Description

医療機器
 本発明は、内視鏡用処置部を備えた医療機器に関する。
 本願は、2024年05月20日に出願された米国仮特許出願第63/649,465号に基づき優先権を主張し、その内容をここに援用する。
 内視鏡的治療において、クリップユニットや鉗子等の処置部が使用されている。処置部は、内視鏡のチャンネルを挿通可能なアプリケータ(導入装置)によって処置位置に導入される。アプリケータは、例えば、処置部に接続されるワイヤと、ワイヤを介して処置部を操作するハンドルと、ワイヤが挿通されるシースと、を備える。
 処置部を用いて治療をするとき、処置部を適宜回転させる必要がある。例えば特許文献1には、シースを回転させることによって、処置具を回転させる構成を有する医療機器が記載されている。処置部を用いて治療をするとき、シースは内視鏡のチャンネルに挿通されているため、シースのうち内視鏡のチャンネルから露出した部分を回転させることによって、処置具を回転させることができる。
米国特許出願公開第2017/0224341号明細書
 しかしながら、特許文献1に記載の医療機器では、シースを回転させることによって処置部を回転させるとき、シースが内視鏡のチャンネルの内壁と接触して摩擦が発生するため、シースの回転が処置部に伝わりづらく、処置部が回転しづらいという問題があった。
 上記事情を踏まえ、本発明は、シースが内視鏡のチャンネルに挿通されているときに、処置部を回転させやすい医療機器を提供することを目的としている。
 上記課題を解決するために、この発明は以下の手段を提案している。
 本発明の第一の態様に係る医療機器は、長手方向に延びるシースと、前記シースの先端側に配された処置部と、前記シースに挿通され、前記処置部が固定されたワイヤと、前記シースの基端側に配され、前記ワイヤが固定されたハンドルと、前記シースの外周側に配され、前記ハンドルから前記先端側に延びる伝達部材と、を備え、前記処置部と前記ワイヤと前記ハンドルと前記伝達部材とは、前記シースに対して、前記長手方向に対する周方向に回転可能である。
 本発明の医療機器は、シースが内視鏡のチャンネルに挿通されているときに、処置部を回転させやすく、処置部を用いた治療を円滑に進めることができる。

 以下、本発明に係る医療機器の実施形態について、図面を参照して説明する。以下の説明では、同一または類似の機能を有する構成に同一の符号を付す。そして、それら構成の重複する説明は省略する場合がある。本実施形態では、医療機器として、クリップデバイスを例に挙げて説明する。
 (第1実施形態)
 [クリップデバイス300]
 本発明の第1実施形態に係るクリップデバイス300について、図1から図26を参照して説明する。
 図1は、第1実施形態に係るクリップデバイス300を示す側面図である。
 クリップデバイス300は、クリップ導入装置(アプリケータ)200と、クリップユニット1と、を備える。クリップユニット1は、クリップ導入装置200に装填される。
 [クリップ導入装置200]
 図2は、クリップ導入装置200を示す斜視図である。
 クリップ導入装置200は、図2に示すように、シース220と、操作ワイヤ230と、操作部(ハンドル)240と、アウターシース(伝達部材)250と、を備える。
 クリップ導入装置200において、シース220の延びる方向を長手方向Kとする。また、長手方向Kのうち、先端側をK1、基端側をK2とする。さらに、長手方向Kに対する周方向を周方向Sとする。
 シース220は、図2に示すように、先端チップ221と、先端側コイル222と、手元側コイル224と、を備え、全体として細長い管状に形成されている。シース220は、長手方向Kに延びる部材である。先端側コイル222は、シース220の先端部側に配置されている。先端チップ221は、先端側コイル222の先端部に配置されている。
 シース220は、後述する内視鏡400の処置具チャンネル(内視鏡チャンネル)430に挿通され、内視鏡と組み合わせて使用される。そのため、シース220は、処置具チャンネル430の長さM6よりも十分に長く形成されている。シース220は、可撓性を有しており、処置具チャンネル430に合わせて湾曲する。
 シース220は、操作ワイヤ230に対して、周方向Sに相対回転可能に設けられている。シース220は、操作部240に対して、周方向Sに相対回転可能に設けられている。シース220は、アウターシース250に対して、周方向Sに相対回転可能に設けられている。シース220は、クリップユニット1に対して、周方向Sに相対回転可能に設けられている。
 操作ワイヤ(動力伝達部)230は、図2に示すように、シース220に挿通される。操作ワイヤ230は、クリップユニット1に接続される矢尻フック(接続部)231と、矢尻フック231を操作するワイヤ232と、を備える。
 矢尻フック231は、図2に示すように、クリップユニット1と係合する略円錐形状の係合部231aと、係合部231aの基端に設けられたワイヤ接続部231bと、を備える。矢尻フック231は、例えばステンレス鋼材等の金属材により形成されている。
 ワイヤ232は、シース220に対して進退自在に挿通されている。ワイヤ232の先端部は、ワイヤ接続部231bの基端に例えば溶接によって固定されている。
