Abstract
本発明は、根治療法が未だ見出されていないレット症候群(RTT)、自閉症スペクトラム障害または統合失調症を処置および/または予防するための治療を目的とする。 本発明は、レトロトランスポゾンL1の逆転写阻害物質を含有する、レット症候群(RTT)、自閉症スペクトラム障害または統合失調症を予防または処置するための医薬組成物に関する。
Description
本特許出願は、日本国特許出願2021-098585号(2021年6月14日出願)に基づくパリ条約上の優先権および利益を主張するものであり、ここに引用することによって、上記出願に記載された内容の全体が本明細書中に組み込まれるものとする。
本発明は、発達障害または精神疾患を処置および/または予防するための医薬組成物、詳細にはレトロトランスポゾンL1の逆転写阻害物質を含有する、レット症候群および自閉症スペクトラム障害などの発達障害、または統合失調症などの精神疾患を処置および/または予防するための医薬組成物に関する。
母体免疫活性化(MIA)、母体ストレス、母子分離ストレス(MSS)および粗悪養育行動などの早期ライフステージ(受精~若年成人期)の環境要因変化は、ライフコース中後期(成人以降)に統合失調症、自閉症スペクトラム障害、うつ、および不安障害などを含む精神・神経疾患の発症リスクを高めると言われている。その発症機序の詳細は不明であるが、早期ライフステージでの変化がライフコース中後期に異常として現れることから、エピジェネティック修飾変化の関与が考えられており、エピジェネティック修飾に関連してレトロトランスポゾンLINE1(long interspersed nucleotide elementsまたはL1)の関与が報告されている(非特許文献1)。
レトロトランスポゾンL1プロモーターは通常、メチル化されており、メチル化DNA結合性転写抑制因子MeCP2(メチル-CpG結合タンパク質2)が結合することによって、L1の転写は抑制されている。しかし、環境要因の変化等により、L1プロモーターが脱メチル化されると、MeCP2が解離してその転写が活性化される。本発明者らは、L1 cDNAの転移は通常、生殖系以外では脳でしか見られないこと、および神経幹細胞(NSC)からニューロンへの分化過程でレトロトランスポゾンL1の発現が高いことを見出している(非特許文献2)。さらに、非特許文献2では、MeCP2欠損NSCではさらにL1の転写が亢進していること、また、DNA脱メチル化剤処理によりNSCの中のL1プロモーターが脱メチル化されると、そこへはMeCP2が結合しなくなることを明らかにしている。
MeCP2はX染色体上に存在し、その変異は進行性精神・神経疾患であるレット症候群(Rett syndrome:RTT)の原因となる(非特許文献3)。レット症候群は、1966年にウィーンの小児神経科医Andreas Rettにより初めてその症例が報告された(非特許文献4)。その後,1980年代に多くの症例が報告されるようになり、レット症候群は主に女児に発症する神経発達障害として世界的に認知されるようになった。
特許文献1およびそのファミリーである特許文献2および3には、神経細胞をトランスポゾン阻害物質で処理することによる神経細胞内の非LTRレトロトランスポゾン(Non-long terminal repeat (LTR) retrotransposons)を減少させる方法が記載されている。レトロトランスポゾンは、LTRレトロトランスポゾンおよび非LTRレトロトランスポゾンに大別され、後者はLINE1またはL1とも呼ばれる。これらの特許文献では、当該神経細胞は神経細胞内のレトロトランスポゾンL1によってある種の疾患状態にある神経細胞であり、当該疾患状態にある神経細胞をもつ疾患としてレット症候群が挙げられ、そして転移阻害物質として逆転写酵素阻害物質が挙げられている。しかし、逆転写酵素阻害物質によってレトロトランスポゾンL1が調節できること、そしてレット症候群を処置できることについて、具体的かつ明示的な記載はなく、実験データにより実証されていない。

レット症候群(RTT)および自閉症スペクトラム障害などの発達障害を処置する際、主な治療法は「療育」と「生活環境の調整」であるが、対象患者における疾患・障害・症状に応じ、薬物療法が必要な場合がある。例えば、てんかん発作がある場合は、抗てんかん薬、睡眠障害、不注意、多動性、衝動性、自傷行為、興奮、攻撃性などによって生活に支障を来している場合は、睡眠導入薬、双極性障害治療薬、ADHD治療薬、抗精神病薬、また、不安、うつ、緊張などの精神症状がある場合は、抗不安薬、抗うつ薬等が処方される。しかし、いずれの薬物療法も、対症療法に過ぎず、根本的に解決できる薬物療法は常に求められている。
また、統合失調症などの精神疾患を処置する場合、モノアミンの変化やグルタミン酸受容体の変化などに着目した薬物療法が施される。統合失調症は、その症状は大きく陽性症状、陰性症状、認知機能障害の三つに分けることができ、それぞれの症状に対応した処置が行われる。陽性症状に対しては主としてドパミン仮説に基づいた薬物が処方されるが、一定の効果が認められるものの、効果が認められない場合があり、陰性症状や認知機能障害に対しては十分な治療効果を示す薬物はない。
この状況に鑑み、本発明は、レット症候群(RTT)および自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患を処置および/または予防するための新たな医薬組成物を目的とする。
本発明者らは、逆転写されたL1cDNAがゲノムに挿入されることによって種々の遺伝子の発現や機能が異常になるという従来技術(特許文献1-3)では、発症は、L1cDNAがゲノムに一旦挿入されて起こるため、挿入されたL1cDNAをゲノムから取り除かない限り治療はできないと想定した。特定遺伝子をゲノムから取り除く作業は煩雑かつ高価である。そこで、本発明者らは、L1cDNA自身を機能性分子として捉え、L1cDNAの存在によって惹起される細胞の異常応答はレトロトランスポゾンL1の逆転写阻害物質によってL1cDNAの産生を抑制すれば軽減できると考え、鋭意研究を重ねたところ、レット症候群(RTT)および自閉症スペクトラム障害などの発達障害ならびに統合失調症などの精神疾患を、予防のみならず、処置することが可能であることを見出し、本発明を完成させた。
