그 예를 일부만 들자면 4-이미다조아세트산 (4-imidazoacetic acid), 또는 알파-메틸-류신 (αMeL) 등을 사용할 수 있다.
본 명세서에서 따로 가리키는 바가 없으면, 본 발명에 따른 삼중 활성체에 대한 발명의 설명이나 청구 범위의 기술은 해당 삼중 활성체는 물론이고, 해당 펩타이드 (삼중 활성체)의 염 (예컨대, 상기 펩타이드의 약학적으로 허용가능한 염), 또는 이의 용매화물의 형태를 모두 포함하는 범주에도 적용된다. 따라서 명세서의 해당 기재 내용은 그 특정 염, 그 특정 용매화물, 그 특정 염의 특정 용매화물에도 마찬가지로 적용된다. 이러한 염 형태는 예를 들어 약학적으로 허용되는 임의의 염을 사용한 형태일 수 있다. 상기 염의 종류는 특별히 제한되지는 않는다. 다만, 개체, 예컨대 포유류에게 안전하고 효과적인 형태인 것이 바람직하나, 특별히 이에 제한되는 것은 아니다.
상기 용어, "약학적으로 허용되는"은 의약학적 판단의 범위 내에서,과도한 독성,자극, 또는 알레르기 반응 등을 유발하지 않고 원하는 용도에 효과적으로 사용 가능한 물질을 의미한다.
본 발명에서 용어, "약학적으로 허용되는 염" 이란 약학적으로 허용되는 무기산, 유기산, 또는 염기로부터 유도된 염을 포함한다. 적합한 산의 예로는 염산, 브롬산, 황산, 질산, 과염소산, 푸마르산,말레산, 인산, 글리콜산, 락트산, 살리실산,숙신산, 톨루엔-p-설폰산, 타르타르산, 아세트산,시트르산, 메탄설폰산,포름산, 벤조산, 말론산, 나프탈렌-2-설폰산, 벤젠설폰산 등을 들 수 있다. 적합한 염기로부터 유도된 염은 나트륨, 칼륨 등의 알칼리 금속,마그네슘 등의 알칼리 토금속,및 암모늄 등을 포함할 수 있다.
또한, 본 발명에서 사용된 용어 "용매화물"은 본 발명에 따른 삼중 활성체, 또는 이의 염이 용매 분자와 복합체를 형성한 것을 말한다.
본 발명의 삼중 활성체는 그 길이에 따라 이 분야에서 잘 알려진 방법, 예를 들어 자동 펩타이드 합성기에 의해 합성할 수 있으며, 유전자 조작 기술에 의하여 생산할 수도 있다.
구체적으로, 본 발명의 삼중 활성체는 표준 합성 방법, 재조합 발현 시스템, 또는 임의의 다른 당해 분야의 방법에 의해 제조될 수 있다. 따라서, 본 발명에 따른 삼중 활성체는, 예를 들어 하기를 포함하는 방법을 포함하는 다수의 방법으로 합성될 수 있다:

본 발명의 삼중 활성체를 포함하는 신장 질환 예방 또는 치료용 약학적 조성물은 상기 삼중 활성체를 약학적 유효량으로 포함하는 것일 수 있다. 또한, 상기 조성물의 삼중 활성체는 아실화된 형태도 포함할 수 있다.

상기 삼중 활성체는 GLP-1 수용체, GIP 수용체, 및 글루카곤 수용체에 대한 작용을 통해 신장 기능 개선 및/또는 신장 섬유화 개선 효과를 나타낼 수 있고, 특히, 천연형 GLP-1 및 GIP 대비 GLP-1 수용체 및 GIP 수용체에 대한 각각의 상대 활성이 천연형 글루카곤 대비 글루카곤 수용체에 대한 상대 활성 보다 상대적으로 높아 신장 질환에 대한 우수한 예방 또는 치료 효과를 나타낼 수 있으나, 이에 제한되지 않는다.
상기 삼중 활성체를 약학적 유효량으로 포함하는 것은, 목적하는 약리 활성 (예를 들어, 신장 질환의 예방, 개선 또는 치료)을 얻을 수 있는 정도를 의미하고, 또한 투여되는 개체에서 독성 또는 부작용이 일어나지 않거나 미미한 수준으로서 약학적으로 허용되는 수준을 의미할 수 있으나, 이에 제한되지 않는다. 이와 같은 약학적 유효량은 투여 횟수, 투여 개체, 제형 등을 종합적으로 고려하여 결정할 수 있다.
