Abstract
強迫性障害を治療するための医薬組成物を提供する。 強迫性障害を治療するための有効成分として(1)ドパミンD1受容体刺激物質を含有する医薬組成物、および/または線条体のドパミンD1受容体を刺激することを目的として(2)ドパミンD1およびD2受容体刺激物質を含有する医薬組成物であって、ドパミンD2受容体阻害物質が組み合わされて投与される該医薬組成物、および/または(3)ドパミンD2受容体阻害物質を含有する医薬組成物であって、ドパミンD1およびD2受容体刺激物質が組み合わされて投与される該医薬組成物に関する。

本特許出願は、日本国特許出願2022-096675号(2022年6月15日出願)およびPCT/JP2022/048379(2022年12月27日出願)に基づくパリ条約上の優先権および利益を主張するものであり、ここに引用することによって、上記出願に記載された内容の全体が本明細書中に組み込まれるものとする。
本発明は強迫性障害を治療するための薬物療法に関するものである。詳細には、本発明は、ドパミンD1受容体刺激物質を使用する、および/または線条体のドパミンD1受容体を選択的に刺激することを目的として、ドパミンD1およびD2受容体刺激物質とドパミンD2受容体阻害物質とを組み合わせて使用する、強迫性障害を治療する薬物療法の分野に属する。
強迫性障害(OCD:Obsessive Compulsive Disorder)は、DSM-5(精神障害の診断・統計マニュアル第5版)の病名・用語翻訳ガイドラインによると「強迫症および関連症群、または強迫性障害および関連障害群」(Obsessive Compulsive and Related Disorders)と表記される一群の疾患群を含む、強迫観念または強迫行為といった強迫症状を特徴とする強迫関連障害カテゴリーの中核をなす疾患である。強迫症状は、反復性・持続的な思考や衝動、イメージにとらわれる強迫観念と、手洗い、確認などの繰り返しや儀式行為、呪文を唱える、数を数えるなど心の中の反復行為を含む強迫行為からなる。強迫性障害患者は強迫観念または強迫行為を制御しようと試みているものの、不安や苦痛のためコントロールすることができず、葛藤やストレスを生じている(非特許文献1、2)。
強迫性障害の有病率は1~2%で、男女比はほぼ同等、平均発症年齢は20歳前後で男性がより早発の傾向がある(非特許文献1、2)。強迫性障害の治療は、抗うつ薬の一種である選択的セロトニン再取り込み阻害剤(SSRI)の内服療法と認知行動療法(Cognitive Behavioral Therapy)を併用するのが一般的である(非特許文献2、3)。SSRIの内服は投与開始から12週間、最大投与量で4~6週間継続することが推奨されている(非特許文献2、3、4)。現在、SSRIは強迫性障害治療の第一選択薬であるが、うつ病に対してよりも高用量で長期間の服薬が必要である。長期予後研究では、SSRI治療に反応する強迫性障害患者は10年以上の経過でも半数程度で、その改善率も最大で30~40%にとどまり、一旦軽快しても再発することが多い(非特許文献2)。強迫性障害の臨床においては、SSRIに代わる治療効率の高い薬物治療の開発が望まれている。

上記の通り、強迫性障害を充分に対処できる医薬・手段はなく、有効な治療方法が希求されている。
このような背景の下、本発明者らは、不随意運動症患者の治療中に、偶然にも少量のレボドパに加え、少量のクロルプロマジンもしくはメトクロプラミドを処方したところ、4週間で強迫性障害が改善することを発見し、本発明を完成した。詳細には、L-3,4-ジヒドロキシフェニルアラニン(L-DOPA;レボドパ)(ドパミンD1およびD2受容体刺激物質)とクロルプロマジン(CPZ)またはメトクロプラミド(D2受容体遮断物質)による二重のドパミン調節療法において標準維持量よりも少量かつ、通常の治療期間よりも短期間で、強迫性障害の治療に効果を示すことを初めて見出した。
したがって、本発明は、以下の態様を含む。

<医薬組成物>
本発明は、ひとつの形態として、ドパミンD1受容体の刺激物質を含有する、強迫性障害を治療するための医薬組成物に関する。本発明者らは、本明細書中の下段に示すように、過去の業績としてLDOPA+CPZのジストニア治療を発見した。その際、ジストニアが強迫性障害やうつ病としばしば合併することを報告した過去の報告から、本発明者らは、LDOPA+CPZにおける後者の疾患に対する相同の治療効果を推論し、実際に当該医薬が強迫性障害を治療できることを初めて証明した。

このような先行研究から、ジストニアと強迫性障害に共通する要素である常同性症状 (Stereotyped symptoms)の発現メカニズムとして、線条体ストリオソーム分画の機能異常が疑われている。すなわち、ジストニアや強迫性障害のような常同性を伴う疾患は、ストリオソーム分画の機能異常に関わる可能性を示唆する仮説論文がいくつか発表されている(Ann Graybiel教授)。

