Abstract
本発明は、低酸素誘導因子プロリン水酸化酵素(HIF-PH)の発現を抑制する又は活性を阻害する物質を含有する抗HBV剤;HBV感染に起因する疾患の治療又は予防のための医薬組成物;前記物質とインターフェロン製剤とを組み合わせてなるHBV感染に起因する疾患の治療又は予防のための医薬;HBV感染者のインターフェロン製剤への応答性を改善するための剤;並びにHIF-PHの発現を抑制する活性又は酵素活性を阻害する活性を指標とした、被験物質の抗HBV活性、又はインターフェロン製剤への応答性改善活性を評価する方法を提供する。
Description
本発明は、低酸素誘導因子プロリン水酸化酵素(hypoxia-inducible factor-prolyl hydroxylase、HIF-PH)の発現を抑制する又は活性を阻害する物質を含有する抗B型肝炎ウイルス剤、及びその使用に関する。
B型肝炎ウイルス(HBV)は、ヘパドナウイルス(Hepadnaviridae)科に属する不完全2本鎖DNAを有するエンベロープウイルスであり、急性及び慢性の肝炎(B型肝炎)を誘発する他、肝がんの原因ともなる。全世界では、2~4億人の感染者が存在し、年間100万人がHBVと関連した肝疾患により死亡しているとされている。
HBVが肝細胞内に侵入すると、エンベロープからヌクレオチドキャプシドが放出され、細胞核に移行する。感染肝細胞の核内に移行したHBVゲノムはcccDNA(covalently closed circular DNA)に変換され、その形で長期にわたって存在し続ける。cccDNAは一過性の不顕性感染や既往感染者のウイルス複製源となり、再活性化の原因となる。また、HBVの複製は、宿主細胞のRNA polymeraseを利用して合成されるpregenome RNAを鋳型としたウイルスのDNA polymeraseの逆転写により行われるため、通常のDNAウイルスと比較して変異が生じ易く、治療耐性株の発生頻度が高い。
現在知られているB型肝炎の治療薬は、エンテカビルやテノホビル等の核酸アナログ製剤と、インターフェロン(IFN)製剤とにほぼ限定されている。核酸アナログ製剤は、HBVのDNA polymeraseを特異的に阻害することによりpregenome RNAからDNAへの逆転写を抑制する。しかしながら、cccDNA抑制効果が限定的である、長期の投与が必要である、適切な中止基準がない、耐性ウイルスが出現し易い、腎障害が発生するといった問題を有する。
またIFN製剤は、cccDNAの抑制効果及び長期でのHBs抗原の低下作用を示すこと、処置期間が一定であることといった利点があるが、IFNが著効を示す患者が少ないこと、すなわちIFN不応答が問題とされている。IFN不応性の原因について、HBVのProtein X(HBx)がprotein phosphatase 2A suppressor(PP2Ac)及びsuppressor of cytokine signaling 3(SOCS3)の発現上昇を介してIFN不応性に関与することが報告されている(非特許文献1)。また、HBVポリメラーゼやHBVコアタンパク質がIFNのシグナル伝達を阻害することが報告されている(非特許文献2、3)。
従来の核酸アナログ製剤及びIFN製剤には上記のような問題があることから、これらとは異なる作用機序に基づく抗B型肝炎ウイルス剤(抗HBV剤)が求められている。

本発明は、従来の抗HBV剤にはない新たな特徴を有する抗HBV剤の提供を目的とする。
本発明者らは、HIF-PHの発現を抑制する又は活性を阻害する物質が、IFN応答性を改善し得ることを見いだし、以下の発明を完成させた。
項1. 低酸素誘導因子プロリン水酸化酵素(HIF-PH)の発現を抑制する又は活性を阻害する物質を含有する、抗B型肝炎ウイルス剤。
項2. 前記物質が、ダプロデュスタット、エナロデュスタット、モリデュスタット、ロキサデュスタット及びバダデュスタット並びに薬学的に許容されるそれらの塩よりなる群から選択される少なくとも1つの化合物である、項1に記載の剤。
項3. HIF-PHの発現を抑制する又は活性を阻害する物質を含有する、B型肝炎ウイルスの感染に起因する疾患の治療又は予防のための医薬組成物。
項4. B型肝炎ウイルスの感染に起因する疾患が、肝炎、肝硬変又は肝臓がんである、項3に記載の医薬組成物。
項5. インターフェロン製剤での前治療歴があり、当該治療が奏功しなかった対象に対して用いるための、項3又は4に記載の医薬組成物。

本発明によれば、新たな抗HBV剤及び医薬組成物が提供され、これによって特にIFN不応性の患者においてHBVの感染に起因する疾患を治療又は予防することが可能になる。
以下に示す本発明の説明は、代表的な実施形態又は具体例に基づくことがあるが、本発明はそのような実施形態又は具体例に限定されるものではない。また、本発明に関連して言及するタンパク質及び遺伝子に関する説明は、特に断らない限り、ヒト由来のタンパク質及び遺伝子を例として行うが、本発明において利用されるタンパク質及び遺伝子はヒト由来のものに限定されず、また本発明の適用対象はヒトに限定されない。
HIF-PH
HIF-PHは、低酸素誘導因子(hypoxia-inducible factor;HIF)のプロリン残基をヒドロキシ化する酵素である。HIF-PHは酸素濃度を検知する役割を担っており、通常の酸素濃度下ではHIFをヒドロキシル化してユビキチン化とプロテアソームによる分解に至らせる。しかしながら、低酸素濃度下ではHIF-PHの酵素活性は低下し、その結果HIFの分解が抑制されてHIFの存在量は増加する。HIFはエリスロポエチンの産生に関与するタンパク質であることから、低酸素濃度下でのHIF-PH活性低下によるHIF量の増加はエリスロポエチン産生を誘導し、赤血球の造血促進をもたらす。
HIF-PHは、低酸素誘導因子(hypoxia-inducible factor;HIF)のプロリン残基をヒドロキシ化する酵素である。HIF-PHは酸素濃度を検知する役割を担っており、通常の酸素濃度下ではHIFをヒドロキシル化してユビキチン化とプロテアソームによる分解に至らせる。しかしながら、低酸素濃度下ではHIF-PHの酵素活性は低下し、その結果HIFの分解が抑制されてHIFの存在量は増加する。HIFはエリスロポエチンの産生に関与するタンパク質であることから、低酸素濃度下でのHIF-PH活性低下によるHIF量の増加はエリスロポエチン産生を誘導し、赤血球の造血促進をもたらす。

