Abstract
본 발명은 T 세포의 세포외 소포체 및 항암제를 유효성분으로 포함하는 병용 투여용 약학적 조성물에 관한 것으로, IL-2, 링커 및 막관통 단백질을 포함하는 벡터로 형질감염된 T 세포에서 분리된 IL-2를 표면에 발현하는 세포외 소포체를 항암제인 시스플라틴(cisplatin) 또는 항-PD-L1 항체와 병용 투여시 항암 효과가 증진되는 것을 확인함으로써, IL-2를 표면에 발현하는 세포외 소포체는 암의 치료를 위한 병용 투여제로 제공된다.
Description
본 발명은 T 세포의 세포외 소포체 및 항암제를 유효성분으로 포함하는 병용 투여용 약학적 조성물에 관한 것이다.
암은 비정상적인 세포의 과잉으로 인하여 발생하는 비제어적이고 무질서한 세포 증식의 산물로서, 분자생물학적인 관점에서 볼 때 유전자의 변이에 의하여 발생하는 질환이라고 할 수 있다. 암의 종류는 현재까지 밝혀진 것만 해도 수십 종에 이르며 주로 발병 조직의 위치에 따라 구분된다. 암은 양성종양과 악성종양으로 구분되는데, 양성종양은 비교적 성장 속도가 느리고 종양의 원발생 부위에서 다른 조직으로 이동되는 전이가 발생하지 않는 반면, 악성종양은 원발부를 떠나 다른 조직으로 침윤되어 빠르게 성장하는 특성을 가져 생명을 위협한다. 대부분의 암은 초기에 증상이 없으며, 증상이 있다고 하더라도 경미하여 대부분의 사람들이 이를 간과하기 쉽고, 이는 암의 사망률을 높이는 원인이 되고 있다.
암의 치료를 위해서 수술 요법, 화학 요법, 방사선 요법 등이 사용되고 있으나 많은 연구에도 불구하고 암 환자 전체의 50% 이상이 결국 치유되지 못하고 사망하는 것으로 보고된다. 그 이유는 외과적으로 절제를 하였다 하더라도 미세하게 전이된 암세포를 제거하지 못하여 암이 재발하거나, 항암제에 대한 암세포의 사멸이 유도되지 않거나 초기에는 반응을 보여 종양이 줄어드는 듯 보이지만 치료 도중이나 치료가 끝난 후 항암제에 대한 내성이 생긴 암세포들이 급격히 증가하기 때문이다. 오늘날에는 약 60여 종의 다양한 항암제가 사용되고 있으며, 암 발생 및 암세포의 특성에 관한 지식이 많이 알려짐에 따라 새로운 항암제 개발에 관한 연구가 활발하게 진행되고 있다. 그러나 대부분의 항암제는 오심과 구토, 탈모, 피부 및 손톱의 변색, 신경계 부작용 등의 심각한 부작용을 일으키며, 반복적으로 장기간 투여되거나 암이 재발된 경우에는 암세포가 항암제에 대한 내성을 획득함으로써 치료 효과를 상실하는 단점이 있다. 또한, 방사선 요법은 고에너지의 방사선을 암 조직에 조사하여 암세포의 사멸을 유도하나, 암 조직 주변에 있는 정상 조직에게도 손상을 주어 부작용을 발생시키는 단점이 있다.
한편, 작은 세포외 소포체(Small extracellular vesicles; sEV)는 대부분의 세포에서 분비되는 막 구조의 작은 소낭이다. sEV의 직경은 대략 30-100㎚로, sEV 안에는 그 세포에서 유래된 다양한 종류의 단백질, 유전물질(DNA, RNA, miRNA), 지질 등이 포함되어 있다. sEV는 원형질막(plasma membrane)으로부터 직접 떨어져 나가는 것이 아니라 다낭체(multivesicular bodies; MVBs)라고 불리는 세포 내 특정 구획에서 기원하여 세포 밖으로 방출 및 분비된다. 즉, 다낭체와 원형질막의 융합이 일어나면 소낭들은 세포 밖 환경으로 방출되는데 이것을 sEV라고 부른다. sEV가 어떤 기작에 의해 만들어지는지 정확하게 밝혀진 바가 없으나, 정상 상태 및 병적 상태 모두에서 다수의 세포 유형으로부터 분리되어 방출되는 것으로 알려져 있다.
