キメラ抗原レセプター(Chimeric Antigen Receptor。以下、「CAR」とも呼ぶ)は、抗体の単鎖可変領域を細胞外ドメインとし、それに膜貫通領域とCD3ζと共刺激シグナルを伝える分子の細胞内ドメインをつないだものである。抗体の特異性に従って抗原に結合することによりT細胞にシグナルが伝達され標的細胞の傷害・サイトカイン放出・刺激後のT細胞分裂が起こる。遺伝子導入によってT細胞に特異性を与えることが可能であり、細胞の調製は内在性の腫瘍特異的T細胞レセプター(TCR)陽性T細胞を培養増幅する場合に比べて総じて容易である。また、CARを利用した治療法(CAR-T療法)は、抗体療法と比べて高い細胞傷害活性を示すこと、1回の輸注で抗原刺激によって自律的に増幅するために複数回の治療が必要ない点において優れている。TCR遺伝子導入では、HLAとその上に提示されるペプチドの複合体を標的としているために、患者が特定のHLAを持つ場合にのみ治療可能であるが(HLA拘束性)、CARにはHLA拘束性がない点においてより有利である。即ち、腫瘍細胞に標的抗原が陽性であれば、標的抗原陽性の全ての患者を対象として治療が可能である。
キメラ抗原レセプター遺伝子導入T細胞(CAR-T細胞)療法はCD19CAR-Tにおいて、先ごろ米国にて薬品として承認されるに至り、その臨床効果は大変強力であり、様々なB細胞性血液悪性腫瘍において臨床的有用性が期待されている(例えば、特許文献1、非特許文献1、2を参照)。

CD19CAR-TはCD19を標的としているため、一定の頻度でCD19陰性となって再発することがある。いわゆるエスケープ現象である。これを回避するための戦略として、CD19同様にB細胞の表面上に発現している他の抗原に対するCAR-T細胞を同時に用いることが考えられる。実際、CD20やCD22などのCD19以外のB細胞分化マーカーを標的としたCAR-T細胞の開発が試みられているが、十分な成果は得られていない。このような状況下で本発明は、臨床応用に適した新たなCARの提供を課題とする。
一方、真にがん特異的な抗原は得られておらず、これからも得られる見込みは少ない。従って、今後もCAR-T療法の標的抗原については、多少の標的細胞外の発現を見込まなければならないことが予想される。即ち、On-target副反応への対策が今後も重要である。本発明は、当該問題への解決策を提供することも課題とする。
上記課題の下で本発明者らは、標的としてCD37に注目し、新たなCAR-T細胞の開発を目指した。予備的検討としてCD37の発現を様々な腫瘍細胞株及び正常細胞で調べた上で、構造の異なる複数のCARを設計し、それらの効果を比較評価した。CD37は成熟B細胞腫瘍に強く発現していることが知られているが、同時に顆粒球やTリンパ球にも弱く発現していることが知られている。そこで、標的となる腫瘍細胞のCD37発現レベルと正常細胞のCD37発現レベルに注目し、CAR-T細胞の反応性を詳細に調べた。
検討の結果、抗原認識に関与する細胞外ドメインのスペーサーの長さ(即ち、VHとVLから構成されるscFvと膜貫通領域の間の距離)がCAR-T細胞の反応性等に重要であることが判明し、スペーサーを短くすることで正常細胞に対する反応性が抑えられた(抗原認識能力が適度に調節された)CD37特異的CAR-T細胞の作製に成功した。細胞外ドメインのスペーサーが長いCARの遺伝子を導入して作製したCAR-T細胞は遺伝子導入後、培養液中に存在するCD37陽性細胞に反応し、同士討ち現象を起こしたが、スペーサーを短くした場合、当該現象を抑制することが可能であった。
CD37に対する抗体(抗体医薬)の開発が進められてきているが、上市には至っていない。また、CD37抗体を利用したCAR-T細胞を開発する試みについても、実質的な成果を上げるに至っていない。CD37が正常細胞の一部に発現していることがその原因として考えられるが、作製に成功したCD37CAR-T細胞は、CD37を高発現する腫瘍細胞に対して特異性が高く(言い換えれば、正常細胞に対する反応性は低い)、臨床応用可能なプロファイルを示すものであった。
主として上記の成果及び考察に基づき、以下の発明が提供される。

1.CD37特異的キメラ抗原レセプター
本発明の第1の局面はCD37特異的キメラ抗原レセプター(慣例に従い、キメラ抗原受容体を「CAR」と呼称することがある)に関する。CD37特異的キメラ抗原レセプターとは、CD37に特異的結合能(結合特異性)を示す細胞外ドメイン、膜貫通領域、及び免疫細胞のエフェクター機能のための細胞内シグナルドメインを含む構造体である。以下、各ドメインについて説明する。
本発明の第1の局面はCD37特異的キメラ抗原レセプター(慣例に従い、キメラ抗原受容体を「CAR」と呼称することがある)に関する。CD37特異的キメラ抗原レセプターとは、CD37に特異的結合能(結合特異性)を示す細胞外ドメイン、膜貫通領域、及び免疫細胞のエフェクター機能のための細胞内シグナルドメインを含む構造体である。以下、各ドメインについて説明する。
(a)細胞外ドメイン
本発明において細胞外ドメインはCD37特異的な結合性を示す。細胞外ドメインは、抗原の認識を担う抗原認識領域とスペーサー・ドメインに大別される。以下、抗原認識領域の詳細を説明するが、スペーサー・ドメインについては、便宜上、次の「(b)膜貫通領域」の項目の中で説明する。
本発明において細胞外ドメインはCD37特異的な結合性を示す。細胞外ドメインは、抗原の認識を担う抗原認識領域とスペーサー・ドメインに大別される。以下、抗原認識領域の詳細を説明するが、スペーサー・ドメインについては、便宜上、次の「(b)膜貫通領域」の項目の中で説明する。
抗原認識領域は、これに限定するものではないが、好ましくは、抗CD37抗体のscFv、即ち、軽鎖可変領域(VL)と重鎖可変領域(VH)がリンカーで連結した構造からなる。リンカーとしては例えばペプチドリンカーを用いることができる。ペプチドリンカーとは、直鎖状にアミノ酸が連結したペプチドからなるリンカーである。ペプチドリンカーの代表例は、グリシンとセリンから構成されるリンカー(GGSリンカーやGSリンカー)である。GGSリンカー及びGSリンカーを構成するアミノ酸であるグリシンとセリンは、それ自体のサイズが小さく、リンカー内で高次構造が形成されにくい。リンカーの長さは特に限定されない。例えば、アミノ酸残基数が5~25個のリンカーを用いることができる。
抗CD37抗体に関して例えば米国特許第8,333,966号(非特許文献3)を参照することができる。
scFvを構成するVLとVHの順序は特に問わない。即ち、VLがN末端側に配置される態様(この場合、VLのC末端とVHのN末端がリンカーで連結されることになる)、VHがC末端側に配置される態様(この場合、VHのC末端とVLのN末端がリンカーで連結されることになる)のいずれを採用してもよい。尚、抗CD37抗体のscFvを構成するVL及びVHのアミノ酸配列の一例を配列番号1(VL)と配列番号2(VH)に示す。
