[0021] 人体生命运动是由机械、 物理、 化学初级运动形式组成的整体高级运动形式, 在生命运动中初级运动形式的规律与在无生命自然系统中起作用的表现形式不 同, 它们服从于更高级的生命运动规律, 以整体方式对外作用。 整体治疗可以 产生直接作用和显著疗效, 部分治疗产生间接作用和有限疗效。 所以, 与现有 技术中机械、 物理、 化学初级运动形式单独治疗不同, 本发明遵循生命运动规 律, 通过采用机械、 物理、 化学相互促进整体作用的高级运动治疗方式来治疗 疾病, 同吋, 根据人体生命运动中机械、 物理、 化学等因素的作用力度, 确定 整体治疗中机械、 物理、 化学治疗力度的大小。 透皮给药治疗技术作用的提高 , 不仅取决于治疗因素的质量和数量, 而且取决于治疗因素的组合方式即整体 治疗。 与膏摩物理治疗为主、 药物治疗为辅的方式不同, 透皮给药是以化学治 疗为主、 物理治疗为辅的方式, 透皮给药通过改变药物剂型和给药途径, 赋予 西药以崭新的使用方法, 打破了 "药物结构"是唯一决定药效的观念。 实践证明, 当局部血药浓度提高 1倍吋, 疗效提高 10倍, 并避免了药物首过肝胃和减少用药 量, 使治疗更加安全有效。

[0024] 皮肤角质层是药物透皮吸收的主要屏障, 传统的药物渗透促进剂主要是降低皮 肤屏障作用, 但治疗效果并不理想。 日本学者野吕俊一在 《药物新制剂的设计 与幵发》 (中国医药科技出版社) 中认为: "现有透皮给药的不足主要是: ①透 皮给药受物理性质的限质。 ②很难达到速效。 ③不能用大量药物治疗"。 现有技 术采用的离子导入法、 电致孔法和超声波透皮给药物理技术及各种声、 光、 电 、 磁促渗技术, 与上述传统的药物渗透促进剂, 由于缺乏机械作功高速转动挤 压摩擦和高热效应, 导致不能对提高透皮给药疗效产生显著的作用。 在实践中 , 外用药物无论是霜剂、 水剂还是油膏剂, 都必须借助于摩擦发挥和提高疗效 , 单纯依靠渗透促进剂、 热能传递, 疗效受到限制。
[0025] 1、 人体皮肤组织中毛细血管壁上存有接受机械刺激、 温热刺激、 化学刺激的 感受装置, 本发明所用的钻头状半圆摩擦头部固定有长方形凸起可以通过摩擦 头高速转动, 挤压药物分子进入皮肤, 并达到进行透皮给药所需要的各种温度 。 转速越高产生温度越高, 由于治疗吋存在各种降温因素, 如药液润滑、 摩擦 阻力大小、 药布隔温、 患者体质差导等, 具体所需温度根据具体情况选择合适 转速。 振法是一种压强刺激和波的传递相结合的治疗方法, 本发明钻头状半圆 摩擦头排列中心按装有理疗振动器, 属现有技术, 采用高频率振动, 振动能量 向患处机体深处传导, 转化为机体的内能, 频率越快传导的程度就越深, 有助 于促进血液循环和药物吸收。
[0026] 2、 人体皮肤角质层的相变发生在 42_70°C之间, 皮肤温度上升 10°C, 药物穿 透性提高 1.4_3倍, 透皮给药剂量与透皮给药面积成 1:1正比。 当用膏摩器进行 大面积移动透皮给药治疗吋, 移动速度为 4厘米 /秒, 高热 70°C吋局部停留吋间为 0.2秒, 不会损伤皮肤, 透皮给药面积比现有技术提高 10倍。 现有技术透皮给药 的药物理想理化性质条件是: 分子量 1000以下, 熔点低于 85°C, 在水和矿物油中 的溶解度最好均在 lmg/ml以上, 饱合水溶液的 PH值在 5— 9。 在本发明钻木取火 过程中分子运动加大出现高热运动, 药物分子量 5000以下均可应用, 从而加大 了透皮给药的应用药物种类范围。 清代外治大师吴师机认为: "摩吋须极力, 令 作热, 乃速效"。 现有物理透皮给药吋间一般在 10分钟以上, 本发明根据"钻木取 火"技术原理, 将钻头状半圆摩擦头高速转动产生的高强度挤压摩擦冲击力和高 热温度结合为一体整体作用, 可以在极短促的 10秒 _30秒内完成局部透皮给药 治疗。 现有技术中膏摩是以摩擦生热为基础, 是物理按摩为主、 药物渗透为辅 的一种治疗方式, 本发明通过提高转速升温和改进摩擦头凸起形状, 具备了摩 擦取火的功能, 可以将物理治疗为主的膏摩转化为化学药物治疗为主、 物理治 疗为辅的透皮给药治疗。 本发明中透皮给药不受物理性质的限制, 可以加大透 皮给药剂量并达到速效, 从而克服了透皮给药现有技术中的不足, 具有显著的 进步。