しかし、これらいずれの文献も、すべて仮説の域を超えるものでは無く、ヒト疾患の治療に関する具体的な記載は存在しない。
そして、ドパミンD1受容体の刺激物質によって、ストリオソームの機能異常が改善できるという今回の本発明者らの発見は、先行研究はおろか、その可能性を示唆する仮説論文にも記載されていない。「ストリオソームの機能異常」といっても、ジストニアや強迫性障害において、機能低下なのか、機能亢進なのか、研究者によって意見が分かれているのが現状である。
そして、ドパミンD1受容体の刺激物質によって、ストリオソームの機能異常が改善できるという今回の本発明者らの発見は、先行研究はおろか、その可能性を示唆する仮説論文にも記載されていない。「ストリオソームの機能異常」といっても、ジストニアや強迫性障害において、機能低下なのか、機能亢進なのか、研究者によって意見が分かれているのが現状である。

本発明において、ドパミンD1受容体の刺激物質は、ドパミンD1受容体アゴニストおよびドパミンD1受容体-陽性アロステリックモジュレーターの中から選ばれる。
本発明において「ドパミンD1受容体アゴニスト」とは、D1、D2、D3、D4およびD5受容体という5種類のドパミン受容体のうち、興奮性を担うD1様受容体ファミリー(D1受容体およびD5受容体)を刺激する物質であり、D1受容体刺激物質とも称される物質である。従って、本発明において「ドパミンD1受容体アゴニスト」は、ドパミンD1受容体に特異的なアゴニストを意味するのでなく、D1様受容体ファミリーのうちD5受容体など他の受容体の刺激作用を排除するものではない。
ドパミンD1受容体アゴニストとしては具体的には、SKF81297、SKF38393、SKF83959、SKF82526 (フェノルドパン(Fenoldpam))、ジヒドレキシジン (Dihydrexidine)、ABT-431、A-86929、A-77636、A-68930、PF-06649751 (タバパンドン(Tavapandon))、PF-06412 などの薬剤が挙げられるが(Jones-Tabah J, et al. The Signaling and Pharmacology of the Dopamine D1 Receptor. Front Cell Neurosci. 2022.)、現在のところ、D1受容体アゴニストとしてヒト臨床試験を通過して認可されている医薬はない。
本発明において、「ドパミンD1受容体-陽性アロステリックモジュレーター」とは、ドパミンD1受容体のアロステリック部位に結合し、D1受容体のアゴニスト活性を促進する物質を意味し、ドパミンD1受容体アゴニストと同等の効果が期待できる。これは、D1-PAM(D1-Positive Allosteric Modulator)と略称される。アロステリックモジュレーター (Allosteric Modulator:アロステリック調節物質)に関連し、受容体の場合、内因性アゴニストのアゴニスト活性を促進するアロステリック部位に結合するリガンドは、陽性アロステリックモジュレーター(Positive Allosteric Modulator, PAM)と呼ばれ、逆にアゴニスト活性を抑制するアロステリックリガンドは、陰性アロステリックモジュレーター (Negative Allosteric Modulator, NAM) と呼ばれる。