본 발명의 목적은 항암제 단독 투여로 나타나는 한계를 극복할 수 있는 항암 효과가 증진된 병용 투여용 약학적 조성물을 제공하는 것이다.
본 발명은 IL-2를 표면에 발현하는 세포외 소포체; 및 항암제를 유효성분으로 포함하는 암의 치료를 위한 병용 투여용 약학적 조성물을 제공한다.
또한, 본 발명은 T 세포를 IL-2, 링커 및 막관통 단백질을 포함하는 벡터로 형질감염시키는 단계; 및 상기 형질감염된 T 세포로부터 표면에 IL-2를 발현하는 세포외 소포체를 분리하는 단계를 포함하는 병용 투여용 약학적 조성물의 제조 방법을 제공한다.
본 발명에 따르면, IL-2, 링커 및 막관통 단백질을 포함하는 벡터로 형질감염된 T 세포에서 분리된 IL-2를 표면에 발현하는 세포외 소포체를 항암제인 시스플라틴(cisplatin) 또는 항-PD-L1 항체와 병용 투여시 항암 효과가 증진되는 것을 확인함으로써, IL-2를 표면에 발현하는 세포외 소포체는 암의 치료를 위한 병용 투여제로 제공될 수 있다.

상기 세포외 소포체는 T 세포 유래일 수 있다. 상기 세포외 소포체는 직경이 30 내지 100㎚인 작은 세포외 소포체(Small extracellular vesicles; sEV)로, 엑소좀(exosome), 미세소포(microvesicle) 등을 포함할 수 있다. 상기 링커는 당업계에 통상적으로 사용되는 링커 서열을 사용할 수 있으며, 구체적으로 서열번호 1(GSTSGSGKPGSGEGSTKG)로 표시되는 아미노산 서열을 갖는 링커가 사용될 수 있다. 상기 막관통 단백질은 표피 성장인자 수용체, 인슐린 수용체, 혈소판 유래 성장인자(Platelet-derived growth factor; PDGF) 수용체, 혈관내피 성장인자 수용체, 섬유아세포 성장인자 수용체, 콜레시스토키닌(Cholecystokinin; CCK) 수용체, 신경영양인자(Neurotrophic factor; NGF) 수용체, 간세포 성장인자(Hepatocyte growth factor; HGF) 수용체, 에프린(Ephrin; Eph) 수용체, 안지오포이에틴 수용체 및 RTK(Related to receptor tyrosine kinase) 수용체로 이루어진 군에서 선택된 어느 하나 이상의 수용체의 막관통 도메인일 수 있다.
상기 항암제는 시스플라틴(cisplatin), 빈블라스틴(vinblastine), 빈크리스틴(vincristine), 액티노마이신-D(actinomycin-D), 5-플로오우라실(5-fluouracil), 도세탁셀(docetaxel), 카바지탁셀(cabazitaxel), 파클리탁셀(paclitaxel) 및 펨브롤리주맙(Pembrolizumab)으로 이루어진 군에서 선택되는 하나 이상일 수 있다.

본 발명의 약학적 조성물은 당해 발명이 속하는 기술분야에서 통상의 지식을 가진 자가 용이하게 실시할 수 있는 방법에 따라, 약제학적으로 허용되는 담체를 이용하여 제제화함으로써 단위 용량 형태로 제조되거나 또는 다용량 용기 내에 내입시켜 제조될 수 있다.
