背景技术
放射治疗是指采用高能射线杀死肿瘤,是一种重要的非入侵式肿瘤治疗手段。用于放射治疗的放射治疗设备主要包括机架和治疗床,在机架上安装有用于发射高能射线的治疗头。在进行放射治疗时,需要使患者的治疗部位与治疗头发射的射线的焦点对准,使射线精确地命中位于体内的靶区,从而避免损伤靶区附近的健康组织,因此需要保证患者的患部在放射治疗期间不发生移动。但是,有时放射治疗的时间会长达十几分钟甚至一个小时,在如此长的时间内很难保证患者的治疗部位不发生移动,因此,需要对患者治疗部位在放射治疗期间是否发生移动进行监测,当发现患者治疗部位发生移动后及时采取相应的措施,以保证放射治疗的顺利进行。
相关技术中采用影像引导放射治疗技术(IGRT),利用成像设备获取靶区及靶区周边一定范围内正常组织的图像并与治疗计划图像对比来对患者治疗部位在放射治疗期间的位置进行监测。
然而,相关技术至少存在以下问题:
采用影像引导放射治疗技术对患者治疗部位在放射治疗期间的位置进行监测需要昂贵的成像设备,从而增加放射治疗成本。
公开内容
为解决上述技术问题,本公开实施例提供一种不需昂贵的成像设备即可对患者治疗部位在放射治疗期间的位置进行监测的位置监测系统及放射治疗设备。

至少一个第一标记部件2,设置于患者S的待治疗部位;
至少一个第二标记部件3,设置于预定位置;
移动监测装置4,设置于第一支架1上,用于监测第一标记部件2相对于第二标记部件3发生的位移;
控制装置(图中未示出),与移动监测装置4连接,用于根据移动监测装置4监测的位移确定患者S的待治疗部位的位置是否发生移动。
使用本公开实施例提供的位置监测系统时,在位于患者支撑装置A上的患者S的待治疗部位设置至少一个第一标记部件2,并将第二标记固定于预定位置,这样,位于第一支架1上的移动监测装置4就可以实时监测位于患者S的待治疗部位上的第一标记部件2相对于固定在预定位置上的第二标记部件3发生的位移大小,再将该位移发送给与该移动监测装置4连接的控制装置,由该控制装置根据该位移判断患者S的待治疗部位是否发生移动。
本公开实施例提供的位置监测系统基于第一标记部件2相对于第二标记部件3的位移来对患者S的待治疗部位的位置进行监测,不需要使用昂贵的成像系统,成本较低。
在本公开实施例一种可能的实现方式中,通过以下原理来对患者S的待治疗部位是否发生移动进行监测。
将第一标记部件2设置于患者S的待治疗部位,将第二标记部件3固定在预定位置之后,移动监测装置4首先获取移动监测装置4与第一标记部件2之 间的第一初始距离信息,以及移动监测装置4与第二标记部件3之间的第二初始距离信息,并根据第一初始距离信息及第二初始距离信息确定第一标记部件2与第二标记部件3之间的初始距离信息,之后移动监测装置4实时获取移动监测装置4与第一标记部件2之间的实时距离信息,根据移动监测装置4与第一标记部件2之间的实时距离信息以及上述移动监测装置4与第二标记部件3的第二初始距离信息确定第一标记部件2与第二标记部件3的实时距离信息,根据第一标记部件2和第二标记部件3这二者之间的初始距离信息和实时距离信息,确定第一标记部件2和第二标记部件3的位移。移动监测装置4将得到的第一标记部件2和第二标记部件3的位移传输至控制装置,控制装置将该位移与预设位移进行比较。如果该位移小于或者等于预设位移,则不认为第一标记部件2相对于其初始位置发生了移动,也就是患者S的待治疗部位没有发生移动;如果该位移大于预设位移,则说明第一标记部件2相对于其初始位置发生了移动,也就是患者S的待治疗部位发生了移动。
移动监测装置4也可获取移动监测装置4与第二标记部件3之间的实时距离信息,并将移动监测装置4与第二标记部件3之间的实时距离信息传输给控制装置,控制装置根据移动监测装置4与第二标记部件3之间的实时距离信息以及上述二者之间的第二初始距离信息判断移动监测装置4与第二标记部件3的相对位置是否发生了变化,以防止由于第二标记部件3和/或移动监测装置4的位置发生变化而导致对患者S待治疗部位的位置变化情况做出错误的判断。
