Abstract
【課題】耐キンク性の向上が図られるとともに、生体管腔の湾曲や蛇行した部位に対する高い追従性を備えたシース部材を有する医療用具を提供する。 【解決手段】シース部材110の内層120の外面120aと外層130の内面130bとの間に配置された補強部材140は、屈曲状態のとき、第1領域131Aの第1側面部131a側に移動して第1領域に力を付与し、第1領域の第1側面部と接触しつつ、第1領域の第2側面部131bとの間に空間150を形成するように外層の第2領域132を変形させる。
Description
本発明は、医療用具に関する。
従来から、生体器官内で行われる各種の治療行為を実施するに際し、可撓性を備える管状の中空部材(シース部材)を備える医療用具が使用されている。この種の医療用具として、バルーンカテーテル等のカテーテルデバイスを生体内の所望の位置まで送達する際に使用されるガイディングカテーテル、造影剤を生体内に吐出する際に使用される造影カテーテル、薬剤の吐出等に使用されるマイクロカテーテル、患者の血管等の生体管腔へガイドワイヤやカテーテル等の医療器具を導入するために使用されるシースイントロデューサー等が知られている。
上記の医療用具では、シース部材に耐キンク性(折れ曲がりの生じ難さ)を備えうるようにするために、所望の物性を備える内層及び外層からなる層構造を構成させ、さらに内層と外層との間に補強部材(例えば、金属製のブレード線)を配置することがある。例えば、特許文献1には、内層と、外層と、内層と外層との間に配置された補強部材と、を備えるシース部材を有するシースイントロデューサーが開示されている。
特許文献1に記載されたシース部材によれば、補強部材によってシース部材の耐キンク性が良好に高められうるものの、補強部材が配置されることによるシース部材の柔軟性の低下が生じてしまう。そのため、特許文献1に記載されたシース部材を使用した手技では、生体管腔の湾曲や蛇行した部位に追従するようにシース部材を変形させることが難しくなる。それにより、シース部材を使用した円滑な手技が妨げられてしまう可能性がある。
本発明は、上記課題を鑑みてなされたものであり、耐キンク性の向上が図られるとともに、生体管腔の湾曲や蛇行した部位に対する高い追従性を備えたシース部材を有する医療用具を提供することを目的とする。

本発明の医療用具が備えるシース部材は、内層と、外層と、内層の外面と外層の内面との間に配置された補強部材を備える。シース部材は、補強部材を備えることにより、耐キンク性の向上が図られる。また、外層は、内層の外面と固着した第1領域と、内層の外面と固着していない第2領域を有する。第2領域は、内層の長手方向に沿って螺旋状に延在しており、内層の外面と外層の内面との間に隙間部を形成している。補強部材は、隙間部に螺旋状に配置されており、シース部材が屈曲状態となったときに、第1領域の第1側面部と接触しつつ、第1領域の第1側面部と対向する第2側面部との間に空間を形成する。シース部材は、屈曲状態となったとき、第1側面部と接触した補強部材によって内層の長手方向に沿う第2領域の幅が大きくなるように変形する。そのため、シース部材は、補強部材が配置された第2領域に位置する外層が屈曲方向に沿って優先的に伸びるように変形し、内層と外層が固着された第1領域の伸びを抑制できる。これにより、シース部材は、屈曲状態となったとき、内層と外層の剥離が軽減されることで、シース部材がキンクし、シース部材の内腔が保持できない状態になることを抑制できる。また、シース部材は、屈曲状態となったとき、外層の第2領域の隙間部内に空間が形成されることによって柔軟性が向上し、生体管腔の湾曲や蛇行した部位等に対する高い追従性を発揮することができる。以上のように、本発明によれば、耐キンク性の向上が図られるとともに、生体管腔の湾曲や蛇行した部位に対する高い追従性を備えたシース部材を有する医療用具を提供することができる。

医療用具100は、シース部材110の内腔115を介して各種の医療デバイスを生体管腔(例えば、血管)へ導入するために使用することができる。医療用具100の具体的な使用方法や使用手順等は特に限定されないが、例えば、医療用具100は、比較的大径な下肢を走行する血管を挿入対象部位とし、下肢の血管から生体内の各部へ各種の医療デバイス(例えば、大動脈留置用のステントや大動脈弁狭窄症の治療のための人工弁置換術に使用されるデバイス、肺血栓症治療用のデバイス等)を送達するためのアクセス経路を形成するために使用することができる。
本明細書の説明において、図1、図2、図3に示す自然状態においてシース部材110が延在する方向を「軸方向」と称し、矢印X1-X2で示す。また、矢印X1で示す方向を軸方向の先端側と定義し、矢印X2で示す方向を軸方向の基端側と定義する。また、自然状態におけるシース部材110の軸方向に沿う仮想直線を符号c1で示す。