[0027] 3、 口服药物难以定位区别, 但膏摩与透皮给药可以做到定位治疗, 透皮给药 的治疗目标是在全身或局部组织深部, 与口服给药有差别。 外用药液配制: 中 药根据现有的口服类、 外用药膏类药方制成浸液或煎剂, 加入相应的接触剂、 助渗剂中, 搅拌均匀即可应用, 也可使用中成药, 如活血化瘀类中的活络油、 红花油等。 西药中的硝酸甘油、 消心痛均可改为皮肤给药, 抗癌药物环磷酰胺 、 长春新碱按其特性分别加入相应的耦合剂中, 搅拌均匀即可应用。 如脂溶性 药物加入羊毛脂中, 配制成油膏或冷霜; 水溶性药物溶于水中等。

[0031] 1、 按摩手法作用力主要有压力、 摩擦力、 振动力和张力四种, 一种手法往往 是一种作用力为主, 同吋包括其它作用力; 如摩擦法是以摩擦为主, 同吋兼有 压力, 按揉法是以压力为主, 同吋兼有摩擦力。 在按摩中, 不同的部位及不同 患者的情况, 需要施加不同的压力, 如在按揉腰背部吋, 要用较大的力使刺激 深透到机体内部。 当压力一定吋, 刺激强度为压力面积小指揉法>压力面积中等 鱼际揉 >压力面积大掌根揉。 体表受力面积越小, 手法强度越大, 对患者的刺激 也越强。 压力刺激强度越大, 牵动的部位就越深。
[0032] 按揉法是一种以压强刺激和内摩擦共同作用的按摩手法, 压紧揉动的主要目的 是为了带动患处皮下组织的运动, 使之产生内摩擦, 揉动速度可以直接影响体 内摩擦力的大小。 摩擦法与按揉法的主要区别是: ①摩擦过程中并不压紧吸定患 者患处。 ②摩擦吋不带动或是基本不带动皮下组织的运动。 这就是说, 在按揉中 医者所做的功几乎全部转化为患处皮下组织热运动的内能, 而在摩擦法中医者 所做的功几乎全部转化为治疗部位表面热运动的内能, 摩擦法使表面皮肤温度 升高, 然后通过热传导向皮下组织渗透, 由于肌肉组织是热的不良导体, 所以 这种升温将比通过按揉内摩擦直接升温慢的多。
[0033] 2、 清代著名外治宗师吴师机在 《理瀹骈文》 中说道: "外治之理即内治之理, 外治之药亦即内治之药。 治虽在外, 无殊治内也", 外治可"与内治并行, 而能补 内治之逮"。 从医疗的起源来看, 应当说外治远早于内治, 既便在 《内经》 吋代 亦主张内外并治, 在我国唐代以前医疗上外治与内治并重, 宋代以后医家受理 学思想影响偏重内治, 外治一道乃渐黜。 后世医家大多沿着内病汤、 丸、 散剂 内服药的治疗思路, 而忽略了外法方法, 逐渐形成了只有外科等疾病方用外治 的重内轻外的治疗思想, 使外治疗法理论体系的建立迟于内治千余年。 (参见 《当代中药外治临床大全》 贾一江等主编、 中国中医药出版社, 1991年 4月第 1 版第 4页) 膏摩疗法作为清代以前中医药物外治的主要方法, 数千年来有着丰富 的临床经验和疗效, 有着巨大的发展空间和潜力, 将传统膏摩工具、 刮痧板、 药巾与现代科学技术相结合, 操作使用方便, 可以显著提高膏摩疗效和扩大治 疗范围, 从而发扬光大中医药外治法、 造福患者。

[0035] 1、 与传统膏摩工具 (药摩工具) 、 刮痧工具中的汤匙、 刮痧板相比较, 汤匙 边缘光滑, 使用方便, 汤匙边缘后有汤匙板壁高度支撑, 保证汤匙光滑边缘在 膏摩过程中不增加受力面积降低强度; 刮痧板边缘是钝圆角并光滑, 刮痧板边 缘后是长方形板状高度支撑, 保证刮痧过程中不增加受力面积降低强度。 上述 工具属于平动摩擦类按摩工具, 摩擦生热效果比较好, 但在按压揉动向下作用 类似钻木取火方面, 存在不足; 手持汤匙、 刮板在局部揉动吋, 每分钟仅 120转 左右, 而达到局部深层次加热效果, 实验证明揉动速度应当在 1000转 /分钟左右 , 这在人工按揉来说是不可能的。 另外, 传统摩擦类工具应用范围小, 如颈椎 和腰椎两侧, 由于有骨头突出 (特别是第七颈椎突出明显) , 应用汤匙、 刮板 进行膏摩按压揉动难度较大, 作用点少, 揉动频率低, 而本发明中的四个钻头 状半圆摩擦头成四方型排列, 中间有空间, 左右两边各有两个摩擦头, 非常适 合对颈椎、 腰椎两侧进行按压揉动和深层次加热, 治疗效果优于传统技术。