上述移动监测装置4与第一标记部件2之间的距离信息至少包括二者之间的距离数值,还可以包括方向信息,上述移动监测装置4与第二标记部件3之间的距离信息至少包括二者之间的距离数值,还可以包括方向信息。
需要说明的是,第一支架1与患者支撑装置A活动连接是指在位置监测系统处于非工作态时第一支架1与患者支撑装置A可发生相对运动,以便于调整移动监测装置4与第一标记部件2、第二标记部件3之间的相对位置,提高对患者S的待治疗部位位置监测的准确性,并且防止患者S在上下患者支撑装置A时发生碰撞;而在位置监测系统工作过程中,第一支架1与患者支撑装置A能够保持固定,第一支架1的位置不发生变化。
对于第一标记部件2和第二标记部件3的数量本公开实施例不做特殊限制,可根据实际需要确定。第一标记部件2的数量与第二标记部件3的数量可以相同,也可以不同。当第一标记部件2和第二标记部件3均设置有多个时, 可分别确定每个第一标记部件2和每个第二标记部件3之间的位移,从而综合判断患者S的待治疗部位是否发生移动,提高对患者S待治疗部位位置监测的准确性。
可选的,患者支撑装置A的具体形式为床,患者S躺在患者支撑装置A上。
本公开实施例提供位置监测系统中,第一支架1与患者支撑装置A活动连接的具体方式包括但不限于以下方式:第一支架1以可绕着某一旋转轴旋转的方式与患者支撑装置A连接;第一支架1以可沿患者支撑装置A的长度方向移动的方式与患者支撑装置A连接。
对于第一支架1以可绕着某一旋转轴旋转的方式与患者支撑装置A连接的方式来说,可在第一支架1和患者支撑装置A上分别开设连接孔,旋转轴穿过第一支架1上的连接孔和患者支撑装置A上的连接孔将第一支架1与患者支撑装置A连接。
对于第一支架1以可沿患者支撑装置A的长度方向移动的方式与患者支撑装置A连接的方式来说,可在患者支撑装置A上设置滑槽,滑槽的长度方向与患者支撑装置A的长度方向平行,第一支架1具有位于滑槽内且与滑槽间隙配合的端部;也可在患者支撑装置A上设置具有凸起的滑轨,滑轨的长度方向与患者支撑装置A的长度方向平行,在第一支架1上设置有与滑轨的凸起间隙配合的凹槽。对于滑槽或者滑轨的数量本公开实施例不做严格限制,例如可以为一条、两条、三条等。
为了保证在位置监测系统工作过程中,第一支架1与患者支撑装置A保持固定,保证对于患者S的治疗部位位置监测的准确性,本公开实施例提供的位置监测系统还可以包括:用于将第一支架1与患者支撑装置A固定锁紧的锁紧装置(图中未示出)。
其中,对于上述第一支架1以可绕着某一旋转轴旋转的方式与患者支撑装置A连接的方式来说,锁紧装置可在第一支架1转动到预设角度时将第一支架1与患者支撑装置A固定锁紧,防止第一支架1在位置监测系统工作过程中发生转动。
对于第一支架1以可沿患者支撑装置A的长度方向移动的方式与患者支撑装置A连接的方式来说,锁紧装置可在第一支架1移动到患者支撑装置A的预定位置处时将第一支架1与患者支撑装置A固定锁紧,防止第一支架1在位 置监测系统工作过程中移动。
本公开实施例中,锁紧装置的具体形式包括但不限于锁紧销和锁紧螺栓,只要能够将第一支架1与患者支撑装置A固定锁紧,使第一支架1在位置监测系统运行过程中保持稳定即可。
对于第一支架1的形状本公开实施例同样不做特殊限定。可选的,第一支架1包括两个分别与患者支撑装置A宽度方向的两侧连接的端部,以及连接两个端部的连接部,并且第一支架1与患者支撑装置A之间可容纳患者S,例如可以为U形支架、H形支架等。第一支架1也可以为I形支架、L形支架等。
下面,以患者支撑装置A的形式为床,第一支架1为U形支架为例,对第一支架1以可沿患者支撑装置A的长度方向移动的方式与患者支撑装置A连接做进一步说明。
