Abstract
本发明涉及医疗器械领域,公开了一种双管插管固定装置。解决了现有采用胶布粘接、佩戴式和耳挂式等方案对经鼻腔插管灌注营养液用于肠道营养的管道固定效果不佳的问题。固定装置包括贴合部件以及设置在贴合部件上的固定架,贴合部件的两端均连接有用于将贴合部件固定于鼻孔下方人中处的连接部,固定架包括至少两个压合部,两个压合部的一侧相互连接,另一侧相互可拆卸连接,两个压合部之间设置有用于放置管道的容纳部。通过连接部将带有固定架的贴合部件定位于鼻孔下方人中处,并通过固定架的两个压合部相对运动对管道夹持固定,使管道在鼻孔处保持定位稳定、不松动,有效避免了因患者体位移动发生的管道移位或滑脱,更不会产生胶带过敏的担忧。
Description
本发明涉及医疗器械领域,具体而言,涉及一种双管插管固定装置。
高分子材料制作的鼻饲营养导管(亦称鼻肠管)是一种由鼻腔插入,经咽部、食道、胃,置入十二指肠或空肠,用于肠内营养输注对患者营养支持起着重要作用的医疗器械。利用鼻肠管经鼻腔灌注营养液已经成为一种广泛使用的临床护理操作技术,是对昏迷患者、植物人患者,或因消化道疾病如食道癌,食管狭窄、喉癌,下咽癌等不能经口进食患者,确保其在治疗期间摄入足够营养维持身体健康所必须的重要医疗技术手段。
按照《T/CNAS20─2021成人鼻肠管的留置与维护》和护理技术操作规范《中国卒中肠内营养护理指南》(2021版),临床使用鼻肠管采用“高举平台法”固定。“高举平台法”是用医用胶带将经鼻腔插入的管道交叉后,直接粘贴在患者鼻翼两侧及面颊部进行管道固定,确保插入鼻腔后的鼻肠管在体内精确定位并能正常工作。胶带上所附粘贴物质的质量、织物的组织结构和经纬密度是控制胶带产品质量的重要参数,直接影响鼻肠管“高举平台法”的使用效果;患者头面部皮肤的清洁度、操作人员的操作手法更是影响鼻肠管固定与定位的重要因素。由于管道是采用胶带直接粘贴在患者皮肤表面,或因患者皮肤过敏,或因汗液影响,或因体位移动,或因胶带粘接力不强,均会造成胶带不能紧贴皮肤,容易翘起松脱,发生管道
固定移位,引起感染和损伤,影响治疗效果,甚至导致管道滑脱成为具有医疗风险的严重不良事件。
鼻肠管固定是护理技术操作培训与肠道营养治疗的关注重点和难点。为了避免患者皮肤过敏和感染的不良事件发生,临床护理对鼻肠管的置管、拔管、并发症预防与处置均有严格规定,要求:每24~48h进行更换粘贴胶布及其固定位置,如有潮湿、松动或移位,均应及时调整或更换导管。这些频繁的日常操作增加了护理劳动强度,加重了护理管理的工作负担,更影响了患者的治疗体验。
传统采用“高举平台法”的鼻肠管固定方式需要对胶带进行裁剪,反复粘贴,操作复杂、耗费时间,固定效果依赖操作人员手感和经验,鼻肠管定时更换时需要撕下面颊部的固定胶带,患者疼痛感强烈,影响治疗体验,长期用胶带固定鼻肠管还会导致皮肤过敏。鉴于不同厂家胶带产品质量的差异性,因患者面部尤其是鼻翼分泌物的影响,长时间的胶带粘贴以及临床人员的操作经验等原因,临床中固定鼻肠管的常见问题主要有:固定费时、操作复杂、管道移位、管道脱落、异物堵塞、流通不畅、末端开裂、拔出困难等等,尤其以管道的移位和脱落最为常见,或给患者造成二次伤害,或出现非医疗拔管,严重影响治疗的有效性,成为医疗安全隐患和发生感染等不良事件的主要因素。定时鼻饲操作时,鼻肠管末端需要抬起灌注,贴在面颊部的胶带需要反复撕开和粘贴,容易造成患者皮肤不适或损伤,上述诸多临床痛点成为长期困惑临床而又一直未能很好解决的问题。
在专利CN211536105U中公开了一种经鼻气管插管固定器,软垫上设置鼻内插进管对气管进行夹持固定,并在软垫的两侧连接带有耳带的弹性带,通过耳带挂在患者的耳部,实现对固定器的固定。由于弹性带的弹性作用,使耳带对患者的耳部产生较大的挤压力,长期的佩戴会对耳部造成伤害。
