Abstract
Ein Implantat (1), insbesondere Gelenk-Implantat, weist ultrahochmolekulares Polyethylen als Grundwerkstoff und eine hierauf befindliche Beschichtung (10) zum Oxidations- und/oder Verschleißschutz auf, wobei die Beschichtung (10) eine Biozid-Schicht sowie eine diese abdeckende Sperrschicht umfasst. Im Vergleich zum unbeschichteten Grundwerkstoff verringert die Beschichtung sowohl den Zutritt von Sauerstoff zum Grundwerkstoff als auch jeglichen Stoffaustritt aus dem Grundwerkstoff.
Description
Implantat und Verfahren zur Beschichtung eines Implantats
Die Erfindung betrifft ein medizintechnisches Implantat sowie ein Verfahren zur Beschichtung eines Implantats.
Die EP 3 035 891 Bl offenbart ein anatomisch angepasstes orthopädisches Implantat. Es handelt sich hierbei um ein Tibialimplantat zur Implantation auf einem proximalen Schienbein eines Kniegelenks eines Patienten. Das Implantat weist eine Kobalt-Chrom-Legierung als Grundwerkstoff auf. Für ein Gelenkelement werden biokompatible Kunststoffe wie hochmolekulares Polyethylen (HMWPE), ultrahochmolekulares Polyethylen (UHMWPE) oder Polyether Ether Keton (PEEK) vorgeschlagen.
Die DE 20 2005 005 405 Ul offenbart ein Implantat zum Einsetzen in einen menschlichen oder tierischen Körper, welches eine Knochenanlagefläche aufweist, die mindestens teilweise mit einer osteointegrativen Schicht bedeckt ist, wobei zwischen der osteointegrativen Schicht und der Knochenanlagefläche des Grundkörpers eine Zwischenschicht vorgesehen ist. Der Grundkörper nach der DE 20 2005 005 405 Ul kann beispielsweise aus einer Keramik oder aus einem Polymer, insbesondere PEEK oder UHMWPE, hergestellt sein. Bei der osteointegrativen Schicht kann es sich um eine poröse Reintitanschicht handeln. Was die Zwischenschicht betrifft, wird vorgeschlagen, diese durch Kaltgasspritzen aufzubringen.
Aus der EP 1 790 224 Bl ist ein antimikrobielles Schichtmaterial bekannt, welches mehrschichtig, nämlich aus einer Biozid-Schicht und einer diese bedeckenden Transportkontrollschicht aufgebaut ist. Das antimikrobielle Schichtmaterial ist unter anderem zur Beschichtung eines Katheters, einer Kontaktlinse, eines Implantats, eines medizinischen Nagels oder eines Dentalimplantats geeignet.
Was den Werkstoff UHMWPE betrifft, wird ferner auf folgende Dissertation hingewiesen:
„Beurteilung von vernetztem UHMWPE hinsichtlich seiner Eignung als Implantatwerkstoff für Hüftgelenkschalen", Dipl.-Ing. Ingo John, TU Berlin, 23.10.2003
In der Dissertation wird insbesondere auf Möglichkeiten der FTIR-Analyse (Fourier-Transform Infrarot-Spektroskopie) eingegangen. Ferner werden in der Dissertation Untersuchungen erläutert, die den Oxidationsindex von gammabestrahltem UHMWPE betreffen.
Der Erfindung liegt die Aufgabe zu Grunde, medizintechnische Implantate, welche UHMWPE aufweisen, gegenüber dem genannten Stand der Technik insbesondere unter dem Aspekt der Beständigkeit weiterzuentwickeln.
Diese Aufgabe wird erfindungsgemäß gelöst durch ein Implantat mit den Merkmalen des Anspruchs 1. Ebenso wird die Aufgabe gelöst durch ein Verfahren zur Beschichtung eines Implantats gemäß Anspruch 7. Im Folgenden im Zusammenhang mit dem Beschichtungsverfahren erläuterte Ausgestaltungen und Vorteile der Erfindung gelten sinngemäß auch für die Vorrichtung, das heißt das medizintechnische Implantat, und umgekehrt.
