本发明属于医疗器械,更详细地讲是属于将按摩与药物治疗相结合,治疗胃癌转移的医疗器械。
背景技术
胃癌转移是胃癌患者死亡的主要原因,所以胃癌转移是治疗的关键。20世纪90年代以来,由于癌症手术后复发、转移率仍很高,为了防止术后复发、转移,开始采用了术后辅助化疗,但其结果并不尽如人意,未能阻止许多患者复发和转移,并且化疗对机体损伤消耗极大,许多病人难以承受其毒副作用。当前肿瘤化学治疗的现状,主要为全身静脉化疗,腹部肿瘤(如胃、肠、肝、胆、胰恶性肿瘤)也均采用全身静脉化疗。《癌症治疗新概念与新方法》(人民军医出版社)作者徐泽认为:消除了化疗不良反应,就是为千万癌症化疗患者造福,解决的办法是改变全身静脉化疗给药途径。
现有技术的给药途径有口服给药、注射给药、透皮给药等主要方法。对于透皮给药,传统医学中的药物外敷、膏药治疗肿瘤,应用非常广泛并且有效。但是对于药物透过皮肤角质层吸收的机制和原理,至今仍在探索中。由中国铁道出版社1994年出版、日本经济计划厅综合计划局编著的《推动经济的2010年的技术预测101项》中,日本武田药品工业编著的“生命科学——药物输送系统”中认为:“药物输送系统是新一代的治疗方法,为了取得突破所必需的关键技术,有对药物经过皮肤、粘膜吸收的研究等。药物(透皮吸收)输送系统是使药效更好,使用过去因为副作用不能开发的化合物,开发成药品,它的市场规模寄希望于非常广泛领域的药品应用,使医药界获得新的生机。”
现有技术、本系列产品《癌症疾病治疗器》ZL200610069987·8,2012.7.27获日本发明专利特许第5047301号,将抗癌药物透过皮肤吸收进行癌症治疗。按摩的主要作用力是按压力、摩擦力、振动力,但是《癌症疾病冶疗器》的药物按摩只能产生按压力与摩擦力,不能产生振动力,而振动冲击力是药物透过皮肤角质层不可缺少的因素,可以将挤压强度提高5-10倍。皮肤角质层是药物透皮吸收的主要障碍,振动可引起皮肤角质层脂质双分子层的振动,造成角质层脂质结构排列的无序化,从而加大加快药物通过皮肤的速度,如产生振动力的空化作用是超声波透皮给药的主要机制。所以,由于现有技术药物按摩给药途径存在着缺乏振动力的不足,不能达到进行透皮给药化疗以代替全身静脉给药的治疗目标。
发明内容
本发明的目的,在于针对现有技术全身静脉化疗只杀癌细胞不恢复功能、效果低 副作用大的不足,将机壳、电机、振动摩擦头、弹簧、调速开关、手把、药布、药布固定套、化疗药物组成的整体,通过透皮给药途径对胃癌转移进行局部化疗和全身化疗的主治疗,采用扶正类外治中药时,可与放疗相结合进行恢复功能辅助治疗。振动摩擦头在高速转动中可产生振动冲击力,促进药物透过皮肤角质层进行治疗。本发明通过机械能、物理热能、化学能相结合产生的整体作用力,按照《胃癌转移产生与发展过程示意图》所述技术方案,进行杀灭癌细胞化疗和恢复功能相结合的整体治疗,可对原发癌灶、转移灶进行局部化疗,进行大面积全身性化疗。可预防胃癌转移产生和显著提高胃癌转移化疗效果与降低副作用,并通过改变给药途径克服现有技术全身静脉化疗的不足,从而采用透皮给药化疗达到控制并治愈胃癌转移、显著降低患者死亡率的发明目的。

在胃癌转移治疗器中,机壳内安装电机、振动摩擦头,电机带动振动摩擦头高速转动,振动摩擦头与固定在机壳上的药布进行摩擦,摩擦产生的挤压冲击力和热能透过药布传导到皮肤上进行透皮给药治疗,调速开关安装在手把部,药布固定套将药布固定在机壳头端部,机壳内按装有4-32个振动摩擦头。胃癌转移治疗器在浅表层次进行透皮给药为主治疗 时,采用低压高速转动手握压力为0.2公斤/平方厘米,转速1500转-3000转/分钟;在转深层次进行物理按摩为主治疗时,采用高压低速转动手握压力为2公斤/平方厘米,转速300转-600转/分钟。
1、本发明的特点
