Abstract
SLC15A3遺伝子を欠損させたマクロファージが優れた肺線維症治療効果を有することを見出した。
Description
本発明は、SLC15A3遺伝子の機能が抑制されたマクロファージを含む、肺線維症治療剤に関する。
特発性肺線維症(IPF)は、全世界で300万人の患者が推定されており、診断後治療なしでは生存期間の中央値が2~3年となる治癒困難な慢性疾患である。また、COVID-19による肺炎は、非定型肺炎を発症し、その後、急性肺障害および急性呼吸窮迫症候群(ARDS)を発症して線維化へと進展する。特に、高齢者や重症患者においては、肺線維化を伴う後遺症を残すケースが存在し、深刻なQOLの低下を招いている。
そこで、これら疾患に対する治療薬が求められているが、現状は支持療法が主流であり、インドリノン誘導体であるニンテダニブ(商品名:オフェブ)が、唯一、医薬品として承認されている。しかしながら、この医薬品は線維芽細胞の増殖を抑制することにより線維化の進行を抑制するものであり、線維化した組織を正常組織に回復させるものではない。
なお、本発明者は、SLC15A3(Solute carrier family 15 member 3)遺伝子を欠損させたマウスでは、ブレオマイシンにより誘導した肺線維症が改善することを報告しているが(非特許文献1)、この報告では、肺線維症の改善におけるマクロファージの役割については開示されていない。

本発明は、このような状況に鑑みてなされたものであり、その目的は、線維化した肺組織を正常組織に回復させることが可能な肺線維症治療剤、特に細胞治療剤を提供することにある。
本発明者らは、上記課題を解決すべく鋭意検討を行った結果、SLC15A3遺伝子を欠損させたマクロファージが、肺線維症の動物モデルにおいて、肺の線維化を回復させる優れた効果を示したことから、当該マクロファージが肺線維症治療のための優れた治療剤となることを見出し、本発明を完成するに至った。
従って、本発明は、SLC15A3遺伝子の機能が抑制されたマクロファージを含む、肺線維症治療剤、および当該治療剤を対象に投与することを含む、肺線維症の治療方法に関する。
本発明の肺線維症治療剤によれば、対象における肺の繊維化を改善することが可能となる。また、肺の繊維化の原因となる肺中コラーゲンの量を低減させるとともに、肺線維症の主な症状の一つである悪化した呼吸機能を回復させることから、本発明の肺線維症治療剤によれば、肺線維症の進行を停止させ、根本的な治療を行うことが可能となる。
本発明は、SLC15A3遺伝子の機能が抑制されたマクロファージを含む、肺線維症治療剤、および当該治療剤を対象に投与することを含む、肺線維症の治療方法を提供する。
本発明における「肺線維症」は、様々な原因により肺に炎症が生じ、その結果、肺胞壁、肺小葉、胸膜などに進行性の瘢痕化およびコラーゲンなどのマトリクスの沈着をきたすことにより、肺機能喪失をもたらす疾患であり、間質性肺炎として総称されている。間質性肺炎は、原因が不明とされる特発性間質性肺線維症、自己免疫疾患が原因となる自己免疫性の間質性肺疾患、職業環境性因子が原因となるじん肺などの間質性肺疾患、放射線や薬剤が原因となる医元性の間質性肺疾患、および呼吸器感染症の後遺症としての間質性肺炎を含む疾患である。
本発明における「SLC15A3」は、プロトン共役型トランスポーターであり、アミノ酸、ジペプチド、オリゴペプチドなどの輸送機能を有するとともに(Wang, Y. et al. J Pharm Sci 2019 Oct;108(10):3416-3424)、NOD1やNOD2を介した病原体認識(Nakamura,N. et al. Nature 2014 May 8;509(7499):240-244),Toll様受容体4が媒介する炎症応答(Song, F. et al. Cell Death Dis. 2018 Jul 10;9(7):770)、ヘルペスウイルスに対する免疫応答(He, L. et al. J. Immunol. Res. 2018 Jul 5;2018:5214187)、リソソーム酸性化(Zhang, H. et al, Proc Natl Acad Sci USA, 2020 Nov 24;117(47):29862-29871)を媒介することも知られている。典型的なヒト由来のSLC15A3タンパク質のアミノ酸配列および当該アミノ酸配列をコードする遺伝子の塩基配列は、GenBankデータベース(アクセション番号BC037974)や文献(Strausberg,R.L. et al. Proc. Natl. Acad. Sci. U.S.A. 2002 99(26):16899-16903)に開示されている。
本発明における「SLC15A3遺伝子の機能の抑制」は、その機序が、遺伝子の翻訳産物(タンパク質)の活性の抑制であっても、遺伝子の発現の抑制であってもよい。遺伝子の翻訳産物(タンパク質)の活性の抑制は、例えば、当該遺伝子への変異(欠失、置換、挿入など)の導入により行うことができる。SLC15A3の活性が抑制されたか否かは、例えば、SLC15A3のトランスポーター活性(Wang, Y. et al. J Pharm Sci 2019 Oct;108(10):3416-3424)を指標に評価することができる。また、SLC15A3によって制御される分子の発現強度を指標に評価することもできる。