HIF-PHの発現を抑制する物質
本発明において用いられるHIF-PHの発現を抑制する物質は、HIF-PHの遺伝子又はタンパク質レベルでの発現を抑制する能力を有する任意の物質である。HIF-PHの発現を抑制する物質の1つの例は、HIF-PHの発現を抑制する核酸である。HIF-PHの発現を抑制する核酸は、HIF-PH遺伝子からのmRNA(HIF-PH mRNA)の転写を抑制することができる核酸、HIF-PH mRNAを分解することができる核酸、及びHIF-PH mRNAからのタンパク質翻訳を抑制することができる核酸を包含し、これらの例としては、HIF-PH遺伝子の塩基配列又はHIF-PH mRNAの塩基配列を基にして当業者が設計、作製することができる、アンチセンス核酸、又はRNA干渉を引き起こす核酸、例えばsiRNA、shRNA若しくはmiRNAを挙げることができる。

また、適当な発現プロモーターの制御下に置かれることでアンチセンス核酸又はRNA干渉を引き起こす核酸を転写誘導することのできるDNA、及びアンチセンス核酸又はRNA干渉を引き起こす核酸の塩基配列の一部が修飾されてヌクレアーゼによる分解に対する安定性が高められた核酸も、アンチセンス核酸又はRNA干渉を引き起こす核酸と機能的に等価な核酸として、本発明にいうHIF-PHの発現を抑制する核酸に包含される。ヌクレアーゼによる分解に対する安定性を向上させるための修飾としては、2'O-メチル化、2'-F化、4'-チオ化等を挙げることができる。
さらに、HIF-PHの発現を抑制するRNAのリボヌクレオチドの一部が、対応するデオキシリボヌクレオチド又はヌクレオチド類似体に置き換えられたキメラRNAもまた、HIF-PHの発現を抑制する核酸に包含される。ヌクレオチド類似体としては、例えば、5位修飾ウリジン又はシチジン、例えば5-(2-アミノ)プロピルウリジン、5-ブロモウリジン等;8位修飾アデノシン又はグアノシン、例えば8-ブロモグアノシン等;デアザヌクレオチド、例えば7-デアザ-アデノシン等;O-又はN-アルキル化ヌクレオチド、例えばN6-メチルアデノシン等を挙げることができる。修飾される又は置換される塩基の種類又は個数は、ターゲット分子の発現を抑制する能力を失わない限り、特に制限はない。
HIF-PHの発現を抑制する核酸は、遺伝子組み換え技術又は化学合成技術を利用して人工的に合成することができる。遺伝子組み換え方法、核酸の化学合成方法、また非天然型の塩基の合成手法又はこれを含む核酸の合成手法は、当業者に周知である。

本発明において用いられるHIF-PHの活性を阻害する物質は、HIF-PHの酵素活性を阻害する能力を有する任意の物質である。HIF-PHの活性を阻害する物質の1つの例は、HIF-PHに対する特異抗体又はその誘導体である。HIF-PHに対する特異抗体は、非標的タンパク質よりもHIF-PHに優先的に結合する抗体、又はHIF-PHに高い結合親和性を有する抗体と表すこともできる。HIF-PHに対する特異抗体は、非標的タンパク質との結合よりも少なくとも5倍強い、好ましくは少なくとも10倍強い、より好ましくは少なくとも100倍強い、最も好ましくは少なくとも1000倍強い親和性を有してHIF-PHに結合する抗体であり得る。HIF-PHに対する特異抗体はまた、10-7M、好ましくは10-8M、より好ましくは10-9Mの解離定数を有してHIF-PHに結合する抗体であってもよい。