摩擦作用力度的大小,主要取决于摩擦面的粗糙不平程度和摩擦速度,对于药物按摩的摩擦生热透皮给药效率来讲,压力和转速的影响是相同的,如果采用表面凸起高低不平的摩擦头并提高转速,可以显著提高摩擦生热透皮给药效率。药物按摩时按压力和摩擦力是物理治疗的主要作用力,但是对于透皮给药为主治疗来讲,产生放松作用的振动冲击力是不可缺少的,它可以提高挤压强度5-10倍使结构致密的角质层产生裂隙,有利于药物分子快速通过皮肤,完成透皮给药治疗。人体四肢腰背部的骨骼肌,可以加压力进行按压摩擦,但腹部内脏是柔软的平滑肌,单靠加大垂直按压力摩擦不如按摩骨骼肌效果好,特别是在促使药物分子透过皮肤角质层方面,效果并不理想,只有增加振动产生冲击力,才能达到透皮给药效果。皮肤角质层是进行透皮给药的主要障碍,缺少按压、摩擦、振动整体因素中的任何一方因素,都不能在较短的时间内完成透皮给药,达到治疗目标。
在本发明中,药液涂在皮肤上与药布接触,当摩擦头挤压药布旋转摩擦,转速与人工按摩相同每分钟200-300转时,会有少量药液通过汗腺毛孔渗入皮肤并产生一定的温度,但是当转速提高10倍或100倍时,就会产生非常强的挤压冲击力和高热温度,并透过药布促进药液和热能透过皮肤角质层进行透皮给药治疗,转速越高,挤压冲击力度和热度就越高。药物按摩的透皮给药治疗,是一种摩擦化学作用,属力化学、物理化学的重要分支,现代科学将摩擦碰撞定义为:“在极短促的时间内集中作用的力”。在实验中高速摩擦3秒钟可达到高热,15秒钟内完成局部透皮给药治疗,由于透皮给药时间短,药物对皮肤的副作用小。在传统医学中,膏摩——人工药物按摩是中医药外治主要方法,有数千年历史,目前与现代科学技术相结合,采用仪器进行药物按摩,可望成为药物外治主要方法。
2、与现有技术对比
当采用人工药物按摩时,无论是绕中心转动和平面直推,摩擦力都会使皮肤吸收部分药液并发热,是一种物理治疗为主、外用药物治疗为辅的方式。但是由于人工按摩效率太低,旋转速度每分钟只能达到200转左右,并且不能持久和药液刺激操作者手部皮肤,由于不能产生透皮给药所需的高强度挤压冲击力和高热力,因此不能进行以透皮给药为主的治疗。现有的物理透皮给药技术:离子导入、电致孔、超声波给药和皮肤渗透剂,局部透皮给药时间均为5-10分钟左右,透皮给药剂量小,不能进行大面积全身性透皮给药,不能在透皮给药过程中进行物理按摩恢复功能治疗,作用单一,并且不能应用大分子药物和某些含有 毒性药物(如抗癌药物),只是一种辅助治疗方法。本发明不但治疗时间短、透皮给药剂量大,可应用抗癌药物,而且在透皮给药过程中进行物理按摩恢复功能治疗,从而克服了现有透皮给药技术的不足。
3、所用外治药物
根据中医药外治法所述:膏药贴敷法是通过药物的直接作用或渗透作用,多采用大毒或剧毒的药物来祛除病邪,此法可大幅度减轻药物毒性给肌体带来的损害,又可直接杀灭位于体表的肿瘤。对于体表肿瘤或晚期内脏肿瘤,不任攻代者(许多患者)常常用之。《常青治疗临证心法》常青主编中国中医药出版社、2013年8月第1版、32页。化学药物中的许多抗癌药物,如环磷酰胺、长春新碱、氟尿嘧啶等,均可外用,由于本发明透皮给药时间短,所产生的副作用小。现有透皮给药是起到局部的治疗作用,那么能不能起到全身的治疗作用呢?这一直是药学家们研究的热门课题。成人皮肤总面积约为2平方米,在人体中约有1/3的血液可以流经皮肤,药物透过皮肤进入血液循环,那么药物就可以起到全身治疗作用,本发明中全身药物按摩在8分钟内就可以起到全身治疗作用。并且透皮给药不会经过肝脏,也不会受胃肠道酸碱性的破坏,因此生物利用度就比较高,小剂量就可以发挥很高的疗效。

阶段性与周期性是螺旋式发展的一个重要特征,每一周期的终点,同时又是下一周期的开始,事物的发展,一个周期接着一个周期,形成了由无数“圆圈”衔接起来的发展链条。任何现象发展到底,都转化为自己的对立物,既“物极必反”,在发展的第三阶段与第一阶段有形式上的类同。所以,事物的发展曲折起伏,往返迂回,在高级阶段上重复低级阶段上的某些特征,是毫不奇怪、合乎规律的。
二、关于胃癌转移发展过程中的阶段性特点