본 발명에서 용어 "예방"은 상기 삼중 활성체 (아실화된 형태 포함), 또는 이를 포함하는 조성물의 투여로 목적하는 질환, 예컨대 신장 질환의 발병을 억제 또는 지연시키는 모든 행위를 의미하며, "치료"는 상기 삼중 활성체 (아실화된 형태 포함), 또는 이를 포함하는 조성물의 투여로 목적하는 질환, 예컨대 신장 질환의 증세가 호전되거나 이롭게 되는 모든 행위를 의미한다.
본 발명에서 용어 "투여"는 어떠한 적절한 방법으로 개체에게 소정의 물질을 도입하는 것을 의미하며, 상기 조성물의 투여 경로는 특별히 이에 제한되지 않으나, 상기 조성물이 생체 내 표적에 도달할 수 있는 어떠한 일반적인 경로를 통하여 투여될 수 있으며, 예를 들어 복강 내 투여, 정맥 내 투여, 근육 내 투여, 피하 투여, 피내 투여, 경구 투여, 국소 투여, 비 내 투여, 폐 내 투여, 직장 내 투여 등이 될 수 있다.
본 발명의 약학적 조성물은 약학적으로 허용가능한 담체, 부형제 또는 희석제를 추가로 포함할 수 있다. 본 발명에서 용어 "약학적으로 허용가능한"이란 치료효과를 나타낼 수 있을 정도의 충분한 양과 부작용을 일으키지 않는 것을 의미하며, 질환의 종류, 개체의 연령, 체중, 건강, 성별, 개체의 약물에 대한 민감도, 투여 경로, 투여 방법, 투여 횟수, 치료 기간, 배합 또는 동시 사용되는 약물 등 의학 분야에 잘 알려진 요소에 따라 당업자에 의해 용이하게 결정할 수 있다.
본 발명의 삼중 활성체를 포함한 약학적 조성물은 약학적으로 허용가능한 담체를 추가로 포함할 수 있다. 상기 담체는 특별히 이에 제한되지는 않으나, 경구 투여 시에는 결합제, 활택제, 붕해제, 부형제, 가용화제, 분산제, 안정화제, 현탁화제, 색소, 향료 등을 사용할 수 있고, 주사제의 경우에는 완충제, 보존제, 무통화제, 가용화제, 등장화제, 안정화제 등을 혼합하여 사용할 수 있으며, 국소투여용의 경우에는 기제, 부형제, 윤활제, 보존제 등을 사용할 수 있다.
본 발명의 조성물의 제형은 상술한 바와 같은 약학적으로 허용가능한 담체와 혼합하여 다양하게 제조될 수 있다. 예를 들어, 경구 투여 시에는 정제, 트로키, 캡슐, 엘릭서, 서스펜션, 시럽, 웨이퍼 등의 형태로 제조할 수 있으며, 주사제의 경우에는 단위 투약 앰플 또는 다수회 투약 형태로 제조할 수 있다. 기타, 용액, 현탁액, 정제, 환약, 캡슐, 서방형 제제 등으로 제형화할 수 있다.
한편, 제제화에 적합한 담체, 부형제 및 희석제의 예로는 락토스, 덱스트로스, 수크로스, 솔비톨, 만니톨, 자일리톨, 에리스리톨, 말티톨, 전분, 아카시아, 알지네이트, 젤라틴, 칼슘 포스페이트, 칼슘 실리케이트, 셀룰로스, 메틸 셀룰로스, 미정질 셀룰로스, 폴리비닐피롤리돈, 물, 메틸하이드록시벤조에이트, 프로필하이드록시벤조에이트, 탈크, 마그네슘 스테아레이트 또는 광물유 등이 사용될 수 있다. 또한, 충진제, 항응집제, 윤활제, 습윤제, 향료, 방부제 등을 추가로 포함할 수 있다.