好ましくは、抗CD37ヒト化モノクローナル抗体のscFvを抗原認識領域として用いる。抗CD37ヒト化モノクローナル抗体とは、CD37を抗原とするヒト化モノクローナル抗体である。ヒト化モノクローナル抗体は、他の動物種(例えばマウスやラット)のモノクローナル抗体の構造をヒトの抗体の構造に類似させた抗体であり、抗体の定常領域のみをヒト抗体のものに置換したヒト型キメラ抗体、及び定常領域及び可変領域に存在するCDR(相補性決定領域)以外の部分をヒト抗体のものに置換したヒト型CDR移植(CDR-grafted)抗体(P.T.Johons et al., Nature 321,522(1986))を含む。ヒト型CDR移植抗体の抗原結合活性を高めるため、マウス抗体と相同性の高いヒト抗体フレームワーク(FR)を選択する方法、相同性の高いヒト型化抗体を作製する方法、ヒト抗体にマウスCDRを移植した後さらにFR領域のアミノ酸を置換する方法の改良技術もすでに開発され(米国特許第5585089号、米国特許第5693761号、米国特許第5693762号、米国特許第6180370号、欧州特許第451216号、欧州特許第682040号、特許第2828340号などを参照)、ヒト化抗体の作製に利用することもできる。
(b)膜貫通領域
膜貫通領域は、細胞外ドメインと細胞内シグナルドメインの間に介在する。膜貫通領域としては、CD28、CD3ε、CD8α、CD3、CD4又は4-1BBなどの膜貫通領域を用いることができる。人工的に構築したポリペプチドからなる膜貫通領域を用いることにしてもよい。尚、膜貫通領域の具体例は後述の実施例に示される。実施例に示したものと機能的に同等のものを用いることもできる。
膜貫通領域は、細胞外ドメインと細胞内シグナルドメインの間に介在する。膜貫通領域としては、CD28、CD3ε、CD8α、CD3、CD4又は4-1BBなどの膜貫通領域を用いることができる。人工的に構築したポリペプチドからなる膜貫通領域を用いることにしてもよい。尚、膜貫通領域の具体例は後述の実施例に示される。実施例に示したものと機能的に同等のものを用いることもできる。
典型的には、膜貫通領域はスペーサー・ドメインを介して、細胞外ドメインの抗原認識領域に連結される。即ち、スペーサーによって、抗原認識領域と膜貫通領域の間に距離(スペース)が設けられる。しかしながら、抗原認識領域と膜貫通領域を直接、連結することにしてもよい。この場合には抗原認識領域と膜貫通領域の距離がゼロ(0)となり、当該構造のCARを細胞内で発現させると、通常、抗原認識領域が細胞膜に直接結合した状態が形成される。
「スペーサー」は、膜貫通領域を抗原認識領域に連結する部材(リンカー)であるが、scFvである場合の抗原認識領域を構成するリンカーとの区別のため、及び抗原認識領域と膜貫通領域の間の距離を調節するという、本発明における本質的且つ重要な役割を明確にするため、用語「スペーサー」を用いて表現される。
スペーサーは1~30個のアミノ酸からなり、直鎖状である(構成アミノ酸が2個以上であれば直鎖状ペプチドとなる)。スペーサーの配列は特に限定されないが、例えばIgG、好ましくはヒトIgG(例えばサブタイプIgG1やIgG4)のヒンジ部又はその一部、ヒンジ部とCH2の一部、或いは膜貫通領域に利用する分子(例えばCD28)の一部等の配列をスペーサーに用いる。尚、ヒンジ部又はその一部の配列を利用することにより、柔軟性に富むスペーサーが構成されることを期待できる。
ところで、一般的なCARは抗体の重鎖Fc領域(ヒンジ部、CH2、CH3)を介して膜貫通領域が抗原認識領域に連結した構造を備える。重鎖Fc領域は220個程度のアミノ酸残基から構成される。従って、一般的なCARの場合、220個程度のアミノ酸残基からなるスペーサーによって、膜貫通領域が抗原認識領域から隔てられることになる。本発明のCARではスペーサーを構成するアミノ酸残基数が1~30個であり、一般的なCARに比べ、抗原認識領域と膜貫通領域の間の距離が圧倒的に短い。この点が本発明の最大の特徴であり、これによって抗原認識能力(標的認識レベル)が適度に調節され、正常細胞に対する反応性が低下するという、本発明に特有の効果が発揮される。スペーサーの長さを短くすることは本発明の効果に対して有利に作用し得る。そこで、スペーサーを構成するアミノ酸残基数は好ましくは1~25個、更に好ましくは1~20個である。尚、スペーサー以外の構造(特に抗原認識領域)、CAR遺伝子が導入される細胞(説明の便宜上、「CAR発現用細胞」と呼称する)、標的細胞/標的疾患等を考慮することにより、或いは予備実験によって検証ないし確認することにより、最適な標的認識レベルが得られるスペーサーの長さを決定すればよい。
(c)細胞内シグナルドメイン
細胞内シグナルドメインは、免疫細胞のエフェクター機能の発揮に必要なシグナルを伝達する。即ち、細胞外ドメインがCD37と結合した際、免疫細胞の活性化に必要なシグナルを伝達することが可能な細胞内シグナルドメインが用いられる。細胞内シグナルドメインには、TCR複合体を介したシグナルを伝達するためのドメイン(便宜上、「第1ドメイン」と呼ぶ)と、共刺激シグナルを伝達するためのドメイン(便宜上、「第2ドメイン」と呼ぶ)が含まれる。第1ドメインとして、CD3ζの他、FcεRIγ等の細胞内ドメインを用いることができる。好ましくは、CD3ζが用いられる。また、第2ドメインとしては共刺激分子の細胞内ドメインが用いられる。共刺激分子としてCD28、4-1BB(CD137)、CD2、CD4、CD5、CD134、OX-40又はICOSを例示することができる。好ましくは、CD28又は4-1BBの細胞内ドメインを採用する。
細胞内シグナルドメインは、免疫細胞のエフェクター機能の発揮に必要なシグナルを伝達する。即ち、細胞外ドメインがCD37と結合した際、免疫細胞の活性化に必要なシグナルを伝達することが可能な細胞内シグナルドメインが用いられる。細胞内シグナルドメインには、TCR複合体を介したシグナルを伝達するためのドメイン(便宜上、「第1ドメイン」と呼ぶ)と、共刺激シグナルを伝達するためのドメイン(便宜上、「第2ドメイン」と呼ぶ)が含まれる。第1ドメインとして、CD3ζの他、FcεRIγ等の細胞内ドメインを用いることができる。好ましくは、CD3ζが用いられる。また、第2ドメインとしては共刺激分子の細胞内ドメインが用いられる。共刺激分子としてCD28、4-1BB(CD137)、CD2、CD4、CD5、CD134、OX-40又はICOSを例示することができる。好ましくは、CD28又は4-1BBの細胞内ドメインを採用する。
第1ドメインと第2ドメインの連結態様は特に限定されないが、好ましくは、過去の事例においてCD3ζを遠位につないだ場合に共刺激が強く伝わったことが知られていることから、膜貫通領域側に第2ドメインを配置する。同一又は異種の複数の細胞内ドメインをタンデム状に連結して第1ドメインを構成してもよい。第2ドメインについても同様である。
第1ドメインと第2ドメインは、これらを直接連結しても、これらの間にリンカーを介在させてもよい。リンカーとしては例えばペプチドリンカーを用いることができる。ペプチドリンカーとは、直鎖状にアミノ酸が連結したペプチドからなるリンカーである。ペプチドリンカーの構造、特徴等は前述の通りである。但し、ここでのリンカーとしては、グリシンのみから構成されるものを用いてもよい。リンカーの長さは特に限定されない。例えば、アミノ酸残基数が2~15個のリンカーを用いることができる。尚、細胞内シグナルドメインの具体例は後述の実施例に示される。実施例に示したものと機能的に同等のものを用いることもできる。
(d)その他の要素
CARの細胞膜上への輸送を促すためにリーダー配列(シグナルペプチド)が用いられる。例えば、GM-CSFレセプターのリーダー配列を用いることができる。一方、膜貫通領域と細胞内シグナルドメインの間にスペーサー・ドメインを設けることにしてもよい。換言すれば、細胞内シグナルドメインを膜貫通領域に直接連結するのではなく、リンカー(通常はペプチドリンカー)を介して連結することにしてもよい。