 操作ワイヤ230は、シース220に対して、周方向Sに相対回転可能に設けられている。操作ワイヤ230は、操作部240に対して固定されており、操作部240と連動して周方向Sに回転する。操作ワイヤ230は、アウターシース250に対して、周方向Sに相対回転不能に設けられている。操作ワイヤ230は、クリップユニット1と接続され、クリップユニット1と連動して周方向Sに回転する。
 図3は、操作部240を示す側面図であって、一部断面を示す。
 操作部240は、図2に示すように、シース220の基端側K2に配される。操作部240は、図3に示すように、操作部本体(ハンドル本体)241と、スライダ242と、サムリング248と、を備える。操作部本体241と、スライダ242と、サムリング248とは、例えば樹脂材によって射出成型されている。操作部本体241は、スリット部241aと、先端側に回転グリップ241bとを備える。スリット部241aは、スライダ242を進退可能に支持する。
 スライダ242は、操作部本体241に対して長手方向Kに進退可能に取り付けられており、ワイヤ232の基端が取り付けられている。スライダ242は、操作部本体241に支持されている。スライダ242が操作部本体241に沿って進退することで、ワイヤ232がシース220に対して進退し、矢尻フック231が進退する。
 サムリング248は、操作部本体241の基端に、操作部本体241に対して周方向Sに回転可能に取り付けられている。サムリング248は、長手方向Kを回転軸として、操作部本体241に対して周方向Sに回転する。
 操作部本体241の内部には、図3に示すように、抜け止め241cが設けられている。抜け止め241cは、長手方向Kに延びる円筒状に形成されている。抜け止め241cは、操作部本体241に対して周方向Sに回転可能に設けられている。抜け止め241cは、長手方向Kを回転軸として、操作部本体241に対して周方向Sに回転する。抜け止め241cは、長手方向Kに移動可能な範囲が制限されている。抜け止め241cの長手方向Kに移動可能な範囲は、抜け止め241cの長手方向Kの長さより短い。なお、抜け止め241cは、長手方向Kに移動可能でなくてもよい。抜け止め241cの先端には、シース220が固定されている。シース220と抜け止め241cとは、一体に動く。
 操作部240は、アウターシース250に接続される第一部分240aと、第一部分240aに対して周方向Sに相対回転可能に接続される第二部分240bと、を有する。第一部分240aは、操作部本体241とスライダ242を有する。第二部分240bは、サムリング248を有する。
 操作部240は、シース220に対して、周方向Sに相対回転可能に設けられている。操作部240は、操作ワイヤ230に対して固定されており、操作ワイヤ230と連動して周方向Sに回転する。操作部240は、アウターシース250に対して固定されており、アウターシース250と連動して周方向Sに回転する。
 図4は、アウターシース250を示す斜視図であって、アウターシース250の構成を示す。
 アウターシース250は、図1に示すように、シース220の外周側に配される。アウターシース250は、具体的に、シース220が挿通されている。アウターシース250は、図4に示すように、シース220が挿通される円筒状に形成されている。アウターシース250は、図3に示すように、操作部本体241の先端から、長手方向Kの先端側K1に延びている。アウターシース250は、操作部本体241に固定されている。アウターシース250は、操作部240より高い可撓性を有し、シース220に沿って湾曲可能である。アウターシース250の長手方向Kの長さM5は、操作部240の長手方向Kの長さM4より長い。アウターシース250の長手方向Kの長さM5は、シース220の長手方向Kの長さM9より短い。
 アウターシース250は、図4に示すように、内層部251と、外層部252と、内層部251と外層部252との径方向の間に配される中層部253と、を有する。アウターシース250は、例えばブレードインチューブである。内層部251は、樹脂を含んで形成されている。外層部252は、樹脂を含んで形成されている。中層部253は、内層部251より硬度が高い部材である。中層部253は、外層部252より硬度が高い部材である。なお、外層部252は、内層部251および中層部253より硬度が高い部材であってもよい。中層部253は、金属素線を含んで形成されている。
 アウターシース250は、シース220に対して、周方向Sに相対回転可能に設けられている。アウターシース250は、操作ワイヤ230に対して、周方向Sに相対回転不能に設けられている。アウターシース250は、操作部240に対して固定されており、操作部240と連動して周方向Sに回転する。クリップユニット1と操作ワイヤ230と操作部240とアウターシース250とは、シース220に対して、周方向Sに相対回転可能である。
 [クリップユニット1]
 図5は、クリップユニット1を示す斜視図である。
 クリップユニット1は、図1に示すように、シース220より先端側K1に配される。クリップユニット1は、図5に示すように、クリップ2と、押さえ部材3と、連結部材4と、を備える。