또한, 본 발명의 약학적 조성물은 정제, 환제, 산제, 과립제, 캡슐제, 현탁제, 내용액제, 유제, 시럽제, 멸균된 수용액, 비수성용제, 동결건조제제 및 좌제로 이루어진 군으로부터 선택되는 어느 하나의 제형을 가질 수 있다.
또한, 상기 조성물은 약학적 분야에서 통상의 방법에 따라 개체의 신체 내 투여에 적합한 단위 투여형의 제제, 구체적으로는 펩타이드 의약품의 투여에 유용한 제제 형태로 제형화시켜 당업계에서 통상적으로 사용하는 투여 방법을 이용하여 경구, 또는 피부, 정맥 내, 근육 내, 동맥 내, 골수 내, 수막강 내, 심실 내, 폐, 경피, 피하, 복 내, 비강 내, 소화관 내, 국소, 설하, 질 내 또는 직장 경로를 포함하는 비경구 투여 경로에 의하여 투여될 수 있으나, 이들에 한정되는 것은 아니다.
또한, 상기 삼중 활성체는 생리식염수 또는 유기용매와 같이 약제로 허용된 여러 전달체 (carrier)와 혼합하여 사용될 수 있고, 안정성이나 흡수성을 증가시키기 위하여 글루코스, 수크로스 또는 덱스트란과 같은 탄수화물, 아스코르브산 (ascorbic acid) 또는 글루타티온과 같은 항산화제 (antioxidants), 킬레이트제, 저분자 단백질 또는 다른 안정화제 (stabilizers) 등이 약제로 사용될 수 있다.
본 발명의 약학적 조성물의 투여량과 횟수는 치료할 질환, 투여 경로, 개체의 연령, 성별 및 체중 및 질환의 중등도 등의 여러 관련 인자와 함께, 활성성분인 약물의 종류에 따라 결정된다.
특별히 이에 제한되지 않으나, 본 발명의 상기 약학적 조성물은 상기 성분 (유효성분)을 0.01 내지 99% 중량 대 부피로 함유할 수 있다.
본 발명의 조성물의 총 유효량은 단일 투여량 (single dose)으로 개체에게 투여될 수 있으며, 다중 투여량 (multiple dose)으로 장기간 투여되는 분할 치료 방법 (fractionated treatment protocol)에 의해 투여될 수 있다. 본 발명의 약학적 조성물은 질환의 정도에 따라 유효성분의 함량을 달리할 수 있다. 구체적으로, 본 발명의 삼중 활성체의 투여 주기는 하루 내지 한달, 예를 들어, 하루, 1주, 2주, 3주, 4주, 또는 한달 일 수 있고, 일주일 동안 투여되는 용량은 개체 체중 1kg당 약 0.005 내지 1.5 mg 일 수 있으나, 이에 제한되지 않는다. 아실화된 삼중 활성체의 경우, 상기 투여량은 삼중 활성체에서 지방산 부위의 질량을 제외한 질량값, 즉 폴리펩타이드 부위들만의 질량을 합산한 값을 기준으로 결정할 수 있다. 그러나 상기 삼중 활성체의 용량은 약학적 조성물의 투여 경로 및 치료 횟수뿐만 아니라 개체의 연령, 체중, 건강 상태, 성별, 질환의 중증도, 식이 및 배설율 등 다양한 요인들을 고려하여 개체에 대한 유효 투여량이 결정되는 것이므로, 이러한 점을 고려할 때 당분야의 통상적인 지식을 가진 자라면 상기 본 발명의 조성물의 특정한 용도에 따른 적절한 유효 투여량을 결정할 수 있을 것이다. 본 발명에 따른 약학적 조성물은 본 발명의 효과를 보이는 한 그 제형, 투여 경로 및 투여 방법에 특별히 제한되지 아니한다.
본 발명의 약학적 조성물은 생체 내 지속성 및 역가가 우수할 수 있으며, 다른 약제에 비해 낮은 투여 횟수 및 빈도를 나타낼 수 있으나, 특별히 이에 제한되는 것은 아니다.
본 발명을 구현하는 또 다른 양태는 상기 삼중 활성체, 또는 이를 포함하는 조성물을 개체에 투여하는 단계를 포함하는, 신장 질환의 예방 또는 치료 방법을 제공한다.
