アルツハイマー病等の遺伝子検査には多大な労力やコストがかかることが知られるが、本発明のバイオマーカーによれば、アルツハイマー病の罹患可能性についての簡便な診断が可能となり得る。
したがって、本発明のバイオマーカーによれば、軽度認知障害(MCI)の罹患可能性についての簡便な診断が可能となり得る。
本発明のバイオマーカーは、認知機能障害の細分類化のために好適に用いることができる。
そのため、本発明のバイオマーカーによれば、病因遺伝子に応じたアルツハイマー病の分類が可能となる。

本発明に係る、認知機能障害の治療又は改善手段を被験者に提示する方法(以下、「本発明の提示方法」ともいう。)は、被験者から採取した血液試料における本発明のバイオマーカー量の入力情報と、既存の正常範囲に関する既知情報とに基づき、正常範囲へと近づけることが既知の手段を被験者へと提示する工程を有する。
本発明に係る、認知機能障害の治療又は改善物質のスクリーニング方法(以下、「本発明のスクリーニング方法」ともいう。)は、候補物質を動物に投与したときの血液試料における本発明のバイオマーカー量の変化を指標として、候補物質を選抜する工程を有する。
以下の方法により、軽度認知障害(MCI)の患者を特定し、これらの患者から得られた試料に基づき、認知機能障害のマーカーを探索した。
弘前市岩木町住民(約1200名)を対象として、以下の1次スクリーニングを行った。
(1-1)アンケートにおいて、本人及び家族ともに、本人に認知機能障害があることを認める。
(1-2)MMSE検査(Mini Mental State Examination)における問題5「短期記憶(遅延再生):相互に無関係な物品名を3個聞かせ、それをそのまま復唱させる。1個答えられるごとに1点。全てについて答えられなければ6回まで繰り返す。次いで、この物品名を再度復唱させる。」(3点満点)で点数が2点以下だった。
(1-3)WMS-R ウエクスラー記憶検査II(遅延再生能力を評価するための試験であり、25の鍵となる用語から構成される2つの文章を被験者に読み聞かせ、その直後、及び30分後にその内容を思い出させる。鍵となる用語を言えた数が点数となる。)が以下の基準を満たす。
教育年数16年以上:8点以下
教育年数10~15年:4点以下
教育年数9年以下:2点以下
(1-4)上記(1-1)乃至(1-3)を全て満たした者を2次スクリーニングに供した。
上記1次スクリーニングで特定された、軽度認知障害を疑われる患者を対象として、弘前大学病院神経内科において、以下の2次スクリーニングを行った。
(2-1)CDR(Clinical Dementia Rating)
患者の家族へのアンケートに基づき、記憶、見当識、判断力及び問題解決、社会適応、家族状況及び趣味/関心、並びに介護状況の6項目について、5段階で軽度認知障害(MCI)の重症度を評価した。
評価結果を総合して、健康(CDR:0)、認知症の疑い(CDR:0.5)、軽度認知症(CDR:1)、中等度認知症(CDR:2)、高度認知症(CDR:3)のいずれかに評定した。
(2-2)血液検査
通常の血液検査で採用される方法により、一般的検査、甲状腺、及びビタミンB12の評価を行った。
[甲状腺の検査]
甲状腺ホルモンの分泌が低下した状態が続くと認知機能が低下することが知られる。検査にあたっては、血液中の遊離サイロキシン(FT4)、遊離トリヨードサイロニン(FT3)、及び甲状腺刺激ホルモン(TSH)の濃度を測定した。評価基準は下記のとおりである。
FT4基準値:上限値1.7ng/dL、下限値0.9ng/dL、この値が高ければ甲状腺機能亢進症であり、低ければ甲状腺機能低下症であると評価した。
FT3基準値:上限値4pg/mL、下限値2.3pg/mL、この値が高ければ甲状腺機能亢進症であり、低ければ甲状腺機能低下症であると評価した。
TSH基準値:上限値5uIU/mL、下限値0.5uIU/mL、血液中の甲状腺ホルモンが高値になるとこの値は低下し、血液中の甲状腺ホルモンが低値になるとこの値は増加する。
[ビタミンB12の検査]