1、预防胃癌转移产生阶段:是器官层次原发病灶与系统层次功能病变相互作用阶段,一般器官病灶是处在刚进行完手术或者是不能手术的情况下,系统功能已经处于部分病变状态。器官病变会促进系统功能病变,是系统功能病变的原因;同样,系统功能正常会促进器官病变好转,本阶段应当进行以恢复系统功能为主、器官病灶抗损伤为辅的治疗方案,因为如果系统功能正常,就不会产生癌转移。
2、胃癌转移产生阶段:是系统功能病变与系统结构病变相互作用阶段,系统功能病变是产生系统结构病变的原因,并促进系统结构病变,此时系统结构已经出现部分病变,同样,系统结构病变好转会促进系统功能好转。本阶段应当进行以系统结构放化疗抗损伤为主、恢复系统功能为辅的治疗方案。因为如果系统结构癌转移被控制或消除,就不会对患者生命产生威胁。
3、胃癌转移产生死亡阶段:是系统结构病变与人体功能病变相互作用阶段,系统结构病变是产生人体功能病变的原因,并促进人本功能病变,此时人体功能已经出现部分病变;同样,人体功能病变好转会促进系统结构好转。本阶段应当进行以恢复人体功能为主、系统结构放化疗抗损伤为辅的治疗方案,因为人体功能病变是死亡产生原因,如果人体功能恢复,患者就不会产生死亡。关于本阶段的治疗方案和治疗工具、技术,已经在现有技术《肝癌转移治疗器》中进行了详细描述,所以不再进行专门说明。
现有技术认为,是结构决定功能,结构治疗与功能治疗应分开进行。通过《示意图》和论述我们可以看到,病变发展过程是呈螺旋状曲折的发展,在结构病变成分为主阶段,是结构决定功能,在功能病变成分为主阶段,是功能决定结构,双方交替发展。同时,在阶段治疗中,结构治疗与功能治疗是双方相互依存共同作用的,无论是那一方为主,它的整体性质是不会改变的,分开进行就会失去整体性改变事物的本质,导致治疗失败。
三、预防胃癌转移的治疗
1、为什么手术放化疗不能阻止胃癌转移?
20世纪90年代以来,由于癌症手术后复发率、转移率仍很高,为了防止术后复发、转移,开始采用了术后系列辅助化疗,但其结果并不尽如人意,许多患者术后辅助化疗 并未能阻止出现复发、转移。现有技术认为,原发癌灶与转移癌灶之间存在着直接联系,采用手术切除或放疗、化疗等治疗原发性肿瘤癌灶,其实质目的就是为了防止癌转移、除去癌细胞转移的策源地。当原发癌只有1cm大小时,并无症状,亦无任何不适,也未造成该器官功能损害,为何也要切除?这是为了及早防止癌转移。至于为什么不能阻止癌转移,现有技术至今缺乏明确的认识,在于癌转移是一种结构病变与功能病变相互制约的整体病变,不能从结构病变中推理出整体病变规律。
2、癌转移的病因是什么?怎样进行预防癌转移治疗?
通过《示意图》可以看到:术后器官结构原发癌灶部与转移癌灶部(系统结构)之间没有直接联系,也就是说手术切除或放化疗原发癌灶,不会对防止癌转移产生直接作用,二者之间没有必然联系,因为术后器官原发癌灶,与系统功能、即原发癌灶周围和远端易发转移部位的功能状态有直接联系,原发癌灶的发展路线与发展目标,是直接奔向系统功能相关部位。所以,系统功能病变是癌转移产生的主要原因,预防胃癌转移应当采用恢复系统功能为主、器官原发癌灶放化疗为辅的治疗方案。由于原发癌灶已切除,术后辅助化疗的目的则主要是针对手术后残留的癌细胞、微转移或正在转移途中的癌细胞,因此,其用药剂量应较小,且不损伤或极少损伤宿主的免疫细胞。所以,只要系统功能正常,就不会出现癌转移。
3、原发癌灶的辅助性化疗方式
首先进行辅助性小剂量化疗,采用隔天注射氟尿嘧啶250毫克的方式。
因为本发明治疗器具备摩擦生热透皮给药功能,所以在原发癌灶部位及怀疑出现癌转移部位涂上氟尿嘧啶乳膏,(附:氟尿嘧啶乳膏主要由含量5—10%氟尿嘧啶、白凡士林、液状石腊、甘油、十六醇、蒸馏水等加热熔化制成,温度为70—80℃。是现有技术,可参照《肝癌转移治疗器》说明书中介绍。)然后手持本发明治疗器,在治疗部位进行移动式摩擦治疗,治疗器摩擦头转速为3000转/分钟,手握压力0.2公斤/平方厘米,治疗时间10—20秒,皮肤接受温度为60℃,摩擦头压在摩擦药布上高速转动,挤压摩擦力和热能传递透过药布可在短时间内使乳膏透过皮肤进入体内产生治疗作用,并且作用时间短,不会对皮肤造成损伤。
