Abstract
A dental prosthetic device for dental crowns which are fixable to bone implants through a suitable abutment (1), with a transverse anchoring screw (V), comprising: a cylindrical insert (3), which is insertable into the tubular body of the abutment (1) after its fixation to the head of the endosseous implant, which insert (3) is provided with an additional transverse threaded hole (4) placed at a transverse hole (2) present in the abutment itself; wherein the conical transverse screw (V) is configured to engage both the thread of the hole (2) in the abutment (3) but also and above all the thread of the corresponding additional transverse hole (4) of the cylindrical insert (3). The length of said hole can cover up to the entire diameter of the insert itself, so that the conical screw (V) is firmly screwed and gripped in the transverse hole (4) over the whole length of the stem of the screw itself.
Description
DENTAL PROSTHETIC DEVICE FOR DENTAL CROWNS WHICH ARE FIXABLE TO BONE IMPLANTS. WITH TRANSVERSE ANCHORING SCREW
DESCRIPTION
The present invention relates to the field of orthodontics and dentistry, and in particular to an innovative dental prosthetic device for dental crowns which are fixable to bone implants.
The term “abutment” in dentistry identifies the detached and emerging component from the gums of an endosseous implant. The abutment is the support of the superstructure which replicates the natural tooth, for example the crown of a single element.
Normally, in modern implantology it can be coupled to the implant screw immediately, in case of “immediate loading” or after an appropriate healing time necessary to ensure the osseoi ntegrati on of the implant screw. The abutment-screw fixing, in two-component systems, takes place by cementing or, preferably, by screwing with a suitable transverse fixing/anchoring screw with a conical head, referred to as “conical screw” (also referred to as retention screw).
In implant systems currently in use, the coupling system must:
reduce the probability of relative movements between abutment and screw-implant.
support and distribute the dynamic stresses received by the masticatory force for
several years.
reduce the likelihood of fluid infiltration, which in many cases is the determining factor in triggering inflammatory processes and the consequent loss of the implant due to detachment from the bone.
- offer an acceptable aesthetic result with reference to the part of the abutment surrounded by soft gum tissue.
It is worth noting that the abutment, by its nature, consists of a substantially tubular body: indeed, the abutment is fixed to the head of the endosseous implant by means of a substantially longitudinal connection screw which passes through a special cavity present in the abutment i tsel f .
According to the above, the aforementioned conical screw, for transverse fixing of the dental crown to the abutment, is screwed into a threaded hole specially prepared on the lateral wall of the tubular body of the abutment itself.
It is apparent that given the size of the objects of interest, the thickness of the wall in which the hole is present is very small, of the order of a millimeter, and in any case comparable with the diameter of the conical screw stem (!). Therefore, in the hole there can be, at most, one or two threads with which the screw can cooperate .
A first problem of this type of technique for fixing the crown to the abutment is that only
one or two threads of the threading of the transverse conical screw can engage the internal thread of the lateral hole present in the abutment .
A second problem is that the space in which the dentist has to work, inside the patient's mouth, is notoriously very small and difficult to reach, so it can occur that when tightening the conical screw - which is often not perfectly aligned with the threaded hole - the thread of the hole is irreparably damaged, compromising the tightness of the fixing.
It is the main object of the present invention to overcome the aforementioned problems and drawbacks of the prior art, by increasing the length of the transverse thread in which the conical screw can be screwed when fixing the dental crown.
This has been achieved, according to the invention, by providing a cylindrical insert, which is insertable into the tubular body of the abutment after it has been fixed to the head of the endosseous implant, which insert has a transverse threaded hole located at the known transverse hole present in the abutment. Thereby, the conical transverse screw can engage both the thread of the hole in the abutment but also - and above all - the thread of the corresponding transversal hole of the cylindrical insert, the length of which can cover up to the entire diameter of the insert itself, which is a few
m ll meters .
A better understand ng of the invention will be achieved through the following description and with reference to the accompanying drawings which show, by way of a non-limiting example, a preferred embodiment thereof.