ビタミンB12や葉酸の欠乏によってホモシステインが蓄積し、アルツハイマー病を引き起こし得る高ホモシステイン血症を生じることが知られる。検査にあたっては、血液中のビタミンB12、及び葉酸の濃度を測定した。評価基準は下記のとおりである。
ビタミンB12基準値:上限値914pg/ml、下限値180pg/ml
葉酸基準値:下限値4ng/ml
(2-3)神経学的検査
医師により、反射神経、平衡感覚、眼球運動、発語、及び知覚検査に関する検査を行った。
(2-4)問診
医師により、患者への問診を行った。
(2-5)3テスラMRI、及びVSRADによる解析
3テスラMRIによる脳の形態画像検査を行い、VSRADによる解析を行った。なお、VSRADはMRIを用いた早期アルツハイマー型認知症診断支援システム、ソフトウェアである。
解析により、アルツハイマー病患者において萎縮が最も早く生じることが知られる海馬傍回付近の状況を評価した。海馬傍回の萎縮の程度はZ-スコア(各種認知機能と逆相関する)で示され、健常者では1.0以下である。
(2-6)上記(2-1)乃至(2-5)の結果に基づき、うつ、せん妄、薬剤性、その他内科学的身体疾患に伴うもの(甲状腺や副腎機能の低下、低血糖、ビタミン欠乏)、てんかん、脳外科学的疾患(慢性硬膜下血腫、脳腫瘍、正常圧水頭症等)との鑑別を行ったうえで軽度認知障害(MCI)の患者を診断した。
2次スクリーニングの結果、軽度認知障害(MCI)であると診断された患者から血液を、ベノジェクトII真空採血管(抗凝固剤EDTA-2Na、テルモ社製)を用いて採取した後、遠心分離により血漿を調製し-80℃にて凍結保存した。PAXgene RNA採血管(日本ベクトン・ディッキンソン社製)を用いて血液を採取した後、-80℃にて凍結保存した。
また、対照として、健常者(少なくとも、1次スクリーニングで行ったMMSE検査のスコアが29点以上で軽度認知障害(MCI)が全く認められなかった者)からも同様に血漿サンプルを得た。
各検体の詳細は表6に示すとおりである。
各血漿から市販のキットを用いてRNA抽出した。次いで、RNAをCy3でラベル標識し、Agilent Human miRNAマイクロアレイとのハイブリダイゼーションを行った。洗浄後、SureScanスキャナーにて蛍光画像を取り込み、発現を統計解析(t検定、p<0.05)した。
(1)p値が0.05以下であり、かつ変化量が大きい。
MCIで増加したmiRNA=log2(Fold change)>2
MCIで減少したmiRNA=log2(Fold change)<-1.5
(2)病理学的に重要な遺伝子をターゲットにしているmiRNAである。
(3)分析精度の指標であるAUC(Area Under the Curve)が0.9を超える。
軽度認知障害(MCI)患者及び対照(上記実施例1における患者及び対照とは異なる。)から試料を採取し、実施例1と同様の方法でmiRNAのマイクロアレイ解析を実施した。
本例では、実施例1よりも検体数が少ない場合であっても、実施例1において検討されたmiRNAが、認知機能障害(特に、軽度認知障害(MCI))のバイオマーカーとして機能するかを確認することを目的として実施した。
対照は、健常者(1次スクリーニングで行ったMMSE検査のスコアが29点以上で軽度認知障害(MCI)が全く認められなかった者)である。
試料は、患者及び対照から採取した血液から、実施例1と同様の方法で調製及び保存した血漿サンプルを用いた。
各検体の詳細は表12に示すとおりである。
実施例1において検討されたmiRNAのうち、表13に示される17種類のmiRNAは、特に良好な再現性を示した。
さらに、表13に示されるmiRNAのうち、「hsa-miR-2278」、「hsa-miR-32-3p」、「hsa-miR-328-3p」(実施例1において「重要度」が「***」と評価されたmiRNA)は、顕著に高い精度、及び、良好な再現性を示し、認知機能障害(特に、軽度認知障害(MCI))のバイオマーカーとして特に良好に機能することが確認された。
<mRNA解析>