したがって、本発明は、以下の態様を含む。

レトロトランスポゾンL1の逆転写阻害を標的とした、レット症候群(RTT)、自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患に対する画期的な医薬組成物を提供することができる。本発明は、根治療法が未だ見出されていないレット症候群(RTT)、自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患を処置および/または予防するための治療を可能とする。
本発明はひとつの実施形態において、レトロトランスポゾンL1の逆転写阻害物質、好ましくは逆転写酵素阻害物質、具体的にはヌクレオシドアナログ逆転写酵素阻害物質、好ましくはラミブジンまたはスタブジンを含有する、レット症候群(RTT)および自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患、好ましくはレット症候群を処置および/または予防するための医薬組成物を提供する。
本発明者らは、神経系細胞におけるレトロトランスポゾンL1cDNAの産生を抑制すれば、レット症候群および自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患を、予防のみならず、処置することが可能であることを見出した。上記の通り、特許文献1-3には、神経細胞をトランスポゾン阻害物質で処理することによる神経細胞内の非LTRレトロトランスポゾンを減少させる方法が例示されているが、具体的な根拠となる実験データで実証されていない。具体的には、レット症候群の患者脳においてレトロトランスポゾンL1 DNA量が健常人脳と比較して多いことが記載されている。しかし、これは、レトロトランスポゾンL1が転移した、すなわちゲノムDNAに組み込まれた状態のL1のcDNA数が多いことを示しており、細胞質におけるL1のcDNA数、すなわち逆転写量自体が多いことを示しているわけではない。この点は、例えばFIG.2において、ここではL1の転移が起こった場合にのみEGFP(蛍光タンパク質)が発現されるように細工されたトランスジェニックマウスでの結果が示され、MeCP2KOマウスにおいて脳の様々な部位でEGFPを発現する細胞が多いことより、多くの転移が起こっている、つまりゲノムに組み込まれたL1の数が多いことが示されていることから、明らかである。
さらには、特許文献1-3およびこれまでの報告では、レトロトランスポゾンL1cDNAがゲノムの中に挿入され、それが遺伝子機能を破壊することでRTT等の神経系疾患を発症させると考えられている。この機序から、一旦発症してしまった場合、遺伝子は既に機能が壊されているため、特許文献1-3に記載されているトランスポゾン阻害物質などでは、予防はできても、処置することは不可能である。他方、本発明者は、細胞質における逆転写されたL1cDNA自体を機能性因子として捉え、レトロトランスポゾンL1cDNA産生の抑制効果が、L1cDNAの存在によって惹起される細胞の異常応答、すなわちレット症候群(RTT)および自閉症スペクトラム障害などの発達障害ならびに統合失調症などの精神疾患を処置できると考えた。レトロトランスポゾンL1cDNA産生の抑制効果は、レトロトランスポゾンL1の逆転写阻害物質によって得ることができる。
本発明における「レトロトランスポゾンL1の逆転写阻害物質」は、上記の通り、神経系細胞の細胞質におけるレトロトランスポゾンL1の逆転写産物cDNAの産生を阻害する物質であれば、何でもよく、例えば、レトロトランスポゾンL1の転写、L1cDNAの産生、転移若しくは機能を阻害する物質、またはこれらを同時に阻害する物質が含まれる。L1逆転写阻害物質には、HIV感染症またはAIDSの治療に使用される抗レトロウイルス薬が含まれ、場合によってはB型肝炎も処置する逆転写酵素阻害物質を挙げることができる。すなわち、逆転写酵素阻害物質は、HIVおよびその他のレトロウイルスの複製に必要なウイルスDNAポリメラーゼである逆転写酵素の活性を阻害するものである。
本発明において、逆転写酵素阻害物質には、ヌクレオシドまたはヌクレオチドアナログ逆転写酵素阻害薬、および非ヌクレオシド系逆転写酵素阻害物質が包含される。
ヌクレオシドまたはヌクレオチドアナログ逆転写酵素阻害物質は、ウイルスDNAを合成するために必要な天然のデオキシヌクレオチドアナログであり、成長中のウイルスDNA鎖への取り込みについて、天然のデオキシヌクレオチドと競合する。しかし、天然のデオキシヌクレオチド基質とは異なり、逆転写酵素阻害物質にはデオキシリボース部分に3'-ヒドロキシル基が無い。結果として、逆転写酵素阻害物質の取り込み後、次のデオキシヌクレオチドはDNA鎖を延長するために必要な次の5'-3'ホスホジエステル結合を形成できない。したがって、逆転写酵素阻害物質が組み込まれると、ウイルスDNA合成が停止する。好ましくは、L1逆転写酵素阻害活性を有する逆転写酵素阻害物質である。

非ヌクレオシド系逆転写酵素阻害物質は、酵素に直接結合することにより逆転写酵素をブロックする。非ヌクレオシド逆転写酵素阻害物質はヌクレオシドアナログ逆転写酵素阻害物質のようにウイルスDNAに組み込まれないが、代わりにDNA合成のプロセスを実行するために必要な逆転写酵素のタンパク質ドメインの動きを抑制する。
非ヌクレオシド系逆転写酵素阻害物質として、以下が例示される:
エファビレンツ(EFVとも呼ばれる;Dai et al., BMC Biochemistry 2011, 12:18)、デラビルジン(DLVとも呼ばれる;Dai et al., BMC Biochemistry 2011, 12:18)、エトラビリン(ETRとも呼ばれる)、リルピビリン(RPVとも呼ばれる)、ドラビリン (MK-1439とも呼ばれる)。好ましくは、L1逆転写酵素阻害活性を有する非ヌクレオシド系逆転写酵素阻害物質である。より好ましくは、L1逆転写酵素阻害活性を有するエファビレンツおよびデラビルジンが挙げられる。さらに好ましくは、エトラビリンまたはリルピビリンである。ネビラピン(NVP)は非ヌクレオシド系逆転写酵素阻害物質でありHIV逆転写酵素阻害活性を有しているが、L1逆転写酵素阻害活性を有していないため(Dai et al., BMC Biochemistry 2011, 12:18)、本発明の発達障害および精神疾患を治療する医薬組成物には当たらない。
エファビレンツ(EFVとも呼ばれる;Dai et al., BMC Biochemistry 2011, 12:18)、デラビルジン(DLVとも呼ばれる;Dai et al., BMC Biochemistry 2011, 12:18)、エトラビリン(ETRとも呼ばれる)、リルピビリン(RPVとも呼ばれる)、ドラビリン (MK-1439とも呼ばれる)。好ましくは、L1逆転写酵素阻害活性を有する非ヌクレオシド系逆転写酵素阻害物質である。より好ましくは、L1逆転写酵素阻害活性を有するエファビレンツおよびデラビルジンが挙げられる。さらに好ましくは、エトラビリンまたはリルピビリンである。ネビラピン(NVP)は非ヌクレオシド系逆転写酵素阻害物質でありHIV逆転写酵素阻害活性を有しているが、L1逆転写酵素阻害活性を有していないため(Dai et al., BMC Biochemistry 2011, 12:18)、本発明の発達障害および精神疾患を治療する医薬組成物には当たらない。

第1世代非ヌクレオシド系逆転写酵素阻害物質であるNVP、EFVおよびDLVは、互いに化合物の構造式は異なっているが、結合した状態では類似し、蝶がはねを広げたような形にたとえられる。結合形式から大きく次の2つのタイプに分けることが出来る。
tight-binding inhibitor:薬剤がRTに結合すると外れない薬剤であり、EFVがこの対応に該当する。Tight-binding inhibitorは、HIVの感染予防に用いる殺菌剤としての使用が検討されている。
rapid equilibrium inhibitor:薬剤は可逆的にRTに結合しており、RTと薬剤が結合している側に極端に平衡が偏っているために抗HIV-1効果を呈するが、殺菌剤としては作用しない薬剤である。NVPとDLVがこのタイプになる。
tight-binding inhibitor:薬剤がRTに結合すると外れない薬剤であり、EFVがこの対応に該当する。Tight-binding inhibitorは、HIVの感染予防に用いる殺菌剤としての使用が検討されている。
rapid equilibrium inhibitor:薬剤は可逆的にRTに結合しており、RTと薬剤が結合している側に極端に平衡が偏っているために抗HIV-1効果を呈するが、殺菌剤としては作用しない薬剤である。NVPとDLVがこのタイプになる。
第2世代非ヌクレオシド系逆転写酵素阻害物質であるETRおよびRPVは、ジアリールピリジン(DAPY)アナログであり、ねじれによる柔軟性(wiggling)と揺れによる再配置(jiggling)といった特徴的な柔軟性(flexibility)をもつ構造を有する。第1世代NNRTIと交差耐性を示さない理由として、この柔軟性があることで耐性変異獲得後の変形した結合部位にも複数箇所の結合が可能となり、逆転写酵素活性を阻害すると考えられている。
本発明において、逆転写酵素阻害物質には、上記例示した物質に加え、例えば経口吸収性が改善されているプロドラッグ、ヌクレアーゼまたはエステラーゼ等により分解されにくく、生体内において長時間安定した血中濃度を維持する改変体、耐性株に奏功する新たな化合物等、およびこれらの合剤も、神経系細胞の細胞質におけるレトロトランスポゾンL1の逆転写産物cDNAの産生を阻害する限り、本発明の「レトロトランスポゾンL1の逆転写阻害物質」に包含される。
本発明の医薬組成物が処置および/または予防する対象疾患は、レット症候群および自閉症スペクトラム障害などの発達障害、ならびに統合失調症などの精神疾患、好ましくはレット症候群である。上記の通り、本発明者は、L1cDNA産生の抑制効果を確かめたことから、細胞質における逆転写されたL1cDNA自体を機能性因子と捉え、L1cDNAの産生を阻害することで、疾患の処置が可能となると考えている。実施例において詳述する通り、レット症候群のモデルマウスとして知られているMeCP2-KOマウス(Chen, RZ, Akbarian S, Tudor M and Jaenisch R. Nat Genet 27, 327-331 (2001))において、HIV治療薬として臨床的に利用されている逆転写酵素阻害薬3TCおよびd4Tが、レット症候群の典型的な症状を明らかに軽減し、またそれらの処置マウスにおける生存率が格段に上がることが証明された。MeCP2-KOマウスではL1の転写が亢進しており(非特許文献2)、そのためレット症候群症状を呈するところ、本実施例において、逆転写酵素阻害物質がL1の逆転写を阻害し、それによりレット症候群の典型的な症状を明らかに軽減したと考えることができる。
自閉症スペクトラム障害(ASD)の患者から採取された末梢血由来の白血球では、L1のメチル化が低下し、その転写量も高いこと(Tangsuwansri C, et al., PLoS One 13, e0201071 (2018)、Shpyleva S, et al. Mol Neurobiol 55, 1740-1749 (2017))、および発達障害であるASD患者でもL1の発現亢進が認められること(Suarez et al., Dev Neurobiol 434-455(2018))が報告され、これらは、L1がASDの症状と関連していることを示している。また、ASD患者の脳でもL1の転移が多く、おそらくは転写も高いと思われる。これらのことから、レット症候群と同様、本発明にかかるレトロトランスポゾンL1の逆転写阻害物質は、ASDを処置および/または予防することができると考えられる。
