Abstract
본 발명은 신규한 다원 융합고리 화합물 및 이를 유효성분으로 포함하는 다발성골수종의 예방 또는 치료용 조성물에 관한 것이다. 본 발명의 화합물은 다양한 다발성골수종 세포주에 대해 현저한 사멸효과를 보이면서도, 천연 화합물인 진세노사이드를 출발 물질로 합성된 유도체로서 정상조혈모세포에 대한 세포독성이 전혀 나타나지 않아 부작용 없는 장기 투여가 가능하다. 아울러, 본 발명의 화합물은 면역조절제인 탈리도마이드나 이의 유사체인 레날리도마이드와 병용 투여시 현저한 상승 효과를 나타내어 난치성 질환인 다발성골수종에 대한 효율적인 치료제 또는 치료 보조제 조성물로 유용하게 이용될 수 있다.
Description
본 발명은 4원-융합고리 구조를 가지는 신규 화합물 및 이를 포함하는 다발성골수종을 비롯한 골수암의 예방 또는 치료용 조성물에 관한 것이다.
다발성골수종은 성인에서 발생하는 혈액암 중 인구 고령화와 맞물려 그 발병율이 가장 급격하게 증가하고 있다. 다발성골수종은 멜팔란(melphalan), 프레니손(prednisone) 요법을 근간으로 치료가 시도되어 왔으나 면역조절제인 탈리도마이드(thalidomide) 및 레날리도마이드(lenalidomide)와 프로테오좀 억제제인 보르테조밉(bortezomib) 계열의 항암제가 임상 현장에서 널리 이용되고 있다. 그러나, 이들 약물은 정상 조혈모세포를 파괴하거나 이의 기능을 억제하여 범혈구감소증 외에도 말초신경염, 색전/혈전증, 이치성 원발암의 발병 등 심각한 부작용을 야기한다. 뿐만 아니라 다발성골수종은 약물 내성을 증가와 함께 사망률 또한 크게 증가하고 있다
이에, 다발성골수종의 성공적 치료와 환자의 생존율 개선을 위해서는 상기 언급한 부작용이 획기적으로 경감된 신규한 항암제의 개발이 요구되고 있다. 따라서, 본 발명은 다발성골수종세포에 대한 현저한 사멸효과를 보이는 신규한 저분자 화합물을 발굴하고 이의 항암 효과를 세포주와 동물 실험을 통해 다각적으로 확인하는 한편, 정상인 공여자 유래 말초혈액 조혈모세포군에 대한 유의한 독성이 나타나지 않음을 관찰함으로써 난치성 혈액암인 다발성골수종에 대한 효율적인 치료 조성물을 제공하고자 한다.

본 발명자들은 다발성골수종을 비롯하여 형질세포(plasma cell)의 비정상적인 증식을 원인으로 하는 난치성 혈액암에 대한 효과적이면서도 부작용 없는 저분자 화합물 치료제를 개발하기 위하여 예의 연구 노력하였다. 그 결과, 하기 화학식 1로 표시되는 사환구조 트리테르펜 화합물이 정상조혈모세포에 대해 유의한 독성을 나타내지 않으면서도 다양한 다발성골수종 세포주에 대해 현저한 사멸효과를 보이고 인 비보에서 상용화된 약물을 상회하는 치료효과를 보인다는 사실을 발견함으로써, 본 발명을 완성하게 되었다.
따라서 본 발명의 목적은 하기 화학식 1로 표시되는 신규한 사환구조 트리테르펜 화합물 및 이를 유효성분으로 포함하는 다발성골수종의 예방 또는 치료용 조성물을 제공하는 데 있다.

보다 구체적으로는 본 발명의 조성물로 예방 또는 치료하고자 하는 혈액암은 골수암이며, 가장 구체적으로는 다발성골수종(multiple myeloma)이다.
본 명세서에서 용어“다발골수종”은 골수에서 면역체계를 담당하는 백혈구의 한 종류인 형질세포(Plasma Cell)가 비정상적으로 분화, 증식하여 발생하는 혈액암을 의미한다.
