Description

リードスルー治療薬
 本発明は、リードスルー治療薬に関する。本願は、2015年6月4日に、米国に出願された仮出願第62/171,205号に基づき優先権を主張し、その内容をここに援用する。
 ナンセンス変異型遺伝性疾患とは、遺伝子上の点変異(ナンセンス変異)により形成される未熟終止コドンによるタンパク質の発現の阻害に起因する疾患である。リードスルー活性とは、ナンセンス変異型遺伝性疾患の患者に特定の化合物を投与した場合に、前記化合物がリボソームに作用し、リボソームが未熟終止コドンを読み越えて翻訳を行う活性をいう。リードスルーの結果、野生型正常タンパク質が合成され、ナンセンス変異型遺伝性疾患の症状が緩和される。
 例えば、特許文献1には、アミノグリコシド系化合物であるアルベカシンがリードスルー活性を有することが記載されている。

 しかしながら、アミノグリコシド系化合物は、静脈内注射により投与する必要がある。臨床での長期投与を考慮すると、コンプライアンスを保つために経口投与可能な薬剤であることが望ましい。そこで、本発明は、経口投与可能なリードスルー治療薬を提供することを目的とする。
 本発明は以下の通りである。
(1)リンコマイシン系化合物を有効成分として含有するリードスルー治療薬。
(2)経口剤である、(1)に記載のリードスルー治療薬。
(3)ナンセンス変異型遺伝性疾患治療用である、(1)又は(2)に記載のリードスルー治療薬。
(4)ナンセンス変異型遺伝性疾患が筋ジストロフィー又は嚢胞性線維症である、(1)~(3)のいずれかに記載のリードスルー治療薬。
 本発明により、経口投与可能なリードスルー治療薬を提供することができる。

 ナンセンス変異による機能性タンパク質の欠損は、筋ジストロフィーをはじめ2,400種を超える致死性疾患(ナンセンス変異型遺伝性疾患)を引き起こす。患者に薬物を投与して、ナンセンス変異によるタンパク質の発現の阻害を抑制して翻訳を進行させ、先天的に欠如した機能性タンパク質を後天的に発現させて症状を改善する治療方法がリードスルー治療である。本療法は遺伝子治療や核酸医薬ではなく、ナンセンス変異症例の包括的治療が可能な概念に基づくものであり、例えば、本邦のデュシェンヌ型筋ジストロフィー患者において19%を占めるナンセンス変異症例の有効かつ迅速な選択肢と考えられる。
 1実施形態において、本発明は、リンコマイシン系化合物を有効成分として含有するリードスルー治療薬を提供する。実施例において後述するように、本実施形態のリードスルー治療薬は、経口投与によりリードスルー活性を発揮することができる。したがって、本実施形態のリードスルー治療薬は、経口剤として用いることができる。本実施形態のリードスルー治療薬によれば、難治性のナンセンス変異型遺伝性疾患を、遺伝子治療、核酸医薬、幹細胞移植等によらず、薬物を用いて治療することができる。
 本実施形態のリードスルー治療薬において、リンコマイシン系化合物としては、クリンダマイシン、リンコマイシン等が挙げられる。中でも、クリンダマイシンは、経口吸収性や組織浸透性が良く、抗菌剤として長年に渡り使用され、安全性についても臨床使用の実績から多くの情報が集積されている薬剤である。実施例において後述するように、発明者らは、クリンダマイシンがリードスルー治療薬としても有効であることを明らかにした。