 クリップユニット1は、内視鏡400の処置具チャンネル430に挿通されて患部の治療に使用される「処置部」である。
 クリップユニット1とシース220とは、固定されていない。さらに、クリップユニット1は、シース220に対して、周方向Sに相対回転可能である。
 以降の説明において、クリップユニット1の長手方向Aにおけるクリップ2側をクリップユニット1の先端側(遠位側)A1とし、連結部材4側をクリップユニット1の基端側(近位側)A2とする。また、長手方向Aに対して垂直な方向を「第二方向B」または「左右方向B」とする。また、長手方向Aおよび第二方向Bに垂直な方向を「第一方向C」または「上下方向C」とする。
 クリップ(クリップアーム)2は、金属製板材を中央部で折り曲げて形成されている。クリップ2は、図5に示すように、開閉可能な一対のアーム21を有する。クリップ2の基端側A2は、押さえ部材3の内部空間に挿入されている。
 一対のアーム21は、図5に示すように、第一アーム211と第二アーム212とを有する。第一アーム211と第二アーム212とは、クリップユニット1の長手方向Aにおける中心軸線O1を挟んで両側に配置される。なお、クリップ2は3つ以上のアームを有してもよい。
 第一アーム211および第二アーム212には、図5に示すように、先端側A1の端部に組織把持部22が形成されている。組織把持部22は、第一アーム211および第二アーム212の先端を内側に向かって折り曲げられて形成されている。
 押さえ部材(管状部材)3は、クリップ2の少なくとも一部を格納可能な円管状の部材である。押さえ部材3は、クリップ2が長手方向Aに進退する内部空間を有する。押さえ部材3は、内部空間に引き込んだクリップ2を閉じた状態に固定できる。押さえ部材3は、図5に示すように、基端側A2に設けられた押さえ管3Aと、先端側A1に設けられた押さえパイプ3Bと、を有する。
 押さえ管(第二管状部材)3Aは、筒状に形成されている。押さえ管3Aは、クリップ2よりも柔らかい材質、例えば、PPA(ポリフタルアミド)、PA(ポリアミド)、PEEK(ポリエーテルエーテルケトン)、LCP(液晶ポリマー)等の適度な弾性を有する熱可塑性樹脂を射出成形することにより形成されている。なお、押さえ管3Aは、熱可塑性樹脂ではなく、金属によって形成されていてもよい。
 押さえパイプ(第一管状部材)3Bは、金属製の円筒部材である。押さえパイプ3Bは、押さえ管3Aの先端部に圧入されている。押さえ管3Aと押さえパイプ3Bとは、熱溶着、接着またはねじ止めにより連結されていてもよい。
 連結部材4は、クリップ2の基端部に分離可能に連結される。また、連結部材4は、シース220内を挿通する矢尻フック231に分離可能に連結される。すなわち、連結部材4は、クリップ2と矢尻フック231とを連結する。
 [カートリッジ5]
 図6は、カートリッジ5を示す平面図である。
 クリップユニット1と、クリップユニット1を収容可能なカートリッジ5と、を有するシステムをカートリッジシステム100とする。カートリッジシステム100は、クリップ導入装置200にクリップユニット1を容易に装填するための支援システムである。
 カートリッジ5は、図6に示すように、クリップユニット1を格納するケースである。カートリッジ5の幅は40mm程度、長さは70mmから80mm程度、厚さは5mm程度で、手に持ちやすい大きさに形成されている。
 カートリッジ5は、例えば、ABS、PC、PP、PS、アクリル、シクロオレフィンポリマー等、適当な硬さがあり、かつ透明な樹脂材により射出形成されて製造されている。透明な樹脂材を用いてカートリッジ5が形成されており、使用者はクリップユニット1が内部に存在しているか否かを判断しやすい。
 図6に示すように、カートリッジ5の長手方向(挿通方向)Lに対して垂直であって互いに垂直な二方向のうち、一方を「幅方向W」、他方を「高さ方向H」とする。また、長手方向Lおよび幅方向Wに水平な面を、「水平面HP」とする。長手方向Lおよび高さ方向Hに水平な面を「垂直面VP」とする。また、クリップユニット1を格納したカートリッジ5において、一対のアーム21側をカートリッジ5の先端側L1とし、連結部材4側をカートリッジ5の基端側L2とする。
 カートリッジ5は、図6に示すように、略矩形箱状に形成されたカートリッジ本体(ケース本体部)70を有する。カートリッジ本体70には、挿入口(開口)67が形成される。カートリッジ本体70の内部には、処置具格納領域7Sが形成されている。
 カートリッジ本体70には、クリップユニット1が長手方向(進退方向)Lにおいて移動可能に格納される処置具格納領域7Sが形成されている。処置具格納領域7Sは、図6に示すように、第一領域71と、シース挿入領域74と、を含む。第一領域71と、シース挿入領域74とは、カートリッジ5の長手方向Lにおいて先端側L1から基端側L2に向かって配列している。
 カートリッジ本体70には、シース220を挿入可能な挿入口(開口)67が形成されている。挿入口67は、開口面が長手方向Lに対して略垂直に形成されている。挿入口67は、カートリッジ本体70の基端側L2の側面に形成されている。挿入口67は、シース挿入領域74と連通している。
 第一領域71は、クリップユニット1が長手方向Lにおいて移動可能に格納される内部空間である。第一領域71は、シース挿入領域74と連通している。