在专利CN205964508U中公开了一种镜架式鼻饲管固定装置,管道穿过安附在眼镜架上的固定管,通过旋紧固定管侧壁安装的螺丝防止管道沿固定管轴向移动,进行固定定位。该技术方案中,摆脱了用胶带固定管道的依赖以及上述专利中的弹性带加耳带的固定方式,减轻了患者佩戴固定装置的不适感,但该专利需要借助眼镜架作为固定管的支撑,患者佩戴眼镜在耳廓进行固定,未能考虑到至关重要的管道在鼻孔处的固定和定位,当患者感觉不适发生体位移动时,极容易造成管道移位。采用佩戴式眼镜架固定,不能确保鼻肠管定位的稳定性。
在专利CN215135581U中公开了一种经鼻腔插管固定带,该技术方案中设置一个可以转动的活动块配合一个固定不动的固定块在鼻孔处对管道进行夹持固定,其中活动块与固定块之间的固定方式采用一个卡块卡入限位槽中,不能适用管径不同的管道的固定,限制了固定装置的使用范围,给临床操作带来不便。
在专利CN215308505U中公开了一种鼻饲管固定装置,利用约束带下压在凸块上,对放置在凸块上的管道进行固定,其中约束带采用挂钩钩住挂栏实现固定,并且挂栏设置有两个,使约束带可以调节下压力,从而对不同管径的管道进行固定。该技术方案中在第一支撑片的两侧设置连接带对固定装置进行固定,位于鼻孔外的管道不能沿着连接带向面颊部进行布置,患者体位改变时,未加固定的管道部分容易引起移位、滑脱,甚至加深对患者鼻腔的机械刺激或二次伤害,存在一定的安全隐患。
在专利202121455621.0中提出一种耳挂式鼻肠管固定器,通过挂于患者耳朵的松紧带固定管道并在座体设置荧光片,解决了管道在鼻孔处固定及夜间观察是否移位的问题,但是长期使用时,在松紧带张力作用下,患者耳廓的不适感加剧,甚至产生面压伤,引起烦躁,以至抗拒使用。该技
术方案中,管道是通过支撑片上设置的通孔进入鼻腔,增加了操作的复杂性,尽管管道在鼻孔处得到固定,但余下较长的管道部分仍不能有效固定,当患者体位移动,未固定的管道部分产生摆动将增加支撑片对患者鼻孔的摩擦,影响患者体验,与此同时紧贴患者鼻孔设置的支撑片阻挡了空气的流入,在一定程度上也影响患者顺畅的呼吸。这种耳挂式固定器,不仅操作繁复,也不能满足临床患者的治疗体验和依从性要求。
在专利CN217162661U中公开了一种鼻罩式预防鼻饲管脱落的固定器,在弧形鼻罩上设置有收纳半环的结构,用于收纳位于鼻孔外的管道,管道容易从收纳半环中脱出,不能很好的实现“高举平台法”的固定效果,达到治疗要求。
在专利CN216877228U中公开了一种鼻胃管固定装置及鼻胃管,通过定位管上的夹合部对管道进行夹持固定,但是不同管径的管道在固定管径的定位管的轴向方向上所受夹持力不尽相同,管道会随患者体位的改变产生位移,导致管道从鼻孔中滑脱、移位,甚至对患者造成二次伤害,存有一定的安全风险。
发明内容
针对上述采用胶布粘接、佩戴式和耳挂式等方案对经鼻腔插管灌注营养液用于肠道营养的管道固定效果不佳的问题,本发明的目的在于提供一种双管插管固定装置,通过连接部将带有固定架的贴合部件固定于鼻孔下方人中处,并通过固定架的两个压合部相对运动对插管进行夹持固定,使插入鼻腔的管道在鼻孔处保持定位稳、不松动,当患者体位移动时,能有效避免管道产生移位或滑脱。
本发明的实施例通过以下技术方案实现:
一种双管插管固定装置,包括贴合部件以及设置在贴合部件上的固定架,所述贴合部件的两端均连接有用于将贴合部件固定于鼻孔下方人中处的连接部,所述固定架包括至少两个压合部,两个所述压合部的一侧相互连接,两个压合部的另一侧相互可拆卸连接,两个所述压合部之间设置有用于放置插管管道的容纳部。
优选的,其中一个所述压合部配置有卡齿,另一个所述压合部配置有与卡齿配合可拆卸连接的卡块。