상기 삼중 활성체, 이를 포함하는 조성물, 신장 질환, 예방, 및 치료에 대해서는 앞서 설명한 바와 같다.
본 발명에서 상기 개체는 신장 질환이 의심되는 개체로서, 상기 신장 질환 의심 개체는 해당 질환이 발병하였거나 발병할 수 있는 인간을 포함한 쥐, 가축 등을 포함하는 포유 동물을 의미하나, 본 발명의 삼중 활성체, 혹은 이를 포함하는 상기 조성물로 치료 가능한 개체는 제한 없이 포함된다. 신장 질환에 대해서는 상기에서 설명한 바와 같다.
본 발명의 방법은 삼중 활성체를 포함하는 약학적 조성물을 약학적 유효량으로 투여하는 것을 포함할 수 있다. 적합한 총 1일 사용량은 올바른 의학적 판단범위 내에서 처치의에 의해 결정할 수 있으며, 1회 또는 수회로 나누어 투여할 수 있다. 그러나 본 발명의 목적상, 특정 개체에 대한 구체적인 치료적 유효량은 달성하고자 하는 반응의 종류와 정도, 경우에 따라 다른 제제가 사용되는지의 여부를 비롯한 구체적 조성물, 개체의 연령, 체중, 일반 건강 상태, 성별 및 식이, 투여 시간, 투여 경로 및 조성물의 분비율, 치료기간, 구체적 조성물과 함께 사용되거나 동시 사용되는 약물을 비롯한 다양한 인자와 의약 분야에 잘 알려진 유사 인자에 따라 다르게 적용하는 것이 바람직하다.
본 발명의 방법은 상기 삼중 활성체, 또는 이를 포함하는 조성물은 생체 내 표적에 도달할 수 있는 일반적인 경로를 통하여 투여될 수 있으며, 상기 투여 경로는, 예를 들어 복강 내 투여, 정맥 내 투여, 근육 내 투여, 피하 투여, 피내 투여, 경구 투여, 국소 투여, 비 내 투여, 폐 내 투여, 직장 내 투여 등이 될 수 있다.
본 발명을 구현하는 또 다른 양태는 상기 삼중 활성체, 혹은 상기 조성물의 신장 질환에 대한 예방 또는 치료 용도를 제공한다.
상기 삼중 활성체, 조성물, 신장 질환, 예방, 및 치료에 대해서는 앞서 설명한 바와 같다.
본 발명을 구현하는 또 다른 양태는 상기 삼중 활성체, 혹은 상기 조성물을 신장 질환에 대한 예방 또는 치료용 약제 (혹은 약학적 조성물)의 제조에 사용하는 용도를 제공한다.
상기 삼중 활성체, 조성물, 신장 질환, 예방, 및 치료에 대해서는 앞서 설명한 바와 같다.
한편, 본원 명세서에서 문맥상 달리 요구하지 않는 한, “포함한다”, “포함하는”, “함유하는”, 등의 표현은 명시된 정수 (integer) 또는 정수 그룹의 포함을 의미하지만, 다른 정수 또는 정수의 집합을 배제하는 것이 아니라고 이해되어야 한다.
이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로, 본 발명의 범위가 이들 실시예에 의해 제한되는 것은 아니다.
실시예: 삼중 활성체의 제조
본 발명의 삼중 활성체의 합성은 고체상 합성법을 이용하는 자동 펩타이드 합성기 (Symphony X, Gyros Protein Tech.)를 이용하였다. C-말단의 아미드화를 위해 Rink amide resin을 사용하였으며 각 아미노산을 C-말단에서 N-말단 순서로 합성하였다.
Fmoc (9H-플루오렌-9-일메톡시카르보닐) 보호된 (protected) 아미노산, (펩타이드-resin 대비 4 당량), HOBt (1-히드록시벤조트리아졸, 펩타이드-resin 대비 4 당량), DIC (디이소프로필카르보디이미드, 펩타이드-resin 대비 8 당량)를 사용하여 아미노산을 순차적으로 결합시켰다. 아실화 되어 있는 아미노산, K(1) 혹은 K(2)를 결합 시킬 때에도 위와 같은 방법을 사용하였다.