HIF-PHに対する特異抗体は、例えばマウス、ラット、サメ、ウサギ、ブタ、ハムスター、ラクダ、ラマ、ヤギ又はヒトを包含する任意の種を起源とすることができる。また特異抗体は、免疫グロブリン分子の任意のクラス(例としてIgG、IgE、IgM、IgD又はIgA)及びサブクラスのものであることができるが、IgGであることが好ましい。
本発明において、HIF-PHに対する特異抗体はポリクローナル抗体であってもモノクローナル抗体であってもよいが、モノクローナル抗体であることが好ましい。また特異抗体は、キメラ抗体、ヒト化抗体又はヒト抗体であり得る。
HIF-PHに対する特異抗体の誘導体は、HIF-PHに対する特異抗体に由来する、抗原と特異的に結合する能力を保持した抗体の部分断片として定義される抗原結合性断片、例えばFab(fragment of antigen binding)、Fab'、F(ab')2、一本鎖抗体(single chain Fv)、ジスルフィド安定化抗体(disulfide stabilized Fv)及びCDRを含むペプチド等であり得るが、これらには限定されない。
HIF-PHに対する特異抗体及びその誘導体は、当業者に公知の方法を用いて作製することができる。例えば、ヒトHIF-PHのアミノ酸配列又はこれをコードするDNAの塩基配列に基づいて遺伝子組み換え手法によりHIF-PHを作製し、これを抗原として適当な動物に免疫することで、さらに当該動物のB細胞をミエローマ細胞と融合させてハイブリドーマを得ることで作製することができる。ハイブリドーマ法については、例えばMeyaard et al. (1997) Immunity 7:283-290; Wright et al. (2000) Immunity 13:233-242; Kaithamana et al. (1999) J.Immunol. 163:5157-5164等を参照されたい。HIF-PHに対する特異抗体又はその誘導体は、さらに他の薬物と結合した抗体薬物複合体であってもよい。
HIFの活性を阻害する物質の別の例は、HIF-PHの酵素活性を阻害する能力を有する物質、すなわちHIF-PHに対する阻害剤である。HIF-PHに対する阻害剤は、HIFの分解抑制を介して低酸素濃度下と同様にHIF存在量を増加させ、エリスロポエチン産生を誘導する薬剤として腎性貧血の治療に用いられている。HIF-PH阻害剤はHIF安定化剤(HIF stabilizer)とも呼ばれている。
本発明において利用可能なHIF-PH阻害剤は、HIFのプロリン残基に対するヒドロキシ化活性を阻害することができる物質であればよく、その例としては、ダプロデュスタット、エナロデュスタット、モリデュスタット、ロキサデュスタット及びバダデュスタットを挙げることができる。
また、本発明において利用可能なHIF-PH阻害剤のさらなる例として、WO2009/070644に記載されているキノリン-8-カルボキサミド誘導体、WO2009/134750に記載されているベンゾイミダゾール化合物、WO2010/025087に記載されているピリミジンジオンN-置換グリシン誘導体、WO2011/007856に記載されているトリアゾロピリジン化合物、WO2014/030716に記載されているピラゾロピリミジン化合物、WO2015/180585に記載されている置換アセチドラジド誘導体、WO2016/034108に記載されているキノリノン化合物、WO2016/155357に記載されている5-ヒドロキシ-1,7-ナフチリジン化合物、WO2018/205928に開示されているインドリジン誘導体等を挙げることもできるが、これらには限定されるものではない。
HIF-PH阻害剤は、HIF-PHの酵素活性を阻害する能力を有するかぎり、酸付加塩又は塩基付加塩といった薬学的に許容される塩の形態であってもよい。酸付加塩としては、例えば塩酸塩、臭化水素酸塩、硫酸塩、ヨウ化水素酸塩、硝酸塩、リン酸塩等の無機酸塩、クエン酸塩、シュウ酸塩、酢酸塩、ギ酸塩、プロピオン酸塩、安息香酸塩、トリフルオロ酢酸塩、マレイン酸塩、酒石酸塩、メタンスルホン酸塩、ベンゼンスルホン酸塩、パラトルエンスルホン酸塩等の有機酸塩が挙げられる。塩基付加塩としては、ナトリウム塩、カリウム塩、カルシウム塩、マグネシウム塩、アンモニウム塩等の無機塩基塩、トリエチルアンモニウム塩、トリエタノールアンモニウム塩、ピリジニウム塩、ジイソプロピルアンモニウム塩等の有機塩基塩等が挙げられる。さらに、アルギニン、アスパラギン酸、グルタミン酸等の塩基性あるいは酸性アミノ酸といったアミノ酸塩が挙げられる。薬学的に許容される好ましい塩としては、例えば塩酸塩、カリウム塩、ナトリウム塩等が挙げられる。また、HIF-PH阻害剤は、水和物又は溶媒和物の形態であってもよい。
本発明において好ましいHIF-PH阻害剤は、ダプロデュスタット、エナロデュスタット、モリデュスタット、ロキサデュスタット及びバダデュスタット、並びに薬学的に許容されるそれらの塩である。好ましいHIF-PH阻害剤の例を以下に示す。