优选的,所述卡齿设置有数个。
优选的,两个所述压合部远离可拆卸连接处的一侧为铰接或者弹性连接。
优选的,其中一个所述压合部的内侧设置有压柱,另一个所述压合部的内侧设置有至少两组限位柱,所述压柱和限位柱均突出于容纳部,所述压柱位于两组限位柱之间。
优选的,所述连接部包括与贴合部件连接的条形板,所述条形板上连接有绑带。
优选的,所述条形板中设置有弯折部,且弯折部延伸至耳廓处。
优选的,所述条形板上连接有至少一个限位件,所述限位件中设置有供插管管道穿过的通道。
优选的,所述限位件的一侧设置有开口。
优选的,所述限位件设置有至少两个,且限位件的开口分别位于条形
板的两侧。
优选的,其中一个所述压合部与贴合部件可拆卸连接,所述条形板与贴合部件可拆卸连接。
优选的,所述贴合部件上设置有数个连接座,所述连接座上开设有通孔,与贴合部件连接的压合部的两端穿过通孔后设置有第二突出部,所述条形板与贴合部件连接的一端穿过通孔后设置有第一突出部。
优选的,所述容纳部朝向鼻孔的方向倾斜设置。
优选的,所述贴合部件为硅胶材质制成。
本发明实施例的技术方案至少具有如下优点和有益效果:
1、通过连接部将带有固定架的贴合部件固定于鼻孔下方的人中处,利用固定架的两个压合部相对运动对经鼻腔插入的管道进行夹持固定,管道安装简便快捷,定位准确、稳定,有效避免了管道在患者体位发生改变时移位或滑脱。
2、设置卡齿和卡块,使两个压合部相互贴合后被卡齿和卡块固定,结构简单,操作方便。
3、通过设置数个卡齿,使卡块可以卡接固定在不同的卡齿处,从而调节两个压合部的贴合程度,使得固定装置可以适合不同管径、尤其是小管径的管道进行固定,并能对管道进一步施加更大静压力,增加固定效果。
4、铰接或者弹性连接的两个压合部均可以实现两个压合部绕一端进行转动,从而对管道进行下压固定,而其中弹性连接的方式可以实现其中一个压合部自动弹开,从而方便管道取出。
5、设置两组限位柱对管道的位置进行限制,使管道不能左右的摆动,再通过压柱对管道进行下压固定,进一步增加管道的固定效果。
6、硬质的条形板在使用时分别位于脸颊部的两侧,以减少贴合部件的摆动,并通过绑带将固定装置整体固定在患者头部,固定装置可以随着头部的摆动或体位的移动而同步移动,进一步确保了固定装置能够稳定贴合在鼻孔下方的人中处不发生位移。
7、条形板的弯折部将绑带的固定位置引导至耳廓上方,使绑带在捆绑固定后可以由耳廓进行限位,避免绑带的下移导致固定装置产生位移。
8、通过设置限位件使余下较长的管道部分可以穿过限位件向脸颊部两侧进行布置,避免管道未固定的部分在患者体位改变时与患者的鼻孔或者面部产生摩擦,使患者产生不适感,增强了治疗的依从性。
9、限位件设置有开口,插管的管道可以从限位件的侧边按压进入到限位件的通道内,使管道布置更加轻松省力、节约时间,符合护理要求。
10、两个开口位置不一致的限位件在对管道进行限位后,可以防止管道受外力后从同一侧的开口处滑脱,保证了限位件对管道的限位效果。
11、压合部和条形板可拆卸连接在贴合部件上,方便对固定架、贴合部件和连接部拆装。
12、设置第一突出部和第二突出部,使压合部和条形板在穿过连接座后起到固定和限位作用,并且设置该种可拆卸结构,安装拆卸都较为方便。
13、容纳部设置成一定倾角对准鼻孔,大大减少了插管对鼻腔的机械刺激,减少了因插管过程中管道弯曲引起的异物感,使鼻饲插管操作更加人性化。
14、贴合部件采用硅胶材质制作,在保证较好的静摩擦力的同时,还提高了患者皮肤的舒适感。

下面结合图1-图6对本发明作详细说明。
实施例
