Description

GNATHIC DEVICE
The invention relates to medicine, more specifically to dentistry, and is intended to be used in gnathic dentistry, as well as for separation of dental arches during medical procedures in the oral cavity.
A conventional device, AQUALIZER muscle deprogrammer, a hydrostatic splint for muscle relaxation, comprises a body made of medical silicone in the form of a splint provided with cavities at the ends (see flexiligner.ru/shop/ produktsiya).
A soft and elastic custom-made splint is automatically adjusted to the patient's anatomical features and easily conforms to the shape of the patient's teeth. Cavities of the splint are filled with distilled water, thereby preventing an active effect on the masticatory muscles due to constant pressure and constant amount of water in the splint cavities, and this is a drawback of the device. In addition, the device is unsuitable for separation of dental arches during medical procedures in the oral cavity.
Another conventional device, LogiUnit bite unit for fixing the jaw in open position during manipulations in the patient's oral cavity and reducing tension in the temporomandibular joint, which is considered as the most relevant prior art, is made in the form of a body of medical silicone, having an occlusal relief along the outer surface and edges bent outward of the body to abut on the dental arch (see shop-dent.ru/category- 0 -...).
The body has the shape of an U-curved tape.
The drawbacks of the LogiUnit bite unit are that it does not provide a relaxing effect on the masticatory muscles and is unsuitable as an active muscle trainer for myogymnastics.
The technical task and the result offered by the present invention include expanded functionality of the device owing to an adjustable and programmable effect on the masticatory muscle hypertonicity, removal of the pain effect in the muscles, elimination of the temporomandibular joint dysfunction, and the ability to separate dental arches during medical procedures in the oral cavity.
The technical result is achieved by a gnathic device comprising a body made of medical silicone and having an occlusal relief along the outer surface and edges bent outward of the body to abut on the dental arch, wherein the device is detachable and consists of two occlusal bite units joined together by a vestibular arch having a fixing relief on the opposite sides, ends of the vestibular arch being mounted in loops rigidly secured to the unit bodies; wherein body of each unit is made in the form of a truncated prism with a cavity inside and a through hole in the larger base of the prism to arrange an air valve, whose housing is made in the form of a cylinder with an axial through opening, and one end of the air valve has flanges along the outer surface to be fixed relative to the unit hole, and the other end comprises an adjusting ring mounted on the outer surface and an internal thread to connect to an air supply source and/or to mount a locking means; wherein stationary vanes are arranged inside the air valve, perpendicular to its axis, the stationary vanes being made in the form of mirror-like arranged sectors rigidly connected in arc to the valve housing, and movable vanes made in the form of joined mirror-like arranged sectors rotatably connected to the stationary vanes; wherein the movable vanes are further connected to the adjusting ring through a rod arranged in a through groove made in the air valve body and enabling rotation of the movable vanes by 90 degrees. The air valve body is made of SBS plastic. The arc of the stationary vanes is larger than that of the movable vanes. The unit body and the adjusting ring of the air valve comprise an indicator in the form of alignable colored stripes for visual observation of position of the movable vanes.
The gnathic device is illustrated in the drawings, where:
Fig.l is a general view of the device;
Fig. 2 is longitudinal section A- A in Fig.l;
Fig. 3 is section B-B in Fig. 2.
The device is detachable and consists of two occlusal separating bite units 1 and 2 joined together by a vestibular arch 3 whose ends are mounted in loops 4 rigidly secured on bodies of the units 1 and 2. Opposite sides of the arch 3 have a fixing relief that holds the arch 3 in the loops 4 when position of the units 1 and 2 relative to the patient's dental arch is adjusted (fixing relief is not shown).
The components of the device are made of transparent or multi-colored medical silicon, safe for human health.
Body of each unit 1 and 2 is made in the form of a truncated prism with a rectangular section, a cavity 5 inside and a through hole in the larger base of the prism (the hole is not shown). Opposite sides of each unit 1 and 2 body have an occlusal relief 6 and edges 7 bent outward of the body to abut on the dental arch and create retention on the dental arch.
The through hole in the prism larger base is intended to install an active air compression limiter configured as an air valve 8 made of SBS plastic by 3D printing.