抗HBV剤
上述のHIF-PHの発現を抑制する物質及びHIF-PHの活性を阻害する物質はHBVの増殖を抑制する作用(抗HBV作用)を有することから、抗HBV剤として利用することができる。本発明は、HIF-PHの発現を抑制する又は活性を阻害する物質を含有する、抗HBV剤を提供する。
上述のHIF-PHの発現を抑制する物質及びHIF-PHの活性を阻害する物質はHBVの増殖を抑制する作用(抗HBV作用)を有することから、抗HBV剤として利用することができる。本発明は、HIF-PHの発現を抑制する又は活性を阻害する物質を含有する、抗HBV剤を提供する。
抗HBV剤は、HBVの増殖を抑制する作用(抗HBV作用)を有する薬剤である。抗HBV作用は、HBV恒常的発現細胞若しくはHBV感染細胞、又はHBV感染動物を用いて、細胞内又は肝臓内でのHBV-RNA及びHBVタンパク質(HBc、HBs、HBx、HBe等)の産生抑制、細胞外又は血中への感染性粒子及びHBs抗原の分泌抑制、細胞内又は肝臓内のcccDNAの減少、感染動物におけるHBV感染に起因する疾患の発症等を指標として確認することができる。
本発明において、抗HBV剤は、少なくとも1種のHIF-PHの発現を抑制する又は活性を阻害する物質を含む。1つの実施形態において、抗HBV剤は、1又は複数種のHIF-PHの発現を抑制する物質を含む。別の実施形態において、抗HBV剤は、1又は複数種のHIF-PHの活性を阻害する物質を含む。さらに別の実施形態において、抗HBV剤は、1又は複数種のHIF-PHの発現を抑制する物質及び1又は複数種のHIF-PHの活性を阻害する物質を含む。
医薬組成物
抗HBV剤は、HBVの感染に起因する疾患の治療又は予防のための、HIF-PHの発現を抑制する又は活性を阻害する物質と薬学的に許容される成分とを含む医薬組成物の形態であり得る。医薬組成物形態の抗HBV剤は、HIF-PHの発現を抑制する又は活性を阻害する物質と薬学的に許容される成分とを含む抗HBV用医薬組成物と表すこともできる。
抗HBV剤は、HBVの感染に起因する疾患の治療又は予防のための、HIF-PHの発現を抑制する又は活性を阻害する物質と薬学的に許容される成分とを含む医薬組成物の形態であり得る。医薬組成物形態の抗HBV剤は、HIF-PHの発現を抑制する又は活性を阻害する物質と薬学的に許容される成分とを含む抗HBV用医薬組成物と表すこともできる。
医薬組成物に含まれる薬学的に許容される成分としては、HIF-PHの発現を抑制する又は活性を阻害する物質以外の薬物、緩衝剤、抗酸化剤、保存剤、タンパク質、親水性ポリマー、アミノ酸、キレート化剤、非イオン性界面活性剤、賦形剤、安定化剤、担体等を挙げることができる。薬学的に許容される成分は当業者において周知であり、当業者が通常の実施能力の範囲内で適宜選択して使用することができる。
医薬組成物の形態は任意であるが、経口剤(錠剤、カプセル剤、散剤、顆粒剤、細粒剤、丸剤、懸濁剤、乳剤、液剤、シロップ剤等)及び非経口製剤(注射剤、点滴剤、外用剤等)の形態を好ましい例として挙げることができる。
医薬組成物は、HBVに感染した対象、特にHBVの感染に起因する疾患を発症するおそれのある/又は発症した対象に投与される。HBVの感染に起因する疾患としては、B型急性肝炎、急性肝不全、B型慢性肝炎、B型肝硬変、B型肝炎から移行する肝臓がん等を挙げることができる。対象は好ましくはヒトである。
医薬組成物は、HBVの感染に起因する疾患の治療を必要とする又はその発症のおそれのある対象に投与されることで、当該疾患を治療又は予防することが可能である。本明細書において用いられる用語「治療」は、疾患の治癒又は一時的寛解を目的とする医学的に許容される全てのタイプの治療的介入を包含する。また用語「予防」は、疾患の罹患又は発症の防止又は抑制を目的とする医学的に許容される全てのタイプの予防的介入を包含する。すなわち、HBVの感染に起因する疾患の治療又は予防とは、HBVの感染に起因する疾患の進行の遅延又は停止、病変の退縮又は消失、発症の予防又は再発の防止等を含む、種々の目的の医学的に許容される介入を包含する。
医薬組成物は、用法、対象の年齢、疾患の状態その他の条件に応じて適宜決定される疾患の治療又は予防に有効な量のHIF-PHの発現を抑制する又は活性を阻害する物質を含む。医薬組成物の投与方法は、特に制限されないが、非経口製剤である場合は、例えば血管内投与(好ましくは静脈内投与)、腹腔内投与、腸管内投与、皮下投与等を挙げることができる。好ましい実施形態の一つにおいて、医薬組成物は、経口投与又は静脈内投与により生体に適用される。
IFN製剤との組み合わせ使用
HIF-PHの発現を抑制する又は活性を阻害する物質は、IFN応答性を改善し得ることから、従来のIFN療法において使用されるIFN又はペグインターフェロン(PEG-IFN)といったIFN製剤の薬効を期待することが難しいHBV感染者、例えばIFN製剤での前治療歴があり、当該治療が奏功しなかったHBVに感染した対象に対しても、抗HBV用医薬組成物として利用することができる。
HIF-PHの発現を抑制する又は活性を阻害する物質は、IFN応答性を改善し得ることから、従来のIFN療法において使用されるIFN又はペグインターフェロン(PEG-IFN)といったIFN製剤の薬効を期待することが難しいHBV感染者、例えばIFN製剤での前治療歴があり、当該治療が奏功しなかったHBVに感染した対象に対しても、抗HBV用医薬組成物として利用することができる。
またHIF-PHの発現を抑制する又は活性を阻害する物質は、IFN応答性を改善し得ることから、IFN製剤と組み合わせて使用することにより、IFN製剤の抗HBV作用を増強することができると期待される。したがって、本発明は、HIF-PHの発現を抑制する又は活性を阻害する物質とIFN製剤とを組み合わせてなる、HBV感染に起因する疾患の治療又は予防のための組み合わせ医薬;HBV感染に起因する疾患の治療又は予防のための組み合わせ医薬において、IFN製剤と組み合わせて使用するための、HIF-PHの発現を抑制する又は活性を阻害する物質;並びにHIF-PHの発現を抑制する又は活性を阻害する物質を含有する、HBV感染者のIFN応答性を改善するための剤をさらなる別の態様として提供する。ここで組み合わせ医薬及びIFN応答性を改善するための剤はいずれも医薬組成物の形態であり得る。
IFN製剤としては、従来からHBe抗原陽性のB型慢性活動性肝炎に対して適用されてきたIFNα及びIFNβや、Peg-IFNα-2aやPeg-IFNα-2b等のPEG(Polyethylene glycol)で修飾されたPEG-IFNを挙げることができる。
「組み合わせ医薬」とは、その必要がある、すなわちHBV感染に起因する疾患の治療又は予防が望まれる対象に、一緒に又は別々に、同時に又は逐次的に投与することを意図した2以上の医薬の組み合わせを意味する。意図される投与の態様としては、HIF-PHの発現を抑制する又は活性を阻害する物質とIFN製剤が一つの製剤中に含まれた製剤の投与や、別々に製剤化されたHIF-PHの発現を抑制する又は活性を阻害する物質とIFN製剤の投与を挙げることができる。また、別々に製剤化され投与される場合、それらの投与順序及び投与時期は特に制限されず、同時に投与されてもよく、時間を置いて異なる時間に又は異なる日に投与されても良い。
IFN製剤の臨床上の使用に関する方針は、例えば日本肝臓学会が発行するB型肝炎治療ガイドライン第3.1版(2019年3月発行)に詳細に説明されている。本発明におけるHIF-PHの発現を抑制する又は活性を阻害する物質との組み合わせ使用においても、IFN製剤の使用については前述のガイドラインその他の方針に従って行えばよい。