本発明は、ひとつの実施態様として、ドパミンD1受容体の刺激物質としての作用を、通常パーキンソン病等の治療に投与されている「ドパミンD1およびD2受容体アゴニスト」、ならびに通常精神疾患の治療に投与されている「ドパミンD2受容体の阻害物質」を組み合わせて投与することで獲得する、「ドパミンD1およびD2受容体アゴニスト」または「ドパミンD2受容体の阻害物質」を含有する、強迫性障害を治療するための医薬組成物に関する。
本発明は、ひとつの実施態様として、強迫性障害を治療するための医薬組成物であって、(1)ドパミンD1受容体の刺激物質を含有する医薬組成物、および/または(2)ドパミンD1およびD2受容体アゴニストを含有する医薬組成物であって、ドパミンD2受容体の阻害物質が組み合わされて投与される該医薬組成物、および/または(3)ドパミンD2受容体の阻害物質を含有する医薬組成物であって、ドパミンD1およびD2受容体アゴニストが組み合わされて投与される該医薬組成物に関する。
本発明において、「組み合わされて投与される」とは、ドパミンD1およびD2受容体アゴニスト、またはドパミンD2受容体の阻害物質を、例えば同時に、時差をもって、または交互に投与することを含む投与形態を意味する。従って、ドパミンD1およびD2受容体アゴニスト、またはドパミンD2受容体の阻害物質は、同時投与の場合、同一の医薬組成物として投与されてよく、または、同時に別々の医薬組成物として投与されてもよい。ドパミンD1およびD2受容体アゴニスト、またはドパミンD2受容体の阻害物質は、例えば、秒、分、時間、日または週の時間差で別個に投与されてよい。さらに、ドパミンD1およびD2受容体アゴニスト、またはドパミンD2受容体の阻害物質は、他の医薬の前後いずれであっても投与されてもよい。
本発明において、ドパミンD1およびD2受容体アゴニスト、またはドパミンD2受容体の阻害物質は、同一の投与形態で投与されてもよく、あるいは異なる投与形態で投与されてもよい。例えば、両方とも経口用製剤として投与されてよく、あるいは、一方の化合物は経口的に投与され、他方は局所的に投与されてよい。
本発明において、「ドパミンD1受容体の刺激物質」は、ドパミンD2受容体アンタゴニストおよびドパミンD2受容体-陰性アロステリックモジュレーターの中から選ばれる。
本発明において「ドパミンD2受容体アンタゴニスト」とは、D1、D2、D3、D4および、D5受容体という5種類のドパミン受容体のうち、抑制性を担うD2様受容体ファミリー(D2受容体、D3受容体およびD4受容体)を阻害する物質であり、ドパミンD2受容体遮断薬あるいはドパミンD2受容体阻害物質とも称される。従って、本発明において「ドパミンD2受容体アンタゴニスト」は、ドパミンD2受容体に特異的なアンタゴニストを意味するのでなく、ドパミンD2受容体を阻害または遮断する物質であれば足り、D3受容体およびD4受容体などD2様受容体ファミリーの他の受容体の阻害または遮断作用を排除するものではない。
ドパミンD2受容体アンタゴニストの例としては、クロルプロマジン、メトクロプラミド、ドンペリドン、レボメプロマジン、フルフェナジン、ペルフェナジン、プロクロルペラジン、プロペリシアジン、ハロペリドール、ピパンペロン、ブロムペリドール 、ドロペリドール、クエチアピン、アセナピン、スルピリド、スルトプリド、チアプリド、リスペリドン、モサプラミン、ゾテピン、パリペリドン、クロカプラミン、スピペロン、ネモナプリド、チミペロン、ペロスピロンなど、が挙げられる。
本発明において「ドパミンD2受容体-陰性アロステリックモジュレーター」とは、ドパミンD2受容体のアロステリック部位に結合し、D2受容体のアゴニスト活性を抑制する物質を意味し、ドパミンD2受容体アゴニストと同等の効果が期待できる。これは、D2-NAM(D2-Negative Allosteric Modulator)と略称される。ドパミンD2受容体-陰性アロステリックモジュレーターは、Molecules 2023, 28, 178. https://doi.org/10.3390/molecules28010178に概説されている。
ドパミンD2受容体-陰性アロステリックモジュレーターとしては具体的には、SB269652(Mol Pharmacol 91:586-594, June 2017;J. Med. Chem. 2015, 58, 6819-6843;Biomedicines 2022, 10, 22. https://doi.org/10.3390/biomedicines10010022;J. Med. Chem. 2019, 62, 174-206)が挙げられる。現在のところ、D2受容体-陰性アロステリックモジュレーターとしてヒト臨床試験を通過して認可されている医薬はない。
ドパミンD1およびD2受容体アゴニストの例としては、レボドパ、レボドパ合剤であるレボドパ・カルビドパ水和物、レボドパ・カルビドパ・エンタカポン配合製剤、レボドパ・ベンセラジド塩酸塩配合剤、ドパミンアゴニストであるペルゴリド、ロチゴチンなど、が挙げられる。
レボドパは、L-3,4-ジヒドロキシフェニルアラニン(L-DOPAまたはLDOPA)とも称され、ドパミンの直接の前駆体であり、D1およびD2受容体(D1RおよびD2R)に対するフル・アゴニストである。
ドパミンD1およびD2受容体アゴニストとしてレボドパを使用する場合において、レボドパは末梢(脳内に移行する前)の段階でレボドパ脱炭酸酵素によって分解され、脳内へ移行できなくなるため、レボドパ製剤には末梢でのレボドパの代謝を抑えるカルビドパやベンセラジドが配合される。その例が、レボドパとカルビドパの配合製剤(主な商品名:ネオドパストン、メネシット)やレボドパとベンセラジドの配合製剤(主な商品名イーシー・ドパール、ネオドパゾール、マドパー)である。本発明において、ドパミンD1およびD2受容体アゴニストとして、いずれの配合製剤も利用できるが、好適には、ドパコール配合錠が用いられる。本明細書において、ドパコール配合錠を投与した場合、その実質的な薬効はレボドパに由来するため、ドパコール配合錠の投与は、レボドパの投与と実質的に同等とみなすことができる。