Das Implantat weist ultrahochmolekulares Polyethylen als Grundwerkstoff und eine hierauf befindliche Beschichtung auf, welche eine Biozid-Schicht sowie eine diese abdeckende, nicht notwendigerweise absolut undurchlässige Sperrschicht umfasst. Die Beschichtung ist dazu ausgebildet, im Vergleich mit dem unbeschichteten Grundwerkstoff sowohl den Zutritt von korrosionsfördernd, das heißt degradationsfördernd, wirkenden Stoffen, insbesondere Sauerstoff, zum Grundwerkstoff zumindest zu hemmen, als auch den Stoffaustritt aus dem Grundwerkstoff zu unterbinden oder zumindest zu verringern. Bei dem genannten Stoffaustritt könnte es sich beispielsweise um den Austritt von Bestandteilen des Grundwerkstoffs selbst oder um die Freisetzung von Abbauprodukten handeln.
Als Beschichtung des Implantats ist insbesondere ein antimikrobielles Schichtmaterial nach der bereits erwähnten Patentschrift EP 1 790 224 Bl geeignet. Im Rahmen des Beschichtungsverfahrens wird allgemein eine mehrlagige, aus einer Biozid-Schicht sowie einer
Sperrschicht aufgebaute Schicht auf ultrahochmolekulares Polyethylen, welches eine mechanische Funktion erfüllt, aufgebracht.
Das ultrahochmolekulare Polyethylen (UHMWPE) kann durch Beta- oder Gammastrahlung strahlenvernetzt werden, wobei die Strahlenvernetzung entweder vor oder nach dem Aufbringen der mehrlagigen antimikrobiellen Schicht erfolgen kann. Das Aufbringen der Beschichtung vor der Beta- oder Gammabestrahlung hat den Vorteil, dass während der Bestrahlung bereits ein Schutz des Grundwerkstoffs vor Sauerstoff gegeben ist. Somit sind nachteilige Auswirkungen von Radikalen, die durch die Bestrahlung entstehen und die Werkstückoberfläche angreifen könnten, vollständig oder weitestgehend unterbunden. Insbesondere kann die Notwendigkeit, die Strahlenvernetzung zum Schutz des Grundwerkstoffs in Stickstoffatmosphäre durchzuführen, entfallen.
Das als Grundwerkstoff des Implantats dienende, strahlenvernetzte UHMWPE kann beispielsweise ein Molekulargewicht im Bereich von 3 Millionen bis 7 Millionen g / mol, ein Molekulargewicht im Bereich von 7 Millionen bis 10 Millionen g / mol, oder auch ein Molekulargewicht von mehr als 10 Millionen g / mol aufweisen.
In vereinfachter Gestaltung entfällt eine gesonderte biozidhaltige Schicht. Das Grundmaterial, das heißt UHMWPE, ist in diesem Fall insbesondere durch eine reine Polysiloxanschicht abgedeckt. Die Polysiloxanschicht verringert den Zutritt von Sauerstoff oder schließt ihn - eine ausreichende Dicke vorausgesetzt - komplett aus, was in jedem Fall die Lebensdauer des Produkts deutlich steigert. Statt einer Polysiloxanschicht kann auch eine andere sauerstoffundurchlässige Schicht, optional ergänzt durch eine Biozid-Schicht, auf das UHMWPE aufgebracht werden.
Die Beschichtung des Implantats schützt den Grundwerkstoff insbesondere vor Oxidation. Gleichzeitig kann durch die Sperrschicht hindurch biozider Wirkstoff abgegeben werden. Der biozide Wirkstoff ist beispielsweise ausgewählt aus der Gruppe bestehend aus Silber, Kupfer und Zink, deren Ionen und deren Metallkomplexe, oder einer Mischung oder Legierung umfassend zwei oder mehr dieser Elemente.
Ein besonderer Vorteil der Biozid-Schicht liegt darin, dass sie die Bildung eines Biofilms auf der Oberfläche des Implantats verhindert. Insgesamt stellt die Beschichtung des Implantats eine hochwirksame Barriere sowohl nach innen als auch nach außen dar. Unter anderem wird durch die Beschichtung eine Versprödung des ultrahochmolekularen Polyethylens verhindert. Letztlich wird durch die Beschichtung die Lebensdauer des Implantats drastisch erhöht.