상기 약제학적으로 허용되는 담체는 제제시에 통상적으로 이용되는 것으로서, 락토스, 덱스트로스, 수크로스, 솔비톨, 만니톨, 전분, 아카시아 고무, 인산 칼슘, 알기네이트, 젤라틴, 규산 칼슘, 미세결정성 셀룰로스, 폴리비닐피롤리돈, 셀룰로스, 물, 시럽, 메틸 셀룰로스, 메틸 히드록시벤조에이트, 프로필히드록시벤조에이트, 활석, 스테아르산 마그네슘 및 미네랄 오일 등을 포함하나, 이에 한정되는 것은 아니다. 본 발명의 약학적 조성물은 상기 성분들 이외에 윤활제, 습윤제, 감미제, 향미제, 유화제, 현탁제, 보존제 등을 추가로 포함할 수 있다.
본 발명에 있어서, 상기 약학적 조성물에 포함되는 첨가제의 함량은 특별히 한정되는 것은 아니며 통상의 제제화에 사용되는 함량 범위 내에서 적절하게 조절될 수 있다.
상기 약학적 조성물은 수용액, 현탁액, 유탁액 등과 같은 주사용 제형, 환약, 캡슐, 과립, 정제, 크림, 젤, 패취, 분무제, 연고제, 경고제, 로션제, 리니멘트제, 파스타제 및 카타플라스마제로 이루어진 군으로부터 선택되는 하나 이상의 외용제 형태로 제형화될 수 있다.
본 발명의 약학적 조성물은 제형화를 위해 추가로 있는 약학적으로 허용가능한 담체 및 희석제를 포함할 수 있다. 상기 약학적으로 허용가능한 담체 및 희석제는 전분, 당, 및 만니톨과 같은 부형제, 칼슘 포스페이트 등과 같은 충전제 및 증량제, 카르복시메틸셀룰로오스, 히드록시프로필셀룰로오스 등과 같은 셀룰로오스 유도체, 젤라틴, 알긴산염, 및 폴리비닐 피롤리돈 등과 같은 결합제, 활석, 스테아린산 칼슘, 수소화 피마자유 및 폴리에틸렌 글리콜과 같은 윤활제, 포비돈, 크로스포비돈과 같은 붕해제, 폴리소르베이트, 세틸알코올, 및 글리세롤 등과 같은 계면활성제를 포함하나, 이에 한정되지 않는다. 상기 약학적으로 허용가능한 담체 및 희석제는 대상체에게 생물학적 및 생리학적으로 친화적인 것일 수 있다. 희석제의 예로는 염수, 수용성 완충액, 용매 및/또는 분산제(dispersion media)를 들 수 있으나, 이에 제한되는 것은 아니다.
본 발명의 약학적 조성물은 목적하는 방법에 따라 경구 투여하거나 비경구 투여(예를 들어, 정맥 내, 피하, 복강 내 또는 국소에 적용)할 수 있다. 경구 투여일 경우, 정제, 트로키제(troches), 로젠지(lozenge), 수용성 현탁액, 유성 현탁액, 조제 분말, 과립, 에멀젼, 하드 캡슐, 소프트 캡슐, 시럽 또는 엘릭시르제 등으로 제형화될 수 있다. 비경구 투여일 경우, 주사액, 좌제, 호흡기 흡입용 분말, 스프레이용 에어로졸제, 연고, 도포용 파우더, 오일, 크림 등으로 제형화 될 수 있다.
본 발명의 약학적 조성물의 투여량은 환자의 상태 및 체중, 연령, 성별, 건강상태, 식이 체질 특이성, 제제의 성질, 질병의 정도, 조성물의 투여시간, 투여방법, 투여기간 또는 간격, 배설율, 및 약물 형태에 따라 그 범위가 다양할 수 있으며, 이 분야 통상의 기술자에 의해 적절하게 선택될 수 있다. 예컨대, 약 0.1 내지 10,000mg/kg의 범위일 수 있으나 이제 제한되지 않으며, 하루 일회 내지 수회에 나누어 투여될 수 있다.