参见图2,其示出了一种U形的第一支架1的结构,其中U形的第一支架1包括第一连接臂11、第二连接臂12和第三连接臂13。其中,第一连接臂11的一端和第三连接臂13的一端分别与第二连接臂12的两端连接。
参见图3-1(a)和图3-1(b),其示出了一种U形的第一支架1与患者支撑装置A的连接方式。在该连接方式中,患者支撑装置A上设置两条滑槽,两条滑槽分别位于患者支撑装置A宽度方向的两侧;第一连接臂11的未连接第二连接臂12的一端和第三连接臂13的未连接第二连接臂12的一端分别位于两条滑槽内且与滑槽间隙配合。移动监测装置4固定在第二连接臂12上。第二连接臂12距离患者支撑装置A上表面的高度可使患者S的身体通过。
滑槽可以如图3-1(a)和图3-1(b)所示设置在患者支撑装置A的侧表面,也可以设置在患者支撑装置A的上表面。
同时,对于图3-1(a)和图3-1(b)所示的连接方式,锁紧装置的具体实现可以为:在滑槽两侧的患者支撑装置A的对应位置处开设连接孔,同时在第一连接臂11和第三连接臂13未连接第二连接臂12的端部上开设连接孔,以上各连接孔的轴线方向均沿患者支撑装置A的宽度方向设置,且患者支撑装置A上靠近患者支撑装置A中心的连接孔为盲孔。当锁紧装置为锁紧销时,各连接孔采用普通的开孔即可;当锁紧装置为锁紧螺栓时,患者支撑装置A上靠近患者支撑装置A中心的连接孔设置为螺纹孔。将第一支架1移动至预设位置、使第一连接臂11和第三连接臂13上的连接孔与患者支撑装置A的连接孔对准后,将锁紧销或者锁紧螺栓依次穿过各个连接孔将第一支架1和患者支撑装置 A固定锁紧;将锁紧销或者锁紧螺栓拆卸后可使第一支架1恢复移动。可以在沿患者支撑装置A的长度方向上每隔一定距离设置一组连接孔,以便于能够将第一支架1根据需要固定在患者支撑装置A的不同位置。
参见图3-2(a)和图3-2(b),其示出了另一种U形的第一支架1与患者支撑装置A的连接方式。在该连接方式中,在第一连接臂11未连接第二连接臂12的端部和第三连接臂13未连接第二连接臂12的端部设置凹槽,在患者支撑装置A上设置两条具有凸起的滑轨。两条滑轨分别位于患者支撑装置A宽度方向的两侧,第一连接臂11上的凹槽和第三连接臂13上的凹槽分别卡接在两条滑轨的凸起上,且凹槽和凸起间隙配合。
同样地,滑轨可以如图3-2(a)和图3-2(b)所示设置在患者支撑装置A的侧表面,也可以设置在患者支撑装置A的上表面。
本公开实施例中,第一标记部件2在患者S待治疗部位的设置方式可根据患者S的待治疗部位的具体情况确定,本公开实施例不做严格限定。以患者S的待治疗部位为头部为例,第一标记部件2可通过粘贴的方式设置在患者S的额头、鼻尖、下巴或耳朵等部位。
第二标记部件3可以直接固定在预定位置,也可以间接固定在预定位置,预定位置包括但不限于患者支撑装置A上的某个位置。
在一种可能的实现方式中,参见图4,其示出了另一种位置监测系统的结构,在该位置监测系统中,至少一个第二标记部件3设置在第二支架5上,第二支架5固定于预定位置,通过第二支架5将第二标记部件3设置在预定位置。
如图5所示,第二支架5也可以为U形支架,该U形支架包括两条U形臂51和连接两条U形臂51的连接臂52。第二标记部件3设置在U形支架的U形臂51上。当第二标记部件3的数量为多个时,可分别在U形支架的两条U形臂51上设置第二标记部件3。并且,当第二标记部件3的数量为偶数时,设置在两条U形臂51上的第二标记部件3的数量可相同。
预定位置可根据患者S待治疗部位的具体情况确定。如图4所示,对于治疗头部肿瘤来说,预定位置可为患者S的头部在患者支撑装置A上的位置。当患者支撑装置A为床时,预定位置为患者支撑装置A长度方向的一端,且U形支架的连接臂52固定在患者支撑装置A上,并使患者S的头部位于U形支架内。
第二支架5也可为其他形状,例如,L形、H形等,本公开实施例不做特 殊限定。
第二标记部件3可通过螺纹连接、粘接、卡接等方式固定在第二支架5上。