統合失調症患者の死後脳において、L1cDNAの転移が増加していることが報告されている(Bundo M, et al., Neuron 81, 306-313 (2014))。また、ASDや統合失調症のモデルマウス作製には、polyI:Cを母マウスに投与するところ、この母マウスから生まれた子マウスでもL1の転写亢進がみられるとの報告がある(Bundo M, et al., Neuron 81, 306-313 (2014))。これらのことから、レット症候群と同様、本発明にかかるレトロトランスポゾンL1の逆転写阻害物質は、統合失調症を処置および/または予防することができると考えられる。
レット症候群(Rett syndrome:RTT)は、1966年にウィーンの小児神経科医Andreas Rettにより初めて症例が報告された(非特許文献4)。その後、1980年代に多くの症例が報告されるようになり、レット症候群は主に女児に発症する神経発達障害として世界的に認知されるようになった。症状や病気の程度には患者ごとに大きな幅があり、病気の約8割以上を占める典型的な患者は、生後6か月くらいまでは一見正常に見えるが、それ以降に、体が柔らかい、四つ這いや歩行などの運動の遅れ、外界への反応が乏しい、視線が合いにくいなどの自閉症状が出る。必ず出る症状ではないが、頻度の高い症状として、けいれん、呼吸の異常、頭囲の発育の伸びが鈍くなるなどの症状が現れる指定難病(156)である(https://www.mhlw.go.jp/file/05-Shingikai-10601000-Daijinkanboukouseikagakuka-Kouseikagakuka/0000084170.pdf)。
自閉症スペクトラム障害は、社会性・コミュニケーション能力の障害,繰り返し行動や偏った興味などにより特徴づけられる複雑な発達障害である。ASDは異なる遺伝的・環境的要因の組み合わせで発症すると考えられており、その病態はきわめて複雑である。最新の研究により、アメリカにおけるASDの発症頻度は8歳以下で68人に1人と報告されている。このように高頻度な発症率にも関わらず、依然としてASDの発症病態は大部分が不明瞭である。
統合失調症は、現実との接触の喪失、幻覚、妄想などによって特徴づけられる精神疾患である。病気の経過によって前駆期、急性期、休息期、回復期に分けられ、急性期には陽性症状が目立ち、無気力で何もしなくなるなどの陰性症状が中心となり休息期に入る。また、複数の段階で認知機能障害が悪化することが示唆されている、原因は不明であるが、脳内で神経伝達物質のバランスが崩れることが関与すると考えられており、早期に治療を開始すれば治療により良く反応する傾向がある。
本発明において、「処置」とは、 (1) レット症候群および自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患の発症を遅延させる; (2) レット症候群および自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患の症状の進行、増悪または悪化を減速または停止させる; (3) レット症候群および自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患の症状の寛解をもたらす;あるいは (4) レット症候群および自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患を治癒させることを目的とする方法またはプロセスを意味する。処置は、予防的措置として疾患または状態の発症前に施してもよいし、あるいは、処置は、疾患の発症後に施してもよい。
本発明において、「予防」とは、レット症候群および自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患の発症を事前に防ぐことを意味する。
本発明の医薬組成物は、当業者に公知の方法で製剤化することが可能である。本発明の医薬組成物は、非経口投与および経口投与のいずれによっても投与することができる。非経口投与の場合、例えば、注射剤型、経鼻投与剤型、経肺投与剤型、経皮投与型の組成物とすることができる。例えば、静脈内注射、筋肉内注射、腹腔内注射、皮下注射などにより全身または局部的に投与することができる。経口投与の場合、錠剤、カプセル剤、丸剤、顆粒剤、散剤、シロップ剤等の組成物とすることができる。
投与方法は、患者の年齢、症状により適宜選択することができる。投与量および投与方法は、患者の体重、年齢、症状などにより変動するが、当業者であればそれらの条件を考慮し適当な投与量および投与方法を設定することが可能である。
レット症候群および自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患を処置および/または予防するための物質は、効果的なレット症候群および自閉症スペクトラム障害などの発達障害または統合失調症などの精神疾患の予防剤または治療剤となる。
本発明は別の態様として、発達障害または精神疾患を処置および/または予防するための方法であって、レトロトランスポゾンL1の逆転写阻害物質を、そのような処置等を必要としている対象に投与することを含む方法、好ましくは、レトロトランスポゾンL1の逆転写阻害物質が逆転写酵素阻害物質である方法、より好ましくは、逆転写酵素阻害物質がヌクレオシドアナログ逆転写酵素阻害物質である方法に関する。
さらに、本発明は別の態様として、発達障害または精神疾患を処置および/または予防するためのレトロトランスポゾンL1の逆転写阻害物質、好ましくは逆転写酵素阻害物質、より好ましくはヌクレオシドアナログ逆転写酵素阻害物質に関する。