Dicke und Porosität der Sperrschicht sind vorzugsweise derart eingestellt, dass der biozide Wirkstoff aus der Biozid-Schicht durch die Sperrschicht hindurch in einer antimikrobiellen, jedoch nicht zytotoxischen Menge abgegeben wird. Der biozide Wirkstoff hat beispielsweise eine mittlere Korngröße von 5 bis 100 nm, wobei die mittlere Dicke der Biozid-Schicht 5 bis 100 nm und die mittlere Dicke der Sperrschicht 5 bis 500 nm beträgt.
Das Grundmaterial der Sperrschicht kann beispielsweise aus der Gruppe ausgewählt sein, die ein organisches Grundmaterial, insbesondere ein Plasmapolymer, ein Sol-Gel, einen Lack und ein silikonisiertes Grundmaterial umfasst. Alternativ kann ein anorganisches Grundmaterial der Sperrschicht zum Beispiel aus der Gruppe ausgewählt sein, die SiCh, SiC, ein Metalloxid, insbesondere TiCh AI2O3, und ein nichtbiozides Metall, insbesondere Titan und medizinischen Edelstahl, umfasst.
Die mehrlagige Beschichtung deckt den Grundwerkstoff nur teilweise ab. Hierbei deckt die mehrlagige Beschichtung den Grundwerkstoff, das heißt das UHMWPE, in einem außerhalb einer Gleitfläche des Implantats liegenden Oberflächenabschnitt ab, wogegen die Gleitfläche desselben Implantats durch unbeschichtetes UHMWPE oder durch in anderer Weise beschichtetes UHMWPE gebildet ist. Neben Artikulationsflächen können beispielsweise auch Konusverbindungen des Implantats unbeschichtet bleiben.
Bei dem Implantat kann es sich insbesondere um ein Gelenkimplantat, beispielsweise um eine Hüft- oder Kniegelenkendoprothese oder um eine Komponente für Ellenbogen, Sprunggelenke oder Schultergelenke, handeln. Im Übrigen ist die mehrlagige Beschichtung,
welche aus einer Biozid-Schicht und einer Sperrschicht aufgebaut ist, zum Beispiel für chirurgisches Nahtmaterial oder für Bandersatz, insbesondere Kreuzband, verwendbar.
In jedem Fall sind durch die Beschichtung des Implantats Vorteile hinsichtlich des Oxidations- Index und/oder des Verschleißes erzielbar. Von Bedeutung ist auch die einstellbare Hydrophilie der Biozid-Schicht. Insbesondere kann die Beschichtung im Vergleich zum hydrophoben UHMWPE zu einer besseren Benetzung mit Wasser oder Gelenkflüssigkeit führen, was das Verschleißverhalten verbessert.
Es wird von der Überlegung ausgegangen, dass eine Strahlenvernetzung im Allgemeinen zwar die Verschleißfestigkeit von Polyethylen verbessert, zugleich aber Nachteile hinsichtlich der Oxidation, das heißt Alterung, des Materials mit sich bringt. Dieser Zielkonflikt wird durch die auf dem strahlenvernetzten Material befindliche Beschichtung aufgehoben oder zumindest gemindert. Die Verbesserungen, welche hinsichtlich der Oxidation im Vergleich zu unbeschichtetem UHMWPE erzielt werden, sind bis zu einer Tiefe von mehr als 1 mm unter der Oberfläche des Grundwerkstoffs feststellbar.
Die Beschichtung wird in typischer Verfahrensführung bei einer Temperatur von ca. 40°C aufgebracht, womit jegliche thermische Schädigung des Grundmaterials zuverlässig vermieden wird. Die Schichtdicke beträgt beispielsweise 90 nm. Je nach Verfahrensparametern kann die Beschichtung transparent oder farbig gefertigt werden. In jedem Fall kann durch den erzielten Oxidationsschutz an der Oberfläche des Implantats, insbesondere Prothesenoberfläche, das Shelf Life des Implantats erhöht werden. Darüber hinaus können bei der Fertigung und Logistik Vorteile bereits dadurch erzielt werden, dass im Unterschied zu herkömmlichen, für entsprechende Verwendungszwecke vorgesehenen Produkten von einer Verpackung unter Schutzgas abgesehen wird.