CARの細胞膜上への輸送を促すためにリーダー配列(シグナルペプチド)が用いられる。例えば、GM-CSFレセプターのリーダー配列を用いることができる。一方、膜貫通領域と細胞内シグナルドメインの間にスペーサー・ドメインを設けることにしてもよい。換言すれば、細胞内シグナルドメインを膜貫通領域に直接連結するのではなく、リンカー(通常はペプチドリンカー)を介して連結することにしてもよい。
尚、これまでにCARを利用した実験、臨床研究などの報告がいくつかあり(例えばRossig C, et al. Mol Ther 10:5-18, 2004; Dotti G, et al. Hum Gene Ther 20:1229-1239, 2009; Ngo MC, et al. Hum Mol Genet 20 (R1):R93-99, 2011; Ahmed N, et al. Mol Ther 17:1779-1787, 2009; Pule MA, et al. Nat Med 14:1264-1270, 2008; Louis CU, et al. Blood 118:6050-6056, 2011; Kochenderfer JN, et al. Blood 116:4099-4102, 2010; Kochenderfer JN, et al. Blood 119 :2709-2720, 2012; Porter DL, et al. N Engl J Med 365:725-733, 2011; Kalos M, et al. Sci Transl Med 3:95ra73,2011; Brentjens RJ, et al. Blood 118:4817-4828, 2011; Brentjens RJ, et al. Sci Transl Med 5:177 ra38, 2013)、これらの報告を参考にして本発明におけるCARを構築することができる。
2.キメラ抗原受容体(CAR)をコードする遺伝子及びその利用
本発明の第2の局面はCARをコードする遺伝子(以下、「CAR遺伝子」と呼称することがある)及びその利用(発現カセット、ベクター、CARを発現する遺伝子改変リンパ球の調製方法、CARを発現する遺伝子改変リンパ球及びその用途など)に関する。本発明のCAR遺伝子は上記構造のCARをコードする。従って、それを適当な細胞(CAR発現用細胞)に導入し、発現させることにより、本発明のCARを細胞表面に発現する遺伝子改変リンパ球(CAR遺伝子導入リンパ球)が得られる。CAR遺伝子導入リンパ球はCAR療法に利用することができる。CAR遺伝子の配列の具体例を配列番号3に示す。当該CAR遺伝子は、5'末端側から3'末端側に向かって、GM-CSFレセプターのリーダー配列(配列番号4)、VLをコードする領域(配列番号5)、リンカー配列(配列番号6)、VHをコードする領域(配列番号7)、抗CD37抗体由来の配列(gatcag)、ヒンジ部をコードする領域(スペーサー配列)(配列番号8)、CD28膜貫通領域をコードする領域(配列番号9)、CD28細胞内ドメインをコードする領域(配列番号10)、リンカー配列(GGCGGAGGG)及びCD3ζ細胞内ドメインをコードする領域(配列番号11)が直列的に並ぶ構造を備える。
本発明の第2の局面はCARをコードする遺伝子(以下、「CAR遺伝子」と呼称することがある)及びその利用(発現カセット、ベクター、CARを発現する遺伝子改変リンパ球の調製方法、CARを発現する遺伝子改変リンパ球及びその用途など)に関する。本発明のCAR遺伝子は上記構造のCARをコードする。従って、それを適当な細胞(CAR発現用細胞)に導入し、発現させることにより、本発明のCARを細胞表面に発現する遺伝子改変リンパ球(CAR遺伝子導入リンパ球)が得られる。CAR遺伝子導入リンパ球はCAR療法に利用することができる。CAR遺伝子の配列の具体例を配列番号3に示す。当該CAR遺伝子は、5'末端側から3'末端側に向かって、GM-CSFレセプターのリーダー配列(配列番号4)、VLをコードする領域(配列番号5)、リンカー配列(配列番号6)、VHをコードする領域(配列番号7)、抗CD37抗体由来の配列(gatcag)、ヒンジ部をコードする領域(スペーサー配列)(配列番号8)、CD28膜貫通領域をコードする領域(配列番号9)、CD28細胞内ドメインをコードする領域(配列番号10)、リンカー配列(GGCGGAGGG)及びCD3ζ細胞内ドメインをコードする領域(配列番号11)が直列的に並ぶ構造を備える。
CAR遺伝子を用い、CAR発現用の発現カセット(以下、「CAR発現カセット」とも呼称する)を構築することができる。CAR発現カセットはプロモーターと、当該プロモーターの制御下にあるCAR遺伝子を含む。プロモーターの制御下となるように、通常、プロモーターの下流にCAR遺伝子を配置する。CAR発現カセットに利用可能なプロモーターの例を示すと、CMV-IE(サイトメガロウイルス初期遺伝子由来プロモーター)、SV40ori、レトロウイルスLTP、SRα、EF1α、βアクチンプロモーター等である。プロモーターはCAR遺伝子に作動可能に連結される。ここで、「プロモーターがCAR遺伝子に作動可能に連結している」とは、「プロモーターの制御下にCAR遺伝子が配置されている」ことと同義であり、通常、プロモーターの3'末端側に直接又は他の配列を介してCAR遺伝子が連結されることになる。CAR遺伝子の下流にはポリA付加シグナル配列を配置する。ポリA付加シグナル配列の使用によって転写を終了させる。ポリA付加シグナル配列としてはSV40のポリA付加配列、ウシ由来成長ホルモン遺伝子のポリA付加配列等を用いることができる。
発現カセット内に検出用遺伝子(レポーター遺伝子、細胞又は組織特異的な遺伝子、選択マーカー遺伝子など)、エンハンサー配列、WRPE配列等を含めることにしてもよい。検出用遺伝子は、発現カセットの導入の成否や効率の判定、CAR遺伝子の発現の検出又は発現効率の判定、CAR遺伝子が発現した細胞の選択や分取等に利用される。一方、エンハンサー配列の使用によって発現効率の向上が図られる。検出用遺伝子としては、ネオマイシンに対する耐性を付与するneo遺伝子、カナマイシン等に対する耐性を付与するnpt遺伝子(Herrera Estrella、EMBO J. 2(1983)、987-995)やnptII遺伝子(Messing & Vierra.Gene 1 9:259-268(1982))、ハイグロマイシンに対する耐性を付与するhph遺伝子(Blochinger & Diggl mann,Mol Cell Bio 4:2929-2931)、メタトレキセートに対する耐性を付与するdhfr遺伝子(Bourouis et al.,EMBO J.2(7))等(以上、マーカー遺伝子)、ルシフェラーゼ遺伝子(Giacomin、P1. Sci. 116(1996)、59~72;Scikantha、J. Bact. 178(1996)、121)、β-グルクロニダーゼ(GUS)遺伝子、GFP(Gerdes、FEBS Lett. 389(1996)、44-47)やその改変体(EGFPやd2EGFPなど)等の蛍光タンパク質の遺伝子(以上、レポーター遺伝子)、細胞内ドメインを欠く上皮成長因子受容体(EGFR)遺伝子等の遺伝子を用いることができる。検出用遺伝子は、例えば、バイシストロニック性制御配列(例えば、リボソーム内部認識配列(IRES))や自己開裂ペプチドをコードする配列を介してCAR遺伝子に連結している。自己開裂ペプチドの例はThosea asigna virus由来の2Aペプチド(T2A)であるが、これに限定されるものではない。自己開裂ペプチドとして蹄疫ウイルス(FMDV)由来の2Aペプチド(F2A)、ウマ鼻炎Aウイルス(ERAV)由来の2Aペプチド(E2A)、porcine teschovirus(PTV-1)由来の2Aペプチド(P2A)等が知られている。