組み合わせて用いられるHIF-PHの発現を抑制する又は活性を阻害する物質とIFN製剤のそれぞれの量は、組み合わせによってHBVの感染に起因する疾患を治療又は予防することができる量であればよく、一般に、単独で用いられる場合の量と同等であるか、又はこれより少ない。単独で用いられる場合の量と同等の量のHIF-PHの発現を抑制する又は活性を阻害する物質とIFN製剤を用いることで、より強力な抗HBV作用が発揮され、より短期間に、又は単独で用いても効果が確認できなかった対象において、HBVの感染に起因する疾患を治療又は予防することができる。
本発明におけるHIF-PHの発現を抑制する又は活性を阻害する物質、及びこれとIFN製剤との組み合わせは、HBVの感染に起因する疾患を有する対象に投与することで、HBVの感染に起因する疾患を治療又は予防することができる。したがって本発明は、HBVの感染に起因する疾患の治療又は予防を必要とする対象に前述の有効量のHIF-PHの発現を抑制する又は活性を阻害する物質、又は組み合わせ有効量のHIF-PHの発現を抑制する又は活性を阻害する物質と組み合わせ有効量のIFN製剤との組み合わせを投与することを含む、HBVの感染に起因する疾患の予防又は治療方法として表すこともできる。ここで組み合わせ有効量とは、組み合わされたときにHBVの感染に起因する疾患を治療又は予防することができる、HIF-PHの発現を抑制する又は活性を阻害する物質、IFN製剤それぞれの量を意味する。
評価方法
本発明はさらに、HIF-PHの発現を抑制する活性又は酵素活性を阻害する活性を指標とした、被験物質の抗HBV活性、又はIFN製剤への応答性改善活性を評価する方法を提供する。
本発明はさらに、HIF-PHの発現を抑制する活性又は酵素活性を阻害する活性を指標とした、被験物質の抗HBV活性、又はIFN製剤への応答性改善活性を評価する方法を提供する。
ある実施形態において、評価方法は、被験物質の存在下及び非存在下のそれぞれにおけるHIF-PHの酵素活性を比較することで、被験物質の抗HBV活性、又はIFN製剤への応答性改善活性を判定することを含む。この方法においてHIF-PH阻害活性があると判定された物質は、抗HBV活性を有すると評価することができる。被験物質の存在下及び非存在下のそれぞれにおけるHIF-PHの酵素活性の比較は、ダプロデュスタットその他の公知のHIF阻害剤の阻害作用の評価又はスクリーニングにおいて採用されている方法を利用して行うことができる。
別の実施形態において、評価方法は、被験物質の存在下及び非存在下のそれぞれにおけるHIF-PH遺伝子のプロモーター活性を比較することで、被験物質の抗HBV活性、又はIFN製剤への応答性改善活性を判定することを含む。この評価方法は、例えば、被験物質を、HIF-PH遺伝子の発現を制御するプロモーター領域の制御下にマーカー遺伝子を有する核酸を含む細胞と共に、当該プロモーターによるマーカー遺伝子の発現が誘導される条件下でインキュベートするステップ;前記細胞におけるマーカー遺伝子由来シグナルの強度を測定するステップ;測定されたマーカー遺伝子由来シグナルの強度を、被験物質非存在下でのマーカー遺伝子由来シグナルの強度と比較するステップ;及びマーカー遺伝子由来シグナルの強度を減少させた被験物質を、抗HBV活性を有すると判定するステップを含み得る。
HIF-PH遺伝子のプロモーター領域としては、例えば、ヒトの遺伝子座1q42.1に存在するPHD2遺伝子の開始コドンの3.5kb上流に位置するTATA boxを含む領域又は開始コドンの0.5kb上流に位置するCpGアイランド内のHIF結合モチーフを含む領域(Metzen E., et al.,Biochem J. 2005 May 1; 387(Pt 3): 711-717)等を使用することができる。
マーカー遺伝子は、当該遺伝子の発現誘導を検出し得るものであれば特に制限はなく、例えばGFP等の蛍光タンパク質やルシフェラーゼ等の化学発光タンパク質をコードする遺伝子、又は薬剤耐性遺伝子や細胞の栄養要求性を補完することができる遺伝子等の細胞にフェノタイプ変化を与える遺伝子等を利用することができる。
HIF-PH遺伝子のプロモーター領域の制御下にマーカー遺伝子を有する核酸を含む細胞は、例えば、元来HIF-PH遺伝子を有する細胞中のHIF-PHのオープンリーディングフレーム領域の塩基配列を、マーカー遺伝子をコードする塩基配列に組み換えることで作製することができる。また、ヒトゲノム塩基配列情報を元にして、ヒトHIF-PH遺伝子のプロモーター領域とその制御下に組み換えたマーカー遺伝子とを含む発現ベクターを構築し、適当な細胞に当該発現ベクターを導入することで作製することもできる。
インキュベーションは、適当な容器内の培地又は緩衝液中で、前記細胞と被験物質とを共存させることにより行うことができる。インキュベーションの温度、時間等の条件は、細胞がマーカー遺伝子を発現することができる条件であればよい。また、マーカー遺伝子由来シグナルの測定は、用いられるマーカー遺伝子に適した方法によって行うことができる。
さらなる実施形態において、評価方法は、被験物質の存在下及び非存在下のそれぞれにおけるHIF-PH遺伝子の発現又はHIF-PHタンパク質の発現を比較することで、被験物質の抗HBV活性、又はIFN製剤への応答性改善活性を判定することを含む。この方法は、例えば、被験物質を、HIF-PH遺伝子又はタンパク質を発現することができる細胞と共にインキュベートするステップ;インキュベート後の前記細胞におけるHIF-PH遺伝子又はタンパク質由来シグナルの強度を測定するステップ;測定されたHIF-PH遺伝子又はタンパク質由来シグナルの強度を被験物質非存在下でのHIF-PH遺伝子又はタンパク質由来シグナルの強度と比較するステップ;及びHIF-PH遺伝子又はタンパク質由来シグナルの強度を低減させた被験物質を、抗HBV活性を有すると判定するステップを含み得る。
HIF-PH遺伝子又はタンパク質を発現することができる細胞は、ヒトのHIF-PH遺伝子又はタンパク質を発現することが好ましく、例えばヒトHIF-PH遺伝子を組み込んだ培養細胞株を用いることができる。
インキュベーションは、適当な容器内の培地又は緩衝液中で、前記細胞と被験物質とを共存させることにより行うことができる。インキュベーションの温度、時間等の条件は、細胞がHIF-PH遺伝子又はタンパク質を発現することができる条件であればよい。
HIF-PH遺伝子由来シグナルの測定は、HIF-PH遺伝子の塩基配列情報を利用したハイブリダイゼーション、定量的PCR、RNA-Seqその他の、特定の遺伝子発現を検出し定量することのできる一般的な方法によって行うことができる。また、HIF-PHタンパク質由来シグナルの測定は、HIF-PHに対する特異抗体を用いたELISA、ウェスタンブロッティング等の免疫学的検出法その他の、特定のタンパク質発現を検出し定量することができる一般的な方法によって行うことができる。
HIF-PHの発現が誘導される条件下で、被験物質とインキュベートしていない細胞と比べて、被験物質とインキュベートした細胞においてHIF-PH遺伝子又はタンパク質由来シグナルの強度を低減させた被験物質は、抗HBV活性を有すると判定することができる。
以下の実施例によって本発明をさらに詳細に説明するが、本発明はこれらに限定されるものではない。