Soweit bei dem beschichteten Implantat, dessen Beschichtung unter anderem eine Abschirmung gegenüber dem Sauerstoff der Luft darstellt, noch eine Oxidation auftritt, ist sie im Vergleich zu herkömmlichen, unbeschichteten UHMWPE-Implantaten drastisch verzögert, wodurch die Lebensdauer des Implantats wesentlich heraufgesetzt ist.
Nachfolgend wird ein Ausführungsbeispiel der Erfindung anhand einer Zeichnung näher erläutert. Hierin zeigen:
Fig. 1 eine beschichtete Komponente eines Implantats in perspektivischer Ansicht,
Fig. 2 weitere Implantatkomponenten in einer Schnittdarstellung,
Fig. 3 in einem Diagramm den tiefenabhängigen Oxidationsindex eines Grundwerkstoffs des Implantats im Vergleich mit einem nicht beanspruchten Vergleichsprodukt.
Ein insgesamt mit dem Bezugszeichen 1 gekennzeichnetes Implantat ist als Inlay eines künstlichen Gelenks ausgebildet. Das Implantat 1 weist eine modifizierte halbkugelige Grundform auf, wobei sich an einen sphärischen Oberflächenabschnitt 2 ein kegelstumpfförmiger Oberflächenabschnitt 3 anschließt. In der Mitte des sphärischen Oberflächenabschnitts 2, in der Anordnung nach Figur 1 an der obersten Stelle des Implantats 1, befindet sich eine Scheibe 4, welche konzentrisch zur Mittelachse des insgesamt rotationssymmetrischen Implantats 1 angeordnet ist. Die mit 5 bezeichnete Stirnfläche der Scheibe 4 ist mit einer Beschichtung 10 versehen, welchem dem Schutz vor Verschleiß sowie Oxidation dient.
Die Beschichtung 10, welche sich partiell auf der Außenseite des Implantats 1 befindet, ist aus einer Biozid-Schicht und einer die Biozid-Schicht abdeckenden Sperrschicht aufgebaut und direkt auf den Grundwerkstoff des Implantats 1 aufgebracht. Bei dem Grundwerkstoff des Implantats 1 handelt es sich um ultrahochmolekulares Polyethylen, welche mit Hilfe von Betaoder Gammastrahlung strahlenvernetzt wurde.
Das Implantat 1 ist dazu vorgesehen, in ein äußeres Teil 6 eingesetzt zu werden, welches in Figur 2 sichtbar ist. Das im Gegensatz zum Implantat 1 aus einem metallischen Werkstoff gefertigte äußere Teil 6 weist auf seiner Innenseite einen konkaven Oberflächenabschnitt 7 und einen hieran anschließenden konischen Abschnitt 8 auf. Die Geometrie der Abschnitte 7,
8 ist der Form des sphärischen Oberflächenabschnitts 2 beziehungsweise des kegelstumpfförmigen Oberflächenabschnitts 3 angepasst. An der Außenoberfläche des Teils 6 ist im Bereich des konischen Abschnitts 8 eine Verzahnungsstruktur 9 erkennbar. Mittig auf der Innenseite des Teils 6 befindet sich eine zentrale Ausnehmung 11, die dem formschlüssigen Einsetzen der Scheibe 4 dient. Eine im Vergleich zur Ausnehmung 11 kleinere, konkav gewölbte Ausnehmung auf der Außenseite des Teils 6, welche ebenfalls konzentrisch zur Mittelachse des Implantats 1 sowie des Teils 6 ausgebildet ist, ist mit 12 bezeichnet. In Figur 2 ist ferner ein mit dem Implantat 1 zusammenwirkendes inneres Teil 13 zu sehen, welches einen kugeligen Abschnitt 14 und einen hieran anschließenden Flanschabschnitt 15 aufweist.
Im Diagramm nach Figur 3 zeigt eine Messkurve MK den Zusammenhang zwischen der Tiefe x (in Millimeter) unter der Oberfläche des Grundwerkstoffs des Implantats 1 und dem gemessenen Oxidationsindex OXI. Die Messungen wurden an Proben durchgeführt, die zwei Wochen künstlich gealtert wurden. Die Proben waren, wie im Zusammenhang mit Figur 1 bereits beschrieben, mehrlagig beschichtet.