4、恢复消化系统功能的主要治疗方式
药物按摩,古称膏摩,是中医药物外治的主要方法,中医药物治则是健脾和胃,恢复功能治疗采用内治、外治同时进行的方式。胃癌常用口服药方:党参10克,吴茱萸12克,桂枝10克,仙灵脾15克,补骨脂10克,鸦胆子15克,制附片10克,猪苓10克,制半夏10克,薏苡仁20克,黄芩10克,白花蛇舌草20克,丹参20克,甘草5克,每天一剂。
药物按摩作用药物有活血化瘀类外用药物(如活络油,红花油等),和健脾理气类外用扶正药物(如黄芪、党参、白术、茯苓、川朴、木香、八月扎等),患者可因人而异选择。 药物按摩时将药物涂在原发癌灶周围及远端可能转移部位,手持本发明治疗器进行移动治疗,治疗器摩擦头转速是600转/分钟,手握压力为1—2公斤/平方厘米,治疗时间为10分钟,皮肤接受温度为40℃,每天治疗一次,药物按摩可将机械做功、热能传递、药物透皮吸收三者合为一体,在10分钟内完成恢复消化系统功能治疗,并且患者自我操作并不劳累。
四、胃癌转移产生后的治疗
在人体消化系统中,消化道也被称为胃肠道,平时我们所说的胃部指的是胸部和骨盆之间的部位,在医学上称为腹部,胃癌转移与其它腹部癌转移,如结肠癌、直肠癌、肝癌、胰腺癌、小肠癌等有着不同的症状、体征和治疗方法,但在其转移途径(如直接蔓延、淋巴转移、血行转移、种植转移等)和手术放化疗治疗原理方面,其共同性是主要的。本发明主要用于腹部与对应背部治疗,在治疗操作中针对不同的病变与部位需要因人而异进行适当调整,在治疗原理和治疗目标方面是一致的,可以用于其它各种腹部癌转移,如肠癌转移、胰腺癌转移等。
目前治疗癌转移常用技术有手术、放疗、化疗、免疫治疗、中药治疗等,其中手术对已有转移肿瘤疗效不好,应用较少,放疗主要作用于局部治疗。放疗、化疗虽然对部分肿瘤治疗效果较好,但对转移癌灶效果较差,同时由于放化疗相结合与化疗对机体损伤消耗极大,许多病人难以承受其毒副作用。中药和免疫治疗能增强人体免疫机能战胜肿瘤,但临床上应用疗效也不尽如人意,大多数只能作为辅助治疗应用。现在医院临床主要采用化疗来治疗腹部癌转移患者,需要研究探索新的治疗方法。
1、胃癌转移的产生与发展
消化系统功能病变是产生消化系统结构病变胃癌转移的主要原因,而消化系统结构病变的发展,是产生人体层次功能病变产生死亡的主要原因。在胃癌转移产生阶段,应采用结构抗损伤为主与恢复功能为辅相互促进的整体治疗,其治疗部位包括原发癌灶、转移灶和病灶部周围相关部位。
2、治疗器恢复功能与放疗相结合
现有技术放射疗法的主要副作用,是因为它给肿瘤周围的健康组织造成了伤害,人们将放射线更直接对准肿瘤——更快、侵入性更小、且缩短总放疗时间,试图通过减少对周围组织的辐射剂量来减轻这种伤害,但是效果并不显著。因为周围健康组织与癌灶结构之间是紧密联系的整体相互依存,不可能分开,只有同时进行对称性恢复功能治疗,才能避免这种伤害,由于本发明可以进行定位性恢复功能,先在原发癌灶、转移癌灶放疗部位涂上活血化瘀类外用药物,(如活络油、红花油等)在放疗部位周围涂上补正类外用药物(如黄氏、党参、白术、云芝等),应用本发明治疗器在放射部位和周围进行1—3分钟药物按摩,然后 进行放疗。应用活血类药物在于增加放疗敏感性,加大放疗力度,应用补正药物是保护正常细胞恢复功能、减少副作用,所用药物因人而异按需确定。治疗器操作采用轻压力为0.2公斤/平方厘米,摩擦头转速为1200转/分钟,接触皮肤温度为2℃—45℃,1分钟内即可产生恢复功能效果,药物按摩结束后立即进行放疗,最好在5分钟内。
3、治疗器恢复功能与全身化疗相结合