本明細書における「自然状態」は、シース部材110に対して外力が付与されておらず、シース部材110が略直線状に延在した状態を意味する。また、以下に説明する「屈曲状態」は、シース部材110の任意の箇所に対して外力を付与し、シース部材110の一部又は全体が所定の屈曲中心O1(図4を参照)を基準にして屈曲した際の状態を意味する。
後述するように、シース部材110の外層130の第2領域132は、自然状態において第1の幅W21を有し(図3に示す状態)、屈曲状態となったときに第2の幅W22を有する(図4に示す状態)ように変形可能に構成されている。
<シース部材110>

図1、図2に示すように、シース部材110は、最先端の位置に先端開口部111aが形成された先端部111と、ハブ部材160の内部に配置された基端部113と、を有する。図示省略するが、シース部材110の基端部113には、ハブ部材160の内部と連通する基端開口部が設けられている。
シース部材110は、先端部111から基端部113まで連続的に延在する内腔115を備える。シース部材110の先端部111は、先端側に向けて外径が先細るテーパー形状を有するように構成することができる。
図3に示すように、シース部材110は、外面120a及び内面120bを有する内層120と、外面130a及び内面130bを有する外層130と、内層120の外面120aと外層130の内面130bとの間に位置する補強部材140と、を備える。
内層120の内面120bは、内腔115に面する内側の面である。内層120の外面120aは、外層130側に位置する外側の面である。
外層130の内面130bは、内腔115側に位置する内側の面である。外層130の外面130aは、シース部材110の外面をなす外側の面である。
図3に示すように、外層130は、内層120の外面120aと固着した第1領域131と、内層120の外面130aと固着していない第2領域132と、を有する。
第1領域131は、各層120、130を形成する樹脂材料同士を第1領域131に相当する部分で互いに融着させることで形成することができる。
第2領域132は、第1領域131と隣り合う位置で、内層120の長手方向に沿って螺旋状に配置されており、かつ、内層120の外面120aと外層130の内面130bとの間に隙間部132aを形成している。
図2、図3に示すように、補強部材140は、内層120及び外層130と固着されていない状態で、隙間部132aに螺旋状に配置されている。
図2~図4において、螺旋状に配置された補強部材140の近接した2つの部分には、両者を明示するために符号140A、140Bを付している。符号140Aで示す部分は、符号140Bで示す部分よりもシース部材110の先端側に位置する。また、図2、図5において、補強部材140の先端(最先端)の部分には符号140Cを付しており、補強部材140の先端の部分140Cの基端側に隣接する部分には符号140Dを付している。
図1、図2に示すように、隙間部132aは、シース部材110の先端部111を除き、シース部材110の先端部111よりも基端側の範囲に亘って軸方向に所定の間隔を空けて連続的に形成されている。そのため、隙間部132aに配置された補強部材140は、隙間部132aと同様に、シース部材110の先端部111を除き、シース部材110の先端部111よりも基端側の範囲に亘って軸方向に所定の間隔を空けて連続的に形成されている。
図2、図3に示すように、補強部材140は、自然状態において、軸方向において隣り合う各部分140A、140Bの間に所定の隙間が形成されるように延在させることができる。つまり、補強部材140は、自然状態において、図2に示すようなシース部材110の側面図で、互いに軸周り方向に重ならない螺旋のピッチ(図2の側面図に示す補強部材140の任意の部分140Aの基端の位置から中心軸c1と直交する方向に延びる仮想直線H1と任意の部分140Aの基端側に隣接する部分140Bとが重ならないピッチ)を有するように配置することができる。シース部材110は、自然状態において補強部材140同士が重ならないように配置されることで、シース部材110の可撓性を高めることができる。特に、シース部材110は、自然状態において補強部材140同士が重ならないように配置されることで、シース部材110が屈曲状態になったとき、補強部材140が第1側面部131a側に移動し、補強部材140が互いに軸周り方向に重なる範囲が増加することを抑制できる。そのため、このように構成することで、シース部材110は、屈曲状態になったときでも、シース部材110の可撓性を高めることができる。
第2領域132は、内層120の長手方向で第1の幅W21を有する自然状態(図3に示す状態)と、内層120の長手方向で第1の幅W21よりも大きい第2の幅W22を有する屈曲状態(図4に示す状態)と、の間で変形可能に構成される。
本明細書において、図3、図5の符号A1で示す「自然状態の内層120の長手方向」は、符号c1で示すシース部材110の軸方向と平行な方向を意味する。つまり、自然状態の内層120の長手方向は、軸方向と同一の方向である。また、図4に示す符号A1’で示す「屈曲状態の内層120の長手方向」は、屈曲した内層120に沿う曲線の線分(弧)に沿った方向を意味する。