本発明はさらなる別の態様として、発達障害または精神疾患を処置および/または予防するための医薬を製造するための、レトロトランスポゾンL1の逆転写阻害物質の使用、好ましくは、レトロトランスポゾンL1の逆転写阻害物質が逆転写酵素阻害物質である使用、より好ましくは、逆転写酵素阻害物質がヌクレオシドアナログ逆転写酵素阻害物質である使用に関する。
さらに、本発明は別の態様として、発達障害または精神疾患を処置および/または予防するためのレトロトランスポゾンL1の逆転写阻害物質、好ましくは逆転写酵素阻害物質、より好ましくはヌクレオシドアナログ逆転写酵素阻害物質に関する。
本発明はさらなる別の態様として、発達障害または精神疾患を処置および/または予防するための医薬を製造するための、レトロトランスポゾンL1の逆転写阻害物質の使用、好ましくは、レトロトランスポゾンL1の逆転写阻害物質が逆転写酵素阻害物質である使用、より好ましくは、逆転写酵素阻害物質がヌクレオシドアナログ逆転写酵素阻害物質である使用に関する。
以下、本発明を参考例および実施例により、詳細に説明するが、これらは本発明の範囲を限定するものでなく、単なる例示であることに留意すべきである。
実施例1
MeCP2-KOマウスの表現型に対するレトロトランスポゾンL1の逆転写阻害物質の効果
逆転写阻害物質処置は、次のように行った。
MeCP2-KOマウス(C57BL/6バックグラウンド)は、Mutant Mouse Resource & Research Centers(MMRRC)から入手した(Chen, RZ, Akbarian S, Tudor M and Jaenisch R. Nat Genet 27, 327-331 (2001))。動物の飼育と処置のすべては、九州大学の実験動物管理委員会のガイドラインに従って実施した。
マウスを12時間の明/暗サイクルで飼育し、餌と水は自由に摂取させた。MeCP2-KOマウスを3TC(QA-4615、Combi-Blocks)投与群、d4T(D3580、Tokyo Chemical Industry Co., Ltd.)投与群およびビークル投与群の3群に分けた。マウスには、4週間目から飲料水中、3TC(2 mg/ml)、d4T(2 mg/ml)およびビークルをそれぞれ経口的に投与した。対照として、ビークル投与のMeCP2-KOマウスを使用した。すべてのマウスを毎日観察した。
MeCP2-KOマウスの表現型に対するレトロトランスポゾンL1の逆転写阻害物質の効果
逆転写阻害物質処置は、次のように行った。

3群それぞれのマウス(ビークル・15匹、3TC・10匹、d4T・10匹)において、6つの神経学的症状のスコアを合計し、それを10週まで観察し、記録した。結果を図1に示す。3TCおよびd4T投与群において、有意なレット症状の軽減が見られた。

実施例2
iPS細胞由来の分化神経系細胞における3TCおよびd4Tの効果
ヒトiPS細胞由来の神経系細胞を使用し、ヌクレオシド系逆転写酵素阻害物質3TCおよびd4Tを同時添加すると、細胞の機能障害を改善することを見出した。

ヒトiPS細胞由来の神経系細胞を使用し、ヌクレオシド系逆転写酵素阻害物質3TCおよびd4Tを同時添加すると、細胞の機能障害を改善することを見出した。
詳細には、正常ヒトiPS細胞およびレット症候群患者から入手したiPS細胞由来(MECP2遺伝子が不活性化されているため、以下、MECP2欠損あるいはMECP2-KOと称する)の分化神経系細胞を培養し、皮質ニューロンに分化させた。ニューロスフェア (神経幹細胞塊) に分化させるため、StemPro Accutase (Life Technologies) を使用してNPC(神経前駆細胞)をプレートから剥がした後に、300~500万個の細胞を、bFGF (塩基性線維芽細胞成長因子、R&D Systems) を添加した神経培養(NG)培地(DMEM/F12、1% Glutamax(Life Technologies)、1% N2 Neuroplex(Gemini Bio-products)、2% Gem21 NeuroPlex (Gemini Bio-products) 、1% ペニシリン-ストレプトマイシン)に懸濁し、6ウェルプレート内にて、95rpm、37℃、48時間、振盪培養した。48時間後にbFGFを除いたNG培地に交換し、ニューロスフェアを2週間培養した。iPS細胞由来の皮質オルガノイドは次のように分化させた (Trujillo et al., Cell Stem Cell 25, 558-569 (2019)) 。培養したiPS細胞を、1:1のダルベッコのリン酸緩衝生理食塩水 (DPBS、Fisher Scientific) とStemPro Accutase溶液を使用して培養皿から分離し、6ウェルプレートに移し、懸濁状態を保った。神経誘導培地は、DMEM/F12、1% Glutamax、1% N2 Neuroplex、1% 非必須アミノ酸 (Gibco) 、1% ペニシリン-ストレプトマイシン、1μMのドルソモルフィン (R&D Systems) 、および10μM SB431542 (SB、Stemgent) で構成されていた。細胞増殖培地は、Neurobasal培地 (Life Technologies) 、2% Gem21 Neuroplex、1% 非必須アミノ酸、1% Glutamax、20 ng/mL上皮成長因子 (EGF、Peprotech) 、および20 ng/mL bFGFで構成されていた。ニューロンの成熟には、成長因子を除いた同じ培地にて細胞を保持した。オルガノイドの結果は、4つの別々の分化バッチからオルガノイドの結果を入手し、それを組み合わせた。
分化した神経系細胞を、bFGFを除去後、8週間維持した。MECP2-KO細胞に由来するin vitroニューロンに見られる主な重要な特徴は、シナプスの数が少なく、細胞体のサイズが小さく、ニューロンの形態が変化することである (Marchetto et al., Cell 143, 527-539 (2010))。