본 발명의 구체적인 구현예에 따르면, 본 발명의 조성물은 하기 화학식 3으로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을 추가적으로 포함한다:

보다 구체적으로는 상기 B1은 -NH2이고 B2는 수소이다. 옥텟 규칙(octet rule)에 의할 때, B2가 수소인 경우 가 단일결합임은 자명하다. B1은 -NH2이고 B2는 수소인 화학식 7 화합물은 레날리도마이드(C13H13N3O3, 3-(4-아미노-1-옥소이소인돌린-2-일)피페리딘-2,6-디온에탈리도마이드)이다.
레날리도마이드(lenalidomide)는 다발성골수종 및 골수이형성증후군 치료에 널리 사용되고 있는 면역조절제로서, 혈관신생억제 및 항염증 활성을 가진다. 본 발명에 따르면, 본 발명의 화합물은 다발성골수종에 대한 대표적인 상용화 치료제인 레날리도마이드와 비교하여 동등이상의 치료 효과를 발휘할 뿐 아니라, 레날리도마이드와 병용 투여시 현저한 상승효과(synergistic effect)를 나타내었다. 이에, 본 발명의 화합물은 그 자체로 단독 항암제로 적용될 수 있음은 물론, 레날리도마이드 또는 유사 면역조절제의 효율적인 치료 보조제로 유용하게 이용될 수 있다.
본 발명의 또 다른 구체적인 구현예에 따르면, 상기 B1은 수소이고 B2는 산소이다. 옥텟 규칙(octet rule)에 의할 때, B2가 산소인 경우 가 이중결합임은 자명하다. B1은 수소이고 B2는 산소인 화학식 7 화합물은 탈리도마이드(C13H10N2O4, 2-(2,6-디옥소피페리딘-3-일)-1H-이소인돌-1,3(2H)-디온)이다.
탈리도마이드(thalidomide)는 레날리도마이드의 원형이 되는 유사체로서 역시 2005년 미국에서 레날리도마이드가 승인되기 전까지 다발성골수종의 치료에 오랜 기간 사용되어 왔다.
본 발명의 조성물이 약제학적 조성물로 제조되는 경우, 본 발명의 약제학적 조성물은 약제학적으로 허용되는 담체를 포함한다. 본 발명의 약제학적 조성물에 포함되는 약제학적으로 허용되는 담체는 제제시에 통상적으로 이용되는 것으로서, 락토스, 덱스트로스, 수크로스, 솔비톨, 만니톨, 전분, 아카시아 고무, 인산 칼슘, 알기네이트, 젤라틴, 규산 칼슘, 미세결정성 셀룰로스, 폴리비닐피롤리돈, 셀룰로스, 물, 시럽, 메틸 셀룰로스, 메틸히드록시벤조에이트, 프로필히드록시벤조에이트, 활석, 스테아르산 마그네슘 및 미네랄 오일 등을 포함하나, 이에 한정되는 것은 아니다. 본 발명의 약제학적 조성물은 상기 성분들 이외에 윤활제, 습윤제, 감미제, 향미제, 유화제, 현탁제, 보존제 등을 추가로 포함할 수 있다. 적합한 약제학적으로 허용되는 담체 및 제제는 Remington's Pharmaceutical Sciences (19th ed., 1995)에 상세히 기재되어 있다.
본 발명의 약제학적 조성물은 다양한 투약 루트를 통해 투여될 수 있으며, 구체적으로는 비경구로 투여될 수 있고, 보다 구체적으로는 경구, 정맥, 동맥, 피하, 복강, 피내, 근육내, 뇌실내, 척추강내, 흡입, 비강, 관절강내 또는 국소 투여될 수 있다.
본 발명의 약제학적 조성물의 적합한 투여량은 제제화 방법, 투여방식, 환자의 연령, 체중, 성, 병적 상태, 음식, 투여 시간, 투여 경로, 배설 속도 및 반응 감응성과 같은 요인들에 의해 다양하게 처방될 수 있다. 본 발명의 약제학적 조성물의 바람직한 투여량은 성인 기준으로 0.0001-100 ㎎/kg 범위 내이다.