Fmoc 보호기는 자동 합성기에서 레진 (resin)이 있는 반응 용기에 8 mL의 20% 피페리딘/DMF를 5분씩 2회 가하여 떼어내었다. 남아 있는 불순물을 제거하기 위해 각 과정 후에는 DMF 12 mL로10초씩 6회 세척하였다. 제거되지 않은 보호기는 펩타이드가 레진에서 절단될 때 함께 탈보호 되었다.
이와 같은 과정을 통해 제조된 삼중 활성체의 서열은 다음 표 1에 나타내었다.
상기 표 1에서, K(1) 및 K(2)는 각각 상기 구조를 갖는 아실화된 아미노산 K(1) 및 K(2)를 의미한다. 밑줄 친 아미노산 (16번째 글루탐산 및 20번째 라이신)은 서로 락탐 링 (lactam ring)을 형성하는 것을 의미한다.
제조된 삼중 활성체의 정제는 역상 크로마토그래피를 이용하였다. 합성된 펩타이드는 분석용 액상 크로마토그램 (RP-HPLC)를 이용하여 순도를 확인하였으며, 90%이상인 경우 실험에 사용 가능한 수준으로 판단하여 진행하였다. 또한, 액상 크로마토그램/질량분석기 (LC/MS)를 사용하여 펩타이드의 분자량 및 정보를 확인하였다.
합성된 펩타이드는 실험에 사용되기 전까지 -20℃에 보관하였다.
실험예 1: 삼중 활성체의

활성 확인
상기 실시예에서 제조된 삼중 활성체의 활성을 측정하기 위해 GLP-1 수용체, GIP 수용체, 및 글루카곤 (GCG) 수용체가 각각 형질전환된 세포주를 이용하여 in vitro에서 세포 활성을 측정하였다.
상기 각 세포주는 CHO (Chinese Hamster Ovary)에 인간 GLP-1 수용체, 인간 GCG 수용체 및 인간 GIP 수용체 유전자를 각각 발현하도록 형질 전환된 것으로서, GLP-1, GIP, 및 GCG에 대한 활성을 각각의 형질 전환 세포주를 이용하여 측정하였다.
본 발명의 삼중 활성체의 GLP-1 수용체에 대한 활성 측정을 위해 인간 GLP-1을 4nM 부터 3배씩 0.00007nM까지 연속적으로 희석하고, 서열번호 1 내지 9의 삼중 활성체를 40nM 부터 3배씩 0.00068nM까지 연속적으로 희석하였다. 상기 GLP-1 수용체가 발현된 CHO 세포에서 배양액을 제거하고 연속적으로 희석된 각 물질들을 10㎕씩 상기 세포에 첨가한 뒤 15분 동안 상온에서 배양하였다. 이후, 세포용해완충액 (cell lysis buffer)이 포함된 Eu-cAMP tracer mix와 anti-cAMP detection mix를 순서대로 각각 5㎕씩 가하여 세포를 용해시키고, 차광조건에서 60분 동안 상온에서 반응시켰다. 상기 반응이 완료된 세포용해물에 축적된 cAMP를 형광으로 측정하고, 이를 통해 EC50값을 산출한 후 상호 비교하였다. GLP-1 대비 상대 역가는 하기 표 2에 나타내었다.
본 발명의 삼중 활성체의 GCG (글루카곤) 수용체에 대한 활성 측정을 위해 인간 GCG를 4nM 부터 3배씩 0.00007nM까지 연속적으로 희석하고, 서열번호 1 내지 9의 삼중 활성체를 40nM 부터 3배씩 0.00068nM까지 연속적으로 희석하였다. 상기 GCG 수용체가 발현된 CHO 세포에서 배양액을 제거하고 연속적으로 희석된 각 물질들을 10㎕씩 상기 세포에 첨가한 뒤 15분 동안 상온에서 배양하였다. 이후, 세포용해완충액 (cell lysis buffer)이 포함된 Eu-cAMP tracer mix와 anti-cAMP detection mix를 순서대로 각각 5㎕씩 가하여 세포를 용해시키고, 차광조건에서 60분 동안 상온에서 반응시켰다. 상기 반응이 완료된 세포용해물에 축적된 cAMP를 형광으로 측정하고, 이를 통해 EC50값을 산출한 후 상호 비교하였다. GCG 대비 상대 역가는 하기 표 2에 나타내었다.