HBVを恒常的に高発現するHepG2.2.15.7細胞(国立感染症研究所から入手)に、IFN応答配列(IFN-stimulated response element、ISRE、(Platanias LC et al, Nat Rev Immunol. 2005;5(5):375-86.))の下流にルシフェラーゼをコードする遺伝子を組み換えたレンチウイルスベクター(ISRE Cignal Lenti Reporter, Qiagen))を導入して、IFNαによってレポータータンパク質としてルシフェラーゼを発現する組換え細胞HepG2.2.15.7-ISRE-Luc細胞を作製した。この細胞にHBxタンパク質の発現をノックダウンすることができるsiRNA(siRNA against HBx; Hokkaido bio system sense strand, 5’-GGUCUUACAUAAGAGGACUdTdT-3’(配列番号1); anti-sense strand, 5’-AGUCCUCUUAUGUAAGACCdTdT-3’(配列番号2))を導入すると、ルシフェラーゼ活性が増加することを確認した。HepG2.2.15.7-ISRE-Luc細胞においてルシフェラーゼ活性を増加させる物質は、HBV感染細胞内のHBVタンパク質産生を減少させることにより、又はHBVタンパク質により惹起されるIFN不応性関連因子(SOCS3、PP2A等)の発現若しくは活性を減少させることによりIFN応答性を改善する作用を有するものと考えられる。また、同細胞においてルシフェラーゼ活性を減少させる物質は、IFN応答性を増悪する作用や、細胞毒性作用を有するものと考えられる。
1)HepG2.2.15.7-ISRE-Luc細胞の作製及び培養
HBVを恒常的に高発現するHepG2.2.15.7細胞(国立感染症研究所から入手)に、IFN応答配列(IFN-stimulated response element、ISRE、(Platanias LC et al, Nat Rev Immunol. 2005;5(5):375-86.))の下流にルシフェラーゼをコードする遺伝子を組み換えたレンチウイルスベクター(ISRE Cignal Lenti Reporter, Qiagen))を導入して、IFNαによってレポータータンパク質としてルシフェラーゼを発現する組換え細胞HepG2.2.15.7-ISRE-Luc細胞を作製した。この細胞にHBxタンパク質の発現をノックダウンすることができるsiRNA(siRNA against HBx; Hokkaido bio system sense strand, 5’-GGUCUUACAUAAGAGGACUdTdT-3’(配列番号1); anti-sense strand, 5’-AGUCCUCUUAUGUAAGACCdTdT-3’(配列番号2))を導入すると、ルシフェラーゼ活性が増加することを確認した。HepG2.2.15.7-ISRE-Luc細胞においてルシフェラーゼ活性を増加させる物質は、HBV感染細胞内のHBVタンパク質産生を減少させることにより、又はHBVタンパク質により惹起されるIFN不応性関連因子(SOCS3、PP2A等)の発現若しくは活性を減少させることによりIFN応答性を改善する作用を有するものと考えられる。また、同細胞においてルシフェラーゼ活性を減少させる物質は、IFN応答性を増悪する作用や、細胞毒性作用を有するものと考えられる。
2)HepG2.2.15.7-ISRE-Luc細胞を用いた化合物スクリーニング
10%FBS(Sigma)、ペニシリン-ストレプトマイシン溶液(Wako)、360μg/ml G418硫酸塩(Wako)及び2.6μg/mlピューロマイシン(Puromycin Dihydrochloride, Thermo)を含むDMEM(High Glucose)(ナカライ)培地に、HepG2.2.15.7-ISRE-Luc細胞を2.5×105細胞/mLとなるように懸濁した。96 wellコラーゲンコートディッシュプレート(イワキ)に1ウェルあたり80μLの細胞懸濁液を分注し、37℃で24時間、CO2インキュベーター内で培養した後、500 IU/mLのIFNα(イントロンA、MSD)及び1.25~50μMの試験化合物を含む培養液(10%FBS及びペニシリン-ストレプトマイシンを添加したDMEM)を1ウェルあたり20μL添加した。また、試験化合物を添加した培養液と同じ濃度(1%)のDMSOを添加したウェルを用意し、コントロールとして用いた。8時間培養した後、ウェルから培養液を全て除去し、100μLのPBS(gibco)を用いて3回洗浄した後、プレートごと-30℃で凍結保存した。
10%FBS(Sigma)、ペニシリン-ストレプトマイシン溶液(Wako)、360μg/ml G418硫酸塩(Wako)及び2.6μg/mlピューロマイシン(Puromycin Dihydrochloride, Thermo)を含むDMEM(High Glucose)(ナカライ)培地に、HepG2.2.15.7-ISRE-Luc細胞を2.5×105細胞/mLとなるように懸濁した。96 wellコラーゲンコートディッシュプレート(イワキ)に1ウェルあたり80μLの細胞懸濁液を分注し、37℃で24時間、CO2インキュベーター内で培養した後、500 IU/mLのIFNα(イントロンA、MSD)及び1.25~50μMの試験化合物を含む培養液(10%FBS及びペニシリン-ストレプトマイシンを添加したDMEM)を1ウェルあたり20μL添加した。また、試験化合物を添加した培養液と同じ濃度(1%)のDMSOを添加したウェルを用意し、コントロールとして用いた。8時間培養した後、ウェルから培養液を全て除去し、100μLのPBS(gibco)を用いて3回洗浄した後、プレートごと-30℃で凍結保存した。
凍結保存した96ウェルプレートに、1ウェルあたり20μLの5倍希釈細胞溶解液(Luciferase Cell Culture Lysis 5x Reagent, Promega)を分注し、室温で10分静置した。得られた細胞溶解液を測定用プレート(Assay Plate, 96 well White, Flat Bottom, CORNING)に分注し、ルシフェラーゼアッセイキット(Luciferase 1000 Assay System Promega)を用いてルシフェラーゼアッセイを行った。サンプルの調製はアッセイキットのプロトコールに従って行い、GloMax(登録商標)-Multi+Detection System with Instinct Software(Promega)を用いて測定した。
3)HepG2.2.15.7細胞を用いた抗HBV薬効評価試験
試験化合物を10 mMの濃度になるようにDMSO(Wako)に溶解し、ストック溶液とした。培養液(10%FBS及びペニシリン-ストレプトマイシンを含むDMEM培地)に懸濁したHepG2.2.15.7細胞を、1ウェルあたり2×105細胞となるようにコラーゲンコート12ウェルプレート(イワキ)に播種し、培養した。翌日、培養液を、最終濃度2.5、5.0又は10.0μMの試験化合物を含む培養液に交換し、72時間培養した。試験化合物を添加した培養液と同じ濃度(1%)のDMSOを添加したウェルを用意し、コントロールとして用いた。試験化合物添加から72時間後に培養上清を回収して-30℃で凍結保存した後、培養上清中のHBs抗原を測定した。

