Definitions
- the invention relates to an implant system for partial or complete replacement of the metatarsophalangeal joint with an artificial metatarsal component for implantation in the metatarsal and / or an artificial phalangeal component for implantation in the natural phalangeal.
- implant systems based on plastics such. highly cross-linked PE materials or PEEK or silicone are known.
- Implant systems based on ceramic materials are also known, but they also have some disadvantages. Mainly to mention is the lack of osseointegration of these implant systems and the associated loosening over time, as well as design-induced unphysiological force discharges, which lead to bone loss, which in turn brings a number of disadvantages. These include bone loss due to mechanical overload or shielding of mechanical stress (stress shielding).
- the invention has for its object to improve an implant system for partial or complete replacement of the metatarsophalangeal joint with an artificial metatarsal component for implantation in the metatarsal and / or an artificial phalangeal component for implantation in the natural phalangeal so that metallic abrasion and resulting adverse effects on the human organism, medical imaging artefacts, aging effects and long-term behavior (fatigue, corrosion, release of metallic ions that may be toxic), allergy-causing effects, and the risk of infection during surgery. Furthermore, the breaking off of structural features of the implant system, for example during installation, should be avoided. Also should the Osseointegration of these implant systems improved and the associated relaxation over time can be avoided.
- an implant system according to the features of claim 1.
- the breaking off of structural features of the implant system, for example during installation is avoided.
- the osseointegration of these implant systems is improved and the associated loosening over time avoided.
- the ceramic material is preferably a zirconia-reinforced aluminum oxide material with high hardness and flexural strength, which has established itself as a biocompatible material in medical technology, especially for tribological applications in the hip or shoulder.
- the osseointegrating surface is preferably a coating based on hydroxyapatite, tri calcium phosphate or a bioglass-based coating, whereby the implant system quickly anchors in the bone.
- the metatarsal component is half shell-shaped and completely made of the ceramic material and has two substantially parallel pins on its inner surface facing the metatarsal in the implanted state (as distal as possible) protrude into the epiphysis of the natural metatarsal. It has been found that by this embodiment, a detachment from the natural bone is avoided and the tribological properties are optimized. This embodiment is particularly suitable for young patients, in which as little as possible of the bone should be changed.
- this artificial metatarsal component directly articulates with natural phalangeal component.
- the metatarsal component is semi-shell-shaped and entirely made of the ceramic material and has a shaft on its inner surface facing the natural metatarsal, which in the implanted state projects into the cavity of the natural metatarsal.
- This is preferably suitable for a total replacement in which the natural metatarsophalangeal joint is completely replaced by an artificial metatarsophalangeal joint.
- the phalangeal component consists of a module of a ceramic support member and a plastic sliding member.
- the sliding element made of a plastic absorbs the rotational forces and minimizes or prevents implant loosening.
- An embodiment of the implant system according to the invention is suitable for a total replacement of the metatarsophalangeal joint.
- the support member has on its underside facing the natural phalangeal a shaft which protrudes in the implanted state into the cavity of the natural Phalangeals and facing on its natural metatarsal top has a plateau on which the slider is mounted freely movable with its underside.
- the leadership of the metatarsal component is carried out in a total endoprosthesis preferably on a superstructure of the phalangeal component, ie that the slider on its upper side facing the natural metatarsal has a bulge for freely movable storage of the artificial metatarsal component.
- at least the parts of the implant system which come into contact with the natural metatarsal and / or the natural phalangeal have a macrostructure of about 300-700 ⁇ .
- the invention thus relates to a ceramic component for large toe joint replacement with a special design for safe and low-damage implantation and secure and permanent anchoring with excellent tribological, especially low-abrasion properties.
- the use of bioinertem ceramic starting material makes the invention extremely patient-friendly.
- the implant according to the invention consists of a ceramic material, preferably oxide ceramic from the class of aluminum oxides or zirconium oxides.