 本実施形態のリードスルー治療薬は、ナンセンス変異型遺伝性疾患の治療に好適に用いることができる。ナンセンス変異型遺伝性疾患としては、筋ジストロフィー以外にも、例えば、嚢胞性線維症、網膜色素変性症、表皮水疱症、色素性乾皮症、ライソゾーム症、多発性硬化症、乳児神経セロイドリポフスチン沈着症、アルツハイマー病、テイ-サックス病、神経組織変性、パーキンソン病、慢性関節リウマチ、対宿主性移植片病、関節炎、血友病、フォンヴィレブラント病、毛細管拡張性運動失調、サラセミア、腎結石、骨形成不全、肝硬変、神経繊維腫症、水疱性疾患、ライソゾーム性蓄積症、ハーラー疾患、家族性コレステロール血症、小脳運動失調、結節性硬化症、家族性赤血球増加症、免疫不全、腎臓病、肺疾患、家族性高コレステロール血症、色素性網膜症、アミロイドーシス、アテローム性動脈硬化症、巨人症、小人症、甲状腺機能低下症、甲状腺機能亢進症、老化、肥満、糖尿病、ニーマン-ピック病、マルファン症候群、癌、等が挙げられる。本実施形態のリードスルー治療薬は、これらの遺伝性疾患に包括的に適用可能である。
 本実施形態のリードスルー治療薬は、人類福祉への直接的還元として大きく寄与し、難病に苦しむ患者に大きな希望を提供できる。更に、既存薬から薬物候補を特定したことにより、非臨床・第I相臨床試験を省くことが可能となり、迅速に実用化することができる。このため、進行性の重篤な疾患の早期治療に寄与し、有用な医薬品を提供する機会を増加させることで社会への貢献は甚大となる。開発期間の短縮だけでなく、開発リスクの低減や開発コストの軽減を図ることもできる。
 本実施形態のリードスルー治療薬は、リンコマイシン系化合物と、薬学的に許容される担体とを含む医薬組成物であってもよい。医薬組成物は、経口的に使用される剤型又は非経口的に使用される剤型に製剤化されていてもよいが、コンプライアンスを保ちやすい観点から、経口的に使用される剤型に製剤化されることが好ましい。
 経口的に使用される剤型としては、例えば錠剤、カプセル剤、エリキシル剤、マイクロカプセル剤等が挙げられる。非経口的に使用される剤型としては例えば注射剤等が挙げられる。
 薬学的に許容される担体としては、一般的に製剤に用いられるものを使用することができ、例えば、ゼラチン、コーンスターチ、トラガント、アラビアゴム等の結合剤;結晶性セルロース等の賦形剤;コーンスターチ、ゼラチン、アルギン酸等の膨化剤;ステアリン酸マグネシウム等の潤滑剤;ショ糖、乳糖、サッカリン等の甘味剤;ペパーミント、アカモノ油、チェリーなどの香味剤;油脂、注射用水、植物油(例えば、ゴマ油、ヤシ油、大豆油)、緩衝剤(例えば、リン酸塩緩衝液、酢酸ナトリウム緩衝液)等の液状担体;溶解補助剤(例えば、エタノール、プロピレングリコール、ポリエチレングリコール、安息香酸ベンジル、ベンジルアルコール);非イオン性界面活性剤(例えば、ポリソルベート80(商標)、HCO-50);無痛化剤(例えば、塩化ベンザルコニウム、塩酸プロカイン);安定剤(例えば、ヒト血清アルブミン、ポリエチレングリコール);保存剤(例えば、ベンジルアルコール、フェノール);酸化防止剤等が挙げられる。
 上記の注射用水としては、例えば、生理食塩水;ブドウ糖、D-ソルビトール、D-マンニトール、塩化ナトリウム等を含む等張液が挙げられる。
 医薬組成物の患者への投与は、経口投与、動脈内注射、静脈内注射、皮下注射等の当業者に公知の方法により行いうる。リンコマイシン系化合物の投与量は、患者の体重や年齢、投与方法等により変動するが、当業者であれば適当な投与量を適宜選択することが可能である。例えば、経口投与の場合、100~500mg(力価)を6時間毎に投与することが一般的である。また、点滴静注の場合、600~2400mg(力価)を1日2~4回投与することが一般的である。

 アルベカシン等の既存のアミドグリコシド系化合物を有効成分とするリードスルー治療薬と、本発明のリードスルー治療薬とを併用することにより、ナンセンス変異型遺伝性疾患の治療効果を維持したまま、薬物の投与量を減少させることもできる。