化疗可以分为局部化疗和全身化疗两种,全身静脉化疗是目前胃癌化疗的主要手段,长期以来,人们试图通过全身静脉化疗来控制复发和转移,但效果并不理想,问题在于化疗在杀伤癌细胞的同时也杀死了大量的健康细胞,副作用过大患者难以承受,不是正确治疗方案,应当采用杀灭癌细胞与恢复功能相互促进的整体治疗方案。
本发明治疗器,可以在全身涂上健脾和胃、活血化瘀、清热解毒等外用中药药液(药物选择因人而异),进行全身药物按摩,每次治疗8分钟,通过向人体全身输送机械能、热能、化学能进行全身消除化疗副作用、恢复功能治疗。全身药物按摩是在全身静脉化疗结束后5分钟开始进行。治疗器操作采用中等压力为0.5—1公斤/平方厘米,摩擦头转速1500转/分钟,接触皮肤温度为45℃—50℃,缓慢移动进行全身按摩,1分钟内产生恢复功能效果,疗效维持在10—20个小时。通常化疗只能间断进行,在化疗时杀伤了大量癌细胞,在化疗间隔时间未被杀伤的癌细胞,继续分裂增殖,甚至更活跃。而患者由于免疫细胞已被杀伤,功能下降,也更有利于促进了癌细胞的发展、转移。而本发明治疗方案,在杀伤癌细胞的同时进行了恢复功能治疗,不存在化疗间断期,治疗可以继续进行。通过全身静脉注射化疗的杀灭癌细胞为主治疗,与全身药物按摩恢复功能辅助治疗相结合,可以消除现有全身化疗技术副作用过大的不足,保证化疗顺利进行。对立与统一是自然界的普遍性规律,正如古希腊哲学家赫拉克利特所说:“它是用对立来产生和谐,而不是用相同的东西。”现有全身静脉化疗技术毒性副作用极大,但没有对称恢复功能的治疗相结合,并不符合自然规律,难以持久进行。
五、胃癌转移产生死亡阶段的治疗
胃癌转移是系统层次结构病变,它的发展是产生人体层次功能病变的主要原因,人体层次功能病变是死亡的主要因素,是死因。虽然胃癌已经产生了多个转移病灶,并且不能控制消除,病变已经进入突变阶段,但毕竟人体层次功能病变才刚刚开始,通过及时进行全身恢复功能为主治疗与放化疗杀灭癌细胞辅助治疗相结合的整体治疗,可以恢复人体层次功能和控制转移灶不发展,从而为患者消除死亡病因,达到显著降低死亡率的目标。
关于本阶段的治疗原理与治疗器械,已经在《肝癌转移治疗器》中做了详细介绍,产品发明专利号是ZL201410524366·9,本发明介绍说明书摘要和列出《肝癌转移治疗器》 附图摘要,《肝癌转移治疗器》是由垫板、俯卧支架、腹部按摩器、药布、链轮支架1、链轮支架2、传动轴、链轮、链条、转动盘、脚蹬、两个转轮支架、转轮、调速电机、手持按摩器组成;在肝癌转移产生死亡阶段,爬行运动、游泳运动、腹部药物按摩和采用手持按摩器进行的全身药物按摩相结合进行恢复全身功能主治疗,手术放化疗杀灭癌细胞为辅助治疗。在此阶段消化道癌转移的治疗原理基本上相同,在具体治疗方法上需因人而异灵活掌握。
六、本发明治疗胃癌转移的技术方案
1、本发明的治疗方法与现有技术的不足
人体生命运动是机械、物理、化学多种运动形式的整体,是与周围环境的物质、能量、信息的交换相统一的。在人体微观层次中,分子与原子之间的功能主要是化学能和热能,机械能相对较小,而在宏观层次细胞、组织、器官等结构的功能中,机械能成分在逐步加大,所以,热疗与化疗可以对分散的癌细胞和微小转移癌灶产生直接作用,但是对于稍大的转移灶和实体肿瘤,效果并不明显。机械运动是热能、化学能相互联系转化的基础,缺少机械运动治疗因素使不能成为整体的高级治疗技术,是治疗作用低的原因,药物按摩,古称膏摩,疗效优于按摩与药物外敷,早在二千年前我国汉代就已经广泛采用膏摩技术治疗疾病。本发明治疗器,根据中医膏摩原理和摩擦化学技术,将机械摩擦挤压、按揉、振动冲击,与摩擦生热物理热能传递和化学药物透皮吸收结合为整体共同作用,可以对稍大的原发癌灶、转移灶部位、实体肿瘤和周围功能下降部位进行整体治疗,产生直接作用显著提高疗效,从而克服了现有技术中全身静脉注射化疗存在的疗效低、盲目性和副作用大等不足。如根据《癌症治疗新概念与新方法》58页、68页、70页作者徐泽所述:“静脉滴注全身化疗时,真正达到癌灶部位可以利用的氟尿嘧啶只有0.4%,而99.6%的化疗药在杀伤正常细胞。目前临床抗肿瘤药物治疗的总有效率仅14%,而局部化疗药物的浓高提高1倍,对肿瘤的杀伤作用可提高6—10倍。”胃癌对于化学药物的敏感性是决定疗效的关键问题,化疗药物与恢复功能的按摩相结合,可以改善和解决这个问题。