レボドパは、L-3,4-ジヒドロキシフェニルアラニン(L-DOPAまたはLDOPA)とも称され、ドパミンの直接の前駆体であり、D1およびD2受容体(D1RおよびD2R)に対するフル・アゴニストである。
ドパミンD1およびD2受容体アゴニストとしてレボドパを使用する場合において、レボドパは末梢(脳内に移行する前)の段階でレボドパ脱炭酸酵素によって分解され、脳内へ移行できなくなるため、レボドパ製剤には末梢でのレボドパの代謝を抑えるカルビドパやベンセラジドが配合される。その例が、レボドパとカルビドパの配合製剤(主な商品名:ネオドパストン、メネシット)やレボドパとベンセラジドの配合製剤(主な商品名イーシー・ドパール、ネオドパゾール、マドパー)である。本発明において、ドパミンD1およびD2受容体アゴニストとして、いずれの配合製剤も利用できるが、好適には、ドパコール配合錠が用いられる。本明細書において、ドパコール配合錠を投与した場合、その実質的な薬効はレボドパに由来するため、ドパコール配合錠の投与は、レボドパの投与と実質的に同等とみなすことができる。
本発明は、ひとつの実施態様として、ドパミンD1受容体の刺激物質を含有する、強迫性障害を治療するための医薬組成物であって、ドパミンD2受容体の阻害物質が組み合わされて投与される該医薬組成物に関する。ドパミンD1受容体の刺激物質を単独で用いるよりも、ドパミンD2受容体の阻害物質と組み合わせた方が、より高い強迫性障害を治療するための効果が出ると期待される。D1受容体を単独で刺激しても、二次的な作用でD2受容体の活性が高まる可能性があるためである。ここに、ドパミンD1受容体の刺激物質およびドパミンD2受容体の阻害物質の定義は上記の通りである。
「治療」とは本明細書において、(1)疾患または状態(例、強迫性障害)の症状の進行、増悪または悪化を減速または停止させる;(2)疾患または状態の症状の寛解をもたらす;あるいは(3)疾患または状態を治癒させることを目的とする方法またはプロセスを意味する。
本発明は、ひとつの実施態様として、強迫性障害を治療するための、レボドパを含有する医薬組成物であって、レボドパ量としてパーキンソン病治療の標準維持量の半量以下で経口投与されるように用いられ、同時にクロルプロマジンがクロルプロマジン塩酸塩として抗精神病治療量の半量以下、メトクロプラミドが抗嘔吐剤としての用量の半分で経口投与されることを特徴とする、医薬組成物に関する。
レボドパ量としてパーキンソン病治療の標準維持量とは、レボドパ量として1回200~250mg、1日3回=1日600~750mgである。クロルプロマジン塩酸塩として抗精神病治療量とは、1日30~100mgである。本発明において、レボドパ量としてパーキンソン病治療の標準維持量の半量以下が、1日50~300mg、好ましくは1日150~300mgである「ドパミンD1およびD2受容体アゴニスト」を含有する医薬組成物、およびクロルプロマジン塩酸塩として抗精神病治療量の半量以下が、1日5~30mg、好ましくは1日5~30mgである「ドパミンD2受容体の阻害物質」を含有する医薬組成物が好適である。本発明において、レボドパ量としてパーキンソン病治療の標準維持量の半量以下が1日50mg である「ドパミンD1およびD2受容体アゴニスト」を含有する医薬組成物、およびクロルプロマジン塩酸塩として抗精神病治療量の半量以下が1日5mgである「ドパミンD2受容体の阻害物質」を含有する医薬組成物であっても、本発明の効果を有することが実証されている。
本発明において、「医薬組成物」とは、有効量の「ドパミンD1およびD2受容体アゴニスト」または「ドパミンD2受容体の阻害物質」、および少なくとも1つの薬学的に許容され得る担体または賦形剤を含む。本明細書で使用する「有効量」なる用語は、その意図する目的、例えば、細胞、組織、系、または被験者における所望の生物学的または薬理応答を満たすために十分である化合物または組成物の任意の量を意味する。例えば、本発明の特定の実施態様において、目的は、強迫性障害の進行、増悪または悪化を減速、軽減または停止させること、疾患の症状の寛解をもたらすこと、および/または強迫性障害を治療することを意味する。
「薬学的に許容され得る担体または賦形剤」は、有効成分とともに製剤化され、所望の投与量でそれを必要とする被験者に任意の適した経路により投与することができる。種々の送達システムが公知であり、例えば、錠剤、カプセル剤、徐放性製剤、注射剤、リポソーム中のカプセル化剤、粉末剤、貼付剤、マイクロカプセル剤、ナノ粒子製剤などを含む。
本発明における治療は、単回用量または複数回用量からなり得る。このように、本発明の「ドパミンD1受容体の刺激物質」、「ドパミンD1およびD2受容体アゴニスト」、「ドパミンD2受容体の阻害物質」、またはその医薬組成物の投与は、特定の期間にわたり一定、または定期的、および特定の間隔、例えば数時間1回、1日1回、週1回、月1回(例、時間放出形態)であり得る。
本発明は別の形態として、強迫性障害を治療するための方法であって、ドパミンD1受容体の刺激物質を、そのような治療を必要としている対象に投与することを含む方法に関する、さらに、本発明は、強迫性障害を治療するための方法であって、ドパミンD1およびD2受容体アゴニストをドパミンD2受容体の阻害物質と組み合わせて、そのような治療を必要としている対象に投与することを含む方法、および強迫性障害を治療するための方法であって、ドパミンD2受容体の阻害物質をドパミンD1およびD2受容体アゴニストと組み合わせて、そのような治療を必要としている対象に投与することを含む方法に関する。
さらに、本発明は別の形態として、強迫性障害を治療するためのドパミンD1受容体の刺激物質に関する。さらに、本発明は、強迫性障害を治療するための、ドパミンD1およびD2受容体アゴニストとドパミンD2受容体の阻害物質との組み合わせに関する。
本発明はさらなる別の形態として、強迫性障害を治療するための医薬を製造するための、ドパミンD1受容体の刺激物質の使用に関する。本発明は、強迫性障害を治療するための医薬を製造するための、ドパミンD1およびD2受容体アゴニストとドパミンD2受容体の阻害物質との組み合わせの使用に関する。
以下、本発明を参考例および実施例により、詳細に説明するが、これらは本発明の範囲を限定するものでなく、単なる例示であることに留意すべきである。
実施例1

