Abstract

The invention is related with Ulna nails that is used in the surgical treatment of ulna fractures, its feature; lower (12) and upper part (11) that is entering the ulna bone, at least one locking pin (2) which is screwed from inside the upper part (11) longitudinally coming out from the nails oblique angled distal hole (8) to provide locking, at least one locking pin sending slot at the locking pin (2) to provide sending and locking of the locking pin (2) from the distal part, at least one nails locking pin slot (5) at the proximal part to provide moving of the locking pin (2) inside the ulna internal safe locking nail (1), at least one locking pin threaded tip (7) on the locking pin (2) which is locking by coming out from the nails distal hole (8), at least one locking pins bone entering tip (9) on the locking pins threaded tip (7) that is entering the bone angled to provide locking, is characterized by including ulna internal safe locking nail (1).

ULNA INTERNAL SAFE LOCKING NAIL
Technological Scope:
The current invention is about Nails used in the surgical treatment of Ulna fractures.
The invention is especially related with the new Ulna internal safe locking nails that eliminate the experienced problems in the treatment of Ulna fractures. Common Status of Technique:
The forearm injuries are most commonly seen in Adults. These injuries can restrict human live for a long time such as they can lead to serious functional loss if they are not treated properly. The aim of the treatment is to ensure rapid fracture healing without causing a motion and function loss.
The most commonly used method in surgical treatment of diaphyseal forearm fractures in adults is plate and screw applications. However, during the application of these techniques there is a surgical intervention to the tissue surrounding the bone such as muscle, vessels, nerves and skin hence, there will be more soft tissue injuries.
The loss of blood accumulation on the fracture area which has a positive contribution to the union of the fracture, disruption of the bone outer membrane (periosteum) capillary blood circulation because of the plate compression and additionally increased infection risk brought by the open surgery method are emerging.
Obligations of the additional fixation such as making a plaster cast and the cosmetic problems in the skin are the disadvantages of this plate fixation method. - -
The disadvantages of plate-screw application, arise the need to develop a new intramedullary nail for surgical treatment of ulna fractures that is minimal invasive, easy applicable, multifunctional with simple distal locking, fast, safe and anatomically suitable.
In recent years' intramedullary fixation methods are preferred for Ulnar fractures. Even the elimination of complication, risk and challenges by intramedullary nailing system mentioned above, distal locking problems occurs in all long bone internal fixations with the intramedullary nails.
The distal part of the ulna is anatomically narrow so the nail system must be suitable to this anatomic structure.
Opening a hole for the locking screw at the distal part of a small diameter nail creates a risk of the implant strength. There are also difficulties in this system at the placing stage of the screws in to the described places. To overcome the difficulties, the primary referenced instrument is the known viewing device fluoroscope which is very harmful to human health by emitting x-ray. Skin incision and soft tissue damage at the distal ulna (wrist surrounding) delays the healing and additionally leaves cosmetic defects.
The given solutions of TR 2008/01447 numbered, named as "ulna intramedullary locking nail" coded, A61 19/00 IPC patent application and TR 2009/03589 numbered, named as "a multi-purpose ulna intramedullary locking nail" coded A61 17/56 IPC Patent applications are not sufficient. To overcome these kinds of problems, a new solution is needed. Explanation of the purpose of the invention:
According to the known way of the technique, the aim of invention is to develop an ulna internal safe locking nail for eliminating the negativities of existing structure.
Another purpose is to eliminate the negativeness of the plate-screw application.
Another object of the current invention is to develop a nail that is suitable to the anatomical narrow diametric Ulna distal part. To overcome the difficulties of implantation and application to a screw hole at a small diametric area.

To achieve the purposes mentioned, a new ulna internal safe locking nail has been developed which is providing more successful treatment of the ulna fractures and obtain contribution to human health.