2、本发明的透皮给药技术
人体皮肤角质层是药物透皮吸收的主要屏障,日本学者野吕俊一在《药物新制剂的设计与开发》(中国医药科技出版社)中认为:“现有透皮给药的不足主要是:①透皮给药受物理性质的限制。②很难达到速效。③不能用大量药物治疗。
本发明根据摩擦生热、钻木取火的原理,将摩擦头转速提高到了2000转/分钟、6000转/分钟、12000转/分钟,振动摩擦头形状设计为双半径挤压横刃凸起高低不平和挤压横刃外缘呈尖锐角,在高速转动中会显著提高挤压冲击力,促使药物分子突破皮肤角质层障碍,产热温度提高到接触皮肤温度为60℃—65℃,在病变部位涂外用化疗药物,摩擦头压紧 药布进行摩擦时,高强度挤压冲击力与高热透过药布将药物挤压透过皮肤角质层从而消除了物理限制,进行透皮给药治疗。在手掌般大的病变部位药物按摩,完成治疗时间只有10秒—20秒,因为皮肤耐热时间为60—65℃时/3秒,所以操作中必须手握治疗器进行往返移动治疗。关于透皮给药量的大小,因为透皮给药量与透皮给药面积成正比,所以增加透皮给药面积就可以增加给药量。在应用抗癌透皮吸收药物方面,环磷酰胺、长春新碱、氟尿嘧啶皆可外用。详细技术资料可参考现有系列技术中《膏摩器》中国专利ZL2016202539444。
3、本发明对原发癌灶部、转移癌灶部和周围的主化疗
按摩与加热同时进行时,会产生促进癌细胞扩散转移可能,当与抗癌药物共同作用时,可在恢复功能的同时杀灭癌细胞,消除扩散转移因素是一种非损伤疗法。化疗是胃癌已经扩散到其它器官的主要治疗方法,可以缩小癌肿,并减缓其生长,但通常不会治愈癌症。大部分这个领域的研究都在关注靶向疗法和化学疗法的结合,或者它们两者之一。化疗新的给药方法也正在研究之中,如美国医生在研究通过直接向腹部灌输化疗药物,看到否取得更好的效果,同时不良反应又比较小。
①皮肤浅层次透皮给药为主化疗,采用本发明进行化疗时,首先在原发癌灶部、转移癌灶部涂上含量20%(含量高低因人而异)的氟尿嘧啶乳膏,在癌灶周围和腹部、背部对应部位和其它相关部位,涂上含量10%的氟尿嘧啶乳膏,采用本发明手持治疗器进行局部、全身化疗药物按摩。对局部病灶旋转移动按摩治疗时,压力为0.1公斤/平方厘米,转速1500转/分钟,摩擦生热温度为45℃,每个病灶部位治疗时间20秒;对癌灶周围、腹部和其它相关部位旋转移动按摩治疗时,压力为0.2公斤/平方厘米,转速3000转/分钟,摩擦生热温度为50℃,治疗总时间为3分钟。
②肌肉组织深层次按揉物理辅助治疗
透皮给药为主化疗结束后,采用本发明的手持治疗器对癌灶周围和腹部、背部对应部位和其它相关部位,进行深层次按揉。通过调速开关调速压力为0.5公斤/平方厘米,转速600转/分钟,深层次组织按揉产生内生热,以恢复微循环功能和促进药物吸收,腹部按摩时绕脐中心四周往返移动。治疗时间为5分钟。
③在透皮给药为主化疗前,可以口服或注射扶正类中药,进行恢复功能辅助治疗。
治疗结束后对治疗部位皮肤进行清洁和养护。治疗每日一次,每次透皮给药主化疗3—5分钟,10天为一疗程,透皮给药化疗中不需要间歇停顿以恢复人体机能,药液成分组成,使用方法参照说明书介绍,治疗过程中根据患者病情、体质因人而异确定所用药物(可用多种抗癌药物)、药物剂量和确定治疗时间长短、治疗次数多少等。治疗结束后(包括每次治疗后和疗程结束后)进行结构与功能双重标准整体检验,结构检验参照现有技术标准,功 能检验参照患者恢复功能程度和生活质量状态。由于在化疗过程中已经同时进行了恢复功能治疗,透皮给药化疗可以连续进行,一般不需要进行全身恢复功能为主治疗。
4、将全身静脉化疗改为透皮给药化疗