 シース挿入領域74は、挿入口67を通過したシース220の先端部が挿入される領域である。シース挿入領域74は、第一領域71の基端側L2に位置して、第一領域71と連通している。
 [クリップユニット1の装填方法]
 次に、図7から図9を参照して、クリップユニット1の装填方法について説明する。
 図7から図9は、カートリッジ5を用いて、クリップユニット1をクリップ導入装置200に装填する方法を説明する図である。
 図7は、クリップ導入装置200に接続されるクリップユニット1を示す図である。
 使用者は、シース220をカートリッジ5の先端側に前進させる。このとき、使用者は、例えば、一方の手でカートリッジ5を持ちながら、他方の手でシース220を動かす。使用者は、シース220をさらに前進させることにより、矢尻フック231と連結部材4を接触させる。
 図8は、クリップ導入装置200に接続されたクリップユニット1を示す図である。
 使用者は、シース220をさらに前進させることにより、矢尻フック231の係合部231aを連結部材4と接続させる。
 図9は、クリップユニット1がカートリッジ5から引き抜かれる様子を示す図である。
 使用者は、シース220をカートリッジ5の基端側L2に後退させる。このとき、使用者は、例えば、一方の手でカートリッジ5を持ちながら、他方の手でシース220を動かす。シース220は、クリップユニット1を基端側L2に牽引する。使用者は、シース220をカートリッジ5の基端側L2に後退させることで、クリップ導入装置200に装填されたクリップユニット1をカートリッジ5から引き抜くことができる。
 カートリッジ5から引き抜かれたクリップユニット1は、後述する内視鏡400の処置具チャンネル430に挿通して使用することができる。
 [内視鏡システム500]
 ここで、図10および図11を参照して、内視鏡システム500について説明する。
 図10は、内視鏡400の処置具チャンネル430にクリップデバイス300のシース220が挿通された状態を示す。図11は、内視鏡システム500を使用している様子を示す斜視図である。
 クリップデバイス300と、内視鏡400と、を有するシステムを内視鏡システム500とする。内視鏡システム500は、内視鏡的治療のときに使用されるシステムである。
 内視鏡400は、公知の直視型の軟性内視鏡であり、長尺の挿入部410と、内視鏡操作部420と、処置具チャンネル430と、を備えている。内視鏡操作部420は挿入部410の近位端部に設けられている。以下の説明において、内視鏡400の内視鏡操作部420側を近位側と称する。挿入部410の長手軸方向における内視鏡操作部420とは反対側を内視鏡400の遠位側と称する。処置具チャンネル430は、クリップデバイス300などの処置具が挿通される。処置具チャンネル430は、クリップユニット1等の処置部を挿通可能な部材である。内視鏡400は側視型の軟性内視鏡であってもよい。
 内視鏡操作部420よりも遠位側には、処置具チャンネル430に連通する鉗子口440が設けられている。使用者は、鉗子口440からクリップデバイス300等の内視鏡用処置具を挿入することができる。鉗子口440には体液の漏れを防ぐために鉗子栓450が取り付けられる。
 ここで、内視鏡400と比較したクリップデバイス300の構成について説明する。
 操作部240は、処置具チャンネル430に挿通不能に形成されている。具体的には、操作部240の外径M1は、処置具チャンネル430の内径M3より大きい。アウターシース250は、処置具チャンネル430に挿通可能に形成されている。具体的には、アウターシース250の外径M2は、処置具チャンネル430の内径M3より小さい。
 クリップデバイス300の長手方向Kにおけるクリップユニット1の先端からアウターシース250の先端までの長さM7は、処置具チャンネル430の長さM6より短い。具体的には、シース220が処置具チャンネル430に挿通され、クリップユニット1が処置具チャンネル430の先端から突出した状態において、アウターシース250の先端は処置具チャンネル430に収容される。
 なお、図12および図13に示すように、クリップユニット1が処置具チャンネル430の先端から突出した状態において、アウターシース250の先端は処置具チャンネル430に収容されていなくてもよい。具体的に、クリップユニット1が処置具チャンネル430の先端から突出した状態において、アウターシース250の先端と鉗子口440との間の長さM8は、30cm以下であってもよい。その場合、使用者は、アウターシース250の一点を掴んだまま、鉗子口440付近で、回転と進退とを両立することができ、使用者は直感的に操作することができる。
 [クリップデバイス300の作用]
 次に、図10および図11を参照して、クリップデバイス300の作用について説明する。
 クリップデバイス300は、図10に示すように、シース220が処置具チャンネル430に挿通された状態で使用される。使用者は、クリップユニット1が処置具チャンネル430から突出するまでシース220を挿通する。このとき、アウターシース250の先端が処置具チャンネル430に収容される。