 すなわち、1実施形態において、本発明は、アミドグリコシド系化合物とリンコマイシン系化合物を併用して投与する工程を備える、アミドグリコシド系化合物の投与量の低減方法を提供する。本実施形態の低減方法においては、リンコマイシン系化合物を併用することにより、同程度のリードスルー治療効果を発揮しながら、アミドグリコシド系化合物の投与量を低減することができる。この結果、アミドグリコシド系化合物の投与による副作用を軽減することができる等の効果が得られる。
 1実施形態において、本発明は、アミドグリコシド系化合物とリンコマイシン系化合物を併用して投与する工程を備える、リンコマイシン系化合物の投与量の低減方法を提供する。本実施形態の低減方法においては、アミドグリコシド系化合物を併用することにより、同程度のリードスルー治療効果を発揮しながら、リンコマイシン系化合物の投与量を低減することができる。この結果、リンコマイシン系化合物の投与による副作用を軽減することができる等の効果が得られる。
 1実施形態において、本発明は、アミドグリコシド系化合物を有効成分として含有する注射剤と、リンコマイシン系化合物を有効成分として含有する経口剤とを含む、ナンセンス変異型遺伝性疾患の治療用キットを提供する。
 1実施形態において、本発明は、患者にアミドグリコシド系化合物を投与する工程と、前記患者にリンコマイシン系化合物を投与する工程と、を備える、ナンセンス変異型遺伝性疾患の治療方法を提供する。
 1実施形態において、本発明は、リンコマイシン系化合物の有効量を、治療を必要とする患者に投与する工程を備える、ナンセンス変異型遺伝性疾患の治療方法を提供する。
 1実施形態において、本発明は、ナンセンス変異型遺伝性疾患の治療のためのリンコマイシン系化合物を提供する。
 1実施形態において、本発明は、ナンセンス変異型遺伝性疾患の治療薬を製造するためのリンコマイシン系化合物の使用を提供する。
 これらの各実施形態において、リンコマイシン系化合物の投与は経口投与により行うことが好ましい。また、ナンセンス変異型遺伝性疾患は、筋ジストロフィー又は嚢胞性線維症であってもよい。
 次に実施例を示して本発明を更に詳細に説明するが、本発明は以下の実施例に限定されるものではない。
[実験例1]
 リードスルー活性検出用遺伝子導入マウス(Readthrough Evaluation and Assessment by Dual-reporter mice、国際公開第2008/004610号を参照。以下、「R.E.A.D.マウス」という。)を用いて、リンコマイシン系化合物(リンコサミド系化合物)であるリンコマイシン及びクリンダマイシンのリードスルー活性を検討した。R.E.A.D.マウスは、β-ガラクトシダーゼ遺伝子と、ルシフェラーゼ遺伝子とを連結し、その繋ぎ目に未熟終止コドン(TGA)を挿入したコンストラクト(mdxマウスのエクソン23の未熟終止コドン前後12mer周辺配列を含む27mer)を含む、サイトメガロウイルスエンハンサー/ニワトリβーアクチンハイブリッドプロモーターを有する発現ベクター(pCAGGS)を生殖細胞系列に組み込んだものである。R.E.A.D.マウスは、正常時にはβ-ガラクトシダーゼのみを発現し、未熟終止コドンをリードスルーするとルシフェラーゼを発現する。R.E.A.D.マウスの心筋や横隔膜を含む筋組織全般でβ-ガラクトシダーゼの発現が確認されている。
 まず、R.E.A.D.マウスに、アルベカシン1mg/日、リンコマイシン3mg/日、リンコマイシン10mg/日、クリンダマイシン6mg/日を、7日間連日皮下投与した。続いて、各マウスにおけるリードスルー活性を測定した。リードスルー活性は、薬物の投与終了時に大腿部/下腿部骨格筋組織抽出液のルシフェラーゼ活性とβ-ガラクトシダーゼ活性をルミノメータで定量することにより測定した。