CAR発現カセットはCAR発現用細胞への導入のためにベクターに搭載される。本明細書において用語「ベクター」は、それに挿入された核酸分子をターゲット(標的細胞)内へと輸送することができる核酸性分子をいい、形態、由来などは特に限定されるものではない。様々な種類のベクターが利用可能である。好適なベクターの例はウイルスベクターであるが、非ウイルスベクターを用いることもできる。ウイルスベクターを利用した方法はウイルスが細胞へと感染する現象を巧みに利用するものであり、高い遺伝子導入効率が得られる。ウイルスベクターとしてレトロウイルスベクター、レンチウイルスベクター、アデノウイルスベクター、アデノ随伴ウイルスベクター、ヘルペスウイルスベクター、センダイウイルスベクター等が開発されている。この中でレトロウイルスベクター、レンチウイルスベクター、アデノ随伴ウイルスベクターではベクターに組み込んだ目的遺伝子が宿主染色体へと組み込まれ、安定かつ長期的な発現が期待できる。各ウイルスベクターは既報の方法に従い又は市販される専用のキットを用いて作製することができる。非ウイルスベクターの例としては、プラスミドベクター、リポソームベクター、正電荷型リポソームベクター(Felgner, P.L., Gadek, T.R., Holm, M. et al., Proc. Natl. Acad. Sci., 84:7413-7417, 1987)、YACベクター、BACベクターを挙げることができる。
組換え操作(CAR発現カセットの構築、CAR発現カセットのベクターへの搭載等)には、制限酵素及びDNAリガーゼを用いた方法等、標準的な組換えDNA技術(例えば、Molecular Cloning, Third Edition, 1.84, Cold Spring Harbor Laboratory Press, New Yorkを参照することができる)を利用することができる。
CAR遺伝子が導入される細胞(CAR発現用細胞)として、CD4陽性CD8陰性T細胞、CD4陰性CD8陽性T細胞、iPS細胞から調製されたT細胞、αβ-T細胞、γδ-T細胞、NK細胞、NKT細胞等を挙げることができる。上記の如きリンパ球又は前駆細胞を含むものであれば、様々な細胞集団を用いることができる。末梢血から採取されるPBMC(末梢血単核細胞)は好ましい標的細胞の一つである。即ち、好ましい一態様では、PBMCに対して遺伝子導入操作を行う。PBMCは常法で調製すればよい。PBMCの調製方法については、例えば、Saha S, Nakazawa Y, Huye LE, Doherty JE, Galvan DL, Rooney CM, Wilson MH. J Vis Exp. 2012 Nov 5;(69):e4235を参照することができる。尚、特に言及しない限り、本明細書における各種細胞(例えばT細胞)はヒト細胞である。
CAR遺伝子の導入の前にCAR発現用細胞を活性化させておくとよい。例えば、抗CD3抗体及び抗CD28抗体で刺激し、CAR発現用細胞を活性化させる。例えば、抗CD3抗体と抗CD28抗体で培養面をコートした培養容器(例えば培養皿)で培養することによって、抗CD3抗体及び抗CD28抗体による刺激を加えることができる。抗CD3抗体(例えばミルテニーバイオテク社が提供する商品名CD3pure抗体を用いることができる)と抗CD28抗体(例えばミルテニーバイオテク社が提供する商品名CD28pure抗体を用いることができる)は市販もされており、容易に入手可能である。抗CD3抗体と抗CD28抗体がコートされた磁気ビーズ(例えば、VERITAS社が提供するDynabeads T-Activator CD3/CD28)を利用して当該刺激を行うことも可能である。尚、抗CD3抗体として「OKT3」クローンを用いることが好ましい。
細胞の生存率/増殖率を高めるために、活性化処理の際、T細胞増殖因子が添加された培養液を使用するとよい。T細胞増殖因子としてはIL-2、IL-15、IL-7等を用いることができる。IL-2、IL-15、IL-7等のT細胞増殖因子は常法に従って調製することができる。また、市販品を利用することもできる。ヒト以外の動物種のT細胞増殖因子の使用を排除するものではないが、通常、T細胞増殖因子はヒト由来のもの(組換え体であってもよい)を用いる。ヒトIL-2、IL-15、ヒトIL-7等のT細胞増殖因子は用意に入手することができる(例えばミルテニーバイオテク社、R&Dシステムズ社等が提供する)。
典型的には、その適用(患者への投与)のため、CAR遺伝子導入後の細胞(CAR遺伝子導入リンパ球)を増殖させる。例えば、T細胞増殖因子が添加された培養液を用いてCAR遺伝子導入リンパ球を培養する(必要に応じて継代培養する)。この培養に加え、CAR発現用細胞の活性化の場合と同様の処理(再活性化)を行うことにしてもよい。
尚、CAR発現用細胞及びCAR遺伝子導入リンパ球の培養には、血清(ヒト血清、ウシ胎仔血清など)を添加した培地を用いてもよいが、無血清培地を採用することにより、臨床応用する際の安全性が高く、且つ血清ロット間の差による培養効率の違いが出にくいという利点を有する細胞を調製することが可能になる。リンパ球用の無血清培地の具体例はTexMACSTM(ミルテニーバイオテク社)、AIM V(登録商標)(Thermo Fisher Scientific社)である。血清を用いる場合には、自己血清、即ち、CAR遺伝子導入リンパ球の投与を受ける患者から採取した血清を用いるとよい。基本培地にはリンパ球の培養に適したものを用いればよく、具体例を挙げれば、上掲のTexMACSTM、AIM V(登録商標)である。その他の培養条件は、リンパ球の生存、増殖に適したものであればよく、一般的なものを採用すればよい。例えば、37℃に設定したCO2インキュベーター(CO2濃度5%)内で培養すればよい。
3.CAR遺伝子導入リンパ球及びその用途
本発明の更なる局面は、本発明の調製方法で得られた、キメラ抗原受容体を発現する遺伝子改変リンパ球(以下、「本発明のCAR遺伝子導入リンパ球」と呼ぶ)及びその用途に関する。本発明のCAR遺伝子導入リンパ球は、典型的には、CD37を高発現する腫瘍/がん(以下、「標的疾患」と呼ぶ)の治療、予防又は改善に広く利用され得る。標的疾患の例を挙げると、各種B細胞性悪性リンパ腫(B細胞性急性リンパ性白血病、濾胞性リンパ腫、びまん性リンパ腫、マントル細胞リンパ腫、MALTリンパ腫、血管内B細胞性リンパ腫、CD20陽性ホジキンリンパ腫など)、骨髄増殖性疾患、骨髄異形成/骨髄増殖性腫瘍(CMML,JMML,CML,MDS/MPN-UC)、骨髄異形成症候群、急性骨髄性白血病、多発性骨髄腫等である。「治療」とは、標的疾患に特徴的な症状又は随伴症状を緩和すること(軽症化)、症状の悪化を阻止ないし遅延すること等が含まれる。「予防」とは、疾病(障害)又はその症状の発症/発現を防止又は遅延すること、或いは発症/発現の危険性を低下させることをいう。一方、「改善」とは、疾病(障害)又はその症状が緩和(軽症化)、好転、寛解、又は治癒(部分的な治癒を含む)することをいう。