被験者

薬物
ドパコール錠L50(登録商標)(L-DOPA50mg+カルビドパ5mg、日医工株式会社、富山県、日本)、ウィンターミン細粒(10%)(登録商標)(フェノールフタル酸クロルプロマジン1gあたり180mg、塩野義製薬株式会社、大阪府、日本)を使用した。以下、ドパコール錠L50を「DOPACOL」、ウィンターミン細粒(10%)を「CPZ」と称することがある。
CPZ投与の投与計画
症例番号1~4について。投与計画として、最初の4週間はDOPACOL(1錠(50mg)/日)+CPZ(5mg)/日、次の4週間はDOPACOL(1錠×2(100mg)/日)+CPZ(5mg)×2/日、最後の4週間はDOPACOL(1錠×3(150mg)/日)+CPZ(5mg)×3/日を処方する計画を立てた。

症例番号5について。メトクロプラミド錠(塩酸メトクロプラミド5mg、武田薬品工業株式会社)を使用する場合の症例は次の通りである。
66歳女性55歳発症。病気に対する心配で複数の医療機関を受診し、入院することもあった。YBOCSでは22点であり、中等度の強迫性障害と診断した。レボドパ50mg、メトクロプラミド5mgを内服し、8週間後 に症状は完全に消失した(YBOCS:0点 )。

ここに、投与薬剤はドパコール配合錠であるが、その実質的な薬効はレボドパに由来するため、レボドパLDOPAの投与と表現している。
表2および図1に示すように、ドパミンD1、D2受容体アゴニスト、ドパミンD2受容体アンタゴニストを少量、短期間内服することで全例、症状は改善した(YBOCSの平均改善率65%、全例に有効)。さらに高用量、長期間投与することにより改善する可能性が高い。

実施例2
CPZ + LDOPA 内服治療を行った強迫性障害(OCD)患者20名に対する効果
被験者

投与計画は実施例1に準じるが、次回の用量に増量するまでの期間(増量間隔)は、被験者の通院状況と服薬コンプライアンスに応じて2週間毎から12週毎の範囲で個別に設定した(表3)。なお、増量間隔は例えば、2週間毎に通院可能な被検者は、2週間毎で固定し、3か月毎に通院可能な被検者は、3か月毎で固定した。すなわち、同一患者では何回増量してもその増量間隔は一定である。初回投与量をDOPACOL(1錠(50mg)/日)+CPZ(5mg)/日とし、2週間以上の一定間隔を開けて次回投与量をDOPACOL(1錠×2(100mg)/日)+CPZ(5mg)×2(10mg)/日、次々回投与量を DOPACOL(1錠×3(150mg)/日)+CPZ(5mg)×3(15mg)/日とした。さらに用量依存性の効果を検証するため、被検者2名(症例番号5、10)については3回目の増量で最大投与量を DOPACOL(1錠×4(200mg)/日)+CPZ(5mg)×4(20mg)/日[各剤:朝2錠、昼1錠、夕1錠]とし、被検者1名(症例番号4)について4回目の増量で最大投与量を DOPACOL(1錠×5(250mg)/日)+CPZ(5mg)×5(25mg)/日[各剤:朝2錠、昼2錠、夕1錠]とした。

得られた結果を表3および図2に示す。表3および図2は、CPZ + LDOPA 内服治療を行った強迫性障害(OCD)患者20名に対する治療効果を示している。上述の投与計画に基づいて、20例全例で症状改善を認めた(YBOCSの平均改善率70%、全例に有効)。治療前と治療後のYBOCS平均値について t検定を行うとP=0.0000004(****P<0.001)と極めて高い統計的有意差が検出された。
また、図3は、CPZ + LDOPA 内服治療を行った強迫性障害(OCD)患者20名に対する用量依存性の改善効果を示している。20例全例で投与量の漸増によって用量依存的にOCD症状(YBOCS)が改善した。最大で CPZ=25mg/日、LDOPA=250mg/日まで用量依存的に症状改善が認められ、症例番号4の被検者では投与開始から10週間後の評価で症状は完全に消失した(YBOCS=0)。
実施例3
CPZ内服 + ロチゴチン貼付治療を行った強迫性障害(OCD)患者12名に対する効果
被験者