実施例1でPAXgene RNA採血管を用いて得られた血液からPAXgene Blood RNA(QIAGEN社製)キットを用いてtotal RNAを抽出した。次いで、「SureSelect strand-specific RNA sample prep kit」(Agilent社製)を用いて、total RNAからシークエンスライブラリを調製した。得られたライブラリを定量化した後に、「HiSeq 2500 sequencing system」(Illumina社製)を用いてシークエンスを行った。
得られた結果に基づき、「STAR」(version 2.5.3a)及び「RSEM」(version 1.3.0)を用いてマッピング及び発現解析を行い、STAR-RSEMによる情報解析(q<0.05)によって、軽度認知障害(MCI)であると診断された患者において特異的に発現が変化する遺伝子を特定した。特定された遺伝子についてパスウェイ解析も行った。また、シークエンスで得られた結果に基づきGene Set Enrichment解析を行い、特定された遺伝子についてt検定を行った。
<プロテオーム解析>
以下の方法に基づき、実施例1で得られた各血漿中のペプチドについてプロテオーム解析を行った。
各血漿を、「Multiple Affinity Removal Spin Cartridge Human 14」(Agilent社製)キットを用いて、血液中の主要タンパク質(14種類)を除去した。
次いで、各血漿30μlに、キットに付属する「Buffer A」を570μl加え、0.22μmフィルターでろ過後、ろ液200μlをアフィニティーカラムに添加し、1,000rpm(100g)で1分間の遠心分離を行った。通過溶液を回収し、アフィニティーカラムを室温に5分間置いた。
400μlのBuffer Aをアフィニティーカラムに加え、1,000rpm(100g)で2分間の遠心分離を行った。通過溶液を回収し、アフィニティーカラムに400μlのBuffer Aを加え、1,000rpm(100g)で2分間の遠心分離を行い、通過溶液を回収した。
回収した溶液を集積してマイナータンパク質画分とした。これを限外ろ過(「Amicon Ultra 3kDa」、Merck Millipore社製)し、脱塩して凍結乾燥した。
次いで、100mMアンモニウムバイカーボネート溶液を12.5μl加えて溶解した。トリフルオロエタノールを12.5μl加えた。200mM DTTを0.5μl加え、90℃に30分間置いて還元反応を行った。反応液に200mMヨードアセトアミドを2μl加え、室温、暗所にて1時間置くことでアルキル化反応を行った。
反応時間経過後、200mM DTTを0.5μl加えて、室温、暗所にて1時間置くことで反応を停止した。
反応液に50mM アンモニウムバイカーボネート溶液を200μl加え、そのうち100μlにトリプシン(1μg/μl、TPCK treated、AB Sciex社製)を1μl加え、37℃に18時間置いた。
反応時間経過後、トリフルオロ酢酸を1μl加えて反応を停止した。
得られた反応物を「ZipTip」(Merck Millipore社製)により脱塩し、ペプチドの精製を行った。「SpeedVac」により乾固した後、20μlの0.1%ギ酸に溶解させ、得られたペプチド試料を液体クロマトグラフィー質量分析(LC-MS/MS)に供した。
ペプチド試料を、ナノフロー液体クロマトグラフィー(「nanoLC Eksigent 400 system」、Eksigent社製)を用いて、ペプチド分画し、溶出液を質量分析器によりMS/MS測定した。
ペプチド分画のカラムにはトラップカラム(「nano trap column」、350μm×0.5mm、3μm、120オングストローム、AB Sciex社製)を接続したC18逆相カラム(「nano ChromXP C18 reverse phase column」、75μm×15cm、3μm、120オングストローム、AB Sciex社製)を用いた。
質量分析器は「TripleTOF6600 mass spectrometer」(AB Sciex社製)を用いた。
流速は300nl/minに設定し、90分間で0.1%ギ酸を含む8~30%のアセトニトリル直線濃度勾配、続いて10分間で30~40%のアセトニトリル直線濃度勾配によりペプチドを溶出した。