- the preferred material here is a zirconia-reinforced AI2O3 material with high hardness and flexural strength, the biocompatible has been established especially for tribological applications in the hip or shoulder.
- the components are molded either as pressed components in the green state or sprayed in large quantities by means of ceramic injection molding. Subsequently, the components are thermally treated, that is sintered, honed, white-burned, and especially the tribological surfaces mechanically aftertreated, for example, ground or polished. Also conceivable is a subsequent coating of the components for bioactivation, which ensures that bone-forming cells adhere well, find cytocompatible conditions and become osteogenically active.
- Such coatings can be based on common coatings
- Coatings based on bioglasses It is known to use a shaft for anchoring the implant in the bone (cementable and cement-free).
- the forces are introduced via the shaft into the bone. This is always done non-physiologically since the natural anatomical force application occurs directly at the joint (via the cartilage into the epiphysis).
- the bone structure (spongiosa) is not designed for this proximal shifted force application. See Fig.1.
- the introduction of force should be made as physiological as possible. Therefore, it is suggested that the forces are introduced via two small pins as distally as possible directly into the epiphysis. See Fig. 2.
- the concrete geometries of the two pins are determined by the anatomical conditions in the human metatarsophalangeal joint. It is recommended a size portfolio.
- a base plate is used, which is firmly connected to the shaft. See Fig. 3.
- Fig. 3 shows a hemiprosthesis according to the prior art.
- the sliding element has a bulge, in which the phalangeal component is mounted freely movable. See Fig. 5.
- the natural bone-contacting parts of the implant system are provided with a hydrogel coating for hemian use.
- the primary stability of the metatarsal component and the phalangeal component is achieved via a press-fit.
- the secondary stability can be achieved via a Direct to Bone technology DtB, which is an osseointegrating surface.
- the implant system according to the invention is suitable for all toe joints and is preferably used as a big toe joint replacement.
- FIG. 1 shows a section of a natural foot of a patient. Clearly visible is the natural metatarsal 3, which is associated with the natural phalangeal 5 via the metatarsophalangeal joint 1 is articulated. The metatarsophalangeal joint 1 of the big toe is also known as the metatarsophalangeal joint.
- FIG. 2 shows the natural metatarsal 3 of the big toe from a natural foot of a patient.
- FIG. 3 shows this one-piece metatarsal component 2a, which consists entirely of a ceramic.
- the metatarsal component 2a has a half-shell-shaped design and has two substantially parallel pins 6 on its inner surface facing the metatarsal 3, which pins project into the epiphysis 7 of the natural metatarsal 3 in the implanted state.
- the ceramic material is a zirconia-reinforced alumina material.
- the parts of the implant system which come into contact with the natural metatarsal 3 and / or the natural phalangeal 5 have an osseointegrating surface.
- the osseointegrating surface is a coating based on hydroxyapatite, tri calcium phosphate or a coating based on bioglasses.
- FIG. 4 shows an artificial phalangeal component 4 according to the invention for implantation in the natural phalangeal 5, see FIGS. 1 or 2.
- the phalangeal component 4 consists in a modular fashion of a ceramic support element 9 and a sliding element 10 of a plastic.
- the support member 9 has on its facing the natural phalangeal 5 underside a shaft 1 1, which protrudes in the implanted state in the cavity of the natural Phalangeals 5 and on its facing the natural metatarsal 3 top plateau 12 on which the Sliding element 10 is freely movably mounted with its underside.
- a bulge 13 is arranged on the sliding element 10, which serves for the freely movable mounting of the artificial metatarsal component 2b.
- FIG. 5 shows an implant system for complete replacement of the natural one
- the artificial phalangeal component 4 is identical to the artificial phalangeal component 4 according to FIG.
- the artificial metatarsal component 2b has a half-shell-shaped design and is made entirely of the ceramic material and has on its inner surface facing the natural metatarsal 3 a shaft 8 which is implanted in the implant
- Implant system a macrostructure of about 300 - 700 ⁇ on.
