Description

一种分体的辅助膀胱排尿装置 技术领域
本实用新型属于医疗器械技术领域,尤指一种分体的辅助膀胱排尿装置。
背景技术
膀胱是人体泌尿系统的重要器官,主要分为充盈和排尿两个主要功能。
充盈功能就是充当容器功能,收集肾脏排泄下来的尿液。排尿功能就是在适合排尿场合中通过逼尿肌的做功将膀胱内尿液挤压排出尿道的过程。
在临床中,如果出现充盈病变时,可以通过多种口服药物及手术方式解决充盈功能障碍。而对于排尿功能障碍而言,现有临床使用的技术中除了留置导尿管将尿液排出,目前没有更好方式。而留置导尿管,特别是终身留置导尿管不但对患者的生活质量造成极大影响还容易造成顽固的泌尿道感染,加重抗生素滥用。
针对现有增强膀胱排尿动力的实用新型研究中,有人提出通过重建人工膀胱并置入磁力来达到逼尿肌挤压排尿的作用(一种人工膀胱铁磁复合体及其制备方法,专利号CN112915266A);也有人提出通过使用处理后的动物膀胱来运用动物膀胱的逼尿肌挤压排尿(一种动物源性生物型人工膀胱,专利号CN110856669A);还有人提出通过机械装置重建人工膀胱通过机械动力推动尿液排出(一种人工膀胱装置,专利号CN106580517A和医用原位植入式人工膀胱,专利号CN1150863C)。
另外,还有人提出在原膀胱基础上通过运用辅助外力(机械、温控等方式)对膀胱挤压的方式来增加排尿(一种原位膀胱辅助排尿装置,专利号CN114469474A、一种原位膀胱的辅助排尿装置及其人造膀胱,专利号CN110279488A和一种辅助膀胱排尿装置,专利号CN109248348B;一种膀胱活动力辅助装置的动力装置,专利号CN108743232A)。
前述的专利中增强膀胱排尿动力方式多样,但仍有很多缺点,以至于目前无临床运行的成熟装置,而仍然采用留置导尿管这种原始的方法,具体来说:
完全重建人工膀胱,主要缺点是需要将原膀胱切除,然后游离的输尿管与新的人工膀胱吻合连接。手术复杂,不可重复。而且这将导致两侧输尿管与人工膀胱连接口部位的输尿管缺血坏死继发输尿管坏死、尿漏可能。
现有的膀胱辅助装置类,将材料包饶于膀胱周围,通过磁力或温控等方式启动机械挤压作用来增强膀胱动力,设想虽好,但包埋于膀胱周围反复挤压会产生膀胱与周围组织纤维化,远期会严重影响膀胱充盈功能,导致尿液充盈量减少等可能。而且,当患者因各种原因(生物相容组织不耐受、包饶材料导致膀胱感觉异常、盆腔新发肿瘤需要手术等原因)要求撤除膀胱辅助装置时,由于大量材料包绕于膀胱周围已经形成广泛瘢痕,剥离面巨大,将严重影响膀胱血供,导致患者身体巨大创伤,远期膀胱缺血挛缩等不可逆的困境。
实用新型内容
本实用新型所要解决的技术问题是提供一种分体的辅助膀胱排尿装置,以克服现有技术中存在的种种问题。
本实用新型解决上述技术问题所采取的技术方案如下:
一种分体的辅助膀胱排尿装置,包括:
至少一个进尿管路(1),其一端与膀胱前壁相连,并通过管路头端开口伸入膀胱内,且具有部分设置于人体内部的第一管路(4)和完全设置于人体外部的第二管路(7);
至少一个排尿管路(11);
所述至少一个进尿管路和所述至少一个排尿管路之间具有动力泵(10);
所述动力泵(10)通过所述至少一个进尿管路向膀胱内提供负压,以将膀胱内的尿液吸入到所述至少一个进尿管路内;
所述至少一个进尿管路接收来自于膀胱内的尿液,并通过所述至少一个排尿管路将上述尿液排出;
其中,所述第一管路的管路末端(6)和第二管路的管路头端相连接在一起,且为可分离结构。
本实用新型不需要将输尿管与原膀胱接口切断,避免了输尿管远端缺血坏死,同时,采用分体的结构,整个装置不需要植入到人体中,只需要将进尿 管路部分植入到人体中即可,操作简单。