質量分析では、まずinformation-dependent acquisition(IDA)によるデータ取得を行った。MSスペクトルスキャンの設定は250ms間で400~1500m/zとした。MS/MSスペクトルスキャンの設定は50msごとに100~1800m/zとして、30回以上のMS/MSスペクトルを取得した。MSとMS/MSスペクトル取得のサイクルタイムは1.8sとした。+2から+5の電荷を持ち、1秒間あたり125カウント以上のイオンについてMS/MSスペクトル取得した。また、MS/MSスペクトルを取得したイオンについて、12秒間は同じイオンを選択しない設定とした。
質量分析器は「Analyst TF 1.7.1ソフトウェア」(AB Sciex社製)により操作した。「data dependent acquisition」(DDA、SWATH acquisition)ではTOF MSのスキャンを100msとし、400~1250m/zの範囲を200m/zごとに区切り50msで測定した。各m/z区間のオーバーラップは1Daとした。これを1サイクルとして9.6sで測定した。
IDAにより取得されたMS/MSスペクトルデータについて、「ProteinPilot 5.0.1ソフトウェア」(AB Sciex社製)を用いて、UniProtタンパク質データベース検索を行った。検索対象の種はHomo sapiensとし、偽陽性率解析(FDR analysis)により得られたタンパク質同定の閾値を>0.05(10% confidence)に設定した。
1%未満のlocal FDRを満たすことが同定されたペプチドについて、「MarkerViewソフトウェア」(version 1.3.0.1、AB Sciex社製)を用いてピーク面積値を算出した。総ピーク面積値により各ピークを標準化したデータを多変量解析に用いた。主成分分析(PCA)と判別分析(OPLS-DA)には「Simcaソフトウェア」(Infocom Corp社製)を用いた。paretoスケーリングによる標準化を適用したデータを解析した。
判別分析により、MCI群と対照群との間で変動が見られたタンパク質の機能分類を「Gene ontology enrichment analyses」(Gene Ontology Consortium社製)により行った。
t検定の結果p値が0.05以下であるもの、又は、p値が0.05より大きいものでも、判別分析(orthogonal partial least square-discriminant analysis)の結果、p[corr](相関比)が>0.5、あるいは<-0.5であるものを探索した。
(1)スピアマンによる相関解析した際に、分析精度の指標であるAUCが0.8以上であり、それぞれのクラスターを代表するタンパク質である。
(2)病理学的に、認知機能障害(特に、軽度認知障害(MCI))と関連のある遺伝子型と相関が判別分析により認められた。
<メタボローム解析>
実施例1で得られた各血漿に対し、市販のキット(「AbsoluteIDQ p180 kit」及び「AbsoluteIDQ p400 kit」、BIOCRATES Life Sciences AG社製)を用いて、以下の前処理を行った。内部標準液血漿10μLをウェルに加え、これに各血漿サンプル10μLを添加した。次いで、窒素ガスを30分間吹き付けて乾燥させた。5% PITCで誘導体化反応を20分間行った。窒素ガスを60分間吹き付けて乾燥させた。抽出用有機溶媒を300μL加え、450rpmで30分間振盪し、前処理された血漿サンプルを得た。
加圧式マニホールドにより、フィルターろ過した抽出液150μLを血漿サンプルに加え、質量分析(LC-MS測定(「Xevo TQ-S」、Waters社製))によりアミノ酸類、カルニチン・アシルカルニチン類等の測定を行った。
脂質類の測定にはFIA用のrunning solventを400μL添加して質量分析(同上)により測定した。
t検定の結果、p<0.05であるものを探索した。
(1)AUCが0.8以上である。
(2)病理学的に、認知機能障害(特に、軽度認知障害(MCI))と関連のある遺伝子型と相関が認められた。