 使用者は、図11に示すように、一方の手を用いて内視鏡操作部420を操作し、他方の手を用いてアウターシース250を操作する。内視鏡システム500を使用するときは、内視鏡400を操作する使用者とは別に、クリップデバイス300の操作部240を操作する介助者が必要となる。
 使用者は、クリップユニット1を適宜回転させる必要がある。使用者は、アウターシース250を周方向Sに回転させることによって、クリップユニット1を周方向Sに回転させることができる。アウターシース250が周方向Sに回転すると、操作部本体241、操作ワイヤ230の順に回転が伝わり、クリップユニット1が周方向Sに回転する。すなわち、アウターシース250を回転させる力は、操作部240および操作ワイヤ230を介して処置部に伝達可能であり、アウターシース250自体は、アウターシース250が回転する力を操作部240に伝達可能な剛性を有する。クリップユニット1を周方向Sに回転させることによって、使用者は、クリップユニット1を適切な向きで操作することができる。
 本実施形態に係るクリップデバイス300によれば、アウターシース250を回転させることによってクリップユニット1を回転させることができるため、使用者は、内視鏡400を操作しながらクリップユニット1を周方向Sに回転させることができる。具体的には、使用者は、一方の手を用いて内視鏡操作部420を操作しながら、他方の手を用いてアウターシース250を操作してクリップユニット1を周方向Sに回転させることができる。そのため、クリップデバイス300は使い勝手がよい。
 本実施形態に係るクリップデバイス300によれば、アウターシース250の回転をクリップユニット1に伝える部材である操作ワイヤ230が、処置具チャンネル430から摩擦を受けないため、回転が伝わりやすい。具体的には、操作ワイヤ230はシース220に挿通されているため、処置具チャンネル430から摩擦を受けることなく回転することができる。そのため、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。
 本実施形態に係るクリップデバイス300によれば、アウターシース250はシース220に対して相対回転可能であるため、アウターシース250を周方向Sに回転させるとき、シース220は周方向Sに回転せず、シース220と処置具チャンネル430との摩擦の影響を受けない。そのため、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。
 本実施形態に係るクリップデバイス300によれば、アウターシース250はシース220が挿通される筒状に形成されているため、使用者は、アウターシース250を周方向Sに回転させやすい。そのため、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。
 本実施形態に係るクリップデバイス300によれば、アウターシース250は、操作部240より高い可撓性を有し、シース220に沿って湾曲可能であるため、操作部240から離れた位置においてクリップユニット1の回転操作を行うことが可能でありながら、シース220の形状を変形させることができる。そのため、クリップデバイス300は使い勝手がよい。
 本実施形態に係るクリップデバイス300によれば、アウターシース250の外径M2は、処置具チャンネル430の内径M3より小さいため、アウターシース250を処置具チャンネル430に挿通した状態で使用することができる。そのため、クリップデバイス300を操作しているときにアウターシース250の先端が鉗子口440に接触する等、操作の妨げとなる原因を抑制でき、クリップデバイス300は使い勝手がよい。
 本実施形態に係るクリップデバイス300によれば、長手方向Kにおけるアウターシース250の長さM5は、操作部240の長さM4より長いため、使用者は、操作部240から離れた位置においてクリップユニット1の操作を行うことができる。そのため、クリップデバイス300は使い勝手がよい。
 本実施形態に係るクリップデバイス300によれば、長手方向Kにおけるアウターシース250の長さM5は、シース220の長さM9より短いため、シース220を処置具チャンネル430に挿通したとき、アウターシース250が処置具チャンネル430に挿通される長さが短い。そのため、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。
 本実施形態に係るクリップデバイス300によれば、長手方向Kにおけるクリップユニット1の先端からアウターシース250の先端までの長さM7は、処置具チャンネル430の長さM6より短いため、アウターシース250を処置具チャンネル430に挿通した状態で使用することができる。処置具チャンネル430の先端とクリップユニット1の先端とが揃っているときも、アウターシース250の先端は処置具チャンネル430に挿通される。そのため、クリップデバイス300を操作しているときにアウターシース250の先端が鉗子口440に接触する等、操作の妨げとなる原因を抑制でき、クリップデバイス300は使い勝手がよい。