4)ヒト初代肝細胞(PXB細胞)を用いた抗HBV薬効評価試験
PXB細胞は、あらかじめ24ウェルプレートに2.1×105細胞/cm2の密度で播種されたものを、株式会社フェニックスバイオから購入した。また、PXB細胞の培養液及びHBV感染源(Genotype C)も、同様に株式会社フェニックスバイオから購入した。

購入したPXB細胞は、到着後すみやかに培養液を交換して3~4日培養した後、供給会社のプロトコールに従ってHBVの接種を行った。接種翌日に培養液を交換し、その更に翌日に、最終濃度2.5、5.0又は10.0μMの試験化合物を含む培養液に交換した。試験化合物添加から5日後にそれぞれ培養上清を回収し、10倍に希釈して-30℃で保存した後、培養上清中のHBs抗原及びHBV DNAを測定した。
5)培養上清中HBs抗原の測定
培養上清中HBs抗原は、Architect HBsAg-QT assay(Abbott Laboratories、検出感度下限0.05IU/mL)を用いた化学発光イムノアッセイで測定した。測定は、SRL社に外注した。

6)培養上清中HBV DNAの測定
培養上清中HBV DNAは、Cobas AmpliPrep/Cobas TaqMan HBV test, v2.0(Roche Diagnostics、検出感度範囲20-170,000,000 IU/mL、2.1-9.0 log10 copies/mL)を用いたリアルタイムPCR法で測定した。測定は、SRL社に外注した。
培養上清中HBV DNAは、Cobas AmpliPrep/Cobas TaqMan HBV test, v2.0(Roche Diagnostics、検出感度範囲20-170,000,000 IU/mL、2.1-9.0 log10 copies/mL)を用いたリアルタイムPCR法で測定した。測定は、SRL社に外注した。
7)細胞内HBV-RNAの測定
Genotype C HBV 感染PXB細胞を2×105 細胞/ウェルの濃度でコラーゲンコート12穴プレート(イワキ)に播種した。翌日、最終濃度2.5、5.0、10.0μMの薬剤を含む培養液に交換し、CO2インキュベーターにおいて72時間培養した。同様に、薬剤を添加した培養液と同じ濃度のDMSOを添加したウェルを用意してコントロールとした。薬物添加から72時間後、細胞を回収しRNeasy(登録商標)Mini Kit (QIAGEN)を用いて、RNAを抽出した。手順はキットに添付の取扱説明書に従った。抽出したRNAはPrime Script RT Master Mix (TaKaRa)を用いて逆転写しcDNAを合成した。リアルタイムPCRは、Platinum SYBER Green qPCR SuperMix-UDG with ROX(Thermo)を用い、Nishitsuji,. et al. Cancer Sci 2015 Nov;106(11):1616-24に記載されているgenotype C HBV RNAの増幅用プライマーを用いて行った。