図3に示すように、第1領域131は、第1側面部131aと、隙間部132aを挟んで第1側面部131aと対向する第2側面部131bと、を有する。
本明細書では、第1領域131においてシース部材110の先端部111よりも基端側に位置する部分には、符号131Aを付している(図3、図4を参照)。また、第1領域131においてシース部材110の先端部111に位置する部分には、符号131B(図5を参照)を付している。
第1側面部131aは、図3に示す自然状態の断面において、補強部材140の先端側に位置する第1側面140c側に配置された面である。また、第2側面部131bは、図3に示す自然状態の断面において、補強部材140の基端側に位置する第2側面140d側に配置された面である。
補強部材140は、図4に示す屈曲状態のとき、第1側面部131a側に移動して第1領域131Aに力を付与し、第1側面部131aと接触しつつ、第2側面部131bとの間に空間150を形成するように第2領域132を変形させる。
具体的には、図4に示すように、シース部材110が屈曲した際、補強部材140(補強部材140の各々の部分140A、140B)は、第1側面部131a側に移動して、第1側面140cを第1側面部131aに接触させつつ、第2領域132の幅W22を内層120の長手方向に沿って広げることで、第2側面140dと第2側面部131bとの間に空間150を形成する。
シース部材110は、上記のように空間150を形成することで、補強部材140が配置された第2領域132に位置する外層130が屈曲方向に沿って優先的に伸びるように変形し、内層120と外層130が固着された第1領域131の伸びを抑制できる。これにより、シース部材110は、屈曲状態となったとき、内層120と外層130の剥離が軽減されるため、シース部材110がキンクし、シース部材110の内腔115が保持できない状態になることを抑制できる。
また、シース部材110は、屈曲状態となったとき、第2領域132の隙間部132a内に空間150が形成されることにより、柔軟性が向上し、生体管腔の湾曲や屈曲した部位等に対して高い追従性を発揮することができる。
図3に示すように、補強部材140は、自然状態のとき、第1側面部131a及び第2側面部131bと接触するように構成される。
具体的には、シース部材110は、自然状態のとき、補強部材140の第1側面140cが第1領域131Aの第1側面部131aに接触し、補強部材140の第2側面140dが第1領域131Aの第2側面部131bに接触する。
上記のようにシース部材110は、自然状態のときに、補強部材140と第1領域131Aの各側面部131a、131bとの間に空間150が形成されないため、補強部材140と第1領域131Aに位置する外層130との間の密着性を高めることができる。そのため、シース部材110は、自然状態において、シース部材110を直線形状に維持し易くなる。それにより、医療用具100は、シース部材110を生体管腔に挿入する際等に、術者等が付与する押込み力をシース部材110の先端側に伝達する押込み性を確保でき、かつ、シース部材110に折れや撓みが発生することを効果的に防止できる。
図3に示すように、隙間部132aは、自然状態のとき、内層120の長手方向に沿う断面が矩形形状を有する。また、補強部材140は、矩形形状の断面を備える金属製の帯状部材で構成することができる。
図3に示すように、補強部材140は、自然状態において、第2領域132に位置する外層130側に配置される外面140aと、内層120の外面120a側に配置される内面140bと、第1領域131Aの第1側面部131aと接触する第1側面140cと、第1領域131Aの第2側面部131bと接触する第2側面140dと、を有する長方形の断面形状を有する。
図3に示すように、隙間部132aは、自然状態において、補強部材140と略同一の長方形の断面形状を有する。
上記のようにシース部材110は、隙間部132aが内層120の長手方向に沿う断面が矩形形状で構成されており、補強部材140が矩形形状の断面を備える金属製の帯状部材で構成されているため、自然状態において、第1領域131Aの各側面部131a、131bと補強部材140の各側面140c、140dの接触面積を大きくすることができる。そのため、シース部材110は、自然状態から屈曲状態へ変形する際、補強部材140の第1側面140cから第1領域131Aの第1側面部131aに対して効率良く力を付与することができる。それにより、シース部材110は、屈曲状態において、所望の幅W4(図4を参照)を備える空間150をより確実に形成することができる。
なお、内層120の長手方向に沿う補強部材140の断面は、矩形形状ではなく、円形や楕円形等であってもよい。ただし、上記のように屈曲状態に所望の幅W4を備える空間150をより確実に形成させる観点より、補強部材140の断面は矩形形状であることが好ましい。