 本実施形態に係るクリップデバイス300によれば、シース220が処置具チャンネル430に挿通され、クリップユニット1が処置具チャンネル430の先端から突出した状態において、アウターシース250の先端は処置具チャンネル430に収容されるため、アウターシース250を処置具チャンネル430に挿通した状態で使用することができる。そのため、クリップデバイス300を操作しているときにアウターシース250の先端が鉗子口440に接触する等、操作の妨げとなる原因を抑制でき、クリップデバイス300は使い勝手がよい。
 本実施形態に係るクリップデバイス300によれば、操作部240は、アウターシース250と連動して回転可能な第一部分240a、と第一部分240aと相対回転可能に接続された第二部分240bと、を有するため、使用者がアウターシース250を周方向Sに回転させて第一部分240aが周方向Sに回転したとき、操作部240を操作している介助者は、第二部分240bを支持することで、第一部分240aの回転を妨げない。そのため、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。
 本実施形態に係るクリップデバイス300によれば、スライダ242は第一部分240aに配されるため、操作部240を操作する介助者は、スライダ242を長手方向Kに進退させやすく、クリップデバイス300は使い勝手がよい。
 本実施形態に係るクリップデバイス300によれば、アウターシース250は、内層部251と、外層部252と、内層部251と外層部252との径方向の間に配され内層部251および外層部252より硬度が高い中層部253と、を有するため、アウターシース250は、先端部にされた回転を操作部240に伝達することができる。そのため、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。
 本実施形態に係るクリップデバイス300によれば、内層部251は樹脂を含んで形成されており、外層部252は樹脂を含んで形成されており、中層部253は金属素線を含んで形成されているため、アウターシース250は、適度な硬度を有しており、先端部にされた回転を操作部240に伝達することができる。そのため、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。

 次に、図14および図15を参照して、変形例1に係る操作部240Bについて説明する。
 図14は、変形例1に係る操作部240Bの断面図であって、長手方向Kに沿った断面を示す。
 操作部240Bは、操作部240の変形例である。操作部240Bは、ストッパー249を有する点が操作部240と異なる。
 ストッパー249は、第一部分240aと第二部分240bとの相対回転を規制する部材である。操作部240Bにおいて、ストッパー249は、操作部本体241とサムリング248との相対回転を規制する部材である。ストッパー249は、例えば、Оリングである。ストッパー249は、図14に示すように、操作部本体241とサムリング248との接続部分に設けられている。
 図15は、図14中に示された操作部240Bの破線D13で示す範囲の拡大図である。
 操作部本体241の基端部241tには、図15に示すように、径方向に埋没する溝部241dが形成されている。溝部241dは、操作部本体241の基端部241tの外周に形成されている。ストッパー249は、リング状に形成されており、溝部241dに嵌め込まれている。サムリング248の先端部248sは、円筒状に形成されており、ストッパー249の外周を覆うように取り付けられている。ストッパー249は、操作部本体241の基端部241tの外周面と、サムリング248の先端部248sの内周面と、によって径方向から圧迫されている。
 ストッパー249は、第一部分240aとの間に摩擦を発生させる摩擦部材である。操作部240Bにおいて、ストッパー249は、操作部本体241との間に摩擦を発生される摩擦部材である。ストッパー249は、第二部分240bとの間に摩擦を発生させる摩擦部材である。操作部240Bにおいて、ストッパー249は、サムリング248との間に摩擦を発生させる摩擦部材である。ストッパー249は、ゴムを含んで形成されている。
 ストッパー249と操作部本体241との間の摩擦は、アウターシース250の回転伝達性に応じて決定される。具体的には、ストッパー249と操作部本体241との間の摩擦力>{アウターシース250の回転角度×(1-アウターシース250の回転伝達率)}×アウターシース250を1度回転させるために必要なトルク(N・mm/deg)となっている。例えば、アウターシース250の回転操作角度が90°、アウターシース250の回転伝達率が33%、アウターシース250を回転させるために必要なトルクが10N・mm/degであるとき、アウターシース250には60°の捻じれが溜まるため、ストッパー249の摩擦力は、例えば、600N・mmより大きい。ストッパー249と操作部本体241との間、およびストッパー249とサムリング248との間、の摩擦は、10mN・mより大きいことが好ましい。なお、ストッパー249と操作部本体241との間、およびストッパー249とサムリング248との間、の摩擦は、5mN・m以上であってもよいし、10mN・m以下であってもよい。