癌症术后辅助化疗已在全国广泛进行,绝大多数患者接受术后化疗,均为全身静脉化疗,但其结果并不尽如人意,在化疗过程中或疗程后即出现复发、转移,或边化疗边转移。全身静脉化疗给药途径,是非定点靶向给药,进入瘤体内的药物极少,抗癌作用甚微,但副作用却很大,使许多患者承受着化疗不良反应的痛苦。全身静脉化疗虽能使瘤体短暂缩小,但不能明显延长患者生命,只能治标不能治本,可以作为辅助治疗,而不能根治。并且在化学治疗耐药性方面,多数实体瘤(如胃癌、大肠癌)对全身静脉化疗低敏感或不敏感,而胃癌对于化疗药物的敏感性是决定疗效的关键问题,如果问题能够解决,则能够彻底改变目前胃癌患者术后较低生存率的问题。所以解决的办法是改变给药途径,这也是胃癌化疗、肠癌化疗努力的方向。
药物按摩,古称膏摩,疗效优于按摩与药物外敷,早在二千年前我国汉代就已经广泛采用膏摩技术进行内病外治,是传统中医药外治的主要方法。本发明根据摩擦化学原理和采用现代科学技术,在传统人工膏摩的基础上采用仪器进行透皮给药治疗,既可以进行全身性化疗,也可以进行局部性化疗,并且无毒副作用,在透皮给药时,向人体内输送机械能和热能,使化疗杀灭癌细胞与恢复功能同时进行。药物按摩透皮给药最大特点就是具有目的性、选择性和可控性,符合中医学辨证施治思想。
本发明透皮给药化疗改变全身静脉化疗只杀癌细胞而忽视机体功能下降的状况,采用杀灭癌细胞与恢复功能两者兼顾整体治疗的方式既可以治疗癌转移,又可以明显延长患者生命。本发明不但可以提高疗效减轻副作用,而且操作简单并使用普通化疗药物,不需要应用那些贵重的化疗药物,可为患者节约医药经费开支,有利于解决看病难、看病贵的问题,为患者造福。

本发明的图1所示胃癌转移治疗器中:1是机壳,2是电机,3是振动摩擦头,4是弹簧,5是调速开关,6是手把,7是药布,8是药布固定套。图2是图1所示治疗器A向视图。图3(a)是图1所示治疗器B向视图。图3(b)是图3(a)C-C的投影视图。图4是肝癌转移治疗器附图。图5是《胃癌转移产生与发展过程示意图》。
本发明是通过以下技术措施实现的:一种胃癌转移治疗器,它是由机壳、电机、振动摩擦头、弹簧、调速开关、手把、药布、药布固定套、化疗药物组成,在手术后预防胃癌转移产生阶段,采用胃癌转移治疗器和外用化疗药物,与放疗结合成整体,对原发癌灶部位及周围进行恢复功能为主、杀灭癌细胞为辅的整体治疗,在胃癌转移产生阶段,进行手术放化疗杀灭癌细胞为主、恢复功能为辅的整体治疗,在胃癌转移产生死亡阶段,进行以恢复全身功能为主、放化疗杀灭癌细胞为辅的整体治疗,其特征在于:振动摩擦头(3)的直径是10毫米,头部呈半圆状,半圆摩擦头表面凸起的两段挤压横刃平行联接呈“一”字型排列,每段挤压横刃长度是半圆摩擦头的半径,其中一段挤压横刃高度是3毫米,另一段挤压横刃高度是2毫米,振动摩擦头(3)底部安装有弹簧(4),半圆摩擦头表面凸起的两段挤压横刃高低不一致,挤压横刃外缘处是尖锐角,在高速转动中会产生振动冲击力;在手术后预防胃癌转移产生阶段,在原发癌灶及周围和远端易扩散部位,涂上外用化疗药物,采用胃癌转移治疗器进行药物按摩恢复功能为主杀灭癌细胞为辅治疗;在胃癌转移产生阶段进行放疗时,先在病灶放疗部位涂上外用扶正药物进行药物按摩恢复功能辅助治疗,然后进行放疗的主治疗;进行化疗时在原发癌灶、转移病灶部位和病灶周围部位、背部对应部位,涂上外用化疗药物,进行药物按摩透皮给药化疗的主治疗,在其它功能下降部位涂上外用扶正药物,进行药物按摩的恢复功能辅助治疗;对采用全身静脉化疗患者进行全身扶正药物按摩恢复功能辅助治疗;在胃癌转移产生死亡阶段,进行全身扶正药物按摩与全身水平伸展运动相结合的恢复全身功能为主治疗,同时进行小剂量静脉化疗杀灭癌细胞的辅助治疗。