본 발명의 약제학적 조성물은 당해 발명이 속하는 기술분야에서 통상의 지식을 가진 자가 용이하게 실시할 수 있는 방법에 따라, 약제학적으로 허용되는 담체 및/또는 부형제를 이용하여 제제화함으로써 단위 용량 형태로 제조되거나 또는 다용량 용기 내에 내입시켜 제조될 수 있다. 이때 제형은 오일 또는 수성 매질중의 용액, 현탁액, 시럽제 또는 유화액 형태이거나 엑스제, 산제, 분말제, 과립제, 정제 또는 캅셀제 형태일 수도 있으며, 분산제 또는 안정화제를 추가적으로 포함할 수 있다.
본 발명의 또 다른 양태에 따르면, 본 발명은 상술한 본 발명의 화학식 1 화합물 또는 이의 약제학적으로 허용되는 염을 유효성분으로 포함하는, 하기 화학식 3으로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염과의 병용 투여용 항암 조성물을 제공한다:

본 발명에서 이용되는 화학식 1 화합물, 화학식 3 화합물 및 이를 통해 예방 또는 치료될 수 있는 암종에 대해서는 이미 상술하였으므로, 과도한 중복을 피하기 위해 그 기재를 생략한다.
상술한 바와 같이, 본 발명의 화합물은 레날리도마이드와 병용 투여시 현저한 상승효과(synergistic effect)를 나타내었으므로, 항암 효과를 극대화하고 환자의 생존률을 개선시키기 위해 본 발명의 화합물을 레날리도마이드를 비롯한 프탈이미드계 화합물과 병용투여할 수 있다.
병용투여는 본 발명의 화학식 1 화합물과 화학식 3 화합물이 모두 포함된 단일 제형을 투여함으로써 수행되거나, 또는 각 화합물이 개별적으로 포함된 별개의 제형이 동시에 투여되거나 또는 임의의 순서로 적절한 시간 차를 두고 순차적으로 투여됨으로써 수행될 수 있다.
본 발명의 또 다른 양태에 따르면, 본 발명은 하기 화학식 1로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을 대상체에 투여하는 단계를 포함하는 암의 예방 또는 치료 방법을 제공한다:

본 발명에서 이용되는 화학식 1 화합물 및 이를 통해 예방 또는 치료될 수 있는 암에 대해서는 이미 상술하였으므로, 과도한 중복을 피하기 위해 그 기재를 생략한다.
본 발명의 구체적인 구현예에 따르면, 본 발명의 방법은 하기 화학식 3으로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을 대상체에 투여하는 단계를 추가적으로 포함한다:

본 발명의 특징 및 이점을 요약하면 다음과 같다:
(a) 본 발명은 신규한 사환구조 트리테르펜 화합물 및 이를 유효성분으로 포함하는 다발성골수종의 예방 또는 치료용 조성물을 제공한다.
(b) 본 발명의 화합물은 다양한 다발성골수종 세포주에 대해 현저한 사멸효과를 보이면서도, 천연 화합물인 진세노사이드를 출발 물질로 합성된 트리테르펜 유도체로서 정상조혈모세포에 대한 세포독성이 전혀 나타나지 않아 부작용 없는 장기 투여가 가능하다.
(c) 아울러, 본 발명의 화합물은 면역조절제인 탈리도마이드나 이의 유사체인 레날리도마이드와 병용 투여시 현저한 상승 효과를 나타내어 난치성 질환인 다발성골수종에 대한 효율적인 치료제 또는 치료 보조제 조성물로 유용하게 이용될 수 있다.
도 1은 본 발명에서 발굴된 저분자 화합물인 KBB-N2의 분광학 데이터를 보여주는 그림이다.
도 2는 KBB-N2의 화학적 특성 분석 결과이다.