 アウターシース250が処置具チャンネル430に挿通された状態において、アウターシース250が周方向Sに回転をするとき、アウターシース250と鉗子栓450との間に摩擦が発生する。そのため、アウターシース250と鉗子栓450との間の摩擦力より小さい力で、使用者がアウターシース250を周方向Sに回転した場合、アウターシース250と鉗子栓450とは周方向Sに相対回転しない。アウターシース250と鉗子栓450とが周方向Sに相対回転しない場合でも、アウターシース250が捻じれることによって、操作部240および操作ワイヤ230を介してクリップユニット1は回転する。しかしながら、アウターシース250の捻じれがほどけると、クリップユニット1は元の状態に戻ってしまう。そのため、使用者は、アウターシース250と鉗子栓450との間に摩擦力より大きい力でアウターシース250を周方向Sに回転させる必要がある。
 操作部240Bがストッパー249を有することによって、使用者は、ストッパー249によって規制される大きさ以上の力でアウターシース250を周方向Sに回転させるため、アウターシース250の捻じれによってクリップユニット1が回転することを抑制することできる。そのため、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。
 操作部本体241とストッパー249との間に摩擦が発生し、サムリング248とストッパー249との間に摩擦が発生することによって、使用者は、操作部本体241とストッパー249との間の摩擦力およびサムリング248とストッパー249との間の摩擦力以上の力でアウターシース250を周方向Sに回転させるため、アウターシース250の捻じれによってクリップユニット1が回転することを抑制することできる。そのため、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。
 操作部本体241および処置具チャンネル430に対して、周方向Sに所定量より大きい力を加えたとき、操作部本体241と処置具チャンネル430とは周方向Sに相対回転をするため、使用者は、操作部本体241とストッパー249との間の摩擦力およびサムリング248とストッパー249との間の摩擦力以上の力でアウターシース250を周方向Sに回転させる。そのため、アウターシース250の捻じれによってクリップユニット1が回転することを抑制することでき、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。
 変形例1に示したストッパー249は、リング状に形成されているが、ストッパー249の形状は限定されない。ストッパー249は、操作部本体241とサムリング248との相対回転を規制可能に構成されていればよい。

 操作部240Cによれば、アウターシース250が周方向Sに回転したとき、回転操作部243が連動して周方向Sに回転し、操作部本体241およびスライダ242は連動して回転しない。そのため、操作部240Cを操作する介助者は、スライダ242の位置を維持しやすい。操作部240Cを備えたクリップデバイス300は使い勝手がよい。

 アウターシース250Eの構成は、限定されない。アウターシース250Eは、図25に示すように、径方向の大きさが異なる複数の筒状部材が長手方向Kに連なる構成であってもよい。

 本実施形態に係るクリップデバイス300では、アウターシース250が設けられているが、操作部240に固定されており、周方向Sの回転を操作部240に伝達可能な伝達部材が設けられていればよい。
 本実施形態に係るクリップデバイス300では、アウターシース250は、内層部251と外層部252と中層部253とを有するが、アウターシース250の構成は限定されない。アウターシース250は、例えば、単層チューブであってもよい。アウターシース250は、例えば、チュービングコイルであってもよい。アウターシース250は、周方向Sの回転を操作部240に伝達可能な構成であればよい。
 本実施形態に係るクリップデバイス300では、クリップユニット1はカートリッジ5を用いてクリップユニット1に装填される構成であったが、クリップユニット1の構成は限定されない。クリップユニット1は、クリップ導入装置200に予め取り付けられていてもよい。また、クリップ導入装置200には、カートリッジ5を用いて、新しいクリップユニット1が再装填(リロード)されてもよい。
 本実施形態では、処置部はクリップユニット1であったが、処置部はこれに限定されない。処置部は、クリップ導入装置200に対して取り付けられ、内視鏡的治療に使用される部材であればよく、例えば鉗子やナイフであってもよい。
 本実施形態では、医療機器として、クリップデバイス300を例に挙げたが、鉗子やナイフ等の処置部がアプリケータに接続される医療機器において、アプリケータとしてクリップ導入装置200を使用することができる。

 図27は、第2実施形態に係るクリップデバイス300Fを示す側面図である。
 クリップデバイス300Fは、クリップ導入装置200Fと、クリップユニット1と、を備える。クリップユニット1は、クリップ導入装置200Fに装填される。
 クリップユニット1とシース220とは、固定されていない。さらに、クリップユニット1は、シース220に対して、周方向Sに相対回転可能である。