図4に示す内層120の長手方向に沿う空間150の幅W4は、図3に示す自然状態における内層120の長手方向に沿う補強部材140の断面の幅W31の半分以下で形成することができる。
シース部材110は、空間150の幅W4が過剰に大きな寸法で構成されていると、屈曲状態となったときに、空間150付近でシース部材110が過剰に折れ曲がり、シース部材110にキンクを発生させてしまう可能性がある。上記のように屈曲状態における空間150の幅W4を補強部材140の断面の幅W32の半分以下の大きさで形成することにより、空間150の大きさが第2領域132において補強部材140に対して過剰に大きくなることを防止できる。それにより、屈曲状態でのシース部材110の剛性を確保し、シース部材110にキンクが発生することを効果的に防止できる。
図3に示す自然状態における内層120の長手方向に沿う補強部材140の断面の幅W31は、自然状態における内層120の長手方向に沿う第1領域131Aの幅W11よりも大きく形成することができる。
シース部材110は、上記のように補強部材140の断面の幅W31が自然状態における第1領域131Aの幅W11よりも大きく形成されているため、シース部材110の軸方向の単位長さ当たりにおける補強部材140の配置割合が大きくなる。そのため、シース部材110は、補強部材140による補強効果を高めることができ、耐キンク性がより一層高められたものとなる。
図5には、シース部材110の先端部111(最先端から基端側の一定範囲を含む部分)の自然状態における断面図を示す。
図5に示すように、シース部材110の先端部111には隙間部132a及び補強部材140が配置されていない。
また、シース部材110の先端部111よりも基端側に配置された補強部材140の最先端に位置する一の部分140Cと一の部分140Cと基端側で隣接する他の部分140Dは固定されている。
補強部材140の一の部分140Cと他の部分140Dは、例えば、一の部分140Cの基端側に位置する第2側面140dと他の部分140Dの先端側に位置する第1側面140cとをレーザー溶着することで固定することができる。
シース部材110は、上記のように補強部材140の最先端に位置する一の部分140Cと、一の部分140Cと基端側に隣接する他の部分140Dとが固定されている。このため、図5に示す自然状態における内層120の長手方向に沿う補強部材140の幅(補強部材140の一の部分140Cと他の部分140Dとを一体化した部分の補強部材140の幅)は、各部140C、140Dよりも基端側に位置する補強部材140の他の部分の幅W31(図3を参照)よりも長い。そのため、シース部材110の先端部111付近が屈曲状態となったとき、補強部材140の最先端に位置する一の部分140Cが内層120の長手方向の先端側に向かって移動しにくくなる。これにより、シース部材110の先端部111付近が屈曲状態となったとき、補強部材140全体が内層120の長手方向の先端側に向かって過度に移動することを抑制できる。
図5に示す自然状態において内層120の長手方向に沿う第1領域131Bの幅W5は、例えば、自然状態における第1領域131Aの幅W11(図3を参照)よりも長い方が好ましい。これは次のような理由による。
シース部材110が屈曲した際、補強部材140の最先端に位置する一の部分140Cは、シース部材110の先端部111よりも基端側に位置する第1領域131の幅W11よりも大きな幅W5を備える第1領域131Bの第1側面部131aと接触する。シース部材110の先端部111に位置する第1領域131Bは、上記のような幅W5を備えることにより、幅W11を備える第1領域131Aよりも内層120及び外層130間の固着力(融着力)が高い。したがって、補強部材140の最先端に位置する一の部分140Cがシース部材110の先端部111に位置する第1領域131Bの第1側面部131aに接触した際、一の部分140Cが先端側へ過剰に移動することを効果的に防止することができる。また、シース部材110は、上記のように一の部分140Cが先端側へ過剰に移動することを防止できるため、補強部材140の他の部分140Dの第2側面140dと、その基端側に位置する第1領域131の第2側面部131bとの間に隙間が生じにくくなる。そのため、先端部111よりも基端側に位置する補強部材140の各部140A、140Bのピッチ間距離が過剰に広がることを防ぐことができる。