도 3는 각 농도 별 KBB-N2에 의한 다발성골수종 세포주인 RPMI 8226(도 3a) 및 IM9(도 3b)의 사멸 효과를 측정한 결과이다.
도 4는 다발성골수종 세포주인 U266(도 4a, 4c), RPMI 8226(도 4b, 4d) 및 IM9(도 4e)에 대해 KBB-N2의 유도체 화합물인 KBB-N2-1, KBB-N2-4, KBB-N2-5 및 KBB-N2-6에 의한 사멸 효과를 측정한 결과이다.
도 5는 정상 조혈줄기세포에 대한 KBB-N2의 독성을 평가한 결과를 보여주는 그림으로 각각 정상조혈모세포 무독성 확인(도 5a) 및 각 약물 농도에 따른 세포 생존률(도 5b)을 나타낸다.
도 6은 KBB-N2와 레날리도마이드의 병용투여 효과를 확인한 그림으로, 레날리도마이드의 단독 투여에 의한 다발골수종세포의 사멸율(도 6a), 각 조합에 의한 다발골수종세포의 사멸율(도 6b 및 6c) 및 각 조합에 의한 세포고사(apoptosis)(도 6d 및 6e)를 보여준다.
도 7은 본 발명에서 적용된 동물 실험 과정의 모식도를 보여주는 그림이다.
도 8은 루시퍼라제 유전자, GFP(green fluorescent protein) 및 퓨로마이신 내성 유전자가 삽입된 RPMI 8226 세포주(#Cat.SC060-LG)(GenTarget Inc.; San Diego, CA, USA)를 복합 면역 결핍 마우스의 꼬리정맥에 주입하여 구축된 다발골수종 마우스 모델에 꼬리정맥으로 KBB-N2를 투여한 결과를 보여주는 그림으로, 대조군, KBB-N2 투여군 및 레날리도마이드 투여군에 대한 생체발광 이미징(bioluminescence imaging) 결과(도 8a) 및 생체발광 강도, 체중 및 생존률 변화(도 8b) 및 면역조직화학염색법을 이용하여 KBB-N2를 투여한 다발성골수종 마우스 모델을 이용한 골수조직검사 결과(도 8c)를 각각 보여준다.
도 9는 KBB-N2 50μM 투여군과 미투여군 간 상이하게 발현되는 유전자에 대한 열지도(도 9a) 및 유전자 목록(도 9b)을 각각 보여주는 그림이다.
도 10은 KBB-N2의 다발골수종세포 사멸기전은 보여주는 모식도이다.
도 11은 KBB-N2와 시스플라틴의 단독 투여에 의한 다발골수종세포의 사멸율를 보여준다.
이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로, 본 발명의 요지에 따라 본 발명의 범위가 이들 실시예에 의해 제한되지 않는다는 것은 당업계에서 통상의 지식을 가진 자에 있어서 자명할 것이다.
실시예
KBB-N2 화합물의 합성
PPD(100mg, 0.22mmol)의 N,N-다이메틸폼아마이드 용액에 60% NaH(15mg, 0.65mmol)과 BnBr(0.03mL, 0.26mmol)을 0℃에서 첨가하였다. 그 후, 혼합물을 실온에서 2시간 동안 교반하였다. 염화암모늄 포화 수용액을 첨가하여 반응을 종결하고, 생성된 혼합물을 EA로 3회 추출합니다. 얻은 유기층을, 물 및 염수로 세척하고, 황산나트륨 상에서 건조하였다. 용매를 진공하에 증발시켜 잔류물을 얻었고, 이것을 에틸아세테이트/헥산을 사용하는 실리카젤 컬럼 크로마토그래피로 정제하여 순수한 화합물(112mg, 93.7 %)을 얻었다.