 クリップデバイス300Fは、図28に示すように、シース220が処置具チャンネル430に挿通された状態で使用される。使用者は、クリップユニット1が処置具チャンネル430から突出するまでシース220を挿通する。
 使用者は、一方の手を用いて内視鏡操作部420を操作し、他方の手を用いてシース220を操作する。内視鏡400を操作する使用者とは別に、クリップデバイス300Fの操作部240を操作する介助者が必要となる。
 使用者は、クリップユニット1を適宜回転させる必要がある。使用者は、シース220を周方向Sに回転させることによって、クリップユニット1を回転させることができる。シース220が周方向Sに回転すると、操作部本体241、操作ワイヤ230の順に回転が伝わり、クリップユニット1が回転する。クリップユニット1を回転させることによって、使用者は、クリップユニット1を適切な向きで操作することができる。
 本実施形態に係るクリップデバイス300Fによれば、シース220を周方向Sに回転させることによってクリップユニット1を回転させることができるため、使用者は、内視鏡400を操作しながらクリップユニット1を回転させることができる。具体的には、使用者は、一方の手を用いて内視鏡操作部420を操作しながら、他方の手を用いてアウターシース250を操作してクリップユニット1を回転させることができる。そのため、クリップデバイス300Fは使い勝手がよい。
 本実施形態に係るクリップデバイス300Fによれば、シース220の周方向Sの回転をクリップユニット1に伝える部材である操作ワイヤ230が、処置具チャンネル430から摩擦を受けないため、回転が伝わりやすい。具体的には、操作ワイヤ230はシース220に挿通されているため、処置具チャンネル430から摩擦を受けることなく周方向Sに回転することができる。シース220が処置具チャンネル430に挿通されているが、図28に示すように、周方向Sの回転を操作部240に伝達させる部分は処置具チャンネル430から露出するため、シース220の周方向Sの回転が操作部240に伝達されるときは、シース220と処置具チャンネル430との摩擦の影響を受けづらい。そのため、シース220が処置具チャンネル430に挿通されているときに、クリップユニット1を回転させやすく、クリップユニット1を用いた治療を円滑に進めることができる。

 次に、図29を参照して、変形例5に係るクリップデバイス300Gについて説明する。
 図29は、変形例5に係るクリップデバイス300Gを示す側面図である。
 クリップデバイス300Gは、クリップデバイス300Fの変形例である。クリップデバイス300Gは、グリップ200gを有する点がクリップデバイス300Fと異なる。
 グリップ200gは、使用者がシース220を周方向Sに回転させるときに、使用者によって把持される部材である。グリップ200gは、シース220に設けられている。グリップ200gは、シース220の外周に接続される。グリップ200gは、シース220上を長手方向Kに摺動可能な部材である。グリップ200gの外径は、シース220の外径より大きい。グリップ200gは、握りやすい形状を有している。グリップ200gは、握りやすい外径を有している。
 グリップ200gは、例えば、内径側にスリットが形成されており、使用者が掴むことによって潰れ、グリップ200gを介してシース220を把持可能な部材である。グリップ200gは、例えば、円筒状に形成されたゴム製の部材である。使用者は、把持しやすい位置にグリップ200gを移動させ、把持することができる。
 グリップ200gが設けられていることによって、使用者は、シース220を周方向Sに回転させやすい。そのため、クリップデバイス300Fは使い勝手がよい。
 以上、本発明の第1実施形態について図面を参照して詳述したが、具体的な構成はこの実施形態に限られるものではなく、本発明の要旨を逸脱しない範囲の設計変更等も含まれる。また、上述の実施形態および変形例において示した構成要素は適宜に組み合わせて構成することが可能である。

  1.  長手方向に延びるシースと、
     前記シースの先端側に配された処置部と、
     前記シースに挿通され、前記処置部が固定されたワイヤと、
     前記シースの基端側に配され、前記ワイヤが固定されたハンドルと、
     前記シースの外周側に配され、前記ハンドルから前記先端側に延びる伝達部材と、
     を備え、
     前記処置部と前記ワイヤと前記ハンドルと前記伝達部材とは、前記シースに対して、前記長手方向に対する周方向に回転可能である、
     医療機器。
  2.  前記伝達部材は、前記シースが挿通される筒状に形成されている、
     請求項1に記載の医療機器。
  3.  前記ハンドルは、
      ハンドル本体と、
      前記ハンドル本体に支持され、前記長手方向に進退可能なスライダと、
     を有し、
     前記ワイヤは、前記スライダに接続され、前記スライダと連動して前記長手方向に進退可能であって、
     前記伝達部材は、前記ハンドルより高い可撓性を有し、前記シースに沿って湾曲可能である、

  4.  前記ハンドルの外径は、前記処置部を挿通可能な内視鏡チャンネルの内径より大きく、
     前記伝達部材の外径は、前記内視鏡チャンネルの内径より小さい、