一方で、シース部材110の先端部111に対して外力が付与されて、補強部材140の各部140C、140Dが配置された第2領域132が屈曲状態になると、補強部材140の各部140C、140Dが一体となった状態で、補強部材140の一の部分140Cの第1側面140cが、その先端側に位置する第1領域131Bの第1側面部131aに接触し、第1領域131Aに力を付与する。そのため、上記のように補強部材140の一の部分140Cの第1側面140cが第1領域131Bの第1側面部131aに接触しつつ、補強部材140の他の部分140Dの第2側面140dと、その基端側に位置する第1領域131の第2側面部131bとの間に、ある程度の空間150(図4を参照)が形成される。したがって、シース部材110は、シース部材110の先端部111付近が屈曲状態となったときにも、シース部材110にキンクが発生することを防止でき、かつ生体管腔の屈曲や湾曲した部位等に対する追従性を発揮することができる。
なお、シース部材110において補強部材140及び第2領域132を設ける位置は特に限定されない。ただし、生体管腔に挿入した際にシース部材110が受ける負荷等を考慮して、補強部材140及び第2領域132は、少なくともシース部材110の先端側を含む一定の範囲に設けることが好ましい。また、前述したように先端側への補強部材140の過度な移動を抑制する観点より、シース部材110の先端部111を含む一定の範囲には補強部材140及び第2領域132を設けないことが好ましい。
シース部材110の各部の寸法は、例えば、次のように形成することができる。
図3に示す自然状態における内層120の長手方向に沿う第1領域131A(先端部111よりも基端側に位置する部分)の幅W11は、例えば、0.2mm以上0.6mm以下に形成することができる。また、図4に示す屈曲状態における内層120の長手方向に沿う第1領域131A(先端部111よりも基端側に位置する部分)の幅W12は、例えば、0.3mm以上0.7mm以下に形成することができる。
図3に示す自然状態における内層120の長手方向に沿う第2領域132の第1の幅W21は、例えば、0.3mm以上0.7mm以下に形成することができる。また、図4に示す屈曲状態における内層120の長手方向に沿う第2領域132の第2の幅W22は、第1の幅21よりも大きく形成することができ、例えば、0.5mm以上1.0mm以下に形成することができる。
図3に示す自然状態における内層120の長手方向に沿う補強部材140の幅W31は、自然状態における内層120の長手方向に沿う第1領域131Aの幅W11よりも大きく形成することができ、例えば、0.3mm以上0.7mm以下に形成することができる。
なお、補強部材140が比較的硬度の高い金属材料で構成されている場合、図3に示す自然状態における内層120の長手方向に沿う補強部材140の幅W31と、図4に示す屈曲状態における内層120の長手方向に沿う補強部材140の幅W32は、略同一の大きさとなる。本実施形態では、図4に示すように、補強部材140は、屈曲状態において内層120の長手方向に沿うように屈曲した形状を呈するように図示している。例えば、本実施形態のように屈曲状態において補強部材140の断面形状が屈曲するように変形する場合、補強部材140の幅W32は、図4に示すように屈曲(湾曲)した内層120の長手方向に沿う曲線A1と同一の曲率の曲線に沿う第1側面140cと第2側面140d間の線分の距離で定義することができる。
図4に示す屈曲状態における内層120の長手方向に沿う空間150の幅W4は、自然状態における内層120の長手方向に沿う補強部材140の幅W31の半分以下で形成することができ、例えば、0.15mm以上0.35mm以下に形成することができる。
図5に示す内層120の長手方向に沿う第1領域131Bの幅W5は、屈曲状態になった際、先端部111以外の部分における第1領域131Aの幅W11(図3を参照)よりも大きく形成することができ、例えば、3mm以上15mm以下に形成することができる。
シース部材110の各部は、例えば、次のような材料で構成することができる。
内層120を構成する材料としては、例えば、ポリテトラフルオロエチレン(PTFE)、四フッ化エチレン・パーフルオロアルコキシエチレン共重合体(PFA)等のフッ素系樹脂材料、ポリスチレン、ポリオレフィン、ポリウレタン、ポリエステル、ポリアミド等の樹脂材料、スチレン系、ポリオレフィン系、ポリウレタン系、ポリエステル系、ポリアミド系、ポリブタジエン系、トランスポリイソプレン系、フッ素ゴム系、塩素化ポリエチレン系等の各種熱可塑性エラストマー材料、及びこれらの材料のうち2種以上を組み合わせたもの(ポリマーアロイ、ポリマーブレンド、積層体等)などを用いることができる。
外層130を構成する材料としては、例えば、ポリスチレン、ポリオレフィン、ポリウレタン、ポリエステル、ポリアミド等の樹脂材料、スチレン系、ポリオレフィン系、ポリウレタン系、ポリエステル系、ポリアミド系、ポリブタジエン系、トランスポリイソプレン系、フッ素ゴム系、塩素化ポリエチレン系等の各種熱可塑性エラストマー材料、及びこれらの材料のうち2種以上を組み合わせたもの(ポリマーアロイ、ポリマーブレンド、積層体等)などを用いることができる。なお、シース部材110は、内層120を構成する材料に、外層130を構成する材料と同じ材料を用いて形成してもよい。