상기 약학적 조성물은 목적하는 방법에 따라 경구 투여되거나 비경구 투여(예를 들면, 정맥 내, 피하 내, 복강 내 또는 국소에 적용)될 수 있다. 본 발명의 약학적 조성물의 약학적 유효량, 유효 투여량은 약학적 조성물의 제제화 방법, 투여 방식, 투여 시간 및/또는 투여 경로 등에 의해 다양해질 수 있으며, 당해 기술 분야에서 통상의 지식을 가진 자는 목적하는 치료에 효과적인 투여량을 용이하게 결정하고 처방할 수 있다. 본 발명의 약학적 조성물의 투여는 하루에 1회 투여될 수 있고, 수회에 나누어 투여될 수도 있다.
또한, 본 발명은 T 세포를 IL-2, 링커 및 막관통 단백질을 포함하는 벡터로 형질감염시키는 단계; 및 상기 형질감염된 T 세포로부터 표면에 IL-2를 발현하는 세포외 소포체를 분리하는 단계를 포함하는 병용 투여용 약학적 조성물의 제조 방법을 제공한다.
이하, 본 발명의 이해를 돕기 위하여 실시예를 들어 상세하게 설명하기로 한다. 다만 하기의 실시예는 본 발명의 내용을 예시하는 것일 뿐 본 발명의 범위가 하기 실시예에 한정되는 것은 아니다. 본 발명의 실시예는 당업계에서 평균적인 지식을 가진 자에게 본 발명을 보다 완전하게 설명하기 위해 제공되는 것이다.
실시예 1. 세포 배양
HEK-293FT 세포(인간 배아 신장)는 10% 우태아혈청, 1% 페니실린 및 스트렙토마이신이 첨가된 DMEM 배지(Hyclone)에서 배양하고, Jurkat 세포(인간 T 림프구)는 10% 우태아혈청, 1% 페니실린 및 스트렙토마이신이 첨가된 RPMI 1640 배지(Hyclone)에서 배양하였다.

렌티바이러스
제조
항체-링커-PDGF 수용체의 막관통 도메인을 포함하는 렌티바이러스 벡터를 제작하고, 이를 IL-2-링커(GSTSGSGKPGSGEGSTKG, 서열번호 1)-PDGF 수용체의 막관통 도메인을 포함하는 렌티바이러스 벡터로 재구성하여 클로닝하였다. 형질감염을 위해, HEK-293FT 세포를 6 웰 플레이트에 약 1 × 106 세포/웰 밀도로 접종하고, Opti-MEM(ref.31985-070; Gibco) 배지 하에 Lipofectamine 2000 Reagent(ref.11668-027; Thermo fisher scientific), 상기 IL-2를 발현하는 렌티바이러스 벡터, pCMVD(# PS100001; Origene) 및 pVSVg(# 1733; Addgene) 바이러스 패키징 벡터를 1:1:1의 비율로 혼합하여 처리한 후 37℃에서 밤새 배양하였다. 다음날 상등액을 제거하고 10% 우태아혈청이 첨가된 신선한 배지로 교체하였다. 이후 48시간째에 바이러스를 함유하는 상등액을 수거하고, 원심분리 및 계면활성제가 없는 셀룰로오스 아세테이트(Surfactant-free cellulose acetate; SFCA) 막 여과 장치(0.22μm; Corning)를 이용하여 세포 파편들을 제거하였다. 렌티바이러스의 역가는 Lenti-X p24 신속 역가 키트(# 632200; Takara)를 사용하여 측정하였다. 렌티바이러스는 분주하여 -80℃에서 동결시켰다.