本発明の更なる局面は、本発明の調製方法で得られた、キメラ抗原受容体を発現する遺伝子改変リンパ球(以下、「本発明のCAR遺伝子導入リンパ球」と呼ぶ)及びその用途に関する。本発明のCAR遺伝子導入リンパ球は、典型的には、CD37を高発現する腫瘍/がん(以下、「標的疾患」と呼ぶ)の治療、予防又は改善に広く利用され得る。標的疾患の例を挙げると、各種B細胞性悪性リンパ腫(B細胞性急性リンパ性白血病、濾胞性リンパ腫、びまん性リンパ腫、マントル細胞リンパ腫、MALTリンパ腫、血管内B細胞性リンパ腫、CD20陽性ホジキンリンパ腫など)、骨髄増殖性疾患、骨髄異形成/骨髄増殖性腫瘍(CMML,JMML,CML,MDS/MPN-UC)、骨髄異形成症候群、急性骨髄性白血病、多発性骨髄腫等である。「治療」とは、標的疾患に特徴的な症状又は随伴症状を緩和すること(軽症化)、症状の悪化を阻止ないし遅延すること等が含まれる。「予防」とは、疾病(障害)又はその症状の発症/発現を防止又は遅延すること、或いは発症/発現の危険性を低下させることをいう。一方、「改善」とは、疾病(障害)又はその症状が緩和(軽症化)、好転、寛解、又は治癒(部分的な治癒を含む)することをいう。
本発明のCAR遺伝子導入リンパ球を細胞製剤の形態で提供することもできる。本発明の細胞製剤には、本発明のCAR遺伝子導入リンパ球が治療上有効量含有される。例えば1回の投与用として、1×104個~1×1010個の細胞を含有させる。細胞の保護を目的としてジメチルスルフォキシド(DMSO)や血清アルブミン等、細菌の混入を阻止する目的で抗生物質等、細胞の活性化、増殖又は分化誘導などを目的とした各種の成分(ビタミン類、サイトカイン、成長因子、ステロイド等)等の成分を細胞製剤に含有させてもよい。
本発明のCAR遺伝子導入リンパ球又は細胞製剤の投与経路は特に限定されない。例えば、静脈内注射、動脈内注射、門脈内注射、皮内注射、皮下注射、筋肉内注射、又は腹腔内注射によって投与する。全身投与によらず、局所投与することにしてもよい。局所投与として、目的の組織・臓器・器官への直接注入を例示することができる。投与スケジュールは、対象(患者)の性別、年齢、体重、病態などを考慮して作成すればよい。単回投与の他、連続的又は定期的に複数回投与することにしてもよい。
本発明のCAR遺伝子導入リンパ球を用いた治療法では、治療上有効量のCAR遺伝子導入リンパ球を患者に投与する。本発明のCAR遺伝子導入リンパ球はその特徴的な構成により、CD37を細胞表面に発現する腫瘍に対して抗腫瘍効果を発揮するという特性を示す。そのため、特に、B細胞性悪性リンパ腫、急性骨髄性白血病の一部、多発性骨髄腫等の治療への適用が期待される。
4.キメラ抗原レセプター(CAR)の構造を決定する方法
本発明の別の局面はCARの設計技術に関する。具体的には、標的認識能力が調節されたCARの構造を決定する方法(以下、「CAR構造決定法」と呼ぶことがある)が提供される。本発明のCAR構造決定法は、正常細胞への反応性が抑えられた、安全性の高いCARの調製を可能とする。本発明のCAR構造決定法では以下の(1)~(3)のステップが行われる。
(1)抗原認識領域と膜貫通領域の間の距離が異なる複数のキメラ抗原レセプターを設計し、各々について、それを発現した遺伝子改変リンパ球を用意するステップ
(2)各遺伝子改変リンパ球の標的認識レベルをin vitroで評価するステップ
(3)最適な認識レベルを示した遺伝子改変リンパ球が発現するキメラ抗原レセプターの構造を選択するステップ
本発明の別の局面はCARの設計技術に関する。具体的には、標的認識能力が調節されたCARの構造を決定する方法(以下、「CAR構造決定法」と呼ぶことがある)が提供される。本発明のCAR構造決定法は、正常細胞への反応性が抑えられた、安全性の高いCARの調製を可能とする。本発明のCAR構造決定法では以下の(1)~(3)のステップが行われる。
(1)抗原認識領域と膜貫通領域の間の距離が異なる複数のキメラ抗原レセプターを設計し、各々について、それを発現した遺伝子改変リンパ球を用意するステップ
(2)各遺伝子改変リンパ球の標的認識レベルをin vitroで評価するステップ
(3)最適な認識レベルを示した遺伝子改変リンパ球が発現するキメラ抗原レセプターの構造を選択するステップ
ステップ(1)では、まず、細胞外ドメインを構成する抗原認識領域と膜貫通領域の間の距離が異なる複数のCARを設計する。換言すれば、抗原認識領域と膜貫通領域を連結するスペーサーの長さを変えた複数のCARを設計する。原則、スペーサー以外の構造/構成は同一とする。但し、ステップ(2)における比較に支障がなければ、例外的ではあるが、スペーサー以外の構造/構成に違いを設けてもよい。
典型的には、抗原認識領域としてscFvが利用されるが、CARとして機能するために支障のない構造であり且つ必要な特異性や反応性が得られる限りにおいて、これに限定されない。例えば、標的分子が受容体の場合にはそのリガンド(例えば自然リガンド)又はその一部、或いはその改変体(但し、当該受容体への結合能を示すもの)を抗原認識領域に用いることもできる。本発明の方法は汎用性に優れ、抗原認識領域が認識する標的分子(抗原)は特に限定されない。標的分子の例として、CD19、CD20、GD2、CD22、CD30、CD33、CD37、CD44variant7/8、CEA(carcinoembryonic antigen)、Her2/neu、MUC1(Mucin 1)、MUC4(Mucin 4)、MUC6(Mucin 6)、IL-13 receptor-alpha2、イムノグロブリン軽鎖、PSMA抗、VEGF receptor2を挙げることができる。
設計された各CARについて、CAR遺伝子の調製及びCAR発現用細胞への導入を行い、CARを発現する遺伝子改変リンパ球(CAR遺伝子導入リンパ球)を用意する。CAR遺伝子の調製等は常法で行えばよい(上記局面の説明を参照することができる)。
ステップ(2)では、ステップ(1)で用意した各遺伝子改変リンパ球の標的認識レベルをin vitroで評価する。具体的には、各遺伝子改変リンパ球について、本来の標的細胞(CARの抗原認識領域が認識する抗原を細胞表面に発現し、CARの攻撃の対象になる腫瘍細胞。細胞株又は患者由来の腫瘍細胞を用いることができる。)に対する反応性と、攻撃の対象になるべきでない他の細胞(典型的には正常細胞)に対する反応性を比較することにより、標的認識レベルが評価される。ここでの評価には例えば、細胞内サイトカイン産生アッセイ(例えばフローサイトメトリーを利用した方法)、細胞外サイトカイン産生アッセイ(例えばELISA(Enzyme-Linked ImmunoSorbent Assay)法、ELISPOT(Enzyme-Linked ImmunoSpot)法)、腫瘍細胞傷害性試験(例えば51Cr放出試験)等を用いることができる。
上記の評価に加え、CAR遺伝子の導入効率、遺伝子改変リンパ球の増殖率、刺激後の細胞分裂反応等も比較・評価するとよい。また、動物モデルを用い、標的細胞に対する効果をin vivoで評価することにしてもよい。
ステップ(2)に続くステップ(3)では、最適な認識レベルを示した遺伝子改変リンパ球が発現するCARの構造を選択する。標的の腫瘍細胞に対して反応性を維持しつつ(但し、標的の腫瘍細胞に対する反応性は高い程よい)、正常細胞に対しては反応性が低い又は実質的に反応しないという指標ないし基準の下、認識レベルの優劣を評価し、最適な認識レベルを示した遺伝子改変リンパ球を決定し、それが発現するCARの構造を最適なものとして選択すればよい。
ステップ(3)で選択された構造に基づきCAR遺伝子を調製し、CAR遺伝子の発現を行えば、標的認識レベルが調節され、安全性が高められた遺伝子改変リンパ球(CAR遺伝子導入リンパ球)が得られる。当該遺伝子改変リンパ球は、臨床での使用により適したものとなる。安全性を更に高めるために、選択された構造を基準とし、抗原認識領域と膜貫通領域の間の距離を最適化することにしてもよい(ステップ(4))。当該最適化では、距離の異なる(但し、ステップ(1)の場合よりも距離の相違の程度は低い)複数のCARを設計し、ステップ(1)~(3)に準じた評価・選択を行うことになる。