補強部材140は、例えば、内層120を構成する樹脂材料及び外層130を構成する樹脂材料よりも硬質な材料で構成することができる。各層120、130を上記に例示した樹脂材料で構成する場合、補強部材140を構成する材料として、例えば、SUS304、SUS316、析出硬化ステンレス(PHステンレス)等のステンレス鋼、タングステン、アルミニウム、Ni-Ti合金等の金属材料を用いることができる。
<ハブ部材160>
図1に示すように、ハブ部材160の内部には、弁体161が配置されている。弁体161は、ダイレーターチューブ210や生体管腔に挿入される各種の医療デバイスを挿通可能に構成されている。

弁体161は、ダイレーターチューブ210や医療デバイスが挿通された状態において、ダイレーターチューブ210や医療デバイスとの間に隙間が形成されることを防止し、血液やハブ部材160の内部に注入した生理食塩水等の液体がハブ部材160の基端側へ逆流することを防止する。
ハブ部材160は、ハブ部材160の内部に連通する第1ポート部165と、第2ポート部167と、を有する。
第1ポート部165には、生理食塩水等の液体をハブ部材160の内部に供給するための三方活栓173と接続されるチューブ171を接続することができる。
第2ポート部167には、所定のチューブを介して吸引用のデバイスを接続することができる。吸引用のデバイスは、静脈内の血栓等を吸引するための手技を実施する場合に使用することができる。なお、第2ポート部167の設置は、医療用具100の使用用途等に応じて適宜省略することができる。
医療用具100は、ハブ部材160よりも基端側に接続されたキャップ部材180を有する。キャップ部材180には、ハブ部材160の内部と連通する開口部が設けられている。術者等は、キャップ部材180の基端側からダイレーターチューブ210を押し込むことにより、ハブ部材160の内部及びシース部材110の内腔115にダイレーターチューブ210を挿通させることができる。
<ダイレーター200>
図1に示すように、ダイレーター200は、ダイレーターチューブ210と、ダイレーターハブ220と、を有する。

ダイレーター200は、ダイレーターチューブ210が医療用具100のハブ部材160の内部及びシース部材110の内腔115を挿通した状態において、ダイレーターハブ220をキャップ部材180に対して着脱可能に固定することができる。
術者等は、ダイレーター200を医療用具100に組付けた状態において、シース部材110の挿入対象となる生体管腔と生体外部とを繋ぐ生体に形成された穿孔にシース部材110を挿入し、穿孔を押し広げる。ダイレーター200は、上記のように穿孔を介してシース部材110を生体管腔に挿入する際に、シース部材110に折れ等が発生することを防止する。
術者等は、上記のようにダイレーター200とともにシース部材110を生体管腔に挿入するのに先立ち、生体に形成された穿孔を介して生体管腔と生体外部に亘って配置されたガイドワイヤをダイレーターチューブ210に挿通させ、ガイドワイヤに沿わせてダイレーター200及びシース部材110を生体管腔に挿入する。術者等は、ダイレーター200の先端部を生体管腔の所望の位置まで挿入した後、ダイレーターチューブ210をシース部材110から抜去する。術者等は、ダイレーターチューブ210が抜去されたシース部材110の内腔115をアクセス経路として使用し、各種の医療デバイスを生体管腔の所望の位置へ送達することができる。
以上のように、本実施形態に係る医療用具100は、管状のシース部材110と、シース部材110の基端部113に接続されたハブ部材160と、を有し、シース部材110は、外面120a及び内面120bを有する内層120と、外面130a及び内面130bを有する外層130と、内層120の外面120aと外層130の内面130bとの間に位置する補強部材140と、を備え、外層130は、内層120の外面120aと固着した第1領域131Aと、内層120の外面120aと固着していない第2領域132と、を有し、第2領域132は、第1領域131Aと隣り合う位置で、内層120の長手方向に沿って螺旋状に配置されており、かつ、内層120の外面120aと外層130の内面130bとの間に隙間部132aを形成しており、補強部材140は、内層120及び外層130と固着されていない状態で、隙間部132aに螺旋状に配置されており、第2領域132は、内層120の長手方向で第1の幅W21を有する自然状態と、内層120の長手方向で第1の幅W21よりも大きい第2の幅W22を有する屈曲状態と、の間で変形可能に構成され、第1領域131Aは第1側面部131aと、隙間部132aを挟んで第1側面部131aと対向する第2側面部131bと、を有し、補強部材140は、屈曲状態のとき、第1側面部131a側に移動して第1領域131Aに力を付与し、第1側面部131aと接触しつつ、第2側面部131bとの間に空間150を形成するように第2領域132を変形させるように構成されている。