렌티바이러스를
이용한
Jurkat
세포로의 형질도입
10μg/mL의 폴리브렌(polybrene) 및 500μg/mL의 IL-2를 발현하는 렌티바이러스를 0.3mL RPMI 배지에 첨가하고 Jurkat 세포(1 × 106 세포/ml)에 처리하였다. 렌티바이러스와 세포 혼합물을 30℃에서 1,200xg으로 90분 동안 원심분리하여 회전접종(spinoculation)을 수행하였다. 이후 세포를 렌티바이러스와 함께 37℃에서 밤새 배양하였다. 초과 바이러스를 제거하고 다음날 10% 우태아혈청이 첨가된 신선한 배지를 첨가하였다.

세포외
소포체(small
extracellular
vesicles;
sEV
) 정제
대조군 Jukat 세포 또는 IL-2를 발현하는 Jukat 세포를 약 2 × 106 세포/웰 밀도로 접종하고, 우태아혈청이 첨가되지 않은 RPMI 배지에서 24시간 배양하였다. 이후 작은 세포외 소포체(small extracellular vesicles; sEV) 또는 sEVIL -2(IL-2 표면 발현 세포외 소포체)를 분리하기 위해, 각 세포에서 얻은 각 상등액을 300xg, 2500xg, 10,000xg으로 연속 원심분리하였다. 이어서, 상등액을 0.2μm 주사기 필터로 여과하고, 120,000xg에서 원심분리하였다. sEV 펠릿을 PBS에 재현탁하고 다시 120,000xg에서 원심분리하였다. 정제된 sEV 펠릿은 다음 실험을 위해 PBS 또는 1X 세포 용해 완충액에 재현탁하였다.
실시예
5. 암내 직접 주입을 통한 항암제 병용 처리에 따른 흑색종 이식 마우스의 암 성장 분석
도 1과 같이 B16F10 마우스 흑색종 세포 5 × 105을 C57BL/6 마우스 우측 옆구리에 피하주사 하였으며, 실험 시작일로부터 7일 후(암조직의 크기가 50mm3 이상일때 약물주입) 주 2회 PBS, 대조군 sEV(20μg/마리), sEVIL -2(20μg/마리)를 암조직 내 직접적으로 주입하였다. 그와 동일한 시간대에 시스플라틴(250μg/마리, Cisplatin, p4394; Sigma-Aldrich/cis-Diammineplatinum(II) dichloride crystalline), 대조군 IgG(be0252; bioxcell/anti-rat IgG2b), 항 PD-L1 항체(100μg/마리, be0101; bioxcell/anti PD-L1)를 마우스 복강 내 주입하였다(sEVIL -2 또는 대조군 sEV; intratumoal injection, 시스플라틴 또는 PBS; intraperitoneal injection, 항 PD-L1 또는 대조군 IgG; intravenous injection). 이후 공학용 디지털 캘리퍼를 이용하여 2일에 한 번씩 마우스의 암 조직(장축*단축*0.52)을 측정하여 비교하였다. 그 결과, 도 2와 같이, sEVIL -2와 함께 시스플라틴 또는 항PD-L1 항체를 주입한 마우스의 암 성장이 대조군보다 현저하게 암의 성장을 억제시켰다. 이후 마우스에서 적출되어진 암 조직의 무게 분석도 같은 결과를 보였다(도 3).
다음으로, 시스플라틴 또는 항PD-L1 항체와 병용처리한 흑색종 이식 마우스 암 조직의 PD-L1 및 Rab27a의 발현 변화를 확인하기 위해 웨스턴블랏을 진행하였다. 그 결과, 도 4와 같이, sEVIL -2 병용주입한 마우스의 암조직에서 PD-L1과 Rab27a 모두 극적으로 발현이 감소하는 것을 확인하였다.