一种双管插管固定装置,包括贴合部件2以及设置在贴合部件2上的固定架3,所述贴合部件2的两端均连接有用于将贴合部件2固定于鼻孔下方人中处的连接部1,所述固定架3包括至少两个压合部31,两个所述压合部31的一侧相互连接,两个压合部31的另一侧相互可拆卸连接,两个所述压合部31之间设置有用于插管时放置管道的容纳部。
设置有两个供插管时放置管道的容纳部,每个容纳部均对准鼻孔,可以用于不同管道的固定,即鼻肠管、吸氧管或者其他经鼻腔进入体内的管
道固定。在对管道进行固定时,可以根据管道插入左右鼻孔的位置灵活进行固定,无需繁琐调整固定装置,方便操作、节约时间。
贴合部件2可以做成硬质的贴合结构,也可以设置的软质可形变的贴合结构,其中软质可形变的贴合结构可以适应不同患者,能更好的贴合在人中处,实用性更强,因此贴合部件2优选软质的材料制成。
容纳部的内侧设置有硅胶垫,通过硅胶垫与管道的接触可以产生较大的静摩擦力,能有效减少管道沿着容纳部轴向的滑动。
其中一个所述压合部31配置有卡齿35,另一个所述压合部31配置有与卡齿35配合可拆卸连接的卡块34,卡齿35设置有数个。设置卡块34和数个卡齿35结构,对两个压合部31进行固定并且可以根据情况进行压合部31压紧度的调节,操作简单快捷。
两个所述压合部31远离可拆卸连接处的一侧为铰接或者弹性连接。
如图1所示,两个压合部31采用的是铰接的方式进行连接,为了可以实现压合部31的自动弹开,可在铰接处设置复位弹簧,例如卷簧或者扭簧。如图5所示,则展示的是两个压合部31弹性连接的方式,两个压合部31为一体成型并且上方的压合部31可以充分利用材料的弹性形变,通过转动上方的压合部31,使卡齿35卡在卡块34上,实现压合部31压紧度的调节固定。采用弹性连接的方式,可在两个压合部31上设置两个容纳部以供插管的放置,或者在贴合部件2上设置两个如图5所示的结构。
其中一个所述压合部31的内侧设置有压柱33,另一个所述压合部31的内侧设置有至少两组限位柱36,所述压柱33和限位柱36均突出于容纳部,所述压柱33位于两组限位柱36之间。设置两组限位柱36对插入管道
的位置进行限制,使管道不能左右的摆动,再通过压柱35对管道进行下压固定,进一步增加管道的固定效果。
所述连接部1包括与贴合部件2连接的条形板11,所述条形板11上连接有绑带。条形板11上设置有连接孔14,绑带可以从连接孔14进行绕接并捆绑固定,增加绑带的固定效果。绑带可以设置成松紧带或者棉质带。
所述条形板11中设置有弯折部,且弯折部延伸至耳廓处。条形板11的弯折部将绑带的固定位置引导至耳廓的上方,使绑带在捆绑固定后可以由耳廓进行限位,避免绑带的下移导致固定装置产生位移。
所述条形板11上连接有至少一个限位件12,所述限位件12中设置有供插管管道穿过的通道,所述限位件12的一侧设置有开口,所述限位件12设置有至少两个,且限位件12的开口分别位于条形板11的两侧。设置开口结构,使管道从限位件12的侧边安装进入到限位件12的通道内,方便管道布置的同时也方便管道的拆卸。通过设置开口位置不一致的数个限位件12对插管管道进行引导并限位,使管道沿着患者面颊两侧或一侧进行布置,实现背景技术中所述的“高举平台法”,保证插管的使用效果。
当只需要一个管道进入到患者的鼻腔中,为了节省材料以及使用成本,可以将用于固定管道的限位件12只设置在一侧的连接部1上,如图6所示,另一侧的连接部1替换成没有限位件12的连接部1,并缩小该连接部1的尺寸。
其中一个所述压合部31与贴合部件2可拆卸连接,所述条形板11与贴合部件2可拆卸连接,所述贴合部件2上设置有数个连接座21,所述连接座21上开设有通孔,与贴合部件2连接的压合部31的两端穿过通孔后设置有第二突出部32,所述条形板11与贴合部件2连接的一端穿过通孔后