<複数のバイオマーカーを用いた検討>
本発明のバイオマーカーのうち、下記を用いて、認知機能障害(特に、軽度認知障害(MCI))の診断モデル性能をROC曲線(Receiver Operatorating Characteristic curve)により評価した。より具体的には、本発明のバイオマーカーと、認知機能障害(特に、軽度認知障害(MCI))との相関についてロジスティック回帰分析を実施し、分析結果のp値とAUCの値に基づき検討した。
(1)(D)SM(OH)C22:1のみ
(2)(C)ゲルゾリン(GELS)及び(D)SM(OH)C22:1の組み合わせ
(3)(C)ゲルゾリン(GELS)及び2種の(D)(SM(OH)C22:1、及びL-アラニン(Ala))の組み合わせ
図1に示されるとおり、本発明のバイオマーカーを複数用いると、AUCの値がより高くなり、より正確な診断が可能となることがわかった。
<本発明のバイオマーカーと既知危険因子との相関>
アルツハイマー病の危険因子として、遺伝子「ApoE4」が知られる。上記2次スクリーニングの結果、ApoE4の遺伝子型を持つ検体の多くが軽度認知障害(MCI)であると診断された。
そこで、ApoE4と、本発明のバイオマーカー((A)hsa-miR-30d-5p、(C)アディポネクチン(ADIPOQ)、(D)SM(OH)C24:1)との相関を、判別分析(OPLS-DA)を用いて検討した。
<本発明のバイオマーカーと、軽度認知障害(MCI)との相関>
上記<既知危険因子と本発明のバイオマーカーとの相関>と同様の方法で、軽度認知障害(MCI)であると診断された患者及び健常者(Control)における、本発明のバイオマーカー((A)hsa-miR-30d-5p、(C)アディポネクチン(ADIPOQ)、(D)SM(OH)C24:1)の発現を検討した。
<認知機能障害の細分類化>
アルツハイマー病の病因としては種々知られるが、そのひとつとして、アルツハイマー病患者の脳に認められるアミロイド斑の主成分である「Aβ42」が知られる。Aβ42はAβ speciesの1つであり、健常者においては「Aβ40」が認められる。
上記を踏まえ、本発明のバイオマーカーと、Aβ42及びAβ40との相関を、判別分析(OPLS-DA)を用いて検討した。
<バイオマーカーの測定-1>
上記<miRNAのマイクロアレイ解析>で特定されたバイオマーカーのうち、特に重要度が高かった「hsa-miR-2278」に着目し、表6に示した被験者と異なる被験者から得た血漿サンプルにおける発現をリアルタイム定量PCRによって測定した。なお、検体を得た被験者は、軽度認知障害(MCI)であると診断された患者(MCI群)、又は健常者(対照群)である。血漿サンプルは、MCI群24検体、及び対照群24検体を準備した。
リアルタイム定量PCRの方法は、以下のとおりである。
血漿(200μL)から、市販のキットを用いてRNA抽出した。次いで、抽出したRNA(2ng)を用いて以下のcDNA合成反応を行った。
まず、RNAキャリアとしてバクテリオファージ由来の「RNA MS2」(MilliporeSigma社製)を3ng添加した。スパイクインとして「cel-miR-2-3p」を4.8×107コピー添加した。
cDNAの合成には「TaqMan MicroRNA Reverse Transcription Kit」(ThermoFisher Scientific社製)を用いた。
合成したcDNAについて、「THUNDERBIRD Probe qPCR Mix」(TOYOBO社製)を用いて、リアルタイム定量PCRを行った。PCR反応は、384ウェルのPCRプレート(Bio-rad社製)において行った。95℃で1分間インキュベートした後、95℃で15秒間と60℃で1分間のPCR反応を45サイクル行った。
反応の測定には、「CFX384 Real-Time System C1000 Touch Thermal Cycler」(Bio-rad社製)を用いた。
スパイクインとして添加した「cel-miR-2-3p」の測定値で各検体のmiRNA測定値を補正し、対照群の平均値に対するMCI群の平均値の相対値を算出して比較を行った。