Housing of the air valve 8 is a cylinder with an axial through opening 9. One end of the valve 8 has flanges 10 (annular protrusions) on the outer surface to fix the valve 8 relative to the body hole of the unit 1 or 2. The other end of the valve 8 has an internal thread to connect to an air supply source or to mount a locking means that prevents air escape from the cavity 5, and an adjusting ring 12 mounted on the outer surface of the valve 8.
The air supply source may be, for example, an air inflation bulb, similar to the rubber inflation bulb of a traditional blood pressure meter, for creating a required pressure in the device cavity 5 (the bulb is not shown). Furthermore, to determine pressure in the system, a pressure sensor (manometer) is mounted in lines coming from the air inflation bulb. The locking means may be a plug with a thread on the outer surface (the plug is not shown).
Side surface of the adjusting ring 12 has a through hole to mount a rod 13, and the housing of the air valve 8 has a through groove 14 for movement of the rod 13. One end of the rod 13 is connected to the adjusting ring 12, while the other end is connected to movable vanes 15. The vanes 15 are configured as joined mirror-like arranged sectors (located in the same plane) that are placed in the opening 9 of the valve 8 and adapted to rotate by 90 degrees in the plane parallel to the position of the stationary vanes 16 (in the plane perpendicular to the valve 8 axis).
Mounted on the inner side of the valve 8 are stationary vanes 16, similar to the vanes 15 in shape and arrangement. The stationary vanes 16 are secured in an arc on the air valve 8 housing (made integral with housing of the valve 8). The vanes 15 are rigidly connected to an axle 17 rotatable relative to the vanes 16. The arc of the stationary vanes 16 has a larger size than that of the movable vanes 15 to exclude the appearance of an air escape gap from the valve cavity 8 in the "valve closed" state.
For visual observation of position of the vanes 15, the body 1 and the adjusting ring 12 are provided with an indicator, such as colored stripes, which when aligned indicate that the rod 13 is at the beginning of the groove 14 in "valve closed" state (the indicator is not shown).
The device is used in the following manner.
To perform a therapeutic effect on masticatory muscles in a case of a pathology such as parafunction or masticatory muscle hypertonicity, the device is placed into the oral cavity, arranging the units 1 and 2 on the masticatory teeth group with a thrust of the bent outward edges 7 of the body 1 on the dental arch, and the length of the arch 3 is adjusted to fix it in the loops 4 so that the arch 3 bends around the dental arch.
Then, the air inflation bulb is connected to the device by screwing a hose into the threaded hole 11 to change atmospheric pressure in the cavity 5 of the body 1.
When the valve 8 is in closed position, the vanes 15 and 16 cross each other with 90 degrees shift and completely cover the opening 9 of the valve 8. When rotating the adjusting ring 12, the movable vanes 15 are simultaneously rotated by the rod 13 relative to the vanes 16, thereby forming an air ingress gap into the cavity 5 of the body 1. Using the bulb, air is injected into the cavity 5 of the body 1 with a specified frequency and amplitude, regulating the effect on the masticatory muscles or the height of separation of dental arches through creation of a specified pressure in the cavity 5 of the device.
During the use of the device for therapeutic effect on masticatory muscles, air can freely circulate from the device cavity to the air bulb cavity and back.
To ensure an equivalent effect on the masticatory muscles, air is injected into cavities 5 of units 1 and 2 at the same time.
Increasing pressure in the cavity 5 of the body 1 will enable direct affect on the masticatory muscles. Force of the pressure and time of its effect are selected for each person individually, depending on the pathological picture.
To fix the volume of opening the mouth, one of the units 1 or 2 is used, which is previously removed from the arch 3. Air is injected into the cavity 5 of unit 1 or 2 with the required number of atmospheres, depending on the tonus of patient's masticatory muscles, the vanes 15 and 16 are moved apart by the ring 12, closing the air valve 8, the air supply source is disconnected and a plug is additionally installed to maintain pressure inside the device. To fix the volume of opening the mouth, the unit 1 or 2 is mounted on the side opposite to the place of medical manipulations. The inventive device design enables its use as a device for muscle relaxation, a muscle training device or a separating bite unit.