次に、得られた分化神経系細胞に3TCおよびd4Tを添加した。培養細胞用の逆転写酵素阻害物質は次のようにして準備した。ラミブジン (3TC、Sigma-Aldrich) をジメチルスルホキシド (DMSO、Sigma Aldrich) で調製し、培地には、終濃度10μMになるように3TCを添加した。スタブジン (d4T、Sigma-Aldrich) は水に懸濁し、終濃度1μMのd4Tを、3TCと組み合わせて使用した。非ヌクレオシド系逆転写酵素阻害物質でありHIV逆転写酵素阻害活性を有しているが、Dai et al., BMC Biochemistry 2011, 12:18においてL1逆転写酵素阻害活性を有していないことが示されていたネビラピン(NVP) (Sigma Aldrich、SML0097) はDMSOに懸濁し、終濃度400 nMのネビラピンを使用した。ネビラピンは対照の細胞(CTRL)には、ビヒクル対照として、10μMのDMSOが含まれていた。得られた結果を図3Aに示す。
患者由来のiPS細胞から誘導したニューロン(MECP2-KO)、すなわちMECP2欠損ニューロンは、正常ヒトiPS細胞から誘導したニューロン(CTRL)と比較して樹状突起の長さおよび分岐点の数が減少し、樹状突起の複雑さが減少した。これに対して、逆転写酵素阻害物質で処理したMECP2欠損ニューロンでは、細胞体の面積と分岐点の数が統計的に有意に増加したのに対し、NVPで処理したMECP2欠損ニューロンではこの回復が認められなかった。NVPで処理しても回復しなかった結果は、既報(Dai et al., BMC Biochem 12, 18 (2011)、 Jones et al., PLoS One 3, 31547 (2008))の通りであった。逆転写酵素阻害物質処理が対照細胞に影響を及ぼしているかどうかを評価するために、それらを慢性的に処理し、形態計測分析を行った。結果、対照細胞を逆転写酵素阻害物質で処理した場合、違いが認められず、これは、これらの抗レトロウイルス薬は対照ニューロンには影響を与えず、RTTの表現型を改善することを示唆している(図3A)。
Soll解析(ショール解析)により、様々な条件における樹状突起の複雑さの違いを調査した。対照ニューロンは、MECP2欠損ニューロン(MECP2-KO)と比較すると有意に複雑である。MECP2欠損ニューロン(MECP2-KO)を逆転写酵素阻害物質で処理した場合、それにより、樹状突起の長さ、分岐点の数、細胞体の面積、およびニューロンの複雑さが回復した。これは、逆転写酵素阻害物質が、MECP2欠損細胞におけるニューロン形態と複雑さの異常を改善できることを示している。さらに、逆転写酵素阻害物質で処理したMECP2欠損ニューロンは、逆転写酵素阻害物質による処理のない場合と比較して、シナプスが形成されていることを示す共局在シナプス斑点 (co-localized puncta) が有意に増加したが、このことは、NVPで処理したMECP2欠損ニューロンでは見られない。逆転写酵素阻害物質で処理した対照細胞では、共局在斑点の増加は観察されなかった(図3C)。
次に、炎症誘発性サイトカインIL6が異常放出しているMECP2-KOアストロサイトに対し、逆転写酵素阻害物質がそのIL6産生能に影響を与えるか調べた。正常ヒトiPS細胞から誘導した対照細胞であるCTRL、MECP2欠損ニューロン(MECP2-KO)およびを逆転写酵素阻害物質で処理したMECP2欠損ニューロン(MECP2-KO)に対し、アストロサイトからIL6産生を誘導することが知られているIL1βを添加48時間後に、IL6の発現量を測定し、比較した。結果を図3Bに示す。MECP2-KO細胞においては、CTRLに比べて6倍近く異常にIL6の発現が上昇する(IL6産生能の異常亢進)のに比べ、逆転写酵素阻害剤で処置したMECP2-KO細胞では、それがCRTLと同じレベルにまで低下(改善)した。
形態およびシナプス形成で観察された変化が神経接続の改善につながるかどうかを完全に理解するため、より複雑な細胞モデルを利用して解析を行った。NPCを3次元の球状細胞塊(スフェロイド)に分化させた。スフェロイドを生成するために使用したNPCを、それぞれの条件で慢性的に処理し、多電極アレイ (MEA) 平板にプレートし、それによりニューラルネットワーク活動を測定した。MEA平板を毎週記録し、4週間後に自発的神経活動を分析した。ラスタープロットは、複数のチャネルにわたるスパイクのタイムスタンプを表示し、これは、MECP2欠損スフェロイドと比較した場合、対照スフェロイドがより多くのスパイクと自発的な神経活動を示すことを示した。したがって、加重平均発火頻度は、MECP2欠損スフェロイドおよびNVP処理スフェロイドにおいてのみ有意に減少する。MECP2欠損スフェロイドを逆転写酵素阻害物質で処理すると、対照レベルに匹敵するレベルまで発火頻度を改善した。対照スフェロイドを逆転写酵素阻害物質で処理した場合、違いは観察されなかった。総合すると、このデータは、MECP2欠損ニューロンで見られる自発的な神経活動の減少が、抗レトロウイルス薬処置で回復することを示している(図3D)。
RTTはしばしば小頭症の表現型と関連しており、その表現型は、脳が適切に発達せず、通常の頭よりも小さくなる。そこで、この3次元モデルで、患者で観察される小頭症の表現型を模倣できるかどうかを調べた。ヒトiPS細胞から分化された3次元皮質オルガノイドは、ヒト新皮質に類似している自発的な自己組織化構造と、胎児中期の出生前のヒト脳に類似した転写プロファイルを示す。分化プロセスの56日後、そのオルガノイドが成熟すると、MECP2欠損オルガノイドは、対照オルガノイドと比較して統計的に有意な直径の減少を示したが、上記逆転写酵素阻害物質処理により、NVPでは見られなかった、皮質オルガノイドのサイズが回復した。対照細胞を逆転写酵素阻害物質で処理した場合、有意な変化は観察されなかった(図3E)。
iPS細胞由来ニューロンの形態分析は、次のようにして行った。