이를 통해, 본 발명의 삼중 활성체 (서열번호 1 내지 9)는 GLP-1, GIP, 및 글루카곤 수용체의 3개 수용체에 대해 모두 활성을 갖는 것을 확인하였다.
실험예 2: 일반 마우스에서 삼중 활성체에 의한 혈당 강하 효과
본 발명의 삼중 활성체의 혈당 강하 효과를 확인하기 위하여 일반 마우스에서 복강내 당부하검사 (ipGTT)를 수행하였다.
이를 위해 서열번호 1 내지 6의 각 삼중 활성체를 해당 검사 전날 0.25 mg/kg 용량으로 단회 피하투여 진행하였다. 검사 당일, 4시간 금식을 한 마우스 복강 내로 포도당을 주입한 후 혈당 변화 정도를 측정하였다. 혈당은 꼬리 정맥에서 전혈을 한 방울 취하여 측정하였고, 검사 1시간 전과 당부하 (1 g/kg) 후 각 0, 0.25, 0.5, 1, 2 시간에 진행하였다. 혈당 변화 그래프를 바탕으로 농도곡선하면적 (Area under curve)을 산출하였고, 부형제 대조군과의 % 비교를 통해 혈당 강하 효과를 확인하였다. 통계 처리는 일원 ANOVA를 사용하였다 (***p<0.001 vs. 일반 마우스 부형제 대조군).
그 결과, 도 1에 나타난 바와 같이, 각 삼중 활성체는 부형제 투여군 대비 통계적으로 유의미한 혈당 강하 효과를 지닌 것을 확인하였다. 구체적으로, 서열번호 6의 삼중 활성체 투여군의 AUC가 부형제 대조군에 비해 58.17% 감소하는 것을 확인하였다.
실험예 3: 만성 신장 질환 래트모델에서 신장기능 개선효과
상기 실시예에서 제조된 삼중 활성체의 만성 신장 질환 (chronic kidney disease, CKD) 개선 효능을 확인하고자, 고혈압 및 CKD 질환 모델로 알려진 spontaneous hypertensive rat (SHR) 모델을 사용하였다.
먼저, 혈압 상승에 의해 신장기능 이상이 생긴다고 알려진 18주령의 SHR 모델을 부형제 대조군, 서열번호 6의 삼중 활성체 (10, 20, 40 nmol/kg, Q3D, 피하) 투여군으로 나누었고, 16주 반복투여를 진행하였다. 음성대조군으로는 위스타-교토 래트 (Wistar-Kyoto Rat; WKY), 부형제 대조군을 사용하였다. 16주 반복투여 중 신장의 기능을 평가하고자, 약물 투약 4주, 7주, 12주, 그리고 16주 시점에서의 소변 알부민 크레아티닌 비율 (uACR)을 측정하였다. 통계 처리는 일원 ANOVA를 사용하였다 (***p<0.001 vs. SHR 부형제 대조군).
그 결과, 4주 후에 서열번호 6의 삼중 활성체의 모든 용량 투여군에서 소변 알부민 크레아티닌 비율 (uACR)이 SHR 부형제 대조군 대비 유의적으로 감소하기 시작하였으며, SHR 부형제 대조군에서 16주까지 uACR이 증가하는 반면, 투여군은 16주까지 SHR 부형제 대조군과 비교하여 유의적 차이가 나타났다 (도 2). 구체적으로, SHR 부형제 대조군에서는 uACR이 566.60 mg/g인 반면, 서열번호 6의 삼중 활성체 40 nmol/kg 투여군에서는 67.71 mg/g인 것을 확인하였다. 위스타-교토 래트 (WKY) 부형제 대조군에서는 uACR이 27.29 mg/g 이었다.
이러한 신장기능 개선을 통해 본 발명의 삼중 활성체가 고혈압 유래 만성 신장 질환 치료에 효능이 있음을 확인하였다.
실험예 4: 급성 신손상 마우스모델에서 신장 섬유화 개선효과
상기 실시예에서 제조된 삼중 활성체의 급성 신손상 신장질환 (acute kidney injury, AKI) 개선 효능을 확인하고자, 일측요로폐쇄 (unilateral ureteral obstruction, UUO) 마우스 모델을 사용하였다.