세포주 및 세포 배양
인간 다발골수종 세포주인 U266(ATCC CCL-155), IM-9(ATCC CCL-159)과 RPMI 8226(ATCC TBI-196) 세포주를 ATCC(American Type Culture Collection, Rockville, MD, USA)에서 구입하였다. 세포는 RPMI1640 배양액(Gibco Laboratories)에 10% 열 불활성화 우태아혈청(Gibco Laboratories), 1% 페니실린/스트렙토마이신(Thermo Fisher Scientific)를 첨가하여 사용하였다. 세포는 5% CO2, 37℃ 조건의 항온배양기에서 배양시켰으며 3-4일 간격으로 계대 배양하였다.
세포생존율 분석
KBB-N2는 98% 이상의 순도를 가지며 청두 바이오퓨리피토케미칼(Chengdu Biopurity Phytochemicals Ltd. Korea, Seoul, Korea)로부터 구입하였다. KBB-N2는 Dimethyl Sulfoxide(DMSO)에 100 mM의 stock으로 만들어 -20℃에 보관하여 배지에 희석하여 사용하였다. 다발골수종 세포주인 U266, IM-9와 RPMI 8226에 50 μM 동일한 농도의 KBB-N2를 처리하여 100 μL의 배양액에 부유하여 96-well plate(5x105 cells/well)에 분주한 뒤 24시간 동안 배양하였다. 그 이후 Cell viability Assay Kit(EZ-CyTox, Daeil Lab Service CO., Seoul, Korea)를 이용하여 배양배지에 EZ-CyTox 10 μL 를 섞어 1시간 동안 5% CO2, 37℃ 조건의 암실에서 반응시켰다.
EZ-CYTOX는 세포의 Dehydrogenase와 반응 하여 주황색의 수용성 formazan을 생성함으로써 450 nm의 흡광도에서 VERSA Max Micro plate reader(Molecular Devices, Sunnyvale, CA, USA)를 이용하여 측정하였다. 진세노사이드 처리한 세포에서 처리되지 않은 세포에 대한 생존세포 백분율 계산식은 다음과 같다.

세포사멸분석
KBB-N2를 48시간 처리한 다발성골수종 세포 IM-9, RPMI 8226(3x106 cells/mL)를 1 x PBS(phosphate-buffered saline, pH7.4)로 세척하고, 70% 에탄올로 고정시켰다. 다시 1 x PBS로 수차례 수세 후 세포가 부유된 1 x PBS 500 μL(1 x 106 cells/mL)에 Annexin V-FITC 5 μL와 PI(propidium iodine) 5μL (Sigma-Aldrich, St. Louis, MO) 첨가 후 실온 암실에서 30분간 염색하였다. 이후 1X 결합 완충액 500 μL를 추가하여 FACSCalibur(Becton Dickinson, Franklin Lakes, NJ, USA)로 유세포 검사를 진행 후 CellQuest software(Becton Dickinson) 프로그램을 통해 분석하였다.
다발골수종 마우스 모델을 이용한 KBB-N2의 다발골수종 치료 유효성평가
동일한 인간유래 다발성골수종 세포주인 ARH77, U266 및 RPMI 8226을 마우스에 주입한 이종이식 모델을 이용하여 꼬리 정맥으로 KBB-N2 7mg/Kg/day과 양성대조군으로 현재 임상에서 다발골수종 치료제로 널리 이용되고 있는 레날리도마이드 1mg/Kg/day를 투여하였다. 그 결과, KBB-N2는 다발성골수종에 대래 레날리도마이드와 동등이상의 치료효과를 가짐을 확인하였다. 아울러, 치료 후 골수조직검사에서도 KBB-N2의 다발골수종 치료효과가 추가적으로 확인되었다(도 9).