上記のように、医療用具100が備えるシース部材110は、内層120と、外層130と、内層120の外面120aと外層130の内面130bとの間に配置された補強部材140と、を備える。シース部材110は、補強部材140を備えることにより、耐キンク性の向上が図られる。また、外層130は、内層120の外面120aと固着した第1領域131Aと、内層120の外面120aと固着していない第2領域132と、を有する。第2領域132は、内層120の長手方向に沿って螺旋状に延在しており、内層120の外面120aと外層130の内面120bとの間に隙間部132aを形成している。補強部材140は、隙間部132aに螺旋状に配置されており、シース部材110が屈曲状態となったときに、第1領域131Aの第1側面部131aと接触しつつ、第1領域131Aの第2側面部131bとの間に空間150を形成する。シース部材110は、屈曲状態となったとき、第1側面部131aと接触した補強部材140によって内層120の長手方向に沿う第2領域132の幅が大きくなるように変形する。そのため、シース部材110は、補強部材140が配置された第2領域132に位置する外層130が屈曲方向に沿って優先的に伸びるように変形し、内層120と外層130が固着された第1領域131の伸びを抑制できる。これにより、シース部材110は、屈曲状態となったとき、内層120と外層130の剥離が軽減されることで、シース部材110がキンクし、シース部材110の内腔115が保持できない状態になることを抑制できる。また、シース部材110は、屈曲状態となったとき、外層130の第2領域132の隙間部132a内に空間150が形成されることによって柔軟性が向上し、生体管腔の湾曲や蛇行した部位等に対する高い追従性を発揮することができる。以上のように、本実施形態によれば、耐キンク性の向上が図られるとともに、生体管腔の湾曲や蛇行した部位に対する高い追従性を備えたシース部材110を有する医療用具100を提供することができる。
また、補強部材140は、自然状態のとき、第1側面部131a及び第2側面部131bと接触するように構成される。
上記のように、シース部材110は、自然状態のとき、補強部材140と第1領域131Aの各側面部131a、131bが接触するように構成されているため、補強部材140と各側面部131a、131bとの間に隙間が形成されない。そのため、シース部材110は、補強部材140と第1領域131Aに位置する外層130との間の密着性を高めることができる。それにより、シース部材110は、自然状態において、シース部材110を直線形状に維持し易くなる。そのため、シース部材110は、シース部材110を生体管腔に挿入する際等に、術者等が付与する押込み力をシース部材110の先端側に伝達する押込み性を確保でき、かつ、シース部材110に折れや撓みが発生することを効果的に防止できる。
また、隙間部132aは、自然状態のとき、内層120の長手方向に沿う断面が矩形形状を有し、補強部材140は、矩形形状の断面を備える金属製の帯状部材で構成されている。
上記のように、シース部材110は、自然状態のとき、隙間部132aが内層120の長手方向に沿う断面が矩形形状で構成されており、補強部材140が矩形形状の断面を備える金属製の帯状部材で構成されているため、第1領域131Aの各側面部131a、131bと補強部材140とが接触する接触面積を大きくすることができる。そのため、シース部材110は、自然状態から屈曲状態へ変形する際、補強部材140から第1領域131Aの第1側面部131aに対して効率良く力を付与することができる。それにより、シース部材110は、屈曲状態において、所望の幅W4を備える空間150を補強部材140と第2側面部131bとの間により確実に形成することができる。
また、内層120の長手方向に沿う空間150の幅W5は、内層120の長手方向に沿う補強部材140の断面の幅W31の半分以下である。
シース部材110は、上記のように内層120の長手方向に沿う空間150の幅W5が内層120の長手方向に沿う補強部材140の断面の幅W31の半分以下であるため、空間150が第2領域132において補強部材140に対して過剰に大きく形成されることを防止できる。これにより、シース部材110は、屈曲状態での剛性を確保し、空間150が形成された屈曲状態においてもシース部材110にキンクが発生することを効果的に防止することができる。
また、自然状態における内層120の長手方向に沿う補強部材140の断面の幅W31は、自然状態における内層120の長手方向に沿う第1領域131Aの幅W11よりも大きい。
シース部材110は、上記のように自然状態における補強部材140の断面の幅W31が自然状態における第1領域131Aの幅W11よりも大きく形成されているため、シース部材110の軸方向の単位長さ当たりにおける補強部材140の配置割合が大きくなる。そのため、シース部材110は、補強部材140による補強効果を高めることができ、耐キンク性がより一層効果的に高められたものとなる。