6. 정맥 주입을 통한 항암제 병용 처리에 따른 흑색종 이식 마우스의 암 성장 분석
도 5와 같이 B16F10 마우스 흑색종 세포 5 × 105을 C57BL/6 마우스 우측 옆구리에 피하주사 하였다. 실험 시작일로부터 7일 후 주 2회 PBS, sEVIL -2(20μg/마리)를 정맥 주입하였다. 그와 동일한 시간대에 시스플라틴(250μg/마리, Cisplatin, p4394; Sigma-Aldrich/cis-Diammineplatinum (II) dichloride crystalline), 대조군 IgG(be0252; bioxcell/anti-rat IgG2b), 항 PD-L1 항체(100μg/마리, be0101; bioxcell/anti PD-L1)를 마우스 복강 내 주입하였다 (sEVIL -2 또는 대조군 sEV; intratumoal injection, 시스플라틴 또는 PBS; intraperitoneal injection, 항 PD-L1 또는 대조군 IgG; intravenous injection). 이후 공학용 디지털 캘리퍼를 이용하여 2일에 한 번씩 마우스의 암 조직(장축*단축*0.52)을 측정하여 비교하였다.
그 결과 도 6과 같이, sEVIL -2와 함께 항암제를 주입한 마우스의 암 성장이 대조군보다 현저하게 암의 성장을 억제시켰다.
이상으로 본 발명 내용의 특정한 부분을 상세히 기술하였는바, 당업계의 통상의 지식을 가진 자에게 있어서, 이러한 구체적 기술은 단지 바람직한 실시양태일 뿐이며, 이에 의해 본 발명의 범위가 제한되는 것이 아닌 점은 명백하다. 즉, 본 발명의 실질적인 범위는 첨부된 청구항들과 그것들의 등가물에 의하여 정의된다.
Claims (6)
- IL-2를 표면에 발현하는 세포외 소포체; 및 항암제를 유효성분으로 포함하는 암의 치료를 위한 병용 투여용 약학적 조성물.
- 제1항에 있어서, 상기 세포외 소포체는 T 세포 유래인 것을 특징으로 하는 병용 투여용 약학적 조성물.
- 제1항에 있어서, 상기 항암제는 시스플라틴(cisplatin), 빈블라스틴(vinblastine), 빈크리스틴(vincristine), 액티노마이신-D(actinomycin-D), 5-플로오우라실(5-fluouracil), 도세탁셀(docetaxel), 카바지탁셀(cabazitaxel), 파클리탁셀(paclitaxel) 및 펨브롤리주맙(Pembrolizumab)으로 이루어진 군에서 선택되는 하나 이상인 것을 특징으로 하는 병용 투여용 약학적 조성물.
- 제1항에 있어서, 상기 항암제는 항-PD-L1 항체, 항-PD-1 항체 또는 항-CTLA-4 항체인 것을 특징으로 하는 병용 투여용 약학적 조성물.
- 제1항에 있어서, 상기 병용 투여용 약학적 조성물은 흑색종, 결장암, 폐암, 피부암, 비소세포성 폐암, 결장암, 골암, 췌장암, 두부 또는 경부 암, 자궁암, 난소암, 직장암, 위암, 항문부근암, 유방암, 나팔관암종, 자궁내막암종, 자궁경부암종, 질암종, 음문암종, 호킨스씨병, 식도암, 소장암, 내분비선암, 갑상선암, 부갑상선암, 부신암, 연조직 육종, 요도암, 음경암, 전립선암, 만성 또는 급성 백혈병, 림프구 림프종, 방광암, 신장 또는 수뇨관암, 신장세포 암종, 신장골반 암종, 중추신경계 종양, 1차 중추신경계 림프종, 척수 종양, 뇌간신경교종 및 뇌하수체 선종으로 이루어진 군에서 선택된 어느 하나 이상의 암 질환을 예방 또는 치료하기 위한 것을 특징으로 하는 병용 투여용 약학적 조성물.
- T 세포를 IL-2, 링커 및 막관통 단백질을 포함하는 벡터로 형질감염시키는 단계; 및상기 형질감염된 T 세포로부터 표면에 IL-2를 발현하는 세포외 소포체를 분리하는 단계를 포함하는 병용 투여용 약학적 조성물의 제조 방법.