以下将结合附图及实施例来详细说明本实用新型的实施方式,借此对本实用新型如何应用技术手段来解决技术问题,并达成技术效果的实现过程能充分理解并据以实施。需要说明的是,只要不构成冲突,本实用新型中的各个实施例以及各实施例中的各个特征可以相互结合,所形成的技术方案均在本实用新型的保护范围之内。
具体来说,如图1所示,本实用新型公开了一种分体的辅助膀胱排尿装置,其特征在于,包括:
至少一个进尿管路(1)(实施例中,采用一个管路),其一端与膀胱前壁相连,并通过管路头端开口伸入膀胱内,且具有部分设置于人体内部的第一管路(4)和完全设置于人体外部的第二管路(7);
至少一个排尿管路(11)(实施例中,采用一个管路);
所述至少一个进尿管路和所述至少一个排尿管路之间具有动力泵(10);
所述动力泵(10)通过所述至少一个进尿管路向膀胱内提供负压,以将膀胱内的尿液吸入到所述至少一个进尿管路内;
所述至少一个进尿管路接收来自于膀胱内的尿液,并通过所述至少一个排尿管路(11)将上述尿液排出;
其中,所述第一管路的管路末端(6)和第二管路的管路头端相连接在一起,且为可分离结构。
其中,实施例中,所述第一管路的管路末端(6)和第二管路的管路头端通过螺旋连接在一起。
其中,所述第一管路的管路末端设有可分离的第一消毒封帽(5),且所述第二管路的管路头端设有可分离的第二消毒封帽。
需要排尿时,打开两个消毒封帽旋盖,将二者连接在一起,以便使得所述动力泵(10)通过所述至少一个进尿管路向膀胱内提供负压,以将膀胱内的尿液吸入到所述至少一个进尿管路内。
当不需要排尿的时候,可以直接第一消毒封帽(5)将第一管路的管路末端关闭,其中,第二消毒封帽对第二管路仅需封闭并进行储尿,期间,为保证管头无菌状态,应当将尿管路头端浸泡于消毒液中。其中消毒液每日更换。
更具体地说,所述进尿管路为具有柔韧性和伸展性的抗尿结石功能的管路,包埋于人体内走行,负责与膀胱相连。通常,可以在膀胱前壁作一小切口,以将上述进尿管路植入膀胱内。
所述第一管路具有靠近膀胱的进尿段和靠近尿道口的出尿段;
所述进尿段和出尿段包埋于人体内部,最终在体内走行至尿道外口附近,并开口于尿道外口旁,出尿段从该开口中伸出。
如图3-6,是本实用新型分体的辅助膀胱排尿装置的放置示意图,具体来说,图3中,男性耻骨后走形:进尿段与膀胱前壁相连,出尿段沿着耻骨后方穿过盆腔走形直到阴茎背侧龟头侧开口。
图4中,男性耻骨前走形:进尿段与膀胱前壁相连,出尿段沿着耻骨前方经腹壁走形直到阴茎背侧龟头侧开口。
图5中,女性耻骨后走形:进尿段与膀胱前壁相连,出尿段沿着耻骨后方穿过盆腔走形直到尿道外口前方开口。
图6中,女性耻骨前走形:进尿段与膀胱前壁相连,出尿段沿着耻骨前方经腹壁走形直到尿道外口前方开口。
如图1所示,所述第一管路上设置有至少一个锚定部件,用于固定相应的管路。
具体来说,所述进尿段上具有第一锚定部件(2),所述出尿段上具有第二锚定部件(3)。所述锚定部件为设置于进尿管路外周的一涤纶层,其与外周组织纤维化形成瘢痕,通过瘢痕使得管路固定,防止置入后位移的发生。
在一个实施例中,为了实现更好的排尿效果,所述动力泵10连接有一个压力监测单元,所述压力监测单元包括:
压力传感器和处理器,所述压力传感器用于监测膀胱内的尿液中的压力变化,通常来说,上述所述压力传感器设置于动力泵与进尿管路交界处;
所述处理器,用于基于所述膀胱内的尿液中的压力变化,控制所述动力泵的开启和关闭。
此外,在一个优选实施例中,所述动力泵和压力检测单元连接有供电控制单元;所述供电控制单元通过电力控制线路与所述动力泵和压力检测单元相连接并通讯;
所述供电控制单元包括电源单元和控制单元;
所述电源单元采取感应性可充电电池、电池或者通过充电口(9)连接外部电源;所述控制单元为采取物理压迫或无线控制的开关器(8)。
当然,在一个实施例中,所述动力泵10也可以直接由开关器(8)直接控制动力泵的开启和关闭。