培養NPCについて、EGFPレポーターの上流にシナプシン1(SYN)プロモーターを含む遺伝子をレンチウイルスを用いて形質導入した(Nageshappa et al., Mol Psychiatry 21, 178-188 (2016))。形質導入の感染効率 (MOI) は2とした。SYN::GFPレンチウイルスで形質導入した細胞を、5~6週間の培養でニューロンに分化させ、固定し、GFPおよびCTIP2抗体で免疫染色した。分化したニューロンのトレーシングには、40倍のオイル対物レンズを装着したZeiss Axio Imager 2 顕微鏡とNeurolucida v.2017 (MBF Bioscience、Williston、VT) を使用した。この分析は、GFPおよびCTIP2の両方が陽性であるニューロンのみについて行った。Neurolucida Explorer v.11 (MBF Bioscience、Williston、VT) を使用してニューロンの形態を定量化し、トレースされたニューロンごとのすべての神経突起と樹状突起の長さの合計値を得た。Neurolucida Explorerを利用し、ショール解析を行った。この解析では、細胞体の中心点を指定し、半径が10 μm刻みで増加する同心リングのグリッドをその周りに作成した。ニューロンの複雑さは、各リング内の交差の数を記録することによって決定した。

皮質オルガノイド直径の測定

皮質オルガノイド直径の測定
皮質オルガノイドを、4倍の倍率のEvos FL Imagine System (ThermoFisher) で画像化した。画像は、Image Jにアップロードし、各オルガノイドの直径を、プログラムの長さ測定機能によって測定した。
多電極アレイ(MEA) は、次のようにして行った。
12ウェルMEA平板 (Axion Biosystems) を、100 μg/mLのポリ-L-オルニチン (Sigma-Aldrich) と5 μg/mLのラミニン (Life Technologies) でコーティングした。NPCから生成されるニューロスフェアをMEAウェルの中央に配置し、電極を完全に覆った。ニューロスフェアを、1%FBSを添加したNG培地にプレートした。プレートの1週間後、ニューロスフェアをNeurobasal培地とNG培地を1:1で混合し、1% FBSを含む培地に交換した。プレートの2週間後、ニューロスフェア培地をニューロベーサルのみに変更し、残りの記録のため、この培地に保持した。培地は週に2回交換し、記録は、培地を交換した1日後に測定した。記録は、Maestro MEAシステムとAxISソフトウェア (Axion Biosystems) を使用して週に1回行った。10Hzおよび2.5kHzのカットオフ周波数のバンド-パスフィルターを使用し、スパイク検出器の閾値を、標準偏差の5.5倍に設定した。記録ごとに、記録前に平板をMaestroに3分間そのままにしてから記録し、その後、3分間のデータを記録した。分析には、Axion Biosystems Neural MetricsToolを使用した。活性化電極を検出するための基準は、毎分5スパイクに設定し、バースト電極を検出するための基準は、毎分5バーストに設定した。
12ウェルMEA平板 (Axion Biosystems) を、100 μg/mLのポリ-L-オルニチン (Sigma-Aldrich) と5 μg/mLのラミニン (Life Technologies) でコーティングした。NPCから生成されるニューロスフェアをMEAウェルの中央に配置し、電極を完全に覆った。ニューロスフェアを、1%FBSを添加したNG培地にプレートした。プレートの1週間後、ニューロスフェアをNeurobasal培地とNG培地を1:1で混合し、1% FBSを含む培地に交換した。プレートの2週間後、ニューロスフェア培地をニューロベーサルのみに変更し、残りの記録のため、この培地に保持した。培地は週に2回交換し、記録は、培地を交換した1日後に測定した。記録は、Maestro MEAシステムとAxISソフトウェア (Axion Biosystems) を使用して週に1回行った。10Hzおよび2.5kHzのカットオフ周波数のバンド-パスフィルターを使用し、スパイク検出器の閾値を、標準偏差の5.5倍に設定した。記録ごとに、記録前に平板をMaestroに3分間そのままにしてから記録し、その後、3分間のデータを記録した。分析には、Axion Biosystems Neural MetricsToolを使用した。活性化電極を検出するための基準は、毎分5スパイクに設定し、バースト電極を検出するための基準は、毎分5バーストに設定した。
定量化と統計分析は、次のようにして行った。
技術的な繰り返しにより、標準誤差を決定した。各図には、Nを表示している。データの整理には、標準のスプレッドシートソフトウェア (Microsoft Excelバージョン15.33) を使用した。図のエラーバーは、GraphPad Prism v6 (Graphpad Software Inc) を用いて計算した平均の標準誤差(S.E.M)である。t検定分析では、α=0.05の両側の対応のない検定(two-tailed unpaired tests)を使用した。多重比較の場合、有意差は、Tukeyの多重比較検定を用いてANOVAにより決定した。α=0.05のGrabb検定で、外れ値の判定を行い、有意な値は、解析から除外した。

実施例3
母体免疫活性化(maternal immune activation:MIA)に対するレトロトランスポゾンL1の逆転写阻害物質の効果
実施例3-1:MIAモデルマウスの作製方法
妊娠マウス(C57BL/6NCrl)のプラグ確認日を0日とし、12日目から3日間、対照 (CTRL)としてPBSまたはPBSに溶解したpolyI:C (Sigma, P1530)を 20 mg/kg体重(1回/日)となるよう妊娠マウスの腹腔内に注射器で投与し、産まれてきたオスマウスをMIAマウス(MIA)として使用した。逆転写酵素阻害物質3TCの投与は、高圧蒸気滅菌をした水道水に3TCを2mg/mlの濃度で溶解し、離乳後(4週齢)からマウスに常飲させた。