<バイオマーカーの測定-2>
上記<軽度認知障害(MCI)の患者の特定-1次スクリーニング>で採用したMMSE検査(Mini Mental State Examination)は、スコアが同値である被験者であっても、軽度認知障害(MCI)の患者、及び健常者が混在している可能性があり、精度の高い検査方法ではない。
そこで、MMSE検査のスコアが同値である被験者について、本発明のバイオマーカーによってMCI患者と健常者とを区別できるかを検討するため、以下の測定を行った。
(2)上記(1)で特定された被験者について、上記<軽度認知障害(MCI)の患者の特定-2次スクリーニング>と同様の方法で2次スクリーニングを行い、被験者がMCI患者であるかどうかを診断した。
(3)上記(1)で特定された被験者から血漿サンプルを得て、「ωScan」及び「MScan」(いずれもヒューマン・メタボローム・テクノロジーズ株式会社による)を行い、本発明のバイオマーカーの発現解析を実施した。
「ωScan」とは、イオン源アダプタ(日本特許6106864号)を用いたヒューマン・メタボローム・テクノロジーズ株式会社による解析サービスであり、試料中の水溶性物質の発現量等を特定する技術である。
「MScan」とは、脂質メディエーター(オキシリピン類、リゾリン脂質類、ステロイド類等)を対象としたヒューマン・メタボローム・テクノロジーズ株式会社による解析サービスであり、試料中の脂溶性物質の発現量等を特定する技術である。
実施例1の2次スクリーニングの結果、被験者は表27に示される3群(重度のMCI、軽度のMCI、又は健常者)に分類することができた。
各検体における本発明のバイオマーカーの発現を、表27に示される3群ごとに確認した。その結果、本発明のバイオマーカーは、MCI患者(特に重度のMCI患者)において、顕著に発現量が増加(又は低下)した。結果の一部を図5及び6に示す。なお、図5及び6の縦軸は、相対面積値(各ピーク面積値を、内部標準物質及び試料量で補正した数値)を意味する。
<バイオマーカーの測定-3>
実施例11と同様の試料を用いて、(ωScanについて)と同様の方法で、本発明のバイオマーカーの発現解析を実施した。
得られた結果を、統計解析ソフトウェア「SPSS Statics」(IBM社製)を用いて解析し、ROC(Receiver Operating Characteristic)曲線を作成した。その結果を図7に示す。

- 前記認知機能障害は軽度認知障害である、請求項1から3のいずれかに記載のバイオマーカー。
- 少なくとも(A)から選択される1種以上を含み、認知機能障害の罹患可能性の検査に用いるための、請求項1から4のいずれかに記載のバイオマーカー。
- 少なくとも(A)乃至(D)から選択される1種以上を含み、認知機能障害の細分類化に用いるための、請求項1から4のいずれかに記載のバイオマーカー。
- (A)乃至(D)から選択される1種以上を含む請求項1から4いずれかに記載のバイオマーカーに選択的に結合可能な物質を含む、認知機能障害バイオマーカーの検出薬。
- 被験者から採取した血液試料における請求項1から4いずれかに記載のバイオマーカーを検出し、その結果に基づいて被験者の認知機能障害を検査する方法。
- 請求項5に記載のバイオマーカーを検出し、その結果に基づいて被験者の認知機能障害の罹患可能性を検査する請求項7に記載の方法。
- 請求項6に記載のバイオマーカーを検出し、その結果に基づいて被験者の認知機能障害を再分類化する請求項7に記載の方法。
- 認知機能障害の治療又は改善手段を被験者に提示する方法であって、
被験者から採取した血液試料における請求項1から6いずれかに記載のバイオマーカー量の入力情報と、既存の正常範囲に関する既知情報とに基づき、正常範囲へと近づけることが既知の手段を被験者へと提示する工程を有する方法。 - 認知機能障害の治療又は改善物質のスクリーニング方法であって、
候補物質を動物に投与したときの血液試料における請求項1から6いずれかに記載のバイオマーカー量の変化を指標として、候補物質を選抜する工程を有する方法。