また、シース部材110の先端部111には隙間部132a及び補強部材140が配置されておらず、シース部材110の先端部111よりも基端側に配置された補強部材140の最先端に位置する一の部分140Cと一の部分140Cと基端側で隣接する他の部分140Dは固定されている。
シース部材110は、上記のように、補強部材140の最先端に位置する一の部分140Cと一の部分140Cと基端側で隣接する他の部分140Dとが固定されている。このため、自然状態における内層120の長手方向に沿う補強部材140の幅(補強部材140の一の部分140Cと他の部分140Dとを一体化した部分の補強部材140の幅)は、各部140C、140Dよりも基端側に位置する補強部材140の幅W31よりも長い。そのため、シース部材110の先端部111付近が屈曲状態となったとき、補強部材140の最先端に位置する一の部分140Cが内層120の長手方向の先端側に向かって移動しにくくなる。これにより、シース部材110の先端部111付近が屈曲状態となったとき、補強部材140全体が内層120の長手方向の先端側に向かって過度に移動することを抑制できる。
(変形例)
前述した実施形態では、医療用具100をシースイントロデューサーに適用した例を説明した。ただし、本発明に係る医療用具の適用対象は、シースイントロデューサーのみに限定されず、例えば、医療分野において公知のカテーテル(例えば、ガイディングカテーテル、マイクロカテーテル、造影カテーテル等)に適用することも可能である。
前述した実施形態では、医療用具100をシースイントロデューサーに適用した例を説明した。ただし、本発明に係る医療用具の適用対象は、シースイントロデューサーのみに限定されず、例えば、医療分野において公知のカテーテル(例えば、ガイディングカテーテル、マイクロカテーテル、造影カテーテル等)に適用することも可能である。
一例として、図6に示す変形例のように、補強部材140及び隙間部132aが配置された内層及び外層を備えるシース部材110と、シース部材110の先端に配置された先端チップ380と、シース部材110の基端側の一部を覆うように配置された耐キンクプロテクタ390と、シース部材110の基端部に配置されたハブ部材360と、を備えるカテーテルとして医療用具300を構成することが可能である。
以上、実施形態及び変形例を通じて本発明に係る医療用具を説明したが、本発明は本明細書で説明した内容のみに限定されるものでなく、特許請求の範囲の記載に基づいて適宜変更することが可能である。
明細書内において説明した各部の構造や部材の配置等は適宜変更することができ、また図示により説明した付加的な部材の使用の省略や、その他の付加的な部材の使用等も適宜に行いうる。

- 管状のシース部材と、
前記シース部材の基端部に接続されたハブ部材と、を有し、
前記シース部材は、
外面及び内面を有する内層と、
外面及び内面を有する外層と、
前記内層の外面と前記外層の内面との間に位置する補強部材と、を備え、
前記外層は、前記内層の外面と固着した第1領域と、前記内層の外面と固着していない第2領域と、を有し、
前記第2領域は、前記第1領域と隣り合う位置で、前記内層の長手方向に沿って螺旋状に配置されており、かつ、前記内層の外面と前記外層の内面との間に隙間部を形成しており、
前記補強部材は、前記内層及び前記外層と固着されていない状態で、前記隙間部に螺旋状に配置されており、
前記第2領域は、前記内層の長手方向で第1の幅を有する自然状態と、前記内層の長手方向で前記第1の幅よりも大きい第2の幅を有する屈曲状態と、の間で変形可能に構成され、
前記第1領域は、第1側面部と、前記隙間部を挟んで前記第1側面部と対向する第2側面部と、を有し、
前記補強部材は、前記屈曲状態のとき、前記第1側面部側に移動して前記第1領域に力を付与し、前記第1側面部と接触しつつ、前記第2側面部との間に空間を形成するように前記第2領域を変形させるように構成される、医療用具。 - 前記補強部材は、前記自然状態のとき、前記第1側面部及び前記第2側面部と接触するように構成される、請求項1に記載の医療用具。
- 前記隙間部は、前記自然状態のとき、前記内層の長手方向に沿う断面が矩形形状を有し、
前記補強部材は、矩形形状の断面を備える金属製の帯状部材である、請求項1に記載の医療用具。 - 前記内層の長手方向に沿う前記空間の幅は、前記自然状態における前記内層の長手方向に沿う前記補強部材の断面の幅の半分以下である、請求項1に記載の医療用具。
- 前記自然状態における前記内層の長手方向に沿う前記補強部材の断面の幅は、前記自然状態における前記内層の長手方向に沿う前記第1領域の幅よりも大きい、請求項1に記載の医療用具。
- 前記シース部材の先端部には前記隙間部及び前記補強部材が配置されておらず、
前記シース部材の先端部よりも基端側に配置された前記補強部材の最先端に位置する一の部分と前記一の部分と基端側で隣接する他の部分は固定されている、請求項1に記載の医療用具。