在胃癌转移治疗器中,机壳(1)内安装电机(2)、振动摩擦头(3),电机(2)带动振动摩擦头(3)高速转动,振动摩擦头(3)与固定在机壳(1)上的药布(7)进行摩擦,摩擦产生的挤压冲击力和热能透过药布(7)传导到皮肤上进行透皮给药治疗,调速开关(5)安装在手把(6)部,药布固定套(8)将药布(7)固定在机壳(1)头端部,机壳(1)内安装有4-32个振动摩擦头(3)。胃癌转移治疗器在浅表层次进行透皮给药为主治疗时,采用低压高速转动手握压力为0.2公斤/平方厘米,转速1500转-3000转/分钟;在软深层次进行物理按摩为主治疗时,采用高压低速转动手握压力为2公斤/平方厘米,转速300转-600转/分钟。
其具体实施例为:
在青岛市肿瘤医院由医生操作在肿瘤病灶相关邻近部位和穴位进行治疗,每次10分钟,每日一次,15次为一疗程。许多癌转移患者由于手术放化疗后体质下降、恶心、呕吐、食欲不振,通过恢复功能药物按摩治疗,患者痛苦减轻、生存质量提高、容易接受放化疗,疗效较好,受到了医生的欢迎,患者们普遍乐于接受,并希望能够自我操作实施和改进提高。本发明申请人李复生患有中度慢性萎缩性胃炎和食道粘膜充血、纵横糜烂食道炎,(参见青岛大学附属医院内镜诊疗报告单:病历号0000309266,日期:2013年12月30日),平时服用奥美拉唑肠溶胶囊和埃索美拉唑肠溶片来消除症状,患病10多年,每年按照医生规定定期进行胃肠复查,共进行了10多次胃镜检查。后在2016年初使用本发明仪器,每晚平卧对准胃、食道部进行药物按摩,采用红花油,每次5分钟,共进行20余次。后于2016年5月4日在青岛大学附属医院复查,结果为10多年服用贵重西药只能减轻症状的食道炎治愈了,胃镜检查所见:食道粘膜光滑,没有食道炎症。原先的中度慢性萎缩性胃类病理改善为中度慢性浅表性胃炎,(参见青岛大学附属医院胃镜检查报告单,登记号:0000309266,日期:2016年5月4日)。所以,采用恢复功能药物按摩为主治疗长期慢性胃病、食道病方面,取得了意想不到的效果,并且节省了大量的药费、检查费。最重要的是,医院医生为预防上述疾疗转化为癌症,规定患者必须每年定期进行胃镜复查,介入性检查不仅每年给患者带来痛苦,也给患者带来有可能产生癌症的心理压力。本发明克服了现有技术依靠药物治疗为主难以治愈慢性食道炎和治疗萎缩性胃炎疗效低的不足,采用本发明恢复功能药物按摩为主、口服药物辅助的治疗,不但对治愈慢性食道炎和治疗萎缩性胃炎产生了意想不到的显著疗效,同时也对预防胃癌、食道癌的产生,解除患者的心里压力,具有突出的作用。

Claims (1)

  1. 一种胃癌转移治疗器,它是由机壳、电机、振动摩擦头、弹簧、调速开关、手把、药布固定套、化疗药物组成,在手术后预防胃癌转移产生阶段,采用胃癌转移治疗器和外用化疗药物,与放疗结合成整体,对原发癌灶部位及周围进行恢复功能为主、杀灭癌细胞为辅的整体治疗,在胃癌转移产生阶段,进行手术放化疗杀灭癌细胞为主、恢复功能为辅的整体治疗,其特征在于:振动摩擦头(3)的直径是10毫米,头部呈半圆状,半圆摩擦头表面凸起的两段挤压横刃平行联接呈“一”字型排列,每段挤压横刃长度是半圆摩擦头的半径,其中一段挤压横刃高度是3毫米,另一段挤压横刃高度是2毫米,振动摩擦头(3)底部安装有弹簧(4),半圆摩擦头表面凸起的两段挤压横刃高低不一致,挤压横刃外缘处是尖锐角,在高速转动中会产生振动冲击力;在手术后预防胃癌转移产生阶段,在原发癌灶及周围和远端易扩散部位,涂上外用化疗药物,采用胃癌转移治疗器进行药物按摩恢复功能为主杀灭癌细胞为辅治疗;在胃癌转移产生阶段进行放疗时,先在病灶放疗部位涂上外用扶正药物进行药物按摩恢复功能辅助治疗,然后进行放疗的主治疗;进行化疗时在原发癌灶、转移病灶部位和病灶周围部位、背部对应部位,涂上外用化疗药物,进行药物按摩透皮给药化疗的主治疗,在其他功能下降部位涂上外用扶正药物,进行药物按摩的恢复功能辅助治疗;对采用全身静脉化疗患者进行全身扶正药物按摩恢复功能辅助治疗;在胃癌转移产生死亡阶段,进行全身扶正药物按摩与全身水平伸展运动相结合的恢复全身功能为主治疗,同时进行小剂量静脉化疗杀灭癌细胞的辅助治疗。