According to the invention described, a cylindrical insert 3 is provided, which can be inserted into the longitudinal cavity of the abutment 1, which insert has a transverse threaded hole 4 capable of being positioned at the threaded transverse hole 2 in the side wall of the abutment itself.
Once the abutment 1 has been fixed in a known manner to the endosseous implant with a connection screw, it is advantageously possible to fix the prosthetic crown to the abutment 1 by screwing the transverse conical screw V into the
lateral hole 1 of the abutment and then into the transverse hole 4 of the cylindrical insert 1 which is present inside the abutment. Thereby, the conical screw is firmly screwed and engaged in the transverse hole 4 over the entire length of the stem of the screw itself.
According to a peculiar feature of the invention, said cylindrical insert 3 is expected to have at least a first male/female abutment element 5 configured to match a respective second female/male abutment element 6 present in the abutment 1, to ensure a unique positioning of the insert 3 itself in the abutment 1, so as to ensure alignment between the transverse hole 2 of the abutment 1 and the transverse hole 4 of the insert 3.
In the preferred embodiment merely described by way of a non-limiting example, said first abutment element 5 consists of a projection provided in the upper part of the insert 3, which is configured to abut with a corresponding abutment element 6 consisting of a hollow having a shape conjugated with that of said projection 5. In other words, in this embodiment the abutment means consist of at least one convex male abutment element 5 and at least one concave female abutment element 6 arranged, on the lower edge of a head of the cylindrical insert 3 and along the upper peripheral edge of the abutment 1, respectively, where said head of the cylindrical insert 3 radially protrudes with
respect to the longitudinal direction of the body of the insert.
From the above, once the cylindrical insert 3 has been inserted into the abutment 1 until it is coupled to the abutment elements 5 and 6, the transverse threaded hole 4 of the insert 3 is perfectly coaxial with the lateral transverse hole 2 present in the same abutment.
With this technical artifice, the threaded portion of the conical screw V, passing through the hole 2 of the abutment 1, is insertable into the transverse threaded hole 4 of the insert 3 advantageously engaging all the turns of the thread of the screw V.
Another advantage of the present invention is that the solid body of the cylindrical insert 3 completely occupies the longitudinal cavity of the abutment preventing entry to the dental pi aque .
A further advantage of the invention is that the presence of the cylindrical insert 3, preferably made of metal, allows using various types of materials for manufacturing the abutment 1, such as for example Titanium, with the latest generation polymers, such as Pekkton* or BioHPP*.
Finally, it is worth noting that it is preferable to provide at least three different sizes of the abutment 1 and therefore of the cylindrical insert 3 to better adapt to the different vertical dimensional requirements of the dental arches:
-Abutment with a height of 4 mm, with a hole for the conical screw 1.5 mm from the abutment coupling line, with the head of the cylindrical insert protruding 2 mm from the upper edge of the abutment;
-Abutment with a height of 8 mm, with a hole for the conical screw 2 mm from the abutment coupling line, with the head of the cylindrical insert protruding 3 mm from the upper edge of the abutment;
-Abutment with a height of 10 mm, with a hole for the conical screw 3 mm from the abutment coupling line, with the head of the cylindrical insert protruding 1 mm from the upper edge of the abutment.
For obtaining a correct positioning of the prosthetic dental crown or implant in the case of several dental elements, in order to have the access hole 2 of the conical screw V in the desired position with respect to the abutment 1, a simulator having the same shape as the abutment 1 and the reference of the access hole of the conical screw to be used during the surgery is preferably provided.
The conical screw V is preferably made of titanium with a total length of 3 mm, two of which for the conical head and one for the threaded stem.
Furthermore, it is advisable that the engagement of the screwdriver is rectangular, with a depth of about 1.8 mm in the conical head.
Claims
1. A dental prosthetic device for dental crowns which is fixable to bone implants through a suitable abutment (1), with a transverse anchoring screw (V), characterized in that it comprises, in combination: a cylindrical insert (3), which is insertable into the tubular body of the abutment (1) after its fixation to the head of the endosseous implant, which insert (3) is provided with an additional transverse threaded hole (4) placed at a transverse hole (2) present in the abutment itself; wherein the conical transverse screw (V) is configured to engage both the thread of the hole (2) in the abutment (3) but also - and above all - the thread of the corresponding additional transverse hole (4) of the cylindrical insert (3); wherein the length of said hole can cover up to the entire diameter of the insert itself; thus obtaining the conical screw (V) being firmly screwed and gripped in the transverse hole (4) over the whole length of the stem of the screw itself.