[0036] 2、 现有技术半圆摩擦头中的各种凸起形状, 如凸起上部是筋状、 锐角切削状 、 三角状、 梯形平面状等, 均没有凸起下部方形体支撑, 在对人体软组织部位 按摩吋, 不能支撑凸起上部压紧患处部位进行深层次按揉。 由于缺乏凸起高度 导致软组织部位与半圆摩擦头部接触受力面积较大, 不能压紧患处, 因而降低 了按压强度。 现有技术中的半圆摩擦头部圆直径为 1.5厘米, 高度为 1.5厘米, 凸 起筋状、 切削状凸起上部高度为 0.2厘米 _0.4厘米, 直径为 0.2厘米 _0.4厘米; 本发明中凸起下部高度为 0.2厘米 _0.5厘为, 宽度为 0.2厘米 _0.5厘米, 其它数 值与现有技术相同, 其半圆摩擦头与凸起的外观形状与挤压摩擦功能与现有技 术中钻头的外观形状挤压摩擦功能相似, 称为钻头状半圆摩擦头。 现有技术中 的各种形状摩擦头凸起在临床治疗使用中, 患者、 医者普遍反映摩擦生热效果 明显, 包括透皮给药效果, 但按压强度不足, 特别是在进行腰背部按压揉动按 摩中。 本发明通过增加凸起下部方形结构, 增加了凸起高度, 减少了受力面积 , 显著提高了按压揉动强度, 克服了现有技术的不足。
[0037] 3、 本发明根据膏摩和药摩工具汤匙、 刮痧板的特点, 钝角钻头摩擦产热量高 和热来源于钻头运动的原理, 设计的钻头状半圆摩擦头长方形凸起, 凸起上部 是钝圆角状, 凸起下部是方形体, 膏摩器由无刷直接转动电机带动四个按装有 半圆摩擦头的行星齿轮转动, 产生钻头运动进行膏摩, 具有摩擦生热量高、 按 揉力度高和使用方便的特点。 现有技术中的筋状凸起上部等摩擦头, 由于缺乏 凸起下部方形体支撑, 凸起高度不足, 不具备挤压功能。 在压迫人体弹性软组 织吋接触面积显著增大, 并受药布包复、 膏摩药汁润滑影响限制, 高速转动吋 摩擦阻力小、 产热量低, 不能达到所需 60-70°C工作温度。 由于摩擦吋被软组织 复盖压力面积过大, 导致摩擦强度降低, 如果加大按压力度只能增加凸起上部 与半圆摩擦头受力面积; 如凸起筋状实际上是类似半圆型实体, 增大受力面积 并不能压紧患处加大挤压强度。 本发明克服了现有技相应措施不足, 凸起上部 钝圆角状与凸起下部方形结合为一体, 凸起下部具有挤压按揉所需高度, 凸起 上部钝圆角受力面积小, 加大挤压强度按揉吋受力面积并不增加, 可以压紧患 处显著提高摩擦强度、 挤压按揉强度。
[0038] 4、 在半圆摩擦头压力下, 人体肌肉软组织部位下陷深度在 0.3厘米一 0.6厘米范 围内, 现有技术半圆摩擦头凸起高度为 0.2厘米 _0.5厘米, 弹性软组织可以复盖 半圆摩擦头凸起和半圆摩擦头大部分面积, 导致体表受力面积大压力强度小, 只能起到摩擦为主、 压力为辅的作用。 而本发明中半圆摩擦头凸起上部与凸起 下部相加高度为 0.4厘米 _0.8厘米范围内, 软组织只能与半圆摩擦头钝圆角状凸 起上部相互作用, 受力面积小压力强度大, 可以起到压力为主、 摩擦为辅的按 摩作用。 通过调整压力和转速, 可以分别进行浅层次的摩擦法治疗和深层次的 按揉法治疗, 如浅层次摩擦法吋转速 1000转 /分钟, 压力 0.5公斤 _1公斤 /平方厘 米吋, 摩擦生热温度可在 5秒钟内达到 50°C, 通过输送热能和促进透皮给药, 满 足膏摩以 3厘米 /秒速度缓慢移动治疗需要。 深层次按揉吋转速 400转 /分钟, 压力 1公斤 _3公斤 /平方厘米, 按压力度可透过皮下组织达到肌肉层次, 带动患处皮 下组织和肌肉的运动产生内摩擦, 对僵硬肌肉进行按揉放松, 改善微循环、 活 血化瘀和促进透皮药物吸收; 而现有技术摩擦头凸起由于缺乏凸起下部高度支 撑, 与弹性软组织相互作用吋挤压强度小, 无论怎样调整压力和转速, 都不能 进行深层次按揉。
[0039] 5、 在现有的手持按摩器中, 是采用有刷电机和齿轮减速装置, 存在着噪音大 、 耗电量高、 结构复杂等不足, 本发明采用无刷直接转动电机, 具有噪音小、 耗电量低、 没有齿轮减速装置结构简单等特点, 克服了现有技术的不足。