実施例3-2:社会性を評価する3チャンバーテストを用いたマウスの行動解析
2枚の仕切りで区切られた3部屋の箱を用意した。2枚の仕切りは、マウスが自由に部屋を行き来できるように出入り口が設けられている。中央の部屋の両サイド(右側と左側)の部屋にはマウスを収納できる大きさのケージをそれぞれ置いた。ただし、ケージには出入り口はなく、入れられたマウスはケージの中に入れない。
2枚の仕切りで区切られた3部屋の箱を用意した。2枚の仕切りは、マウスが自由に部屋を行き来できるように出入り口が設けられている。中央の部屋の両サイド(右側と左側)の部屋にはマウスを収納できる大きさのケージをそれぞれ置いた。ただし、ケージには出入り口はなく、入れられたマウスはケージの中に入れない。
まず、第1セッションとして、中央の部屋にテストを行うテストマウスとして、対照(CTRL)マウス、MIAモデルマウスおよび3TCを投与したMIAモデルマウスを置き、それぞれ、5分間自由に移動させ環境に慣らし、その後テストマウスを元の飼育していたケージに戻した。第2セッションとして、箱の右側のケージにテストマウスと同じ月齢のオスマウス(Social target)を入れ、箱の左側のケージは空(Empty)のままにした。5分後、中央の部屋にテストマウスを放し、10分間自由に行動させ、ビデオ解析プログラムにより、それぞれのケージへの接近時間を測定した。通常、マウスは新規のマウスに対して興味を示すため、正常なマウスはテストマウスと同じ月齢のオスマウスを入れた右側のケージへの接近時間が長くなる。得られた結果を図4に示す。MIAモデルマウスでは社会性に異常を示すが、3TCの投与により社会性が改善した。
実施例4
ゴルジ染色を用いたニューロンの細胞体の面積とスパイン密度の定量化
まず8週齢のWT(野生型)マウス、MeCP2-KO(KO)マウス、3TCを投与したMeCP2-KO(KO-3TC)から脳を取り出しFD Rapid GolgiStainTM Kit (FD Neuro Technologies: PK401)を使用して添付のプロトコールに従って、ゴルジ染色を行なった。
具体的には、取り出した脳を蒸留水中でリンスし、24時間前に調製した溶液Aと溶液Bの混合溶液に組織を浸潤し、室温・暗所で2週間保存した。翌日に浸透液を交換した。2週間後、溶液Cに移し、室温・暗所で1週間保管し、浸透液も翌日に交換した。その後、マウス脳を、凍結組織切片作製用包埋剤TFM(ファルマ:303-100-1)を用いて、凍結切片作製用包埋皿クリオモルド3号(三商: 83-2254)に包埋し、-80℃にて凍結させた。凍結させた試料はLeica CM 1900 cryostatを用いて-22℃で100 μmの厚さの切片を作製し、溶液Cが滴下されたゼラチンコーティング顕微鏡スライドにマウントした。切片作製後、室温で自然乾燥させた。切片を蒸留水にて各4分ずつ2回リンスし、その後、溶液D:溶液E:蒸留水=1:1:2の混合液に移し、10分間浸漬した。蒸留水で各4分ずつ2回リンスし、50%、75%及び95%エタノールで4分ずつ、切片を脱水した。その後、無水エタノールで各4分ずつ4回脱水し、キシレンで各4分ずつ3回透徹後、Permount(ファルマ: SP15-100-1)で切片を封入し標本を作製した。
ゴルジ染色を用いたニューロンの細胞体の面積とスパイン密度の定量化
まず8週齢のWT(野生型)マウス、MeCP2-KO(KO)マウス、3TCを投与したMeCP2-KO(KO-3TC)から脳を取り出しFD Rapid GolgiStainTM Kit (FD Neuro Technologies: PK401)を使用して添付のプロトコールに従って、ゴルジ染色を行なった。

マウス脳標本のゴルジ染色画像取得には、キーエンス蛍光顕微鏡BZ-X800を使用した。細胞体面積の定量化は20倍の対物レンズを用いて各マウスの前頭前皮質の領域を、スパイン密度の定量化は100倍の対物レンズを用いて各マウスの前頭前皮質ニューロンの樹状突起を撮影し、Image Jを用いて測定した。得られた結果をそれぞれ、図5Aおよび図5Bに示す。WTと比較しKOマウスでは細胞体の面積とスパイン密度の減少が観察されたが、3TCを投与することでその表現型が改善した。
実施例5
逆転写酵素阻害物質を用いた逆転写酵素の阻害効果
ヒト胎児腎細胞由来の細胞株(HEK293)に逆転写酵素阻害物質3TC、d4Tおよびテノホビル(Tenofovir disoproxil fumarate(TDF)、QA-2704、Combi-Blocks)を終濃度20μMになるように添加し、L1が活性化した細胞だけがEGFPタンパク質を発現するベクター(L1-EGFP, Macia et al. Genome Res 27, 335-348 (2017))を導入した。4日間培養後、GFPに対する抗体と核を染色するHoechestを用いて免疫染色を行い、蛍光顕微鏡(Leica AF600)を用い10倍の対物レンズを使用して撮影した。逆転写酵素阻害物質の投与によりEGFP陽性細胞の割合が減少することがわかり、逆転写酵素の阻害により、L1の活性が阻害されることが確認できた。

- レトロトランスポゾンL1の逆転写阻害物質(非ヌクレオシド系逆転写酵素阻害物質ネビラピンを除く)を含有する、発達障害または精神疾患を処置および/または予防するための医薬組成物。
- レトロトランスポゾンL1の逆転写阻害物質が、逆転写酵素阻害物質である、請求項1記載の医薬組成物。
- 逆転写酵素阻害物質が、ヌクレオシドアナログ逆転写酵素阻害物質である、請求項1または2記載の医薬組成物。
- ヌクレオシドアナログ逆転写酵素阻害物質が、ラミブジンまたはスタブジンである、請求項1から3までのいずれか記載の医薬組成物。
- 発達障害が、レット症候群である、請求項1から4までのいずれか記載の医薬組成物。
- 発達障害が、自閉症スペクトラム障害である、請求項1から4までのいずれか記載の医薬組成物。
- 精神疾患が、統合失調症である、請求項1から4までのいずれか記載の医薬組成物。