需要说明的是,第二支架5也可与患者支撑装置A活动连接,在位置监测系统处于非工作态时第二支架5与患者支撑装置A可发生相对运动,而在位置监测系统工作过程中,第二支架5与患者支撑装置A能够保持固定,使设置在第二支架5上第二标记部件3的位置不发生变化。第二支架5与患者支撑装置A活动连接的具体方式包括但不限于:第二支架5以可绕着某一旋转轴旋转的方式与患者支撑装置A连接;第二支架5以可沿患者支撑装置A的长度方向移动的方式与患者支撑装置A连接。也可设置锁紧装置来将第二支架5固定与患者支撑装置A固定。具体的实现方式可参见上文对于第一支架1与患者支撑装置A的连接方式的描述,在此不再赘述。
本公开实施例中,移动监测装置4可通过与第一标记部件2和第二标记部件3之间进行信息交互来获取移动监测装置4与第一标记部件2、第二标记部件3的距离信息。
在一种可能的实现方式中,移动监测装置4通过光束的发射与接收来获取移动监测装置4与第一标记部件2、第二标记部件3的距离信息。
移动监测装置4可以包括至少两组光束收发单元,相应的,第一标记部件2和第二标记部件3均为可反射光束的标记部件;其中,一组光束收发单元与一个可反射光束的标记部件相对应。
移动监测装置4还可以包括监控单元,监控单元与光束收发单元电连接。监控单元控制与第一标记部件2相对应的光束收发单元向第一标记部件2发射光束,并接收由第一标记部件2反射回的光束,根据收发光束之间的时间差确定移动监测装置4与第一标记部件2之间的第一初始距离信息和实时距离信息;控制与第二标记部件3相对应的光束收发单元向第二标记部件3发射光束,并接收由第二标记部件3反射回的光束,根据收发光束之间的时间差确定移动监测装置4与第二标记部件3之间的第二初始距离信息,从而监测第一标记部件2和第二标记部件3的位移,并将第一标记部件2和第二标记部件3的位移发送给控制装置。监控单元还可以获取光束传播的方向,根据收发光束之间的时间差以及光束传播的方向综合确定移动监测装置4与第一标记部件2、第二标记部件3之间的距离信息。
第一标记部件2和第二标记部件3可以为由对光束具有反射能力的材料 (例如金属、陶瓷等)制成的反射块、反射片、反射条等,也可以在块状、片状或者条状的基底上形成光反射层。
可以理解的是,对于通过光束收发来监测第一标记部件2和第二标记部件3的位移来说,移动监测装置4、第一标记部件2和第二标记部件3所设置的位置应当使移动监测装置4发出的光束能够达到第一标记部件2和第二标记部件3,并且,移动监测装置4能够接收到由第一标记部件2、第二标记部件3反射回的光束。
光束收发单元包括但不限于激光收发器或者红外线收发器。激光收发器即能够发射和接收激光的器件,红外线收发器即能够发射和接收红外线的器件。
本公开实施例提供的位置监测系统还可以包括:与控制装置电连接的提示装置,用于提示用户患者S的待治疗部位的位置发生移动。提示装置可以为声音提示装置,也可以是灯光提示装置,也可以是图像、文字提示装置,还可以是多种提示方式的组合。
另一方面,本公开实施例提供了一种放射治疗设备,参见图6,其示出了本公开实施例提供的一种放射治疗设备的结构,该放射治疗设备包括患者支撑装置A,还包括上述的位置监测系统,其中,位置监测系统中的第一支架1与患者支撑装置A活动连接。
如图6所示,该放射治疗设备中,患者支撑装置A为床,且在患者支撑装置A长度方向的一端设置有治疗舱B。
在患者S进行放射治疗之前,首先对位于患者支撑装置A上的患者S进行摆位固定。摆位固定的过程包括但不限于:在患者S的待治疗部位固定第一标记部件2;将第二标记部件3固定在预定位置,对于治疗头部肿瘤来说,第二标记部件3固定在第二支架5上,第二支架5固定在患者支撑装置A靠近治疗舱B的一端;将第一支架1移动调整至合适的位置并将第一支架1与患者支撑装置A固定。之后,使患者S的待治疗部位进入放射治疗设备的治疗舱B进行治疗的过程中,该位置监测系统就可以实时的监控该患者S的待治疗部位是否发生移动。