決定された構造に対し、抗原認識領域と膜貫通領域の間の距離以外の点に関する更なる改変を加えてもよい。例えば、抗原認識領域を構成するアミノ酸配列の改変(例えば、アミノ酸配列を構成するアミノ酸残基の欠失、置換、付加等。改変対象となるアミノ酸の数は例えば50個以下、好ましくは25個以下、更に好ましくは15個以下、更に更に好ましくは10個以下、最も好ましくは5個以下である。)、膜貫通領域の同様の改変又は別の分子の膜貫通領域への置換、細胞内シグナルドメインの同様の改変又は別の分子の細胞内シグナルドメインへの置換、細胞内シグナルドメインの追加等の改変を加えることが可能である。
CD37は主として成熟B細胞に発現する4回膜貫通型の蛋白質であり、インターロイキン6受容体やインテグリンα4β1との相互作用において、生存・アポトーシスシグナルの両方に関わる。血液悪性腫瘍においてCD37は慢性リンパ性白血病や非ホジキンリンパ腫に高発現している。CD37を標的とした新規CAR分子の開発を目指し、以下の検討を行った。
1.CD37の発現
各種腫瘍細胞株及び正常細胞におけるCD37の発現を調べた。CD37は主として分化したBリンパ球において高発現していることが報告されている。B細胞性悪性リンパ腫細胞株を含む各種腫瘍細胞株におけるCD37の発現をフローサイトメトリーで確認した。フローサイトメトリーの結果を図1に示す。B細胞性悪性リンパ腫株であるRaji、OCI-Ly1、SU-DHL10、Ramos、Daudi及びKARPAS1106Pでは強陽性である。多発性骨髄腫細胞株であるRPMI8226、KMS12BM及びMM1Sでは弱い発現が見られた(U266Hは多発性骨髄腫細胞株であるが発現が見られなかった)。一方で慢性骨髄性白血病由来細胞株であるK562やT細胞性白血病細胞株であるSUP-T1及びJurkatではほとんど発現が見られなかった(Molt-4では弱い発現が見られた)。B細胞性急性リンパ性白血病細胞株であるB-ALL1では発現は高かった。急性骨髄性白血病細胞株HL-60、Kasumi-1、MOLM-13、MV4-11、NOMO-1及びOCI-AML3では弱い発現が見られた。フィラデルフィア染色体陽性急性リンパ性白血病細胞株であるNPhA1では発現は見られなかった。
各種腫瘍細胞株及び正常細胞におけるCD37の発現を調べた。CD37は主として分化したBリンパ球において高発現していることが報告されている。B細胞性悪性リンパ腫細胞株を含む各種腫瘍細胞株におけるCD37の発現をフローサイトメトリーで確認した。フローサイトメトリーの結果を図1に示す。B細胞性悪性リンパ腫株であるRaji、OCI-Ly1、SU-DHL10、Ramos、Daudi及びKARPAS1106Pでは強陽性である。多発性骨髄腫細胞株であるRPMI8226、KMS12BM及びMM1Sでは弱い発現が見られた(U266Hは多発性骨髄腫細胞株であるが発現が見られなかった)。一方で慢性骨髄性白血病由来細胞株であるK562やT細胞性白血病細胞株であるSUP-T1及びJurkatではほとんど発現が見られなかった(Molt-4では弱い発現が見られた)。B細胞性急性リンパ性白血病細胞株であるB-ALL1では発現は高かった。急性骨髄性白血病細胞株HL-60、Kasumi-1、MOLM-13、MV4-11、NOMO-1及びOCI-AML3では弱い発現が見られた。フィラデルフィア染色体陽性急性リンパ性白血病細胞株であるNPhA1では発現は見られなかった。
以上の結果を踏まえると、B細胞性悪性リンパ腫及びB細胞性急性リンパ性白血病がCD37特異的CAR(CD37CAR)の標的疾患になるといえる。また、急性骨髄性白血病の一部や多発性骨髄腫も標的疾患の候補になり得る。
次に、健常人ドナー由来の末梢血の各分画においてCD37発現を検討した(図2)。Gate-C/Dは顆粒球と考えられ、ほとんど発現は見られなかった。Gate-Aは小リンパ球であり、こちらもごく弱い発現が見られたのみであった。Gate-Bはリンパ球を含む分画であり、B細胞を含むため発現の高い細胞が一部に見られた。
前述のデータをアイソタイプ抗体とCD37抗体の染色の平均蛍光強度(Mean fluorescence intensity; MFI)の比として示した(図3)。正常細胞の染色では単球および顆粒球のゲートでは弱い発現が見られた。
前出の腫瘍細胞株のCD37発現をCD37抗体の結合量で定量化した(QIFIKIT)。K562/CD37はCD37を遺伝子導入したK562であり、さらにLow、Int及びHighはそれぞれ低発現、中発現及び高発現に分画した細胞である。
図4の結果を表にまとめた(図5)。腫瘍細胞株では1細胞あたり100000を超えるCD37抗体の結合が見られた。
健常ドナーの末梢血細胞を図2に示したゲートで定量化した(図6)。Bリンパ球が含まれる分画では100000を超えるCD37抗体の結合が見られるが、それ以外の分画では総じて3000-5000程度の発現レベルであった。尚、図6のデータをグラフに示した(図7)。
健常ドナー由来の末梢血CD3陽性T細胞上のCD37発現を経時的に検討した。具体的には、末梢血CD3陽性T細胞をCD3/28ビーズで刺激を行い、その後のCD37発現を経時的に観察した。刺激の翌日に一過性に発現上昇が見られたが、その後はほぼ一定の発現レベルでCD37発現が観察された(図8)。
2.CD37-CARのデザイン
正常B細胞およびB細胞性腫瘍に主として発現が見られるCD37に対するキメラ抗原レセプター(CAR)を設計・作成した。従来のCD19-CARの場合と同様に、抗CD37抗体の特異性に従ってCD37に結合することによりT細胞にシグナルが伝達され、標的細胞の傷害・サイトカイン放出・刺激後のT細胞分裂が起こることを期待した。前述の通り、CAR-T細胞療法には、細胞の調製が容易であること、抗体療法と比べて高い細胞傷害活性を示すこと、1回の治療でも十分な効果を期待できること、更には、HLA拘束性がないため、腫瘍細胞に標的抗原が陽性であれば、標的抗原陽性の全ての患者を対象として治療が可能であることなどの利点がある。
正常B細胞およびB細胞性腫瘍に主として発現が見られるCD37に対するキメラ抗原レセプター(CAR)を設計・作成した。従来のCD19-CARの場合と同様に、抗CD37抗体の特異性に従ってCD37に結合することによりT細胞にシグナルが伝達され、標的細胞の傷害・サイトカイン放出・刺激後のT細胞分裂が起こることを期待した。前述の通り、CAR-T細胞療法には、細胞の調製が容易であること、抗体療法と比べて高い細胞傷害活性を示すこと、1回の治療でも十分な効果を期待できること、更には、HLA拘束性がないため、腫瘍細胞に標的抗原が陽性であれば、標的抗原陽性の全ての患者を対象として治療が可能であることなどの利点がある。
今回、単鎖抗体(scFv)の下流の長さを調節した配列を複数作成した(図9、10)。これにより、標的抗原との結合の度合いをコントロールできることを期待した。抗CD37抗体のscFv(重鎖可変領域-軽鎖可変領域の順で連結した配列と、軽鎖可変領域-重鎖可変領域の順で連結した配列を使用した)をCD28膜貫通領域(CD28TM)、CD28の細胞内ドメイン(CD28IC)及びCD3ζ細胞内ドメイン(CD3z)と結合させて構成した。