SLC15A3遺伝子への変異の導入は、例えば、ゲノム編集系を利用して行うことができる。ゲノム編集系としては、例えば、SLC15A3遺伝子を標的とするCRISPR-Cas(例えば、CRISPR-Cas9)、TALENs、ZFNsなどの部位特異的人工ヌクレアーゼなどが挙げられる。ゲノム編集系の作用により標的部位で遺伝子が切断され、細胞のDNA修復機構を介して、遺伝子に変異(塩基の欠失、挿入など)が導入される。変異の導入により遺伝子を欠損させる場合、欠損は、遺伝子全体の欠損であっても、部分的な欠損であってもよい。Casのヌクレアーゼ活性の一部または全部を消失させた上でデアミナーゼ(例えば、シチジンデアミナーゼやアデノシンデアミナーゼ)を付加した改変Casを構成要素とするCRISPR-Casを用いることにより、あるいは、TALENsやZFNsなどのヌクレアーゼドメインに代えてデアミナーゼを用いることにより、遺伝子の特定の塩基を効率的に置換することもできる。ヌクレアーゼ活性を利用したゲノム編集系に、ホモロジーアームの間に所望の変異配列が導入されたドナーベクターを組み合わせて利用することにより、相同組換えを通じて遺伝子に所望の変異を導入することもできる。
本発明における「SLC15A3遺伝子の機能の抑制」は、また、当該遺伝子の発現の抑制によって行うこともできる。SLC15A3遺伝子の発現の抑制は、遺伝子の翻訳の抑制であっても、転写の抑制であってもよい。SLC15A3遺伝子の翻訳の抑制は、例えば、SLC15A3遺伝子の転写産物に結合する二重鎖RNA(dsRNA)を利用して行うことができる。二重鎖RNAとしては、例えば、siRNA、shRNA(short haipin RNA)、miRNAなどが挙げられる。SLC15A3遺伝子の転写の抑制は、例えば、SLC15A3遺伝子の発現制御領域への変異の導入によって行うことができる。変異の導入には、当該発現制御領域を標的化する上記のゲノム編集系を利用することができる。SLC15A3遺伝子の発現が抑制されたか否かは、翻訳レベルであれば、例えば、翻訳産物に結合する抗体を利用した免疫学的測定法により、転写レベルであれば、例えば、RT-PCR法やノーザンブロッティング法により、それぞれ評価することができる。
これらSLC15A3遺伝子の機能を抑制するための分子は、当該分子自体をマクロファージに導入してもよく、また、当該分子をマクロファージ内で発現させるために、当該分子をコードするDNA(典型的には、当該DNAが挿入されたベクター)をマクロファージに導入してもよい。これら分子のマクロファージへの導入の方法としては、特に制限はなく、例えば、ウイルスベクター(例えば、アデノ随伴ウイルスベクター)を用いた感染、マイクロインジェクション法、電気穿孔法、リン酸カルシウム法、リポソーム法、DEAEデキストラン法、カチオン性脂質媒介トランスフェクション、エレクトロポレーションなどが挙げられる。
SLC15A3遺伝子の機能を抑制するためのマクロファージは、例えば、ヒト末梢血より単球を分離し、マクロファージ培養液にて培養することにより調製することができる。マクロファージ培養においては、市販のマクロファージ培養液を使用してもよい。患者への適合性の観点から、マクロファージは、患者の末梢血(すなわち、自家の末梢血)から調製することが好ましい。また、単球やマクロファージは、iPS細胞などの多能性幹細胞から誘導して調製することもできる。
こうして調製されたSLC15A3遺伝子の機能が抑制されたマクロファージは、対象(患者)に投与することにより、肺線維症を治療することができる。対象への投与は、経気道的な肺への直接投与が好ましい。経気道的投与においては、例えば、気管支ファイバースコープを用いることができる。対象へのマクロファージの投与量および投与頻度は、対象の症状などを考慮して適宜調整される。
本発明の肺線維症治療剤は、対象に対して単独で投与してもよく、また、肺線維症の治療に有効な他の薬剤と組み合わせて対象に投与してもよい。
[実施例1] SLC15A3欠損マクロファージの肺線維症動物モデルにおける病態改善効果
SLC15A3欠損および野生型マウス骨髄より単核球画分を分離し、溶血により赤血球を除去した後、107個の細胞を10cmプラスチック培養皿に播種し、10% ウシ胎児血清を含むRPMI1640完全培地にて組み換えM-CSF存在下で7日間培養した。得られたマクロファージ4×106個を、シリカ微粒子投与後15日目の肺に経気道的に移植した。肺線維症の病態は、1週間おきにマイクロCTによる解析を行った。また、移植後25日目(シリカ投与後35日目)にマウスを安楽死させ、マイクロCT、肺活量、およびコラーゲン量を評価した。

その結果、SLC15A3欠損マウスにおいては、肺の線維化が改善するだけでなく、呼吸機能も回復し、線維化肺中のコラーゲン量が有意に減少することが判明した(図1)。
本発明によれば肺線維症を効果的に治療することができる。従って、本発明は、医療分野において大きく貢献するものである。