在放射治疗开始前,对患者S进行初始摆位后,可以先获取多组移动监测装置4与第一标记部件2的第一初始距离信息以及移动监测装置4与第二标记的初始距离信息,待上述第一初始距离信息和第二初始距离信息数据稳定之 后,再确定第一标记部件2和第二标记部件3的初始距离信息。
本公开实施例提供放射治疗设备中,可以将位置监测系统中的控制装置与用于控制放射治疗开始或者停止的装置电连接,当位置监测系统的控制装置监控到患者S的待治疗部位发生移动后,向用于控制放射治疗开始或者停止的装置发出信号,使放射治疗停止,等待医务人员对患者S进行调整;也可以通过发出提示信息提示医务人员患者S的待治疗部位发生了移动,由医务人员停止放射治疗并对患者S进行调整,避免射线对患者S靶区周围正常组织造成损伤。
当医务人员对患者S进行调整之后,需要将之前的第一标记部件2和第二标记部件3的初始距离信息清零,并重新确定第一标记部件2和第二标记部件3的初始距离信息,在后续的放射治疗过程中,以新的初始距离信息作为基准,来判断患者S待治疗部位是否发生移动。
以上所述仅是为了便于本领域的技术人员理解本公开的技术方案,并不用以限制本公开。凡在本公开的精神和原则之内,所作的任何修改、等同替换、改进等,均应包含在本公开的保护范围之内。

Claims (13)

  1. 一种位置监测系统,包括:
    第一支架,用于与支撑患者的患者支撑装置活动连接;
    至少一个第一标记部件,设置于所述患者的待治疗部位;
    至少一个第二标记部件,设置于预定位置;
    移动监测装置,设置于所述第一支架上,用于监测所述第一标记部件相对于所述第二标记部件发生的位移;
    控制装置,与所述移动监测装置连接,用于根据所述移动监测装置监测的位移确定所述患者的待治疗部位的位置是否发生移动。
  2. 根据权利要求1所述的位置监测系统,其中,所述第一支架以可沿所述患者支撑装置的长度方向移动的方式与所述患者支撑装置连接。
  3. 根据权利要求2所述的位置监测系统,其中,在所述患者支撑装置上设置有滑槽,所述滑槽的长度方向与所述患者支撑装置的长度方向平行;
    所述第一支架具有位于所述滑槽内且与所述滑槽间隙配合的端部。
  4. 根据权利要求1所述的位置监测系统,其中,所述位置监测系统还包括:用于将所述第一支架与所述患者支撑装置固定锁紧的锁紧装置。
  5. 根据权利要求4所述的位置监测系统,其中,所述锁紧装置为锁紧销或者锁紧螺栓。
  6. 根据权利要求1所述的位置监测系统,其中,所述第一支架包括两个分别与所述患者支撑装置宽度方向的两侧连接的端部,以及连接两个所述端部的连接部,并且所述第一支架与所述患者支撑装置之间可容纳所述患者。
  7. 根据权利要求1所述的位置监测系统,其中,所述至少一个第二标记部件设置在第二支架上,所述第二支架固定于所述预定位置。
  8. 根据权利要求7所述的位置监测系统,其中,所述第二支架为U形支架,所述U形支架的U形臂上分别设置有所述第二标记部件。
  9. 根据权利要求7所述的位置监测系统,其中,所述预定位置为所述患者支撑装置的一端。
  10. 根据权利要求1所述的位置监测系统,其中,所述移动监测装置包括至少两组光束收发单元,相应的,所述第一标记部件和所述第二标记部件均为可反射光束的标记部件;其中,一组光束收发单元与一个可反射光束的标记部件相对应。
  11. 根据权利要求1所述的位置监测系统,其中,所述光束收发单元为激光收发器或者红外线收发器。
  12. 根据权利要求1所述的位置监测系统,其中,所述位置监测系统还包括:与所述控制装置电连接的提示装置,用于提示用户所述患者的待治疗部位的位置发生移动。
  13. 一种放射治疗设备,包括:患者支撑装置,所述放射治疗设备还包括如权利要求1~12任一项所述的位置监测系统,其中,所述第一支架与所述患者支撑装置活动连接。