 図1は、各マウスのリードスルー活性の測定結果を示すグラフである。その結果、リンコマイシン又はクリンダマイシンを皮下投与することにより、リードスルー活性を有することが知られているアルベカシンを皮下投与した場合と同程度のリードスルー活性が発揮されることが明らかとなった。
[実験例2]
 皮下投与又は経口投与により、R.E.A.D.マウスにクリンダマイシンを投与し、リードスルー活性を検討した。
 まず、R.E.A.D.マウスに、クリンダマイシン6mg/日を7日間連日投与した。投与は皮下投与又は経口投与(強制胃内投与)により行った。対照として、R.E.A.D.マウスに生理食塩水を皮下投与した群を用いた。
 続いて、各マウスにおけるリードスルー活性を実験例1と同様にして測定した。図2は、各マウスのリードスルー活性の測定結果を示すグラフである。その結果、クリンダマイシンを投与することにより、対照と比較してリードスルー活性の上昇が観察された。また、リードスルー活性の上昇の程度は、皮下投与でも経口投与でも同程度であった。この結果は、クリンダマイシンが、皮下投与だけでなく経口投与によっても、リードスルー活性を発揮することができることを示す。
[実験例3]
 デュシェンヌ型筋ジストロフィーのモデルマウスであるmdxマウスにクリンダマイシンを投与し、ジストロフィンタンパク質の発現を検討した。なお、mdxマウスはジストロフィン遺伝子にナンセンス変異を有しており、ジストロフィンタンパク質を欠損している。
 mdxマウスに、クリンダマイシン6mg/日を4週間連日投与した。クリンダマイシンの投与は、皮下投与又は経口投与により行った。続いて、各マウスの下腿筋組織を採取して薄切切片を作製し、免疫染色によりジストロフィンタンパク質の発現を検討した。また、比較のために、正常マウスであるB10マウス、及びクリンダマイシンを投与していないmdxマウスについても下腿筋組織を採取して薄切切片を作製し、同様の免疫染色を行った。
 図3(a)~(d)は、免疫染色の結果を示す写真である。図3(a)はB10マウスの結果を示し、図3(b)はクリンダマイシンを投与していないmdxマウスの結果を示し、図3(c)は皮下投与によりクリンダマイシンを投与したmdxマウスの結果を示し、図3(d)は経口投与によりクリンダマイシンを投与したmdxマウスの結果を示す。
 その結果、クリンダマイシンを投与していないmdxマウスの組織では、ジストロフィンタンパク質の発現が検出されなかった(図3(b))。これに対し、B10マウス(図3(a))、皮下投与によりクリンダマイシンを投与したmdxマウス(図3(c))、経口投与によりクリンダマイシンを投与したmdxマウス(図3(d))の組織からは、全長ジストロフィンタンパク質の発現が検出された。
 この結果は、クリンダマイシンの皮下投与又は経口投与により、ジストロフィン遺伝子にナンセンス変異を有するmdxマウスの組織でジストロフィンタンパク質を発現させることができることを示す。
[実験例4]
 mdxマウスにクリンダマイシンを投与し、血液中のクレアチンキナーゼ活性を測定した。なお、クレアチンキナーゼは、筋ジストロフィーの臨床マーカーである。クレアチンキナーゼは筋線維中に含まれる酵素であり、筋が破壊されると血液中に漏出する。このため、血液中のクレアチンキナーゼ活性が高いほど筋崩壊が進んでいることを示す。
 mdxマウスに、クリンダマイシン6mg/日を4週間連日投与した。クリンダマイシンの投与は、皮下投与又は経口投与により行った。また、比較のために、正常マウスであるB10マウス及びmdxマウスに生理食塩水を投与した群、並びにmdxマウスにアルベカシン1mg/日を4週間連日皮下投与した群を用意した。