得られた結果を表4および図4に示す。表4および図4は、CPZ内服 + ロチゴチン貼付治療を行った強迫性障害(OCD)患者12名に対する治療効果を示している。上述の治療計画に基づいて、12例全例で症状改善を認めた(YBOCSの平均改善率69%、全例に有効)。治療前と治療後のYBOCS平均値についてt検定を行うとP=0.0000006(****P<0.001)と極めて高い統計的有意差が検出された。
また、図5は、CPZ内服 + ロチゴチン貼付治療を行った強迫性障害(OCD)患者12名に対する用量依存性の改善効果を示している。12例全例で投与量の漸増によって用量依存性にOCD症状(YBOCS)が改善した。被検者2名(症例23、症例26)では、投与開始からそれぞれ10週間後(CPZ=15, NP=13.5)、8週間後(CPZ=10, NP=9)の評価で症状は完全に消失(YBOCS=0)した。
実施例4
CPZ以外のD2受容体アンタゴニスト + LDOPA 内服治療を行った強迫性障害(OCD)患者2名に対する効果
実質的に実施例1の手法に従い、CPZを除く他のD2受容体アンタゴニストとしてメトクロプラミドとスルピリドの治療効果を検討した。
メトクロプラミド錠(塩酸メトクロプラミド5mg、武田薬品工業株式会社)を使用する場合の症例は次の通りである。

ドグマチール錠(スルピリド50mg、日医工株式会社、富山、日本)を使用する場合の症例は次の通りである。55才男性。37歳発症。失敗の確認、書き直し、読み直しなどの強迫行為があり生活は苦痛であった。YBOCSは11点であり、軽度の強迫性障害と診断した。LDOPA=50mg、スルピリド=50mgをそれぞれ1日1回朝内服し、4週後にYBOCSを評価した。なお、投与薬剤はドパコール配合錠であるが、その実質的な薬効はレボドパに由来するため、レボドパLDOPAの投与と表現している。
得られた結果を図6に示す。図6は、CPZ以外のD2受容体アンタゴニスト + LDOPA 内服治療を行った強迫性障害(OCD)患者2名に対するOCD症状(YBOCS)の改善効果を示している。CPZの代わりにメトクロプラミドを使用した1例、スルピリドを使用した1例ともに、低用量の初回投与量(スルピリド50mg+LDOPA50mg, メトクロプラミド5mg+LDOPA50mg)で4週後の評価時に著名な症状の改善を認めた。メトクロプラミドを用いた系列2では症状は完全に消失し(YBOCS=0)、またスルピリドを用いた系列1でも、症状はほぼ消失(YBOCS=1)した。これにより、CPZ以外のD2受容体アンタゴニストとLDOPAの併用においても、十分に有効な臨床効果が得られることが示唆された。
なお、本発明者は、以下の参考例の事実に基づき、ひとつの態様として、ジストニアを治療するための、レボドパを含有する医薬組成物であって、レボドパ量としてパーキンソン病治療の標準維持量の半量以下で経口投与されるように用いられ、同時にクロルプロマジンがクロルプロマジン塩酸塩として抗精神病治療量の半量以下で経口投与されることを特徴とする、医薬組成物も見出している。その点について説明しておく。

ドパコール錠L50(登録商標)(L-DOPA50mg+カルビドパ5mg、日医工株式会社、富山県、日本)、ウィンターミン細粒(10%)(登録商標)(フェノールフタル酸クロルプロマジン1gあたり180mg、塩野義製薬株式会社、大阪府、日本)を使用した。以下、ドパコール錠L50を「DOPACOL」、ウィンターミン細粒(10%)を「CPZ」と称することがある。

患者の仕分けと薬物の投与
来院順に、患者を無作為に3群(L-DOPA、CPZ、L-DOPA+CPZ)に分けた。二重盲検法により、参加者と評価者は各群の識別を知らされていない。
L-DOPA群:このグループには眼瞼痙攣の患者7人(男性3人、女性4人)が含まれ、DOPACOLを単独で摂取した。年齢層は51~79歳(年齢平均66.0±10.1歳)、平均罹病期間は10.7±9.5年であった。最初の2週間はDOPACOL(1錠/日)、次の2週間はDOPACOL(1錠×2/日)、最後の4週間はDOPACOL(1錠×3/日)を処方した。

CPZ群:このグループには眼瞼痙攣患者7名(男性1名、女性6名)が含まれ、CPZを単独で摂取した。年齢層は60-72歳(年齢平均66.4±3.9歳)、平均罹病期間は8.4±5.9年であった。最初の2週間はCPZ(5 mg/日)、次の2週間はCPZ(5 mg x 2/日)、最後の4週間はCPZ(5 mg x 3/日)を処方した。
L-DOPA+CPZ群:このグループには眼瞼痙攣の患者7名(男性2名、女性5名)が含まれ、DOPACOLとCPZの両方を摂取した。年齢層は66~78歳(年齢平均73.5±5.9歳)、平均罹病期間は8.0±7.6年であった。最初の2週間はDOPACOL(1錠/日)とCPZ(5mg/日)、次の2週間はDOPACOL(1錠×2/日)とCPZ(5mg×2/日)、最後の4週間はDOPACOL(1錠×3/日)とCPZ(5mg×3/日)の処方を行った。
統計解析
統計的有意性は、ウィルコクソンの符号順位検定(Wilcoxon signed-rank test)または一元配置分散分析(ANOVA)を用いて評価した。統計的有意性はP < 0.05とした。統計解析は、SPSS統計ソフトウェア(Windows用バージョン11.0)(IMB Corp., Armonk、New York、USA)を使用して行った。