设置有第一突出部13。设置第一突出部13和第二突出部32,使压合部31和条形板11在穿过连接座21后起到限位作用,并且贴合部件2采用软质的材料,其连接座21也为软质的材料,在需要拆卸固定架3和连接部1时,将连接座21变形使第一突出部13和第二突出部32抽出即可实现快速拆卸。
所述容纳部朝向鼻孔的方向倾斜设置。倾斜对准鼻孔的容纳部使插管保持与鼻孔的角度,减少插管操作时因管道弯曲引起对鼻腔的机械刺激。
所述贴合部件2为硅胶材质制成。硅胶材质的贴合部件2在保证较好的静摩擦力的同时,可提高面部贴合处的舒适感。
使用过程:将贴合部件2放置在鼻孔下方的人中处,并通过绑带捆绑固定在患者头部,实现对固定装置的固定。打开其中一个压合部31,将插管的管道放置在容纳部中,再盖上上述压合部31,两个压合部31通过卡齿35与卡块34配合进行固定,固定后的两个压合部31对管道进行夹持固定。
鼻孔外的管道部分沿着面颊两侧的条形板11进行布置,将管道从有开口的一侧按压至限位件12的通道内,即可方便的完成对管道的布置和固定。

Claims (14)
- 一种双管插管固定装置,包括贴合部件(2)以及设置在贴合部件(2)上的固定架(3),所述贴合部件(2)的两端均连接有用于将贴合部件(2)固定于鼻孔下方人中处的连接部(1),其特征在于,所述固定架(3)包括至少两个压合部(31),两个所述压合部(31)的一侧相互连接,两个压合部(31)的另一侧相互可拆卸连接,两个所述压合部(31)之间设置有用于放置插管管道的容纳部。
- 根据权利要求1所述的一种双管插管固定装置,其特征在于,其中一个所述压合部(31)配置有卡齿(35),另一个所述压合部(31)配置有与卡齿(35)配合可拆卸连接的卡块(34)。
- 根据权利要求2所述的一种双管插管固定装置,其特征在于,所述卡齿(35)设置有数个。
- 根据权利要求1所述的一种双管插管固定装置,其特征在于,两个所述压合部(31)远离可拆卸连接处的一侧为铰接或者弹性连接。
- 根据权利要求1所述的一种双管插管固定装置,其特征在于,其中一个所述压合部(31)的内侧设置有压柱(33),另一个所述压合部(31)的内侧设置有至少两组限位柱(36),所述压柱(33)和限位柱(36)均突出于容纳部,所述压柱(33)位于两组限位柱(36)之间。
- 根据权利要求1所述的一种双管插管固定装置,其特征在于,所述连接部(1)包括与贴合部件(2)连接的条形板(11),所述条形板(11)上连接有绑带。
- 根据权利要求6所述的一种双管插管固定装置,其特征在于,所述条形板(11)中设置有弯折部,且弯折部延伸至耳廓处。
- 根据权利要求6所述的一种双管插管固定装置,其特征在于,所述条形板(11)上连接有至少一个限位件(12),所述限位件(12)中设置有供插管管道穿过的通道。
- 根据权利要求8所述的一种双管插管固定装置,其特征在于,所述限位件(12)的一侧设置有开口。
- 根据权利要求9所述的一种双管插管固定装置,其特征在于,所述限位件(12)设置有至少两个,且限位件(12)的开口分别位于条形板(11)的两侧。
- 根据权利要求6所述的一种双管插管固定装置,其特征在于,其中一个所述压合部(31)与贴合部件(2)可拆卸连接,所述条形板(11)与贴合部件(2)可拆卸连接。
- 根据权利要求11所述的一种双管插管固定装置,其特征在于,所述贴合部件(2)上设置有数个连接座(21),所述连接座(21)上开设有通孔,与贴合部件(2)连接的压合部(31)的两端穿过通孔后设置有第二突出部(32),所述条形板(11)与贴合部件(2)连接的一端穿过通孔后设置有第一突出部(13)。
- 根据权利要求1所述的一种双管插管固定装置,其特征在于,所述容纳部朝向鼻孔的方向倾斜设置。
- 根据权利要求1所述的一种双管插管固定装置,其特征在于,所述贴合部件(2)为硅胶材质制成。