 続いて、各マウスの血液を採取し、血液中のクレアチンキナーゼ活性を測定した。クレアチンキナーゼ活性の測定は、富士ドライケム生化学分析装置(富士フイルム社)を用いて行った。図4は、クレアチンキナーゼ活性の測定結果を示すグラフである。
 その結果、クリンダマイシンを皮下投与又は経口投与することにより、アルベカシンの皮下投与と同程度まで、血液中のクレアチンキナーゼ活性が低下したことが明らかとなった。
 この結果は、クリンダマイシンの皮下投与又は経口投与により、筋崩壊が抑制されたことを示す。すなわち、クリンダマイシンの皮下投与又は経口投与により、筋ジストロフィーの症状を低減することができることを示す。
[実験例5]
 mdxマウスに、アルベカシンとクリンダマイシンとを併用して投与し、その効果を検討した。
 mdxマウスにアルベカシン1mg/日を3週間連日皮下投与した。また、mdxマウスに3週間連日で、アルベカシン0.5mg/日を皮下投与し、クリンダマイシン3mg/日を経口投与した。続いて、実験例3と同様にして各マウスの下腿筋組織を採取して薄切切片を作製し、免疫染色によりジストロフィンタンパク質の発現を検討した。また、対照として、mdxマウスに生理食塩水を3週間連日皮下投与した群についても下腿筋組織を採取して薄切切片を作製し、同様の免疫染色を行った。
 図5(a)~(c)は、免疫染色の結果を示す写真である。図5(a)は対照のmdxマウスの結果を示し、図5(b)はアルベカシンを投与したmdxマウスの結果を示し、図5(c)はアルベカシンとクリンダマイシンを併用して投与したmdxマウスの結果を示す。
 その結果、生理食塩水を投与したmdxマウスの組織では、ジストロフィンタンパク質の発現が検出されなかった(図5(a))。これに対し、アルベカシン1mg/日を皮下投与したmdxマウス(図5(b))、アルベカシン0.5mg/日を皮下投与し、クリンダマイシン3mg/日を経口投与したmdxマウス(図3(c))の組織からは、同程度にジストロフィンタンパク質の発現が検出された。
 この結果は、クリンダマイシンを併用して経口投与することにより、アルベカシンの投与量を0.5mg/日に減らしても、アルベカシンを1mg/日皮下投与した場合と同程度にジストロフィンタンパク質を発現させることができることを示す。
[実験例6]
 実験例5で用いたマウスの血液中のクレアチンキナーゼ活性を実験例4と同様にして測定した。また、比較のために、正常マウスであるB10マウスの血液中のクレアチンキナーゼ活性も測定した。図6は、クレアチンキナーゼ活性の測定結果を示すグラフである。
 その結果、クリンダマイシンを併用して経口投与することにより、アルベカシンの投与量を0.5mg/日に減らしても、アルベカシンを1mg/日皮下投与した場合と同程度に、血液中のクレアチンキナーゼ活性が低下したことが明らかとなった。
 この結果は、アルベカシンとクリンダマイシンを併用することにより、薬物の投与量を低減することができることを更に支持するものである。
 本発明により、経口投与可能なリードスルー治療薬を提供することができる。リードスルー治療薬によれば、難治性のナンセンス変異型遺伝性疾患を、遺伝子治療、核酸医薬、幹細胞移植等によらず、薬物を用いて治療することができる。

Claims (4)

  1.  リンコマイシン系化合物を有効成分として含有するリードスルー治療薬。
  2.  経口剤である、請求項1に記載のリードスルー治療薬。
  3.  ナンセンス変異型遺伝性疾患治療用である、請求項1又は2に記載のリードスルー治療薬。
  4.  ナンセンス変異型遺伝性疾患が筋ジストロフィー又は嚢胞性線維症である、請求項1~3のいずれか一項に記載のリードスルー治療薬。