結果:
眼瞼痙攣の重症度は、自覚症状としてVASとBSDIを、客観症状としてmVASとJRSを用いて評価した(図7)。まず、L-DOPA、CPZ、L-DOPA+CPZの全群において、薬物チャレンジテスト前の重症度を比較すると、すべての自覚症状および客観症状において有意差は認められなかった(P > 0.05, one-way ANOVA)。つまり、薬物チャレンジテスト前の重症度には、3群間で明らかな偏りはないことが確認された。
眼瞼痙攣の重症度は、自覚症状としてVASとBSDIを、客観症状としてmVASとJRSを用いて評価した(図7)。まず、L-DOPA、CPZ、L-DOPA+CPZの全群において、薬物チャレンジテスト前の重症度を比較すると、すべての自覚症状および客観症状において有意差は認められなかった(P > 0.05, one-way ANOVA)。つまり、薬物チャレンジテスト前の重症度には、3群間で明らかな偏りはないことが確認された。
次に、各症状において、薬物チャレンジテスト前後の重症度を対比した。
L-DOPA群(図7;上):
修正VAS[薬物チャレンジテスト前 141.3 ± 8.4 (n=7), 薬物チャレンジテスト後 178.8 ± 24.3 (n=7), 「前」vs「後」; P < 0.05, ウィルコクソンの符号順位検定]では、症状の重篤化が有意に進行したが、VAS、BSDI、およびJRSでは進行しなかった。
L-DOPA群(図7;上):
修正VAS[薬物チャレンジテスト前 141.3 ± 8.4 (n=7), 薬物チャレンジテスト後 178.8 ± 24.3 (n=7), 「前」vs「後」; P < 0.05, ウィルコクソンの符号順位検定]では、症状の重篤化が有意に進行したが、VAS、BSDI、およびJRSでは進行しなかった。
CPZ群(図7;中央):
VAS、BSDI、mVAS、およびJRSによる判定のすべてにおいて、薬物チャレンジテスト前後の重症度に有意な変化は見られなかった。