設計したCD37-CARの構造の詳細を、ショートスペーサータイプの一つである37S-LH(図10の右端の構造)を代表例として説明すると、当該CARは、抗CD37ヒト化モノクローナル抗体の軽鎖可変領域(VL)(配列番号1)に重鎖可変領域(VH)(配列番号2)がリンカー(配列番号12)で連結した構造のscFvにヒトIgG4由来のヒンジ部(配列番号13)、CD28膜貫通領域(配列番号14)、CD28細胞内ドメイン(配列番号15)、グリシンリンカー(GGG)、CD3ζ(配列番号16)が連結した構造を備える。当該CARの発現用コンストラクトの配列を配列番号17に示す。当該配列では、HindIIIサイトとKozak配列に続いて、CAR遺伝子(配列番号3)(VHの配列とヒンジ部の配列の間に介在する配列gatcag(6bp)は抗CD37抗体由来の配列)、T2A配列(配列番号18)、tEGF(truncated EGF)をコードする配列(配列番号19)、ストップコドン、及びNotIサイトが順に配置されている。
3.T細胞性腫瘍細胞株であるSUP-T1に対する遺伝子導入効率及び健常人ドナー由来CD3陽性細胞に対するCD37-CARの遺伝子導入効率
SUP-T1に対するロングスペーサータイプ(37L-HL、37L-LH)とショートスペーサータイプ(37S-HL、37S-LH)のそれぞれの遺伝子導入効率を検討した(図11の下二段)。いずれのタイプも1回の遺伝子導入で20~35%の遺伝子導入効率が得られ、ロングスペーサーとショートスペーサーの間に遺伝子導入効率の差は見られなかった。一方で、健常人ドナー由来CD3への遺伝子導入効率(図11の上二段)は、CD37L-HL及びCD37L-LHのロングスペーサーを持つCD37-CARにおいて遺伝子導入効率が高かった。注目すべきことに、ロングスペーサータイプを遺伝子導入した細胞では培養中に死細胞が多数見られ、総細胞数も増えなかった(図12)。活性化したCD3上に発現する弱いCD37発現を認識してCD37CAR-T細胞同士の攻撃、いわゆる同士討ち(fractricide)が起こり、見かけ上遺伝子導入効率が高いものと考えられた。対照的にショートスペーサータイプ(CD37S-HL、CD37S-LH)に関しては死細胞はそれほど見られず、総細胞数の増加も良好であった(図12)。尚、ショートスペーサータイプについては、その後の検討によって(3日目及び4日目に遺伝子導入)、遺伝子導入効率50~70%程度を達成している。
SUP-T1に対するロングスペーサータイプ(37L-HL、37L-LH)とショートスペーサータイプ(37S-HL、37S-LH)のそれぞれの遺伝子導入効率を検討した(図11の下二段)。いずれのタイプも1回の遺伝子導入で20~35%の遺伝子導入効率が得られ、ロングスペーサーとショートスペーサーの間に遺伝子導入効率の差は見られなかった。一方で、健常人ドナー由来CD3への遺伝子導入効率(図11の上二段)は、CD37L-HL及びCD37L-LHのロングスペーサーを持つCD37-CARにおいて遺伝子導入効率が高かった。注目すべきことに、ロングスペーサータイプを遺伝子導入した細胞では培養中に死細胞が多数見られ、総細胞数も増えなかった(図12)。活性化したCD3上に発現する弱いCD37発現を認識してCD37CAR-T細胞同士の攻撃、いわゆる同士討ち(fractricide)が起こり、見かけ上遺伝子導入効率が高いものと考えられた。対照的にショートスペーサータイプ(CD37S-HL、CD37S-LH)に関しては死細胞はそれほど見られず、総細胞数の増加も良好であった(図12)。尚、ショートスペーサータイプについては、その後の検討によって(3日目及び4日目に遺伝子導入)、遺伝子導入効率50~70%程度を達成している。
作成したCD37-CAR-T細胞のサイトカイン産生能をIFN-γの細胞内染色で評価した(図13)。K562(CD37陰性標的)、Ramos(CD37陽性標的)と共培養し(コントロールは標的細胞なし)、その後のサイトカイン産生を評価した。CD37L-HL及びCD37L-LHにおいてコントロール(標的細胞なし)、K562を標的の場合でもサイトカイン産生が見られ、CD37陽性標的(Ramos)に対しては顕著なサイトカイン産生が見られた。一方でCD37S-HL及びCD37S-LHにおいてはコントロール(標的細胞なし)、K562を標的の場合はほとんどサイトカイン産生が見られず、また、CD37陽性標的(Ramos)に対するサイトカイン産生は有意に見られた。尚、IL-2の産生能も同様の結果であった(図14)。
次に、CD37-CAR-T細胞をK562-CD37で刺激し、その後の細胞増殖を評価した(図15)。IL-2なしの条件下でもIL-2ありの条件下でもロングスペーサータイプ(37L-LH、37L-HL)はほとんど増殖が見られなかったが、ショートスペーサータイプ(37S-LH、37S-HL)はいずれも良好な刺激後増殖が確認された。ロングスペーサータイプのCD37L-CAR-T細胞では、T細胞上にごくわずかに発現しているCD37を標的として細胞傷害が起こり、CAR-T細胞の増殖を妨げたものと考えられた。
4.CD37S-CAR-T細胞の認識できるCD37抗原の表面密度の検討
ショートスペーサータイプのCD37S-CAR-T細胞上に発現するCD37をQIFIKITを用いて定量化した(図16)。遺伝子導入マーカーのみを遺伝子導入した陰性コントロールでは1000-2000程度のCD37抗体の結合が見られたものの、CD37S-CAR-T細胞では約200程度のCD37抗体の結合に低下していた。このことは、CD37S-CAR-T細胞はこれ以上の発現のある細胞は認識してしまう可能性を示唆している。CD37L-CAR-T細胞では同士討ち現象が観察されていることから、標的抗原の認識の閾値はより低いものと予想された。
ショートスペーサータイプのCD37S-CAR-T細胞上に発現するCD37をQIFIKITを用いて定量化した(図16)。遺伝子導入マーカーのみを遺伝子導入した陰性コントロールでは1000-2000程度のCD37抗体の結合が見られたものの、CD37S-CAR-T細胞では約200程度のCD37抗体の結合に低下していた。このことは、CD37S-CAR-T細胞はこれ以上の発現のある細胞は認識してしまう可能性を示唆している。CD37L-CAR-T細胞では同士討ち現象が観察されていることから、標的抗原の認識の閾値はより低いものと予想された。
5.CD37S-CAR-T細胞の腫瘍細胞株に対する作用(in vitroでの機能評価)
CD37S-CAR-T細胞を作成し、各種CD37陽性標的に対するサイトカイン産生能を評価した。腫瘍細胞株に関しては、CD37発現が低いもの(CD37抗体結合能で10000程度)であっても良好なサイトカイン産生能を示した(図17)。

一方、CD37S-CAR-T細胞を用いて共培養アッセイを行なった。CD37S-HL、CD37S-LHともにすべてのE:T比においてCD37陽性標的細胞を良好に抑制した(図18)。
6.CD37S-CAR-T細胞の正常細胞に対する作用(in vitroでの機能評価)
健常ドナーの末梢血をCD37低発現分画とCD37高発現分画に分取した(図19)。分画した細胞(CD37高発現細胞、CD37低発現細胞)とCD37S-CAR-T細胞との反応性の検討を行なった(図20)。陽性コントロールはRamos細胞に対するサイトカイン産生を示すものである。陰性コントロールとして、遺伝子導入マーカー(tEGFR)のみを遺伝子導入した細胞とCD37低発現細胞分画との反応を3名のドナー由来細胞において示した。CD37S-CAR-T細胞はCD37低発現細胞分画との共培養ではサイトカインを産生しなかったが、CD37陽性分画との共培養では有意なサイトカイン産生を示した。図6に示したように正常ドナーのCD37低発現分画は3000-5000程度のCD37抗体結合部位を持っているため、図16に示したCD37S-CAR-T細胞上のCD37抗体結合部位よりは多い。認識が起こるか起こらないかは、単なるCD37抗体結合部位の多寡のみでは決定されない可能性が示唆された。