일측성 요관 폐쇄 (UUO)와 sham 수술을 시행한 다음날 부형제 대조군, 서열번호 6의 삼중 활성체 (10, 20, 60 nmol/kg, Q2D, 피하) 투여군으로 나누었고, 3회 반복투여를 진행하였다. 음성대조군으로는 sham 수술을 시행한 마우스, UUO 부형제 대조군을 사용하였다. 반복투여 후 (수술 후 7일 후) 개별 동물의 폐쇄시킨 요관 측 신장을 적출하여 신장 섬유화 정도를 측정하고자 Pro-collagen 1 α1 ELISA (enzyme-linked immunosorbent assay)를 시행하였다. 통계 처리는 일원 ANOVA를 사용하였다 (*p<0.05, **p<0.01, ***p<0.001 vs. UUO 부형제 대조군).
그 결과, 서열번호 6의 삼중 활성체 20, 60 nmol/kg 투여 후 신장의 pro-collagen1α1이 UUO 부형제 대조군 대비 유의적으로 감소하였다 (도 3). 구체적으로, UUO 부형제 대조군에서는 pro-collagen1α1이 14.51 ug/g 인 반면, 서열번호 6의 삼중 활성체 60 nmol/kg 투여군에서는 5.25 ug/g 인 것을 확인하였다. sham 수술 마우스 부형제 대조군에서는 12 ug/g 이었다.
이러한 신장 섬유화 개선을 통해 본 발명의 삼중 활성체가 급성 신손상 치료에 효능이 있음을 확인하였다.
이상의 설명으로부터, 본 발명이 속하는 기술분야의 당업자는 본 발명이 그 기술적 사상이나 필수적 특징을 변경하지 않고서 다른 구체적인 형태로 실시될 수 있음을 이해할 수 있다. 이와 관련하여, 이상에서 기술한 실시예들은 모든 면에서 예시적인 것이며 제한적인 것이 아닌 것으로 이해하여야 한다. 본 발명의 범위는 전술한 상세한 설명보다는 후술되는 특허 청구범위의 의미 및 범위, 그리고 그 등가 개념으로부터 도출되는 모든 변경 또는 변형된 형태가 본 발명의 범위에 포함되는 것으로 해석되어야 한다.

- GLP-1 (Glucagon-like peptide-1) 수용체, GIP (Glucose-dependent insulinotropic polypeptide) 수용체, 및 글루카곤 수용체에 대해 활성을 갖는 펩타이드를 포함하는 신장 질환 예방 또는 치료용 약학적 조성물로서,상기 펩타이드는 하기 일반식 1로 표시된 아미노산 서열을 포함하는 것인, 조성물:X1-Aib-X3-G-T-F-T-S-D-Y-S-X12-X13-L-D-E-X17-X18-A-K-X21-F-V-Q-W-L-L-D-X29-H-P-S-S-G-Q-P-P-P-S (일반식 1, 서열번호 29)상기 일반식 1에서,X1은 히스티딘 또는 티로신이고;X3은 글루타민 또는 히스티딘이고;X12는 라이신 또는 아실화된 아미노산이고;X13은 알파-메틸 류신, 티로신 또는 알라닌이고;X17은 라이신 또는 아실화된 아미노산이고;X18은 알라닌 또는 아르기닌이고;X21은 아스파르트산 또는 글루탐산이고;X29는 히스티딘 또는 글루타민이고,상기 -은 펩타이드 결합을 나타내고, 상기 펩타이드는 시스테인을 포함하지 않음.
- 제1항에 있어서, 상기 펩타이드는 천연형 글루카곤 대비 글루카곤 수용체에 대한 상대적 활성에 비해 각각의 천연형 GIP 및 GLP-1 대비 GIP 수용체 및 GLP-1 수용체에 대한 상대적 활성이 높은 것인, 조성물.
- 제1항 또는 제2항에 있어서, 상기 펩타이드는 천연형 글루카곤 대비 글루카곤 수용체에 대한 상대적 활성에 비해 각각의 천연형 GIP 및 GLP-1 대비 GIP 수용체 및 GLP-1 수용체에 대한 상대적 활성이 각각 4배 이상인 것인, 조성물.