L-DOPA+CPZの投与により、VAS、BSDI、mVAS、およびJRSの自覚症状と客観症状のすべてにおいて、薬物チャレンジテスト後の重症度が前に比較して有意に減少した。VAS [薬物チャレンジテスト前 67.1 ± 14.6 (n=7), 薬物チャレンジテスト後 44.8 ± 27.5 (n=7),「前」vs「後」; P < 0.05, ウィルコクソンの符号順位検定]、BSDI [薬物チャレンジテスト前 15.1 ± 6.6 (n=7) , 薬物チャレンジテスト後 10.6 ± 6.7 (n=7), 「前」vs「後」; P < 0.05, ウィルコクソンの符号順位検定]、mVAS [薬物チャレンジテスト前 148.0 ± 17.4 (n=7); 薬物チャレンジテスト後 116.5 ± 14.5 (n=7), 「前」vs「後」; P < 0.05, ウィルコクソンの符号順位検定]、およびJRS [薬物チャレンジテスト前 5.9 ± 2.2 (n=7), 薬物チャレンジテスト後 3.0±2.3 (n=7), 「前」vs「後」; P < 0.05, ウィルコクソンの符号順位検定]。
L-DOPA+CPZの投与により、VAS、BSDI、mVAS、およびJRSの自覚症状と客観症状のすべてにおいて、薬物チャレンジテスト後の重症度が前に比較して有意に減少した。VAS [薬物チャレンジテスト前 67.1 ± 14.6 (n=7), 薬物チャレンジテスト後 44.8 ± 27.5 (n=7),「前」vs「後」; P < 0.05, ウィルコクソンの符号順位検定]、BSDI [薬物チャレンジテスト前 15.1 ± 6.6 (n=7) , 薬物チャレンジテスト後 10.6 ± 6.7 (n=7), 「前」vs「後」; P < 0.05, ウィルコクソンの符号順位検定]、mVAS [薬物チャレンジテスト前 148.0 ± 17.4 (n=7); 薬物チャレンジテスト後 116.5 ± 14.5 (n=7), 「前」vs「後」; P < 0.05, ウィルコクソンの符号順位検定]、およびJRS [薬物チャレンジテスト前 5.9 ± 2.2 (n=7), 薬物チャレンジテスト後 3.0±2.3 (n=7), 「前」vs「後」; P < 0.05, ウィルコクソンの符号順位検定]。
L-DOPA、CPZ、L-DOPA+CPZの全群において、薬物チャレンジテスト中に明らかな神経学的あるいは精神神経学的な変化は認められなかった。したがって、DOPACOL単独またはCPZ単独の投与ではなく、DOPACOL(1錠×3/日)とCPZ(5mg×3/日)の併用によってのみ、眼瞼痙攣に対する治療効果を生じ得ることが確認された。
ジストニアとは、持続的な筋収縮により異常姿勢や運動の障害を来たす病態である。小児例では遺伝子変異を伴う全身性ジストニアが多いが、成人例では局所性ジストニアの場合が多い。局所性ジストニアは頻度が高く、眼瞼痙攣、痙性斜頸、書痙などがある。本発明者らは、学会開催日程:World Congress of Neurology - Kyoto, 16-21 September 2017において、レボドパに加え、スルピリドまたはメトクロプラミドを処方したところ、偶然にもジストニアを軽減させることを見出し、発表した(World Congress of Neurology - Kyoto, 16-21 September 2017 抄録集掲載:VOLUME 381, SUPPLEMENT , 580, OCTOBER 15, 2017 Three cases of dystonia with effective levodopa and D2 blocker combination therapy S. - ドパミンD1受容体の刺激物質を含有する、強迫性障害を治療するための医薬組成物。
- ドパミンD1受容体の刺激物質が、ドパミンD1受容体アゴニストおよびドパミンD1受容体-陽性アロステリックモジュレーターの中から選ばれる、請求項1記載の医薬組成物。
- ドパミンD1受容体の刺激物質が、ドパミンD1受容体アゴニストである、請求項2記載の医薬組成物。
- ドパミンD1受容体アゴニストが、SKF81297、SKF38393、SKF83959、SKF82526 (フェノルドパン(Fenoldpam))、ジヒドレキシジン (Dihydrexidine)、ABT-431、A-86929、A-77636、A-68930、PF-06649751 (タバパンドン(Tavapandon))、PF-06412の中から選ばれる、請求項3記載の医薬組成物。
- ドパミンD1受容体の刺激物質が、ドパミンD1受容体-陽性アロステリックモジュレーターである、請求項2記載の医薬組成物。
- ドパミンD1受容体-陽性アロステリックモジュレーターが、DETQ (2-(2,6-ジクロロフェニル)-1-((1S,3R)-3-(ヒドロキシメチル)-5-(2-ヒドロキシプロパン-2-イル)-1-メチル-3,4-ジヒドロイソキノリン-2(1H)-イル)エタン-1-オン)、メビダレン、MLS1082、MLS6585、ピラゾリル-ジヒドロイソキノリン、DPTQ、CID 2886111([N-(6-tert-ブチル-3-カルバモイル-4,5,6,7-テトラヒドロ-1-ベンゾチオフェン-2-イル)ピリジン-4-カルボキサミド)、LY3154885、ASP4345の中から選ばれる、請求項5記載の医薬組成物。
- ドパミンD1およびD2受容体アゴニストを含有する、強迫性障害を治療するための医薬組成物であって、ドパミンD2受容体の阻害物質が組み合わされて投与される該医薬組成物。
- ドパミンD2受容体の阻害物質が、ドパミンD2受容体アンタゴニストおよびドパミンD2受容体-陰性アロステリックモジュレーターの中から選ばれる、請求項7記載の医薬組成物。

- ドパミンD2受容体アンタゴニストが、クロルプロマジン、メトクロプラミド、ドンペリドン、レボメプロマジン、フルフェナジン、ペルフェナジン、プロクロルペラジン、プロペリシアジン、ハロペリドール、ピパンペロン、ブロムペリドール 、ドロペリドール、クエチアピン、アセナピン、スルピリド、スルトプリド、チアプリド、リスペリドン、モサプラミン、ゾテピン、パリペリドン、クロカプラミン、スピペロン、ネモナプリド、チミペロン、ペロスピロンの中から選ばれる、請求項9記載の医薬組成物。
- ドパミンD2受容体の阻害物質が、ドパミンD2受容体-陰性アロステリックモジュレーターである、請求項8記載の医薬組成物。
- ドパミンD2受容体-陰性アロステリックモジュレーターが、SB269652である、請求項11記載の医薬組成物。

- レボドパ、およびクロルプロマジンが組み合わされて投与される、請求項13記載の医薬組成物。
- レボドパ量としてパーキンソン病治療の標準維持量の半量以下で経口投与されるように用いられることを特徴とする、請求項7から14のいずれか記載の医薬組成物。
- レボドパ量としてパーキンソン病治療の標準維持量の半量以下が、1日50~300mgである、請求項15記載の医薬組成物。
- レボドパ量としてパーキンソン病治療の標準維持量の半量以下が、1日50mgである、請求項16記載の医薬組成物。
- ドパミンD2受容体の阻害物質を含有する、強迫性障害を治療するための医薬組成物であって、ドパミンD1およびD2受容体アゴニストが組み合わされて投与される該医薬組成物。
- ドパミンD2受容体の阻害物質が、ドパミンD2受容体アンタゴニストおよびドパミンD2受容体-陰性アロステリックモジュレーターの中から選ばれる、請求項18記載の医薬組成物。
- ドパミンD2受容体の阻害物質が、ドパミンD2受容体アンタゴニストである、請求項19記載の医薬組成物。