KBB-N2의 정상조혈모세포에 대한 무독성 확인
정상인 공여자에 G-CSF를 피하주사하여 조혈줄기세포를 가동화(mobilization)함으로써 공여자의 말초혈액에 백혈구분획 분반술(leukapheresis)을 시행하여 말초혈액 조혈줄기세포(peripheral blood stem cell, PBSC) 분획을 추출하였다. 획득된 정상 PBSC 분획과 KBB-N2를 2주간 메틸셀룰로오스 배양을 실시하여 적혈구 전구세포군(erythroid progenitor colony)과 백혈구/혈소판 전구세포군(myeloid/ megakaryocytic progenitor colony) 형성 및 숫자를 계수하여 KBB-N2의 조혈줄기세포 독성을 평가하였다. 그 결과, KBB-N2은 60μM까지 농도를 증가시켜 처리하여도 전구세포군의 수와 형태/형성이 유의하게 억제되거나 이상소견이 관찰되지 않아, 본 발명의 화합물이 정상 조혈줄기세포에 독성을 나타내지 않음을 확인니하였다(도 5).
KBB-N2와 레날리도마이드 병용치료
다발골수종세포주에 레날리도마이드를 단독으로 처리했을 때 보다 KBB-N2와 병용치료했을 때 사멸효과가 증대됨을 확인하였다(도 6).
KBB-N2의 다발성골수종 세포주 사멸기전 조사
KBB-N2의 처리에 따른 세포사멸 기전을 조사하고자 다발성골수종 세포주에서의 세포사멸 관련 유전자 발현 변화를 측정하였다. KBB-N2 50μM를 다발성골수종세포(백만개/mL)에 투여하여 48시간 배양 후 총 RNA를 추출하여 84개의 세포사멸사 관련 유전자의 발현량을 정량적 RT-PCR array(Cat# PAHS-012Z; Qiagen Seoul, Seoul, Korea)를 사용하여 분석하였다. KBB-N2 50μM를 세포에 처리 후 48시간 배양하고 RNAqueous Total RNA Isolation Kit(Thermo Fisher Scientific)를 사용하여 RPMI 8226 세포에서 전체 RNA를 분리하였다. 추출된 RNA에 오염된 지놈 DNA는 RNase-Free DNase set(Qiagen)을 이용하여 제거하였다. 총 RNA(5μg)는 RT2 First Strand Kit(Qiagen)을 사용하여 10μL의 반응 부피의 cDNA로 변환하였다. 그 후, 10μL의 cDNA를 인간 세포사멸 관련 RT2 Profiler PCR Array(Qiagen)의 각 프라이머 세트와 제조사의 프로토콜에 따라 결합시켰다. 증폭 반응은 실시간 중합효소 연쇄반응기인 CFX96(Bio-Rad, Hercules, CA, USA)에서 자동화된 기준점 및 역치주기 검출을 사용하였다. 성장인자 관련 유전자의 상대적 발현은 5개의 내장된 하우스키핑 유전자에 대해 정규화하였다. 각 그룹은 웹 기반 소프트웨어인 GeneGlobe Data Analysis Center(Qiagen)를 이용하여 분석하였다.
분석 결과, KBB-N2 50μM을 다발성골수종 세포주에 투여 후 PCR 어레이를 실시하여 아래와 같은 세포고사 관련 유전자의 발현이 현저히 변화되었음을 확인하였다(도 10):
4배이상 과발현 세포고사 관련 유전자: DAPK1
3배이상 과발현 세포고사 관련 유전자: BCL2, CAS7
2배이상 과발현 세포고사 관련 유전자: APAF1, BAD, BCL2L1, CASP2, CASP3, FADD, HRK, IGF1R, LTBR, NAIP, NOD1, NOL3, TNF, TNFRSF10A, TNFRSF10B, TNFRF1A, TNFRSF1B, TNFRSF25, TP53, TRADD, XIAP
이상으로 본 발명의 특정한 부분을 상세히 기술하였는 바, 당업계의 통상의 지식을 가진 자에게 있어서 이러한 구체적인 기술은 단지 바람직한 구현예일 뿐이며, 이에 본 발명의 범위가 제한되는 것이 아닌 점은 명백하다. 따라서, 본 발명의 실질적인 범위는 첨부된 청구항과 그의 등가물에 의하여 정의된다고 할 것이다.

- 제 1 항에 있어서, 상기 암은 다발성골수종(Multiple myeloma)인 것을 특징으로 하는 조성물.