健常ドナーの末梢血をCD37低発現分画とCD37高発現分画に分取した(図19)。分画した細胞(CD37高発現細胞、CD37低発現細胞)とCD37S-CAR-T細胞との反応性の検討を行なった(図20)。陽性コントロールはRamos細胞に対するサイトカイン産生を示すものである。陰性コントロールとして、遺伝子導入マーカー(tEGFR)のみを遺伝子導入した細胞とCD37低発現細胞分画との反応を3名のドナー由来細胞において示した。CD37S-CAR-T細胞はCD37低発現細胞分画との共培養ではサイトカインを産生しなかったが、CD37陽性分画との共培養では有意なサイトカイン産生を示した。図6に示したように正常ドナーのCD37低発現分画は3000-5000程度のCD37抗体結合部位を持っているため、図16に示したCD37S-CAR-T細胞上のCD37抗体結合部位よりは多い。認識が起こるか起こらないかは、単なるCD37抗体結合部位の多寡のみでは決定されない可能性が示唆された。
7.考察/まとめ
(1)CD37CAR-Tの構造を変化させることによって、標的の認識の閾値を変化させ得ることを示した。CD37のように、標的とする腫瘍細胞以外にも発現が見られる抗原をターゲットとしたCARの開発においてはCARの細胞外ドメインの構造を変化させることによって、必要とする程度の認識能力を得ることができる。従来、CD19-CARでも細胞外ドメインのスペーサーの長さを調節することで効果に差が出ることは知られていたが、長期的なin vivoでの効果の差を見ているにすぎず、直接的なin vitroでの効果の違いは検討されていなかった。今回、スペーサー・ドメインの長さの調節によって、in vitroでの認識できる抗原密度をコントロールし得ることを示した。このような制御は、他の標的に対するCARの開発にも利用でき、一般化し得ると考えられる。
(2)今回の成果はCD37CARに限らず、他の抗原を標的としたCARに応用可能である。真にがん特異的な抗原はこれまで得られておらず、これからも得られる見込みは少ない。すなわち今後もCAR-T細胞療法の標的抗原は多少の標的細胞外の発現は見込まなければならないことが予想される。そういった状況において新規のCARの開発にあたって安全性を担保するために、今回の検討で実証した「スペーサー・ドメイン構造の調節による標的抗原の認識閾値のコントロール」は、所望の標的認識レベルを得る手法として極めて有用と考えられる。今後、当該手法を利用又は応用することにより、正常細胞上にもある程度の発現が認められる抗原に対するCAR-Tの開発が可能になると考えられる。

<スペーサーの長さに関する検討>
1.スペーサーの長さが異なるCARの設計
CD37-CARのスペーサーの長さが抗原認識に与える影響を詳細に検討するため、以下に示す、スペーサーの長さが異なる複数のCD37-CARを作成した(以下のa~f。図21も参照)。スペーサーにはIgG4のFcヒンジとCH2領域及びCH3領域を利用した。Fc側から徐々に延長したものとしてスペーサーを設計し、FcヒンジからCH3領域の最後までを持つものを最長のスペーサーとした。

1.スペーサーの長さが異なるCARの設計
CD37-CARのスペーサーの長さが抗原認識に与える影響を詳細に検討するため、以下に示す、スペーサーの長さが異なる複数のCD37-CARを作成した(以下のa~f。図21も参照)。スペーサーにはIgG4のFcヒンジとCH2領域及びCH3領域を利用した。Fc側から徐々に延長したものとしてスペーサーを設計し、FcヒンジからCH3領域の最後までを持つものを最長のスペーサーとした。

各CDR37-CARをコードする遺伝子をoverlapping PCRを用いて作成し、pLZRS-BMN-Zベクターに挿入した。このベクターを用いて遺伝子導入用レトロウイルスを作成し、CD3に純化した健常ドナー由来リンパ球に遺伝子導入を行なった。尚、各CAR37-CARの発現用コンストラクトの配列(HindIIIサイトとKozak配列に続いて、CAR遺伝子、T2A配列(配列番号18)、tEGF(truncated EGF)をコードする配列(配列番号19)、ストップコドン、及びNotIサイトが順に配置される)は以下の通りである。

2.結果
0日目にCD3/CD28ビーズで刺激後、3日目にレトロウイルスを用いて遺伝子導入し、7日目に遺伝子導入マーカーを利用してCAR陽性細胞を選択した。選択した細胞をRamosで刺激し、細胞を増殖させた。

遺伝子導入後の細胞数を計測したところ、d-fのCD37-CARは細胞増殖(遺伝子導入後の細胞数)が不良であった(図22A)。一方で、これらの遺伝子導入効率(図22B)は高く、遺伝子導入細胞数(遺伝子導入細胞数×遺伝子導入効率。図22C)は少なくなった。このことは、CD37-CARを遺伝子導入した直後から同士討ち(fratricide)が発生していることを示唆する。生細胞の割合(図22中段)は、スペーサーの長さが30アミノ酸より長くなると下がり、刺激を受けて死ぬ細胞がいることがわかる。また、スペーサーの長さが30アミノ酸より長くなると遺伝子導入効率は上昇し(図22下段)、これは、CARがCAR陰性細胞のCD37を認識・傷害した結果と考えられる。
Ramosで刺激した後の細胞増殖(図23上段)をみると、スペーサーの長さが125アミノ酸と232アミノ酸のCAR(e、f)では10日目から13日目にかけて増殖効率が低下し、スペーサーが長いと増殖能力が低下することを示唆した。
以上の結果を総合すると、同士討ちによるシグナルの発生を防止し、CAR-T細胞の増殖能力を維持するためには、スペーサーの長さが0~30アミノ酸のCARを用いるのが良いことがわかる。
本発明は、CAR療法で問題とされるOn-target副反応への解決策となり得るものであり、安全性が高められたCAR療法の確立に貢献する。従って、その臨床上の意義は大きい。特に、本発明のCAR構造決定法は汎用性に優れるものであり、標的分子が異なる様々なCARの設計に応用でき、その利用価値は極めて大きい。
この発明は、上記発明の実施の形態及び実施例の説明に何ら限定されるものではない。特許請求の範囲の記載を逸脱せず、当業者が容易に想到できる範囲で種々の変形態様もこの発明に含まれる。本明細書の中で明示した論文、公開特許公報、及び特許公報などの内容は、その全ての内容を援用によって引用することとする。

- CD37特異的な抗原認識領域を含む細胞外ドメインと、
前記抗原認識領域に直接、又は1~30個のアミノ酸からなる直鎖状スペーサーを介して連結した膜貫通領域と、
前記膜貫通領域に連結した、免疫細胞のエフェクター機能のための細胞内シグナルドメインと、を含む、CD37特異的キメラ抗原レセプター。 - 前記抗原認識領域が、抗CD37抗体の軽鎖可変領域(VL)と重鎖可変領域(VH)がリンカーで連結した構造の単鎖抗体である、請求項1に記載のCD37特異的キメラ抗原レセプター。
- 前記スペーサーがヒトIgGのヒンジ部又はその一部からなる、請求項1又は2に記載のCD37特異的キメラ抗原レセプター。
- 前記軽鎖可変領域が配列番号1のアミノ酸配列を含み、前記重鎖可変領域が配列番号2のアミノ酸配列を含む、請求項1~3のいずれか一項に記載のCD37特異的キメラ抗原レセプター。
