Definitions
- the invention relates to coding medical records and more particularly to a system and a method managing such records in a dynamic manner with a minimal risk of coding error which enables clinical data exchange for the sake of promotion of medical treatment quality, account management in the health system, medical researches and regulatory reporting system.
- the main task of medical coders is to review clinical statements and assign standard codes such as CPT®, ICD-10-CM, and HCPCS Level II classification systems.
- Medical billers process and follow up on claims sent to health insurance companies for reimbursement of services rendered by a healthcare provider.
- the medical coder and medical biller may be the same person or may work with each other to ensure invoices are paid properly.
- the coder checks the patient's medical record (i.e., the transcription of the doctor's notes, ordered laboratory tests, requested imaging studies, and other sources) to verify the work that was done.
- US 20150039330 discloses an episode builder which is a web-based computer application and its associated architecture that allow health care professionals to define and describe episodes of care.
- the episode builder provides a tool for describing an episode of care so that the health care professional can create and modify episodes for a variety of purposes.
- the episode builder includes a configurable and extensive code set management system, a suite of display panels that define the episodes themselves based upon the code sets, and a means to export the episode definition tables created to be used as inputs into an analytic program or a claims adjudication engine.
- Many categories of the abovementioned medical classification systems include the codes attributed to a group of diseases which are classified as "others". In other words, one code is attributed to a plurality of diseases which brings incertitude between medical billers and medical insurance companies.
- the aforesaid method comprises the steps of: (a) providing a computer system comprising: (i) a processor; (ii) memory configured for storing electronic medical records (EMR) concerning medical services provided by the healthcare organization; and (iii) a graphic user interface configured for displaying a categorized Classification of Diseases and Related Health Problems, searching a relevant medical condition and a coded clinical term thereof and selecting thereof; (b) querying a patient's personal EMR stored in the memory by inputting a patient's ID or creating if the personal EMR does not exist; (c) establishing a diagnosis and a concomitant treatment characterizing the medical condition of the patient; (d) searching the coded clinical term relevant to the medical condition of the patient within the categorized Classification of Diseases and Related Health Problems; (e) selecting the relevant coded clinical term if the coded clinical term is uniquely

- Figure is a flowchart of a computer-implemented method of creating or updating medical records associated with patient's encounter with a healthcare organization.
- a system for implementing the abovementioned method comprises a processor; (ii) memory configured for storing electronic medical records (EMR) concerning medical services provided by the healthcare organization; and (iii) a graphic user interface configured (GUI) for displaying a categorized Classification of Diseases and Related Health Problems, searching a relevant medical condition and a code thereof and selecting thereof.

- a diagnosis and/or concomitant treatment characterizing a medical condition of a patient is established (step 130).
- a health care worker for example a doctor, carries out a search of coded clinical terms suitable to the medical condition of the patient by means of the system GUI (step 140).
- the method 100 is bifurcated at step 150 according to the stipulation if the obtained coded clinical term is uniquely attributable to the medical condition of the patient or not.
- the health care worker selects the relevant coded clinical term (step 160) and saves it in the personal EMR( step 170).
- the health care worker saves a descriptor of the medical condition in a form of free text in the personal EMR (step 180). Then, the saved free-text descriptor is analyzed by a medical record clerk (step 190), who assigns an additional code uniquely attributable to the medical condition (step 200). The additional code is also saved in the personal EMR (step 210). It should be appreciated that that the present invention is also directed to facilitating big data management in order to identify the same condition among different patients by means of identical coded clinical terms.
- the computer-implemented system for creating or updating medical records associated with patient's encounter with a healthcare organization is preprogrammed for (a) displaying a patient's personal EMR stored in the memory; (b) displaying categorized Classification of Diseases and Related Health Problems; (c) saving the relevant code in the personal EMR.
- the computer-implemented system is configured for (a) saving the medical condition in a form of a free text in the personal EMR if no code is uniquely attributable to the medical condition of the patient; (b) assigning an additional code uniquely attributable to the medical condition of the patient by a medical statistics clerk after analyzing the free text; and saving the additional code in the personal EMR.