- 제1항 내지 제3항 중 어느 한 항에 있어서, 상기 일반식 1에서,X3이 히스티딘인, 조성물.
- 제1항 내지 제4항 중 어느 한 항에 있어서, 상기 일반식 1에서,X13은 티로신이고;X18은 알라닌인, 조성물.
- 제1항 내지 제5항 중 어느 한 항에 있어서, 상기 일반식 1에서,X1은 티로신이고;X3은 글루타민이고;X13은 티로신이고;X18은 아르기닌인, 조성물.
- 제1항 내지 제6항 중 어느 한 항에 있어서, 상기 일반식 1에서,X29는 글루타민인, 조성물.
- 제1항 내지 제7항 중 어느 한 항에 있어서, 상기 펩타이드는 펩타이드의 하나 이상의 아미노산에 AEEA ((2-(2-aminoethoxy)ethoxy)acetic acid), GABA (4-Aminobutyric acid), Ava (5-Aminovaleric acid), Ahx (Aminohexanoic acid), 트리아졸 (Triazole), 및 폴리에틸렌 글리콜 (PEG)로 이루어진 그룹에서 선택되는 링커를 통해 아실기가 부착된 것인, 조성물.
- 제8항에 있어서, 상기 링커는 AEEA를 포함하는 것인, 조성물.
- 제8항 또는 제9항에 있어서, 상기 링커는 AEEA를 0 내지 3개 포함하는 것인, 조성물.
- 제1항 내지 제10항 중 어느 한 항에 있어서, 상기 아실화된 아미노산은 하기 K(1) 또는 K(2)로 표시되는 아미노산인, 조성물:.
- 제1항 내지 제11항 중 어느 한 항에 있어서, 상기 펩타이드는 C-말단이 아미드화된 것인, 조성물.
- 제1항 내지 제12항 중 어느 한 항에 있어서, 상기 펩타이드는 하기 (i) 내지 (iv)의 구조 중 어느 하나의 구조를 갖는 것인, 조성물:(i);(ii);(iii);(iv).
- 제1항 내지 제13항 중 어느 한 항에 있어서, 상기 펩타이드는 서열번호 1 내지 28의 아미노산 서열로 이루어진 군에서 선택되는 어느 하나의 서열을 포함하는 것인, 조성물.
- 제1항 내지 제14항 중 어느 한 항에 있어서, 상기 일반식 1의 N-말단으로부터 16번 아미노산과 20번 아미노산은 서로 고리를 형성하는, 조성물.
- 제1항 내지 제15항 중 어느 한 항에 있어서, 상기 펩타이드는 하기 일반식 2의 구조를 갖는 것인, 조성물:[일반식 2]상기 일반식 2에서,상기 n은 16 또는 18이고,X1은 히스티딘 또는 티로신이고;X3은 글루타민 또는 히스티딘이고;X13은 알파-메틸 류신, 티로신 또는 알라닌이고;X18은 알라닌 또는 아르기닌이고;X21은 아스파르트산 또는 글루탐산이고;X29는 히스티딘 또는 글루타민임.
- 제1항 내지 제16항 중 어느 한 항에 있어서, 상기 신장 질환은 급성 신부전, 급성 신장염, 당뇨병성 신부전, 사구체 신염, 만성 신장 질환, 만성 신부전, 세관 장애, 신증후군, 신우신염, 신성 고혈압, 요독증, 고혈압성 신장병, 신장 섬유증, 신장 아밀로이드증, 요독증성 발작, 신장 결석, 수신증, 신장낭종, 신장종양, 다낭성 신장 질환, 급성 신염 증후군, 무증상 요이상, 진통제성 신병증, 신동맥 협착, 및 신경화증으로 이루어진 군에서 선택되는 어느 하나 이상인 것인, 조성물.
- 제1항 내지 제17항 중 어느 한 항에 있어서, 상기 조성물은 약학적으로 허용 가능한 담체를 추가로 포함하는 것인, 조성물.
- 제1항 내지 제18항 중 어느 한 항에 있어서, 상기 조성물은 투여시 신장 기능 회복 효과 또는 신장 섬유화 개선 효과를 갖는 것인, 조성물.