本实用新型采取了上述方案后,在原有膀胱或肠代膀胱中增加辅助动力,不需要将输尿管与原膀胱接口切断,避免了输尿管远端缺血坏死;同时,采用分体的结构,整个装置不需要植入到人体中,只需要将进尿管路部分植入到人体中即可,操作简单。
与最接近现有技术相比,可以减少对人体损害,尽量减少对原有膀胱的干扰,不破坏原有输尿管及尿道结构,后期如果需要拆除该装置时,对人体影响小,不改变任何原有尿路结构。
优选的是,上述进尿管路(1)与排尿管路(11)所述造型为单纯直管路, 当然也可以设置为其他形状,比如带球囊固定、猪尾盘曲、带侧孔或者其他造型,本实用新型对此不进行限定。

  1. 一种分体的辅助膀胱排尿装置,其特征在于,包括:
    至少一个进尿管路(1),其一端与膀胱前壁相连,并通过管路头端开口伸入膀胱内,且具有部分设置于人体内部的第一管路(4)和完全设置于人体外部的第二管路(7);
    至少一个排尿管路(11);
    所述至少一个进尿管路和所述至少一个排尿管路之间具有动力泵(10);
    所述动力泵(10)通过所述至少一个进尿管路向膀胱内提供负压,以将膀胱内的尿液吸入到所述至少一个进尿管路内;
    所述至少一个进尿管路接收来自于膀胱内的尿液,并通过所述至少一个排尿管路将上述尿液排出;
    其中,所述第一管路的管路末端(6)和第二管路的管路头端相连接在一起,且为可分离结构。
  2. 根据权利要求1所述的分体的辅助膀胱排尿装置,其特征在于,所述第一管路的管路末端(6)和第二管路的管路头端通过螺旋连接在一起。
  3. 根据权利要求1或2所述的分体的辅助膀胱排尿装置,其特征在于,所述第一管路的管路末端设有可分离的第一消毒封帽(5),且所述第二管路的管路头端设有可分离的第二消毒封帽。
  4. 根据权利要求1所述的辅助膀胱排尿装置,其特征在于,所述第一管路具有靠近膀胱的进尿段和靠近尿道口的出尿段;
    所述进尿段和出尿段包埋于人体内部,最终在体内走行至尿道外口附近,并开口于尿道外口旁,出尿段从该开口中伸出。
  5. 根据权利要求1或4所述的分体的辅助膀胱排尿装置,其特征在于,所述第一管路上设置有至少一个锚定部件。

  8. 根据权利要求4所述的分体的辅助膀胱排尿装置,其特征在于,所述进尿段与膀胱前壁相连,所述出尿段沿着耻骨后方穿过盆腔走形直到阴茎背侧龟头侧开口;或,
    所述进尿段与膀胱前壁相连,所述出尿段沿着耻骨前方经腹壁走形直到阴茎背侧龟头侧开口;或,
    所述进尿段与膀胱前壁相连,所述出尿段沿着耻骨后方穿过盆腔走形直到尿道外口前方开口;或,
    所述进尿段与膀胱前壁相连,所述出尿段沿着耻骨前方经腹壁走形直到尿道外口前方开口。
  9. 根据权利要求1所述的分体的辅助膀胱排尿装置,其特征在于,所述动力泵连接有压力监测单元,所述压力监测单元包括:
    压力传感器和处理器,所述压力传感器设置于动力泵与进尿管路交界处,并与尿液接触,用于监测膀胱内的尿液中的压力变化;
    所述处理器,用于基于所述膀胱内的尿液中的压力变化,控制所述动力泵的开启和关闭。
  10. 根据权利要求9所述的分体的辅助膀胱排尿装置,其特征在于,所述动力泵和压力检测单元连接有供电控制单元;
    所述供电控制单元通过电力控制线路与所述动力泵和压力检测单元相连接并通讯;
    所述供电控制单元包括电源单元和控制单元;
    所述电源单元采取感应性可充电电池、电池或者通过充电口(9)连接外部电源;所述控制单元为采取物理压迫或无线控制的开关器(8)。
  11. 根据权利要求1所述的分体的辅助膀胱排尿装置,其特征在于,所述进尿管路(1)与排尿管路(11)为单纯直管路、带球囊固定管路、猪尾盘曲管路、带侧孔管路中的一种或者多种。