Detailed Disclosure of the Invention
Invention, as ulna internal safe locking nail (1 ) provides many advantages, has locking pin sending slot (3) on the locking pin (2), is send on the upper parts (1 1 ) inner side. At the lower part (12) on the nails distal hole (8) the threaded tip (7) of the locking pin (2) is coming out and for fixating the position the locking pins bone entering tip (9) is locked by screwing at the distal part. The locking pin (2) is sent with by screwing with a screw driver from the locking pin sending slot (3) which is at the upper part (1 1 ) of the Ulna internal safe locking pin (1 ) in the locking pin slot (5).
At the upper part (1 1 ) it has a nail compression hole (4), angled screw hole at the proximal of the nail (6) and nails locking pin slot (5). The locking pin (2) is sent from inside the nails locking pin slot (5) at the locking pin sending slot (3) which is then in the nails locking pin slot (5) with a screw driver by rotation and at the distal the locking pins bone entering tip (9) which is on the locking pins threaded tip (7) is locking by screwing. The developed ulna internal safe locking nail (1 ) (ulna insafelock nail), newly and differently protected the soft tissue reduce the infection risk by using the minimal invasive incision method due to the application feature. The proximal (upper) and distal (lower) ends of the nail must be locked at the long bone fracture fixations with the intramedullar nail fixation system.
The distal locking, which is for years defined as a difficult procedure, is tried to make it easier with various auxiliary instruments (hand tools) and imaging methods (fluoroscopy).
The design of the ulna internal safe locking nail (ulna insafelock nail) (1) which is developed for ulna makes the distal locking simple, fast and more reliable.
The locking pin (2) which is sent inside the cannulated internal safe locking nail (1 ) is coming out with an angle from the ulna internal safe locking nails (1 ) distal hole (8) and owing to the locking pins threaded end (7) is holding the ulna bones distal cortex (bone structure).
The threaded structure of the proximal part of the locking pin (10) matches with the ulna internal locking nails (1 ) locking pin slot (5). Owing to the 0-20° oblique angled screw hole at the proximal of the nail (6) which is at the proximal of the ulna internal safe locking nail (1 ), olecranon area fractures can be fixed with an angled screw fixation. The locking pin (2) with the locking pin sending slot (3) in the Ulna internal safe locking nail (1 ) at the upper part (1 1 ), the compression screw parallel to the long axis of the ulna internal safe locking nail (1 ) at the proximal of the nails locking pin slot (5) and works together with the locking screw through the nail compression hole (4), compression can be achieved between the fracture fragments. Owing to this technique for locating, the distal locking place there is no need to use the x-ray emitting fluoroscopy device there is also no need of any guide or additional auxiliary instrument usage, there is any need to make a new incision for distal locking.
Thanks to all these achievements, the failure possibility of the distal locking is eliminated, at the distal part possible vessel, nerve injuries with additional skin incision could be seen and following soft tissue damage is prevented. The Surgical process facilitated, the X-ray exposure to the patient and to the surgeon is reduced and the operation time is shortened. The migration, irritation and possible loosening of the distal screws are prevented. Thus, a more comfortable recovery for the patient with ulna fracture has taken place in the care system. Reduction of the postoperative seen function loss and maintaining union in a short time are some of the important advantages.
In a preferred application of the invention, the ulna internal safe locking nail (1 ) includes, upper part (1 1 ) and lower part (12) which goes inside the Ulna bone has at least one locking pin (2) which is longitudinally inside the upper part (1 1 ) screwed from proximal, came out from lower parts (12) oblique angled nails distal hole (6) for providing locking, at least one locking pin sending slot (3) on the locking pin which provides sending the locking pin (2) and locking at the distal part, at least one nails locking pin slot (5) at the proximal part of the ulna internal safe locking nail (1 ) providing the advance of the locking pin (2) inside the ulna internal safe locking nail (1 ), on the locking pin (2), the threaded proximal part of the locking pin (2), at least one locking pins threaded tip (7) on the locking pin (2) that is locking by going out of the nails distal hole (8) and at least one locking pins bone entering tip (9) providing locking by entering the bone at an angle has been established.
It has a bone entering tip (9) on the locking pins threaded proximal part (10) which is coming out from the nails distal hole (8) at the lower part (12) by moving forward with a screwdriver via locking pin sending slot (3) and locks from the distal of the ulna bone. The Angled screw hole at the proximal of the nail (6), with 0-20 ° angle on the upper part (1 1 ) of the ulna internal safe locking nail (1 ) which provides angled screw fixation of the fractures of olecranon area is created.
The locking pin (2), to make locking at the lower part (12) by screwing with a control of screw driver from inside the upper part (1 1 ) moving in the inner part in nails locking pin slots (5), at least one nails distal hole (8), with an oblique angle at the lower part (12) from the distal tip between 20-50 mm above, fixed to the bone by coming out from the nails distal hole (8) and screwed in an angle is created.
It has a nails locking pin slot (5) at the proximal of ulna internal safe locking nail (1 ) that is matching with the locking pins threaded proximal part (10) at the locking pin (2).
It has a nails Anti-rotation tip structure (13) at the lower part (12) which makes easier to enter the bone.

Claims

The invention is related with Ulna nails used in the surgical treatment of ulna fractures, its feature;
— The lower part (12) and the upper part (1 1 ) which enter to the ulna bone,
— At least one locking pin (2) which is screwed from inside the upper part (1 1 ) longitudinally coming out from the nails oblique angled distal hole (8) to provide locking,
— At least one locking pin sending slot (3) at the locking pin (2) to provide sending and locking of the locking pin (2) from the distal part,
— At least one nails locking pin slot (5) at the proximal of the ulna internal safe locking nail (1 ) to provide moving of the locking pin (2) inside the ulna internal safe locking nail (1 ),
— At least one locking pin threaded proximal part (10) on the locking pin (2),
— At least one locking pin threaded tip (7) on the locking pin (2) which is coming out by screwing from the nails distal hole (8) for locking,
— At least one locking pins bone entering tip (9) on the locking pins threaded tip (7) which enter the bone angled by screwing to provide locking, is characterized by including ulna internal safe locking nail (1 ).

— Including locking pin (2), that is fixed to the bone by coming out from the nails distal hole (8) and screwed in an angle.