Zusätzlich ist im Diagramm nach Figur 3 eine Vergleichskurve VK aufgetragen, die sich auf unbeschichtete Proben bezieht, wobei - abgesehen von der Beschichtung - die Vorbereitung der Proben sowie die Durchführung der Versuche genauso erfolgte, wie im Fall der Messkurve MK. Wie aus Figur 3 deutlich hervorgeht, sind im gesamten Messbereich mit den beschichteten Proben deutlich bessere Ergebnisse erzielt worden als mit den unbeschichteten Vergleichsproben, was sich darin ausdrückt, dass die Messkurve MK signifikant unterhalb der Vergleichskurve VK liegt. Somit bietet die Beschichtung 10 einen wirksamen Schutz gegen Oxidation des Grundwerkstoffs, das heißt ultrahochmolekularen Polyethylens.

Patentansprüche Implantat (1), mit ultrahochmolekularem Polyethylen als Grundwerkstoff und einer hierauf befindlichen Beschichtung (10), welche eine Biozid-Schicht sowie eine diese abdeckende Sperrschicht umfasst und, verglichen mit dem unbeschichteten Grundwerkstoff, zur Verringerung sowohl des Zutritts von Sauerstoff zum Grundwerkstoff als auch des Stoffaustritts aus dem Grundwerkstoff ausgebildet ist, wobei die Beschichtung (10) den Grundwerkstoff nur teilweise, nämlich in einem außerhalb einer Gleitfläche des Implantats (1) liegenden Oberflächenabschnitt, abdeckt. Implantat (1) nach Anspruch 1, dadurch gekennzeichnet, dass die Biozid-Schicht einen bioziden Wirkstoff umfasst, welcher ausgewählt ist aus der Gruppe bestehend aus Silber, Kupfer und Zink, deren Ionen und deren Metallkomplexe, oder einer Mischung oder Legierung umfassend zwei oder mehr dieser Elemente. Implantat (1) nach Anspruch 2, dadurch gekennzeichnet, dass die Sperrschicht eine auf eine Abgabe des bioziden Wirkstoffs aus der Biozid-Schicht durch die Sperrschicht hindurch in einer antimikrobiellen nicht zytotoxischen Menge eingestellte Dicke und Porosität aufweist. Implantat (1) nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass der biozide Wirkstoff eine mittlere Korngröße von 5 bis 100 nm hat, die mittlere Dicke der Biozid-Schicht 5 bis 100 nm beträgt, und die mittlere Dicke der Sperrschicht 5 bis 500 nm beträgt. Implantat (1) nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass die Sperrschicht ein Grundmaterial besitzt, welches aus der Gruppe ausgewählt ist, die ein organisches Grundmaterial, insbesondere ein Plasmapolymer, ein Sol- Gel, einen Lack und ein silikonisiertes Grundmaterial umfasst. Implantat (1) nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, dass die Sperrschicht ein anorganisches Grundmaterial besitzt, welches aus der Gruppe ausgewählt ist, die SiC>2, SiC, ein Metalloxid, insbesondere TiCh AI2O3, und
ein nichtbiozides Metall, insbesondere Titan und medizinischen Edelstahl, umfasst. Verfahren zur Beschichtung eines Implantats (1), wobei auf ultrahochmolekulares Polyethylen eine mehrlagige, aus einer Biozid-Schicht sowie einer Sperrschicht aufgebaute Beschichtung (10) derart aufgebracht wird, dass die Beschichtung (10) den Grundwerkstoff nur teilweise, nämlich in einem außerhalb einer Gleitfläche des Implantats (1) liegenden Oberflächenabschnitt, abdeckt. Verfahren nach Anspruch?, dadurch gekennzeichnet, dass als Sperrschicht eine sauerstoffundurchlässige Schicht aufgebracht wird. Verfahren nach Anspruch 7 oder8, dadurch gekennzeichnet, dass das Polyethylen durch Beta- oder Gammastrahlung strahlenvernetzt wird. Verfahren nach Anspruch 9, dadurch gekennzeichnet, dass das Polyethylen vor dem Aufbringen der mehrlagigen Beschichtung (10) strahlenvernetzt wird. Verfahren nach Anspruch 9, dadurch gekennzeichnet, dass das Polyethylen nach dem Aufbringen der mehrlagigen Beschichtung (10) strahlenvernetzt wird.