実施例1
1)スクリーニング
北海道大学が保有する化合物・薬物ライブラリーに含まれる化合物3200個に対してHepG2.2.15.7-ISRE-Luc細胞を用いたスクリーニングを行い、同細胞のルシフェラーゼ活性を増加させた化合物を選抜し、さらにHepG2.2.15.7細胞を用いた培養上清中のHBs抗原量測定による抗HBV薬効評価試験を行った結果、HIF-PH阻害剤であるモリデュスタット、ダプロデュスタット、エナロデュスタット、ロキサデュスタット及びバダデュスタットが抗HBV剤として選抜された。これらの化合物のレポーターアッセイの結果を図1及び図2に示す。いずれの化合物を用いた場合もルシフェラーゼ活性の増加が観察され、特にモリデュスタットではより低濃度でもルシフェラーゼ活性の増加が確認された。

2)HepG2.2.15.7細胞を用いた抗HBV薬効評価試験
1)で選抜されたモリデュスタット、ダプロデュスタット、エナロデュスタット、ロキサデュスタット及びバダデュスタットについて、HepG2.2.15.7細胞を用いた抗HBV薬効評価試験を行った。培養上清中のHBs抗原及びHBV-DNAの測定結果を図3に示す。本試験では、モリデュスタット、ダプロデュスタット、エナロデュスタット及びロキサデュスタットはHepG2.2.15.7細胞のHBs抗原分泌及びHBV-DNA量を減少させること、並びにバダデュスタットはHepG2.2.15.7細胞のHBV-DNA量を減少させることが確認された。
1)で選抜されたモリデュスタット、ダプロデュスタット、エナロデュスタット、ロキサデュスタット及びバダデュスタットについて、HepG2.2.15.7細胞を用いた抗HBV薬効評価試験を行った。培養上清中のHBs抗原及びHBV-DNAの測定結果を図3に示す。本試験では、モリデュスタット、ダプロデュスタット、エナロデュスタット及びロキサデュスタットはHepG2.2.15.7細胞のHBs抗原分泌及びHBV-DNA量を減少させること、並びにバダデュスタットはHepG2.2.15.7細胞のHBV-DNA量を減少させることが確認された。
3)PXB細胞を用いた抗HBV薬効評価試験
2)の5種の化合物について、ヒト初代肝細胞(PXB細胞)を用いたHBV増殖抑制試験を行った。試験化合物添加から5日後の培養上清中HBs抗原の測定結果を図4に、培養上清中HBV-DNAの測定結果を図5に、細胞内HBV-RNAの測定結果を図6に、それぞれ示す。5種の化合物はいずれも、PXB細胞のHBs抗原量及びHBV-DNA量を減少させることが確認された。また、添加した化合物の種類及び濃度によってばらつきはあるものの、細胞内HBV-RNA量の減少も確認された。
2)の5種の化合物について、ヒト初代肝細胞(PXB細胞)を用いたHBV増殖抑制試験を行った。試験化合物添加から5日後の培養上清中HBs抗原の測定結果を図4に、培養上清中HBV-DNAの測定結果を図5に、細胞内HBV-RNAの測定結果を図6に、それぞれ示す。5種の化合物はいずれも、PXB細胞のHBs抗原量及びHBV-DNA量を減少させることが確認された。また、添加した化合物の種類及び濃度によってばらつきはあるものの、細胞内HBV-RNA量の減少も確認された。

Claims (15)
- 低酸素誘導因子プロリン水酸化酵素(HIF-PH)の発現を抑制する又は活性を阻害する物質を含有する、抗B型肝炎ウイルス剤。
- 前記物質が、ダプロデュスタット、エナロデュスタット、モリデュスタット、ロキサデュスタット及びバダデュスタット並びに薬学的に許容されるそれらの塩よりなる群から選択される少なくとも1つの化合物である、請求項1に記載の剤。
- HIF-PHの発現を抑制する又は活性を阻害する物質を含有する、B型肝炎ウイルスの感染に起因する疾患の治療又は予防のための医薬組成物。
- B型肝炎ウイルスの感染に起因する疾患が、肝炎、肝硬変又は肝臓がんである、請求項3に記載の医薬組成物。
- インターフェロン製剤での前治療歴があり、当該治療が奏功しなかった対象に対して用いるための、請求項3又は4に記載の医薬組成物。
- B型肝炎ウイルスの感染に起因する疾患の治療又は予防が、インターフェロン製剤の使用を含むものである、請求項3~5のいずれか一項に記載の医薬組成物。
- 前記物質が、ダプロデュスタット、エナロデュスタット、モリデュスタット、ロキサデュスタット及びバダデュスタット並びに薬学的に許容されるそれらの塩よりなる群から選択される少なくとも1つの化合物である、請求項3~6のいずれか一項に記載の医薬組成物。
- HIF-PHの発現を抑制する又は活性を阻害する物質とインターフェロン製剤とを組み合わせてなる、B型肝炎ウイルスの感染に起因する疾患の治療又は予防のための医薬。
- B型肝炎ウイルスの感染に起因する疾患が、肝炎、肝硬変又は肝臓がんである、請求項8に記載の医薬。
- インターフェロン製剤での前治療歴があり、当該治療が奏功しなかった対象に対して用いるための、請求項8又は9に記載の医薬。

- 前記物質が、ダプロデュスタット、エナロデュスタット、モリデュスタット、ロキサデュスタット及びバダデュスタット並びに薬学的に許容されるそれらの塩よりなる群から選択される少なくとも1つの化合物である、請求項12又は13に記載の剤。
- HIF-PHの発現を抑制する活性又は酵素活性を阻害する活性を指標とした、被験物質の抗B型肝炎ウイルス活性、又はインターフェロン製剤への応答性改善活性を評価する方法。