A dental prosthetic device according to the preceding claim, characterized in that said cylindrical insert (3) has at least a first male/female abutment element (5) configured to match a respective second female/male abutment element (6) present in the abutment (1), to provide a unique positioning of the insert (3) itself in the abutment (1), so as to ensure the alignment between the transverse hole (2) of the
abutment (1) and the transverse hole (4) of the insert (3).

A dental prosthetic device according to the preceding claim, characterized in that said first abutment element (5) consists of a projection provided in the upper part of the insert (3), which projection is configured to abut with a corresponding abutment element (6) consisting of a hollow having a shape conjugated with that of said projection (5).

A dental prosthetic device according to the preceding claim, characterized in that said projection (5) is substantially a convex male element and said hollow (6) is substantially a concave female which are arranged on the lower edge of a head of the cylindrical insert (3) and along the upper peripheral edge of the abutment (1) , respectively, wherein said head of the cylindrical insert (3) is radially protruding with respect to the longitudinal direction of the body of the insert.

A dental prosthetic device according to one of the claims from 2 onwards, characterized in that said abutment elements (5 and 6) mated together are configured and positioned so that, once the cylindrical insert (3) has been inserted into the abutment (1) until the mutual coupling of the abutment elements (5 and 6) is obtained, the transverse threaded hole (4) of the insert (3) is perfectly coaxial with the lateral transverse hole (2) present in the same abutment;
thus obtaining that a threaded portion of the conical screw (V), passing through the hole (2) of the abutment (1), is insertable into the transverse threaded hole (4) of the insert (3) engaging all the turns of the screw thread (V).
6. A dental prosthetic device according to one of the preceding claims, characterized in that said cylindrical insert (3) has a full body configured to completely occupy the longitudinal cavity of the abutment (1), preventing entry to the dental plate.
7. A dental prosthetic device according to one of the preceding claims, characterized in that said cylindrical insert (3) is made of metal , thus obtaining the abutment (1) being manufacturable of Titanium or with latest generation polymers, such as for example Pekkton® or BioHPP®.
8. A dental prosthetic device according to one of the preceding claims, characterized in that said abutment (1) and said cylindrical insert (3) are made in at least three different sizes to be adaptable to the different vertical dimension requirements of the dental arches:
-Abutment with a height of 4 mm, with a hole for the conical screw 1.5 mm from the abutment coupling line, with the head of the cylindrical insert protruding 2 mm from the upper edge of the abutment;
-Abutment with a height of 8 mm, with a hole for the conical screw 2 mm from the abutment
coupling line, with the head of the cylindrical insert protruding 3 mm from the upper edge of the abutment;
-Abutment with a height of 10 mm, with a hole for the conical screw B mm from the abutment coupling line, with the head of the cylindrical insert protruding 1 mm from the upper edge of the abutment.
9. A dental prosthetic device according to one of the preceding claims, characterized in that for obtaining a correct positioning of the prosthetic dental crown or implant in the case of several dental elements, and for having the access hole (2) of the conical screw (V ) in the desired position with respect to the abutment (1) , a simulator having the same shape as the abutment (1) and a reference of the access hole of the conical screw to be used during the surgery is provided.
10. A dental prosthetic device according to one of the preceding claims, characterized in that said conical screw (V) is made of titanium with a total length of 3 mm, two of which for the conical head and one for the threaded stem.
